Narrative:
On Thursday, August 29, 2019 while assigned to Traffic unit N525, I responded to 837 Beacon Street, Newton for a report of a motor vehicle crash involving a City of Newton Fire truck. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I met with the operator of MV1, Newton Firefighter Keive Mullings. Mullings stated he was operating Newton Fire Ladder 2(MA MF4951) Eastbound on Beacon Street towards Centre Street. Mullings stated the rear passenger side bumper area of MV1 made contact with a unoccupied motor vehicle parked Eastbound on Beacon Street in a metered spot across from 837 Beacon Street. No injuries were reported by the occupants of Ladder 2, I observed minor damage to the rear passenger bumper area of MV1. I spoke with the owner of MV2, Pakizeh Motiee (2017 BMW X3, MA: 7NCC90). I observed minor damage to the front driver side quarter panel area of MV2. Motiee was advised. Pictures of both vehicles were taken and submitted to the IT Bureau.

Vehicle 1 a city of Newton Fire Engine was parked in the driveway of Newton Wellesley Hospital 2014 Washington St. Vehicle 2 was located in the driveway directly behind Vehicle 1. Vehicle 2 collided with Vehicle 1 while attempting to pass Vehicle 1 on the left. As a result of the impact Vehicle 1 sustained minor damage to the drivers side. Vehicle 2 sustained damage to the passenger's side front bumper and fender. No injuries were reported. No tows were required. It should be noted that Vehicle 1 was on scene due to a fire alarm activation at Newton Wellesley Hospital. The operator of Vehicle 2 stated that while behind Vehicle 1 he noticed another Fire Engine approaching. The operator of Vehicle 2 stated that he collided with Vehicle 1 while trying to move out of the way of the approaching Fire Engine. Pictures were taken on scene and submitted to the NPD IT Bureau.

On 8/25/2020 at approx 1245hrs while assigned to 497 I responded to NWH emergency ambulance entrance for a report of a past hit and run reported by Walpole Fire Dept personnel. Upon arrival I met with Firefighter Albert KING who stated he was travelling E/B on Washington St towards NWH with a patient and 2 other Firefighters on board when a black Toyota Prius being operated by a Asian female bumped into his front passenger side. FF KING stated he tried to communicate with the women to pull into the hospital and meet him at the ER but upon turning she continued straight on Washington St. KING stated he did not get a license plate number. Minor paint transfer to front passenger side as well as damage to front passenger wheel guard. Patient information not included for privacy, see Walpole Fire dept trip sheet if needed.

On 03/08/2021 at 1249hrs I responded to 25 Armory St for a motor vehicle crash involving a city of Newton owned motor vehicle. Vehicle 1 described as city of Newton Fire Engine #4 (MA Fire Reg 444) backed into Vehicle 2 which was parked and unoccupied facing northbound on Armory St. As a result of the impact Vehicle 1 sustained little to no visible damage, Vehicle 2 sustained minor damage to the passenger side front bumper. No injuries were reported on scene. No tows were required. Photographs were taken and submitted to the NPD IT Bureau.

On Thursday May 13th, 2021 at approximately 1353 hours myself and several units responded 55 Chapel Street for a motor vehicle that struck a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. Chapel Street is a public way owned and maintained by the City of Newton. Upon arrival Fire and medical personnel were attempting to remove the pedestrian from underneath the vehicle as her right leg was stuck in the undercarriage. Fire and medics were able to successfully free the pedestrian from the vehicle and render medical attention. The pedestrian was identified as Beth Herr ( S58524067) and it was determined that she was also the owner of the vehicle involved. I spoke to witness Joe Shields who stated he was outside his house located at 56 Chapel Street when his attention was brought to a blue SUV that was unoccupied moving backwards down Chapel Street. Joe stated he first witnessed the operator of the motor vehicle pull up to the UPS drop box located on the sidewalk across the street from his house. He stated he saw the female operator step out of the vehicle and drop off a package. Next he saw the vehicle start to roll backwards and the female operator opened the drivers side door and attempted to pull the emergency break. He stated the vehicle overtook her and she fell to the ground with her foot stuck under the car. Joe states he heard her yell and saw her get dragged by the vehicle to the opposite side of the street where the vehicle was stopped by the curb. Joe stated he tried to assist the woman but she was yelling in pain and he did not want to cause more injury so he waited for help to arrive. Witness Robyn Preston stated she was walking on Chapel Street when she saw the blue SUV rolling backwards with no one inside the vehicle. She states she then saw a woman try to stop the vehicle but she got dragged underneath the vehicle to its final resting place. Robyn stated she stayed on scene until help arrived. I was able to speak with Beth inside the ambulance prior to being transported to the hospital. Beth was conscious and alert but in a considerable amount of pain and discomfort. I asked her what happened to which she stated she drove her blue Toyota Rav4 MA REG: 1FHM53 to the UPS drop box located in the area of 55 Chapel Street. She stated she thought she put the vehicle in park but must not have as it started to roll backwards when she got out of the car. She states she tried to stop the car by opening the drivers side door and attempting to pull the emergency break. The car over took her and she was dragged to the ground with her right foot stuck underneath the vehicle. She was dragged across the street where the vehicle was finally stopped by a curb. Beth sustained minor scrapes and road rash on various parts of her body and her right leg was injured. Fallon medics transported her to the Brigham & Womens Hospital to be treated. Her husband was contacted using Beth's cell phone and notified of the accident. Captain Boudreau and I were able to drive her vehicle to her residence at 50 Broadway Street, Newton. Her vehicle was left parked outside her house and her car keys were left with her husband who was inside the residence. The vehicle did not sustain any damage. Pictures of the scene were taken and submitted to the IT Bureau. I will be following up with Beth at the hospital to check on her status. On Friday May 14th at 0900 hours I spoke with Beth on the phone. She stated she is home from the hospital and doing much better. She states she has right leg pain and road rash from the accident.

Narrative:
I responded to 234 Langley Road for a two car M/V crash with entrapment. Upon arrival with other officers, Fire, and medics I saw the following. MV1 was flipped over on the road way and the operator was still in the vehicle, but the passenger was outside the vehicle. MV1 Hit a parked MV2 unoccupied. MV1 was traveling north on Langley Rd when MV1 swerved to its right and made contact with the left rear bumper of the MV 2. The impact of the two MV's caused MV1 to flip over and land on its roof. Based on my observation the scene indicated : day light, clear and there were no road defects. I took numerous pictures which were submitted to IT bureau so they can be attached to this report. Both MV1 driver and passenger were taken to Beth Israel Hospital in Boston. Tody's arrived on scene and took possession of both MV's. A canvas of the area resulted with one neighbor at 255 Langley who witnessed the accident. Mr. Vish Showdhary stated that he saw MV1 driving on Langley Rd when MV1 crashed into the parked MV. He stated that there were no other vehicles on the road and that MV1 drove right into the parked vehicle. The scene was cleaned and I went to Beth Israel Hospital to speak with the operator and passengers of MV1. The operator of MV1 stated that they have no recollection of what happened. She stated that she does not know how she hit the parked MV and that the last thing she remembers is being pulled out of the rolled over MV by the Fire Department. The passenger in MV1 stated that he does not remember how the crash occurred. He stated that he was unsure how MV1 crashed into MV2. He stated thathe was able to get out of the MV without a problem. After a review of the crash I am citing MV1 with the following. M.G.L 89/4A Marked lanes and M.G.L 90/24E Negligent Operation.(Citation # 1445881) Furthermore a request for immediate threat license revocation was filled and submitted to the RMV. The MV Citation was delivered to the operator of MV1 in hand at the Beth Israel Hospital.
