Witness stated that she was in her vehicle west bound on Washington St behind vehicle one when the bike came off of the north bound Elm St sidewalk and into the front passenger side of vehicle one. Witness stated that west bound traffic had a green light. Bicyclist stated that he believed the red light that he saw for north bound elm St traffic was for west bound Washington St traffic, so he proceeded into vehicle one's lane of traffic. Bicyclist was not injured and his bike had no damage. Bicyclist refused medical treatment at scene and singed a patient refusal form. Mr. Maguire wanted to report a crash the he was involved in this morning with his City of Newton owned vehicle. Mr. Maguire reported the crash occurred at 612 Washington Street, Newton. Mr. Maguire stated he was traveling (E) through the 7-11 convince store parking lot at approximately 0755 hours this morning. Mr. Maguire stated he was operating a City of Newton owned 2010 Ford Ecovan (MA MVN: M99243) at the time of the crash. Mr. Maguire stated at this time a vehicle to his right backed out of it's parking spot and it's rear driver side bumper area crashed into the middle passenger side of his vehicle. I observed minor damage to the middle passenger side door area of MV1. Mr. Maguire reported no injuries. Mr. Maguire reported MV2 was a 2015 Honda Civic (MA: 588LG8) and the vehicle was being operated by Mr. Donald Sampson (S29010431). Mr. Maguire stated Mr. Sampson exchange information with him and he observed damage to the rear driver side bumper area of MV2. Mr. Maguire provided me with a picture of the damage to MV2. There I observed MV1 in contact with the building used by retail store TJ MAXX. The operator, Beverly Linsky, was already outside the vehicle. I assisted Linsky to a bench where she was evaluated by Fallon medics. Linsky refused medical treatment. She stated that she was ""fine"". It should be noted that despite the heavy front end damage to MV1 the airbag did not deploy. Linsky states that while attempting to park her vehicle she accidently stepped onto the gas pedal instead of the brakes and drove into the TJ MAXX store building. MV1 was towed from the scene by Tody's towing service. The store manager was notified. Due to the statements made by the operator and witness, I will be submitting an Immediate Threat form to the RMV. He stated the light went green and they began to travel towards Waltham St at approximately 10mph when MV#1 stopped abruptly with no warning. MV#2 was not able to stop and rear ended MV#1. Passenger of MV#2 corroborated the driver's story and stated the truck in front of them stopped suddenly. There is a traffic light at the location of the crash that was red, however this light is for traffic coming down Washington St. MV#1 and MV#2 were travelling down Watertown St and that light did not apply to them. MV#1 was driven from the scene with no damage. MV#2 was towed by Perfection with heavy front end damage. Operator of MV#1(back pain) and the Passenger of MV#2 (knee pain) were evaluated by medics. Based off where the debris and front bumper were located, it was believed the impact of the crash was in that area. A witness called the police and stated he heard two loud bangs outside and observed what he believed to be a dark colored jeep wrangler with heavy front end damage leaving the scene. I spoke to witness, Mr. Kenis who stated he observed the operator struggle to start the engine after the impact and took off before he could obtain a registration plate. Mr. Kenis stated he fled towards Watertown St. Units checked the area for the other vehicle believed to be involved, however had negative results. MV1 sustained heavy front end damage along with minor left rear end damage where the vehicle struck the fence. MV1 was unoccupied during the collision. No injuries. Pictures were taken on scene and placed into the IT Bureau's box. Property owner, Joe Russo of 88 Hawthorn St. was notified of the damage to his fence. Mr. Russo stated he was going to contact his insurance company and was satisfied. Owner of MV1 was also contacted and notified of the accident. Tody's responded and towed MV1.
