Vehicle #1 was traveling South on Waverly Ave. Vehicle # 2 was attempting to enter the Southbound lane of travel on Waverly Ave from Franklin St. Vehicle #2 pulled out into the lane that Vehicle #1 was already established in. Vehicle #1 had no time to stop and struck Vehicle #2 on the drivers side front wheel well. Both vehicles sustained damage. Vehicle #1 had major front end damage and Vehicle #2's tire was bent in. Vehicle #2 was towed from scene by Todys. No injuries reported on scene. Vehicle #2 operator was cited citation # T034419 89-9 failure to use care when entering intersection.

On 07/02/2019, while assigned to N494, I, Officer Conary, and Officer Anderson (N502) responded to Kenrick Street for a MVA. Upon arrival, I met with witness GRAU, who told me that he observed MV1 traveling Westbound on Kenrick Street. MV2 was exiting Dorr Road onto Kenrick Street and quickly went across the lanes of traffic and hit the stone wall at 337 Kenrick Street. MV1 swerved and hit MV2 on the front right side. Operator of MV1 stated that he was traveling Westbound when MV2 quickly exited Dorr Road and went across Kenrick Street. Operator of MV1 swerved but hit MV2 on the front right side. MV1 had heavy front end damage. Operator of MV2 stated that she stopped on Dorr Road waiting to turn onto Kenrick Street. MV2 began to roll back slightly due to being on a hill. Operator of MV2 pressed the gas to move up but pressed it too much which caused her to accelerated across Kenrick Street. MV2 then hit the stone wall at 337 and then MV2 collided with MV1. MV2 had heavy front end and right side damage. All operators and passengers were offered and signed patient refusals from the medics. Both motor vehicles were towed from the scene. Pictures were taken of the property damage to be submitted to IT accordingly. No further incident to report.

MV1 was traveling SOUTH on Chestnut St when he proceeded through a green light showing at the intersection. When he entered the intersection at Commonwealth Ave he was struck by MV2. MV2 had a stop sign on Commonwealth Ave in the carriage lane and proceeded into the intersection resulting in a collision with MV1. MV1, a 2010 Toyt Scion (RI Reg: ZI954), was being operated by a Kebin Eduardo Morales Pena. MV2, 2013 Subaru Legacy (MA Reg: 616WF8), was being operated by a Leonid Natanzon. There was heavy front end damage to MV2 and the driver's side back door was crushed in on MV1. Airbag deployment on MV1, Pena signed a refusal with the medics and his vehicle was towed by Tody's. Natanzon was uninjured and was issued in hand MA Uniform Citation T1441751 for 89/9 - Stop Sign, Fail.

MV#1 was parked and unoccupied in front of #334 River St when it was struck by MV#2. The operator of MV#2 stated that he was traveling east on River St when his MV struck MV#1, which was parked on the side of the roadway. No injuries, no tows.

Mv#1 operator stated she was travelling straight ahead on Centre St N/B slowing down with her right signal on preparing to pull over in front of 1400 Centre St beyond the corner of Paul St. #1 stated she was struck passenger broadside by Mv#2 exiting from Paul St. #1 sustained passenger side damage and towed by Tody's. #1 operator signed a patient refusal. #2 operator stated he came to a stop at the end of Paul St preparing to turn left on Centre St S/B. #2 stated he assumed #1 was turning right on Paul St with her signal on. #2 proceeded left onto Centre St as #1 was passing by and struck #1 broadside. #2 sustained front end damage and towed by Tody's. #2 operator also signed a patient refusal.

The operator of MV#1 stated that she was stopped at the red light on Commonwealth Ave at Lexington St when her MV was rear ended by MV#2. The operator of MV#2 stated that he was traveling directly behind MV#1 when he thought traffic started moving straight ahead due to the traffic light turning green and he rear ended MV#1. No injuries, no tows.

(cid:31)Operator of vehicle #1 was travelling Westbound on Washington proceeding through the intersection at Washington And Adams St. Operator #1 stated she observed vehicle #2 which was heading Eastbound on Washington St. attempt to make a left turn onto Adams St. in front of her. Operator attempted to stop but was unable to in time and collided with vehicle #2. Operator of vehicle #2 stated he was travelling Eastbound on Washington St. and had a green light. Operator #2 attempted to make a left turn onto Adams St. in front of vehicle #1 and collided with vehicle #1.

On 07/16/19 at approximately 09:38 I responded to a two vehicle crash on Needham St. outside of #141. The Operator of Veh #1 stated she was turning left on Needham St when the South bound traffic stopped to left her turn. As she turned, Veh#2 drove South down the multi-turn lane and they collided nearly head on. Heavy damage and air bag deployment to Veh #1. The operator was checked out by Medics and signed a patient refusal. The Operator of Veh #2 stated she was driving down the multi-turn lane South on Needham St.. She was preparing to take a left onto Jaconnet St when Veh #1 pulled out of parking lot and collided with her vehicle. I observed moderate to heavy damage to Veh #2 and noted that the vehicle didn't have its directional on. The operator of Veh #2 complained to left leg pain and was transported to Newton-Wellesley Hospital By Ambulance #1. Both vehicles were towed by Tody's Towing. At the conclusion of my investigation it appears as if both vehicles were at fault during this crash; Veh#1 for not yielding to right of way and Veh #2 for traveling too soon in a turn lane, no directional and driving too fast for the heavy traffic conditions. Both operators were given verbal warnings and there will be no citations issued at this time.

On Wednesday 7/17/2019 at approx 1635hrs, while assigned to N-494, I responded to the intersection of Comm Ave @ Centre St for a 2 car MVA. Operator of MV1 states she was traveling EB on Comm Ave. She states that while attempting to travel straight across the intersection MV2 cut right in front her striking the front passenger side of MV1. Operator of MV2 states that she was traveling WB on Comm Ave as she approached the intersection. She states that she didn't see incoming cars as she attempting to take a left onto Centre St. She states that MV1 "came out of nowhere" and must have been speeding." Operator of MV2 was cited for failing to yield right of way traffic MGL c89 s9. Citation #T1444757

The operator of MV#1 stated that she was in the left hand turn lane on Grove St (north) in front of #275 Grove St. When her arrow to turn left turned green she proceeded to turn when her MV was struck by MV#2. The operator of MV#2 stated that she was traveling south on Grove St in front of #275 when she noticed she had a red light at the last minute and attempted to stop, but entered the intersection and struck MV#1. A witness walking in front of #275 Grove St stated she observed MV#2 go through her red light and strike MV#1 as she was turning into parking area. Operator #2 cited for failure to comply w/red light.

The operator of Vehicle #1 stated that as he was travelling Westbound on Washington St just past the MassPike bridge, his vehicle was struck on the rear driver's side by Vehicle #2 as she attempted to merge into his lane of travel. The operator of Vehicle #2 stated that she realized that she was in the incorrect lane, and was heading onto the MassPike, when she indicated several times that she wished to merge. She then attempted to merge right, prior to colliding with Vehicle #1. No Parties on scene complained of injury, nor was a tow requested.

On Tuesday, July 23rd 2019, at approximately 5:24pm, I, Officer Brooks, responded to 444 Lowell avenue for a 2 car MVA. Upon my arrival I observed MV1(NH REG 4120041) with heavy front end damage. The vehicle was located in the driveway of 444 Lowell avenue, with the front end sticking out into the roadway. I spoke with the operator of MV1 who stated he was trying to exit the driveway and turn left onto Lowell avenue southbound, when he was struck by MV2(MA REG 3AP243) which had been traveling northbound on Lowell avenue. According to the operator of MV1, MV2 had been traveling at a high rate of speed and tried to swerve around him. There was airbag deployment in MV1 however the operator signed a patient refusal with Cataldo. MV1 was towed by Tody's. I then spoke with the operator of MV2 who stated she was traveling northbound on Lowell avenue and saw MV1 exiting the driveway. She assumed he was going to stop pulling out of the driveway, and when he did not she stated she attempted to swerve around him. MV1 made contact with the passenger side rear wheel well of MV2 causing MV2 to spin out. The operator was not injured and arranged for a private tow due to the damage in the wheel well area.

operator 1 stated he was travelling through the intersection from Auburn St on to Central St. when vehicle two possibly ran the red light crashing into him. Operator of vehicle 2 stated she was travelling straight on Grove St and does not remember seeing the lights and thought she had the right of way. There were no injuries due to this accident and neither vehicle was towed.

Operator of veh#1 stated he was driving North on Centre St when veh #2 pulled out of her parking spot and struck his vehicle. Minimal damage to veh #1 front bumper. Operator of veh #2 stated she was pulling out of her parking spot due North on Centre St when veh #1 came out of no where and she collided with him. Front bumper removed from veh #2. Operator of veh #2 cited #T1271258 in hand for N.C.O. 19/75 - Failure to use care in starting, stopping and turning. Both operators stated their speed was slow at the time of impact. Centre St is a public way in the City of Newton as it is serviced and maintained by this city.

Operator of Veh # 1 stated that she was coming from Mount Alvernia Road and didn't look right when she pulled onto Comm Ave., and was struck by MV #2. Oper # 2 stated he was traveling down Comm Ave when vehicle # 1 entered Comm Ave, Oper # 2 was unable to stop before hitting MV #1. Medics responded and got all refusals. Veh # 1 towed by Tody's.

On 07/28/19 at approximately 12:26 I responded to a two vehicle crash at the intersection of Needham St and Tower Rd. The operator of veh #1 stated she was driving Southbound on Needham St. when veh #2 pulled out of Tower Rd. from her right and collided with her car. The side airbags went off in veh #1 but the driver stated she wasn't hurt. She was checked out and signed a patient refusal with the Medics. Moderate to heavy damage to veh #1 as it was towed by Tody's towing. The operator of veh#1 has a License and Registration issued by the Department of The State (Brazilian Consulate) and isn't in our in house system to plug into this report. The operator of veh #2 stated she was pulling out of Tower Rd attempting to go left (Northbound) on Needham St. and didn't see veh #1 passing as she turned. Minor damage to veh #2 as the operator was cited in hand Mass. #T1271259 for N.C.O. Failure to Use Care in Turning. All roads traveled/ mentioned in this report are public ways in the City of Newton as they are serviced and maintained by this city.

Operator of vehicle 1 states she was in the middle (left turn/straight) lane on Needham St turning left onto Winchester St when vehicle 2 came up on her right from the right turn/straight lane and tried to go around her but instead hit her passenger side causing minor damage and a flat tire. Operator of vehicle 2 stated he was turning left and vehicle 1 swerved in front of him. I issued operator of vehicle 2 MA uniform citation #T1444587 for 89/4a failure to use care when passing. Neither vehicle required a tow but AAA did respond to change the tire on vehicle 1. Neither operator reported any injuries.

Motor vehicle 1 (MV1) was traveling southbound on Langley Rd. when motor vehicle 2 (MV2) attempted to merge into the straight travel lane from the left turn lane, causing MV1 to crash into the side of MV2. MV 1 sustained front drivers side damage. MV2 sustained passengers side damage. I issued the operator of MV2 MA uniform citation T12713227 for Ch. 89/8 (fail to yield right of way) and Ch. 90/11 (license not in possession).

Motor vehicle 1 (MV1) was traveling southbound on Centre St. when motor vehicle 2 (MV2), which was also traveling southbound on Centre St. attempted to go around an uninvolved vehicle that was waiting to turn left onto Langley Road. MV2 failed to yield to MV1 causing MV1 to crash into the passengers side of MV2. MV1 sustained front end damage and MV2 sustained passengers side damage.

MV#2 was traveling south on Waverly Avenue when it was struck from Franklin Street by MV#1. -Operator of MV#1 stated she was traveling east on Franklin Street and that she stopped at the intersection. She stated as she was about to enter Waverly Avenue from Franklin Street and after checking to ensure she was clear to enter, she collided into MV#2. She stated he was flying up the road. -Operator of MV#2 stated he was traveling south on Waverly Ave when he was struck by MV#1. Operator of MV#2 stated the collision spun his vehicle 180 degrees. He stated he was facing the direction he was just comming from. He stated he didn't think she (Operator of MV#1) stopped. - I observed the front of MV#1 sustained heavy damage. The hood was bent in, the bumper was cracked and hanging, and fluids were leaking from the vehicle's engine. I next observed the damage to the doors on the right side of MV#2. I observed the base of the doors were pressed in, those doors would not open and the tires were grinding against the flared edges of the wheelwells. - There were no injuries reported at the scene, however, both operators were checked by Ambulance 1. Newton Fire responded to scene to contain the Hazmat spill from MV#1. MV#1 was towed from the scene. OpMV#1 was cited (T2015541) for Failure to come to a Complete stop at stop sign. I transported Operator of MV#1 to Tody's towing from the scene.

On Thursday, August 8, 2019, while assigned to Traffic unit N525, I responded to the area of Centre Street and Mt. Ida Street for a report of a 3 car motor vehicle crash with entrapment. The weather at the time of the crash was clear and sunny. The road surface was dry. Centre Street and Mt. Ida Street are both public ways maintained by the City of Newton. On arrival, I was updated by Sgt. Chisholm that one of the vehicles (MV3) had a female operator that was entrapped as a result of the collision. She was freed from the vehicle by the Newton Fire Department and transported to Beth Israel Hospital. Another operator (MV2) was transported to Newton Wellesley Hospital with minor injuries. A third operator (MV1) remained on scene with no injuries. With the assistance of Officer Wade we conducted a survey of the crash scene. We observed a large debris field at the intersection of Centre Street and Mt. Ida Street with debris from all three vehicles. The point of impact was observed to be on Centre Street (N) before Mt. Ida Street. There were fresh scrapes in the pavement as a result of the colission. We also observed skid marks from MV1 measuring 57 feet 8 inches from the start of it's skid to the vehicles final rest MV1 crashed into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane of Centre Street and crash into MV3 as it was passing by. The impact from MV2's front end crashing into MV3's driver side resulted in the operator of MV3 becoming entrapped. MV2 and MV3 came to final rest on Wesley Street (W) at Centre Street (S). The driver door of MV3 was inaccessible. The collsion resulted in air bag deployment for MV2 and MV3. I observed moderate damage to MV1's front grill/bumper area. I observed substantial damage to the front and rear of MV2 and substantial damage to the front and driver side of MV3. Tody's responded and removed MV2 and MV3 from the roadway. Photos were taken of the scene and submitted to the IT Bureau. Officer Martin of the Watertown Police Department was requested by Sgt. Lee to respond to the scene and assist with a commercial truck inspection of MV1. Officer Martin contacted Trooper Grey of the Massachusetts State Police Truck Team who also responded to assist with the truck inspection. Sgt. Lee remained with Officer Martin and Trooper Grey during the inspection. MV1 was relocated to Richardson Street for the inspection. Trooper Grey determined the truck was in compliance with all required paperwork and it passed it's mechanical inspection. MV1 was removed from the roadway by a state tow. I spoke with the operator of MV1, William Tisbert (S24792655). Tisbert stated he was operating a 2006 KW W900 Semi Truck (NH SEMI: AP2638) Northbound on Centre Street. Tisbert stated as he was approaching Mt. Ida Street, a vehicle came out of no where and he hit it. Tisbert stated there was no one in front of him and then a vehicle came off of a side street (Mt. Ida Street) and attempted to turn left in front of him to return to the side street. Tisbert stated he could not avoid hitting the vehicle. Tisbert crashed into the rear of MV2. MV2 then crossed into the opposite lane and crashed into another vehicle. Tisbert remained on scene and reported no injuries. Tisbert stated he was wearing his seatbelt at the time of the accident and was not distracted. Tisbert later admitted to crashing into the rear of MV2. I spoke with the operator of MV2, Janet Vanbuskirk (S59829146), at Newton Wellesley Hospital. Vanbuskirk stated while operating her 2004 Toyota Highlander (MA: RS 529Y), she was stopped on Centre Street (N) at Mt. Ida Street. Vanbuskirk had her left directional on and was waiting for traffic in the Southbound lane to clear to take a left turn onto Mt. Ida Street. Vanbuskirk stated MV1 crashed into the rear of her vehicle. As a result of the impact, Vanbuskirk's vehicle traveled into the opposite lane and crashed into the driver side of MV3. Vanbuskirk reported minor injuries to her neck, left arm, and right hand. I spoke with the operator of MV3, Madeleine Karolian (S23979811), at Beth Israel Deaconess Medical Center in Boston. Karolian stated she was operating her 2016 Toyota Prius (MA: EV5746) Southbound on Centre Street. Karolian stated MV1 crashed into MV2, and MV2 crashed into her vehicle. Karolian stated she did not have time to avoid the collision. Karolian stated she had minor injuries to her neck and chest. Karolian's vehicle had a dash cam recording at the time of the accident. I viewed the footage with her through an app on a cell phone. The footage shows MV3 traveling Southbound on Centre Street on August 8, 2019 at 1308 hours. Ahead of MV3 in the Northbound lane, MV2 is seen stopped on Centre Street before Mt. Ida Street with it's left directional activated. I observed MV1 travel Northbound on Centre Street and crash into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane and crashed into the driver side of MV3. The footage then ends. A copy of the footage was emailed to my work email. The footage was then forwarded to IT Director Steve Smith to be added to the media for this report. As a result of my investigation, the operator of MV1, William Tisbert, will be cited for Newton City Ordinance Chapter 19, Section 71 (Following too closely). Massachusetts Uniform Citation T1270942 was completed and mailed to Tisbert's residence.

OPERATOR #1 STATED HE WAS TRAVELLING W/B ON WASHINGTON ST IN THE PASSING LANE WHEN VEHICLE #2 PULLED OUT IN FRONT OF HIM FROM WHOLE FOODS PARKING LOT, #647 WASHINGTON ST CAUSING HIM TO CRASH INTO VEHICLE.#2. OPERATOR #2 STATED HE WAS EXITING WHOLE FOODS, ATTEMPTING TO MAKE A LEFT TURN ONTO WASHINGTON ST HEADING EAST WHEN HE WAS STRUCK BY VEHICLE #1. OPERATOR#2 FURTHER STATED HIS VIEW WAS BLOCKED BY NUMEROUS VEHICLES ENTERING INTO WHOLE FOODS FROM THE W/B SIDE OF WASHINGTON ST FROM THE NON PASSING LANE. HE NEVER SAW VEHICLE #1 COMING IN THE OTHER LANE( PASSING LANE) UNTIL THE CRASH. THERE WERE NO REPORTED INJURIES AND NO VEHICLE WAS TOWED. BOTH VEHICLES SUSTAINED MINOR DAMAGE.

The operator of MV#1 stated that he was traveling south on Grove St in heavy traffic when he stopped in front of #275 Grove St to let a MV enter the roadway. Operator #1 stated that his MV was then rear ended by MV#2. There did not appear to be any damage to MV#1 and there was a strong language barrier when speaking with the operator. The operator of MV#2 had left he scene prior to my arrival and this concerned operator #1. I then made contact with the owner of MV#2 who identified himself as the operator during a very minor crash on the date and time in question. Operator #2 stated he was also traveling south on Grove St directly behind MV#1 when he made slight contact with MV#1. Operator #2 stated he got of his MV, observed that there was no damage to either MV and then left the scene after speak with operator #1. I advised both parties that I would file a MV crash report and that they should notify their insurance provider. No injuries, no tows.

The operator of MV#1 stated that she was traveling east (left lane) on Commonwealth Ave when MV#2 entered her travel lane from #2102 Comm. Ave and crash occurred. The operator of MV#2 stated that he was attempting to enter Commonwealth Ave (west bound) from #2102 Commonwealth Ave and the MV in the right hand lane had stopped to allow him to enter when crash occurred with MV#1. No injuries, no tows.

OPER OF MV#1 STATED THAT SHE WAS TRAVELLING SB IN THE PARKING OF LOT OF #220 BOYLSTON ST, WHEN MV#2 PROCEEDED THROUGH A STOP SIGN (LOT SIGN), AND STRUCK HER VEHICLE. OPER OF MV#2 STATED THAT SHE WAS STOPPED AT A STOP SIGN IN THE PARKING LOT, AND THAT SHE DID NOT SEE MV#1 APPROACHING. MV#1 THEN STRUCK THE SIDE OF MV#2 AS SHE PULLED OUT. MV#1 SUSTAINED MINOR FRONT DAMAGE AND WAS TOWED BY AAA. MV#2 SUSTAINED MODERATED DRIVER SIDE DAMAGE TO THE DOORS AND WAS TOWED BY TODYS. THERE WERE NO INJURIES DUE TO THIS ACCIDENT.

OPERATOR#1 STATED SHE WAS GOING E/B ON CENTRE AVE JUST EAST OF CENTRE ST IN THE FAR LEFT LANE WHEN VEHICLE #2 CHANGED INTO HER LANE CAUSING THE CRASH. OPERATOR #2 STATED SHE WAS IN THE LANE NEXT TO VEHICLE #1 ALSO HEADING EAST ON CENTRE AVE WHEN SHE ATTEMPTED TO CHANGE INTO THAT LANE THINKING SHE HAD ENOUGH CLEARANCE. VEHICLE #2 STRUCK VEHICLE #1 AS SHE CHANGED LANES.

OPERATOR #1 STATED SHE WAS GOING N/B OVER THE CENTRE ST BRIDGE FROM WASHINGTON ST WHEN VEHICLE #2 CHANGED INTO HER LANE CAUSING THE CRASH. OPERATOR #2 STATED HE WAS GOING N/B MERGING INTO TRAFFIC ON THE CENTRE ST BRIDGE WHEN HE STRUCK VEHICLE #1. THERE WERE NO INJURIES DUE TO THIS ACCIDENT AND NEITHER VEHICLE WAS TOWED.

The OP. of MV#1 states while travelling westbound on Washington Street., MV#2 suddenly came out from the side and entered her lane. She tried to accelerate past MV#2 but was still sideswiped. The OP. of MV#2 states she was attempting to leave 1897 Washington St. (Woodland Golf Club) to make a left turn onto Washington St. Traffic was stopped for a nearby red light and a vehicle on Washington St. gave her room to exit. She slowly pulled out into traffic and thought MV#1 was slowing down to let her out as well, that's when there was a collision.

Operator #1 states he was traveling westbound on Auburn St when he collided with vehicle #2, which was pulling out of the driveway of #3 Auburn St. Operator #1 states he did not see vehicle #2 until contact was unavoidable. Operator #2 states he was pulling out of his driveway to turn right onto Auburn St when vehicle #1 hit the left driver's side of the vehicle. Operator #2 did not see vehicle #1 approaching. All parties declined medical attention but vehicle #2 had to be towed to Tody's due to moderate damage to the left driver's side door and bumper. Vehicle #1 sustained minor damage to the front right bumper but was driveable.

On Thursday 8/22/2019 at approx 1608hrs, while assigned to N-494, I responded to 249 Centre St in Newton for a 2 car MVA. Operator of MV1 states he was travelling straight on Centre St SB when MV2, entered his lane of traffic and struck the driver side of his vehicle. Operator of MV2 states that she was taking a left onto Centre St from the Sunoco gas station heading SB. She states that before entering traffic she saw no cars in the SB lane closest to her. She also states the operator of MV1 was distracted while talking to his wife.

On 8/26/19 at 1640 hrs I responded to the intersection of High St and Rockland Pl for a MVA with no injuries. Upon arrival I spoke with both operators who confirmed that there were no injuries. I first spoke with the operator of MV#1 who stated she was driving southbound on High St when MV#2 came out of Rockland PL and struck her driver side door and rear driver side door. MV#1 sustained moderate damage to the driver side of the vehicle. I then spoke with the operator of MV#2 who stated that it was her fault. She was coming down Rockland PL, westbound and didn't see MV#1 and attempted to turn left onto High St but struck MV#1. MV#2 sustained moderate damage to the front of the vehicle.

The operator of MV#1 stated that she was traveling north on Highland St when MV#2 exited the parking area at #60 Highland St and struck her MV. The operator of MV#2 stated she was attempting to exit the parking area of #60 Highland St and didn't see MV#1 when she crashed into her MV. Operator of MV#2 stated, "it's my fault." Operator #2 cited for failure to use care. No injuries, no tows.

On 8/28/2019 at approx 1058hrs while assigned to 497 I responded to the area of 2014 Washington St for a report of a 2 car crash w/o injuries. Upon arrival I observed the vehicles on opposite sides of Washington St with NFD Lt Bourgeois standing by with his emergency lights activated, Lt Bourgeois stated both operators were shaken up but not injured. I spoke with the operator of Ma Reg 1XM680 Elizabeth CHISOLM who stated she was attempting to take a left onto Washington St, a vehicle in the right hand eastbound lane was turning right into the parking lot, CHISOLM stated she did not see Ma Reg EV3452 being operated by Julien GRANT in the left eastbound travel lane until it was too late colliding with him. I spoke with GRANT who stated he was traveling EB in the left travel lane of Washington St when struck by CHISOLM exiting the hospital.

OPMV#1 Was traveling Eastbound on Beacon St when she was struck by the OPMV#2 pulling out of the Gulf Gas Station located at 716 Beacon St. OPMV#1 Stated she was driving straight on Beacon St when a car pulled out of the gas station and collided with her vehicle. OPMV#2 Was pulling out of the Gulf Gas Station attempting to make a right turn onto Beacon St traveling Eastbound when he collided with the OPMV#1. OPMV#2 Stated he looked both ways before making a right turn but believes a utility pole blocked his view causing a blind spot, when he struck a vehicle driving on Beacon St..

On Monday 9/9/19 at approximately 0517 hours while assigned to n499 I was dispatched to the intersection of Dedham and Nahanton Street for a report of a Motor Vehicle Crash. Upon arrival I spoke with the operator of MV1, KAUFMAN Judith, who states that she was travelling eastbound on Dedham Street approaching the traffic light at the intersection of Nahanton Street that was displaying a green light. Kaufman states as she attempted to proceed through the green signal MV2 turned into her lane attempting to make a left turn causing MV1 to strike MV2. MV2 Operator, COLON Marie, states she was travelling Westbound on Dedham Street attempting to turn left onto Nahanton Street. she states the traffic signal displayed a green light with a flashing yellow left turn signal. She attempted to turn left and proceeded into the path of MV1 causing the crash between MV1 and MV2. Neither party was injured and both vehicles were driven away from the crash

The operator of MV#1 stated she was traveling south on Waltham St when she stopped her MV due to traffic in front of her and was rear ended by MV#2. The operator of MV#2 stated that she was also traveling south on Waltham St directly behind MV#1 when according to her MV#1 stopped suddenly while she was adjusting her AC and crash occurred. No injuries, no tows.

Operator #1 states he was traveling southbound on Cherry St when he observed vehicle #2 drive through the stop sign at the end of Harris Rd, resulting in a collision with the front end of his vehicle, vehicle #1. Operator #2 states she was traveling westbound on Harris Rd and did not see the stop sign as she approached Cherry St. She subsequently drove through the stop sign and made contact with vehicle #1. Her intention was to cross Cherry St and go down Cherry Place, which is directly across the street from Harris Rd. No injuries were reported. Vehicle #2 sustained damage to the right rear corner as well as the undercarriage and was towed by Tody's. Vehicle #1 was parked legally on Cherry St and the operator has his own tow coming, as the vehicle is a contracted Waltham school bus van. It should be noted that no students were on board the van at the time of the collision.

Opr of V1 stated he was stopped at the traffic light on the exit 17 exit ramp. V1 stated when it turned green he was traveling through the intersection. Opr of V1 stated he heard a car driving on his left, looked left, observed a dark colored van travel trough the intersection, and made contact with him. Opr of V1 stated V2 drove through the red light. Opr of V1 stated V2 took off on Centre Street towards Watertown, MA. V1 was towed by Tody's. Dispatch was notified to contact Watertown PD. Watertown PD found V2 in Watertown at the corner of Warren and Endenfield. Officer Brandon spoke to the Opr of V2. V2 stated she was nervous so she left the area. Officer Brandon advised her she needs to stay on location next time. Officer Brandon gave the Opr of V2 a copy of the criminal application (T2014370). See incident report # 19043731 for more information. Opr of V2 is being charged with the following: MGL 90/24 Leaving the Scene of an Accident MGL 89/9 Red Light Violation

Operator#1 stated she was going w/b on Cabot St when vehicle #2 turned left in front of her attempting to enter onto Langdon St from Cabot St e/b, causing the crash. Operator #2 stated she was going e/b on Cabot St and was turning left onto Langdon St when she was struck by vehicle #1. Operator #2 further stated she didn't see vehicle #1 coming until it was to late. She thought vehicle #1 was going too fast. There were no injuries and no vehicles towed.

MV#1 was entering Center Street from a parking stall when he was struck by MV#2. -OpMV#1 stated he was entering Center Street from a parking stall when he was struck by MV#2. He stated he observed a far distance buffer between him and MV #2. MV#1 was occupied by two passengers. -OpMV#2 stated she was traveling north on Center Street. She stated she observed a MV#1 entering the roadway. She stated MV#1 pulled out suddenly in front of MV #2. She collided with MV#1. - I observe the damage to both vehicles. I observed the heavy damage and deep gouge to the driver(cid:31)s side door of MV#1. The gouge had cut open the outer panel of the door revealing the mechanical components inside the door. OpMV#1 stated he could not open the door, but the vehicle was drivable. I next conducted an inspection of the exterior damage of MV#2. I observed that the plastic bumper was separated from the frame and several cracks and breaks on the bumper. I further observed damage to the wheel well panel. OpMV#2 stated the vehicle was drivable. -There were no injuries reported at the scene and all parties declined medical assistance. Neither vehicle was towed from the scene. OpMV#1 was cited (T2015553) for Failure to Use Care Entering the Roadway.

Operator#1 stated she was going W/B on Washington St when she attempted to make a left turn into #2366 Washington St,, Lower Falls Wine parking lot but was struck by vehicle #2. Operator #2 stated he was going E/B on Washington St just passing Concord St intersection when vehicle #1 turned left in front of him causing the crash.

Operator #1 stated she was first stopped on Newtonville Ave @ Centre St E/B. She then attempted to pull out into the intersection and make a left turn northbound onto Centre St when vehicle #2 struck her vehicle as she entered out into the intersection. She further stated she thought she had enough clearance to safely enter out onto Centre St. Operator #2 stated she was going southbound on Centre St when vehicle #1 pulled out from a side street( Newtonville Ave) causing her to crash into her vehicle. Operator had no time to react. Witness who was behind vehicle #1 on Newtonville Ave stated, "vehicle just pulled out into the intersection". Both vehicles were towed by Todys. There were no reported injuries.

The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave making a left turn onto Morton St when he was struck by MV#2. The operator of MV#1 stated he was unsure of where MV#2 came from. There was a strong solar glare facing westbound on Commonwealth Ave during this accident. MV#1 sustained heavy driver's side front end damages. Both front airbags were deployed. The operator of MV#1 stated he was not injured. The front passenger of MV#1 was not injured and had already left the scene to attend school. The driver's side rear passenger sustained a bleeding nose from this accident. She was unsure what caused her injuries because it happened too quickly. She was evluated by Newton Paramedics and signed a patient refusal of treatment. The initial point of impact for both vehicles was in the middle intersection of Commonwealth Ave at Morton St. There were broken vehicle parts and tire skid marks starting from the point of impact and onto the berm on Commonwealth Ave on the carriage lane side. MV#2's final resting spot was directly accross from 808 Commonwealth Ave. MV#2 sustained heavy front end damages to its driver's side wheel (Broken axle) and flat front right side tire. MV#2's driver's side airbag was deployed. The operator of MV#2 was transported by Newton Paramedics to the Boston Medical Center. I spoke to the operator of MV#2 via telephone and she stated she was travelling westbound on Commonwealth Ave when she was struck by MV#1 and caused her swerve onto the carriage lane berm. The operator of MV#1 is 6.5 months into her pregnancy and sustained neck, back, bruises and left hand injuries. She was transported to Boston Medical Center and was later discharged the same day. She was provided with this accident report number and was advised to contact her insurance provider. I spoke to the witness who was at the intersection of Cedar St and Commonwealth Ave. The witness stated MV#1 was travelling eastbound on Commonwealth Ave and had stopped, attempting to make a left turn onto Morton St. The witness stated she then observed MV#2 travelling westbound on Commonwealth Ave and struck MV#1. The witness stated she believed MV#2 was travelling at a high rate of speed because it continued to travel onto the berm after striking MV#1. I took digital photographs of the damaged berm on Commonwealth Ave. Based on the statements made to me, I issued in hand to the operator of MV#1 citation #T1268357 for violation of c89 s8 failure to yield when turning left. Both vehicles were towed by "Tody's Towing".

On Saturday, September 28, 2019, I responded to the intersection of Crafts Street and Albemarle Road, Newton with Sgt. Lee for a report of a motor vehicle crash involving a motorcycle with serious injury to it's operator. The weather at the time of the crash was clear and sunny. The road surface was dry. Crafts Street and Albemarle Road are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jussara Pontes (S92930243). Pontes stated she was operating a 2017 Toyota Sienna (MA: 4GX777) at the time of the crash. Pontes stated she was traveling North on Albemarle Road and came to a stop at the stop sign located at the Crafts Street intersection. Pontes stated she looked both ways and began to inch out into the roadway. Pontes stated she stopped the vehicle again and was assisting a child sitting in the 2nd row of the vehicle. Pontes stated she then turned her attention back to the roadway. Pontes stated she looked to her right and left again and did not see any vehicles in either direction. Pontes stated she proceeded to cross the intersection to continue North on Albemarle Road. Pontes stated the front bumper area of MV1 crashed into the left side of a passing motorcycle. Pontes stated the motorcycle and it's operator fell to the roadway and slid across the street to the side of the road. Pontes stated she pulled her vehicle over onto Albemarle Road. Pontes stated she did not see the motorcycle prior to the crash and was not distracted. The operator of the motorcycle (MV2), Irene Pistoftzian (S06099449) was transported to Brigham and Women's Hospital with a serious left leg injury prior to my arrival. I observed a red 1990 Kawasaki ZR550 (MA MCN: 1V1063) resting on it's right side on the NE sidewalk area of Albemarle Road at Crafts Street with it's front wheel/handlebar facing Crafts Street. The motorcycle had moderate damage to it's left side from the impact of MV1 crashing into it. The right side of the motorcycle had damage from sliding on the roadway and the crashing into a fence on the sidewalk on Crafts Street (W) at Albemarle Road. There was also damamge to the front headlight/right handlebar area. The motorcycle and Toyota Sienna were towed from the scene by Tody's towing at Sgt. Lee's request. Pictures were taken of the scene and submitted to the IT Bureau. I spoke with Pistoftzian at Brigham and Women's Hospital in Boston on Monday, September 30, 2019. Pistoftzian stated she was operating her motorcycle on Crafts Street (W). Pistoftzian stated as she approached the intersection of Crafts Street and Albemarle Road, she observed a mini van to her left out of her peripheral. Pistoftzian stated she remembers being hit on her left side and then laying on the roadway looking up at the sky on the sidewalk with her head facing Albemarle Field. Pistoftzian stated as a result of the crash she has a significant injury to her left leg and also has an injury to her left wrist/arm. Pistoftzian stated she was wearing a helmet at the time of the crash. I spoke with Kymberley Mitchell via telephone who stated she witnessed the crash. Mitchell stated she was traveling behind the motorcycle involved in the crash on Craft Street (W) at North Street. Mitchell stated they were stopped at a red light at this time. Mitchell stated there were no vehicles in front of the motorcycle at the light. Mitchell stated the light turned green and they continued on Crafts Street at a low rate of speed. Mitchell stated she could see ahead at the intersection of Albemarle Street and Crafts Street, traffic was stopped on Crafts Street (E) letting traffic exit Albemarle Road (N). Mitchell stated vehicles were taking left and right turns out of Albemarle Road before the motorcycle reached the intersection. Mitchell stated a mini van was crossing past the double yellow line of Crafts Street to continue straight when it crashed into the left side of the motorcycle. I spoke with Cheryl Wermer via telephone who stated she witnessed the crash. Wermer stated she was traveling on Crafts Street (E) towards Albemarle Road. Wermer stated traffic ahead of her was stopped to allow vehicles to turn off of Albemarle Road. Wermer stated she observed a motorcycle traveling towards her on Crafts Street (W). Wermer stated she then saw a vehicle travel straight from Albemarle Road across Crafts Street and crash into the motorcycle. Wermer stated the operator of the motorcycle flew off the bike. Wermer stated the car then came to a stop and the driver got out and stated "Oh my god! I didn't see you!" Wermer stated both vehicles were traveling at a low rate of speed prior to the crash. I spoke with Bill Frabizio via telephone who stated he witnessed the crash. Frabizio stated he was behind the mini van (MV1) on Albemarle Road prior to the crash. Frabizio stated the vehicle in front of him began to cross the intersection as he was monitoring the traffic on to take a right turn onto Crafts Street. Frabizio stated he heard a loud "Boom" and saw a motorcycle slide across the street with its operator. Frabizio stated the motorcycle crashed into a fence post and the operator came off the motorcycle. Frabizio stated he pulled over and assisted operator of the motorcycle. After concluding my investigation, the operator of MV1, Jussara Pontes, was mailed Massachusetts Uniform Citation T2016424 for Newton City Ordinance Chapter 19, Section 75 (Failure To Use Care).

ON 9-29-19 AT APPROX. 0933HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT THE INTERSECTION OF FESSENDEN LANE AND CRAFT ST. I SPOKE TO THE OPERATOR OF VEHICLE #2. DRIVER STATES SHE WAS TRAVELING N-BOUND ON FESSENDEN LANE . AT THE INTERSECTION WAITING TO GO LEFT ONTO CRAFT ST. DRIVER STATES A VEHICLE TRAVELING E-BOUND ON CRAFT TURNED RIGHT INTO FESSENDEN LANE. AT THIS POINT DRIVER STATES SHE ENTERED THE TRAFFIC LANE AND TURNED ONTO CRAFTS ST. SHE STATES HER VEHICLE WAS HIT IN THE LEFT SIDE BY VEHICLE #1. SHE STATES SHE NEVER SAW THE MOTORCYCLE UNTIL IT WAS HITTING HER VEHICLE. OPERATOR OF VEHICLE #1 WAS BEING ATTENDED TO BY THE FIRE AND MEDICS. OPERATOR OF VEHICLE #2 WAS GIVEN MOTOR VEHICLE CITATION ( T1445177, 89/9, FAILURE TO YIELD AT INTERSECTION ). OPERATOR SERVED CITATION IN HAND. I SPOKE TO A WITNESS ( TIMOTHY CHANGOGARRETT ). HE STATED HE WAS TRAVELING E-BOUND BEHIND THE MOTORCYCLE. HE SAW VEHICLE #2 ENTER THE TRAFFIC LANE AND VEHICLE #1 HIT HER VEHICLE. HE STATES THE OPERATOR OF VEHICLE #1 WENT OVER HIS HANDLE BARS UPON COLLISION. LANDED ON THE HOOD OF VEHICLE #2 AND THEN FELL TO THE GROUND. AT THIS POINT HE CALLED 911. OPERATOR OF VEHICLE #1 WAS TRANSPORTED TO BRIGHAM AND WOMEN'S HOSPITAL BY MEDIC2 FOR FURTHER TREATMENT. I WENT TO THE HOSPITAL AND SPOKE TO THE OPERATOR IN HIS ROOM. HE STATES HE WAS TRAVELING E-BOUND ON CRAFT INTENDING TO GO TO THE POST 440 ON CALIFORNIA ST. TO ATTEND AN AA ( ALCOHOLICS ANONYMOUS ) MEETING. HE STATES HE TOOK HIS SON'S MOTORCYCLE WITH THE INTENTION OF SEEING IF THERE WERE ANY REPAIRS NECESSARY BECAUSE HIS SON IS TRYING TO SELL IT. WHILE TRAVELING HE APPROACHED THE INTERSECTION. HE SAW VEHICLE #2 ENTER ONTO CRAFT ST. HE STATES HE COULD NOT DIVERT AND HE WAS UNABLE TO AVOID HITTING HER VEHICLE. OPERATOR HAS BROKEN BONES IN HIS LEFT ARM. CUTS ALONG HIS FACE AND NECK AS WELL AS LOWER BODY PAIN. HE STATED THE HOSPITAL INTENDS ON KEEPING HIM OVER NIGHT FOR OBSERVATION. I ASKED THE DRIVER IF HE WAS AWARE THAT HIS LICENSE TO OPERATE A MOTOR VEHICLE WAS REVOKED. HE STATED HE HAD A CHANCE TO BE REINSTATED BUT CHOSE NOT TO. DRIVER WAS ISSUED MOTOR VEHICLE CITATION ( T1445178, 90/23/C, LICENSE REVOKED AS HTO ). I INFORMED THE OPERATOR THAT I WOULD MAIL HIM THE CRIMINAL APPLICATION. ALL OTHER PARTIES REPORTED NO INJURIES. BOTH VEHICLES WERE TOWED BY TODYS. VEHICLE #1 HAD FRONT END DAMAGE AND FRAME DAMAGE. VEHICLE #2 HAD LEFT SIDE DAMAGE AND A BROKEN AXLE. WEST NEWTON SQUARE ( OFF L. MIKOLEIT ) ARRIVED ON SCENE AND TOOK 10 PICTURES. PICTURES PLACED IN I.T. BOX. I CLEARED THE AREA WITHOUT FURTHER INCIDENT.

The OP. of MV#1 states she was travelling northbound on Chestnut Street and noticed MV#2 slowing down to a stop at the intersection of Gordon Road. She thought MV#2 was going to remain stopped there waiting for traffic so she continued forward and that's when she felt a collision. The OP. of MV#2 states he was on Gordon Road wanting to make a right-turn to Chestnut Street. He did not see any oncoming traffic and made the turn, suddenly colliding with MV#1. Tere were no injuries due to this accident and vehicle#2 was towed away by Todys.

Based on observations, and statements made, the following occurred. The operator of M/V#1 was traveling east on Boylston St. and took the Ellis St. exit. He stated that he remained in the left lane, intending to go straight after the stop sign. He stated that he stopped, then began to proceed through the Ellis St. intersection, when he collided with M/V#2 which was traveling North. He stated that he did not realize that there was no stop sign under the Boylston ST. overpass. The operator of M/V#2 stated that he was traveling south on Quinobequin Rd., and stopped at the stop sign, then proceeded through. As he was traveling under the Boylston St. over pass, to proceed onto Ellis St. M/V#1 entered the intersection and collided with him.

On 10/04/19 at 11:49 hours while assigned to Traffic Unit 523, I responded to a motor vehicle/bicyclist accident at Centre Street and Nathan Road. The weather at the time was cloudy. The road surface was dry and Centre Street and Nathan Road are public ways maintained by the City of Newton. Traffic conditions were heavy and Centre Street was congested with vehicles at the time. There is no dedicated bicycle lane in this area but there are bicycle indicators painted on the roadway to make drivers aware of bicyclists traveling. I spoke to the bicyclist involved (Unit 2), Marvin Goldman (S67373495). Goldman was wearing a helmet, gray sweatshirt, gray shorts, and black/orange reflective sneakers. Mr. Goldman stated that he was traveling southbound on Centre Street on the right side of the road on his black Fuji Sagres bicycle. He further says that he was peddling fast at the time while crossing over Nathan Road and that is when he suddenly struck Unit 1 on it's passenger side front quarter panel/door area. He does not recall any other details of the incident along with Unit 1 attempting to turn left towards him. Goldman sustained scraped up knees and a small laceration below his left eye. He refused medical attention signing a patient refusal with Cataldo Medic One. I observed that his front wheel was bent as a result of the accident. The rest of the bicycle appeared to be intact. I then spoke to the operator of Unit 1, Darlene Martins (S27966454). Ms. Martins spoke with the assistance of Ofc. Calderon who was translating for her. She says that she was traveling northbound on Centre Street and was attempting to turn left on to Nathan Road in order to turn around and travel the other way on Centre Street. She says that a vehicle in the southbound travel lane left space for her to make her left turn. She then proceeded to turn thinking it was clear and that is when the bicyclist suddenly approached and struck her on her passenger side. She also stated that it was hard to see past the congestion of vehicles waiting in the southbound travel lane. I observed scratch marks and a dent to her passenger side front quarter panel area along with minor scuff marks to her front passenger door. The scuff marks appear to be from the bicyclist rubbing against the vehicle. I also observed a dent on the top of the passenger door frame. It is unknown if the damage is the result of the accident or old damage Ofc. Calderon spoke to Rosangela Dias (Passenger in Unit 1), who stated that everything happened so quick but she never saw the bicyclist approaching when Martins attempted to turn left. I then spoke to a witness on scene identified as Matthew Miner. He says that he was two vehicles behind an unknown vehicle in the southbound travel lane of Centre Street. He says that the unknown vehicle left space for Unit 1 to turn left and waved Unit 1 on to turn left. He does not recall what transpired with the bicyclist. As a result of the accident, it was determined that Martins did not yield the right of way to the bicyclist while she was turning left. She also does not have a license to operate in Massachusetts or any other state. She was issued MA Uniform Citation # T2012393 for Ch. 89/8 Failure to yield and Ch. 90/10 Unlicensed Operation of a Motor Vehicle. Digital photos taken by me of the scene.

Operator of vehicle 1 stated he was traveling eastbound on River St and was taking a left turn onto Cherry St and was struck head on with vehicle 2. Operator of vehicle 2 stated she was traveling westbound on River St and as she was entering the intersection of Cherry St the traffic light turned yellow and vehicle 1 which was traveling eastbound made a sudden left turn leaving her no time to stop and the vehicles collided. Vehicle 1 then sped off northbound on Cherry St and returned a minute or two later. I spoke to a witness, Jena Castro-Casbon who stated she saw vehicle 1 traveling northbound on Cherry St at a high rate of speed and drop of a child at 187 Cherry St then turn around and return to the accident scene at a high rate of speed again. 187 Cherry St is 0.4 miles from the intersection of River St. Operator of vehicle 1 stated he had two of his sons friends in the vehicle and he thought it would be best to get one of them home to 187 Cherry St and then come back to exchange information. I followed up with the child and his parents at 187 Cherry St and he was uninjured. I issued the operator of vehicle 1 MA Uniform Citation #T1445381 for 89/8 failure to yield. No one reported any injures and neither vehicle required a tow.

Vehicle 1 was traveling west down Clements St and attempted to cross the intersection at Waverly Ave onto Mandalay Rd. Vehicle 1 thought she had enough time to cross the intersection but failed to do so and crashed into Vehicle 2. Vehicle 2 was traveling north on Waverly Ave when she was struck by Vehicle 1 attempting to cross the intersection.

Operator of vehicle 1 states he was traveling eastbound on Webster St when vehicle 2 pulled out of Border St in front of him. Operator of vehicle 1 tried to swerve to avoid a collision but was unable to. Operator of vehicle 2 stated she was pulling out of Border St to take a left onto Webster St when she was struck on the drivers side by vehicle 1. All operators and passengers were checked by the medics. Only the operator of vehicle 2 went to Newton Wellesley Hospital with Cataldo Medics. Both vehicles were towed by Todys.

Operator#1 stated he was travelling n/b on Washington St and was about to pass by the intersection of Waverly Ave when vehicle #2 pulled out into the intersection from Waverly Ave causing the collision. Operator#2 stated she was first stopped at the intersection of Waverly Ave @ Washington St. She then attempted to make a left turn onto Washington St, to go n/b when she struck vehicle #1 in the intersection. She never saw vehicle #1 coming. Witness reported seeing vehicle #2 pull right out into the intersection causing the crash.

Operator of vehicle two stated that while tuning from the west bound lane of Watertown St onto the north bound lane of Eddy St vehicle one struck the rear driver's side of his vehicle. Operator of vehicle two stated that he was not injured and his vehicle did not require a tow. Operator of vehicle one stated that while driving east bound on Watertown St she could not stop her vehicle time to prevent it from crashing into vehicle two. Vehicle one had damage to the front passenger side and did not require a tow. Operator of vehicle one stated that she was not injured and her vehicle was towed by AAA. Witness Christina Colantonio stated that she was behind vehicle two and saw vehicle one " speeding and crash into the rear passenger side of vehicle two. Operator of vehicle one was cited for CO 19/75, failure to use care. Watertown St and Eddy St are pubic ways in the city of Newton.

The operator of MV #1(Ma Reg 7TR785) stated she was traveling eastbound on Commonwealth Ave. when she attempted to turn left onto Temple St. Operator of MV#1 stated her vehicle was struck as she turned onto Temple St by the operator of MV #2. The operator of MV#2 (Pupils reg 44301) stated he was traveling westbound on Commonwealth Ave. approaching the intersection of Temple St. Operator of MV2 stated as he was traveling straight ahead he observed MV1 start to turn left onto Temple St. Operator of MV2 stated he hit the breaks and attempted to swerve out of the way but ended up hitting MV1. MV #2 struck MV #1's passenger side rear bumper causing minor damage. MV #2 sustained damage to its passenger side front bumper. No injuries were reported and both vehicles were able to be driven away from the scene. The operator of MV #1 will be mailed a citation (T2079066) for MGL. Ch. 89 Sec. 8; Failure to yield to oncoming traffic while turning left.

On 10/18/2019 at approx 0727Hrs while assigned to 497 I responded to the intersection of Carleton Rd and Waban Av for a report of a two car crash . Upon arrival I found both vehicles pulled to the side of Waban Av. I spoke with the operator of Ma Reg 6309KI, Morgan BURNS who stated she was travelling straight on Waban Av when Ma Reg 3SS575 pulled out from Carleton Rd striking her. I spoke with the operator of Ma Reg 3SS575 Maria Munoz who initially stated she had stopped for the stop sign but it was difficult to see as the road is off set than later stated she stopped but that BURNS was travelling to fast.

Operator of MV1; MA reg 9RXW70 (2017 Hyundai Elantra red) said he was driving southbound on Lowell Ave when he was hit by MV2; MA reg BC1995 (2013 Ford Fusion black), who was taking a left turn off of Austin St turning westbound. Operator of MV2 stated he was driving westbound on Austin St and stopped at the Lowell Ave intersections. Operator of MV2 said he looked both ways before trying to take a left onto Lowell Ave southbound but struck MV1. MV1 sustained damage to the driverside wheelwell and driver side door. MV2 had front end damage. MV1 was towed by Tody's. No injuries.

OPER OF MV1 WAS TRAVELLING EB ON LAGRANGE ST, WHEN MV2 PROCEEDED (NB) FROM BRYON RD, COLLIDING WITH HER VEHICLE (PASS SIDE). MV2 WAS TURNING ON TO LAGRANGE ST, FROM BRYON RD, WHEN SHE STRUCK THE PASSENGER SIDE OF MV1. OPER OF MV2 STATED THAT MV1 HAD THEIR RIGHT TURN INDICATOR ON, AND BELIEVED THAT MV1 WAS GOING TO TURN. MV1 SUSTAINED MODERATE PASS SIDE DAMAGE. MV2 SUSTAINED MINOR FRONT DAMAGE.

Operator #1 reports she was pulling out from a parked position near #10 Austin St when she struck a passing vehicle (#2 MV). Operator #2 stated she was going e/b on Austin St when vehicle #1 pulled out into traffic from a parking spot striking her vehicle.

MV #1 was traveling northbound on Lagrange St. MV #2 was traveling southbound on Lagrange St. when the operator turned eastbound onto Corey St.(Boston). As MV#2 turned eastbound it collided with MV #1. Significant damage was observed to both MV#1 and MV#2's front bumpers and engine compartments. Both MV#1 and MV#2's airbags were deployed upon impact. The Operator of MV#1 reported she was uninjured while the Operator of MV#2 was transported to the hospital. Both vehicles were towed from the scene by Tody's Tow Service.

OPMV#1 Was traveling Eastbound on Beacon St when she collided with the OPMV#2 at the intersection of Beacon St at Woodward. OPMV#1 Stated she was traveling straight and all of a sudden a car pulled out in front of her causing the accident. OPMV#2 Stated she was stopped at the intersection of Beacon St and Woodward St waiting to take a left turn onto Beacon St. OPMV#2 proceeded out into Beacon St and was struck by the OPMV#1. OPMV#2 Stated she observed a vehicle traveling Eastbound on Beacon St taking a right turn onto Woodward St and thought she had enough time to take a left turn onto Beacon St. She stated when she entered the intersection she was struck by a vehicle on Beacon St. OPMV#2 Was transported to NWH. Both vehicles were towed by Tody's Towing.

Operator of MV1 (Mass. Reg. 9VV485) states he was traveling east bound on Commonwealth Ave when MV2 pulled out into his lane of travel onto Commonwealth Ave from Temple street causing him to collide with MV2. Operator of MV2 states she was traveling south bound on Temple Street when she crossed over the Commonwealth Ave carriage lane and then Commonwealth Ave. She states there were no cars coming on Commonwealth Ave when she entered the roadway and MV1 suddenly appeared and was traveling at atleast 60 mph and collided with her.

Operator of vehicle 1 states she was traveling west on Derby St when vehicle 2 pulled out from Parmenter Rd. Vehicle 2 struck the right passenger side door of Vehicle 1 and continued to turn onto Derby St., traveling west. Vehicle 1 followed vehicle 2 into Waltham and obtained a photo of the license plate of vehicle 2. Operator of vehicle 1 reports no injuries. I attempted to contact the operator of vehicle 2 with negative results. Waltham PD was unable to make contact with the registered owner of vehicle 2 and vehicle 2 was not at the registered address. Citation to be mailed to registered owner of vehicle 2 for 90/24/c leaving the scene after a motor vehicle crash with property damage, as well as 89/9 failure to yield right of way.

Opr of V1 stated he was operating west on Auburn Street. Opr of V1 stated V2 started to take a left turn out of Woodbine Street. Opr of V1 stated V2 pulled out and he made contact with V2. V1 suffered minor damage. Opr of V2 stated she was attempting to make a left turn out of Woodbine Street. Opr of V2 stated she looked both ways and did not see any cars. At this time, Opr of V2 pulled out and made contact with V1. V2 sustained damage to the left and left front side. V2 was towed by Todys's. No injuries reported.

Vehicle 1 was traveling Southbound on Beach St. Vehicle 1 came to a stop at Beach St. and Washington St. before attempting to take a left turn onto Washington St. When Vehicle 1 crossed into the far right westbound travel lane onto Washington St. it was struck by Vehicle 2 on its left driver side. Vehicle 2 was traveling Westbound in the far right travel lane on Washington St. Vehicle 2 while traveling straight ahead when it collided with Vehicle 1 who was attempting to cross the westbound travel lanes and turn east onto Washington St. Vehicle 1 sustained minor damage to it's driver side. Vehicle 2 sustained minor damage to it's front end. The Owner of Vehicle 2 decided to have AAA tow the vehicle from the scene. No drivers or passengers were injured.

Motor vehicle 1 (MV1) was traveling eastbound on Hammond St. when motor vehicle 2 (MV2), pulled out of Hammondswood Rd. (southbound) and crashed into the front end of MV1. As a result of the crash both MV1 and MV2 sustained significant front end damage. The Cataldo medics responded to the scene, and all parties involved signed patient refusals.

On Sunday 11/24/19 at approximately 1730 hours while working in marked unit n499 I was dispatched to the parking lot on the rt9 side of 225 Boylston Street (Bloomingdales) for a report of a minor motor vehicle crash. Upon arrival I spoke with the operator of motor vehicle 2, GIRARD Kathleen, who states that she was driving from the 2nd aisle of parking spots into the main travel lane to attempt to exit the mall. When she attempted to turn left she didnt see motor vehicle 1 already in the lane of traffic striking the side of the vehicle. After speaking with GIRARD I spoke with the operator of motor vehicle 1, SONG Guang min. SONG did not speak english, but he corroborated what GIRARD had stated by using a translation app on his cell phone. GIRARD had damage to the front of her vehicle and SONG had damage to the drivers side of his vehicle. Both vehicles were still operable and were driven from the scene. Neither party was injured

MV 1 was changing lanes from the right lane to the left lane southbound on Centre St. When MV 1 changed lanes in front of MV 2, he was rear ended by MV 2. MV 2 was traveling straight southbound on Centre St when MV 1 changed lanes and crossed in front of her. At this time she rear ended MV 1. There were no injuries to either party and each operator was advised with an accident report number.

Operator of MV1 states he was in the intersection waiting to take a left from Washington Street westbound to Highland Street southbound. Operator of MV1 stated the light was changing and he thought he had time to make his turn. MV1 was struck in the right rear by MV2. MV2 states she had a green light and was traveling on Washington Street eastbound when MV1 turned in front of her. MV2 did not have enough time to stop and struck the right rear of MV1 with the front of MV2. It should be noted it was snowing and there was also snow present on the roadways. MV1 was towed by Tody's. No injuries reported

The operator of MV#1 stated that he was traveling east on Commonwealth Ave near Lexington St when his MV was rear ended by MV#2. The operator of MV#2 stated he was also traveling east on Commonwealth Ave directly behind MV#1 when he states, "he lost focus" and crashed into MV#1. No injuries, no tows.

MV 1 was traveling Westbound on Beacon St when MV 2 pulled out of the driveway of 853 Beacon St and struck his right rear side. MV 2 was traveling Southbound entering traffic from the 853 Beacon St driveway when she struck the right side of MV 1. The operator of MV 2 suffered minor back injury and was transported by Cataldo Ambulance to Newton Wellesley.

On Monday, December 2nd, 2019 at approximately 1828 hours I, Officer Newton, and Officer Brooks responded to a 3 car motor vehicle accident at Watertown St and Eddy St. The operator of vehicle 1 stated he was stopped at the stop sign on Eddy St southbound when he proceeded to pull out to cross Watertown St when Vehicle 2, traveling eastbound on Watertown St, hit their car causing them to also collide with vehicle 3. The operator of vehicle 2 stated she was driving eastbound on Watertown St. when vehicle 1 suddenly pulled out in front of her causing the collision. The operator of vehicle 3 stated she was at the stop sign on Eddy St northbound, opposite of vehicle 1's stop sign on Eddy St, when Vehicle 1 pulled out and caused vehicle 2 to hit vehicle 1 and vehicle 2 to hit her. All parties involved declined medical attention. All 3 vehicles were able to safely drive away from the scene.

MV1 was heading EASTBOUND on Washington St when the turned onto Cross St from the left lane. MV1 stated that he was distracted by a driver to his right weaving in and out of the lines so when he took his left hand turn, he didn't see MV2 approaching him from the oncoming lane. There were no witnesses to the crash. MV2 was heading WESTBOUND on Washington St in the left lane when it collided with MV1. Both parties signed refusals with the medics. Both vehicles were towed by Tody's.

Operator #1 states he was driving northbound on Seminary Ave in MA reg 5LE114 (vehicle #1), when he was t-boned on the driver's side by MA reg 959SD7 (vehicle #2). Operator #2 states that he was attempting to turn left out of the Arnow Campus Center parking garage and did not see vehicle #1 approaching from the right. It should be noted that the physical address of the parking garage appears to be 131 Woodland Rd, however the collision occurred on the Seminary Ave side. All parties declined medical attention and no tows were needed. Vehicle #1 sustained significant damage along the driver's side doors. Vehicle #2 has moderate front end damage. Operator #2 was issued Ma citation T1268432 in hand for Failure to Yield to Oncoming Traffic. 131 Woodland Rd will be the address used in documenting this crash.

On 12/10/2019 at approx 1036hrs while assigned to 497 i responded to the area of 2102 Commonwealth Av for a report of a 2 car crash. upon arrival I observed Ma Reg V10183 a red Ford F150 pick up with front plow partially on the curb blocking the EB lane of Commonwealth Ave and Ma Reg 8PE124 a black Ford Focus pulled off to the side WB with extensive front end damage. I spoke with the operator of the pick up, Leonardo TAPIA who stated he had been travelling WB on Commonwealth Av when the Ford Focus pulled out in front of him causing the collision. I spoke to the operator of the Ford Focus John KELLY who stated he was taking a left out of the parking lot of 2102 Commonwealth Av, traffic in the EB lanes stopped to let him out, as he was entering the WB lane he collided with the pick up. Ford Focus towed by Todys, both operators signed refusals with Cataldo.

Operator of mv1 states she was traveling west on Newtonville Ave. when mv2 reversed quickly onto Newtonville Ave. into the front of mv1. The operator of mv2 states he was backing up onto Newtonville Ave. from Bellevue St. when he was struck by mv1. Both vehicles were towed by Tody's. Both operators where evaluated by medics and signed refusals.

Oper # 1 stated he was driving slowly EB on Commonwealth ave when MV #2 pulled out of Chestnut Ter and struck his vehicle. Oper # 2 stated he stopped at stop sign and he thought MV#1 stopped to let him out and then when he started pulling out MV#1 started moving again causing a collision. No Injures. No Tows.

On December 19th, 2019 at approximately 07:19 hours while assigned to N-491 cruiser I responded to the intersection of Walnut St @ Lindwood Ave for a report of a two vehicle crash. On my arrival I spoke with both operators involved in this crash. Operator # 1 reported that she was travelling S/B on Walnut St when vehicle #2 came out from Lindwood Ave heading W/B and crashed into her vehicle in the intersection. Operator #2 reports that he was stopped on Lindwood Ave @ Walnut St(stop sign) attempting to get to the other side of the intersection to continue W/B on Lindwood Ave. He further stated he had looked both ways and didn't see any oncoming traffic. He then proceeded to cross Walnut St heading to the other side of Lindwood Ave when he crashed into vehicle #1 in the intersection. There were no reported injuries, Vehicle #1 was towed by a private tow company. Vehicle #2 was able to drive away.

MV #1 was traveling southbound on Walnut St. in the area of Lake Ave. MV #2 was parked along the curb on the shoulder of Walnut St. by Lake Ave. MV#2 attempted to pull out into the southbound travel lane of Walnut St. failed to yield to oncoming traffic and struck MV#1 on its passenger side. MV#1 suffered minor damage to its passenger side and no occupants were injured. MV#2 suffered minor damage to its driver's side front end and driver side wheel well. MV#2 was towed by Tody's at the owners request. The operator of MV#2 was cited with MGL 89-8 Fail to yield to oncoming traffic (citation #T2080186). A towed MV report was filled out and filed.

The operator of MV#1 stated that he was traveling west on Washington St (#2281) in the left hand lane when MV#2 entered his travel lane from the entrance/exit of #2281 Washington St and crash occurred. The operator of MV#2 stated that she was attempting to exit the property at #2281 Washington St and turn left when the MV in the right hand lane (west bound) had stopped to allow her to enter. MV#2 proceeded to enter Washington St when crash occurred with MV#1. No injuries, no tows.

Operator #1 stated he was going S/B on Walnut St and was slowing to a stop as he approached the intersection of Austin St due to traffic stopped ahead for pedestrians crossing the street. Howeveras vehicle #1 was slowing down as he approached the stopped traffic, Vehicle #2 came out of the other side of Walnut St, N//B side and struck his vehicle on his rear drivers side as he was attempting to turn left, S/B onto Walnut St. Operator #2 stated he came out from Newtonville Ave and was crossing the N/B side of Walnut St attempting to go S/B on Walnut St when he crashed into vehicle #1 as he was turning left(S/B) onto Walnut St. Operator #2 further stated he never saw vehicle #1 coming prior to making his turn. However he did state he saw the two vehicles stopped further south by Austin St.

On 12/23/19 at 12:06 hours, I came across a two car motor vehicle accident that already occurred at California Street and Bridge Street. No injuries reported. Both streets are public ways and maintained by the City of Newton. Operator of M/V # 1 states he was traveling straight ahead (EB) on California Street through the intersection at Bridge Street. He states that he had the green light signal and attempted to drive through when he was struck by M/V # 2 on his front bumper on the driver's side. He further says that M/V # 2 was attempting to take a left turn on to Bridge Street at the time of the accident. I observed moderate damage to the driver's side front bumper/quarter panel area but the vehicle was still driveable. Operator of M/V # 2 states through translation by her husband (Huseyn Imamaliyev) she was operating (WB) on California Street and was attempting to take a left turn through the intersection on to Bridge Street. She says that she stopped first and then attempted to turn left and that is when M/V# 1 crashed in to her. I observed minor damage to the middle front bumper/registration plate area. Based on the locations and damage to the vehicles, it appears that the operator of M/V # 2 did not yield the right of way when turning left on to Bridge Street. The operator of M/V # 2 (Gulnar Imamaliyeva MA ID# SA1320134) also could not provide a driver's license to operate a motor vehicle in Massachusetts. A query of her revealed that she only had a Massachusetts identification. She was issued MA Uniform Citation # T2012157 for Ch. 89/8 Fail to Yield Right of Way When Turning Left and Ch. 90/10 Operating a M/V Without a License.
