Narrative:
At approximately 1620HRs on Thursday, July, 25, 2019, I was dispatched to 444 Washington St. in reference to a truck vs tree motor vehicle crash. Upon arrival, I noted MV1 (MA Vet: 22098) had partially left the roadway and came to rest when it struck a city tree on the sidewalk. Additionally, I noted MV1 struck MV2 (MA Pass: 5CN826), which was parked on the road in front of the tree/aforementioned address. MV1 sustained damage to its front clip and was leaking fluids. MV2 sustained damage to its right rear bumper/quarter panel area. Upon speaking with the operator of MV1, an employee of Tony's Auto Services, he stated that he was test riding the vehicle after they made repairs to its power steering system. He stated that as he traveled eastbound on Watertown St. at approx 30MPH, the vehicle's brakes failed and he guided MV1 towards the tree in an attempt to stop it without injuring anyone. Todys Services took custody of MV1. Newton Fire cleaned up the leaking fluids and the operator of MV1 signed a patient refusal with Newton Medics. Newton Dispatch notified the city to see if they wished to inspect the tree which was still freestanding and appeared to only suffer minor damage. Tony's Auto Services notified the owner of MV1 and I notified the owner of MV2 of the crash. I submitted a picture of the tree to IT to be attached to this report. I cleared without further incident.

ON 11-24-21 AT APPROX. 1244HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT 33 AUSTIN ST. ( STAR MARKET LOT ) I SPOKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES HE PULLED INTO THE LOT AND STOPPED BECAUSE VEHICLE #2 WAS BACKING OUT OF A STALL. AS VEHICLE #2 GOT CLOSER THE DRIVER OF VEHICLE #1 BEEPED AND WAS THEN HIT IN THE FRONT RIGHT QTR. PANEL. AFTER CRASHING HE NOTICED THAT NO ONE WAS OPERATING VEHICLE #2. THE OWNER OF VEHICLE #2 HEARD THE CRASH BEHIND HIM AS HE WAS WALKING TOWARD STAR MARKET. WHEN THE OWNER NOTICED IT WAS HIS VEHICLE HE STOOD BY TO EXCHANGE INFORMATION. OWNER OF VEHICLE #2 STATED HE PARKED HIS CAR IN A STALL AND WALKED AWAY. HE STATED THE CAR IS 17 YEARS OLD AND THE EMERGENCY BRAKE MUST HAVE FAILED AND AS A RESULT HIS VEHICLE ROLLED BACKWARDS OUT OF THE STALL AND HIT VEHICLE #1. VEHICLE #1 HAD RIGHT FRONT QTR. PANEL DAMAGE. VEHICLE #2 HAD LEFT SIDE SCRAPES AND DENTS. BOTH PARTIES REPORTED NO INJURIES. BOTH PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES. CLEARED WITHOUT FURTHER INCIDENT.NEITHER VEHICLE WAS TOWED.

On Thursday 4/21/22 at approximately 805 hours while assigned to marked unit n498 I was dispatched to the intersection of Beacon and Chestnut Streets for a report of a motor vehicle crash into a utility pole. Upon arrival I observed two motor vehicles that had crashed in front of 619 Chestnut street and a utility pole also in front of 619 Chestnut Street with significant damage (pole# 85/75). While on scene I interviewed the operator, NUNEZ Ralphie, of Motor Vehicle 1, (MD Reg 5EP2140), who states he just dropped his Girlfriend off at a house on Beacon Street and turned right from Beacon Street onto Chestnut Street and began travelling southbound on Chestnut Street. NUNEZ states as he was driving he felt the steering wheel lock up and the vehicle was heading straight for the Utility Pole. He states he struck the Utility Pole and the force from the crash pushed his vehicle into the Northbound lane of traffic. After speaking with NUNEZ I spoke with the operator, MILLER Michael, of Motor vehicle #2, (Ma reg 1BTT36) who states he was travelling Northbound on Chestnut street when he observed Motor Vehicle#1 drive into the utility pole causing the vehicle to enter his lane of travel. MILLER states he attempted to veer to the right of the road to attempt to avoid the motor vehicle, but was unsuccessful and struck Motor Vehicle#1. Also on scene was, PISANO Christian, who witnessed the crash. See his attached witness statement given to me verbally on scene. 3 pictures were taken of the damage to the utility pole and forwarded to IT to be attached this crash report

IT WAS EVIDENT BY OBSERVING HIS VEHICLE THAT HIS FRONT TIRES WERE GOING IN OPPOSITE DIRECTIONS. POSSIBLY AN AXLE OR TIRE ROD HAD BROKEN. OWNER OF VEHICLE #2 STATED SHE LEFT HER CAR PARKED IN FRONT OF 133 BEAUMONT AVE. WHEN SHE HEARD THE CRASH SHE CAME TO THE SCENE. VEHICLE #1 HAD MINOR RIGHT FRONT FENDER DAMAGE AS WELL AS TIRE DISPLACEMENT AND WAS TOWED BY PRIVATE TOW. VEHICLE #2 HAD MINOR LEFT REAR FENDER DAMAGE BUT WAS STILL OPERATIONAL. ALL PARTIES REPORTED NO INJURIES. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES. CLEARED WITHOUT FURTHER INCIDENT.
