On 01/15/2019, at 1919 hrs, I responded to the intersection of Washington St and Centre St for a two car MVA. Upon arrival, I observed heavy damage to the front end of MV1 and moderate damage to the rear end of MV2. The operator of MV2 complained of head/neck injuries and was transported to Newton Wellesley Hospital by Cataldo for an evaluation. The operator of MV1 stated she was attempting to merge onto Washington St from Centre St when she rear ended MV2. The operator of MV2 stated when she slowed to merge onto Washington St from Centre St she was rear ended by MV1. Both vehicles were towed by Tody's. Towed MV inventory forms were filled out and filed.

Narrative:
On the above date/time I responded to Lexington Street / Freeman Street for a two motor vehicle crash with air bag deployment. Upon arrival on scene I checked all parties for injuries. Operator of vehicle one Ms. Farnese complained of head / neck injuries and Fire and Medics responded on scene. All parties in vehicle two did not have any injuries. I spoke with the operator of vehicle one Ms. Farnese in the ambulance she stated she was slowing traveling south on Lexington Street at the intersection of Freeman Street due to the traffic signal for Lexington Street was yellow. She stated as she was slowing she felt a heavy impact from in the rear by vehicle two. I spoke to the operator of two Ms. Heimbrock, she stated she is a pastor for the Salvation Army in New York and she was transporting two children Shamah Lapaix d.o.b. 4-18-14 and Grace Lapaix d.o.b. 1-10-11 to a Christian school in Wayland. She stated that she is staying with the children at 65 Falmouth Road Newton MA with the childrens grandmother due to the fact the parents are in New York doing a continuing education program until Friday January 18, 2019. She stated she is not familiar with the area, she was traveling south on Lexington Street approaching Freeman Street when she glanced at the map on her cell phone. She stated she did not observe the color of the traffic signal and did not observe the rear brake lights in vehicle one. All parties were evaluated in vehilce two by medics with no injury. I spoke with the childrens mother Sarah Lapaix 1-203-685-2809 who confirmed Ms. Heimbrock's statement about the care of the children. I did observe an expired inspection sticker as of 12/18 on vehicle one. Therefore due to operator of vehicle two's statement I checked the rear brake lights. When the brake pedal was pressed the left brake light and top(3rd) brake light was operational but the right rear brake light was not operational. Tody's towing arrived on scene and towed both vehicles top their lot. Ms. Heimbrock made arrangements for transport of herself and the children. Ms. Farnese was transported to Newton Wellesley Hospital for non life threatening injuries. I advised all parties of the process and gave them each others information. I checked the traffic signals in the intersection and they were fully functional. Therefore with all the above mentioned I am issuing citations to both parties Operator of vehicle one citation # T1440538 for Chapter 90 Section 20 EXPIRED INSPECTION STICKER $ 55.00 Operator of vehicle two citation # T1440539 for City Ordinance 19-75 FAILURE TO USE CARE IN STOPPING $ 20.00 Both citations were filled out after investigation and will be mailed to their license address.

MV1 did not require a tow, was evaluated and cleared by Cataldo Ambulance and departed the scene. MV2 stated she was "trying to get in the right lane but couldn't and when I came back I hit that car" causing minor damage. The operator of MV2 complained of pain to her head from striking the windshield and was transported by Cataldo to Newton Wellesley Hospital for an evaluation. Her vehicle was left parked legally on Washington St.

Narrative:
On Sunday, May 5th, 2019 at approximately 1315 hours while working the City of Newton detail for the Walk for Hunger I was standing at my assigned post at the intersection of Centre Street and Willow Street. At the time, the road surface was dry and the weather was cloudy. Traffic was particulary heavy as there were several streets in the area still blocked off for the Walk for Hunger and there were participants still walking the route in the area. My attention was drawn to the intersection of Centre Street and Bowen Street as there was a motor vehicle travelling southbound on Centre Street with their left turn signal on attempting to take a left turn on to Gibbs Street. Gibbs Street was temporarily blocked to non-residential motorists as there were several pedestrians in the road crossing over Gibbs Street. As the motor vehicle attempted to turn, there were several pedestrians in the crosswalk crossing over Gibbs Street. The vehicle had stopped halfway into the northbound lane on Centre Street blocking on coming traffic. The operator appeared to realize Gibbs Street was blocked and attemped to turn the vehicle back into the southbound lane to continue southbound on Centre Street. As the vehicle turned back into the southbound traffic lane, the vehicles rear end collided with Officer Richard Clements, who was operating NPD motorcycle unit 533. Officer Clements was traveling southbound on Centre Street and was passing the vehicles in the southbound traffic lane which were currently all at a standstill. Officer Clements had his emergency lights activated as he was on his way to remove traffic cones from the street to open up all the traffic lanes along the event route. Officer Clements was working the City of Newton detail for the Walk for Hunger assigned to the motorcycle to assist with traffic. Officer Clements front wheel, left side of the motorcycle and left leg collided with the rear end of the motor vehicle. The impact of the crash caused him to fall off the left side of the motorcycle and land on the street face first. At this time I immediately ran from my post to Officer Clements location and radioed dispatch that a motorcycle officer was down and that I needed assistance. As I approached Officer Clements he was face down, unconcious and breathing. I was able to roll Officer Clements into the recovery position and at this time Officer Clements regained conciousness and was able to speak and stated " What happened? What was that lady doing she turned right into me". I advised Officer Clements that he had just been involved in an accident and to stay still and that help was on the way. The operator of the motor vehicle identified as Helen Leibner (S52717988) stopped her vehicle immediatley after the crash and got out of her vehicle to offer her assistance. I advised Helen and several other witnesses and bystanders who were now crowding around Officer Clements and I to back away from the scene and to not move any of the vehicles involved. Officer Selig arrived on scene and was able to help crowd control until further help arrived. Helen and all witnesses were advised to stand by for police to take their statements. While tending to Officer Clements he was disoriented and made several attempts to move his body to which I had to physically hold him still to prevent possible further injuries. While speaking with Officer Clements I noticed blood coming from the inside of his mouth. Officer Clements was in a noticeable amount of pain. He complained of head, back and left leg pain. Officer Clements was wearing a yellow ANSI certified Newton Police reflective jacket and a Newton Police issued motorcycle helmet. Once the paramedics and fire personnel arrived I was able to speak to the operator of the motor vehicle and several other witnesses. Helen stated she was traveling southbound on Centre Street operating her blue 2014 Audi A4 (MA REG 213BC9). Helen states she was attempting to take a left turn onto Gibbs Street and activated her left turn signal. Helen states she had stopped halfway into the northbound lane as several pedestrians were crossing Gibbs Street in the crosswalk at the time. Helen states after the pedestrians cleared she saw the traffic cones blocking the street and realized the street was closed. Helen states she turned her vehicle back into the southbound traffic lane and did not see Officer Clements or the motorcycle until impact. Helen was not injured and her motor vehicle sustained moderate rear end damage. Her motor vehicle was able to drive away from the scene. Witness Adron Gordon (S90357161) stated he was participating in the Walk for Hunger and had just crossed over Gibbs Street when the crash occured. He stated he witnessed the Officer on the motorcycle with the emergency lights on traveling southbound on Centre Street behind the motor vehicle. The motor vehicle stopped in traffic attempting to turn left onto Gibbs Street. He states then the vehicle made a slight turn back into the southbound traffic lane and the motorcycle collided with the rear end of the motor vehicle. He states the Officer then fell off the motorcycle landing in the middle of Centre Street. Witness Elizabeth McCarthy (S00242346) and Katherine Regan (S43486601) stated they both were in the same vehicle together stopped in traffic in the northbound lane of Centre Street just before the set of lights at Centre Street and Bowen Street. Both witnesses stated they do not remember if the driver of the motor vehicle had the turn signal on but they did see her attempt to turn onto Gibbs Street. They stated they also saw the motorcycle with emergency lights activated travel southbound on Centre Street approaching behind the vehicle. Witnesses state the motor vehicle then suddenly turned back into the southbound lane and the motorcycle collided with the passenger side tailight of the motor vehicle. Both witnesses state the motorcycle attemped to turn away from the vehicle to avoid the crash but was unable to do so. Witness Nina Wang (S39460985) stated she was parked on the southbound lane of Centre Street just prior to the crash scene. Nina states she was in her City of Newton DPW marked vehicle with the hazards on as she was working to assist with traffic safety for the Walk for Hunger. Nina states she witnessed the motorcycle travel southbound on Centre Street and pass the vehicles stopped in the traffic lane. Nina states the motorcycle passed the traffic using the left side and had the emergency lights activated. Nina states the motorcycle was just passing through the green light at Centre Street and Bowen Street when the motor vehicle that was stopped at the lights attempting to take a left turn onto Gibbs Street. Nina states the vehicle then abruptly jerked her car to the right and collided with the motorcycles left side. Nina states she heard a loud crash and witnessed the Officer fall off the motorcycle and land face first onto the street. The motorcycle's final resting place was on its left side approximately two feet away from where Officer Clements was located. The motorcycle sustained moderate damage to its left side, including the front wheel well and undercarriage. The motorcycle left scuff marks on the street which were measured and photographed. Several photographs of the scene and damage to both vehicles were taken and submitted to the IT Bureau. The motorcycle was towed by Todys. Centre Street and Bowen Street are both public ways maintained by the City of Newton. The injured motorcyclist, Officer Clements, was transported to Saint Elizabeth Hospital with fire fighters on board. I was able to follow up with Officer Clements on the phone the next day who was admitted over night at the hospital. Officer Clements stated he has no recollection of the accident and can only remember driving down Centre Street to begin to pick up cones to open up all lanes of traffic. Officer Clements stated he suffered a brain bleed, head, back, left foot and right thumb injuries. A Massachusetts Uniform Citation was issued and mailed to Helen for Chapter 89 Section 7A, failure to yield to emergency vehicle, police motorcycle.

Lisa Pattison was operating vehicle #2. Pattison states that she was struck from behind while at the stop sign on Mill St. at Walnut St. Lisa Lundwall was operating vehicle #1. Lundwall was unable to answer any questions. Lundwall appeared to be suffering from a medical emergency. I called Lundwalls father with her cell phone, who stated that he lives in Utah and Lisa has no medical issues. Lundwall is a dog walker and had 3 dogs in her vehicle. Cataldo responded and took Lundwall to NWH. Tody's transported the vehicle and dogs to Rotherwood Animal Hospital on Winchester St. The staff at Rotherwood and Sgt. O'Connell called the dog owners to make arrangements for their return. Tody's towed Lundwalls vehicle.

HE STATES VEHICLE #1 STOPPED ABRUPTLY AND HE WAS UNABLE TO AVOID HITTING HER. VEHICLE #1 HAD EXTENSIVE REAR AND FRONT END DAMAGE AND WAS TOWED BY TODYS. VEHICLE #2 HAD EXTENSIVE FRONT END DAMAGE AND MULTIPLE AIR BAG DEPLOYMENT AND WAS ALSO TOWED BY TODYS. OPERATOR OF VEHICLE #1 WAS TRANSPORTED TO NEWTON WELLESLEY HOSPITAL FOR POSSIBLE WRIST INJURY. Operator of vehicle one, Benjamin Beaver stated that while driving south bound near 321 Walnut St his vehicle ( MA reg 551DK8 2014 Lexus ) made contact with a pedestrian in a marked crosswalk. Walnut St is a public way in the city of Newton. Beaver stated that he did not see the pedestrian prior to the crash and that his vehicle was travailing approximately 5 to 10 MPH. Beaver stated that the pedestrian was knocked to the ground but got up and walked around. The pedestrian, NNHS student Benjamin Donovan stated that he was crossing east to west in the marked crosswalk in front of 321 Walnut St. Donovan stated that he saw Beaver's vehicle approaching him at approximately 5 to 10 MPH. Believing that Beaver was stopping to let him cross , Donovan stated that he held up his left hand to signal to Beaver that he was going to cross. Donovan stated that Beaver's vehicle struck him on his left side and knocked him to the ground. Donovan stated that he had minor scrapes on his right side as a result of falling to the ground. Donovan stated that he did not lose consciousness and was transported via EMS to Newton Wellesley Hospital for treatment. I took photos of Beaver's vehicle and forward the disk to NPD IT Bureau. I issued Beaver MA Citation T 1269995 and cited him for a violation of MGL Ch 89 C 11, failure to yield to a pedestrian in a crosswalk. On 06/12/2019 I was forwarded an E-mail from Capt Anastasia of a video of the crash that was taken from a camera that was mounted on the roof of 321 Walnut St. The video shows Donovan walking into the marked crosswalk from the east side of Walnut St and hold up his left had to Beaver's on oncoming vehicle. Beaver vehicle appears to be travailing faster than the 5 to 10 MPH that Beaver believed that he was travailing. The video shows that Beaver did not brake until Donovan was struck and landed on his vehicles hood. Donovan was thrown approximately 10 feet from Beaver's vehicle and landed on his right side on the roadway. The video shows Beaver stopping and pulling to the side of the roadway after the crash.

On Friday, July 5th 2019, at approximately 23:30pm, I, Officer Brooks, responded to Pearl street at Emerson street, for a 2 car MVA. Upon my arrival the medics were treating the operator of MV1(MA REG 8MD241). The operator was complaining of leg pain. According to the operator of MV1 she was traveling Northbound on Pearl street, and looked out the window at something, at which point she struck MV2(MA REG 1EHV54), which was parked legally on Pearl street at the corner of Emerson. MV1 had heavy front end damage, and MV2 had damage to the rear drivers side wheel well area and rear bumper. MV1 was towed by Tody's and the operator was transported to Newton Wellesley hospital. I left a note on MV2 letting the owner know a report was on file.

On 9/08/19 Kevin Coston came into NPD Headquarters to report a MV/Ped with Injury that had occurred on 8/23/19 at about 06:15 in the parking lot of Whole Foods (916 Walnut Street). The RP stated that he had left work (Whole Foods) at the back receiving door and was walking to his car, he bent over to take his knee pads off, while he was doing this he was struck by a delivery van that backed into him. The driver took out his headphones and said sorry before driving away. RP stated that he told the driver that he had been hit but the driver just drove away. RP went to hospital (Faulkner) on his own for lower back pain and has not been to work since. RP was able to take picture of the side of the delivery van. The van belonged to Shortstop Distribution Inc (508-655-5948). RP left a message that morning. He was texted by Gedeon (786-352-6695) who stated there was a language barrier and he didn't understand what had happened. RP advised Gedeon to talk to his lawyer or the police. Exact location of accident unknown. Diagram is a estimate. Report printed out and placed in traffic box for follow up. Traffic Bureau Update (Officer Gaudet): On Monday, September 9, 2019 I attempted to make contact with the operator if MV1, Gedeon Duarte, with a negative result. A Hit and Run motor vehicle inquiry was mailed to Duarte. I spoke with the injured party, Kevin Coston, via telephone on September 9, 2019. Coston stated he is an employee at the Whole Foods located at 916 Walnut Street, Newton. Coston stated on August 23, 2019 a black van backed into him as he was kneeling on the ground taking off his knee pads in the loading area of the store. Coston stated he approached the driver who remained in the vehicle to speak with him after he was hit. Coston described the driver as a heavy set Spanish male. Coston stated the operator of the vehicle rolled down his window, took his headphones out of his ear, said sorry, and left the area. Coston stated he contacted the Newton Police to report the accident but was told he needed to come in person to file the report. Coston stated he was unable to come to the station to file the report until September 8, 2019. On Friday, September 13, 2019, I spoke with Gedeon Duarte at the Newton Police Department. Duarte stated he spoke Portuguese and did not speak English. With the assistance of David Sargent from the Language Line (1-800-225-5254) translating, I was to speak with Mr. Duarte in the Traffic Bureau. Duarte stated he was making a delivery at Whole Foods and was waiting for another truck to leave so he could leave the area. Duarte stated a male party (Coston) came up to his driver door and was yelling him. Duarte stated that due to language barrier he was unable to understand what the person was saying to him, but stated the male party was yelling, "Fuck you! Fuck you!" Duarte stated he did not know what was going on and stated he left the area to allow the male party to calm down. Duarte stated he returned a minute later the male party did not want to speak with him. Duarte stated he returned a second time later in the morning and Coston was not there. Duarte stated he received a call from his employer stating Kevin Coston had called for him and he was provided with Coston's phone number. Duarte stated he texted Coston to find out what happened that morning. Duarte showed me a text message exchange from August 23, 2019 at 9:03 A.M. between himself and a party listed as "Kevin Whole". In the text messages, Duarte stated he is the driver of the van and that his supervisor called him. The other party responded and stated "Talk to the police and my lawyer". Duarte stated that he did not understand what happened and that he returned to whole foods. Duarte ended the text exchange with "927535425 my number of policy..." Duarte stated he had no idea he made contact with Coston and stated at the time of the alleged accident he did not hear/see/feel anything otherwise he would have stopped. Duarte's information has been updated in this report to reflect him as the operator/vehicle involved. Duarte also submitted pictures of the rear of his vehicle, a 2008 black Ford E350 (MA: 8NY171), from the day of the accident. Photos were submitted to the IT Bureau. Officer Wade went to the Whole Foods located at 916 Walnut Street, Newton in an attempt to view surveillance footage of the accident. Officer Wade stated the surveillance camera did not capture the incident and the van not viewed at all.

M.V. #1 was merging onto Rt. East when they were bumped from behind by veh #2. There was a newborn in veh #1 so as a precaution, the baby and its mother was transported by Medics to Newton-Wellesley Hospital to get checked out. Passenger #1 complained of headache after Medics left but didn't want me to call for a transport. She stated she would take some Tylenol and see how she felt later on. Very minor damage to veh #1. The operator of veh#2 stated she was merging onto Rt. 9 East when she didn't realize veh #1 in front of her didn't proceed and that's when she struck the rear of the car. Operator handed Mass Citation #T1445110 for N.C.O. -Failure to Use Care in Starting. Very minor damage to veh #2.

The operator of MV1 states that she was traveling northbound on Needham Street with flow of traffic when she struck a pedestrian with the right front corner of her vehicle. The operator of MV1 stated that she did not see the pedestrian in the cross walk until after she hit her and that there was poor lighting in the area. Through the help of coworker from Anthony's Coal Fired Pizza, the victim stated that she was crossing Needham Street in the crosswalk and was almost on the side walk when she was struck by MV1 from behind. MV1 sustained minor front end damage while the victim sustained noticeable injuries and was transported to Newton Wellesley Hospital by Cataldo. Photographs of the scene were taken and submitted to IT to be attached to the report. The operator of MV1 was issued a Massachusetts Uniform Citation (T2079284) for 89-11, failure to yield to a pedestrian in a crosswalk.

Katerina Iliades stated that on 10/23/2019 at 11:56 hours she was standing in front of the rear driver's side door of her vehicle MA reg 7BM656 2019 Land Rover while it was parked facing north in front of 316 Walnut St. Walnut St is a public way in the city of Newton. Iliades stated that she was placing her daughter Melina Bichelen in her car seat and when she observed MA reg 8EP212 a white Subaru Outback driving north bound on Walnut St near the marked crosswalk at 312 Walnut St. Iliades stated that she observed the Subaru was being operated by an elderly white male. Iliades stated that the she observed the Subaru's front drivers side strike several orange plastic three foot poles that are affixed to the roadway around the crosswalk. Iliades stated that after the Subaru struck the poles at the crosswalk it veered towards her. Iliades stated that she leaned back into the back seat of her vehicle and pulled the door towards herself as the Subaru's passenger side mirror made conact with her vehicle. Iliades stated that the Subaru continued north bound on Walnut St without stopping or slowing. Iliades complained of lower leg pain and refused medical aid or transport to a hospital at the crash scene. I observed no damage or markings on Iliades vehicle. MA reg 8EP212 is registered to Harilaos Papadogiannis ( DOB 01/02/1941 ) of 134 Apt 4 North St , Newton. I went to 134 North St and found MA reg 8EP212 a 2014 Subaru parked in the lot of 134 North St. I observed that the Subaru's passenger side mirror was lose from it's mounting and was turned down and was facing the wrong way. I further observed that the out side casing of the Subaru's mirror had scuff marks consistent with it forcefully making contact with an other object. I spoke with Papadogiannis who stated that he was driving his Subaru north bound on Walnut St at 11:56 hours on 10/23/2019. Papadogiannis stated that he could only recall striking the plastic poles near the crosswalk and nothing else regarding the crash. I as spoke with Papadogiannis he was confused and I had to repeat myself and my questions to him several times. I instructed Papadogiannis what steps he needs to take regarding filing out an operator crash report and contacting his insurance provider , but I believe that he did not fully comprehend what I was conveying to him. Based upon my observations at the crash seen and my conversations with Iliades and Papadogiannis , I believe that Papadogiannis was at fault for the crash. Based upon my observations of Papadogiannis I filed an Immediate Threat Request form and faxed it to the MA RMV Driver Control Unit. Photos of both vehicles were taken and downloaded by the NPD's IT Bureau.

On Monday, November 4, 2019, while assigned to Traffic unit N525, I responded to the intersection of Walnut Street and Walnut Place, Newton for a report of a motor vehicle crash involving a pedestrian. The weather at the time of the accident was clear and sunny. The road surface was dry. I spoke with the injured pedestrian, Ernestine Gianelly, as Newton Medics were preparing her for transport to the hospital. Gianelly stated she was crossing Walnut Street in a marked crosswalk towards Washington Park after leaving the Newton Senior Center located at 345 Walnut Street, Newton. Gianelly stated a vehicle exited Washington Park and turned left onto Walnut Street directly at her. Gianelly stated she believes the driver did not see her. Gianelly stated the vehicle hit her while she was in the marked crosswalk on her left side. Gianelly stated she then fell to the roadway. Gianelly was taken to St. Elizabeth's hospital in Boston for injuries to her ribs, back, and sides. I spoke with the operator of MV1, Alex Cook-Williams (S13317988). Cook-Williams stated he was operating his 2009 Pontiac Vibe (MA: 222FV5) Westbound on Washington Park towards Walnut Street. Cook-Williams stated he came to stop at the intersection and observed no traffic in either direction of Walnut Street. Cook-Williams stated he made a left turn onto Walnut Street and did not see the crossing pedestrian until it was too late. Cook-Williams stated he slammed on his brakes and the pedestrian landed on his hood. Cook-Williams stated the pedestrian then fell to the roadway. Cook-Williams reported no injuries. Cook-Williams stated he did not know if the pedestrian was in the crosswalk or not at the time of the crash. I observed minor damage to the passenger side hood/bumper area. Cook-Williams stated he was not distracted at the time of the accident. After speaking with all parties involved, Cook-Williams was issued Massachusetts Uniform Citation T2016426 for Chapter 89, Section 11 (Fail to Yield, Pedestrian in Crosswalk). Walnut Street, Walnut Place, and Washington park are all public ways maintained by the City of Newton. Photos were taken and submitted to the IT Bureau.

MV#1 was traveling east on Commonwealth Ave. Pedestrian was crossing the cross walk at Cedar St from the north side of Commonwealth ave to the south side. The pedestrian stated that he was more than half way across the cross walk when he was struck by a vehicle. The pedestrian stated that he did not see the vehicle until it was to late. Operator of MV#1 stated that she did not see the pedestrian crossing the cross walk until right before she struck him. Operator of MV#1 stated that as soon as she saw him she attempted to stop, but was not able to do so until after she struck the pedestrian. A witness at the scene stated that he was stopped in the west bound side of Commonwealth Ave allowing the pedestrian to cross. He stated that he saw MV#1 approaching the crosswalk and not stopping. He stated that he was not able to signal the pedestrian or operator of MV#1. The witness stated that he observed the pedestrian struck, and immediately got out of his vehicle to help. The pedestrian was transported to the hospital by medics. Pictures were taken of his injuries to be attached to this report. MV#1 was driven off scene. Pictures were take of the damage to MV#1. A citation was issued to the operator of MV#1.

On 11/12/2019 at 12:26 hours, I responded to the Star Market parking lot located at 2040 Commonwealth Avenue for a motor vehicle pedestrian accident. The parking lot is a public/thru way that motorists use to access the Star Market along with other businesses. The weather at the time was cloudy and rainy and the parking lot was saturated. Prior to my arrival, the pedestrian involved left the scene and did not want to speak to the officer nor wanted medical attention. Ofc. Claflin then cancelled Cataldo Ambulance. The pedestrian, Mary Alexander, left her information with a store employee prior to leaving. I spoke to the operator of MV # 1 (Joshua Randel MD Lic.# R534440603244) who stated that he was travelling through the third row of the Star Market parking lot at an appropriate speed. He was then attempting to make a left turn near the entrance/exit of the store. He says that there was a large white van parked in the end handicapped spot towards the left of him. He says the van was sticking out in the spot limiting his visibility. As he was half way through his left turn and coming around the corner, he was not able to see a pedestrian pushing a shopping carriage. He then struck the carriage that was being pushed by Mary Alexander. He further says that the cart jolted Ms. Alexander and she fell down to the ground. At the time, Alexander was walking through the lot near the store entrance heading back towards the parked cars. She was not in the crosswalk at the time which was approximately ten feet down from her location. After, I attempted to go to Ms. Alexander's residence to follow up. I spoke to her friend who was bringing up Alexander's groceries and she stated that she decided to go to Newton Wellesley Hospital for treatment. I then spoke to Ms. Alexander in the lobby of the hospital. She stated that she was exiting the store through the lot with her shopping carriage. She believes she was approximately eight feet away from the door when all of a sudden she only saw red coming at her. She believes MV # 1 was speeding at the time. I asked her if she was struck by the vehicle or was the shopping carriage only struck. She replied that she did not know and only remembers falling to the ground after. She sustained a large bruise to the inside palm area of her right hand and her left knee was swollen. Digital photos of the scene were taken and pictures of Ms. Alexander's injuries were taken at the hospital. As far as possible video surveillance, I was told that the Loss Prevention Department has to access the video. On 11/13/19 at approximately 14:20 hours, I went in to the Star Market and spoke to the manager, Nidal Rajeh, inquiring about possible video surveillance. He said there is coverage in the parking lot and he was able to show me the video. A copy of the video would have to be done by Loss Prevention which I requested. I observed at the 12:12pm mark in the video, MV # 1 was parked five spots up from the third row of the lot that he was exiting. I also observed a white transport van parked in the end handicapped spot mentioned by the operator but it was parked correctly and not limiting visibility. Next, MV # 1 backs out of the spot and drives forward going at a reasonable speed. The pedestrian (Alexander) has already exited the store with her carriage and is walking at an angle toward the same third row of parked cars. As the pedestrian is walking towards the direction of MV # 1, it appears that the pedestrian should be visible to him. MV # 1 then starts to take the left turn around the corner without slowing or stopping and strikes the carriage head on. Due to the impact, Alexander then loses grasp of the carriage and it goes past her twisting her body around. She then hits and rolls up on the hood of the vehicle on her left side. MV # 1 then suddenly stops and immediately gets out and provides assistance to the pedestrian. Due to the video surveillance, Mr. Randel will be issued/mailed MA Uniform Citation # T2012347 for Ch. 90/14/A Failure to slow for a pedestrian.

On 11/30/2019, while assigned to N494, I, Officer Conary, and other units, responded to Charlesbank Road at Nonantum Road for a single motor vehicle crash into a tree. Upon arrival, I observed a white mini van MA registration 5DV184 with heavy front end damage into a tree on Charlesbank Road, a public way in Newton. The operator of the motor vehicle, CORDERO, Ericka, was sitting in the back seat of the vehicle and was talking on the phone. CORDERO's primary language was Spanish. I asked CORDERO if she was okay and she said yes and I asked CORDERO if she was driving the car and she said yes. CORDERO also told me that she did not have a license. MA State Police were on scene and were able to asist with translating. Medics arrived and CORDERO was transported to Saint Elizabeth's Hospital. CORDERO's son and husband arrived on scene and I was able to confirm her information. Both stated that CORDERO did not have an active license. Newton Dispatch ran all information for CORDERO and confirmed she was unlicensed. CORDERO will be charged with MGL Ch.90/10 Unlicensed Operation of a Motor Vehicle and 89/4A Marked Lanes Violation, Uniform MA Citation T2079476. Due to CORDERO being transported to the hospital, the citation will be mailed to her active address. Pictures of the incident were taken and uploaded to IT accordingly. No further incident to report.

On Monday, December 9, 2019, while assigned to Traffic unit N525, I responded to the intersection of California Street at Crafts Street, Newton for a report of a motor vehicle crash involving a pedestrian. The weather at the time of the crash was rain. The road surface was wet. California Street at Crafts Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Kevin Gilmore (S27275907). Gilmore stated he was operating his 2015 Volkswagon Golf (MA: 1EHH93) Westbound on California Street towards Crafts Street. Gilmore stated he came to a stop at the stop sign on California Street. Gilmore stated he looked to his right, and then to his left. Gilmore stated he began to make his right turn onto Crafts Street when his vehicle made contact with a pedestrian in the roadway. Gilmore stated he did not see the pedestrian until contact was made. I observed no damage to the right passenger side of MV1 where the contact occurred. Gilmore reported no injuries. The injured pedestrian, Charles Doris-Pierce (S68331436), was transported to Brigham and Women's Hospital in Boston for his injuries. I followed up with him in the Emergency Department. Doris-Pierce stated MV1 was stopped at the stop sign on California Street. Doris-Pierce stated he entered the marked crosswalk and began to cross California Street at Crafts Street. Doris-Pierce stated MV1's right passenger side tire ran over his foot. Doris-Pierce stated he then slammed on the hood of MV1. Doris-Pierce stated MV1 continued and he was knocked to the ground. Doris-Pierce reported injuries to his foot, thigh, and back. After speaking with all parties involved, the operator of MV1 was issued Massachusetts Uniform Citation T2016430 for Chapter 89, Section 11 (Failure to Yield to a Pedestrian in a Crosswalk). Photos were taken of the scene and submitted to the IT Bureau.

Sgt. McNeil, N495, and I responded to 474 Wodward St. for an auto pedestrian accident. Upon arrival the victim, Elizabeth Cantwell was laying in the street and receiving treatment from NFD and Cataldo. The operator of Ma Reg. 192HV0 was Margaret Meehan. Meehan was visibly upset and was hearing impaired, as a result Meehan was unable to communicate what happened in the accident. At this time we were able to contact Meehans daughter, Bernadette Conley 781-248-6222. Cantwell was transported to Beth Israel Hospital. Traffic Bureau was notified. I went to the BI hospital to get a statement from Cantwell. Cantwell states that her husband parked their vehicle across the street from Starbucks. Cantwell states that she stood next to her car to cross the street. Vehicle 1 came to a stop and Cantwell believed that the vehicle was going to let her cross the street (not in crosswalk). Cantwell states that when she went to cross the street vehicle 1 hit her, causing her to land on the hood and then rolled off onto the street. At this time Cantwell has a dislocated hip and head injury. I took photos of the scene. Meehans vehicle was parked in a legal spot and I transported her to NHQ where she was picked up by her daughter Bernadette Conley. I filled and immediate threat with the RMV and a copy of the report was attached with the form.

On Thursday, January 23, 2020 while assigned to Traffic unit N525, I responded to the intersection of Brookline Street and Hartman Road, Newton for a report of a motor vehicle/pedestrian crash involving a City of Newton Crossing Guard. The weather at the time of the crash was overcast. The road surface where the crash took place was dry. Brookline Street and Hartman Road are both public ways maintained by the City of Newton. On arrival, the injured pedestrian was in the process of being transported to Newton Wellesley Hospital with minor injuries. I spoke with the operator of the vehicle involved in the crash, Menachem Uminer (S43996134). Uminer stated he was operating his vehicle on Brookline Street (S) towards Hartman Road. Uminer stated he was stopped in traffic on Brookline Street prior to the crash. Uminer stated the vehicle in front of him proceeded to travel straight from it's stopped position and he then began to follow the traffic on Brookline Street. Uminer stated he did not see anyone in the vicinity of his vehicle as he began to travel straight from his stopped position. Uminer stated as he was traveling by Hartman Road, the front passenger side area of his vehicle made contact with a pedestrian on the right side of Brookline Street at Hartman Road. Uminer stated he did not see this party until after they were struck and he then proceeded to pull his vehicle over. Uminer stated he was operating his vehicle at a very low speed at the time the collision took place. After speaking with Uminer, I observed the white 2017 GMC Yukon (MA: 9NX525) involved in the crash parked on Brookline Street (S) just past Hartman Road. Uminer stated the vehicle had been moved after the collision took place. I observed no obvious damage to the body of the vehicle on the front passenger side area where the collision took place. The front passenger door mirror was folded inward towards the door. Uminer stated he believes the front passenger door mirror was the point of contact between his vehicle and the pedestrian. I spoke with a witness to the crash, Ashley Garcia (S94193596). Garcia stated she was attempting to cross Brookline Street at Spaulding Lane. Garcia stated she entered the marked crosswalk and was a few steps into Brookline Street when a black colored vehicle traveling on Brookline Street (N) slammed on its brakes near the crosswalk. Garcia stated she looked to her left at the vehicle and then heard a thud to the right from the area where the crossing guard was standing near Hartman Road. Garcia stated she looked over and saw a crossing guard on the roadway on Brookline Street (S) at Hartman Road. The injured pedestrian involved in the crash is Cassandra Alexander (S23636307). Alexander is a crossing guard for the City of Newton and is assigned to the intersection of Brookline Street and Hartman Road for the Memorial Spaulding school crossings. This crash occurred while Alexander was performing her official duties. Alexander was wearing her department issued yellow reflective jacket at the time of the crash. I spoke with Alexander at Newton Wellesley Hospital where she was being treated for her injuries. Alexander stated she was performing her duties as a crossing guard at the intersection of Brookline Street and Hartman Road. Alexander stated she had just finished crossing two pedestrians in the marked crosswalk on Hartman Road when observed a pedestrian enter the crosswalk on Brookline Street at Spaulding Lane. Alexander stated she began to enter the intersection of Brookline Street at Hartman Road with her hands up and her department issued yellow reflective jacket on in an effort to stop traffic in the area to allow the pedestrian in the marked crosswalk safely cross. Alexander stated she was struck on her left side by a passing vehicle. Alexander stated she did not see the vehicle and fell to the roadway. Alexander stated as she was falling to the roadway she extended her right arm in an effort to brace her fall. Alexander stated she had injuries to her arms and legs and was sore from the collision. I spoke with a second witness to the crash via telephone, Anand Vaidya (S43996134). Vaidya stated he was stopped in traffic behind MV1 on Brookline Street (Southbound). Vaidya stated he observed the vehicle in front of MV1 proceed straight on Brookline Street after being stopped in traffic. Vaidya stated he then observed MV1 proceed straight on Brookline Street at Hartman Road at a low rate of speed. Vaidya estimated the speed of MV1 at this time to be 5 miles per hour. Vaidya stated a crossing guard was on the right side of the roadway on Brookline Street at Hartman Road wearing a yellow and black reflective jacket and snow pants. Vaidya stated the crossing guard had her back towards him and MV1, and extended her arm out to stop traffic. Vaidya stated at this time MV1 rolled by the crossing guard at a low rate of speed and the front right fender area of the vehicle hit the left rear leg/lower back area of the crossing guard. Vaidya stated the crossing guard bounced off of the car and fell to the roadway. Vaidya stated he believes MV1 was unaware he hit the crossing guard and observed the vehicle travel another 10 feet until it came to stop on Brookline Street. Vaidya stated the crossing guard was not in a marked crosswalk at the time of the crash. After speaking with all parties involved, Uminer will be cited with Massachusetts Uniform Citation T0115905 for Chapter 90, Section 14 (Failure to slow for a pedestrian in the roadway). Photos were taken of the vehicle and scene and submitted to the IT Bureau.

Operator of vehicle one , Allan Wint stated on 01/27/2019 at 09:40 hours he was driving his employer's pick up truck south bound on Lexington St near number 227. Lexington St is a public way in the city of Newton. Wint is a driver for Auto Zone auto parts located at 140 Lexington St , Waltham, MA ( 781 894-0656 ) . Wint was driving a 2018 Chevrolet Colorado pick up tuck ( MA reg 8CA823 ) that Auto Zone leases from Lease Plan USA Ltd. Wint stated that he was headed to Webster St in Newton to pick up a delivery. Wint stated that as he approached the area of 277 Lexington St he believed that the traffic light that is at the marked crosswalk was green for south and north bound traffic. Wint stated that he did not see any pedestrians in the crosswalk, until he heard the sound of an object impacting into the front driver's side of his vehicle. Wint stated that he stopped his vehicle approximately ten yards past the crosswalk and got out. Wint stated it was then he realized that his vehicle had stuck a female and a dog. Wint stated that he was not operating any electronic devices at the time of the crash, and I observed none in his vehicle. Wint stated he was not injured in crash. Wint's vehicle had damage to the driver's side mirror and lower molding of the driver's side front door. Wint's vehicle did not require a tow. Witness Bessy Shanks stated that she was driving in her vehicle behind Wint's vehicle south bound on Lexington St. Shanks stated that Wint's vehicle was driving slow and swerving approximately two blocks prior to the crash. Shanks stated that she believed that Wint may of been distracted. Shanks stated that south and north bound traffic was stopped because that cross walk light was red and that a female and a dog were crossing from the east to west in the marked crosswalk in font 227 of Lexington St. Shanks stated that the front driver's side of Wint's vehicle srtuck the female and the dog as they were approximately halfway into the crosswalk. Shanks stated that the female and dog both fell to the ground after the crash. Witness Elizabeth Keegan stated that she was driving in her vehicle south bound on Lexington St two vehicle behind Wint's vehicle. Keegan stated that she was not sure what color the traffic light was at the time of the crash, but traffic was stopped both not and south bound traffic was stopped. Keegan also stated that Wint's vehicle " didn't even slow down ". Keegan stated that Wint's vehicle struck the female pedestrian and the dog as they were in the marked crosswalk in front of 277 Lexington St. Witness Henry Taylor stated that his vehicle was the first one stopped at the red crosswalk light at 227 Lexington St in the north bound lane. Taylor stated that north and south bound traffic was stopped and that Wint's vehicle did not stop and stuck a female pedestrian and a dog in the crosswalk. Witness Jacqueline Deos stated that her vehicle was stopped behind Taylor's north bound on Lexington St. Deos stated that north and south bound traffic was stopped for the red light . Deos stated that Wint's vehicle did not stop and that it struck a female pedestrian and a dog in the marked crosswalk. The pedestrian in the crosswalk was identified as Xiaoping Sun. Sun stated that she and her dog stopped on the south bound side of the marked crosswalk across from 227 Lexington St. Sun stated that she activated the crosswalk light and waited until the light was red for north and south bound traffic, and that the walk single was activated. Sun stated that she looked to her left and saw traffic north bound traffic was stopping. Sun stated that she turned to her right and entered the marked crosswalk. Sun stated that she and her dog had crossed over the north bound side of Lexington St and she then looked to her right and saw Wint's vehicle not stopping and " going very fast ". Sun stated that the front driver's side mirror of Wint's vehicle stuck her in the face . Sun stated that as she fell to the ground she was struck on the left leg by Wint's driver's side. Sun stated that she did not lose consciousness after she had fallen to the ground. Sun was transported via EMS to The Brigham and Women's Hospital for treatment for head and neck injuries and abrasions to her legs. Sun's dog died as a result of being struck by Wint's vehicle. Photo's of the crash site and of Wint's vehicle were taken and the disc was downloaded by the NPD's IT Bureau. Before leaving the crash site, I tested the crosswalk lights and they were in working order. Based upon my observations of the crash site, Wint's vehicle, and the statements made to me, I concluded that Wint was at fault for this crash. I issued Wint MA Citation T 2013958 and cited him for a violation of MGL 89/11 , failure to yield to a pedestrian in a crosswalk.

On Tuesday, February 18th 2020 at 1908 hours, I responded to the area of 94 Vine St for a single motor vehicle accident. The vehicle (Mass Reg # 5FRT20, 2013 ELANTRA SEDAN COLOR BLUE) was being operated by William Lake. Lake was headed east on Vine St toward West Roxbury when he crashed into a City of Newton Tree. The road conditions were wet with heavy rain. Lake stated the wet conditions caused him to lose control of the vehicle. There was major front end damage to the vehicle and airbag deployment. Lake suffered a laceration to his left eye and complained of neck pain. Lake was transported to Newton Wellesley Hospital by the Medics. The vehicle was towed by Tody's. I took pictures of the accident and submitted them to the IT Bureau to be uploaded.

Operator of MV1 stated he was traveling Westbound on Washington St. attempting to merge Northbound on Washington St. and as he approached the intersection, he didn't see MV2 and collided with MV2. On 09/21/20 at 14:46 hours, I (Traffic Unit # 523) responded to the parking lot of 88 Chestnut Street for a motor vehicle accident involving a pedestrian. The weather at the time was 60 degrees ( localconditions.com) and the sky was clear and sunny. The road surface was dry. The parking lot is a thru way that the public use to enter/exit the lot or park there. On arrival at the same time with the Fire and Medics, I observed a female laying in the middle of the lot facing down on her back. A small red and gray CVS like type shopping carriage was knocked over and resting on it's side in front of the victim's head. The carriage was moved to the side in order to tend to the victim I also observed a black Honda Civic bearing MA reg.# 3741TW that was involved in the accident. The Honda was out of the parking spot resting approximately two feet away from the pedestrian's legs. I then spoke to the pedestrian identified as Darla Gipson in the rear of the ambulance. She sustained an abrasion to her front left shoulder and arm area which I observed. She also complained of left knee and elbow pain. She stated that she walking through the middle of the parking lot pushing the carriage by the door entrance in a westerly direction towards Chestnut Street. She states that she never saw a vehicle backing up and then she was suddenly struck and believes she was pushed underneath the vehicle. She was then transported to Newton Wellesley Hospital by Cataldo Ambulance. I then spoke to the operator of the Honda Civic identified as Linda Burke (MA S70320414). Ms. Burke was parked in the first spot to the right of the walkway that leads to the entrance/exit doorway. I asked her what occurred and she stated that she was backing her vehicle up to exit the parking lot when she looked behind her not seeing anything in sight. She then started backing up and struck the pedestrian without realizing it. She says the driver of a green cab and his female customer who were no longer on scene were yelling at her to stop her vehicle because the pedestrian was underneath it. She then stopped her vehicle. I observed minor scrapes to the entire rear bumper of the vehicle so it is unknown where it actually struck the pedestrian/carriage. I observed no obvious signs of damage to the carriage. It should be noted that Ms. Burke's Honda does not have a back up camera or back up sensors. There are no surveillance cameras to the apartment complex and no witnesses came forward for the accident. As a result of the investigation, Ms. Burke will be issued Uniform Citation # T1271069 for Newton City Ordinance 19-75 Failure To Use Care in Backing. Digital photos were taken by me of the scene.

Operator of Motor Vehicle # 1 stated that she stopped at the green light on Washington Street (Eastbound ) waiting to take a left turn onto Beacon Street to gain access to the facility of CareOne at Newton. Operator stated she looked and was able to turn left and was halfway into the intersection when a jeep wrangler came out of nowhere driving fast causing her to make impact with vehicle #2. Passenger of Motor Vehicle # 1 stated that they had a green light and were stopped at the intersection of Washington Street and Beacon Street traveling Eastbound attempting to turn left onto Beacon Street. Passenger of Motor Vehicle # 1 stated that they started turning left because they had a clear path becasue a box truck was allowing them to turn and a jeep came out of nowhere which caused them to make impact with vehicle #2. Operator of Motor Vehicle # 2 stated that he was traveling Westbound on Washington Street and he had a green light at the intersection of Beacon Street and Washington Street. Operator of motor vehicle #2 stated he switched lanes to pass the box truck in front of him and he didn't see motor vehicle #1 attempting to take a left turn on to Beacon Street. Operator of Motor Vehicle #2 stated that he sped up to avoid impact with motor vehicle number #1however was unable to avoid impact. Motor vehicle #1 turned right into his drivers side door. Motor vehicle #1 sustained major front end damage and motor vehicle #2 sustained major damage throughout the entire vehicle. Both vehicles were towed by Tody's Towing. Operator and Passenger of Motor Vehicle #1 were transported to Newton Wellesley Hospital with no apparant injuries but for protocal and Operator of Motor Vehicle #2 was transported to Brigham and Women's Hospital with possibly moderate injuries. Several photographs were were taken of the scene and S.D. card was placed in the I.T Bureau.

On November 6th, 2020 at approximately 14:20 hours while assigned to N491 I responded to Centre St on the bridge over the Mass pike N/B for a report of a multiple MV crash with possible injuries. On my arrival to the scene I observed vehicle #1, a grey Honda civic, ma reg. rs794x resting half on the sidewalk the other half crushed into the rear passenger side of vehicle #2. I was then informed there were two additional vehicles involved. Vehicle #1 apparently first crashed into vehicle #2 after abruptly changing lanes into the lane MV#2 was stopped in. This set of a chain reaction in which MV #2 to was forced into the rear of vehicle(#3) which then forced vehicle #3 into the vehicle(#4). Vehicles 2 thru 4 were all stopped due to the signal light being red at the time of the crash. I located the operator to vehicle #1 sitting on the sidewalk bleeding from several areas to his face which he sustained from the crash. He was identified as Terrence Donnelly. He reported driving N/B over the Centre St Bridge and was changing lanes when he realized the traffic was stopped in that lane. He attempted to avoid hitting the vehicle(#2) now in front of him, by swerving to the right to go onto the sidewalk but he still slammed half his vehicle into the rear passenger side of vehicle #2. I spoke with the operator to vehicle #2 identified as Linda Borque. She stated she was stopped on the bridge N/B near the off ramp to the mass pike(exit17 W/B) when she was rear ended by vehicle #1. The impact forced her M/V into the vehicle stopped in front of her(#3). Operator to vehicle #3 was identified as Sylvie Miskovitch. She stated she was also stopped on the bridge N/B for the signal light when she was rear ended by vehicle #2 which forced her vehicle into the vehicle stopped in front of her(#4). Operator of vehicle #4 was identified as Ronise Dorce. She stated she was stopped on the bridge N/B just at the exit to the Mass pike W/B when she was rear ended by vehicle #3. All drivers sustained minor injuries and all declined to go to the hospital. Patient refusal forms were signed by all parties. Vehicle # 1 was the only MV towed from the scene by Todys.

MV2, also exiting the parking lot, struck the driver side of his vehicle. MV1 sustained damage to the driver side bumper. Operator of MV2 stated she was exiting the parking lot, turning eastbound as well and struck MV1 with the passenger side of her vehicle. Operator of MV2 stated MV1 was already out on to the road. Operator of MV2 stated she was distracted due to the recent news she had received at the hospital. Operator of MV2 signed a patient refusal. There was minor damage to the passenger side bumper.

On 04/29/2021 at 15:22 hours I was dispatched to 53 Brentwood Ave for a report of a motor vehicle crash involving a bicyclist. Upon my arrival, I made the following observations of the crash scene. Brentwood Ave is a two way non divided public way in the City of Newton that runs east to west from Greenlawn Ave to Walnut St. Brentwood Ave is a thickly settled residential area with a 25 MPH speed limit. The roadway conditions at the time of the crash were wet due to rain. I observed MA reg 111ZS1 a 2020 Honda Civic parked on the side of Brentwood Ave just passed the intersection of Judith Rd facing westbound. I observed that the Honda had damage to it's front driver's side bumper. When I inspected the Honda I observed that the driver's side headlight casing was cracked and a piece of it was missing. I further observed an approximately six inch in diameter spider web like crack in the lower driver's side windshield. The windshield crack was inward , indicating that an external object had caused the damage. There was another single crack onn the driver's side windshield that extended from the hood to the roof. The operator of the Honda , Lisabeth Ioannilli stated that on 04/29/2021 at 15:20 hours she was driving westbound on Brentwood Ave near the intersection of Judith Rd. Ioannilli stated that she estimated that the speed of her vehicle was approximately 25 to 30 MPH. Ioannilli stated that she did not see a bicyclist in her path of travel until the point of impact when her vehicle was aproximately 3/4 of the way passed the intersection of Judith Rd. Ioannilli stated that her vehicle's front driver's side bumper made contact with the bicyclist. Ioannilli stated that the bicyclist was tossed off of her bike and onto the hood and windshield of her vehicle before rolling onto the ground. Ioannilli stated that she pulled her vehicle to the side of the road and went to check on the bicyclist. Judith Rd is a public way in the City of Newton that runs east and then turns south from Walnut St to Brentwood Ave. There are no traffic control devices at the intersection of Judith Rd and Brentwood Ave. Ioannilli stated that she was not injured in the crash and her vehicle did not require a tow. At the crash scene, the bicyclist , Lucia Campbell, a student a FA Day Middle School ( DOB 11/08/2006 ) was being treated in an ambulance and was transported to Boston Children's Hospital for treatment. Campbell was conscious and was able to answer questions from EMS members. Campbell stated that she was wearing a helmet at the time of the crash and that she rode her bike south on Judith Rd and was turning eastbound onto Brentwood Ave when the crash occurred. I spoke with Campbell's parents Adam Campbell and Iris Delasota who stated that their daughter was under going x rays and other tests to determine if she has internal injuries. I took photo's of the bike, Honda and crash scene and the disk was downloaded by the NPD's IT Bureau. Based upon my investigation of this crash I concluded that Ioannilli was at fault for it. I issued via US mail Ioannilli MA Uniform Citation T1446633 and cited her for a violation of MGL 90/14 failure to use care while approaching a bicyclist. Campbell's bike was turned over to her parents at the crash screen.

On Monday, May 17, 2021 I responded to the parking lot entrance of the Avalon at Chestnut Hill (160 Boylston Street, Newton) for a report of a motor vehicle/pedestrian crash. The weather at the time of the crash was clear and sunny. The road surface was dry. Boylston Street is a public way in the City of Newton. I spoke with the operator of MV1, Mr. Joshua Champion (SA1310472). Mr. Champion stated he was operating his 2013 Lincoln MKS (MA: VT41980) while attempting to exit the Avalon parking lot at the Boylston Street (E) off ramp. Mr. Champion stated he was monitoring the traffic to merge and as he started to make his right turn his vehicle crashed into the left side of a pedestrian in the roadway. Mr. Champion stated he did not see the pedestrian as he was making his turn. I observed no damage to the area where the pedestrian made contact with the car. Photos were taken and submitted to the IT Bureau. The injured pedestrian, Mrs. Joan Contompasis, stated she was walking on Boylston Street (W) with her husband. Mrs. Contompasis stated she entered the roadway from off the sidewalk to cross the Avalon parking lot exit and continue on Boylston Street. Mrs. Contompasis stated she believed MV1 had stopped to allow her and her husband to cross. Mrs. Contompasis stated the vehicle then accelerated and it's front passenger side bumper area crashed into her left side. Mrs. Contompasis fell to the roadway. Mrs. Contompasis was transported to Newton Wellesley Hospital for injuries to her right foot and knee. I spoke with a witness to the crash, Mr. Michl Contompasis. Mr.Contompasis stated he was walking with his wife Joan on Boylston Street (W). Mr. Contompasis stated he was walking just ahead of his wife as they crossed the Avalon at Chestnut Hill exit at Rt. 9. Mr. Contompasis stated he observed a black sedan to his left as he crossed. Mr. Contompasis stated he believed the operator was not paying attention as he exited the parking lot and the vehicle accelerated into his wife's left side.

On Wednesday, June 16, 2021 while assigned to Traffic unit N525, I responded to the front entrance of the Brown Middle School located at 125 Meadowbrook Road in Newton for a single vehicle crash involving a City of Newton contracted school bus. It was also reported by Newton Dispatch that one of the occupants on the bus had suffered an arm injury as a result of the crash. Meadowbrook Road is a public way maintained by the City of Newton. The road surface at the time of the crash was dry. The weather at the time of the crash was clear and sunny. When I arrived, I was updated by Officer Iarossi that the crash involved a City of Newton contracted school bus transporting students from the Williams School (141 Grove Street, Newton). The students were participating in "Step Up" day and were leaving Brown to go back to Williams. Officer Iarossi stated a student on the bus was injured as a result of the crash. Tegan Quirk was transported by Fallon Medics to Newton Wellesley Hospital with a right arm injury. The remaining students that were on the bus reported no injuries to staff at the time and were moved to another bus to be transported back to the Williams School. I asked the Brown staff that was assembled in front of the school to contact the Williams School and ask them to hold the children involved in the crash at the school once they arrived. I made this request so we could make sure there weren't any new injuries reported; and I wanted to inquire if any of the children witnessed the incident. The staff stated they would contact Williams and relay the request. I spoke with the operator of the school bus (MA BUN: BU44536), Mr. John Aubert (S48613114). Mr. Aubert stated he was operating bus #361 while transporting students from Brown Middle School to the Williams School. Mr. Aubert stated as he was preparing to leave Brown, he made an announcement to students on the bus to sit down. Mr. Aubert stated he got the okay to leave from one of the staff members on the bus. Mr. Aubert stated there was another bus parked in front of him prior to leaving. Mr. Aubert stated he began to reverse the bus to create distance from the bus in front of him, and then operated the bus forward to exit from his position on the right side of the road and merge into the travel lane to his left around the bus parked in front of him. Mr. Aubert stated as he merged onto the roadway, he heard screaming from the students on the bus and the rear passenger side of the bus made contact with a brick support column in front of the school positioned along the sidewalks edge next to the roadway. Mr. Aubert stated he exited the bus and observed a student's arm pinned on the rear passenger side of the bus between the side of the bus and the brick column. Mr. Aubert stated he got back in the bus and reversed it until the students arm was free. Mr. Aubert stated at this time, staff helped the injured student. The staff there helped facilitate transportation for the remaining students back to the Williams School. Mr. Aubert reported no injuries, but was concerned for the welfare of the student. After speaking with Mr. Aubert, I observed damage to the school bus and the brick support column as a result of the crash. I observed fresh damage (scrapes) to the rear passenger side of the school bus between the last passenger side window and the back of the bus. I also observed fresh damage to the brick support column. The column is a support to an overhead covering that runs from the sidewalk to the front entrance of the school. There was fresh paint transfer from the bus and a crack in the column. Pictures were taken of the damage and submitted to the IT Bureau. The travel lane where the crash took place is a one way access road that runs from Meadowbrook Road to the front of the school, and exits onto a parking lot between the Brown Middle School and the Oak Hill Middle School. I responded to the Williams School to follow up with staff there regarding this crash. All of the students that were involved in the crash had been released prior to my arrival. I spoke with the Principal of the Williams School, Ms. Ayesha Farag, Ms. Michal Shapiro (Social Worker), and Ms. Katie Costello (School Nurse) about the students on the bus. They stated no injuries were reported to them, and they notified the parents of the involved parties about the crash. They also provided me with contact information for all of the students on the bus at the time of the crash. I spoke with the three teachers that were on the bus at the time of the crash, Ms. Kimberly Andersen, Ms. Tess McLean, and Ms. Katherine O'Leary. All three teachers stated prior to the bus leaving Brown Middle School they accounted for all of the students and were ready to leave to travel back to the Williams School. One of the teachers recalled hearing the bus driver make an announcement to the bus that they were about to leave, but could not recall the exact wording of the announcement. The teachers stated as the bus began to move, they heard screaming from the rear of the bus, looked back, and saw a student's arm stuck outside the window. After realizing the student was injured, they assisted with helping to free the student from his pinned position. They helped the injured student until EMS arrived on scene. They assisted the remaining students by directing them away from the crash and onto another bus for transportation back to Williams. The three teachers reported no injuries as a result of the crash. I responded to the Newton Wellesley Hospital Pediatric Emergency Room to check on the status of the injured student, Tegan Quirk. I spoke with Tegan and his mother, Kathryn Quirk in his room. While attempting to talk to Tegan and his mother, Tegan became very upset and did not want to talk about the specifics of the crash. Out of courtesy for Tegan, I exited the room and spoke with his mother outside the room. I explained to Ms. Quirk the events of the crash, and how her son's arm became injured. I asked Ms. Quirk if she could assist me with faciliating a conversation with Tegan about the events of the crash. I explained to her I understood how traumatic the events of the crash were for her son, but as part of my investigation I would need to find out what led to Tegan's arm being outside the bus window at the time of the crash. While speaking to his mother, Tegan explained that he was waving to a friend with his arm out of the bus window when the bus started to move. Tegan stated his arm hit the column, and became stuck between the bus and the column. Tegan stated he was screaming for the bus to move and that it took forever for the bus to move so he could free his arm. Tegan stated the bus driver gave no warning to the students that the bus was going to start moving. The staff at Newton Wellesley Hospital stated Tegan has a right arm injury. The severity of the injury is unknown at this time due to further examination that needs to take place. I advised Ms. Quirk about the conversations I had with the other parties involved. I spoke with Mr. David Stickney (Head of Facilities) from the Newton Public Schools to try and obtain footage of the crash. Mr. Stickney stated unfortunately there is not footage of the crash from the Brown Middle School or Oak Hill Middle School surveillance cameras. I advised Ms. Regina Moody (Newton Public Schools Transportation Director) about the crash. Ms. Moody was able to show me footage of the incident from the camera inside the bus. In the footage, the bus driver can be heard announcing to the students prior to the bus moving, "Guys, guys, get off the seat and sit correctly". One of the teachers can be seen doing a count of the students. The teacher is heard saying to the student, "Everyone needs to be in a seat and sitting at 3 and 2 and 1 and thank you." Student are observed still standing up. The teacher is then heard saying, "sit down sit down." After the teacher passes the students in the back of the bus, she says, "We are good." to the front of the bus. Students are then observed standing after she passes. As the bus begins to back up, students can be observed standing in the rear of the bus and not sitting down on their seats. As the bus moves forward, a student (Tegan Quirk) can be seen standing on his seat and extending his right arm out of the last window on the passenger side of the bus. Screaming can then be heard as his arm makes contact with a support column on the passenger side of the bus. The bus is stopped, the driver exits the bus to asses what happend, gets back on the bus, and reverses the bus a short distance. The injured students right arm is released and he brings his arm back into the bus through the window. The amount of time between the students arm being pinned against the column to when it is released is approximately 40 seconds. After speaking with all parties involved and reviewing the footage from inside the bus, it appears that Bus #361 was pulling out of it's parked position in front of the Brown Middle School to enter the roadway and return students to the Williams School. A student that was sitting in the last row on the passenger side of the bus extended his right arm out of a window next to his seat to say goodbye to a friend. While his right arm was extended out of the rear passenger side window of the bus , the bus collided with a brick support column affixed to the sidewalk in front of the school. When the bus made contact with the column, the students right arm made contact with a brick support column and became pinned between the column and the passegner side of the bus. After the students arm was freed from it's pinned position, the student was able to bring his arm back into the school bus and was transported to Newton Wellesley Hospital for a right arm injury.

On Wednesday July 14th, 2021 at approximately 1434 hours I responded to the back of the police station for a report of a motor vehicle accident involving a Newton Fallon Ambulance. At the time of the accident the weather was sunny and the road surface was dry. Washington Street and Highland Street are owned and maintained by the City of Newton. Upon arrival I spoke with detail officer A. Boudreau who stated he was directing traffic on Washington Street in the area of Highland Street and had the westbound traffic on Washington Street stopped as a Newton Fallon Ambulance was passing with its lights and sirens on. Detail officer Boudreau stated he stopped MA REG 765RL2, a 2013 Chrysler town van so the ambulance could get by and as the ambulance went to pass the vehicle on the left the Chrysler van proceeded to attempt to take a left turn onto Highland Street and struck the ambulance on its passenger side. The ambulance continued onto Washington Street as it was transporting a patient to NWH. Detail officer Boudreau instructed the driver of the van identified as Janet Waitz (S76373427) to pull into the parking lot of the police station and wait for an officer to file a report. The ambulance was advised to respond to the police station after transport. No injuries were reported. I spoke with Janet who stated she was attempting to take a left turn onto Highland Street and was stopped by an officer. She stated she thought the officer waved her through so she proceeded to take the left turn and struck the ambulance as it passed her. Her vehicle had minor front end damage and she reported no injuries. The operator of the ambulance arrived shortly after and was identified as Shane McMahon (S249990773). He stated he was transporting a patient to the hospital and was traveling westbound on Washington Street. An officer had the traffic stopped so he could pass safely and as he passed the Chrysler van it attempted to take a left and struck the passenger side of the ambulance causing minor damage. He stopped and spoke with the officer who advised him to continue to transport the patient and respond back to the police station after to file a report. No injuries were reported. Pictures of both vehicles were taken and submitted to the IT Bureau.

The weather at the time of the crash was cloudy and overcast. The road surface at the time of the crash was dry. The Newton Centre Municipal Parking Lot is open for public travel and use and is maintained by the City of Newton I spoke with the operator of the 2016 Audi 32 (MA: 4PL345) involved in the crash, Mr. Peter Schiller (S26414610). Mr. Schiller stated he entered the Newton Centre lot from Langley Road and made a right turn to enter the back row of parking stalls. Mr. Schiller stated he then took a left turn into the parking area to look for a spot. Mr. Schiller stated he did not see the pedestrian in the roadway in time and his vehicle's front end crashed into the pedestrian. Mr. Schiller estimated the crash happened right near the first car parked in the row closest to where he entered. I showed Mr. Schiller the believed area of impact was actually 3 car stalls in, and the crash occurred in the middle of the travel way. I asked Mr. Schiller if he has any medical conditions or takes any medication that would effect his ability to operator a motor vehicle. Mr. Schiller stated he does not. Mr. Schiller reported no injuries as a result of the crash. I observed very minor damage to the front hood/bumper/fender area of his vehicle from where the crash took place. I did observe soil covering the hood area. The injured pedestrian was carrying a plant with soil in the pot. I asked Mr. Schiller if he could recall where he made contact with the pedestrian or how the pedestrian ended up in front of his vehicle. Mr. Schiller stated he thought the injured pedestrian was running towards his car when the made his turn. I spoke with a witness that remained on scene, Mr. Nicholas Ryland. Mr. Ryland stated he was walking in the Newton Centre lot while pushing his daughter in her stroller. Mr. Ryland stated he observed MV1 enter the lot from Langley Road and took the right turn into the lot at a high rate of speed. Mr. Ryland stated he stopped with the stroller next to the first vehicle parked in the roadway where the last parking stalls begin. The MV1's driver side operated past Mr. Ryland's left side, took a left turn, and then Mr. Ryland stated he heard the crash. The crash took place in the middle of the roadway in between the parking stalls to the East and West of MV1. I responded to Brigham and Women's Hospital in Boston to follow up with the pedestrian injured in the crash, Ms. Sophia Alexandridies (S49399467). Ms. Alexandridies was transported to the emergency department by Fallon Medics with multiple injuries including a head injury. Due to Ms. Alexandridies's injuries, I was unable to obtain a statement from her about the events of the crash. I spoke with Ms. Alexandridies's daughter, Christina, in the emergency department. Christina stated Ms. Alexandridies suffered a head injury, along with possible broken ribs and an injury to her left shoulder. Most of the injuries reported are on Ms. Alexandridies's left side, the presumed area of impact with the vehicle. Due to the circumstances of this crash, and Mr. Schiller being involved in another surchargeable crash on April 28, 2021, a Request for Immediate Threat License Suspension was submitted to the Registry of Motor Vehicles at DCUImmediateThreat@massmail.state.ma.us. Mr. Schiller is also being cited with Massachusetts Uniform Citation 119853AB for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). On September 2, 2021, I contacted Mr. Schiller to provide him with additional information about the crash. Mr. Schiller stated he did not realize Ms. Alexandridies was injuried in the crash. Mr. Schiller also stated he has no recollection of being involved in a crash in April. Mr. Schiller was advised to contact his insurance company to inquire about his last crash. Photos were taken of the area and MV1 and submitted to the IT Bureau. I canvassed the area for survellance footage of the crash with a negative result.

The pedestrian, identified as Yuanhua Wei stated that she was walking west on Derby St and attempting to cross Parmenter Rd in the cross walk when she was struck by MV#1. The operator of MV#1 stated that she was traveling east on Derby St and began turning left onto Parmenter Rd because in her words, she didn't observe the pedestrian. The operator stated as she turned through the intersection, she observed the front of her MV strike the pedestrian, the pedestrian dropped to the ground and the MV#1 pulled over to the right side of Parmenter Rd. When asked, the operator stated she were not distracted while operating her MV. When I arrived Mrs. Wei was sitting in the cross walk directly next to the north west corner of that intersection. When asked about her injuries, she stated her hip hurt, she showed me an abrasion on her right elbow/forearm and her lower body was sore. I also observed one of Mrs. Wei's sneakers was on the ground next to her. Mrs. Wei was sitting the entire tire time, did not appear to have any serious injuries and attempted to get up shortly after I arrived, but advised her to remain sitting. Paramedics evaluated Mrs. Wei for minor injuries at the scene and she chose not to be transported to a local hospital for her injuries. I did not observe any damage to MV#1, which the operator confirmed. I informed both parties that I was filing a crash report with charges pending.

On Thursday, September 23, 2021, while assigned to Traffic unit N525, I responded to the area of 358 Beacon Street, Newton for a report of a motor vehicle/bicycle crash with injury. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of the vehicle involved, Mr. Thomas Mogan (S29907615). Mr. Mogan stated he was operating his 2015 Jeep Wrangler (MA: 6EBH20) on Beacon Street (W) towards his residence of 358 Beacon Street. Mr. Mogan stated he observed a break in the traffic traveling on Beacon Street (E) towards Hammond Street in front of his driveway. Mr. Mogan took a left turn from the Westbound lane of Beacon Street to enter his driveway. While making his turn, his vehicle crashed into a bicyclist traveling on Beacon Street (E) in the marked bicycle lane in front of 358 Beacon Street. Mr. Mogan stated he did not see the bicyclist until the front passenger side fender/door area of his vehicle made contact with her. I observed minor damage to this area on his vehicle. Mr. Mogan reported no injuries. The injured bicyclist was identified as Ms. Louisa Palmer (S67185637). Ms. Palmer was treated on scene by Fallon Medics for injuries to her left wrist and ankle and transported to Brigham and Women's Hospital for further evaluation. Ms. Palmer was wearing a helmet at the time of the crash and I observed her blue Bianchi Pista bicycle resting on the sidewalk in front of 358 Beacon Street. The bicycle had minor damage in to it's front wheel area and fresh scrapes near the handlebars. I transported the bicycle to Ms. Palmer's residence. I responded to Brigham and Women's Hospital in Boston and spoke with Ms. Palmer in the Emergency Department. Ms. Palmer stated she was riding her bicycle on Beacon Street (E) towards Boston College. Ms. Palmer stated she was riding in the marked bicycle lane with the motor vehicle traffic to her left. Ms. Palmer stated she observed a break in the traffic on Beacon Street ahead of her, and then a Jeep entered her travel lane after it took a left turn from the Westbound travel lane on Beacon Street right in front of her. Ms. Palmer stated her left side crashed into the front passenger side wheel/hood area. Ms. Palmer stated she then fell to the roadway. Ms. Palmer stated she has injuries to her left wrist/arm area, her left ankle, and her hips. I spoke with a witness to the crash, Ms. Kelli McCusker. Ms. McCusker stated she was traveling on Beacon Street (E) towards Boston College at the time of the crash. Ms. McCusker stated there was heavy traffic in the area as she observed a bicyclist travel by the passenger side of her car. Ms. McCusker stated there was a break in the traffic ahead of her in the Eastbound lane and she observed a Jeep in the Westbound lane of Beacon Street make a left turn in front of the car in front of her. Ms. McCusker stated the Jeep and the bicyclist crashed into each other at this time. After speaking with all parties involved, Mr. Mogan did not yield to the right of way while Ms. Palmer was traveling on Beacon Street (E) on her bicycle. Mr. Mogan will be issued Massachusetts Uniform Citation for Chapter 90, Section 14B (Fail To Yield To Bicyclist on Turn). Photos were taken of the scene and submitted to the IT Bureau.

On 10/01/2021, while assigned to N494, I, Officer Conary, responded to Centre Street and Hyde Ave for a report of a single vehicle accident. Upon arrival, I met with operator of MV1, who was outside of the vehicle. There was significant damage to the left front tire and front end. Operator of MV1 explained to me that she was traveling Northbound on Centre Street and next thing she remembered was being pulled over on the side of the road and not being able to drive the vehicle. Operator of MV1 stated she was not traveling fast because there was another vehicle in front of her. At this time, medics were called on scene for an evaluation. I was able to determine that MV1 was traveling Northbound on Centre Street, veered off to the right, struck the curb, which caused the tire to pop, and then the front end grazed the utility pole. Tody's towed the vehicle. Operator of MV1 was transported to the hospital. There was minor damage to the Verizon Utility Pole. No structural damage. Verizon was notified. The city was notified to clean debris in the road. Pictures of the utility pole were sent to IT to be uploaded accoridingly.

MV2 operator stated he had exited Rt.95 northbound at the Grove St exit. MV2 operator said he was attempting to drive straight on to Recreation Rd/Rt.95 northbound on-ramp. MV2 operator said he stopped at the stop sign at Grove St and looked both ways. MV2 operator stated he did observe MV1 driving southbound on Grove St at a reasonable speed. However, MV2 operator said he thought MV1 had stopped to let him go. MV2 operator said as he attempted to drive straight onto Recreation Road/Rt.95 northbound on-ramp, he struck the driver side of MV1. MV2 operator reported no injuries at this time. MV2 sustained extensive front end damage, and the driving wheel airbag was deployed. MV2 was towed by Todys.

Operator of MV1 further stated he proceeded Eastbound on Beacon St. and bypassed three vehicles stopped at the intersection of Beacon St. at Hammond pond parkway. Operator of MV1 was transported to Newton Wellesley Hospital for an evaluation. Operator of MV2 did not complain of any injuries on scene and refused medical attention. MV1 sustained heavy drivers side damage with both side air bags deployed. MV2 sustained moderate right front passenger side damage. MV1 was towed by Tody's service on scene. A towed motor vehicle form was filled out and attached to this report. A copy of the towed motor vehicle form was placed on file. Pictures were taken on scene by Sgt. On Tuesday 3/1/22 at approximately 1300 hours while assigned to marked unit n498 I was dispatched to the area of 980 Boylston Street for a report of a motor vehicle into a building. Upon arrival fire and medics were on scene treating the operator of motor vehicle #1 (EDMAN Brigitta). EDMAN's vehicle crashed into the front of Mr K's Liquor store smashing out the front window and doing minor damage to the inside of the store. Prior to striking the building she also slightly struck an Anton Cleaner's van doing very little to no damage to the van. EDMAN states she was attempting to park in the handicap parking spot in front of the store when she accidentally stepped on the accelerator instead of the brake, which caused her vehicle to lunge forward and crash into the front of the building. EDMAN was not trapped in the vehicle but had difficulty exiting the vehicle so she was assisted by Newton Fire. EDMAN was also evaluated and transported to Newton Wellesley Hospital by Medi 4. While on scene Newton Inspectional Services was notified and responded to determine the structural integrity of the building. After looking inside the building Inspectional Services requested the vehicle be removed at which time Tody's was notified. Upon removal Inspectional Services also inspected the outside of the structure and determined the structural integrity of the building was not compromised and Mr. K's was able to stay open for business. Due to the nature of the crash I requested through the Massachusetts Registry of Motor Vehicle an immediate threat license suspension / revocation for EDMAN's drivers license. Seven pictures were taken and forwarded to IT to be attached to this report.

Operator of MV3 was transported to Newton Wellesley Hospital for possible minor injuries. MV1, MV2 and MV3 were towed on scene by Tody's towing. A towed motor vehicle form was filled out for each. All streets listed are considered public ways in the city of Newton.

MV1 and MV2 were stopped in traffic in front of 821 Beacon St when MV1 rear ended MV2. Operator of MV2 stated he was waiting in traffic when he was struck from behind by MV1. The operator of MV1 stated he thought the vehicle in front of him was moving forward and proceeded to accelerate and struck the vehicle in front of him. I asked him if he saw the brake lights of the vehicle in front of him which replied that he did not see the brake lights. Both vehicles were operable and were able to leave the scene. The back seat passenger of MV2 sustained minor injuries, which she stated she had not been wearing her seat belt and stated she was having neck and head pain from striking her head off the front seat. Fallon Ambulance Service responded and transported the injured passenger to Newton Wellesley Hospital to be evaluated.

MV 1 was traveling southbound on Craft St when he rear ended MV 2. MV 1 sustained major front end damage and the front air bag was deployed. MV 1 was towed by Tody's due to it being disabled. The operator and owner of MV 1, Mr. Alan Green, sustained a head and neck injury and was transported to Newton Wellesley Hospital by Fallon medics. MV 2 was parked in front of 131 Craft St with its hazards on delivering food, when MV 1 rear ended him. MV 2 sustained major rear end damage and was towed by Tody's due to it being disabled. The operator and passenger of MV 2 sustained neck injuries and were transported to Newton Wellesley by Fallon medics. MV 3 was legally parked and unoccupied in front of MV 2 when MV 1 rear ended MV 2 and subsequently rear ended MV 3. MV 3 sustained minor damage to the rear end. A crash report number was left for the uninvolved owner, Ms. Janet Goff, of MV 3.

The operator of MV#1 stated he was traveling northbound on Highland St. when he became distracted attempting to locate the street sign to input into his GPS. The driver's lack of attention to the roadway resulted in his vehicle rear ending MV#2 (unoccupied) which was parked facing northbound in front of 60 Highland St. MV#1 then continued into the parking lot of 60 Highland St. MV#1 sustained heavy front end damage including deployment of both front and side airbags. The operator of MV#1 was later transported to the hospital and his vehicle towed from the scene. MV#2 which sustained a flat rear tire and apparent damage to its axle was also towed from the scene, as it was obstructing the flow of traffic. Attempts to make contact with the owner of MV#2, were unsuccesfull.

Operator of MV1 was parking in a parking spot outside of Pressed Cafe located at 300 Needham St. Operator believed she was in Park but was actually in Drive. Operator mistakenly pressed on the gas and accelerated over the curb and through a large window and ended up inside of Pressed Cafe. Operator and her daughter, Brynn Bryant, were trapped in the vehicle and freed by NFD. Operator and Brynn were transported to Newton Wellesley Hospital. The other 2 passengers, Gabe Cohen and Marc Andrew, were not trapped and declined any medical attention and left the scene after speaking with the medics. MV1 was towed by Todys. Nobody inside Pressed Cafe was injured. John Faggiano was standing next to where the car went through the building but declined any medical attention. Pictures and a video of the accident from the security system was sent to IT to attach to the report.
