Narrative:
The operator of MV#1 stated that she was waiting to exit NWH at the east exit/entrance. When her light turned green she proceeded to enter Washington St and her MV was struck by MV#2, who was traveling south on Washington St. The operator of MV#2 stated that she was traveling south on Washington St approaching NWH and had a green light to proceed thru the intersection at the east exit/entrance. An independent witness stated that she was in a MV directly behind MV#1 at the east exit when she observed their traffic light turn green and both her and MV#1 proceeded into the intersection. The witness then observed MV#2 traveling south on Washington St enter the intersection crashing into MV#1 while their light was still green. Operator #2 cited for a red light violation.

On Wednesday, February 27, 2019 while assigned to Traffic unit N525, I responded to the area of 843 Beacon Street, Newton for a report of a 2 car motor vehicle accident. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1, Omer Chaudhry (NY: 122909834). Chaudhry stated he as operating his 2015 blue Mazda 3 (NY Reg: GVJ1096) Eastbound on Beacon Street towards Centre Street. Chaudhry stated he observed MV2 exiting a parking spot on his right. As MV2 attempted to merge into traffic it's front driver side crashed into the passenger side of his vehicle. I observed damage along the passenger side of Chaudhry's vehicle from the rear passenger tire to the front passenger bumper area. Chaudhry reported no injuries at this time. I spoke with the operator of MV2, Elizabeth Libert (S75526057). Libert stated she was attempting to pulled out of a marked parking spot (E) on Beacon Street when the front driver side of her 2017 gray Mercedes E400 (MA: 1YT269) crashed into the passenger side of MV1. I observed damage to the front driver side tire area of MV2. Libert reported no injuries at this time. Libert's vehicle was removed from the roadway by Tody's towing. Libert was cited with Massachusetts Uniform Citation T1269154 for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care).

The operator of MV#2 stated that he was attempting to exit the parking area at #2102 Comm Ave when the traffic in the right hand lane on Comm Ave east bound had stopped to allow him to enter. MV#2 the entered the left hand lane of Comm Ave east bound and crash occurred with MV#1. No injuries, air bags deployed in MV#1 and both MV's towed by Tody's.

On Sunday 3/17/19 at approximately 1606 while assigned to marked unit n499 I was dispatched to the area of 89 Wiswall Rd. for a report of a two car motor vehicle crash with no injuries. Upon arrival I spoke with WONG Cheung, the operator of motor vehicle #1. WONG states that he was traveling southbound on Wiswall Rd. from Dedham Street when he was attempting to pull into his driveway at 89 Wiswall Rd. WONG further states that as he attempted to turn Motor vehicle #2 passed him on the inside causing him to strike motor vehicle#2 on the drivers side. WONG stated to me that he used his right turning signal. Operator of motor vehicle#2, CAI Wei, states that he was traveling behind motor vehicle#1 and he thought that motor vehicle #1 was taking a left into 88 Wiswall Rd. He states that Motor vehicle #1 made movement to the left toward the center yellow double line as if he was turning left so he attempted to pass motor vehicle#1 when it suddenly turned right into 89 Wiswall Rd. causing him to strike the front passenger side of the motor vehicle. CAI states that motor vehicle #1 never used a turn signal.

On Thursday, April 11, 2019 while assigned to Traffic unit N525, I responded to the area of 1302 Washington Street, Newton for a report of a motor vehicle accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Washington Street is a public way maintained by the City of Newton. The operator of MV1, Jacqueline Dupre (S29256883), stated she was parked in front of 1302 Washington Street (E) in her 2018 Honda CRV (MA: 5054RF). Dupre stated she was attempting to exit her street parking spot and did not see any vehicles traveling towards her as she entered Washington Street. Dupre stated MV2's passenger side hit her front driver side area as she was attempting to enter the lane. I observed minor damage to the front driver side quarter panel/headlight area. Dupre reported no injuries at this time. The operator of MV2, Donnell Stoute (S53698210), stated he was operating a 2017 Porsche Macan (MA: 7ZC447) East on Washington Street. Stoute stated as he was traveling by 1302 Washington Street, MV1 pulled out of her parking spot and hit the passenger side of the vehicle he was operating. I observed moderate damage to the passenger side of MV2. Stoute reported no injuries at this time.

The owner of MV#1 states that he returned to his unoccupied MV to find a note from a witness stating they observed another MV strike his MV while they were attempting to park in the parking spot to the left of MV#1. Witness identified MV#2 as Ma Reg 1705XY, a Honda CRV. Witness states he observed an older W/M operating Ma. Reg. 1705XY (MV#2) attempt to park in a parking spot while striking MV#1 and then quickly exit the spot. Witness feels confident operator of MV#2 knew he struck MV#1. I then spoke with operator/owner of MV#2 who admitted he was operating MV#2 during minor crash, but didn't think it was significant so he left the area. Operator #2 cited for leaving the scene of a MV crash.

Operator 1 stated he was stopped at the red light Northbound on Langley Rd. at the intersection of Beacon St. When the light turned green he proceeded forward and vehicle 2 pulled out of a metered spot striking vehicle 1. Operator 2 stated that she was parked facing Northbound on Langley Rd. Operator 2 thought vehicle 1 was letting her pull out of her spot. She proceeded forward into the travel lane and the two vehicles collided. 2 witnesses to the accident stated that vehicle 2 pulled out of the spot while vehicle 1 proceeded forward. Both witnesses believe that due to the height of the truck vehicle 1 couldn't see vehicle 2 pulling out. Vehicle 1 sustained minor damage, vehicle 2 sustained moderate damage, there were no tows. There were no injuries.

On Wednesday, May 29, 2019 while assigned to Traffic unit N525, I responded to the area of Centre Street and Washington Street, Newton for a report of a motor vehicle crash involving a school bus. The weather at the time of the crash was overcast and the road surface was dry. Centre Street and Washington Street are both public ways maintained by the City of Newton. The operator of MV1, Norma Cedeno (S60008138), stated she was operating a 2016 Dodge Caravan (MA SPN: 104893) around the Washington Street/Centre Street rotary. Cedeno stated as she was traveling in the left turn lane on Centre Street, she attempted to travel straight (N) through the intersection at Washington Street towards 255 Washington Street. Cedeno stated the front passenger side of her vehicle crashed into the driver side of MV2. MV2 was attempted to continue left around the rotary and was located on the passenger side of her vehicle. Cedeno reported no injuries. I observed minor damage to the front passenger side tire area of MV1. Cendeno's passenger, Cristina Velez (S04276767), reported not injuries. The operator of MV2, Mary Sanker (S62890912), stated she was operating her 2016 BMW 328XI around the Washington Street/Centre Street rotary. Sanker stated as she was continuing through the rotary in her lane, the vehicle traveling in the lane to her left (MV2) entered her travel lane in an attempt to travel straight towards 255 Washington Street and crashed into the driver side of her vehicle. I observed moderate damage to the front and rear driver side doors of MV1. Sanker reported no injuries. The operator of MV1 was cited for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care)

When he pulled out, he collided with MV1. Witness stated she did not observe the crash as it happened but it appeared that MV2 was perpendicular to the roadway as if he was attempting a U-turn. No injuries were reported and no tows were needed.

On 07/16/19 at approximately 09:38 I responded to a two vehicle crash on Needham St. outside of #141. The Operator of Veh #1 stated she was turning left on Needham St when the South bound traffic stopped to left her turn. As she turned, Veh#2 drove South down the multi-turn lane and they collided nearly head on. Heavy damage and air bag deployment to Veh #1. The operator was checked out by Medics and signed a patient refusal. The Operator of Veh #2 stated she was driving down the multi-turn lane South on Needham St.. She was preparing to take a left onto Jaconnet St when Veh #1 pulled out of parking lot and collided with her vehicle. I observed moderate to heavy damage to Veh #2 and noted that the vehicle didn't have its directional on. The operator of Veh #2 complained to left leg pain and was transported to Newton-Wellesley Hospital By Ambulance #1. Both vehicles were towed by Tody's Towing. At the conclusion of my investigation it appears as if both vehicles were at fault during this crash; Veh#1 for not yielding to right of way and Veh #2 for traveling too soon in a turn lane, no directional and driving too fast for the heavy traffic conditions. Both operators were given verbal warnings and there will be no citations issued at this time.

The operator of MV#1 stated that he was traveling south on Grove St in heavy traffic when he stopped in front of #275 Grove St to let a MV enter the roadway. Operator #1 stated that his MV was then rear ended by MV#2. There did not appear to be any damage to MV#1 and there was a strong language barrier when speaking with the operator. The operator of MV#2 had left he scene prior to my arrival and this concerned operator #1. I then made contact with the owner of MV#2 who identified himself as the operator during a very minor crash on the date and time in question. Operator #2 stated he was also traveling south on Grove St directly behind MV#1 when he made slight contact with MV#1. Operator #2 stated he got of his MV, observed that there was no damage to either MV and then left the scene after speak with operator #1. I advised both parties that I would file a MV crash report and that they should notify their insurance provider. No injuries, no tows.

On 8/28/2019 at approx 1058hrs while assigned to 497 I responded to the area of 2014 Washington St for a report of a 2 car crash w/o injuries. Upon arrival I observed the vehicles on opposite sides of Washington St with NFD Lt Bourgeois standing by with his emergency lights activated, Lt Bourgeois stated both operators were shaken up but not injured. I spoke with the operator of Ma Reg 1XM680 Elizabeth CHISOLM who stated she was attempting to take a left onto Washington St, a vehicle in the right hand eastbound lane was turning right into the parking lot, CHISOLM stated she did not see Ma Reg EV3452 being operated by Julien GRANT in the left eastbound travel lane until it was too late colliding with him. I spoke with GRANT who stated he was traveling EB in the left travel lane of Washington St when struck by CHISOLM exiting the hospital.

On Sunday 11/24/19 at approximately 1730 hours while working in marked unit n499 I was dispatched to the parking lot on the rt9 side of 225 Boylston Street (Bloomingdales) for a report of a minor motor vehicle crash. Upon arrival I spoke with the operator of motor vehicle 2, GIRARD Kathleen, who states that she was driving from the 2nd aisle of parking spots into the main travel lane to attempt to exit the mall. When she attempted to turn left she didnt see motor vehicle 1 already in the lane of traffic striking the side of the vehicle. After speaking with GIRARD I spoke with the operator of motor vehicle 1, SONG Guang min. SONG did not speak english, but he corroborated what GIRARD had stated by using a translation app on his cell phone. GIRARD had damage to the front of her vehicle and SONG had damage to the drivers side of his vehicle. Both vehicles were still operable and were driven from the scene. Neither party was injured

Operator #2 states that he was attempting to turn left out of the Arnow Campus Center parking garage and did not see vehicle #1 approaching from the right. It should be noted that the physical address of the parking garage appears to be 131 Woodland Rd, however the collision occurred on the Seminary Ave side. All parties declined medical attention and no tows were needed. Vehicle #1 sustained significant damage along the driver's side doors. Vehicle #2 has moderate front end damage. Operator #2 was issued Ma citation T1268432 in hand for Failure to Yield to Oncoming Traffic. 131 Woodland Rd will be the address used in documenting this crash.

I spoke with the operator of the pick up, Leonardo TAPIA who stated he had been travelling WB on Commonwealth Av when the Ford Focus pulled out in front of him causing the collision. I spoke to the operator of the Ford Focus John KELLY who stated he was taking a left out of the parking lot of 2102 Commonwealth Av, traffic in the EB lanes stopped to let him out, as he was entering the WB lane he collided with the pick up. Ford Focus towed by Todys, both operators signed refusals with Cataldo.

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

Both were transported to NWH for further evaluation. #1 sustained heavy damage to the front end with multiple airbag deployment. #2 sustained heavy damage to the driver's side and front end. Both were towed by Tody's to their storage yard.

On July 30, 2020, while on patrol in marked unit N525, I responded to a 2 car MVA in front of 380 Watertown Street, Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. Watertown Street is a public way maintained by the City of Newton. The operator of MV1, Joseph Vara (S27929046), stated he was operating his 2006 Ford dump truck (MA CON: V42297) East on Watertown Street. Vara stated as he was traveling past 380 Watertown Street, MV2 attempted to merge into his lane from a parking spot on the roadway it's front driver side crashed into the rear passenger side tire area of his vehicle. Vara reported no injuries. I observed damage to the outer rear passenger tire of MV1. The operator of MV2, Willis Herlihy (S63431207), stated his vehicle, a 2010 Toyota Avalon (MA: AG2122) was parked in front of 380 Watertown Street. Herlihy stated as he was exiting the parking spot, he looked back and did not see any traffic. Herlihy stated as he merged into the travel lane the front passenger side of his vehicle crashed into the rear driver side tire area of MV1. Herlihy reported no injuries. I observed damage to the front end of MV2. MV2 was removed from the roadway by Tody's Towing.

I spoke with Patricia who stated she was travelling Eastbound on Washington St when the other vehicle pulled out across her traffic lane. Patricia had a passport only and admitted she had no license in the U.S. or in Brazil. Patricia issued criminal application citation for violation of MGL 90/10 Unlicensed Operation , her car was towed by Todys Towing. Emma stated she had struck her head but signed a refusal with Cataldo Ambulance.

The operator of MV#1 stated that she entered Melrose St from Commonwealth Ave and after stopping to let another MV pass south bound, she proceeded north on a very congested Melrose St. The operator of MV#1 then noticed that something was wrong with her MV and pulled to the side of the road. Operator was then approached by the operator of MV#2 and informed that she had struck his MV. The operator of MV#2 stated that he was attempting to pull to the side of Melrose St (#230) because it was very congested with parked MV's on both sides, when his MV was side swiped (same direction) by MV#1. MV#2 was then pushed into MV#3, which was parked and unoccupied at the time of the crash. The operator of MV#2, a witness and an uninvolved resident all stated to me that the operator of MV#1 had attempted to leave the scene and lie about what MV actually struck the damaged MV's. I discussed this with operator #1 who stated she was unaware she had struck another MV until she noticed issues with her car and at first assumed it must have been the MV traveling in front of her. There were no reported injuries and MV#1 was towed due to a front broken axel.

On Tuesday, December 01, 2020, while assigned to Traffic unit N525, I responded to the rear parking lot of 200 Boylston Street, Newton for a report of a motor vehicle crash. I spoke with the reporting party and operator of MV1, Sungmi Lee (S54407875). Ms. Lee stated she was operating her 2017 Hyundai Santa Fe (MA: 6KR971) Eastbound through the second floor of the Wegmans parking garage, a public right of way. The area she was traveling through is designated for one was travel with a vehicle ramp to her right that allows access for vehicle to go up or come down from the third floor. Ms. Lee stated MV2 (2018 Jeep Compass, CT: AR52330), traveled down the ramp from the third floor and did not stop at the stop sign at the entrance to the second floor. Ms. Lee stated the front end of MV2 crashed into the passenger side of her vehicle. I observed damage to the passenger side area of MV1. Ms. Lee stated her neck was sore from the crash but declined having EMS respond to evaluate her. Ms. Lee stated both vehicles pulled over to exchange information after the crash. Ms. Lee stated the operator of the other vehicle, Avery Adams, stated to her she was at fault for the crash. Ms. Lee stated she noticed the license Ms. Adams showed her was expired. Ms. Adams left the area after they exchanged information and Ms. Lee contacted Newton PD about Ms. Adams' license status. I queried Ms. Adams' license and registration status through Newton PD Dispatch who informed me both were suspended. I spoke with Ms. Adams via telephone. Ms. Adams stated she was traveling down the ramp to the second floor of the parking garage and came to stop at the stop sign. Ms. Adams stated she began making her turn onto the second floor when MV1 sped up to go around her. Ms. Adams stated the front driver side of her vehicle crashed into the rear passenger side of MV2. Ms. Adams was advised of her license and registration status and was mailed Massachusetts Uniform Citation T0115918 for Chapter 90, Section 23 (Operate MV with a Suspended License) and Chapter 90, Section 23 (Operate MV with a Suspended Registration).

The weather at the time of the crash was clear and sunny. The road surface was dry. Walnut Street is a public way maintained by the City of Newton. The Operator of MV1, Paula Khoury, stated she was parked in front of 1185 Walnut Street, Newton. Ms. Khoury stated while operating a 2018 Kia Sorento (MA: 7HK496), she attempted to leave her parking spot. Ms. Khoury stated she looked back into traffic and did not observe any vehicles. Ms. Khoury stated she attempted to merge into the southbound travel lane of Walnut Street when the front driver side of her vehicle crashed into the middle passenger side of a passing pick up truck. Ms. Khoury reported no injuries. I observed damage to the front driver side of MV1. The operator of MV2, Brian Napolitano, stated he was operating a City of Newton owned 2008 Ford pick up truck (MA MVN: M1653A) southbound on Walnut Street. Mr. Napolitano stated as he was passing the area of 1185 Walnut Street, a black SUV parked to his right attempted to leave it's parking spot and merge into traffic. Mr. Napolitano stated MV1 crashed into the passenger side of his vehicle. I observed minor damage to the middle passenger side of MV2. Mr. Napolitano reported no injuries. Mr. Napolitano worked for the City of Newton DPW. Photos were taken of the damage to both vehicle and submitted to the IT Bureau.

On 3/31/21 I was working N503 when I responded to 68 Brierfield Rd. for a report of a past hit and run MVA. Upon arrival I spoke with the rp Eli Weisberger (the operator of Veh 2) and his brother Leo who stated that they were traveling westbound on Dedham St. when a car backed out from 98 Winchester (in front of 5 Dedham St.) colliding with their vehicle. Eli stated he saw the vehicle backing out and came to a complete stop and the vehicle backed right into him. Eli stated the operator of Veh 1 got out of his car and asked if they were ok to which they stated they were. Eli then stated the man said you know me I work at 711 and then drove away. I then spoke with Salim Grereige the operator of Veh 1. Salim stated he was backing out of 98 Winchester St. when he collided with Veh 2. Salim stated he looked both ways before he backed out and thought it was clear. Salim believed either Veh 2 was speeding or had just pulled out of the park right next to the parking lot. Salim stated they both got out of the cars to examine the damage. Salim said both parties agreed their was minor damage and Salim stated if you need me I work at 711. Salim stated he knows Eli from he being a consistent customer their. Salim stated they never asked for any of his information. It should be noted this accident occurred around 330pm and was reported at around 6pm. All parties stated they were uninjured. Both parties were provided an accident report number.

On Thursday 4/8/21 at approximately 0728 hours while assigned to marked unit n499 I was dipatched to the parking lot of Seasons 52 (220 Boylston St) for a report of a motor vehicle crash with minor injuries. Upon arrival I spoke with the operator (KRAUSE Liza) of MV#1 (Mazda CX-5). KRAUSE states she pulled into the parking lot from rte9 EB and was travelling straight when she observed a dump truck about to enter her lane of travel from the left. KRAUSE states she blew her horn, but the dump truck coninued on striking her vehicle on the drivers side rear. Impact from the collision caused significant damage to the drivers side of the vehicle in addition to deploying the side air bags in her vehicle. KRAUSE was evaluated by medics and refused transport to the hospital. She states she is uninjured just shaken up. Secondly, I spoke with the operator (BENNETT Paul A Jr) of MV#2 (KW Dump Truck). BENNETT states he was following another dump truck through the parking lot to the back exit on Florence Street to deliver asphalt to a construction job. In front of Seasons 52 he states he stopped at the stop sign and proceeded through unaware that MV#1 was in front of him striking the MV on the drivers side. There was no damage to the dump truck other than some marks on the front bumper. Motor vehicle crash occurred in a parking lot containing many stores, restaurants and a supermarket (Wegmans). Where the crash occurred was in front of Seasons 52 at a stop sign. At the stop sign is an island angled in such a manner to push vehicles to stay to the right and not make any turns. However, there are no signs displaying turning restrictions. The dump truck attempted to cross over the main entrance from rte9 to the parking lot containing the tent for SoulCycle and exit through the back of the parking lot. Vehicles entering from rte 9 do not have a stop sign and have the right of way to travel straight upon entry.

I went inside Loyal Companion and spoke with Manager Micheal Wade. Wade stated no pets were hurt during the accident and he had already notified the buildindgs mainternece company to repsonde and board up the windows.

On 10/02/2021 while assigned to marked cruiser N503 at approximately 20:46 hours I, Officer Vazhenin, responded to 601 Washington Street for a report of 2-car motor vehicle accident. Upon arrival on scene, I spoke to Mr. Jeffrey Yanfeng Xiao (Driver 1), who stated that he was driving eastbound on Washington Street when he observed another car (Vehicle 2) pulling out of 7/11 parking lot at 612 Washington street. Driver 1 said that in order to avoid collision with Vehicle 2, he had to swerve to the left in oncoming traffic lane crossing double yellow line. He said that while he was passing Vehicle 2, Vehicle 2 kept driving and crashed into the passenger side of his car (Vehicle 1). I then spoke to Ms. Shahrbanou Asadi, the operator of Vehicle 2, who stated that she was pulling out of 7/11 parking lot and was trying to make a left turn on Washington street. She stated that Vehicle 1 "showed up out of nowhere" and was traveling at high rate of speed. Ms. Asadi stated that she proceeded making her left turn when Vehicle 1 suddenly appeared in the front of her car and her car (Vehicle 2) crashed into passenger side of Vehicle 1. It should be noted that Ms. Asadi did not have her driver's license in possession. License plate inquiry of her car (Vehicle 2, MA Reg.#883MH8) revealed the fact that her driver's license was expired, which she failed to disclose to me when asked about status of her registration and driver's license. I ussued a Criminal Application to Ms. Shahrbanou Asadi on Massachusetts Uniform Citation #T1445734 for Ch.90 S.10 "Unlicensed Operation of Motor Vehicle", Ch.89 S.8 "Failure to yield", Ch.90 S.11 "License not in possession".

MV#1 was parked and unoccupied in front of #32 Brae Burn Rd, when according to a witness, MV#2 side swiped MV#1 and left the area. MV#1 sustained damage to rear passengers side. Witness, who was in the home at #22 Brae Burn Rd, describes MV#2 as JSC Transportation van that picked up a student at #22 Brae Burn Rd. Request Traffic Bureau follow up with JSC Transportation. Traffic Bureau update (Officer Gaudet): On November 23, 2021, I contacted JSC Transportation to follow up with them and get information for the operator of the van that was involved in the crash. JSC Transportation identified the operator of the vehicle as Ms. Brittany Flannery(S78277983) and provided me with her contact information. I also asked for information on the student that was on the vehicle at the time of the crash, Davine Archibald. JSC Transportation stated they did not realize this was the student on the van at the time of the crash. They immediately contact Davine's mother, Beula Effion to make her aware of the crash. I also contacted Ms. Effion to make her aware of the crash. Ms. Effion stated she received a call from JSC Transportation and that she is ok with everything. Ms. Effion stated she does not believe Davine was injured. Ms. Effion was advised. I spoke with Ms. Flannery about the crash. Ms. Flannery stated she was operating the JSC Transportation van (2019 Ford Transit, MA SPN: SP10940) involved in the crash. Ms. Flannery stated she was on her way to a pick up and was coming down Brae Burn Road. Ms. Flannery stated there were a couple of work vans parked on either side of the driveway where her pick up was. Ms. Flannery stated she pulled off the street and tried to park in between the two work vehicles on either side of the driveway. Ms. Flannery stated as she attempted to do this, the back passenger side of her van clipped the back passenger side of the van parked to her right. Ms. Flannery stated she asked the 1 passenger on the van, Davine Archibald, if she was ok. Ms. Flannery stated Davine stated she was. Ms. Flannery stated she exited the vehicle and checked the damage. Ms. Flannery reported the damage to the van as a scrape and scratch on the rear passenger side. Ms. Flannery stated she continued with her pick up at 22 Brae Burn Road and told the resident there she was going to contact the company that owns the van later. Ms. Flannery then left the area. Ms. Flannery stated she dropped the students off at the Newton Early Education Center on Jackson Road. I asked Ms. Flannery why she did not contact the Newton Police Department to report the crash and help her exchange information with the parked vehicle she hit. Ms. Flannery stated she did not know she had to because the vehicle was unoccupied. Ms. Flannery was advised that she needs to report being involved in a collision, especially when transporting children for JSC Transportation. Ms. Flannery will be cited with Massachusetts Uniform Citation 203510AB for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Brae Burn Road is a public way maintained by the City of Newton.

On 3/28/22 at approximately 0825 hours I was dispatched to 27 Oak Street for a report of a minor motor vehicle crash that just occurred. The original call for the incident was placed by the operator of motor vehicle #1, LEVIN Nina. LEVIN stated to Dispatcher Ciccone she was just involved in a minor crash with her neighbor at 23 Oak St. NEVIN stated she was leaving to pick up someone that needed a ride but would be right back. She further stated there is a long standing and ongoing neighbor problem with the person in the other vehicle and she did not want to speak with her without an Officer present. Upon arrival at the address LEVIN was already back on scene and began telling me what had occurred when the operator of the other vehicle, DELORIE Diane, came down the driveway and became confrontational with LEVIN and myself. I Instructed her to go back to her vehicle and wait for me to come up to her. LEVIN continued to tell me that she was backing down the driveway toward Oak St. and as she backed onto Oak St. she did not see the other vehicle and struck the front of DELORIE's vehicle with the rear of hers. After speaking with LEVIN I went to speak with DELORIE and stated there was no need to be confrontational as there was no intent and this was simply an accident. I asked DELORIE what happened and she stated she was travelling Southbound on Oak Street and slowed to a stop to take a right into her driveway at 23 Oak Street. DELORIE stated LEVIN was backing down the driveway at a high rate of speed and backed out of the driveway striking the front of her vehicle. Both LEVIN and DELORIE have a shared driveway and DELORIE needed to wait for LEVIN to exit before she could proceed into her driveway. DELORIE stated to me she felt it was done on purpose and wanted LEVIN cited and charged for leaving the scene. I informed DELORIE this did not constitute Leaving the Scene of a Motor Vehicle Crash - Property Damage as LEVIN reported the crash before DELORIE called. DELORIE and her husband disagreed with my assessment of the incident. DELORIE's husband insisted this constituted Leaving the Scene and DELORIE insisted backing out of a driveway was not permitted by State Law. I informed both parties that LEVIN would be given a verbal warning regarding failure to use care in backing but no citations would be issued There was minor damage caused to the passenger side front bumper of DELORIE's vehicle and no damage to the rear of LEVIN's vehicle.

The weather at the time of the crash was clear and sunny. The road surface was dry. Commonwealth Avenue is a public way maintained by the City of Newton. The operator of MV1, Mr. Pouya Jalili (S24275452), stated he was operating his 2019 Honda CR-V (MA: 1NPG25) on Commonwealth Avenue (E) near Melrose Street. Mr. Lalili stated as he was coming around a bend in the road, a vehicle in front of him has entered his travel lane from their driveway and was attempting to cross Commonwealth Avenue. Mr. Lalili stated he beeped his horn and swerved to the left to try and avoid the collision. MV1's from driver side crashed into the from driver side of MV2. MV1 had damage to it's front end and was removed from the roadway by Tody's towing. Mr. Lalili reported no injuries. The operator of MV2, Ms. Brianna Cataldo (S24925616), stated she was attempting to cross Commonwealth Avenue from her driveway while operating her 2018 Hyundai Elantra (MA: 7XB298). Ms. Cataldo stated while exiting her driveway, she stopped before entering to roadway to allow traffic to clear to her right and left. Ms. Cataldo stated she looked to her left for some time and did not observe any vehicles traveling towards her. Ms. Cataldo stated she proceeded to enter Commonwealth Avenue and make her left turn to travel Westbound. Ms. Cataldo stated she then heard a beep and MV1 was in the Westbound lane, crashing into the front of her vehicle. Ms. Cataldo stated she did not see the vehicle until it was too late. I observed damage to the front end of MV1. Ms. Cataldo reported no injuries.

On Sunday 6/12/22 at approximately 1333 hours while assigned to marked unit n498 I was dispatched to the area of 832 Dedham Street for a report of a 2 car motor vehicle crash with no injuries. Upon arrival I spoke with the operator, GERSHON Andrew, of motor vehicle #1 (6HS292). GERSHON states he was traveling Westbound on Dedham Street toward Brookline Street when he approached the driveway at 832 Dedham Street and motor vehicle #2 started to pull out in front of him. GERSHON states as he saw the vehicle pull out he swerved into oncoming traffic to avoid the collision, however motor vehicle #2 struck the passenger side of his vehicle. After the collision GERSHON attempted to pull back into his lane of travel, but lost control causing him to overcorrect and go off the road on the westbound side of the road in front of 824 Dedham Street causing some damage to the front landscaping of the yard. Secondly, I spoke with the operator, JIN Zhien, of motor vehicle #2 (EV87SA). JIN states she was attempting to pull out of the driveway of 832 Dedham Street and didn't see motor vehicle #1. JIN states she was distracted because there was a lot going on inside the car. She stated she was not on her cell phone. Finally I also spoke with a witness, CURTIS Ron, who was travelling in the opposite direction and witnessed the crash. Ron stated he saw motor vehicle #1 travelling straight in the opposite direction on Dedham Street. He saw the Tesla (MV#2) attempting to pull out of the driveway at 832 Dedham Street and drive into the side of MV#1. Ron stated he also observed MV#1 swerve into his lane of traffic trying to avoid the collision and then spinning out into the front of 824 Dedham Street. Taking the witness, operator, and passenger statements into account as well as my observations, I determined the operator, (JIN Zhien) of MV#2 (EV87SA) is at fault for the crash and she has been issued Massachusetts Uniform Citation T2012918 C89 S8 Failure to Yield Right of Way.

At approximately 1242 HRs, on Sunday, June 26, 2022, I was dispatched to 199 Boylston St. for a report of a two vehicle motor vehicle crash. upon speaking with the operator of MV1 (MA Pass:TC84GZ), he stated that he was traveling northbound in the parking lot of 199 Boylston St., in the travel section. It was not until a few moments prior to impact, that he observed MV2 driving across empty parking stalls and then ultimately striking MV1. Upon speaking with the operator of MV2 (MA Pass:2HXE78), she stated that she was attempting to park MV2. To this end, she was driving westbound through empty parking stalls and as she entered the travel section, struck MV1. The operator of MV2 could not offer an explanation as to why she did not properly yield the right of way to MV1. I noted damage to MV1's passenger side. The front passenger side door had a dent and scrapes and the right rear passenger side door had the door skin cut and dislodged, leaving a section approx. 1.5-2 feet long and the same wide, bent out and approx. 6 inches away from the body of MV1. I noted damage to the front left bumper cover and driving light of MV2. Due to safety concerns and to prevent further damage, the operator of MV1 began the process of securing a private tow for the vehicle. MV2 was able to continue to operate under its own power. I cleared without incident.

On Tuesday, June 28, 2022, while assigned to unit N496, I responded to the area of Langley Road and Hamlet Street, Newton for a report of a roll over crash with 3 injured parties. The weather at the time of this crash was clear and sunny. The road surface in the area of the crash was dry. Langley Road and Hamlet Street are both public ways maintained by the City of Newton. I approached the area of the crash coming from Boylston Street and when I arrived my attention was drawn to two locations on Langley Road. I observed a female party laying prone on the sidewalk area of Langley Road (E) at the Hamlet Street intersection. This female party, later identified as Ms. Melinda Conroy, was being attended to by passerby's in the area. My attention was then drawn to an overturned vehicle resting half in the roadway/half on the sidewalk (W) in front of 323 Langley Road. As I began to run over to the vehicle, I observed a woman standing next to the vehicle screaming frantically. This party identified herself as the driver of the lone vehicle involved, Ms. Natalya Kabanovsky (S65512051). There were parties at the back of the vehicle yelling to me that there was a woman trapped under the vehicle and that they were holding the vehicle up to get her out. I ran over to the rear of the vehicle and observed a female party, later identified as Ms. Annette Sooper, lying underneath the vehicle in the roadway. There was a large pool of blood on the roadway as the result of a laceration to what appeared to be her head/face area. Due to the unknown stability of the vehicle resting on its roof, and due to the passerby's stating they were holding the vehicle up so it did not roll over onto Ms. Sooper, I reached under the vehicle from the rear, grabbed Ms. Sooper's person, and dragged her away from the vehicle to a safe location on the sidewalk. At this time, I observed a large laceration to Ms. Sooper's head and she stated she had suffered a leg injury. Newton Fire and Medics along with additional Police units arrived on scene and took over rendering aid to the victims. I then started to conduct my investigation into the crash. I spoke with the operator of the vehicle, Ms. Kabanovsky. Ms. Kabanovsky stated she was operating her 2005 Lexus ES330 (MA: 293TAB) on Hamlet Street (W) towards Langley Road. Ms. Kabanovsky stated as she approached the intersection, a female party (Ms. Conroy) ahead of her was running across Langley Road (S) at Hamlet Street. Ms. Kabanovsky stated she attempted to hit her brakes, but instead hit her gas pedal. Ms. Kabanovsky's vehicle crashed into Ms. Conroy and then she panicked. Ms. Kabanovsky stated her car then accelerated across the street and crashed. Ms. Kabanovsky stated she was not wearing a seatbelt at the time of the crash and extricated herself from her vehicle after it rolled over onto its roof. I requested Ms. Kabonovsky's license while speaking to her, but she was too upset to located it. I queried Ms. Kabanovsky's license and registration status through Newton Dispatch. Ms. Kabanovsky's license stated is expired as of 12/18/2016. Ms. Kabanovsky registration is active. I observed substantial damage to the entire front end, driver, and passenger side of Ms. Kabanovsky vehicle. Ms. Kabanovsky was evaluated by Newton Medics and complained of right arm pain. Ms. Kabanovsky was also visibly distraught and expressed concern over the two pedestrians involved in the crash. Ms. Kabanovsky was transported to Brigham and Women's Hospital for treatment. I briefly spoke with Ms. Melinda Conroy as she was being attended to by Newton Medics and Fire. Ms. Conroy stated she was running on Langley Road (S) and observed MV1 traveling on Hamlet Street (W) towards Langley Road. Ms. Conroy stated she believed MV1 was slowing down and coming to a stop. Ms. Conroy stated she waved at the vehicle and as she crossed the intersection, MV1 accelerated and crashed into her left side. Ms. Conroy stated she flipped over the vehicle and landed on the roadway. Ms. Conroy was conscious and alert when speaking to Newton Medics and was transported to Beth Israel Deaconess Hospital in Boston. I later spoke with Ms. Conroy after her release from the hospital. Ms. Conroy stated she suffered some bumps and bruises had no major injuries. Officer Pohlman spoke with Ms. Sooper prior to her transport to Beth Israel Deaconess Hospital in Boston by Newton Medics. Ms. Sooper stated she was walking on the sidewalk in front of 323 Langley Road after returning from Newton Centre. Ms. Sooper stated as she was walking, she observed MV1 hit Ms. Conroy and then travel across the street in front of her into a tree. Ms. Sooper stated she does not remember what happened after that. I spoke with Ms. Sooper's son at NPD HQ and was advised Ms. Sooper was admitted to the hospital and suffered a fracture of her left hip and required 6 staples to close a laceration on her head. Tody's Towing responded to the scene and removed MV1 from the roadway. Traffic Officer Wilson took pictures of the scene and submitted them to the IT Bureau. After speaking with all parties involved, Ms. Kabanovsky will be cited with Massachusetts Uniform Citation 494688AB in hand at her residence for Chapter 90, Section 10 (Unlicensed Operation), Chapter 90, Section 24 (Operate To Endanger), and Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Ms Kabanovsky was also cited with Massachusetts Uniform Citation 494800AB in hand at her residence for Chapter 90, Section 13A (Fail To Wear A Seat Belt). A Request for Immediate Threat License Suspension/Revocation was completed and submitted to the Registry of Motor Vehicles with supporting documentation via e-mail, DCUImmediateThreat@massmail.state.ma.us.

On Wednesday, July 6, 2022, while assigned to unit N495, I responded to the area of Beacon Street and Walnut Street, Newton for a report of an MVA with no injuries. I met with both vehicles in the parking lot behind 1087 Beacon Street. The operator of MV1, Ms. Anne Bowers (SA6690354), stated she was operating her 2020 Mazda CX-5 (MA: 2SWC86) on Walnut Street (S) towards Beacon Street in the left turn only lane. Ms. Bowers stated the vehicle to her right entered her lane as she was traveling straight and crashed into the front passenger side area of her vehicle. I observed damage to the front passenger side tire/bumper/fender area of MV1. Ms. Bowers reported no injuries. The operator of MV2, Ms. Katia Inga-Martin (S74928812) stated she was operating her 2021 Nissan Rogue (MA:1VAX73) on Walnut Street (S) towards Beacon Street. Ms. Inga-Martin stated she put her left directional on to move into the other lane and did not see any other vehicles. Ms. Inga-Martin stated when she attempted to move into the left lane the driver side of her vehicle crashed into the passenger side of MV1. I observed moderate damage to the front driver side tire/fender/bumper/door area of MV2. Ms. Inga-Martin reported no injuries. Tody's towing responded and removed MV2 from the roadway due to the driver door not being able to secure. Both parties were advised.
