At the time, the road surface was dry and the weather was cloudy. Traffic was particulary heavy as there were several streets in the area still blocked off for the Walk for Hunger and there were participants still walking the route in the area. My attention was drawn to the intersection of Centre Street and Bowen Street as there was a motor vehicle travelling southbound on Centre Street with their left turn signal on attempting to take a left turn on to Gibbs Street. Gibbs Street was temporarily blocked to non-residential motorists as there were several pedestrians in the road crossing over Gibbs Street. As the motor vehicle attempted to turn, there were several pedestrians in the crosswalk crossing over Gibbs Street. The vehicle had stopped halfway into the northbound lane on Centre Street blocking on coming traffic. The operator appeared to realize Gibbs Street was blocked and attemped to turn the vehicle back into the southbound lane to continue southbound on Centre Street. As the vehicle turned back into the southbound traffic lane, the vehicles rear end collided with Officer Richard Clements, who was operating NPD motorcycle unit 533. Officer Clements was traveling southbound on Centre Street and was passing the vehicles in the southbound traffic lane which were currently all at a standstill. Officer Clements had his emergency lights activated as he was on his way to remove traffic cones from the street to open up all the traffic lanes along the event route. Officer Clements was working the City of Newton detail for the Walk for Hunger assigned to the motorcycle to assist with traffic. Officer Clements front wheel, left side of the motorcycle and left leg collided with the rear end of the motor vehicle. The impact of the crash caused him to fall off the left side of the motorcycle and land on the street face first. At this time I immediately ran from my post to Officer Clements location and radioed dispatch that a motorcycle officer was down and that I needed assistance. As I approached Officer Clements he was face down, unconcious and breathing. I was able to roll Officer Clements into the recovery position and at this time Officer Clements regained conciousness and was able to speak and stated "" What happened? What was that lady doing she turned right into me"". I advised Officer Clements that he had just been involved in an accident and to stay still and that help was on the way. The operator of the motor vehicle identified as Helen Leibner (S52717988) stopped her vehicle immediatley after the crash and got out of her vehicle to offer her assistance. I advised Helen and several other witnesses and bystanders who were now crowding around Officer Clements and I to back away from the scene and to not move any of the vehicles involved. Officer Selig arrived on scene and was able to help crowd control until further help arrived. Helen and all witnesses were advised to stand by for police to take their statements. While tending to Officer Clements he was disoriented and made several attempts to move his body to which I had to physically hold him still to prevent possible further injuries. While speaking with Officer Clements I noticed blood coming from the inside of his mouth. Officer Clements was in a noticeable amount of pain. He complained of head, back and left leg pain. Officer Clements was wearing a yellow ANSI certified Newton Police reflective jacket and a Newton Police issued motorcycle helmet. Once the paramedics and fire personnel arrived I was able to speak to the operator of the motor vehicle and several other witnesses. Helen stated she was traveling southbound on Centre Street operating her blue 2014 Audi A4 (MA REG 213BC9). Helen states she was attempting to take a left turn onto Gibbs Street and activated her left turn signal. Helen states she had stopped halfway into the northbound lane as several pedestrians were crossing Gibbs Street in the crosswalk at the time. Helen states after the pedestrians cleared she saw the traffic cones blocking the street and realized the street was closed. Helen states she turned her vehicle back into the southbound traffic lane and did not see Officer Clements or the motorcycle until impact. Helen was not injured and her motor vehicle sustained moderate rear end damage. Her motor vehicle was able to drive away from the scene. Witness Adron Gordon (S90357161) stated he was participating in the Walk for Hunger and had just crossed over Gibbs Street when the crash occured. He stated he witnessed the Officer on the motorcycle with the emergency lights on traveling southbound on Centre Street behind the motor vehicle. The motor vehicle stopped in traffic attempting to turn left onto Gibbs Street. He states then the vehicle made a slight turn back into the southbound traffic lane and the motorcycle collided with the rear end of the motor vehicle. He states the Officer then fell off the motorcycle landing in the middle of Centre Street. Witness Elizabeth McCarthy (S00242346) and Katherine Regan (S43486601) stated they both were in the same vehicle together stopped in traffic in the northbound lane of Centre Street just before the set of lights at Centre Street and Bowen Street. Both witnesses stated they do not remember if the driver of the motor vehicle had the turn signal on but they did see her attempt to turn onto Gibbs Street. They stated they also saw the motorcycle with emergency lights activated travel southbound on Centre Street approaching behind the vehicle. Witnesses state the motor vehicle then suddenly turned back into the southbound lane and the motorcycle collided with the passenger side tailight of the motor vehicle. Both witnesses state the motorcycle attemped to turn away from the vehicle to avoid the crash but was unable to do so. Witness Nina Wang (S39460985) stated she was parked on the southbound lane of Centre Street just prior to the crash scene. Nina states she was in her City of Newton DPW marked vehicle with the hazards on as she was working to assist with traffic safety for the Walk for Hunger. Nina states she witnessed the motorcycle travel southbound on Centre Street and pass the vehicles stopped in the traffic lane. Nina states the motorcycle passed the traffic using the left side and had the emergency lights activated. Nina states the motorcycle was just passing through the green light at Centre Street and Bowen Street when the motor vehicle that was stopped at the lights attempting to take a left turn onto Gibbs Street. Nina states the vehicle then abruptly jerked her car to the right and collided with the motorcycles left side. Nina states she heard a loud crash and witnessed the Officer fall off the motorcycle and land face first onto the street. The motorcycle's final resting place was on its left side approximately two feet away from where Officer Clements was located. The motorcycle sustained moderate damage to its left side, including the front wheel well and undercarriage. The motorcycle left scuff marks on the street which were measured and photographed. Several photographs of the scene and damage to both vehicles were taken and submitted to the IT Bureau. The motorcycle was towed by Todys. Centre Street and Bowen Street are both public ways maintained by the City of Newton. The injured motorcyclist, Officer Clements, was transported to Saint Elizabeth Hospital with fire fighters on board. I was able to follow up with Officer Clements on the phone the next day who was admitted over night at the hospital. Officer Clements stated he has no recollection of the accident and can only remember driving down Centre Street to begin to pick up cones to open up all lanes of traffic. Officer Clements stated he suffered a brain bleed, head, back, left foot and right thumb injuries. At the time of the accident the weather was sunny and the road conditions were dry. Langley Road is a public way maintained by the City of Newton. Upon arrival I first spoke to the pedestrian who was inside the ambulance getting medical attention. The pedestrian identified as Donna Kalikow (S24151638) stated she was standing in an open parking space for a few seconds, attempting to walk across the street to enter J.P. Licks. Donna stated she was standing in the marked parking space waiting for traffic to stop to safely cross the street. Donna states the cars stopped to let her cross and she proceeded into the roadway. She states MV1 was attempting to back into the parking space that she was standing in and struck her right side and knocked her to the ground. Donna states the operator of MV1 stopped the vehicle when he realized he had struck her and got out to contact 911. Donna appeared to have minor injuries and complained of head pain. Medics transported Donna to NWH to be further evaluated. The operator of MV1 identified as Christopher Moos (S24090966) stated he was attempting to parallel park into an empty parking space and never saw the pedestrian standing in the space. Christopher stated as he was backing up into the space he was notified by a bystander that he just struck a pedestrian. Christopher stated he immediately stopped the vehicle and got out to assist the pedestrian and contact 911. Christopher stated he was not travelling no more than 5mph at the time of the accident. MV1 did not have any damage to the vehicle and was able to leave the scene. Christopher reported no injuries. Several photographs were taken of the scene and submitted to the IT Bureau. Thangthong stated that her three old son Mighty Mettanant was a passenger in her vehicle. Thangthong stated that she was driving approximately 5 MPH as she drove into the driveway. The driveway runs east to west and Thangthong's vehicle was heading west into the driveway as the sun was in the Western sky creating a glare. Thangthong stated that she did no see anything in the driveway prior to her pulling into it. Thangthong stated that she felt the front of her vehicle strike what she described as "" something "" and stopped her vehicle. Thangthong stated that she put her vehicle in reverse and then into park and exited her vehicle to see what she had struck. Thangthong stated that she saw her two nieces, Kalla Mongkhonlertsirkul age three and Kelley Mongkhonlertsirkul age seven laying in the driveway in front of her vehicle. Thangthong stated that she then realized that her vehicle had stuck her two nieces as they were in the driveway. Both of the Mongkhonlertsirkul sisters were conscious and alert and were able to walk to an ambulance for transport to Newton Wellesley Hospital for treatment. Thangthong stated that she and her son were not injured in the crash. I observed no damage to Thangthong's vehicle. I responded to the Newton Wellesley Hospital and spoke with the Mongkhonlertsirkul sisters and their mother Piyada Kellum. Kellum stated that her oldest daughter Kelly is a student at the Franklin Elementary School . Kellum stated that her daughters told her that they were playing in their driveway when their aunt's vehicle entered the driveway and did not stop. I spoke with both of the Mongkhonlertsirkul sisters who told me that they tried to get out of the way of their aunt's vehicle but could not in time. The Mongkhonlertsirkul sisters stated that they were both struck in the back and fell to the ground between the two front tires of their aunt's vehicle. Kalla Mongkhonlertsirkul had scrapes to her right knuckles. Kelly Mongkhonlertsirkul had lacerations to her lower left arm and the left side of her face near her eye. I spoke with hospital staff who stated that there did not appear to be any broken bones or other internal injures. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street and Paulson Road are both public ways maintained by the City of Newton. I spoke with the injured bicyclist in front of 165 Paulson Road, Alexander Mendonez. Mendonez stated he was traveling Westbound on Beacon Street on his bicycle near Paulson Road. Mendonez stated he observed a late model gray sedan ""inch to a stop"" on Beacon Street (W) at Paulson Road. Mendonez stated he did not remember if the vehicle had it's right directional on at the time. Mendonez stated as he was traveling through the intersection with the vehicle on his left, the vehicle took a right turn onto Paulson in front of him. Mendonez stated he attempted to brake and veer to the right of the vehicle, but was unable to avoid the collision. Mendonez stated his left side made contact with the rear passenger door/rear fender area of the vehicle. Mendonez fell to the roadway and the vehicle left the area Northbound on Paulson Road. Mendonez was unable to see who was operating the vehicle. Newton Medics responded and treated Mendonez for injuries to his left knee/arm area. Mendonez signed a patient refusal with Newton Medics. I observed minor damage to the left handlebar area of Mendonez's bicycle (black Trek). Mendonez was wearing a white helmet at the time of the accident with a blue shirt and white and black shorts. Pictures were taken of the area and bicycle and submitted to the IT Bureau. Mendonez was able to provide a partial Massachusetts registration of ""7BS32"". I attempted run this plate with multiple combinations. I searched the area for a vehicle matching the description given by Mendonez with a negative result. On July 16, 2019, I spoke with a witness to the incident Mark O'Hearne. O'Hearne stated he was traveling Westbound on Beacon Street a few car lengths behind where the accident occurred. O'Hearne stated he observed a bicyclist on the right side of the road abruptly stop and fall to the roadway. O'Hearne stated he observed a vehicle in the path of the bicycle. O'Hearne stated the vehicle did not stop after the bicyclist fell and took a right onto Paulson Road from Beacon Street. O'Hearne stated he followed the vehicle involved in the accident to Commonwealth Avenue, and while doing so took a picture of the vehicle's license plate. O'Hearne stated he did not observe who was driving the vehicle. The Massachusetts registration provided to me by O'Hearne is 7BS322. This vehicle comes back to a 2010 Volkswagon Jetta registered to Peter Giorgi (S20952090) out of Paxton, Ma. I spoke with Peter via telephone. Peter acknowledged he is the registered owner of the vehicle, but stated his daughter was using the vehicle and was in the Newton area. I spoke with Lindsey Giorgi (S35711247) via telephone. Giorgi stated she was operating MA: 7BS322 in Newton yesterday, but does not recall being in an accident. Giorgi stated she is not familiar with the area and does not know the area where the accident occurred. Giorgi stated there is no damage to her vehicle and stated she does not believe she hit anyone. Giorgi emailed me 4 photos of her vehicle where I described the damage would be. I observed a small area of minor damage to the rear passenger side door area of this vehicle. After speaking with all parties involved, I am requesting a hearing in Newton District Court for Chapter 90, Section 24 (Leaving the Scene, Personal Injury). Giorgi was advised and mailed Massachusetts Uniform Citation T1269847. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Ida Street for a report of a 3 car motor vehicle crash with entrapment. The weather at the time of the crash was clear and sunny. The road surface was dry. Centre Street and Mt. Ida Street are both public ways maintained by the City of Newton. On arrival, I was updated by Sgt. Chisholm that one of the vehicles (MV3) had a female operator that was entrapped as a result of the collision. She was freed from the vehicle by the Newton Fire Department and transported to Beth Israel Hospital. Another operator (MV2) was transported to Newton Wellesley Hospital with minor injuries. A third operator (MV1) remained on scene with no injuries. With the assistance of Officer Wade we conducted a survey of the crash scene. We observed a large debris field at the intersection of Centre Street and Mt. Ida Street with debris from all three vehicles. The point of impact was observed to be on Centre Street (N) before Mt. Ida Street. There were fresh scrapes in the pavement as a result of the colission. We also observed skid marks from MV1 measuring 57 feet 8 inches from the start of it's skid to the vehicles final rest MV1 crashed into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane of Centre Street and crash into MV3 as it was passing by. The impact from MV2's front end crashing into MV3's driver side resulted in the operator of MV3 becoming entrapped. MV2 and MV3 came to final rest on Wesley Street (W) at Centre Street (S). The driver door of MV3 was inaccessible. The collsion resulted in air bag deployment for MV2 and MV3. I observed moderate damage to MV1's front grill/bumper area. I observed substantial damage to the front and rear of MV2 and substantial damage to the front and driver side of MV3. Tody's responded and removed MV2 and MV3 from the roadway. Photos were taken of the scene and submitted to the IT Bureau. Officer Martin of the Watertown Police Department was requested by Sgt. Lee to respond to the scene and assist with a commercial truck inspection of MV1. Officer Martin contacted Trooper Grey of the Massachusetts State Police Truck Team who also responded to assist with the truck inspection. Sgt. Lee remained with Officer Martin and Trooper Grey during the inspection. MV1 was relocated to Richardson Street for the inspection. Trooper Grey determined the truck was in compliance with all required paperwork and it passed it's mechanical inspection. MV1 was removed from the roadway by a state tow. I spoke with the operator of MV1, William Tisbert (S24792655). Tisbert stated he was operating a 2006 KW W900 Semi Truck (NH SEMI: AP2638) Northbound on Centre Street. Tisbert stated as he was approaching Mt. Ida Street, a vehicle came out of no where and he hit it. Tisbert stated there was no one in front of him and then a vehicle came off of a side street (Mt. Ida Street) and attempted to turn left in front of him to return to the side street. Tisbert stated he could not avoid hitting the vehicle. Tisbert crashed into the rear of MV2. MV2 then crossed into the opposite lane and crashed into another vehicle. Tisbert remained on scene and reported no injuries. Tisbert stated he was wearing his seatbelt at the time of the accident and was not distracted. Tisbert later admitted to crashing into the rear of MV2. I spoke with the operator of MV2, Janet Vanbuskirk (S59829146), at Newton Wellesley Hospital. Vanbuskirk stated while operating her 2004 Toyota Highlander (MA: RS 529Y), she was stopped on Centre Street (N) at Mt. Ida Street. Vanbuskirk had her left directional on and was waiting for traffic in the Southbound lane to clear to take a left turn onto Mt. Ida Street. Vanbuskirk stated MV1 crashed into the rear of her vehicle. As a result of the impact, Vanbuskirk's vehicle traveled into the opposite lane and crashed into the driver side of MV3. Vanbuskirk reported minor injuries to her neck, left arm, and right hand. I spoke with the operator of MV3, Madeleine Karolian (S23979811), at Beth Israel Deaconess Medical Center in Boston. Karolian stated she was operating her 2016 Toyota Prius (MA: EV5746) Southbound on Centre Street. Karolian stated MV1 crashed into MV2, and MV2 crashed into her vehicle. Karolian stated she did not have time to avoid the collision. Karolian stated she had minor injuries to her neck and chest. Karolian's vehicle had a dash cam recording at the time of the accident. I viewed the footage with her through an app on a cell phone. The footage shows MV3 traveling Southbound on Centre Street on August 8, 2019 at 1308 hours. Ahead of MV3 in the Northbound lane, MV2 is seen stopped on Centre Street before Mt. Ida Street with it's left directional activated. I observed MV1 travel Northbound on Centre Street and crash into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane and crashed into the driver side of MV3. The footage then ends. A copy of the footage was emailed to my work email. The footage was then forwarded to IT Director Steve Smith to be added to the media for this report. As a result of my investigation, the operator of MV1, William Tisbert, will be cited for Newton City Ordinance Chapter 19, Section 71 (Following too closely). Gianelly stated she was crossing Walnut Street in a marked crosswalk towards Washington Park after leaving the Newton Senior Center located at 345 Walnut Street, Newton. Gianelly stated a vehicle exited Washington Park and turned left onto Walnut Street directly at her. Gianelly stated she believes the driver did not see her. Gianelly stated the vehicle hit her while she was in the marked crosswalk on her left side. Gianelly stated she then fell to the roadway. Gianelly was taken to St. Elizabeth's hospital in Boston for injuries to her ribs, back, and sides. I spoke with the operator of MV1, Alex Cook-Williams (S13317988). Cook-Williams stated he was operating his 2009 Pontiac Vibe (MA: 222FV5) Westbound on Washington Park towards Walnut Street. Cook-Williams stated he came to stop at the intersection and observed no traffic in either direction of Walnut Street. Cook-Williams stated he made a left turn onto Walnut Street and did not see the crossing pedestrian until it was too late. Cook-Williams stated he slammed on his brakes and the pedestrian landed on his hood. Cook-Williams stated the pedestrian then fell to the roadway. Cook-Williams reported no injuries. Cook-Williams stated he did not know if the pedestrian was in the crosswalk or not at the time of the crash. I observed minor damage to the passenger side hood/bumper area. Cook-Williams stated he was not distracted at the time of the accident. After speaking with all parties involved, Cook-Williams was issued Massachusetts Uniform Citation T2016426 for Chapter 89, Section 11 (Fail to Yield, Pedestrian in Crosswalk). The operator of MV2, Patrice Lys, stated he was traveling Westbound in the parking lot of the Angier Elementary school in a 2018 Ford EcoVan (MA MVN: M2546A). Lys stated a vehicle on his driver side backed out of a parking spot and the rear end of MV1 crashed into the rear driver side of MV2. Lys reported no injuries. I observed minor damage to the rear driver side area of MV2. Wint was driving a 2018 Chevrolet Colorado pick up tuck ( MA reg 8CA823 ) that Auto Zone leases from Lease Plan USA Ltd. Wint stated that he was headed to Webster St in Newton to pick up a delivery. Wint stated that as he approached the area of 277 Lexington St he believed that the traffic light that is at the marked crosswalk was green for south and north bound traffic. Wint stated that he did not see any pedestrians in the crosswalk, until he heard the sound of an object impacting into the front driver's side of his vehicle. Wint stated that he stopped his vehicle approximately ten yards past the crosswalk and got out. Wint stated it was then he realized that his vehicle had stuck a female and a dog. Wint stated that he was not operating any electronic devices at the time of the crash, and I observed none in his vehicle. Wint stated he was not injured in crash. Wint's vehicle had damage to the driver's side mirror and lower molding of the driver's side front door. Wint's vehicle did not require a tow. Witness Bessy Shanks stated that she was driving in her vehicle behind Wint's vehicle south bound on Lexington St. Shanks stated that Wint's vehicle was driving slow and swerving approximately two blocks prior to the crash. Shanks stated that she believed that Wint may of been distracted. Shanks stated that south and north bound traffic was stopped because that cross walk light was red and that a female and a dog were crossing from the east to west in the marked crosswalk in font 227 of Lexington St. Shanks stated that the front driver's side of Wint's vehicle srtuck the female and the dog as they were approximately halfway into the crosswalk. Shanks stated that the female and dog both fell to the ground after the crash. Witness Elizabeth Keegan stated that she was driving in her vehicle south bound on Lexington St two vehicle behind Wint's vehicle. Keegan stated that she was not sure what color the traffic light was at the time of the crash, but traffic was stopped both not and south bound traffic was stopped. Keegan also stated that Wint's vehicle "" didn't even slow down "". Keegan stated that Wint's vehicle struck the female pedestrian and the dog as they were in the marked crosswalk in front of 277 Lexington St. Witness Henry Taylor stated that his vehicle was the first one stopped at the red crosswalk light at 227 Lexington St in the north bound lane. Taylor stated that north and south bound traffic was stopped and that Wint's vehicle did not stop and stuck a female pedestrian and a dog in the crosswalk. Witness Jacqueline Deos stated that her vehicle was stopped behind Taylor's north bound on Lexington St. Deos stated that north and south bound traffic was stopped for the red light . Deos stated that Wint's vehicle did not stop and that it struck a female pedestrian and a dog in the marked crosswalk. The pedestrian in the crosswalk was identified as Xiaoping Sun. Sun stated that she and her dog stopped on the south bound side of the marked crosswalk across from 227 Lexington St. Sun stated that she activated the crosswalk light and waited until the light was red for north and south bound traffic, and that the walk single was activated. Sun stated that she looked to her left and saw traffic north bound traffic was stopping. Sun stated that she turned to her right and entered the marked crosswalk. Sun stated that she and her dog had crossed over the north bound side of Lexington St and she then looked to her right and saw Wint's vehicle not stopping and "" going very fast "". Sun stated that the front driver's side mirror of Wint's vehicle stuck her in the face . Sun stated that as she fell to the ground she was struck on the left leg by Wint's driver's side. Sun stated that she did not lose consciousness after she had fallen to the ground. Sun was transported via EMS to The Brigham and Women's Hospital for treatment for head and neck injuries and abrasions to her legs. Sun's dog died as a result of being struck by Wint's vehicle. Photo's of the crash site and of Wint's vehicle were taken and the disc was downloaded by the NPD's IT Bureau. Before leaving the crash site, I tested the crosswalk lights and they were in working order. Based upon my observations of the crash site, Wint's vehicle, and the statements made to me, I concluded that Wint was at fault for this crash. The bicyclist was dressed in black pants, and a gray sweater, and was not wearing a helmet. The bicycle sustained damage to the handlebars. At the time of the accident the sun was setting, however the street lights had not yet come on. I then spoke with the operator of MV1 who stated she was traveling southbound on Linwood avenue, and was attempting to turn right onto Watertown street when she struck the bicyclist. The operator stated she never saw the bicyclist before the collision. The front of MV1 sustained minor damage. I was able to speak with a witness to the accident over the phone after clearing the scene. The witness stated that MV1 failed to stop at the stop sign at the intersection of Linwood and Watertown street, and that the bicyclist was already well into the crosswalk at the time of the collision. Officer Howes took pictures of the accident scene that have been submitted to the IT bureau. Bertram stated he was stopped in the roadway waiting for traffic in the Southbound lane of Grant Avenue to clear so he could take a left turn onto Marshall Street. Bertram stated as he made his turn MV1 crashed into him. Bertram fell to the roadway and sustained an injury to his left leg/foot. Bertram was wearing a helmet at the time of the crash. Bertram signed a patient refusal with Newton Medics. Bertram has a learner permit to operate the moped. Bertram's moped was removed from the roadway by Tody's Towing. A witness to the crash, Michael Rothstein, stated he observed the crash right at impact and observed the moped fall to the roadway. Rothstein stated he pulled over to check on the operator of the moped. Rothstein provided a ride for Bertram after the crash. A query of Ms. Owen's license status through Newton Dispatch showed it was suspended. The temporary Pennsylvania registration for the vehicle was also expired as of August 2020. A small red and gray CVS like type shopping carriage was knocked over and resting on it's side in front of the victim's head. The carriage was moved to the side in order to tend to the victim I also observed a black Honda Civic bearing MA reg.# 3741TW that was involved in the accident. The Honda was out of the parking spot resting approximately two feet away from the pedestrian's legs. I then spoke to the pedestrian identified as Darla Gipson in the rear of the ambulance. She sustained an abrasion to her front left shoulder and arm area which I observed. She also complained of left knee and elbow pain. She stated that she walking through the middle of the parking lot pushing the carriage by the door entrance in a westerly direction towards Chestnut Street. She states that she never saw a vehicle backing up and then she was suddenly struck and believes she was pushed underneath the vehicle. She was then transported to Newton Wellesley Hospital by Cataldo Ambulance. I then spoke to the operator of the Honda Civic identified as Linda Burke (MA S70320414). Ms. Burke was parked in the first spot to the right of the walkway that leads to the entrance/exit doorway. I asked her what occurred and she stated that she was backing her vehicle up to exit the parking lot when she looked behind her not seeing anything in sight. She then started backing up and struck the pedestrian without realizing it. She says the driver of a green cab and his female customer who were no longer on scene were yelling at her to stop her vehicle because the pedestrian was underneath it. She then stopped her vehicle. I observed minor scrapes to the entire rear bumper of the vehicle so it is unknown where it actually struck the pedestrian/carriage. I observed no obvious signs of damage to the carriage. It should be noted that Ms. Burke's Honda does not have a back up camera or back up sensors. There are no surveillance cameras to the apartment complex and no witnesses came forward for the accident. As a result of the investigation, Ms. Burke will be issued Uniform Citation # T1271069 for Newton City Ordinance 19-75 Failure To Use Care in Backing. On Monday, March 15, 2021, I contacted Hertz rental company via e-mail in an attempt to identify the party operating the rental vehicle that left the scene of this crash. Hertz Corporate Security and Safety Manager Mr. Robert Irwin was able to provide me with contact information for Mr. Michael Giannetti (S38856696). Mr. Giannetti's rental agreement lists him as being the rentee of the involved 2019 Chevrolet Cruze (NY: JBE2636). I spoke with Mr. Giannetti via telephone. Mr. Giannetti acknowledged being involved in the crash with MV1 (MA: EV957L). Mr. Giannetti stated he was traveling down Adams Street when MV1 backed out of a driveway in front of him. Mr. Giannetti stated he attempted to drive around MV1, but was unsuccessful. Mr. Giannetti stated MV1 backed into his front passenger fender area. Mr. Giannetti stated as a result of the crash his vehicle sustained a flat tire to the front passenger side. Mr. Giannetti stated he left the area and then circled back around for the other vehicle but could not find it. Mr. Giannetti stated because of the flat tire he continued to a nearby gas station to try and get the flat tire repaired. Mr. Giannetti stated he could not get the flat fixed at this time and arranged for alternate transportation. Mr. Giannetti stated he contacted Hertz today to make them aware of the damage. Mr Giannetti stated Hertz notified him that Newton PD had called and made an inquiry about his vehicle being involved in a crash. Liu stated that she did not observed any "" cars "" driving east bound on Commonwealth Ave so she proceed to make the left turn onto Chestnut St. Liu stated that she realized that her vehicle has struck an unknown object as she was turning left. Liu stopped her vehicle and saw that her vehicle had struck a female bicyclist. Liu stated that the bicyclist was thrown from their bike and was on the ground. Liu stated that she was not injured as a result of the crash. Liu's vehicle had damage to the front passenger side hood . The front passenger side of Liu's windshield had an inward crack that was approximately 12 inches in length and 12 inches in width. The type of inward crack on the windshield is indicative of an outside force making contact with it. Liu's vehicle was towed by Tody's Services to their lot for storage. I filled out a towed motor vehicle form. Bicyclist Isabella Turinovic stated that she was ridding her bike , east bound on commonwealth Ave near Chestnut St. Turinovic stated that east bound traffic had a green light. Turinovic stated that as she was crossing over Chestnut St Liu's vehicle struck her causing her to hit the windshield. Turinovic was tossed from her bike and onto the roadway. Turinovic was wearing a helmet at the time of the crash. Turinovic stated that she did not lose consciousness and she was transported to Newton Wellesley Hospital via EMS from treatment . Turinovic agreed to have her bike taken by NFD Ladder One to be stored at NFD station Two. I took photo's of Liu's vehicle, Turinovic's bike and the disk was downloaded by the NPD IT Bureau. Witnees Alexander Delima stated that he was the first vehicle stopped a red light on Chestnut St in the south bound lane. Delima stated that Liu's vehicle did not yield to Turinovic as she turned left onto the south bound lane of Chestnut St from the west bound lane of Commonwealth Ave .A second witness Lindsay Bentis stated that she was the second vehicle stopped a the red light in the south bound lane of Chestnut St behind Delima. Bentis stated that she also saw Liu fail to yield to Turinovic as she turned onto Chestnut St. Based upon statements made to be by all involved parties, I concluded that Liu is at fault for this crash. Photos were taken and submitted to the IT Bureau. The injured pedestrian, Mrs. Joan Contompasis, stated she was walking on Boylston Street (W) with her husband. Mrs. Contompasis stated she entered the roadway from off the sidewalk to cross the Avalon parking lot exit and continue on Boylston Street. Mrs. Contompasis stated she believed MV1 had stopped to allow her and her husband to cross. Mrs. Contompasis stated the vehicle then accelerated and it's front passenger side bumper area crashed into her left side. Mrs. Contompasis fell to the roadway. It was also reported by Newton Dispatch that one of the occupants on the bus had suffered an arm injury as a result of the crash. Meadowbrook Road is a public way maintained by the City of Newton. The road surface at the time of the crash was dry. The weather at the time of the crash was clear and sunny. When I arrived, I was updated by Officer Iarossi that the crash involved a City of Newton contracted school bus transporting students from the Williams School (141 Grove Street, Newton). The students were participating in ""Step Up"" day and were leaving Brown to go back to Williams. Officer Iarossi stated a student on the bus was injured as a result of the crash. Tegan Quirk was transported by Fallon Medics to Newton Wellesley Hospital with a right arm injury. The remaining students that were on the bus reported no injuries to staff at the time and were moved to another bus to be transported back to the Williams School. I asked the Brown staff that was assembled in front of the school to contact the Williams School and ask them to hold the children involved in the crash at the school once they arrived. I made this request so we could make sure there weren't any new injuries reported; and I wanted to inquire if any of the children witnessed the incident. The staff stated they would contact Williams and relay the request. I spoke with the operator of the school bus (MA BUN: BU44536), Mr. John Aubert (S48613114). Mr. Aubert stated he was operating bus #361 while transporting students from Brown Middle School to the Williams School. Mr. Aubert stated as he was preparing to leave Brown, he made an announcement to students on the bus to sit down. Mr. Aubert stated he got the okay to leave from one of the staff members on the bus. Mr. Aubert stated there was another bus parked in front of him prior to leaving. Mr. Aubert stated he began to reverse the bus to create distance from the bus in front of him, and then operated the bus forward to exit from his position on the right side of the road and merge into the travel lane to his left around the bus parked in front of him. Mr. Aubert stated as he merged onto the roadway, he heard screaming from the students on the bus and the rear passenger side of the bus made contact with a brick support column in front of the school positioned along the sidewalks edge next to the roadway. Mr. Aubert stated he exited the bus and observed a student's arm pinned on the rear passenger side of the bus between the side of the bus and the brick column. Mr. Aubert stated he got back in the bus and reversed it until the students arm was free. Mr. Aubert stated at this time, staff helped the injured student. The staff there helped facilitate transportation for the remaining students back to the Williams School. Mr. Aubert reported no injuries, but was concerned for the welfare of the student. After speaking with Mr. Aubert, I observed damage to the school bus and the brick support column as a result of the crash. I observed fresh damage (scrapes) to the rear passenger side of the school bus between the last passenger side window and the back of the bus. I also observed fresh damage to the brick support column. The column is a support to an overhead covering that runs from the sidewalk to the front entrance of the school. There was fresh paint transfer from the bus and a crack in the column. Pictures were taken of the damage and submitted to the IT Bureau. The travel lane where the crash took place is a one way access road that runs from Meadowbrook Road to the front of the school, and exits onto a parking lot between the Brown Middle School and the Oak Hill Middle School. I responded to the Williams School to follow up with staff there regarding this crash. All of the students that were involved in the crash had been released prior to my arrival. I spoke with the Principal of the Williams School, Ms. Ayesha Farag, Ms. Michal Shapiro (Social Worker), and Ms. Katie Costello (School Nurse) about the students on the bus. They stated no injuries were reported to them, and they notified the parents of the involved parties about the crash. They also provided me with contact information for all of the students on the bus at the time of the crash. I spoke with the three teachers that were on the bus at the time of the crash, Ms. Kimberly Andersen, Ms. Tess McLean, and Ms. Katherine O'Leary. All three teachers stated prior to the bus leaving Brown Middle School they accounted for all of the students and were ready to leave to travel back to the Williams School. One of the teachers recalled hearing the bus driver make an announcement to the bus that they were about to leave, but could not recall the exact wording of the announcement. The teachers stated as the bus began to move, they heard screaming from the rear of the bus, looked back, and saw a student's arm stuck outside the window. After realizing the student was injured, they assisted with helping to free the student from his pinned position. They helped the injured student until EMS arrived on scene. They assisted the remaining students by directing them away from the crash and onto another bus for transportation back to Williams. The three teachers reported no injuries as a result of the crash. I responded to the Newton Wellesley Hospital Pediatric Emergency Room to check on the status of the injured student, Tegan Quirk. I spoke with Tegan and his mother, Kathryn Quirk in his room. While attempting to talk to Tegan and his mother, Tegan became very upset and did not want to talk about the specifics of the crash. Out of courtesy for Tegan, I exited the room and spoke with his mother outside the room. I explained to Ms. Quirk the events of the crash, and how her son's arm became injured. I asked Ms. Quirk if she could assist me with faciliating a conversation with Tegan about the events of the crash. I explained to her I understood how traumatic the events of the crash were for her son, but as part of my investigation I would need to find out what led to Tegan's arm being outside the bus window at the time of the crash. While speaking to his mother, Tegan explained that he was waving to a friend with his arm out of the bus window when the bus started to move. Tegan stated his arm hit the column, and became stuck between the bus and the column. Tegan stated he was screaming for the bus to move and that it took forever for the bus to move so he could free his arm. Tegan stated the bus driver gave no warning to the students that the bus was going to start moving. The staff at Newton Wellesley Hospital stated Tegan has a right arm injury. The severity of the injury is unknown at this time due to further examination that needs to take place. I advised Ms. Quirk about the conversations I had with the other parties involved. I spoke with Mr. David Stickney (Head of Facilities) from the Newton Public Schools to try and obtain footage of the crash. Mr. Stickney stated unfortunately there is not footage of the crash from the Brown Middle School or Oak Hill Middle School surveillance cameras. I advised Ms. Regina Moody (Newton Public Schools Transportation Director) about the crash. Ms. Moody was able to show me footage of the incident from the camera inside the bus. In the footage, the bus driver can be heard announcing to the students prior to the bus moving, ""Guys, guys, get off the seat and sit correctly"". One of the teachers can be seen doing a count of the students. The teacher is heard saying to the student, ""Everyone needs to be in a seat and sitting at 3 and 2 and 1 and thank you."" Student are observed still standing up. The teacher is then heard saying, ""sit down sit down."" After the teacher passes the students in the back of the bus, she says, ""We are good."" to the front of the bus. Students are then observed standing after she passes. As the bus begins to back up, students can be observed standing in the rear of the bus and not sitting down on their seats. As the bus moves forward, a student (Tegan Quirk) can be seen standing on his seat and extending his right arm out of the last window on the passenger side of the bus. Screaming can then be heard as his arm makes contact with a support column on the passenger side of the bus. The bus is stopped, the driver exits the bus to asses what happend, gets back on the bus, and reverses the bus a short distance. The injured students right arm is released and he brings his arm back into the bus through the window. The amount of time between the students arm being pinned against the column to when it is released is approximately 40 seconds. After speaking with all parties involved and reviewing the footage from inside the bus, it appears that Bus #361 was pulling out of it's parked position in front of the Brown Middle School to enter the roadway and return students to the Williams School. A student that was sitting in the last row on the passenger side of the bus extended his right arm out of a window next to his seat to say goodbye to a friend. While his right arm was extended out of the rear passenger side window of the bus , the bus collided with a brick support column affixed to the sidewalk in front of the school. When the bus made contact with the column, the students right arm made contact with a brick support column and became pinned between the column and the passegner side of the bus. Detail officer Boudreau stated he stopped MA REG 765RL2, a 2013 Chrysler town van so the ambulance could get by and as the ambulance went to pass the vehicle on the left the Chrysler van proceeded to attempt to take a left turn onto Highland Street and struck the ambulance on its passenger side. The ambulance continued onto Washington Street as it was transporting a patient to NWH. Detail officer Boudreau instructed the driver of the van identified as Janet Waitz (S76373427) to pull into the parking lot of the police station and wait for an officer to file a report. The ambulance was advised to respond to the police station after transport. No injuries were reported. I spoke with Janet who stated she was attempting to take a left turn onto Highland Street and was stopped by an officer. She stated she thought the officer waved her through so she proceeded to take the left turn and struck the ambulance as it passed her. Her vehicle had minor front end damage and she reported no injuries. The operator of the ambulance arrived shortly after and was identified as Shane McMahon (S249990773). He stated he was transporting a patient to the hospital and was traveling westbound on Washington Street. An officer had the traffic stopped so he could pass safely and as he passed the Chrysler van it attempted to take a left and struck the passenger side of the ambulance causing minor damage. He stopped and spoke with the officer who advised him to continue to transport the patient and respond back to the police station after to file a report. No injuries were reported. MV2's front driver side bumper/fender area crashed into the middle passenger side of MV2. The operator of MV1 reported no injuries. While making his turn, his vehicle crashed into a bicyclist traveling on Beacon Street (E) in the marked bicycle lane in front of 358 Beacon Street. Mr. Mogan stated he did not see the bicyclist until the front passenger side fender/door area of his vehicle made contact with her. I observed minor damage to this area on his vehicle. Mr. Mogan reported no injuries. The injured bicyclist was identified as Ms. Louisa Palmer (S67185637). Ms. Palmer was treated on scene by Fallon Medics for injuries to her left wrist and ankle and transported to Brigham and Women's Hospital for further evaluation. Ms. Palmer was wearing a helmet at the time of the crash and I observed her blue Bianchi Pista bicycle resting on the sidewalk in front of 358 Beacon Street. The bicycle had minor damage in to it's front wheel area and fresh scrapes near the handlebars. I transported the bicycle to Ms. Palmer's residence. I responded to Brigham and Women's Hospital in Boston and spoke with Ms. Palmer in the Emergency Department. Ms. Palmer stated she was riding her bicycle on Beacon Street (E) towards Boston College. Ms. Palmer stated she was riding in the marked bicycle lane with the motor vehicle traffic to her left. Ms. Palmer stated she observed a break in the traffic on Beacon Street ahead of her, and then a Jeep entered her travel lane after it took a left turn from the Westbound travel lane on Beacon Street right in front of her. Ms. Palmer stated her left side crashed into the front passenger side wheel/hood area. Ms. Palmer stated she then fell to the roadway. Ms. Palmer stated she has injuries to her left wrist/arm area, her left ankle, and her hips. I spoke with a witness to the crash, Ms. Kelli McCusker. Ms. McCusker stated she was traveling on Beacon Street (E) towards Boston College at the time of the crash. Ms. McCusker stated there was heavy traffic in the area as she observed a bicyclist travel by the passenger side of her car. Ms. McCusker stated there was a break in the traffic ahead of her in the Eastbound lane and she observed a Jeep in the Westbound lane of Beacon Street make a left turn in front of the car in front of her. Ms. McCusker stated the Jeep and the bicyclist crashed into each other at this time. After speaking with all parties involved, Mr. Mogan did not yield to the right of way while Ms. Palmer was traveling on Beacon Street (E) on her bicycle. Mr. Mogan will be issued Massachusetts Uniform Citation for Chapter 90, Section 14B (Fail To Yield To Bicyclist on Turn). Wade notified me that he received an email regarding a motor vehicle accident involving a pedestrian/student that occurred on 10/15/21 between the hours of 08:20am and 08:35am. The crash was never reported by the operator of the vehicle or the student/pedestrian. Approximately two hours later, the student went to the nurse's office seeking assistance for neck pain which is how the school became aware. This crash occurred in the marked crosswalk on Brandeis Road in front of Newton South High School which is located at 139 Brandeis Road. The weather at the time was 63 degrees with mostly cloudy and sunny conditions (wunderground.com). Brandeis Road is a two way roadway divided by an unprotected median that runs east and westbound. In front of the school there is a marked crosswalk in the roadway. There is also a one way driveway near the main entrance of the school with parking stalls and parents can drop their children off there on the right side of the driveway. The driveway then leads back out to Brandeis Road. Right after you exit the lot there is the above mentioned crosswalk to the left of the driveway which is very visible with nothing obstructing your view. On arrival, I spoke to the Dean of the school named Charlie Myette. Mr. Myette brought me to his office and started looking at the school's video surveillance system. He also requested that the student/pedestrian involved to report to his office. The student was identified as Tsukasa Iinishi. I asked Mr. Iinishi what had occurred and he stated that he was walking to school on the morning of 10/15/21. He says that he was wearing a pink hoody sweatshirt with gray shorts. He was walking through the woods which is across the street from the school which exit out to Brandeis Road. He says that he waited at the crosswalk and let two vehicles go westbound by him. He then thought it was clear to proceed and started walking. At the tail end of the crosswalk he was struck by a red colored Toyota Prius which exited left from the driveway and was on the left side of the roadway. He was struck on the left side of his body and he says that he used his left hand to brace himself against the hood and that's when the Prius stopped. He says that the operator of the Prius asked him if he was okay and he replied that he was. He then kept walking toward the school. The operator then continued on it's way without providing any information or notifying us. He described the operator as an Asian female with black hair approximately 40 to 50 yrs in age. During this time, Mr. Myette was able to find surveillance video of the crash. There are two camera angles. One shows the driveway along with the walkway of the school building. The other one shows the same but it also overlooks out in to Brandeis Road. The view of the road is partially blocked from the parked cars in the lot. The video shows a red colored Toyota Prius pull in to the driveway of the school. An Asian female student wearing a white long sleeve shirt with green lettering and dark colored jeans exits the passenger front of the Prius and eventually enters the school. The Prius then pulls out and goes toward the left side of the driveway around other vehicles waiting to exit. At 08:27 hours, the Prius pulls out angling towards the left side of the driveway going around other vehicles waiting to exit. The Prius then exits the lot on the left side (wrong side) of the roadway and makes contact with the pedestrian. In the video you cannot see the front hood area of the car, but you see Iinishi's torso bend towards the vehicle on impact. It shows the Prius stopped there for approximately ten seconds and then it drives away. In the video you are not able to see the registration plate number. Mr. Myette is going to send the photo of the student dropped off in the Prius to his staff to have her identified so I can follow up with the operator. On 10/21/21, I received an email from Mr. Myette stating that some of his staff has not come forward on the identification of the student. He stated that he would now send a message out to a larger group of staff to see if he gets a response. He also attempted to enhance the registration plate using the surveillance software but the plate is still too blurry to make out. On 10/25/21 at approximately 08:40 hours, I went over to Newton South High School to see if I could locate the red Toyota Prius involved in the crash. As I was driving on Olde Field Road approaching Brandeis Road near the school, I observed a red Prius approximately four cars ahead of me in heavy school traffic. Eventually, I observed the Prius pull in to the drop off driveway of the school and drop an Asian female student off. I identified her as the same student that got out of Prius prior to the past accident on 10/15/21. The operator was an Asian female which appeared to be in her late forties. After, the Prius exits the lot on to Brandeis Road and I flagged her down. I had her pull in to the parking lot across the street from the school. I asked her if she was involved in a accident with a student on the morning of 10/15/21 to which she replied that she was. She was identified as Xiaojin Liu through her MA Driver's License # S49033927. She was operating a 2010 Toyota Prius bearing MA reg.# 8JY630. I asked her what occurred and she stated that she was exiting the driveway of the school to turn left on to Brandeis Road. She says that she has a habit to think that no one is ever walking through the crosswalk and thought it was clear to proceed. She turned and then saw the student crossing and attempted to stop but was not able to in time. She then bumped the student with the front of her vehicle. She says that she stopped and asked the student if he was ok several times and he stated that he was. The student then walked away and she says that she did not know what else to do. I advised her that she should of gave her information to the student, notify the school, or call us to which she understood. I issued MA Uniform Citation # 172129AB to Ms. Liu for Ch. 90/24/A Leaving the scene of personal injury. After, I went inside the school and notified Mr. Myette of my findings. I asked him for the surveillance footage as I previously did in person and through email. At this location, there is a marked crosswalk with a walk signal along with traffic signals above in both directions. On arrival, the scene was cleared up and both parties were waiting in the parking lot of the Burke & Blackington Funeral Home at 1479 Washington Street. I spoke to the bicyclist involved who was identified as Rajmattie Thakoordeen. I asked her what occurred and she stated that she was attempting to cross Washington Street in a southerly direction on her electric bicycle. She says that she pushed for the crosswalk signal to activate and once it did she began riding her bicycle when she was suddenly struck by a silver Saab 93. She made it out in to the crosswalk approximately four feet when she was struck by the Saab which was travelling westbound in the right lane of Washington Street. She says that she was struck on the left side of her leg area. She complained of her left leg being sore near her knee area but signed a patient refusal with Fallon Ambulance declining transport. At the time, she was riding a Rad Power Bike Roadrunner 1 Plus that is silver and black in color. It is a Class 2 electric bicycle (750W) that can go up to 20 m.p.h. The only damage it sustained was scrapes to the outside left pedal. Ms. Thakoordeen was wearing black/gray/white pants, yellow reflective jacket, and a black bicycle helmet at the time. Next, I spoke to the operator of the Saab 93 bearing RI registration # FN193. He was identified as Nevan Hanumara with Rhode Island Driver's License # 9960501. He stated that he was travelling westbound on Washington Street in the right travel lane when he came around the curve and noticed the red light signal above in the last second. He says by the time he reacted and attempted to stop he went through the red light and made contact with the bicyclist in the crosswalk. He reported no injuries and no damage to his vehicle. Due to his statement and inattention to the traffic signal, I issued him MA Uniform Citation # 168985AB for Chapter 89/9 Fail to Stop/Yield for the red traffic signal. Both parties work for the City of Newton Department of Public Works. Mr. Napolitano was operating a 2009 Ford F350 bearing MA MVN registration# M78129 at the time. He stated that he was attempting to back out of the driveway of 612 Washington Street. Once it was clear to proceed he backed out on to Washington Street and that is when a Subaru Impreza made contact with his driver's side front quarter panel area. He says the Subaru was making a left turn from the left westbound lane of Washington Street in to the same driveway he was backing out of at the time. There was no visible damage to the vehicle. I then spoke to the operator of the Subaru (MA reg.# 3GYT41) identified as Jennifer Shippie (MA D/L# S23299202). Ms. Shippie states she was in the left westbound lane of Washington Street and was turning left in to the driveway of 612 Washington Street. She further stated that a vehicle in the eastbound lane stopped for her and waved her on to proceed. She then made the turn and never saw the Ford F350 backing out and made contact with it on her passenger front bumper area. The entire vehicle had damage to the front bumper consisting of scratches and dents but she stated none of it was new from this accident. Mr. Isme works for the Eastern Bus Company which is contracted to transport children for the City of Newton public schools. At the time, he was operating a yellow 2022 Thomas School Bus bearing MA reg.# 1A6DJ. He states that he was traveling on the off ramp of Boylston Street (WB) at Parker Street and turned left on to Parker St. He was then trying to take a left turn on to the Boylston Street (EB) on ramp but could not due to traffic congestion. At this point, he was rear ended by Vehicle # 1. I observed no dents or paint damage to the rear of the school bus. I then spoke to the operator of Vehicle # 1 identified as Madeline Fordyce (MA D/L # SA3410026) who was operating a 2011 Chrysler 200 gray bearing MA reg. # 95S120 at the time. Ms. Fordyce stated she was behind the school bus coming off the westbound off ramp and was also making a left turn on to Parker Street. As she made the turn she noticed that the bus had stopped and attempted to brake, but she could not stop in time. The Chrysler sustained damage to the entire front bumper and the hood was pushed up. The vehicle was towed by Tody's towing. The operator of MV1, Mr. Michael Leone (SA4940776), stated he was operating his 2013 Dodge Challenger (MA: 2HZW73) on Washington Street (E) towards Armory Street. Mr. Leone stated a vehicle parked on the right side of the street pulled out from it's parking spot to merge into traffic and crashed into the front passenger side of this vehicle. Mr. Leone and his passenger reported no injuries. I observed damage to the front passenger bumper/fender/wheel/door area of MV1. The operator of MV2, Mr. Chad Natichioni (AZ: D08314037), stated he was operating a 2020 Nissan Rogue (MA:7BH214) at the time of the crash. Mr. Natichioni stated he was parked across from 1149 Washington Street in the Eastbound parking area. Mr. Natichioni stated when he attempted to merge into the Eastbound travel lane of Washington Street, the front driver side area of his vehicle crashed into MV1 as it passed. MV1 had the right of way in the travel lane at the time of the crash. Mr. Natichioni apologized and stated he was at fault for the crash. I observed damage to the entire front end of MV2. Tody's towing responded and removed the vehicle from the roadway. No injuries were reported by Mr. Natichioni and his passengers. The weather at the time of the crash was clear and sunny. The road Surface was dry. I spoke with Officer Durickas of the Newton Police Department in front of Brown Middle School. Officer Durickas stated he witnessed a City of Newton contracted school bus sideswipe a National Grid contractor's vehicle on the Boylston Street (E) offramp just before Parker Street. Officer Durickas stated the school bus was filled with students headed to the Brown Middle School. Officer Durickas collected information for the owner of the vehicle that was hit and escorted the school bus to the Brown Middle School. At Brown Middle School, I also spoke Principle Kim Lysaght. The 48 students that were on the school bus were escorted to the auditorum of the school by staff on arrival. They were all accounted for and evaluated by the school nurse. No injuries were reported and the students were released to attend classes. Brown Middle School provided me with a roster of the students on the school bus at the time of the crash. I spoke with the operator of the school bus, Mr. Adriano Docanto (S08942704). Mr. Docanto stated he was operating a 2018 Thomas School Bus (MA BUN: BU43164) at the time of the crash. Mr. Docanto stated he was attemtping to make a right turn from the Boylston Street (E) offramp onto Parker Street, when the rear passenger side of his bus made contact with the front driver side of a pick up truck parked on the right side of the roadway. Mr. Docanto reported no injuries. I observed minor damage to the rear passegner side of the school bus. The school bus is owned by Eastern Bus Company and is contracted by the City of Newton. I spoke with the owner of the other vehicle involved, Mr. David Nowlan (S18764520). Mr. Nowlan stated he is a contactor for National Grid and was conducting a survey of the immediate area where the crash happened for a reported gas leak. Mr. Nowlan stated his 2017 Toyota Tacoma (MA: 7WDW0) was parked unoccupied on the Boylston Street (E) offramp before Parker Street at the time of the crash. The accident occurred on Nevada St. just past a driveway of the Horace Mann School approaching the intersection of Nevada St and Linwood Ave. The weather at the time was sunny/clear and the road conditions were dry. School dismissal is at 14:50 hours and for approximately ten minutes Nevada Street is very congested with vehicles parked on both sides of the street. On arrival, I spoke to the operator of M/V # 2 identified as James Rice (MA D/L # S83251000). Mr. Rice's vehicle is a 2012 Ford Escape color white bearing MA reg. # 6725ZA. The Ford Escape was parked just past a school driveway facing southbound in a legal parking spot. He says he was picking up his child from the school. He further states that he looked and thought it was clear to open his door. He then opened his door and put his left foot and left arm out attempting to exit the vehicle when it was struck by M/V # 1. M/V # 1 struck the outer edge of the door and Mr. Rice's left elbow was also struck during the collision. Mr. Rice complained of left arm pain so Officer Boudreau had Fallon Ambulance respond to the scene. Rice then signed a patient refusal with them. Also inside the vehicle at the time of the crash was his son, Charger Rice. His son was seated and secured in a car seat in the second row of the driver's side of the Ford Escape. There was no injuries reported for him. I observed a crease to the outside edge of his driver's side door. Mr. Rice also stated to me that the crossing guard told him the other operator was speeding before the crash going approximately 25 miles per hour. Next, I spoke to the operator of M/V # 1 identified as Olga Kulakova (MA D/L# S63003743). She was operating a 2015 Infiniti QX60 color black bearing MA reg. # 5TJ151 at the time. Ms. Kulakova states that she was parked approximately twenty five feet back from the crash location also picking up her daughter from the school. After doing the pickup, she then proceeded out in to roadway going southbound on Nevada St. As she was approaching M/V # 2, she says the operator suddenly opened their door and she was not able to react in time striking his car door. She did not believe she ever struck the operator though. No injuries were reported. I observed white paint scrape marks to both her passenger side doors and the side view mirror was also damaged. I then spoke to the crossing guard assigned to the intersection of Nevada St. and Linwood Ave. The guard Jack Boudreau is employed through the City of Newton. I asked him what had occurred with the crash. He stated that he had his back turned away from the crash location when it occurred. He heard the collision which made him turn around. He then motioned for the driver of the Infiniti to pull over and stop. I asked him if he ever saw the Infiniti driving prior to the crash and he stated that he did not. I then inquired about the supposed speeding comment that he made to Mr. Rice regarding the Infiniti. He stated that he never said that to him. Ms. Wilson stated after she completed her right turn, she observed the pedestrian signal activate in front of her and a pedestrian began entering a marked crosswalk on Washington Street to cross. Ms. Wilson stated she stopped her vehicle to allow the pedestrian to cross. Ms. Wilson stated at this time, MV2 crashed into the rear of her vehicle. Ms. Wilson reported no injuries. I observed damage to the rear passenger side of MV1. The operator of MV2, Ms. Geraldine Bartlett (S68389347), stated she was operating her 2020 Jeep Grand Cherokee (Ma: 371TV3) Southbound on Lowell Avenue towards Washington Street. Ms. Bartlett stated she came to a stop at the red light on Lowell Avenue behind MV1. Ms. Bartlett stated she observed MV1 take a right turn onto Washington Street. Ms. Bartlett stated after MV1 made her turn, she observed no traffic in the area. Ms. Bartlett stated she then made a right turn onto Washington Street. Ms. Bartlett stated MV1 stopped in front of her in the roadway and she crashed into the rear of MV1. I observed damage to the front passenger side of MV2. Ms. Bartlett report not injuries. The operator of MV1, Mr. Norman Fryar (S20854611), stated he was operating a City of Newton owned 2017 Elgn Pelican street sweeper (MA: MVN M96170) on Berkshire Road. Mr. Fryar stated as he was sweeping the street he was in the Eastbound lane traveling North. Mr. Fryar stated there was a vehicle parked ahead of him and to avoid it he began to change lanes to his left to go around it. Mr. Fryar stated as he was chaning lanes his phone was ringing and as he reached for the phone the driver side of his sweeper crashed into the front passenger side of a passing school bus. Mr. Fryar reported no injuries. I observed damage to the driver side of the sweeper. I spoke with the operator of MV2, Ms. Ana Mendes (S72311538). Ms. Mendes stated she was operating a City of Newton contracted school bus (MA: BU44536) on Berkshire Road with 25 students on board with a final destination of Bigelow Middle School. Ms. Mendes stated she was traveling North on Berkshire Road when she observed a street sweeper in front of her. Ms. Mendes stated she entered the Westbound land of Berkshire Road to go around the street sweeper. Ms. Mendes stated as she was passing the street sweeper, it made an abrupt left turn and it's driver side crashed into the front passenger side door/mirror of the school bus. Ms. Mendes reported no injuries. I observed moderate damage to the passegner side of MV2. After the initial crash, it was reported by both parties that MV1 attempted to back up to allow students to exit the bus through the passenger side door. As MV1 was backing up, it crashed into the middle passenger side of the school bus. There were 25 Bigelow Middle School students on the bus at the time of the crash. Ms. Mendes state she contacted her dispatch for further instructions on what to do with the students. Ms. Mendes stated to Sgt. Wade that the Eastern Bus Company dispatch instructed her to keep the students on the bus and wait for Police and EMS to arrive. Ms. Mendes stated after speaking with her dispatch, another school bus arrived at the scene. The decision was made by Ms. Mendes to have the students exit her school bus and be transported to Bigelow Middle School prior to my arrival. Ms. Mendes was advised in the future to follow the policy set forth by her Eastern Bus Company, the City of Newton, and the Newton Police Department. After clearing Berkshire Road, myself and Sgt. M. Wade responded to the Bigelow Middle School to check on the welfare of the students that were on the bus at the time of the crash. We were greeted by the school's Asssitant Principle Mr. Lucas Mogensen. Mr. Mogensen stated all of the students that were on the bus at the time of the crash were staged on the bus when it arrived at the school. The weather at the time was cloudy and sunny and the road conditions were dry. It appears the crash occurred at approximately 14:30 hours on this date according to statements. On arrival, I spoke to an employee identified as Alan Hayden of the tree company. Mr. Hayden works for Tree Technology and Landscape Company Inc. and at the time of the crash he was up in the air inside the bucket of his truck conducting tree trimming work. No other workers were inside the truck at the time. The truck is a 2022 Freightliner M2 bearing MA commercial reg. # W23962. Hayden states the truck was parked at the end of Cragmore Road a bit out into the Boylston Street off ramp. While doing work, Hayden states a full size yellow school bus attempted to go around his truck while turning right on to the Boylston Street off ramp from Cragmore Road. The school bus did not make a wide enough right turn and struck the driver's side black metal cage which supports the bucket to the truck. The bucket then started shaking back and forth while Mr. Hayden was in it. The metal cage goes overhead in front of the truck and sticks out on both sides of it. He says that the school bus scraped along the side of the cage and there should be long sized scrape marks along the top right side of the bus. I observed the scrape marks to the cage and noticed that the cage was also bent outward slightly on the passenger side. No injuries were reported by Mr. Hayden. I took several photos of the Freightliner truck and cage area and submitted them to the I.T. Bureau. Two other employees for Tree Technology were outside of the truck observing the work being done at the time. They were identified as Edgar Ortiz and Kemwrick Thames. Both of them did not observe the collision but heard the school bus scrape along side their company's truck. They stated that they yelled out to the bus operator but the driver continued westbound on to Boylston Street without stopping. Neither of them saw any writing on the bus or a registration number for it. They did observe some type of paper sign attached to the side of it. They stated that numerous students were on the school bus at the time. They did not observe the sex of the bus operator. Dispatch attempted to contact the City of Newton Transportation Dept. with negative results. Sgt. Wade left a voicemail with the director Regina Moody to see if the bus involved was transporting students for the City of Newton/Public Schools. It is unknown at this time if this bus was contracted by the city. Traffic Bureau update (Officer Gaudet): On Thursday, June 16, 2022, I was asked to conduct a follow up investigation for this crash. I spoke with Ms. Regina Moody from the City of Newton Transportation Department. Ms. Moody identified the operator of the school bus as Mr. Urban Mills (S49560217) and stated he was operating bus #351, a 2019 Thomas School Bus (MA BUN: BU43104). Ms. Moody was able to provide me with a copy of the audio/video surveillance from the bus involved in the crash. I viewed this footage in Ms. Moody's office. The footage shows Mr. Mills operating the school bus on Cragmore Road (S) towards Boylston Street. As the bus approached Boylston Street, a tree trimming crew can be seen working ahead at the corner of Cragmore Road and Boylston Street. There is also a stop sign at this intersection that can be clearly viewed from the footage. As the school bus approaches the intersection it does not come to a stop at the stop sign posted on the right side of the roadway. This was confirmed by Ms. Moody using the GPS system that gives the speed of every bus traveling during it's route, including when it comes to a complete stop. The school bus takes a right turn and a loud ""crunching/scraping"" sound can be heard. The attention of the children on the bus is turned to the passenger side of the bus. Through my training and experience, the sound heard on the footage is consistent with a vehicle hitting a large object. Children on the bus can be heard yelling, ""What the fuck!"" and discussing what has just happened with one another. The bus completes it's turn and travels on it's way with out stopping. All of the students on the bus were dropped off at their usual destination. There have been no injuries reported by any of the parties on the bus at the time of the crash. All of the students on the bus at the time of the crash were from Brown Middle School. Ms. Moody stated she would contact the Brown Middle School to try and get a list of the students that were on the bus at the time of the crash. A copy of the footage from the school bus and pictures of the damage to bus #351 was submitted to the IT Bureau. Mr. Mills provided a statement to Ms. Moody sometime after the crash occurred. From Mr. Mills's written statement, he states, ""On approaching the intersection, I noticed a couple of tree cutting vehicles parked on the right side of the roadway (Cragmore Road) at the very corner. Nevertheless, I negotiated the turn and proceeded. This will require me to maneuver my bus onto the sidewalk. While executing this maneuver, I heard some noises which at the time I thought it was as a result of the bus getting onto the sidewalk because while executing the maneuver my eyes were fixed on the right side mirrors and was a great distance away from the vehicles to my right."" After viewing the footage from the crash, Mr. Mills will be mailed Massachusetts Uniform Citation 477181AB for Chapter 90, Section 24 (Leaving the Scene of Property Damage), Chapter 89, Section 9, (Fail To Yield, Stop Sign), and Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). A copy of the criminal application and associated paperwork was submitted to the Police Prosecutor. Ms. Moody was apprised of the findings of my investigation. On arrival, I spoke to the operator of M/V # 1 identified as Elizabeth Langston (MA D/L # S89458234) who was operating a 2013 Nissan Rogue color black bearing MA reg. # 3BMK31. Ms. Langston states that she was parked across the street from 38 Eldredge Street. When she went to pull out in to the travel lane going southbound, she never saw another vehicle going by her and struck M/V # 2 on it's passenger side. She further stated that she believed it was a blind spot in her side view mirror. I observed damage to her driver's side door handle and driver's side bumper. As a result of the crash, the entire bumper was dislodged from the vehicle. Langston called for a private tow (Pro-Tech Waltham) to have her vehicle removed. Next, I spoke to the operator of M/V # 2 Richard Leone (MA D/L # S61672498) who works for the City of Newton Department of Public Works. At the time, he was operating a 2019 Ford 350 color white bearing MA Official reg.# M3106A. He stated that he was traveling straight ahead (SB) on Eldredge Street when M/V # 1 pulled out and struck him on the passenger side. I observed scrapes to the passenger side door that extend down to the wheel well of the cab to the truck. I also observed a dent on the truck between the passenger side door and bed of the truck. No injuries reported by either operator. Photos taken of the scene and submitted to the I.T. The weather at the time of the crash was clear and sunny. The road surface was dry. Detective Sgt. Claflin was working a paid detail for Feeney Brothers at the intersection of Lowell Avenue and Otis Street. While monitoring the traffic in the area, Detective Sgt. Claflin heard what he believed to be a vehicle crash in his vicinity. Detective Sgt. Claflin looked across the intersection and observed a 2020 Honda Civic (MA: 7MR818) leaving the scene down Lowell Avenue (S) towards Commonwealth Avenue. This vehicle had damage to it's passenger side door area. Detective Sgt. Claflin also observed fresh damage to the rear driver side bumper area of a 2021 Dodge Ram (MA: 5CFR30) that was parked in front of 296 Lowell Avenue (S). Detective Sgt. Claflin then entered his personal vehicle and attempted to catch up to the Honda Civic that left the scene to check on their well-being. Detective Sgt. Claflin stated as the vehicle approached the intersection of Lowell Avenue (S) and Commonwealth Avenue, it proceeded through the intersection on to Homer Street with out stopping at the red light activated at this intersection. Detective Sgt. Claflin was finally able to stop the vehicle on Homer Street just after Commonwealth Avenue. I arrived on scene and spoke with the operator, Ms. Ludmila Gelman (S30392030). I asked Ms. Gelman about the events that took place prior to Detective Sgt. Claflin stopping her. Ms. Gelman stated she did not know what happened. I observed moderate damage to the front passenger side door area of her vehicle. The front passenger door mirror was damaged and folded in. The glass window from the front passenger door was shattered and missing. There was glass from the middle covering the entire front passenger seat/floor area inside the vehicle. I showed Ms. Gelman the glass inside her vehicle and the damage to her door and asked her if she knew what caused it. Ms. Gelman stated she did not know and that something must have hit her. I explained to Ms. Gelman the events that has just transpired as conveyed to me by Detective Sgt. Claflin. Ms. Gelman seemed to be confused and did not understand what happened. Ms. Gelman called her son Steven and asked me to speak with him. I explained to Steven the events leading up to me speaking with him. I explained to Steven I did not feel comfortable allowing Ms. Gelman to drive away from the scene. At Steven's request, I moved Ms. Gelman's car away from the intersection and parked it in front of 1103 Commonwealth Avenue for him to pick it up. Ms. Gelman reported no injuries and was transported to her residence by myself in unit N492. Ms. Gelman was given Massachusetts Uniform Citation 487210AB in hand for Chapter 90, Section 24 (Leaving The Scene Of Property Damage), Chapter 89, Section 9 (Fail To Yield, Red Light), and Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Ms. Gelman was also advised I would be filing an Immediate Threat with the Registry of Motor Vehicles. A Request for Immediate Threat License Suspension/Revocation was completed and submitted tot he Registry of Motor Vehicles with supporting documentation via e-mail, DCUImmediateThreat@massmail.state.ma.us. I returned to the area of the crash and observed damage to the rear driver side bumper area of the involved Dodge Ram. The vehicle was unoccupied at the time of the crash. The registered owner, Mr. Zachary Maclaughlin, was advised. A copy of the issued Criminal Application was submitted to the Court Prosecutor with supporting documentation. Ms. Lima Marte stated traffic was stopped at a red light when MV2 crashed into the rear of her vehicle. I observed damage to the rear bumper of MV1. Ms. Lime Marte and her passenger, Ms. Adianel Diaz Lima reported no injuries. The operator of MV2, Mr. Travis Roth (SA4230808) stated he was operating his 2019 Subaru Crosstrek (MA: 1WCL95) on Centre Street (N) towards Commonwealth Avenue. Mr. Roth stated he wasn't paying attention to the roadway and was focusing on the GPS in his vehicle. Mr. Roth stated the front end of his vehicle crashed into the rear of MV1. I observed damage to the front bumper area of MV2. I spoke with the operator of the vehicle involved, Ms. Svetlana Shur (S58558433). Ms. Shur stated she was operating her 2007 Toyota Camry (MA: 852KY1) out of the parking lot next to 714 Beacon Street, Newton. Ms. Shur stated she approached the intersection and her attention was focused to her left as she was attempting to merge onto Beacon Street. Ms. Shur stated when she started to make her turn, she crashed into a crossing female pedestrian. Ms. Shur and her passenger, Mr. Leonid Shur reported no injuries. I observed minor damage to the front passenger side of MV1. I spoke with the pedestrian involved in the crash, Ms. Sonig Varadian. Ms. Varadian stated she was crossing Beacon Street in the marked crosswalk (S to N) towards Dalton Road. Ms. Varadian stated as she was in the crosswalk, MV1 took a right turn out of the parking lot of 714 Beacon Street and crashed into her left side. Ms. Varadian stated she then rolled and fell onto the roadway. Ms. Varadian had numerous scrapes and cuts on her person as a result of the crash and signed a patient refusal with Newton Medics. A witness to the crash, Ms. Rebecca Fishman, stated she observed the crash as she was traveling on Beacon Street (W) towards Dalton Road. Ms. Fishman stated she observed the female pedestrian in the roadway crossing and observed MV1 take a right turn out of the parking lot to 714 Beacon Street and crash into the pedestrian.
