Vehicle #1 was traveling South on Waverly Ave. Vehicle # 2 was attempting to enter the Southbound lane of travel on Waverly Ave from Franklin St. Vehicle #2 pulled out into the lane that Vehicle #1 was already established in. Vehicle #1 had no time to stop and struck Vehicle #2 on the drivers side front wheel well. Both vehicles sustained damage. Vehicle #1 had major front end damage and Vehicle #2's tire was bent in. Vehicle #2 was towed from scene by Todys. No injuries reported on scene. Vehicle #2 operator was cited citation # T034419 89-9 failure to use care when entering intersection.

On 07/02/2019, while assigned to N494, I, Officer Conary, and Officer Anderson (N502) responded to Kenrick Street for a MVA. Upon arrival, I met with witness GRAU, who told me that he observed MV1 traveling Westbound on Kenrick Street. MV2 was exiting Dorr Road onto Kenrick Street and quickly went across the lanes of traffic and hit the stone wall at 337 Kenrick Street. MV1 swerved and hit MV2 on the front right side. Operator of MV1 stated that he was traveling Westbound when MV2 quickly exited Dorr Road and went across Kenrick Street. Operator of MV1 swerved but hit MV2 on the front right side. MV1 had heavy front end damage. Operator of MV2 stated that she stopped on Dorr Road waiting to turn onto Kenrick Street. MV2 began to roll back slightly due to being on a hill. Operator of MV2 pressed the gas to move up but pressed it too much which caused her to accelerated across Kenrick Street. MV2 then hit the stone wall at 337 and then MV2 collided with MV1. MV2 had heavy front end and right side damage. All operators and passengers were offered and signed patient refusals from the medics. Both motor vehicles were towed from the scene. Pictures were taken of the property damage to be submitted to IT accordingly. No further incident to report.

MV1 was traveling SOUTH on Chestnut St when he proceeded through a green light showing at the intersection. When he entered the intersection at Commonwealth Ave he was struck by MV2. MV2 had a stop sign on Commonwealth Ave in the carriage lane and proceeded into the intersection resulting in a collision with MV1. MV1, a 2010 Toyt Scion (RI Reg: ZI954), was being operated by a Kebin Eduardo Morales Pena. MV2, 2013 Subaru Legacy (MA Reg: 616WF8), was being operated by a Leonid Natanzon. There was heavy front end damage to MV2 and the driver's side back door was crushed in on MV1. Airbag deployment on MV1, Pena signed a refusal with the medics and his vehicle was towed by Tody's. Natanzon was uninjured and was issued in hand MA Uniform Citation T1441751 for 89/9 - Stop Sign, Fail.

On 07/16/19 at approximately 09:38 I responded to a two vehicle crash on Needham St. outside of #141. The Operator of Veh #1 stated she was turning left on Needham St when the South bound traffic stopped to left her turn. As she turned, Veh#2 drove South down the multi-turn lane and they collided nearly head on. Heavy damage and air bag deployment to Veh #1. The operator was checked out by Medics and signed a patient refusal. The Operator of Veh #2 stated she was driving down the multi-turn lane South on Needham St.. She was preparing to take a left onto Jaconnet St when Veh #1 pulled out of parking lot and collided with her vehicle. I observed moderate to heavy damage to Veh #2 and noted that the vehicle didn't have its directional on. The operator of Veh #2 complained to left leg pain and was transported to Newton-Wellesley Hospital By Ambulance #1. Both vehicles were towed by Tody's Towing. At the conclusion of my investigation it appears as if both vehicles were at fault during this crash; Veh#1 for not yielding to right of way and Veh #2 for traveling too soon in a turn lane, no directional and driving too fast for the heavy traffic conditions. Both operators were given verbal warnings and there will be no citations issued at this time.

On Tuesday, July 23rd 2019, at approximately 5:24pm, I, Officer Brooks, responded to 444 Lowell avenue for a 2 car MVA. Upon my arrival I observed MV1(NH REG 4120041) with heavy front end damage. The vehicle was located in the driveway of 444 Lowell avenue, with the front end sticking out into the roadway. I spoke with the operator of MV1 who stated he was trying to exit the driveway and turn left onto Lowell avenue southbound, when he was struck by MV2(MA REG 3AP243) which had been traveling northbound on Lowell avenue. According to the operator of MV1, MV2 had been traveling at a high rate of speed and tried to swerve around him. There was airbag deployment in MV1 however the operator signed a patient refusal with Cataldo. MV1 was towed by Tody's. I then spoke with the operator of MV2 who stated she was traveling northbound on Lowell avenue and saw MV1 exiting the driveway. She assumed he was going to stop pulling out of the driveway, and when he did not she stated she attempted to swerve around him. MV1 made contact with the passenger side rear wheel well of MV2 causing MV2 to spin out. The operator was not injured and arranged for a private tow due to the damage in the wheel well area.

The operator of veh #1 stated she was driving Southbound on Needham St. when veh #2 pulled out of Tower Rd. from her right and collided with her car. The side airbags went off in veh #1 but the driver stated she wasn't hurt. She was checked out and signed a patient refusal with the Medics. Moderate to heavy damage to veh #1 as it was towed by Tody's towing. The operator of veh#1 has a License and Registration issued by the Department of The State (Brazilian Consulate) and isn't in our in house system to plug into this report. The operator of veh #2 stated she was pulling out of Tower Rd attempting to go left (Northbound) on Needham St. and didn't see veh #1 passing as she turned. Minor damage to veh #2 as the operator was cited in hand Mass. #T1271259 for N.C.O. Failure to Use Care in Turning. All roads traveled/ mentioned in this report are public ways in the City of Newton as they are serviced and maintained by this city.

MV#2 was traveling south on Waverly Avenue when it was struck from Franklin Street by MV#1. -Operator of MV#1 stated she was traveling east on Franklin Street and that she stopped at the intersection. She stated as she was about to enter Waverly Avenue from Franklin Street and after checking to ensure she was clear to enter, she collided into MV#2. She stated he was flying up the road. -Operator of MV#2 stated he was traveling south on Waverly Ave when he was struck by MV#1. Operator of MV#2 stated the collision spun his vehicle 180 degrees. He stated he was facing the direction he was just comming from. He stated he didn't think she (Operator of MV#1) stopped. - I observed the front of MV#1 sustained heavy damage. The hood was bent in, the bumper was cracked and hanging, and fluids were leaking from the vehicle's engine. I next observed the damage to the doors on the right side of MV#2. I observed the base of the doors were pressed in, those doors would not open and the tires were grinding against the flared edges of the wheelwells. - There were no injuries reported at the scene, however, both operators were checked by Ambulance 1. Newton Fire responded to scene to contain the Hazmat spill from MV#1. MV#1 was towed from the scene. OpMV#1 was cited (T2015541) for Failure to come to a Complete stop at stop sign. I transported Operator of MV#1 to Tody's towing from the scene.

Narrative:
On Thursday, August 8, 2019, while assigned to Traffic unit N525, I responded to the area of Centre Street and Mt. Ida Street for a report of a 3 car motor vehicle crash with entrapment. The weather at the time of the crash was clear and sunny. The road surface was dry. Centre Street and Mt. Ida Street are both public ways maintained by the City of Newton. On arrival, I was updated by Sgt. Chisholm that one of the vehicles (MV3) had a female operator that was entrapped as a result of the collision. She was freed from the vehicle by the Newton Fire Department and transported to Beth Israel Hospital. Another operator (MV2) was transported to Newton Wellesley Hospital with minor injuries. A third operator (MV1) remained on scene with no injuries. With the assistance of Officer Wade we conducted a survey of the crash scene. We observed a large debris field at the intersection of Centre Street and Mt. Ida Street with debris from all three vehicles. The point of impact was observed to be on Centre Street (N) before Mt. Ida Street. There were fresh scrapes in the pavement as a result of the colission. We also observed skid marks from MV1 measuring 57 feet 8 inches from the start of it's skid to the vehicles final rest MV1 crashed into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane of Centre Street and crash into MV3 as it was passing by. The impact from MV2's front end crashing into MV3's driver side resulted in the operator of MV3 becoming entrapped. MV2 and MV3 came to final rest on Wesley Street (W) at Centre Street (S). The driver door of MV3 was inaccessible. The collsion resulted in air bag deployment for MV2 and MV3. I observed moderate damage to MV1's front grill/bumper area. I observed substantial damage to the front and rear of MV2 and substantial damage to the front and driver side of MV3. Tody's responded and removed MV2 and MV3 from the roadway. Photos were taken of the scene and submitted to the IT Bureau. Officer Martin of the Watertown Police Department was requested by Sgt. Lee to respond to the scene and assist with a commercial truck inspection of MV1. Officer Martin contacted Trooper Grey of the Massachusetts State Police Truck Team who also responded to assist with the truck inspection. Sgt. Lee remained with Officer Martin and Trooper Grey during the inspection. MV1 was relocated to Richardson Street for the inspection. Trooper Grey determined the truck was in compliance with all required paperwork and it passed it's mechanical inspection. MV1 was removed from the roadway by a state tow. I spoke with the operator of MV1, William Tisbert (S24792655). Tisbert stated he was operating a 2006 KW W900 Semi Truck (NH SEMI: AP2638) Northbound on Centre Street. Tisbert stated as he was approaching Mt. Ida Street, a vehicle came out of no where and he hit it. Tisbert stated there was no one in front of him and then a vehicle came off of a side street (Mt. Ida Street) and attempted to turn left in front of him to return to the side street. Tisbert stated he could not avoid hitting the vehicle. Tisbert crashed into the rear of MV2. MV2 then crossed into the opposite lane and crashed into another vehicle. Tisbert remained on scene and reported no injuries. Tisbert stated he was wearing his seatbelt at the time of the accident and was not distracted. Tisbert later admitted to crashing into the rear of MV2. I spoke with the operator of MV2, Janet Vanbuskirk (S59829146), at Newton Wellesley Hospital. Vanbuskirk stated while operating her 2004 Toyota Highlander (MA: RS 529Y), she was stopped on Centre Street (N) at Mt. Ida Street. Vanbuskirk had her left directional on and was waiting for traffic in the Southbound lane to clear to take a left turn onto Mt. Ida Street. Vanbuskirk stated MV1 crashed into the rear of her vehicle. As a result of the impact, Vanbuskirk's vehicle traveled into the opposite lane and crashed into the driver side of MV3. Vanbuskirk reported minor injuries to her neck, left arm, and right hand. I spoke with the operator of MV3, Madeleine Karolian (S23979811), at Beth Israel Deaconess Medical Center in Boston. Karolian stated she was operating her 2016 Toyota Prius (MA: EV5746) Southbound on Centre Street. Karolian stated MV1 crashed into MV2, and MV2 crashed into her vehicle. Karolian stated she did not have time to avoid the collision. Karolian stated she had minor injuries to her neck and chest. Karolian's vehicle had a dash cam recording at the time of the accident. I viewed the footage with her through an app on a cell phone. The footage shows MV3 traveling Southbound on Centre Street on August 8, 2019 at 1308 hours. Ahead of MV3 in the Northbound lane, MV2 is seen stopped on Centre Street before Mt. Ida Street with it's left directional activated. I observed MV1 travel Northbound on Centre Street and crash into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane and crashed into the driver side of MV3. The footage then ends. A copy of the footage was emailed to my work email. The footage was then forwarded to IT Director Steve Smith to be added to the media for this report. As a result of my investigation, the operator of MV1, William Tisbert, will be cited for Newton City Ordinance Chapter 19, Section 71 (Following too closely). Massachusetts Uniform Citation T1270942 was completed and mailed to Tisbert's residence.

The operator of MV#1 stated that he was traveling south on Grove St in heavy traffic when he stopped in front of #275 Grove St to let a MV enter the roadway. Operator #1 stated that his MV was then rear ended by MV#2. There did not appear to be any damage to MV#1 and there was a strong language barrier when speaking with the operator. The operator of MV#2 had left he scene prior to my arrival and this concerned operator #1. I then made contact with the owner of MV#2 who identified himself as the operator during a very minor crash on the date and time in question. Operator #2 stated he was also traveling south on Grove St directly behind MV#1 when he made slight contact with MV#1. Operator #2 stated he got of his MV, observed that there was no damage to either MV and then left the scene after speak with operator #1. I advised both parties that I would file a MV crash report and that they should notify their insurance provider. No injuries, no tows.

I first spoke with the operator of MV#1 who stated she was driving southbound on High St when MV#2 came out of Rockland PL and struck her driver side door and rear driver side door. MV#1 sustained moderate damage to the driver side of the vehicle. I then spoke with the operator of MV#2 who stated that it was her fault. She was coming down Rockland PL, westbound and didn't see MV#1 and attempted to turn left onto High St but struck MV#1. MV#2 sustained moderate damage to the front of the vehicle.

I spoke with the operator of Ma Reg 1XM680 Elizabeth CHISOLM who stated she was attempting to take a left onto Washington St, a vehicle in the right hand eastbound lane was turning right into the parking lot, CHISOLM stated she did not see Ma Reg EV3452 being operated by Julien GRANT in the left eastbound travel lane until it was too late colliding with him. I spoke with GRANT who stated he was traveling EB in the left travel lane of Washington St when struck by CHISOLM exiting the hospital.

Kaufman states as she attempted to proceed through the green signal MV2 turned into her lane attempting to make a left turn causing MV1 to strike MV2. MV2 Operator, COLON Marie, states she was travelling Westbound on Dedham Street attempting to turn left onto Nahanton Street. she states the traffic signal displayed a green light with a flashing yellow left turn signal. She attempted to turn left and proceeded into the path of MV1 causing the crash between MV1 and MV2. Neither party was injured and both vehicles were driven away from the crash

Her intention was to cross Cherry St and go down Cherry Place, which is directly across the street from Harris Rd. No injuries were reported. Vehicle #2 sustained damage to the right rear corner as well as the undercarriage and was towed by Tody's. Vehicle #1 was parked legally on Cherry St and the operator has his own tow coming, as the vehicle is a contracted Waltham school bus van. It should be noted that no students were on board the van at the time of the collision.

Opr of V1 stated he was stopped at the traffic light on the exit 17 exit ramp. V1 stated when it turned green he was traveling through the intersection. Opr of V1 stated he heard a car driving on his left, looked left, observed a dark colored van travel trough the intersection, and made contact with him. Opr of V1 stated V2 drove through the red light. Opr of V1 stated V2 took off on Centre Street towards Watertown, MA. V1 was towed by Tody's. Dispatch was notified to contact Watertown PD. Watertown PD found V2 in Watertown at the corner of Warren and Endenfield. Officer Brandon spoke to the Opr of V2. V2 stated she was nervous so she left the area. Officer Brandon advised her she needs to stay on location next time. Officer Brandon gave the Opr of V2 a copy of the criminal application (T2014370). He stated he observed a far distance buffer between him and MV #2. MV#1 was occupied by two passengers. -OpMV#2 stated she was traveling north on Center Street. She stated she observed a MV#1 entering the roadway. She stated MV#1 pulled out suddenly in front of MV #2. She collided with MV#1. - I observe the damage to both vehicles. I observed the heavy damage and deep gouge to the driver(cid:31)s side door of MV#1. The gouge had cut open the outer panel of the door revealing the mechanical components inside the door. OpMV#1 stated he could not open the door, but the vehicle was drivable. I next conducted an inspection of the exterior damage of MV#2. I observed that the plastic bumper was separated from the frame and several cracks and breaks on the bumper. I further observed damage to the wheel well panel. OpMV#2 stated the vehicle was drivable. -There were no injuries reported at the scene and all parties declined medical assistance. Neither vehicle was towed from the scene. OpMV#1 was cited (T2015553) for Failure to Use Care Entering the Roadway.

The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave making a left turn onto Morton St when he was struck by MV#2. The operator of MV#1 stated he was unsure of where MV#2 came from. There was a strong solar glare facing westbound on Commonwealth Ave during this accident. MV#1 sustained heavy driver's side front end damages. Both front airbags were deployed. The operator of MV#1 stated he was not injured. The front passenger of MV#1 was not injured and had already left the scene to attend school. The driver's side rear passenger sustained a bleeding nose from this accident. She was unsure what caused her injuries because it happened too quickly. She was evluated by Newton Paramedics and signed a patient refusal of treatment. The initial point of impact for both vehicles was in the middle intersection of Commonwealth Ave at Morton St. There were broken vehicle parts and tire skid marks starting from the point of impact and onto the berm on Commonwealth Ave on the carriage lane side. MV#2's final resting spot was directly accross from 808 Commonwealth Ave. MV#2 sustained heavy front end damages to its driver's side wheel (Broken axle) and flat front right side tire. MV#2's driver's side airbag was deployed. The operator of MV#2 was transported by Newton Paramedics to the Boston Medical Center. I spoke to the operator of MV#2 via telephone and she stated she was travelling westbound on Commonwealth Ave when she was struck by MV#1 and caused her swerve onto the carriage lane berm. The operator of MV#1 is 6.5 months into her pregnancy and sustained neck, back, bruises and left hand injuries. She was transported to Boston Medical Center and was later discharged the same day. She was provided with this accident report number and was advised to contact her insurance provider. I spoke to the witness who was at the intersection of Cedar St and Commonwealth Ave. The witness stated MV#1 was travelling eastbound on Commonwealth Ave and had stopped, attempting to make a left turn onto Morton St. The witness stated she then observed MV#2 travelling westbound on Commonwealth Ave and struck MV#1. The witness stated she believed MV#2 was travelling at a high rate of speed because it continued to travel onto the berm after striking MV#1. I took digital photographs of the damaged berm on Commonwealth Ave. Based on the statements made to me, I issued in hand to the operator of MV#1 citation #T1268357 for violation of c89 s8 failure to yield when turning left. Both vehicles were towed by "Tody's Towing".

On Saturday, September 28, 2019, I responded to the intersection of Crafts Street and Albemarle Road, Newton with Sgt. Lee for a report of a motor vehicle crash involving a motorcycle with serious injury to it's operator. The weather at the time of the crash was clear and sunny. The road surface was dry. Crafts Street and Albemarle Road are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jussara Pontes (S92930243). Pontes stated she was operating a 2017 Toyota Sienna (MA: 4GX777) at the time of the crash. Pontes stated she was traveling North on Albemarle Road and came to a stop at the stop sign located at the Crafts Street intersection. Pontes stated she looked both ways and began to inch out into the roadway. Pontes stated she stopped the vehicle again and was assisting a child sitting in the 2nd row of the vehicle. Pontes stated she then turned her attention back to the roadway. Pontes stated she looked to her right and left again and did not see any vehicles in either direction. Pontes stated she proceeded to cross the intersection to continue North on Albemarle Road. Pontes stated the front bumper area of MV1 crashed into the left side of a passing motorcycle. Pontes stated the motorcycle and it's operator fell to the roadway and slid across the street to the side of the road. Pontes stated she pulled her vehicle over onto Albemarle Road. Pontes stated she did not see the motorcycle prior to the crash and was not distracted. The operator of the motorcycle (MV2), Irene Pistoftzian (S06099449) was transported to Brigham and Women's Hospital with a serious left leg injury prior to my arrival. I observed a red 1990 Kawasaki ZR550 (MA MCN: 1V1063) resting on it's right side on the NE sidewalk area of Albemarle Road at Crafts Street with it's front wheel/handlebar facing Crafts Street. The motorcycle had moderate damage to it's left side from the impact of MV1 crashing into it. The right side of the motorcycle had damage from sliding on the roadway and the crashing into a fence on the sidewalk on Crafts Street (W) at Albemarle Road. There was also damamge to the front headlight/right handlebar area. The motorcycle and Toyota Sienna were towed from the scene by Tody's towing at Sgt. Lee's request. Pictures were taken of the scene and submitted to the IT Bureau. I spoke with Pistoftzian at Brigham and Women's Hospital in Boston on Monday, September 30, 2019. Pistoftzian stated she was operating her motorcycle on Crafts Street (W). Pistoftzian stated as she approached the intersection of Crafts Street and Albemarle Road, she observed a mini van to her left out of her peripheral. Pistoftzian stated she remembers being hit on her left side and then laying on the roadway looking up at the sky on the sidewalk with her head facing Albemarle Field. Pistoftzian stated as a result of the crash she has a significant injury to her left leg and also has an injury to her left wrist/arm. Pistoftzian stated she was wearing a helmet at the time of the crash. I spoke with Kymberley Mitchell via telephone who stated she witnessed the crash. Mitchell stated she was traveling behind the motorcycle involved in the crash on Craft Street (W) at North Street. Mitchell stated they were stopped at a red light at this time. Mitchell stated there were no vehicles in front of the motorcycle at the light. Mitchell stated the light turned green and they continued on Crafts Street at a low rate of speed. Mitchell stated she could see ahead at the intersection of Albemarle Street and Crafts Street, traffic was stopped on Crafts Street (E) letting traffic exit Albemarle Road (N). Mitchell stated vehicles were taking left and right turns out of Albemarle Road before the motorcycle reached the intersection. Mitchell stated a mini van was crossing past the double yellow line of Crafts Street to continue straight when it crashed into the left side of the motorcycle. I spoke with Cheryl Wermer via telephone who stated she witnessed the crash. Wermer stated she was traveling on Crafts Street (E) towards Albemarle Road. Wermer stated traffic ahead of her was stopped to allow vehicles to turn off of Albemarle Road. Wermer stated she observed a motorcycle traveling towards her on Crafts Street (W). Wermer stated she then saw a vehicle travel straight from Albemarle Road across Crafts Street and crash into the motorcycle. Wermer stated the operator of the motorcycle flew off the bike. Wermer stated the car then came to a stop and the driver got out and stated "Oh my god! I didn't see you!" Wermer stated both vehicles were traveling at a low rate of speed prior to the crash. I spoke with Bill Frabizio via telephone who stated he witnessed the crash. Frabizio stated he was behind the mini van (MV1) on Albemarle Road prior to the crash. Frabizio stated the vehicle in front of him began to cross the intersection as he was monitoring the traffic on to take a right turn onto Crafts Street. Frabizio stated he heard a loud "Boom" and saw a motorcycle slide across the street with its operator. Frabizio stated the motorcycle crashed into a fence post and the operator came off the motorcycle. Frabizio stated he pulled over and assisted operator of the motorcycle. After concluding my investigation, the operator of MV1, Jussara Pontes, was mailed Massachusetts Uniform Citation T2016424 for Newton City Ordinance Chapter 19, Section 75 (Failure To Use Care).

On 10/04/19 at 11:49 hours while assigned to Traffic Unit 523, I responded to a motor vehicle/bicyclist accident at Centre Street and Nathan Road. The weather at the time was cloudy. The road surface was dry and Centre Street and Nathan Road are public ways maintained by the City of Newton. Traffic conditions were heavy and Centre Street was congested with vehicles at the time. There is no dedicated bicycle lane in this area but there are bicycle indicators painted on the roadway to make drivers aware of bicyclists traveling. I spoke to the bicyclist involved (Unit 2), Marvin Goldman (S67373495). Goldman was wearing a helmet, gray sweatshirt, gray shorts, and black/orange reflective sneakers. Mr. Goldman stated that he was traveling southbound on Centre Street on the right side of the road on his black Fuji Sagres bicycle. He further says that he was peddling fast at the time while crossing over Nathan Road and that is when he suddenly struck Unit 1 on it's passenger side front quarter panel/door area. He does not recall any other details of the incident along with Unit 1 attempting to turn left towards him. Goldman sustained scraped up knees and a small laceration below his left eye. He refused medical attention signing a patient refusal with Cataldo Medic One. I observed that his front wheel was bent as a result of the accident. The rest of the bicycle appeared to be intact. I then spoke to the operator of Unit 1, Darlene Martins (S27966454). Ms. Martins spoke with the assistance of Ofc. Calderon who was translating for her. She says that she was traveling northbound on Centre Street and was attempting to turn left on to Nathan Road in order to turn around and travel the other way on Centre Street. She says that a vehicle in the southbound travel lane left space for her to make her left turn. She then proceeded to turn thinking it was clear and that is when the bicyclist suddenly approached and struck her on her passenger side. She also stated that it was hard to see past the congestion of vehicles waiting in the southbound travel lane. I observed scratch marks and a dent to her passenger side front quarter panel area along with minor scuff marks to her front passenger door. The scuff marks appear to be from the bicyclist rubbing against the vehicle. I also observed a dent on the top of the passenger door frame. It is unknown if the damage is the result of the accident or old damage Ofc. Calderon spoke to Rosangela Dias (Passenger in Unit 1), who stated that everything happened so quick but she never saw the bicyclist approaching when Martins attempted to turn left. I then spoke to a witness on scene identified as Matthew Miner. He says that he was two vehicles behind an unknown vehicle in the southbound travel lane of Centre Street. He says that the unknown vehicle left space for Unit 1 to turn left and waved Unit 1 on to turn left. He does not recall what transpired with the bicyclist. As a result of the accident, it was determined that Martins did not yield the right of way to the bicyclist while she was turning left. She also does not have a license to operate in Massachusetts or any other state. She was issued MA Uniform Citation # T2012393 for Ch. 89/8 Failure to yield and Ch. 90/10 Unlicensed Operation of a Motor Vehicle. Digital photos taken by me of the scene.

Operator of vehicle 1 stated he was traveling eastbound on River St and was taking a left turn onto Cherry St and was struck head on with vehicle 2. Operator of vehicle 2 stated she was traveling westbound on River St and as she was entering the intersection of Cherry St the traffic light turned yellow and vehicle 1 which was traveling eastbound made a sudden left turn leaving her no time to stop and the vehicles collided. Vehicle 1 then sped off northbound on Cherry St and returned a minute or two later. I spoke to a witness, Jena Castro-Casbon who stated she saw vehicle 1 traveling northbound on Cherry St at a high rate of speed and drop of a child at 187 Cherry St then turn around and return to the accident scene at a high rate of speed again. 187 Cherry St is 0.4 miles from the intersection of River St. Operator of vehicle 1 stated he had two of his sons friends in the vehicle and he thought it would be best to get one of them home to 187 Cherry St and then come back to exchange information. I followed up with the child and his parents at 187 Cherry St and he was uninjured. I issued the operator of vehicle 1 MA Uniform Citation #T1445381 for 89/8 failure to yield. No one reported any injures and neither vehicle required a tow.

Operator of vehicle 1 states she was traveling west on Derby St when vehicle 2 pulled out from Parmenter Rd. Vehicle 2 struck the right passenger side door of Vehicle 1 and continued to turn onto Derby St., traveling west. Vehicle 1 followed vehicle 2 into Waltham and obtained a photo of the license plate of vehicle 2. Operator of vehicle 1 reports no injuries. I attempted to contact the operator of vehicle 2 with negative results. Waltham PD was unable to make contact with the registered owner of vehicle 2 and vehicle 2 was not at the registered address. Citation to be mailed to registered owner of vehicle 2 for 90/24/c leaving the scene after a motor vehicle crash with property damage, as well as 89/9 failure to yield right of way.

On Sunday 11/24/19 at approximately 1730 hours while working in marked unit n499 I was dispatched to the parking lot on the rt9 side of 225 Boylston Street (Bloomingdales) for a report of a minor motor vehicle crash. Upon arrival I spoke with the operator of motor vehicle 2, GIRARD Kathleen, who states that she was driving from the 2nd aisle of parking spots into the main travel lane to attempt to exit the mall. When she attempted to turn left she didnt see motor vehicle 1 already in the lane of traffic striking the side of the vehicle. After speaking with GIRARD I spoke with the operator of motor vehicle 1, SONG Guang min. SONG did not speak english, but he corroborated what GIRARD had stated by using a translation app on his cell phone. GIRARD had damage to the front of her vehicle and SONG had damage to the drivers side of his vehicle. Both vehicles were still operable and were driven from the scene. Neither party was injured

On Monday, December 2nd, 2019 at approximately 1828 hours I, Officer Newton, and Officer Brooks responded to a 3 car motor vehicle accident at Watertown St and Eddy St. The operator of vehicle 1 stated he was stopped at the stop sign on Eddy St southbound when he proceeded to pull out to cross Watertown St when Vehicle 2, traveling eastbound on Watertown St, hit their car causing them to also collide with vehicle 3. The operator of vehicle 2 stated she was driving eastbound on Watertown St. when vehicle 1 suddenly pulled out in front of her causing the collision. The operator of vehicle 3 stated she was at the stop sign on Eddy St northbound, opposite of vehicle 1's stop sign on Eddy St, when Vehicle 1 pulled out and caused vehicle 2 to hit vehicle 1 and vehicle 2 to hit her. All parties involved declined medical attention. All 3 vehicles were able to safely drive away from the scene.

On 12/10/2019 at approx 1036hrs while assigned to 497 i responded to the area of 2102 Commonwealth Av for a report of a 2 car crash. upon arrival I observed Ma Reg V10183 a red Ford F150 pick up with front plow partially on the curb blocking the EB lane of Commonwealth Ave and Ma Reg 8PE124 a black Ford Focus pulled off to the side WB with extensive front end damage. I spoke with the operator of the pick up, Leonardo TAPIA who stated he had been travelling WB on Commonwealth Av when the Ford Focus pulled out in front of him causing the collision. I spoke to the operator of the Ford Focus John KELLY who stated he was taking a left out of the parking lot of 2102 Commonwealth Av, traffic in the EB lanes stopped to let him out, as he was entering the WB lane he collided with the pick up. Ford Focus towed by Todys, both operators signed refusals with Cataldo.

On December 19th, 2019 at approximately 07:19 hours while assigned to N-491 cruiser I responded to the intersection of Walnut St @ Lindwood Ave for a report of a two vehicle crash. On my arrival I spoke with both operators involved in this crash. Operator # 1 reported that she was travelling S/B on Walnut St when vehicle #2 came out from Lindwood Ave heading W/B and crashed into her vehicle in the intersection. Operator #2 reports that he was stopped on Lindwood Ave @ Walnut St(stop sign) attempting to get to the other side of the intersection to continue W/B on Lindwood Ave. He further stated he had looked both ways and didn't see any oncoming traffic. He then proceeded to cross Walnut St heading to the other side of Lindwood Ave when he crashed into vehicle #1 in the intersection. There were no reported injuries, Vehicle #1 was towed by a private tow company. Vehicle #2 was able to drive away.

On 12/23/19 at 12:06 hours, I came across a two car motor vehicle accident that already occurred at California Street and Bridge Street. No injuries reported. Both streets are public ways and maintained by the City of Newton. Operator of M/V # 1 states he was traveling straight ahead (EB) on California Street through the intersection at Bridge Street. He states that he had the green light signal and attempted to drive through when he was struck by M/V # 2 on his front bumper on the driver's side. He further says that M/V # 2 was attempting to take a left turn on to Bridge Street at the time of the accident. I observed moderate damage to the driver's side front bumper/quarter panel area but the vehicle was still driveable. Operator of M/V # 2 states through translation by her husband (Huseyn Imamaliyev) she was operating (WB) on California Street and was attempting to take a left turn through the intersection on to Bridge Street. She says that she stopped first and then attempted to turn left and that is when M/V# 1 crashed in to her. I observed minor damage to the middle front bumper/registration plate area. Based on the locations and damage to the vehicles, it appears that the operator of M/V # 2 did not yield the right of way when turning left on to Bridge Street. The operator of M/V # 2 (Gulnar Imamaliyeva MA ID# SA1320134) also could not provide a driver's license to operate a motor vehicle in Massachusetts. A query of her revealed that she only had a Massachusetts identification. She was issued MA Uniform Citation # T2012157 for Ch. 89/8 Fail to Yield Right of Way When Turning Left and Ch. 90/10 Operating a M/V Without a License.
