Operator of vehicle #1 stated he was travelling Westbound on Crafts St. and stated he became distracted by a noise coming from the left front wheel or axel. Operator #1 stated he took his eyes off the road and drifted to the right striking a utility pole. Operator # 1 had only a Guatemala ID card. Operator of M/V # 1 states she was traveling straight ahead eastbound on the on ramp for Boylston St (RT. 9) near 1268 Boylston Street when her passenger front side tire popped and went flat. She then lost control of her vehicle and side swiped Eversource utility pole 48/155 which is located on Boylston at High Streets. The vehicle sustained a broken axle and damage to the passenger front quarter panel area and was towed by Tody's towing. No injuries reported Eversource notified. The utility pole sustained minor scrapes to it.

On the above date/time I responded to the area Dedham St/Parker St for a report of two motor vehicle crash with minor injuries. Upon arrival with Fire/medics I spoke to the oper of v1 Ms. Hollowell who had a shoulder injury due to side impact. She stated she was traveling on Dedham Street (east) applied her left turn signal and was turning left onto Parker Street when v2 went through the stop sign and struck her vehicle. I spoke to the operator of v2 Ms. Kuznetsova who stated she was stopped at the stop sign on Parker St ( south) She stated she accelerated forward turning left (east) onto Dedham St when vehicle one struck into her vehicle. Oper of v1 signed a patient refusal and oper of v2 had no injury. Both parties were unsure of the area in the intersection of impact, all vehicles were moved by the operators prior to my arrival and I could not find any physical characteristics in/ on the roadway to find impact area. Therefore the diagram reflects the damage points on the vehicles but not the impact area on the roadway. All parties advised of the process.

Narrative:
On the above date/time I responded to the area of Cherry Street and Webster Street for a three motor vehicle crash with injuries. Upon arrival along with Sgt. Dempsey, Fire/medics. I spoke with the operator of vehicle one. He stated he was stopped on Webster Street facing westbound a the red traffic signal. He further stated the traffic signal on Webster Street turned green, he accelerated forward. He stated vehicle two traveled north on Cherry Street at a high rate of speed through the red traffic signal. He stated he had no reaction time due to the speed of vehicle two and had impact. I spoke to the operator of vehicle two. He stated he was traveling north on Cherry Street going too fast, he stated he thought it was a school zone so he slowed down and he did not observe the color of the traffic signal. He stated he observed vehicle one come out of the Webster Street at the last second at impact. I spoke with the operator of vehicle three. She stated only remembered being stopped on Cherry Street facing south. She stated she did not remember what color the traffic signal was, just knew she was stopped when she felt heavy impact from vehicle two. Injuries: Oper of V1 none at this time- patient refusal Oper of V2 none at this time - patient refusal Oper of V3 Head laceration and head trauma, transported to Newton- Wellesley Hospital Tody's towing arrived on scene and towed vehicles two, three to their tow lot. All proper paper work was done for a towed vehicle(s). All parties advised of the process. After the vehicles were towed, and the scene was cleared I watched the cycling of the traffic signals in the intersection and observed they were functioning properly. It appears a this time based on statements and physical characteristics of the impact area on the roadway, that vehicle two went through the red light on Cherry Street, which caused the crash.

The owner of vehicle 1 arrived to the parking lot of the Boston Sports Club at 135 Wells Ave, at approximately 09:30 hours. He left the gym at approximately 15:00 hours, at which point he noticed that his vehicle (vehicle 1) had a white paint mark on the rear left side. There was a note left on vehicle 1 from an unknown witness, which provided the license plate of vehicle 2. The note stated that the person witnessed vehicle 2 hit vehicle 1. I was able to locate vehicle 2 at the owners residence. I asked them if they were aware that they hit a vehicle today at Boston Sports Club. They stated that they did not hit any vehicle. I asked if I could see the vehicle in their garage, and they stated I could. I noticed green paint on the right rear side of vehicle 2. The owner of vehicle 2 acted surprised, and stated that they must have hit a vehicle backing up. She stated her radio was on very loud, and did not hear or feel any collision. I advised her if she hits a car that she needs to leave her information or to locate the owner, and she stated she would have if she had known. I supplied the owner of vehicle 1 with all the information he needed to file a claim with his insurance if he would like. He was satisfied.

On 03/16/2019, at 1652 hrs, I responded to the intersection of Newtonville Ave and Harvard St for a two car MVA. Upon arrival, I observed damage to the front drivers side of MV1 and damage to the front passenger side of MV2. The operator of MV1 stated she had a green light and was struck by MV2. The operator of MV2 stated she had a green light and was struck by MV1. Two witnesses stated that Newtonville Ave had a red light and MV2 was proceeding through a green light. Neither witness provided their personal information.

Operator of MV1 stated he had got off the Mass Pike at exit 17, heading Eastbound on Washington Street. As he was heading Eastbound, operating of MV 1 stated MV2 crossed lanes and struck the passenger door. Operator of MV2 stated while traveling Eastbound on Washington Street, she was in the 2nd lane from the left, heading toward St. James Street when MV1 struck the driver side of the vehicle. MV1 had minor damage to the passenger door. MV2 had significant damage to the driver side front and rear door, along with a flat front driver side tire. Both operators were checked by the medics and signed refusals. MV2 was towed by Tody's towing.
