The operator of MV2 (MA Reg: 5NHV50) stated that he was travelling westbound on Washington St when he crashed into the rear of MV1 (MA Reg:6EZ983) which was parked and unoccupied. MV2 stated that he had just worked his first overnight shift and was very tired. MV2 stated that he may have fallen asleep at the wheel momentarily and crashed into MV1. The owner of MV1 was inside his home at 1177 Washington St and came outside when he heard the accident happen. The operator of MV2 sustained minor injuries and signed a patient refusal. MV2 sustained heavy front end damage and was towed by Tody's. MV1 sustained heavy rear end damage. MV2 was issued MA Uniform Citation T1272257 (c89/s4A).

Based on observations, and statements made, the following occoured. M/V#1 was travelling west on Wheeler Rd. The operator stated that she had been suffering from a migrane, and was fatigued, and believe she may have dosed off. The M/v crossed into the left lane, and struck M/V#2 which was parked in front of 44 Wheeler Rd. Citation #T2013109 was issued to the operator of M/V#1 for MGL 89/4/A Marked lanes violation.

The operator of MV1, Mr. Tong Zhi, stated that he was traveling west bound on Needham street when he fell asleep causing him to lose control of the vehicle and crash into a Verizon utility pole 305/17. The motor vehicle sustained major front end damage and full airbag deployment. The utility pole was snapped, and was leaning as a result of the impact. Mr. Zhi stated that he had wrist and chest pain due to the crash and was transported to Newton Wellesley Hospital by Cataldo. Todys arrived on scene and towed the vehicle. Verizon was notified regarding their damaged utility pole. Photographs of the crash were taken and place in the IT Bureau's mailbox. Due to the facts that Mr. Zhi was asleep at the wheel while traveling on a major public way in the City of Newton, that he crossed the double yellow line into the on coming travel lane, and subsequently crashed into a utility pole causing heavy damage, Mr Zhi is being charged with 90-24 Operating to Endanger. A Massachusetts Uniform Citation, #T2080141, was completed back at the station for the offenses of 89-4A Marked Lanes violation and 90-24 Operating to Endanger. The violator's copy of the citation was placed in the Chiefs mailbox to be mailed to the offender while the criminal application portion was placed in the court prosecutors mailbox attached to this report.

Operator of MV1 stated he was driving westbound on Homer Street when he looked down at his car radio and struck a MV2, which was parked unoccupied on the right side of the roadway in front of 301 Homer Street. The MV2 was located to the right of the fog line. MV1 sustained damage to the front passenger wheel and axle. MV2 sustained heavy rear end drivers side damage. Both vehicles were towed by David Donahue of Tody's Towing. Operator of MV1 declined medical attention. Operator of MV1 consented to perform Standardized Field Sobriety Tests. He stated he did not have any medical conditions that would prohibit him from completely the testing properly. He was taken to the side of the road on Homer Street that was well lit by a street light, and we used the fog line as a guide. Operator of MV1 completed the testing and it was determined that he was not under the influence. He did state that he had been up since 0630 and was fatigued while driving. Operator of MV1 was provided a ride home by N-493 and was issued Citation #T2079603 for 90/13-Impeded Operation (Using Radio while driving) and 89/4A-Marked Lanes Violation (Crossing fog line).
