On 01/22/19 I responded to a three vehicle crash on Needham St. in front on #241- Marshalls Plaza. The operator of veh #1 stated she was driving due West on Needham St. when she looked right at a car waiting to pull out of the parking lot. When she looked back to Needham St., the traffic was stopped and she hit her brakes and snow slid covered her windshield. Operator of veh #1 never saw the impact, but veh #1 struck stopped veh #2 which was pushed into stopped veh #3. Moderate damage to veh #1 and 2 but no damage at all to veh #3. Operator of veh #2 said she was going "2-3 MPH " when she was pushed into veh #3. Operator of veh #3 claimed she was dizzy from the accident and signed a patient refusal from Ambulance #1. Engine #7 was also on scene assisting. Operator of veh #1 was cited in hand Mass citation #T1441211 for Ch 90/ Sec 13- failure to remove snow from vehicle . All vehicles were driven away from the scene.

On Saturday March 9, 2019, at approximately 0230 hours, I, Officer Brandon, responded to the Weston/Newton line on Commonwealth Ave to assist Weston PD with field sobriety. On arrival Weston PD had concluded field sobriety and determined that the party, Glenn Matot, was not under the influence. Weston PD then explained that they had been contacted by AAA stating that a single vehicle accident occurred in Weston's jurisdiction. After further review, Weston PD and I determined that the accident was in fact in Newton. I then spoke with the operator of MV1, Glenn Matot, who stated that he was on his way home from work traveling westbound on Commonwealth ave in front of the Charles River Canoe and Kayak when he became distracted by a text message. Mr. Matot further explained that while texting, his car drifted to the right causing him to drive up on the snow bank and up onto the guard rail before then falling off back into the road. MV1 sustained major damage to both the passenger side and undercarriage. Mr. Matot did not sustain any injuries and denied any medical attention. AAA arrived on scene and removed the vehicle from the road and Mr. Matot took an Uber home. Two pictures were taken of the damaged City of Newton guard rail and submitted to IT to be attached to the report.

On Monday 4/8/2019 at approx. 1706 hrs, while assigned to N-494, I responded to the intersection of Park St and Tremont St for a 2 car MVA. On scene both operators said they were uninjured. Operator of vehicle #1 states that he was traveling WB on Tremont St and attempted to take a left onto Park St while observing a green light. He said that he saw vehicle #2 not moving with the operator "looking down" and "probably looking at her phone." He then began to turn left but then he said that vehicle #2 suddenly moved forward and they collided. Operator of vehicle #1 believes he had his left turn signal activated. Operator of vehicle #2 states that she was on Park St, facing SB, about to turn left onto Tremont St. She said that prior to the accident that she was looking at her "Waze" app for directions on her phone just prior to the accident. She believes that she was moving "slowly" before the collision.

On 4/14/2019 at approx 0923Hrs while assigned to 497 I responded to the area of Commonwealth Av and Ash St for a report of a three car crash. upon arrival I observed three cars off to the side of the road. i spoke with the operator of ma Reg T6T, Robert MURPHY who stated he had stopped for the red light at Commonwealth and Ash St when the operator of TN Reg 2N37P1, Jennifer BURNELL failed to stop striking the operator of Ma Reg 8CL834 Evan GUIDRY who in turn than ran into the back of MURPHY. I spoke with BURNELL who stated she was lost and had been looking down onto her phone looking for a map when she ran into the back of GUIDRY. BURNELL cited for violation MGL90/13 distracted Operation , GUIDRY transported to NWH by Cataldo Ambulance for neck and head pain. GUIDRY and BURN ELLS vehicles towed by Todys. NFD responded and cleared roadway or debris and fluids.

On 7/20/19 at 23:24 hrs I responded to the area of 552 Commonwealth Avenue for a report of a MVA. Upon arrival it was determined MV#1 had struck two unoccupied parked vehicles in the eastbound parking shoulder. The operator of MV#1 stated he was on his phone while driving and when he picked his head up another vehicle coming westbound was in his lane so he swerved to avoid collision and struck the parked vehicles. The impact caused MV#1 to spin and came to final rest facing Westbound. All vehicles were towed from he scene by Tody's towing. The operator of MV#1 was transported to the hospital for potential injuries. The Operator of MV#1 is being issued MA Citation T1442692 for 90/24 OUI Drugs, 89/4a Marked Lane Violation and 90/13B Impeded Operation/Phone Use and MA Citation T1442693 for 90/24 Operating to Endanger.

On May 15, 2020 at approximately 22:26 hours Officer Stake and I responded to 163 Suffolk Rd for a hit and run that had just occurred. Upon arrival, we observed MV1,a black Cadillac Escalate with heavy rear end damage parked facing westbound in front of 163 Suffolk Rd. MV1 also was had damage to the passenger side due to it being pushed into a utility pole. MV1 was parked and unoccupied at the time of the crash. I spoke with the residents of 170 Suffolk Rd, who said that they heard the crash and went outside and saw a dark colored Jeep Wrangler leaving the scene at a high rate of speed. They described the Jeep Wrangler as a "newer" model, possibly a four door with a hard top. The residents of 163 Suffolk Rd have a camera system and will view the footage at a later time. Units checked the area for the Jeep Wrangler but were unable to locate it. Todys's responded and removed MV1 from the scene. **Update by Ofc. Hamann** At 0600 hours, Dispatch advised me that a male party that was involved in an accident last night (5/15/20). Brett Catlin stated that he was driving home from CVS last night and admitted to looking down at his phone and then had crashed. He stated after he crashed he drove to his home, which is 4 houses down. He stated he had called the owner of the vehicle and apologized and stated he was going to report it in the morning. I advised him that there will charges pending.****

On June 13, 2020 Officers responded to 129 Parker St for a report that a vehicle crashed into a parked car. Upon arrival we spoke with the operator of MV1 who was being evaluated by medics. The operator of MV1 declined further medical attention for leg and back pain. The operator of MV1 informed me that he was traveling southbound on Parker St. when he became distracted by his cell phone GPS falling on the floor. He went to retrieve the item and struck the rear of MV2 which was parked in front of 129 Parker St. The vehicle owner of MV2 was on scene and was advised of the situation. Due to the damage, both vehicles were towed from the cent by Tody's towing.

On Sunday 8/2/20 at approximately 701 hours while assigned to marked unit n499 I was dispatched to the rear of the Lifetime Center (300 Boylston Street) for a single vehicle motor vehicle crash. Upon arrival I observed a Black Honda CRV with significant front end damage next to a dumpster in the shoulder of Florence Street behind the Lifetime Center. Dumpster was placedt there for the ongoing construction at the Lifetime Center. I spoke with the operator of the CRV, LYSGAARD Ingrid, who states she was frustrated by all the noise being made by the construction workers. Ingrid states she drove to the site to take pictures of the workers whom she believed to be working in violation of Newton's city noise ordinance. She pulled over to the side of the road to take a picture with her cell phone, but didnt realize her foot came off the brake pedal causing her to strike the dumpster. Fire and Medics also responded to the crash and ingrid signed a patient refusal after being evaluated by the medics Tody's responded and towed the vehicle. I stayed on scene until the vehicle was removed and Ingid's son came to transport her hiome.

BOTH PARTIES WERE EXAMINED BY MEDICS AND SIGNED PATIENT REFUSAL. WHILE CRASHING THE VEHICLE MOTION TOOK THEM ONTO THE SIDEWALK AT THE CORNER OF HARRINGTON AND WASHINGTON. OWNER OF 45 HARRINGTON REPORTS DAMAGE TO HIS FRONT BUSHES AND A SMALL TREE. THERE WAS A CITY STREET SIGN THAT GOT BENT DURING THE CRASH. PICTURES TAKEN BY N522 ( OFF MCSWEENEY ) AND TURNED OVER TO I.T. BUREAU. BOTH PARTIES WERE ADVISED TO CONTACT THEIR INSURANCE AGENCIES.

I responded to the area of 63 Pierrepoint Rd for a report of an MVA single car crash into a tree. I responded to the area at approx. 1752 hrs 09/19/2020. I was advised by NPD Dispatch that the vehicle was unoccupied and that the vehicle has been there all day according to the anonymous reporting party.. When I arrived on location I observed MA Reg. 31542 off the roadway facing westbound directly in front of a tree. This was located directly in front of the residence of 63 Pierrepont Rd. The vehicle was unoccupied with heavy front end damage and front and side airbag deployment. The windows were left 2/3 of the way open and the cars ignition key was left on the driver's seat. The vehicle had collided into a city tree. There was debris from the vehicle at the base of the tree. The tree had visibly been hit but there was no damage to the tree by my observation. I contacted the vehicle owner Edward G. Murray Jr. He confirmed that he had knowledge of the crash. His son Edward G Murray III was the operator at the time of the crash. Murray III had a female passenger Diana Moreno 07/12/2004. I spoke with both Murray Jr. and Murray III via telephone. Murray III stated he was driving following his GPS and suddenly an animal which he believed to be a small dog suddenly ran in front of the car. Murray III swerved to avoid hitting the animal and drove off the road and collided into the tree. He stated the crash was at approx. 0950 on 09/19/2020. I also spoke with Diana Moreno (passenger), via telephone. She could not tell me what preceded the crash as she was looking at her cell phone just prior to the crash. Murray III and Moreno confirmed they were not injured as a result of the crash. Murray III contacted his father and informed him of the crash. Neither party contacted police. Murray Jr contacted AAA to respond advising the vehicle needed to be towed. Murray Jr. stated he was not further contacted by AAA and assumed that the vehicle was towed. Murray III and Moreno left the vehicle parked off the roadway as AAA was not immediately responding. I photographed the crash scene. The photo memory disk has been submitted to NPD IT bureau. The vehicle was towed by Tody's Service Inc.

Vehicle one was driving westbound on Washington St when he stated that he was looking at the buildings across from 327 Washington St. Vehicle one drifted into parked car, vehicle two. Vehicle two was parked and unoccupied in a parking spot in front of 327 Washington St. Vehicle two moved forward from the impact and struck vehicle three. It should be noted that vehicle two already had extensive damage to the front end of the vehicle prior to the crash holding the bumper intact with duck tape and clear tape. Vehicle three had minor damage to the rear end bumper. No injuries were reported on scene.

On 12/10/20 at approximately 13:23 myself along with other Police, Fire and Ambulance units, responded to the intersection of Chestnut St. and Elliot St. for a two vehicle crash. Upon arrival I spoke with the operator of veh#1 who stated she was driving on Elliot St. due West when she proceeded through the intersection and was struck on her passenger side by veh #2. The operators 2 young children were in back of her vehicle in child seats. Her son was behind the passenger and her daughter was behind her. Both front and side airbags were deployed in veh #1. All three people in the vehicle were taken to Newton-Wellesley Hospital to be checked out as a precaution. The operator stated she had pain in her right wrist. The operator of veh#2 stated he was traveling due South on Chestnut St. when he looked down as he approached the intersection. The operator stated he was prepared to take a right onto Elliot St. but when he looked up he had already entered the intersection and struck veh #1. The air bags went off in veh #2 as the operator had cuts on his left hand . He was checked out by the Medics and signed a patient refusal. The operator of veh #2 had left the scene after I initally spoke with him but while I was attending to the occupants of veh #1. Because of this I was unable to issue him a citation on scene. I mailed him Mass. Citation #T2080850 for Ch 89/ Sec 9 Failure to obey the Red Light. Both vehicles sustained heavy damage and were both towed by Bobby and David from Tody's towing.

On February 1st, 2021 at approximately 07:53 hours while assigned to N491 I responded to MV crash in front of #41 Bridge St. On my arrival I observed a large white box style truck crashed into a gray SUV both vehicles with extensive damage blocking the street completely. I first identified the operator of the box truck, Maurizio Didonna. The truck belonged to Russo's & Sons out of Pleasant St Watertown Ma. Mr Didonna reported driving S/B on Bridge St when he temporarily got distracted before crashing into a parked vehicle in front of #41 Bridge St. The owner of the parked vehicle was not on scene but a male construction worker who was building a new house at #41 Bridge St identified the vehicles owner as Jose Ramirez. He was able to make contact with him and informed him about the accident. Both vehicles were towed from the scene. One by Todys. Russo's & Sons truck was towed by Pro Tech out of Watertown. Mr Didonna had a minor injury but refused any medical treatment. Note:The parked SUV had a lot of construction equipment in the rear but was removed by the construction workers from the job site at #41 Bridge St. The parked car was affiliated with the construction site. The vehicles owner was notified to where his vehicle was being towed to. .

MV1 was pulling out of Lothrop St to turn left on Crafts St to head Eastbound. He believes he did not turn the steering wheel hard enough and consequently collided with the utility pole across the street from Lothrop St. MV1 operator, Dylan Newman also admits to texting on his cellular device during driving. Dylan was given MA Uniform Citation T2080930 in hand for 90/8M Texting While Driving. No injuries were reported, a verbal refusal was given by Dylan's mother to the medics and the vehicle was towed by Tody's. The vehicle showed heavy damage to all front areas. All airbags were deployed during the crash. John Reiner, a witness, stopped to give a statement on scene. John stated that he was driving Eastbound on Crafts St at Watertown St when he saw a van pull out of Lothrop St to turn left and then all of the sudden stop. Once John got closer to the scene, he discovered that the van had actually collided with a pole and went to assist Dylan out of the vehicle. John said that Dylan was immediately looking for his cellular phone when he was getting out of the car and told John that he was on his phone when the crash happened.

A witness who was in Vehicle #3 behind Vehicle #1 stated they were stopped at a red light on hammond st. and when the light turned green she witnessed Vehicle #2 blow the red light and was struck by Vehicle #1. The witness stated she and Vehicle #1 had a solid green light at the time they entered the intersection. At the time of the crash there were four parties inside Vehicle #2. After the crash Vehicle #2 left the scene after they thought Vehicle #1 had left and dropped off two passengers before returning to the scene of the accident. I spoke with the two passengers who were not present at the scene who both stated they were inside Vehicle #2 at the time of the crash and Vehicle #2 had a clear red light which they failed to stop for. One of these passengers was complaining of a head injury and was currently at a hospital being evaluated. Vehicle #1 had moderate damage to the front of the vehicle and was able to be driven away. Vehicle #2 had moderate damage to the right rear side and was able to be driven away. All drivers and passengers on scene were asked if they were injured which they stated they were not and declined any medical attention. Due to multiple witnesses stating Vehicle #2 failed to stop for the red light, a citation was issued to the driver for MGL 89/9 Fail to stop/red light on MA Uniform Citation T2080447

MV1(Corona) was traveling southbound on Centre St through the intersection of Centre St at Commonwealth Ave. MV2 (VAZ) was traveling eastbound on Commonwealth Ave through the intersection of Centre St at Commonwealth Ave. Both vehicles collided at the intersection. The operator of MV1 stated she did not know what happened at the time of the crash and might have gone through the red light, the operator of MV2 stated the same. The operator of MV2 stated he spoke with the operator of MV1 and she stated to him that she was looking at her GPS. There was heavy front end damage to MV2 and passenger front and side damage to MV1. A witness (Danielle Hite), was two car lengths behind MV2 stated that their line of traffic had a green light and that she saw MV1 drive through a red light causing the collision. Fallon Medics transported the passenger of MV2 to Beth Israel as she was lying in the road unconscious and the medics obtained a patient refusal from the operator of MV1 and the operator of MV2. On August 15th, 2021 at approximately 09:55 hours I along with NFD and Fallon Ambulance responded to a report of a single car crash into a stone wall at #144 Chapel St. On my arrival I observed a 2006 Brown Honda Accord, MA REG. 93ES83 crashed into a wall at #144 Chapel St. The operator was located outside the vehicle sitting in a chair inside the parking lot adjacent to #144 Chapel ST. She identified herself as Cleausa Cobden, resident of #145 Chapel St,(across the street from accident scene). She stated she had just turned onto Chapel St heading S/B from Watertown St, heading home when she got distracted for a moment taking her eyes briefly off the road and the next thing she remembers was crashing into the wall. She sustained minor injuries to her ankle and some minor cuts to her arm. She later signed a patient refusal for Fallon Ambulance. Tody responded and towed the vehicle. The property owner for the wall was later determined to be the property of the US Postal Office, #326 Watertown St. The wall sustained no damage from the impact that I could see.

On Sunday, August 22, 2021, I was contacted by Sgt. M. Wade of the Newton Police Traffic Bureau at 0032 hours. Sgt. Wade stated a serious motor vehicle crash occurred on Washington Street in Newton and it involved a City of Newton marked police cruiser (N494). Sgt. Wade stated Officer Katelyn Pohlman and the operator of the other vehicle involved were transported to the hospital with potential serious injuries. I was requested by Sgt. Wade to respond to the scene from my residence and investigate the crash further. On arrival at the scene, I observed the two vehicles involved in the crash to be a 2015 Ford Explorer (MA: 586RT7) and Newton Police marked unit N494 (MA MVN: MP494B). These vehicles were observed disabled on the roadway in front of 612 Washington Street, Newton. There was a large debris field comprised of various parts from both vehicles on the roadway and in the parking lot of 612 Washington Street. The road surface at the time of my arrival was wet and there was light rain falling. Sgt. Wade reported that at the time of the crash the road was completely dry and it had not started to rain yet. Washington Street is a public way maintained by the City of Newton. The portion of Washington Street where the crash took place supports two- way travel, Eastbound and Westbound. There are two travel lanes in both directions. Travel direction is divided by a double yellow line in the middle of the roadway. The area is well lit and features numerous light poles illuminating the roadway. The closest intersection to this location is Crafts Street just west of the crash location. MV1 (MA: 586RT7) had substantial front-end damage, especially to its front passenger side and was inoperable. I observed this vehicle at its final rest on the roadway with its front end facing Northbound on Washington Street just over the double yellow line in the middle of the roadway. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV1 was removed from the roadway by Tody's Towing. MV2 (MA MVN: MP494B) had substantial front end damage and was inoperable. The impact from the collision caused the vehicle to spin uncontrolled and the final rest, with the front end of the vehicle facing the front entrance to the 7-11 (S). The vehicle's final rest was on the sidewalk in front of 612 Washington Street. The front and side impact airbags had deployed inside the vehicle as a result of the collision. I did not observe anything hanging inside the vehicle that would have obstructed the view of the operator prior to the collision. MV2 was removed from the roadway by Tody's Towing. The Officer involved in the crash is Officer Katelyn Pohlman. Officer Pohlman stated she was operating Newton Police marked unit N494 on Washington Street (E) past Crafts Street. Officer Pohlman stated as she was operating her vehicle past 612 Washington Street (7-11), a vehicle in the westbound lane of Washington Street abruptly crossed the double yellow line in the roadway and entered her travel lane head-on. Officer Pohlman stated she did not have an opportunity to react to the vehicle in front of her and the front passenger side of MV1 crashed into the front end of her vehicle (MV2). Officer Pohlman sustained injuries to her left forehead area and both of her arms. Officer Pohlman was later transported to Brigham and Women's Hospital for evaluation and was released a short time later. It was reported to me by numerous Officers on scene that Officer Pohlman exited her cruiser after the crash and rather than tend to her own injuries, she focused her efforts on rendering aid to the operator of the other vehicle involved. Officer Pohlman found the operator of the other vehicle lying on the floor of the front passenger seat under the dash. Officer Pohlman rendered aid to the operator while waiting for Newton Medics to arrive. Officer Pohlman was also assisted in rendering aid to the other operator by Officer Conary and Officer Larson. Officer Larson reported that while interacting with the operator of MV1, the operator repeated numerous times, "Is she ok? Is she ok?". Officer Larson also stated the operator said to him "I feel bad because I was texting on my phone". Officer Larson stated the operator of MV1 works for the Department of Corrections Special Operations Unit and has Department issues equipment inside the vehicle. The following items were placed into Support Services, locker 9 for safekeeping by Officer Larson: 1 Gray Pelican Gun Case, 1 Black Gun Belt, 1 Taser, 1 Can OC Spray, 15 (50) Round Cases Of Ammunition. The operator of MV1 (MA: 586RT7) was identified as Mr. David Cafarelli (S16965940). Mr. Cafarelli was transported to Newton Wellesley Hospital for a potential head injury before my arrival. I spoke with Mr. Cafarelli at Newton Wellesley Hospital in the Emergency Room with Sgt. Wade. Mr. Cafarelli's mother and father were present when we spoke with him. Mr. Cafarelli stated he was out with friends before the crash and was driving to the 7-11 on Washington Street. Mr. Cafarelli admitted that before the crash, he had his cell phone in his hand while he was operating his vehicle. Mr. Cafarelli stated he does not remember if he was texting or changing the music on his phone at the time of the crash. Mr. Cafarelli stated he looked up and was unable to avoid the vehicle (N494) directly in front of him. Mr. Cafarelli stated he also does not believe he applied his brakes before the collision. When we spoke with Mr. Cafarelli in the Emergency Room, he was still waiting for test results to determine the severity of his injuries. On Monday, August 23, 2021, Sgt. Wade responded to 612 Washington Street, Newton to retrieve surveillance footage from the 7-11 of the crash. 7-11 stated their outside surveillance system was not operating correctly, but they were able to capture some of the incident from a camera inside the store. I observed this footage. MV1 is observed traveling on Washington Street (W) in the westbound travel lane in front of the storefront. MV1's driver side is then observed traveling over the double yellow line in the middle of the roadway and into the eastbound lane of Washington Street. A head on collision ensues and MV1 can be seen rotating counterclockwise in the roadway backwards to its final rest location on Washington Street. MV2 is then observed at its final rest position on the sidewalk facing southbound in front of the 7-11 storefront. After completing my investigation, I have surmised the following. Mr. Cafarelli was operating his 2015 Ford Explorer (MA: 586RT7) in the area of 612 Washington Street, Newton. Mr. Cafarelli was holding his cell phone in his hand while operating his vehicle. Mr. Cafarelli became distracted while he was operating his vehicle due to his attention being focused on his cell phone. Mr. Cafarelli traveled over the double yellow line from the westbound lane of Washington Street and entered the eastbound lane of Washington Street. Newton Police marked unit N494 was traveling on Washington Street (E) in the area of 612 Washington Street at this time. Mr. Cafarelli's vehicle crashed into the front end of Newton Police marked unit N494. Mr. Cafarelli was not wearing a seatbelt and as a result of the impact from the crash, he was ejected from the driver seat and his final rest inside the vehicle was in the flood of the front passenger seat. As a result of my investigation, Mr. Cafarelli was issued Massachusetts Uniform Citation for Chapter 90, Section 13B (Hands-Free Violation), Chapter 89, Section 4a (Marked Lanes Violation), and Chapter 90, Section 13a (Seat Belt Violation). Photos from the crash scene and the surveillance media from 7-11 capturing the crash were submitted to the IT Bureau.

On September 14th, 2021 at approximately 09:22 hours while working N491 I responded to the area of #227 California St for a report of a MV crash. On my arrival I located both involved vehicles. Vehicle #1 was a 2000 Toyota Avalon XL, Ct reg AK76968, operated by a Molly Parrott. She stated she was going W/B on California St when she came to a flashing yellow light and slowed/stopped because she thought it was about to turn red. A few seconds later she was rear ended by vehicle #2. Vehicle #2 was a 2011 Brown Chevrolet. Malibu, Ma reg. 9JB522, operated by a Dion Tribbett. He reported travelling W/B on California St when he had to go around onto the wrong side of the road due to a truck blocking the west bound side of California St. Due to that distraction of him looking at the truck as he went around it he didn't see that vehicle #1 had stopped just a little further down the street. His vehicle then drove into the rear of vehicle #1 that had stopped by Jasset St/ Forte Park entrance. Both operators sustained minor injuries and both refused to be transported to the hospital. Vehicle #2 was towed by Todys.

Pedestrian/witness stated she was crossing in the crosswalk when the collision occurred. Pedestrian was not injured.

Upon arrival, vehicle 1 was rolled over and facing Eastbound in the Westbound lane. There was extensive damage all over the vehicle and it was leaking fluids. It appears that vehicle 1 was traveling Westbound on Commonwealth Ave and struck a tree and then crashed into the guard rail causing it to roll over. There was damage to a city tree and damage to the guard rail. Operator 1 stated (through translation from Officer Acuna), he was traveling Eastbound on Commonwealth Ave at approximately 25 MPH when he was looking at his GPS and his brakes gave out causing him to crash. When asked again, his story changed and he was not sure what direction he was traveling in. Newton Fire was able to retrieve operator 1's cell phone in the vehicle which was on a loop playing videos on instagram. Operator 1 was transported to Beth Israel Hospital by Fallon Ambulance. Tody's towed vehicle 1 and pictures were taken of the damage to city property and will be submitted to the IT Bureau to be attached to this report. At approximately 1425HRs on Wednesday, November 24, 2021, I was dispatched to a motor vehicle crash, which took place on Nahanton St., between number 293 and Winchester St. Upon arrival, I observed MV1 to be facing eastbound in the westbound travel lane. It had its passenger's side front bumper contacting the passenger's side front bumper of MV3 and MV2 was off of the roadway, to the left of MV1's front driver's side. MV1: I noted damage to its front driver's side bumper, head light, fender, hood area and flat tire. On MV1's front passenger's side, I noted damage to its bumper, head light, fender and hood area. MV2: I noted damage to its front driver's side bumper, head light, fender, hood area and flat tire. MV3: I noted damage to its front passenger's side bumper. Upon speaking with the operator of MV1, they stated they were traveling eastbound on Nahanton St. At this time, they began to retrieve something from their bag on the passenger's seat. They then crossed MV 1 over the center yellow divider lines and struck MV2 and MV3. Upon speaking with the operator of MV2, they stated the last thing they remember is that they were traveling westbound on Nahanton St. and then that their vehicle was traveling backwards; They do not remember the crash. Upon speaking with the operator of MV3, they stated they were traveling westbound on Nahanton St. when MV1 crossed over the center yellow divider lines at a high rate of speed. They observe MV1 crash into MV2, pushing it backwards and to the side. MV1 then continued forward and impacted MV3. The operator of MV1 received a ride from their father, after signing a patient refusal. Newton Medics transported the operator of MV2 to the hospital. And the operator of MV3 was able to drive their vehicle away, after signing a patient refusal. Both MV1 and MV2 were removed by the scene by Todys Services (NPD Towed Motor Vehicle Form Filled). I cleared without further incident.

On Thursday 2/24/22 at approximately 1130 hours while assigned to marked unit n498 I was dispatched to the area of 1985 Beacon Street for a 2 car motor vehicle crash with injuries. Upon arrival I spoke to the operator, CECCHINI Michael, of a UPS delivery truck. CECCHINI states he was travelling westbound on Beacon Street and activated his right turn signal, began to slowly pull to the right, activate his 4 way emergency hazard lights and stop in the shoulder/breakdown lane in front of 1985 Beacon Street to make a delivery at that address. Upon stopping he was struck in the rear by motor vehicle #2. After speaking with CECCHINI I spoke to the operator, CHABOT Shane, of motor vehicle #2. CHABOT was in the back of the ambulance being prepared for transport to Newton Wellesley Hospital. CHABOT states he was travelling Westbound on Beacon Street and was looking down at his GPS on his cell phone in the cup holder of his motor vehicle and adjusting his radio when he struck the rear of the UPS truck on the side of the road. A Witness, CERRA Rob, spoke to Sgt. Poutas and stated he witnessed the pickup truck travelling westbound on Beacon Street and observed the vehicle to veer to the right and strike the back of the parked UPS truck. Based on the statements given to me by the operator of motor vehicle #2 , CHABOT Shane, he is being issued Ma Uniform Citation T2012914 for C90 S13B using an electrical device while operating a motor vehicle and C89 S4A Marked Lanes Violation. The citation was mailed to CHABOT's address as I was unable to issue it in hand prior to him being transported to the hospital.

Operator of Vehicle 1 states he was travelling Northbound on Ferncroft Rd when the operator became briefly distracted by his phone. Vehicle 1 then struck the curb and a tree on the West side of the road in front of #64 Ferncroft Rd, causing extensive damage to the front of the vehicle and airbag deployment. The weather at the time of the crash was clear and sunny. The road surface was dry. The operator of MV1, Ms. Maria Lima Marte (S26852151) stated she was operating her 2009 Nissan Versa (MA: 8TL115) on Centre Street (N) towards Commonwealth Avenue. Ms. Lima Marte stated traffic was stopped at a red light when MV2 crashed into the rear of her vehicle. I observed damage to the rear bumper of MV1. Ms. Lime Marte and her passenger, Ms. Adianel Diaz Lima reported no injuries. The operator of MV2, Mr. Travis Roth (SA4230808) stated he was operating his 2019 Subaru Crosstrek (MA: 1WCL95) on Centre Street (N) towards Commonwealth Avenue. Mr. Roth stated he wasn't paying attention to the roadway and was focusing on the GPS in his vehicle. Mr. Roth stated the front end of his vehicle crashed into the rear of MV1. I observed damage to the front bumper area of MV2. Mr. Roth reported no injuries.

Ms. Fabrizio stated while stopped at the light, MV2 crashed into the rear of her vehicle. I observed damage to the rear bumper area of MV1. Mr. Kirby-Thomas stated while operating his vehicle he became distracted by the GPS in his vehicle and his front end crashed into the rear of MV1. I observed damage to the front bumper area of MV2. Mr. Kirby-Thomas reported no injuries. Mr. Kirby-Thomas was provided with Massachusetts Uniform Citation 519268AB in hand for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care).
