ON JUNE 6TH, 2019 AT APPROXIMATELY 14:11 HOURS WHILE ASSIGNED TO N491 I WAS DISPATCHED TO #349 WATERTOWN ST FOR A REPORTED HIT & RUN CRASH THAT HAD JUST OCCURRED. DISPATCH FURTHER REPORTED SUSPECT VEHICLE HAD FLED THE SCENE WEST ON WATERTOWN ST IN A BROWN COLORED TOYOTA BEARING MA REG. 31Y660. ON MY ARRIVAL TO THE SCENE I SPOKE WITH THE OWNER OF VEHICLE #1 IDENTIFIED AS ROBERT RANKINGS. HE REPORTS THAT WHILE EATING LUNCH IN TOMMY DOYLES A WOMEN(WITNESS), LATER IDENTIFIED AS PATTY TUCKER ENTERED THE RESTAURANT ASKING IF ANYONE OWNED A RED TOYOTA PARKED OUT FRONT? HE REPLIED, "HE DID". WITNESS THEN TOLD HIM SHE WITNESSED ANOTHER VEHICLE HIT HIS CAR THEN DRIVE AWAY. MS TUCKER(WI) TOLD MR RANKINGS THAT WHILE TRAVELLING W/B ON WATERTOWN ST SHE WITNESSED SUSPECT VEHICLE WHO WAS GOING IN THE OPOSITE DIRECTION(E/B) ATTEMPT TO MAKE A U TURN IN FRONT OF HER. SUSPECT MADE TWO ATTEMPTS MAKING HIS U/TURN THE SECOND ATTEMPT HE CRASHED INTO HIS VEHICLE. WITNESS TOOK A PICTURE OF TH SUSPECTS LICENSE PLATE, MA REG 31Y660 AND GAVE IT TO MR RANKINGS. A REGISTRATION INQUERY BY POLICE SHOWED THE VEHICLES REGISTERED OWNER AS OUCIBLE OUSSAMA, DOB 12/16/1984, ADDRESS OF 62 DEHON ST, REVERE MA, 02151.A BOLO WAS PUT OUT ON THIS VEHICLE WITH NO SUCCESS. REVERE PD WAS NOTIFIED TO CHECK SUSPECT VEHICLES ADDRESS WITH NEGATIVE RESULTS. THERE WAS NO TELEPHONE LISTING FOR THE REVERE ADDRESS. THE FOLLOWING DAY,JUNE 7TH, 2019, AT 09:30HOURS I SPOKE WITH THE WITNESS, PATTY TUCKER. MS TUCKER GAVE THE FOLLOWING STATEMENT TO ME REGARDING THIS INCIDENT: I WAS GOING W/B ON WATERTOWN ST HEADING TOWARDS WEST ST WHEN SUSPECT VEHICLE WHO WAS COMING E/B ON WATERTOWN ST(OPOSITE DIRECTION) SUDDENLY MAKE A U TURN IN FRONT OF HER. SUSPECT VEHICLE HAD TO MAKE TWO TURNS TO COMPLETE HIS U/TURN HOWEVER ON THE SECOND ATTEMPT HE STRUCK THE VICTIMS PARKED VEHICLE. SUSPECT THEN DROVE A SHORT DISTANCE AWAY STOPPING IN FRONT OF MAGNI FUNERAL PARLOR. SUSPECT, WHO WAS DESCRIBED AS A W/M, IN HIS THIRTIES WITH A SHAVED HEAD AND WEARING SUNGLASSES GOT HALFWAY OUT OF HIS VEHICLE, LOOKED BACK THEN GOT BACK INTO HIS VEHICLE AND DROVE AWAY. MS TUCKER WAS ABLE TO TAKE A PICTURE OF THE SUSPECTS LICENSE PLATE WITH HER CELL PHONE PRIOR TO HIM FLEEING. SHE LATER PROVIDED IT TO THE VICTIM, ROBERT RANKINGS. I WILL BE MAILING A CITATION#T1444062 TO THE REGISTERED OWNER FOR LEAVING THE SCENE OF ACCIDENT, 90/24C AND MAKING AN ILLEGAL U/TURN, CITY ORDINANCE VIOLATION 19/86

Operator of MV1 states she was backing out of the parking lot behind the Village Bank when MV2 struck MV1's left rear fender due MV2 performing a U-Turn from the southbound side of Elm St to the northbound side of Elm St. MV1 sustained minor damage to the left rear fender. Operator of MV2 states she was traveling southbound on Elm St when she performed a U-Turn in front of 117 Elm St and struck MV1. MV2 sustained minor damage to the front right fender. Witness, Christopher Bryan, states he was in the parking lot behind the Village Bank when he observed MV2 perform a U-Turn, which struck MV1 in the left rear fender. I issued the operator of MV2 in hand Massachusetts Uniform Citation #T1442617 for the following: Ch. 89 Sec. 8 Fail to Yield/Making U-Turn while interrupting traffic

MV1 operator states she was driving in the left lane going eastbound on Centre St heading towards Charles Banks Rd when MV2 hit her car on the right side. MV1 operator reported general pain and shock from accident. Medics responded and a refusal was signed. MV2 operator states he was driving eastbound on Centre St when he missed his left turn to go over the bridge heading towards Watertown. He then attempted to make an illegal left turn at the next set of lights but did not see MV1 in left lane. MV2 then hit MV1. Damage was observed on front left of MV2. I then cleared without further incident.

Operator of vehicle 1 stated he was in the right lane of the eastbound side of Washington St with his right blinker on waiting to turn right into 2000 Washington St. As he proceeded to turn right vehicle 2 came from his left around the front of his car clipping the front end. On Wednesday October 28, 2020 at approx. 0900 I responded to 24 Maguire Court for a report of a crash with property damage involving a tractor trailer. Upon arrival I met with the operator of the tractor trailer Mr. Williams, he stated he was traveling on Maguire Court delivering equipment to a local business, and while turning into the business parking lot he misjudged the turn and the trailer struck a fence,posts,wall belonging to 24 Maguire Court. I advised the owners son who was on the property of the crash and advised I would have a report. There were no reported injuries and all parties advised of the process. I also conducted a Level one inspection on the truck/trailer. The following below is a list of violations and citations VIOLATIONS C.O. 19-75 FAILURE TO USE CARE IN TURNING 393.47D INSUFFICIENT BRAKE LINING- AXLE 5 RIGHT SIDE BRAKE LINING HAS APPROX. 3 INCH CRACK IN LENGTH. CITATIONS T2016068 C.O. 19-75 FAILURE TO USE CARE IN TURNING $ 20.00 T201069 CHAPTER 90 SECTION 7 BRAKE VIOLATION $ 40.00 All paperwork/inspections/citations were issued to the operator and advised of the process.

On 11/28/2020 at approx 0917hrs while assigned to 497 I responded to the area of Chestnut St and Beacon St for a report of a 2 car crash w/o injuries. Upon arrival I found Ma Reg 3GEH51 operated by Patrick Bowe to the side, facing the wrong way, in front of 584 Chestnut St, Ma Reg P73262, a Clifford s Flower delivery Van, being operated by Rochelle Driscoll was in the driveway of 584 Chestnut St with no damage. I spoke with Driscoll who stated she was travelling SB on Chestnut St attempting to turn left into 584 Chestnut St, Bowe was behind her and he swerved when she turned, trying to avoid colliding and ran into the utility pole on the corner. I spoke with Bowe who stated he was travelling SB on Chestnut St behind Driscoll , she turned left ,than backed into the driveway of 585 Chestnut St, as he began to pull forward she pulled out of the driveway of 585 across the roadway into 584 causing him the brake and swerve to avoid hitting her instead glancing off the utility pole in front of 584 Chestnut St. No damage to pole, Bowes vehicle had significant front left damage , when he attempted to pull away it appeared his front left axle was broken, Bowe pulled safely onto a side St and elected to call AAA for a tow.

On Sunday June 13, 2021 at approximately 1933 hours I was dispatched to 2322 Washington for a two car MVA with no injuries. MV1 operator stated she was operating eastbound on Washington St (a public way) driving the speed limit. MV1 operator stated she observed MV2 on the westbound side of Washington St attempt to turn left into Starbucks (2322 Washington St). MV1 operator stated by the time she saw MV2 cut across in front of her and into her lane, it was too late and MV1 struck the passenger side of MV2. MV2 operator stated the same story. MV2 operator said she was also going the speed limit on Washington St westbound and attempted to turn left into Starbucks but misjudged MV1's speed, causing the MVA. MV1 sustained extensive total front end damage and was towed by Tody's. MV1 operator requested an evaluation by Newton Medics and was transported to Newton-Wellesley Hospital. MV2 sustained extensive and total passenger side damage and was towed by Tody's. MV2 operator reported no injuries.

Homeowners at #289 Cherry St., identified as Midge Connolly and Walter Devine called NPD to report that a delivery truck had driven on their property causing damage. On arrival I observed significant damage to the property at #289 Cherry St along the Upham St side of the property. Damage included the lawn, bushes between the driveway and Upham St, a tree and barrels. I then spoke with the operator of MV#1 who stated that while trying to back his truck into the driveway at #297 Cherry St, which is actually located on Upham St, he backed over the property at #289 Cherry St causing the damage listed above. No injuries, no tows and I took a few photographs of the damage to the property at #289 Cherry St.

The operator of MV#1 stated he was travelling westbound on Beacon St when he saw an available open parking spot and made a u-turn onto the eastbound lane striking MV#2. MV#1 sustained heavy damages to its driver's side front and rear doors with driver's side front and rear airbag curtain deployments. The operator of MV#1 sustained injuries to his left side ear and left side bruising of his ribs area. The operator of MV#1 was evaluated by Newton Paramedic 1 and signed a patient refusal or treatment. MV#1 was towed by Tody's Towing. The operator of MV#2 stated she was travelling westbound on Beacon St attempting to make a left turn onto Hammond St when she was struck by MV#1. The operator of MV#2 stated MV#1 was to her right side when MV#1 made an abrupt left turn into her lane striking her vehicle on her front passenger side and pushing her into the eastbound travel lane. MV#2 sustained heavy front end damages. The operator of MV#2 sustained injuries to her left side torso area. The operator of MV#2 was evaluated by Newton Paramedic 1 and signed a patient refusal of treatment. It should be noted that the point of impact for both vehicles occurred on Beacon St westbound by the Boston College "McElroy Commons" (325 Beacon St at the St Jude Gate entrance) and there are no intersections.

Vehicle 1 and Vehicle 2 were headed eastbound on Auburn St when Vehicle 1 attempted to parallel park on the south side of Auburn St. Vehicle 2 attempted to drive around Vehicle 1 while they attempted to park, at which time Vehicle 1 decided to abandon their attempt to parallel park and instead took a wide left turn in an attempt to pull into the angled parking spaces across the street on the north side of Auburn St. When Vehicle 1 attempted this turn, the front driver's side quarter panel of Vehicle 1 made contact with the passenger side front and rear quarter panel of Vehicle 2. I observed minor scrapes to the side of Vehicle 2 and minor scrapes to the front quarter panel of Vehicle 1. The operator of Vehicle 1 stated that she was having difficulty turning the steering wheel of her vehicle following the accident, and I observed that her two front wheels were no longer parallel with each other and instead pointed at an inward angle. Both parties denied injuries. Vehicle 2 was able to drive away from the scene while Vehicle 1 requested a tow from AAA.

On May 18th, 2022 at approximately 11:47 hours while assigned to N491 I responded to a report of a MV crash in the area of #727 Washington St, Newtonville. On my arrival to the scene there was a MSP trooper on scene that happened to be driving by. Vehicle #1 was a gray 2018 Hyundai Electra, MA plate 3VMR89, operated by Paula R. Lima with heavy damage. Ms Lima stated she was parked facing W/B on Washington St near #721 when she attempted to make a U-Turn to go E/B when she was struck by vehicle #2. Vehicle #2 was a 2016 Jeep Compass, MA. reg 298XH9, operated by a Jevon D. Foster. He stated he was going W/B on Washington St when vehicle #1 pulled out in front of him from a parked position causing him to crash into her vehicle. Both vehicles were towed by Todys and there were no reported injuries.
