On 10/16/2019 at 1434hrs, I responded to Cherry St at Webster St for a two vehicle accident. The operator of MV1 (MA Reg: 519PW3) stated that she was travelling eastbound on Webster St and was proceeding through the intersection of Cherry St when the accident occurred. MV1 stated that she had the green light while going through the intersection. The operator of MV2 (MA Reg: 1BYL34) stated that she was on Cherry St travelling southbound and had a green light at the Webster St intersection. MV2 stated that she believed she had a green light and the accident occurred when she entered the intersection. MV1 sustained heavy front end damage. MV2 sustained passenger side damage. Both vehicles were towed by Tody's. The operator of MV1 was transported to NWH for further evaluation. After the initial collision MV1 was directed into the traffic light and bushes at the southeast corner of the intersection. Pictures were taken of the damage by Ofc. Ferguson. MV2 was issued Mass Uniform Citation T2079681 (c89/s9 - Failure to Stop Red Light). It should be noted that the accident caused damage to the bushes of the business at 432 Cherry St. I contacted the property owner, Dennis Cameron, and made him aware for insurance purposes.

Narrative:
On Wednesday, October 16, 2019, I, Patrolman Hatfield, was assigned to marked cruiser N 495 to Division 5 for the 1530 to 2330 hour shift. At approximately 1742 hours, FTO Guarino and I were dispatched to Commonwealth Ave at Cedar St for a motor vehicle accident with minor injuries. FTO Guarino and I arrived on scene and noticed two MVs with damage. I spoke with the RO/operator of MV2, Janet Haines (Janet). Janet stated she was not suffering from injuries. Janet said she was operating southbound on Cedar St crossing Commonwealth Ave when she had a collision with MV2. Janet stated prior to crossing Commonwealth Ave, an unknown MV stopped at the intersection to allow her to cross. Janet then stated she proceeded accross when she crashed into MV1, who drove around the stopped MV. Janet said her brakes did not work and she continued to drive forward and ended on top of the curb of Commonwealth Ave facing eastbound. MV2 suffered damage to the whole front bumper and wheel wells. FTO Guarino spoke with the operator of MV1, Madeleine Richard (Madeleine). Be advised both front and side driver airbags were deployed, and the front driver's side door was unable to be opened. I then spoke with Madeleine. Madeleine stated she was operating eastbound on Commonwealth Ave before being involved in the collision. Madeleine stated she drove around the MV infront of her that was attempting to take a left from Commonwealth Ave onto Cedar St, and she did not see MV2 prior to the accident. MV1 was struck on the driver's side by MV2, and ended facing westbound in the middle of Commonwealth Ave. EMTs and fire arrived on scene. Janet stated she did not need to be transported. Madeleine was transported to Newton Wellesley Hospital for an evaluation. Both MVs were towed by Todys.

The operator of vehicle number 1 stated she was traveling westbound on Washington Street in the area of 1201 Washington Street when a tree limb fell from a city tree striking her patrol car. Several pictures where taken of both the city tree and the damage to patrol unit 504. The operator of vehicle number 1 was not injured. Vehicle number 1 sustained moderate damage to the passengers side mirror as well as a cracked windshield.

Narrative:
On 10/16/19 at 2338 I responded to the area of 15 Needham St for a report of a single vehicle accident with the car into a pole and no one inside the vehicle. Upon arrival MA Reg 7TM878 was facing SW on Needham St leaned up against a telephone pole (#2) with heavy front end damage. Units searched the area for the operator and Officer Keefe located the party in the McDonalds up the street. The operator of MV#1 stated he was turning right onto Needham St from Winchester St when his power steering locked and he couldn't redirect his truck away from the curb and telephone pole. He further stated he left his truck because it was raining and his door wouldn't close. Medics responded to evaluate the operator, who signed a patient refusal. Tody's towing responded and towed the vehicle.
