ON 1-27-2020 AT APPROX. 1251HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT THE INTERSECTION OF CHESTNUT AND OTIS ST. I SPOKE TO THE OPERATOR OF VEHICLE #1 DRIVER STATES SHE WAS TRAVELING S-BOUND ON CHESTNUT ST. SHE STATES SHE DOES NOT RECALL FADING LEFT GOING OVER THE YELLOW LINE AND BANGING INTO VEHICLE #2. COMP. WAS TRANSPORTED BY MEDIC2 TO NEWTON WELLESLEY HOSPITAL FOR MINOR INJURIES AND TO POSSIBLY DETERMINE WHY SHE BLACKED OUT. VEHICLE #2 STATES HE WAS TRAVELING N-BOUND ON CHESTNUT. HE NOTICED VEHICLE #2 VEER INTO HIS LANE BUT BELIEVED SHE WAS GOING TO READJUST HER VEHICLE. HE STATES WHEN SHE DIDN'T HE WAS UNABLE TO AVOID HITTING HER. VEHICLE #1 HAD EXTENSIVE FRONT END DAMAGE AND AIRBAG DEPLOYMENTS AND WAS TOWED BY TODYS. VEHICLE #2 HAD EXTENSIVE FRONT END DAMAGE AND WAS TOWED BY TODYS. OPERATOR OF VEHICLE #1 WAS TRANSPORTED TO NEWTON WELLESLEY HOSPITAL FOR MINOR INJURIES AND EVALUATION. OPERATOR OF VEHICLE #2 REPORTED NO INJURIES AND SIGNED A PATIENT REFUSAL. Operator of Vehicle #1 stated that he was traveling east on Beacon Street. Operator stated that he was making a right turn into the driveway of his parents home (1250 Beacon) when Vehicle #2 tried to pass him on the right. This area of Beacon Street is a one lane road in each direction with an established breakdown lane. Vehicle #2 struck Vehicle #1 on the passenger side front and middle. Vehicle #1 was then pushed back into the lane of travel on Beacon street. Vehicle #2 left the roadway and came to the rest on the sidewalk. Vehicle #2 also popped the driver side front tire. Operator of Vehicle #2 stated that he was traveling east on Beacon street behind Vehicle #1. Operator stated that Vehicle #1 had a left turn signal on and swung over the double yellow line as if he was turning left onto Warren Rd. Operator stated that he believed Vehicle #1 was making a left and therefore went around the vehicle. I again spoke with Operator #1 who stated that it was not possible for him to be making a left turn as he was well aware where the driveway to his parents house was. Operator #2 complained of bodily injury and was transported to Newton-Wellesley Hospital by Cataldo Medics. Vehicle #2 was towed from the scene by Todys.

THE OPER OF MV#1 WAS TRAVELLING SB ON PARKER ST (#546), WHEN HE COLLIDED WITH THE REAR OF MV#2 ( PARKED LANDSCAPE TRAILER). ONLOOKERS, FIRE AND MEDICS, ATTENDED TO THE DRIVER PRIOR TO MY ARRIVAL. THE OPERATOR APPEARED TO BE HAVING SOME SORT OF MEDICAL EPISODE (SEIZURE LIKE) AFTER THE CRASH. IT IS UNKNOWN IF THE PARTY WAS HAVING THE MEDICAL EPISODE PRIOR TO THE CRASH. MV1 WAS TOWED WITH FRONT END/WHEEL DAMAGE. THE OPERATOR WAS TRANSPORTED TO THE HOSPITAL.

On Wednesday, October 21st 2020 at 0341 hours, Myself N-499 and Ofc Crowe N498 responded to 524 Parker St for a single vehicle accident into a city tree. Upon arrival, I identified Mass Vet #VT36122, 2016 Dodge Sedan, Color Black had crashed into Verizon Pole number 340/50A. The airbags had been deployed. The operator was no longer in the vehicle. The operator was sitting down on the ground nearby . He was identified as Joshua Stomberg. Stomberg admitted he had operated the vehicle. Stomberg stated he was driving south on Parker St toward the VA Hospital for his work shift. Stomberg stated he was tired and did not feel alert. Stomberg also stated there was a heavy fog that made the road difficult to see. Stomberg's vehicle veered off the side of the road and collided with the pole causing heavy front end damage. Stomberg did not show any signs of being impaired by alcohol. An inventory of Stomberg's vehicle showed no discovery of any alcohol. It should be noted that on my way to the accident scene, I observed a very heavy fog on Parker St that made it difficult to see the road in front of me. Stomberg suffered an injury to his back and was transported to the VA Hospital by Cataldo Ambulance. Tody's towed the vehicle. Stomberg was mailed MA Uniform Citation # T2013041 for the amount of $ 105 for M.G.L Ch 89/S4A Marked Lanes Violation. Pictures were taken of the accident and submitted to Newton IT Bureau.

Narrative:
On Wednesday, August 4, 2021, while assigned to marked unit N493, I responded to the area 2115 Commonwealth Avenue, Newton with several other units for a report of a motor vehicle crash with entrapment. Commonwealth Avenue is a public way maintained by the City of Newton. The weather at the time of the crash was clear and sunny, The road surface was dry. On arrival, I spoke with the operator of MV1, Mr. Robert Bemis (S57110680). Mr. Bemis stated he was operating his 2019 Nissan Murano (MA: 1WHT71) on Commonwealth Avenue (W) towards Melrose Street. Mr. Bemis stated a red SUV traveling on Commonwealth Avenue (E) entered his travel lane and he had to swerve to avoid crashing head on. The front driver side of MV2 crashed into the driver side of MV1. I observed moderate damage to the entire length of MV1's driver side. Mr. Bemis reported no injuries and signed a patient refusal with Fallon Medics. Mr. Bemis was able to operate his vehicle away from the area. I spoke with the operator of MV2, Mr. Robert Shea (S86188377). Mr. Shea stated he was operating his 2011 Chrysler Town and Country (MA: 2LYT84) on Commonwealth Avenue (E) towards Lexington Street. Mr. Shea stated he was attempting to make left turn towards Lexington Street when MV1 hit him. Mr. Shea was transported to Newton Wellesely Hospital for evaluation. I observed signifcant damage to the front driver side of MV2. MV2 was removed from the roadway by Tody's Towing. The point of impact of the crash occured in the Westbound lane of Commonwealth Avenue. While traveling Eastbound on Commonwealth Avenue, MV2 crossed the double yellow unprotected median on the roadway and entered the Westbound travel lane. MV2 applied his brakes leaving skid marks to the point of impact and crashed into the driver side of MV1. The operator of MV2 will be cited with Massachusetts Uniform Citation 090421AB for Chapter 89, Section 4a (Marked Lanes Violation).

Narrative:
On Monday, July 11, 2022, while assigned to unit N492, I responded to the area of Commonwealth Avenue and Temple Street, Newton for a report of a crash with minor injuries. The weather at the time of the crash was clear and sunny. The road surface was dry. Commonwealth Avenue and Temple Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Ms. Rui Zhang (SA4940828). Ms. Zhang stated she was operating her 2016 Subaru Outback (1WRP35) on Commonwealth Avenue (W) towards Temple Street. Ms. Zhang stated she activated her right turn directional and was slowing down to take a right turn onto Temple Street. Ms. Zhang stated she began to take her right turn, a vehicle appeared on her right side and crashed into the front passenger side of her vehicle. Ms. Zhang her and her son Kenneth reported no injuries as a result of the crash. I observed damage to the front passenger side bumper/tire/fender area of MV1. Tody's responded and removed the vehicle from the roadway. I spoke with the operator of MV2, Mr. Thomas Conneely (S30459555) as he was being treated by Newton Medics. Mr. Conneely stated he was operating his 2019 Honda CRV (MA: 58252) on Commonwealth Avenue (W) towards Temple Street. Mr. Conneely stated he was traveling behind MV1 for some time and the vehicle was slowing down and speeding up down the length of Commonwealth Avenue. Mr. Conneely stated as they approached the Temple Street intersection, MV1 had stopped in the middle of the road and he believed it was taking a left turn on to Temple Street. Mr. Conneely stated he went to around the right side of the vehicle when it suddenly took a right turn into his vehicle as it was passing. Mr. Conneely said his vehicle then went airborne twice and he ended up in the yard of 1633 Commonwealth Avenue. Mr. Conneely and his passenger, Ms. Susan Conneely were transported to Newton Wellesley Hospital by Newton Medics for evaluation. Tody's towing reponded and removed MV2 from the property of 1633 Commonwealth Avenue. Ms. Nancy Ghabi stated she was operating her vehicle on Commonwealth Avenue (W) behind MV1 and MV2 at the time of the crash. Ms. Ghabi stated she observed MV2 to be traveling at a high rate of speed and observed it travel to the right of MV1 and try to go around it. Ms. Ghabi stated she two vehicles then crashed and MV2 ended up in the lawn of 1633 Commonwealth Avenue. Ms. Ghabi described the actions of MV2 to be "erratic". Mr. John Carey stated he witness the crashed as he was standing on the sidewalk in the area of 1633 Commonwealth Avenue. Mr. Carey stated MV1 was slowed down in the intersection and MV2 tried to go around MV1's passenger side. The vehicles then crashed and Mr. Carey believes MV2 did not attempt to brake post collision and MV2 ended up on the lawn of 1633 Commonwealth Avenue. Mr. Conneely will be cited with Massachusetts Uniform Citation 510670AB for Chapter 89, Section 4A (Unsafe Lane Change) and Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Pictures of the property damage to 1633 Commonwealth Avenue were taken and submitted to the IT Bureau.
