Narrative:
On 10/13/2021 at approx 0803Hrs while assigned to 497 I responded to the area of 349 Auburn St for a report of a two car crash with injury. Upon arrival I observed Ma Reg 1FLA38 disabled in the middle of the road with heavy front passenger side damage, with both front and passenger side airbags deployed. Fallon ambulance was on scene with the operator , John McHugh on the stretcher with a head injury. The Fallon EMT's Alex Honrado and Steve Hough stated they had been parked on Auburn St when McHugh ran into the back of a parked landscape trailer. I spoke with McHugh who stated he could not see out his windshield as he had just left his home on Bourne St and his windshield was not defrosted yet and ran into the trailer. McHugh and Fallon Ambulance personnel said as there had been no damage to the trailer the landscaper had left and nobody got a plate number or company name. McHugh transported to NWH, Todys towed the vehicle. A routine registry check showed McHugh has a suspended license.Ma Uniform Citation T2015320 for criminal application for violation of MGL Chec90 Sec 23 Operating After Suspension mailed to McHugh.

MV1 proceeding straight ahead on Westbound via Washington Street collided with MV2 negotiating a Northbound turn from Washington Street to Lowell Ave. Collision resulted in MV2 rolling and coming to rest on roof. Operator MV2 was entrapped in vehicle, Officer Tocci able to break the window and assist Operator out of vehicle. Operator MV2 transported to Newton Wellesley Hospital via Medic 2. MV1 and MV2 towed to Tody's. A towed motor vehicle form completed and filed.

On 10/13/2021 at approx 0938Hrs while assigned to 497 I responded to the area of Washington St and Quinobequin Rd for a report of a 2 car crash. Upon arrival both operators where pulled to the side of the road on Quinobequin Rd. I spoke with the operator of Ma Reg 3FWN51 Alicione Goncalves who stated she had the green light travelling Quinobequin Rd southbound when the pickup truck turned in front of her causing the collision. i spoke with the operator of NH Com Reg RBAC02, Robert Allen who stated he was travelling northbound on Quinobequin Rd , mistakenly though the green light was a green arrow to turn left onto Washington St colliding with Goncalves.

Narrative:
On 10/13/21 at 08:50 hours, I responded to Washington Street & Brookside Avenue for a motor vehicle accident involving a bicyclist. The weather at the time was clear and sunny (70 degrees) and the road conditions were dry. The accident location is a t type intersection with Brookside Avenue having a stop sign in order to turn onto Washington Street. Washington Street is a four lane road with a painted divider. There is no crosswalk at the intersection as it appears it was recently paved. On arrival, the accident scene was already clear as the bicyclist and his bicycle were waiting on the sidewalk. The motor vehicle involved was parked further back on Brookside Avenue away from the intersection. I spoke to the operator of the motor vehicle identified as Balarama Thavanati. He was operating a 2012 Toyota Camry color white bearing MA reg. # 44VN32. He states that he was operating southbound on Brookside Avenue and stopped at the stop sign at Washington Street. He was attempting to make a left turn onto Washington Street. He states that he inched out after because you can not see the vehicles approaching you. Next, a vehicle stopped on Washington Street to let him out and he thought it was clear never seeing the bicyclist on the right of him. He says the bicyclist approached him very fast and it appeared that he never stopped the bicycle prior to entering the street from the sidewalk. Once he attempted to pull out for the turn he then made contact with the bicycle on the left side. Thavanati pointed to the approximate location where the bicycle came to rest and it appears he was pushed forward approximately three feet from the impact. I observed black scrape marks on the right side of the front bumper which appear to be from the bicycle tires. At this time, the bicyclist, Jacob Harrison, was being tended to by Fallon Medics and Jacob's mother (Juliet Harrison) was already on scene. Jacob stated he was fine but since he was not wearing a helmet and hit his head on the pavement his mother had him transported to Newton Wellesley Hospital by Fallon Ambulance. After, I spoke to Jacob in the emergency room along with his parents. He sustained whiplash to his neck and a scrape to his right inner thigh area. I then asked him what had occurred during the collision. He states he was bicycling on the sidewalk eastbound on Washington Street. He says he stopped at Brookside Avenue and saw the Toyota Camry which was also stopped. He thought it was clear to proceed as if the vehicle was going to stay stopped. He then pedaled into the street and was struck by the Camry on the left side of his body. He fell down and he does not recall what else occurred as the incident happened so fast. The bicycle sustained damage to the front tire rim as it became bent. No other visible damage. At the time, Jacob was wearing black shorts and a white t-shirt with a red picture on it. He was not wearing a helmet and was going against the traffic on Washington Street. On scene there were three witnesses who were walking southbound on Brookside Avenue near Washington Street at the time of the crash. There names were Christine Butera, Marlene Austen, and Marta Travers. All three of them stated that the operator of the Toyota Camry was stopped on Brookside Avenue waiting to make the left turn. He then pulled out and in a split second the bicycle entered the intersection and was struck. They did not have the vantage point to see if the bicyclist ever stopped first before entering the roadway from the sidewalk. Digital photos were taken of the scene.

Operator #1 stated he was travelling westbound on Washington St passing the Sheraton Four Points Hotel, 320 Washington St, when he was hit by another vehicle. Operator #1 stated the operator of a white sedan ( Operator #2) was attempting to change lanes behind him and hit the rear driver side of his vehicle. Operator #2 fled the area without stopping. Vehicle #1 had minor damage to the rear of his vehicle. He stated Vehicle #2 would have damage to the front of their vehicle but unsure what side. He stated Vehicle #2 was a white sedan with a black male driver and a person in the back seat, possibly a rideshare service. Operator #1 was able to drive his vehicle away from the scene.

MV2 WAS AN SUV WHITE IN COLOR AND THE OPERATOR WAS DESCRIBED AS A WHITE MALE WITH GRAY HAIR AND GLASSES POSSIBLY IN HIS 50'S OR 60'S. MV 2 DID NOT STOP TO EXCHANGE INFORMATION AND NO INJURIES WERE REPORTED.

M/V #1 was traveling north on Elliot St. As it approached and attempted to pass under the MBTA bridge, the top of the trailer struck the underside of the bridge. It should be noted, that there are no signs of the bridge height, or max. allowable height facing north. The top left corner of the trailer was damaged. There was no visible damage to the bridge.

On 10/13/2021 at 18:33 hours I responded to 30 Myrtle St for a report of a single vehicle crash involving a motorcycle that had a driver and a passenger. Myrtle St is a public way in the City of Newton. Upon my arrival I spoke with Roberto Ventura who stated that he and his passenger Melissa Mendoza were ridding westbound on Myrtle St on his 2009 Yamaha Raider motorcycle. Ventura and Mendoza were both wearing DOT approved helmets at the time of the crash. Ventura stated that as he approached 30 Myrtle St he drove over a patch of compacted dirt and gravel that was approximately three feet wide and four feet long. The patch was not coved by asphalt and was dug out and filled by the City of Newton Utilities Department. The patch of dirt and gravel was not marked by any cones, barrels or barricades. There were no street lights in the area so the roadway was not lit. There was a similar patch approximately 10 feet east that had four cones around it. Ventura stated that he did not see the dirt patch until he drove over it. Ventura stated that after driving over the dirt patch that he did not see at approximately 20 MPH he lost control of the motorcycle. Both Ventura and Mendoza stated that he were then thrown from the motorcycle and landed on the roadway. Ventura complained about pain in his left shoulder and knee, and had road rash abrasions to his left leg and arm. Mendoza stated that she had road rash abrasions to her left arm and leg. Both Ventura and Mendoza declined medical treatment at the crash scene and singed patient refusal forms provided to them by Fallon medic. Ventura's motorcycle had damage to it's left side tail light, and handlebar, as well as scrapes on the left side. Ventura's motorcycle did not require a tow. I placed reflective cones around the dirt and gravel patch and had dispatch leave a message for Newton Utilities to pave over both patches.

Operator of MV1 states she was operating MV1 east on Washington Street in the West Newton rotary, near the off ramp from the Mass Pike. MV1 was in the 2nd travel lane. While operating MV1, the rear passenger, a child, was eating. The rear passenger was eating something spicy and the operator of MV1 thought they were choking. The operator of MV1 turned around to check on the rear passenger. MV1 then jumped the median, striking a traffic light pole, ultimately coming to rest in the 3rd travel lane. No air bag deployment and no reported injuries. Todys towed MV1. Mass DOT notified for th etraffic light and pictures taken and submitted to IT.

Narrative:
MV1 and MV2 were involved in a road rage incident in the area of City Hall on Commonwealth Ave. Commonwealth Ave is a public way that is owned and maintained by the City of Newton. Stated by MV2 operator, James DAVIS: MV1 was operating his vehicle erratically behind and tailgating closely behind MV2. MV1 turned on his flashing white lights that were mounted in front of his driver and passenger side visors. MV2 believed that he was being pulled over by law enforcement so he turned right onto Chestnut St in order to pull over safely. MV2 operator, DAVIS observed the flashing light vehicle continue straight on Commonwealth Ave. MV2 continued his drive from Chestnut St to Temple St to Putnam St where he observed that same vehicle, MV1, driving around the rotary on Washington St over the Mass Pike heading in his direction. DAVIS stated that MV1 pinned MV2 by the driver's side door (B pillar). DAVIS was exiting his vehicle when MV1 closed in on his leg, injuring DAVIS with the car door and HELMS front bumper. Damage was visible on MV1 passenger side front bumper and MV2 driver's side door. DAVIS did not want medical treatment for his leg. Stated by operator HELMS in MV1: MV1 was being followed by MV2 in a dangerous manner in front of City Hall on Commonwealth Ave. MV2 operator, DAVIS, exited his vehicle when MV1 was stopped at the red light (Commonwealth Ave/Chestnut St). MV2 operator, DAVIS, threatened HELMS and brandished a knife. HELMS stated that he was scared for his life. At some point in the interaction, DAVIS got out of his vehicle again and brandished the knife again. HELMS was able to escape DAVIS and continue driving until he reached the backyard of the police station. During my interview with HELMS in the backyard I observed him trembling uncontrollably. I asked if he took anything tonight to which he said no, he was just cold. I asked if he smoked anything and he said he smoked a little marijuana earlier this afternoon but nothing since. HELMS is physically unable to complete field sobriety due to his past injuries including a TBI from a motorcycle accident, a rollover car accident and a bicycle accident where he was not wearing a helmet. He stated that he has multiple pins and plates in various parts of his body and does not feel safe doing field sobriety. I did not observe any odor of alcohol and HELMS stated that he has been sober for 10 years now. A MV search was conducted as well as pat frisk of DAVIS for officer safety. No knife was found on his person or in his vehicle. A MV search was conducted of HELMS vehicle due to arrest. Tody's towed HELMS' vehicle. Theodore Helms is being charged with the following: 90/7E - Improper Display of Flashing Lights (HELMS' mounted white light bar that flashes oscillating lights when turned on). 265/15A - Assault & Battery with a Dangerous Weapon (HELMS' vehicle pinning DAVIS' leg between his driver's side car door). UPDATE: 0030hrs DAVIS returned to the front desk of headquarters to claim that his leg was throbbing (the leg that was pinned between his car door by HELMS' vehicle). Pictures were taken of his knee and the SD card was sent to IT.
