MV1 operator said he was not injured at this time. MV1 sustained minor driver side bumper damage. AMB1 operator stated he was on Boylston St Eastbound in the left lane. AMB1 operator said in front of him was unknown debris that he was trying to avoid so the ambulance would not be damaged. AMB1 operator said before attempting to merge into the right lane he checked all his mirrors and could not see any other MV. AMB1 stated MV1 must have been in his blind spot and close to AMB1. AMB1 operator said after he checked his mirrors he attempted to merge into the right lane but struck MV1. AMB1 operator stated he was not injured at this time. AMB1 sustained minor damage to the rear passenger tire wheel well. No tows were required. MV1 and AMB1 operators signed refusals. Both were given copies of the accident report number and advised. Photos were taken and forwarded to the I.T. I observed minor scrapes to the side of Vehicle 2 and minor scrapes to the front quarter panel of Vehicle 1. The operator of Vehicle 1 stated that she was having difficulty turning the steering wheel of her vehicle following the accident, and I observed that her two front wheels were no longer parallel with each other and instead pointed at an inward angle. Both parties denied injuries. Operator of MV1 states when the traffic light turned green she accelerated and struck a male party on a recumbent trike with the front bumper of MV1. Operator of MV1 states her visibility was obstructed from the hood of her vehicle because the recumbent trike is very low to the ground. It should be noted the trike had a flag attached to it, which the operator of MV1 stated she didn't see until after the accident happened. MV1 sustained minor damage to front bumpers license plate. No injuries. The bicyclist, Christopher Tsu, states he was traveling eastbound on Beacon Street on his recumbent trike when he came to a stop at the red traffic light at Centre Street. Mr. Tsu states when the traffic light turned green, MV1 struck him in the rear of his trike causing him to fall to the ground. Fallon Medic 5 evaluated Mr. Tsu and he signed a patient refusal.
