All operators state MV3 was stopped in traffic, with MV2 stopped in traffic behind it. Both vehicles were heading southbound on Centre St. MV1 failed to stop behind MV2, colliding with the rear of MV2. MV2 was sent forward from the impact and collided with MV3. No injuries were reported on scene and no tows were needed. On my arrival all parties had exchanged information. After further investigation it was determined the operator of MV2 was driving with a suspended license. I spoke to her via phone and she stated she thought it was cleared up. I advised her that I would be mailing a citation to her (Citation #T1441680) for operating after a suspended license. She was driven away from the scene by a neighbor (Michael Manna, S20209078).

Vehicle One stated while travelling Westbound on Commonwealth Ave he was rear ended by Vehicle Two. Vehicle One stated he stopped when an unidentified vehicle was attempting to turn left in front of him when the collision occurred. Vehicle Two stated he was traveling Westbound on Commonwealth Ave when he attempted to reach for a beverage then traffic stopped abruptly in front of him. Vehicle One had two passengers in the rear one of which was transported to NWH along with the operator for their injuries. Vehicle Two's operator was transported to Framingham Union Hospital for his injuries. Vehicle Two was towed by Todys. On Monday, November 4th, 2019 at approximately 1602 hours I, Officer Newton and Officer Healy responded to a motor vehicle accident at 22 Randlett Park. Upon arrival we spoke to the operator of vehicle 1. He stated the accident occurred on Watertown st. He was stopped at the traffic light on Watertown St at Walnut St when he was rear ended by vehicle 2. He attempted to pull over and speak to the operator of vehicle 2 but vehicle 2 proceeded to keep driving westbound on Watertown St. Vehicle 1 followed vehicle 2 until vehicle 2 drove to his residence at 22 Randlett Pk. When speaking to the operator of vehicle 2 he seemed confused. He could not answer questions regarding the accident until he was asked multiple times. He stated he was driving from Newtonville to his residence. He stated that he did not realize he was involved in an accident so he proceeded to drive to his residence. It should be noted that in our observations of Mr. Usher, it did not appear that he had a chemical impairment at this time, although he displayed confusion, and shaky hands as we spoke to him. Mr. Usher's mother stated that he had been released from a rehab facility recently, and has "not been himself". As a result of my investigation, Mr Usher was issued Citation#T2079939 for a violation of M.G.L.Ch.90/24-Leaving the Scene Property Damage. Based on the totality of the circumstances, an Immediate Threat Request was faxed to the RMV, see Incident number 19047444.

On 2/14/20 at approximately 1809 hrs I responded to the area of 455 Nahanton St for a report of a MVA with air bag deployment. Upon arrival I spoke with the operator of MV#2 who stated she was not hurt. She further stated that the vehicle in front of her, MV#1, stopped suddenly because a deer ran across the street in front of him while he was driving. The operator of MV#2 stated she tried to brake in time but couldn't avoid collision with MV#1. I then spoke with the operator of MV#1 who stated the same story, he was traveling WB on Nahanton St when a deer ran across the road. He slammed on his brakes to avoid hitting the deer and was then rear-ended by MV#2. There were no injuries on scene, however the medics did respond and the operator of MV#2 signed a patient refusal. MV#2 was towed due to heavy front end damage. the operator was given in hand MA citation T2080487 for C.O violation 19-71; Following too Closely.

Narrative:
On Saturday, 4/3/2021, at approximately 0400 hours, I Ofc Murphy and 492 Ofc Helms were dispatched to a MVA hit and run on 767 Washington St. The victim and operator of MV 1, Mr. Jesse Huggins stated he was rear ended and MV 2 continued eastbound on Washington St and took a right onto Harvard St. I arrived on scene and spoke to Jesse. Jesse stated the accident occurred further westbound on Washington St at the approximate location of 821 Washington St. Jesse stated MV 1 was traveling eastbound on Washington when he was rear ended by MV 2. MV 2 continued driving eastbound on Washington St and took the first right hand turn onto Harvard St. MV 1 sustained heavy damage to the rear and both rear tires. Jesse was not injured from the accident. MV 1 was towed by Todys. A towed motor vehicle sheet was filled out and filed in the proper basket. Ofc Helms and Sgt Fleming followed a fluid trail from Harvard St to Potter St where they located a red mid sized sedan matching the description of the vehicle with heavy front end damage. After MV 1 was towed away, I arrived on Potter St. MV 2 sustained major front end damage. The radiator was still hissing from the fluid leak that had lead officers to the location. The vehicle is registered to Mr. Jordan Greeley of Avon MA. Attempts to locate the operator of MV 2 yielded negative results. Dispatch contacted the Avon Police Department and requested they check for him at his address of 654 West Main St in Avon MA. At approximately 0530 hours, Avon police spoke with Jordan's mother who stated he was not home. Debris from MV 2 was collected at the scene of 821 Washington St and recorded into property and evidence. See incident report number 21014394. This incident will be further investigated. On Saturday, 4/3/2021, at approximately 1500 hours, Mr. Kevin Pineda called the station and admitted he was operating MV 2. Kevin is the brother in law to the owner of MV 2, Jordan Greeley. At approximately 1600 hours, Kevin came into the station and spoke with me. Kevin stated he was driving MV 2 home from work to his brothers house 28 R Meadow St from the job site on Calvary St in Waltham. Kevin further stated he remembered being behind MV 1 on Washington St and began to fall asleep behind the wheel near the intersection of Washington St and Walnut St. At 821 Washington St MV 2 rear ended MV 1. Kevin stated the impact woke him up and panicked him. Kevin stated he was scared because he has a bad driving record and did not want to lose his license again. Kevin further stated he continued operating MV 2 Eastbound on Washington St to Potter St. Kevin was issued Massachusetts Uniform Citation T 2080797 for MGL Ch 90/24(2) Leaving the Scene of property damage. For crashing into MV 1, acknowledging the crash and continuing to operate MV 2 away from the scene, MGL Ch 90/ 24(2)A Operating to Endanger. For falling asleep at the wheel and creating an unsafe environment for other drivers, MGL Ch 85/36 Spilling or inadequately covered load for continuing to operate MV 2 while fluid leaked from radiator onto the road way. All streets listed are public ways in the City of Newton. Due to Kevin's driver history and today's incident, an immediate threat form was filled out and emailed to the RMV.

Driver #1 stated she was driving southbound in front of 437 Centre St in slow traffic when Driver #2 rear ended her. She stated Driver #2 got out of the vehicle, looked at her car and stated he did not see any damage. She asked for his license and registration which he declined to provide. Driver #2 then drove away. Vehicle #1 suffered minor damage to the rear and was driven from the scene. Driver #1 was able to provide me with a picture of the vehicle license plate and a description of Driver #2 which matches the RMV image of the registered owner of Vehicle #2. I attempted to locate a number for Driver #2 with negative results. I also contacted Waltham PD for information for Driver #2 and they were unable to provide anything further. I mailed Driver #2 Citation #2081383 for Chapter 90 Section 24 Leaving the Scene of Property Damage.

Operator of MV1 stated she was traveling Northbound on Parker St. when a vehicle in front of her stopped short. Operator of MV1 further stated she then had to stop suddenly to avoid hitting the vehicle in front of her, however stated that is when MV2 rear ended her vehicle. Operator of MV2 stated he was traveling Northbound on Parker St. when MV1 stopped short in front of him. Operator of MV1 stated he attempted to avoid colliding with MV1, however could not and collided with MV1 in front of 110 Parker St. No injuries. MV1 sustained heavy rear end damage along with rear windshield damage. MV2 sustained heavy front end damage. Both vehicles involved were towed on scene by Tody's towing. A towed motor vehicle form was filled out for both vehicles and placed on file. Operator of MV2 was operating the vehicle without a drivers license. Mr. Marcelo De Oliveira Siqueira was cited in hand for Ch. 90 Sec. 10 (Unlicensed Operation). MA Uniform citation #T2012676. Incident #21039385 was generated in regards.

Operator 1 states he was operating MV1 east on River Street. Operator 1 stated he had to stop quickly for pedestrians in the cross walk at 276 River Street. At this time MV2 crashed into the rear of MV1. The operator of MV2 states he was traveling east on River Street when MV1 stopped abruptly in front of him and he crashed into MV1. The operator of MV2 stated he witnessed the operator driving MV1 and the operator of MV1 stated he was operating MV1 which is registered to him. When I arrived on scene both vehicles where facing east on River Street. The keys were in the ignition of MV1 and it was still running. River Street is a public way in the City of Newton. No reported injuries. MV1 was towed from the scene. Citation issued to Operator of MV1 for MGL 90/10 no license.

On Wednesday, February 23, 2022, while assigned to Traffic unit N525, I responded to the intersection of Washington Street and Elm Street, Newton with N492 for a report of a minor motor vehicle crash. Washington Street and Elm Street are both public ways maintained by the City of Newton. The weather at the time of the crash was clear and sunny. The road surface at the time of the crash was dry. I spoke with the operator of MV1, Mr. Kumar Bhol (S19441804). Mr. Bhol stated his vehicle (2018 Toyota Rav-4 (MA: 8HH159) was parked unoccupied on Elm Street next to the Village Bank in a metered spot. Mr. Bhol stated when he returned to his vehicle after leaving the gym, he observed another vehicle illegally parked behind his vehicle with it's front bumper very close to the rear of his vehicle. Mr. Bhol stated he observed minor fresh damage to the bumper area of his vehicle at this time. Mr. Bhol stated the owner of MV2 returned to her vehicle at this time and he attempted to exchange information with her. Mr. Bhol stated the owner did not want to exchange information and attempted to leave the area. Mr. Bhol stated he positioned himself in front of her vehicle in the roadway as it was leaving to prevent MV2 from leaving. Mr. Bhol stated at this time MV2 made light contact with his knee. Mr. Bhol stated he has no injuries as a result of the contact. Mr. Bhol declined my request to have him evaluated by Newton Medics. Mr. Bhol was also advised to not position himself in front of a moving vehicle to try and stop them from leaving the area. I observed very minor damage to the rear bumper area of MV1. It is inconclusive if the damage is fresh from this crash. I did observe obvious old damage to a sensor on the rear bumper of MV2 with black tape over it. I spoke with the operator of MV2, Ms. Jennifer North (S41819258). Ms. North stated she parked her 2015 Jeep Grand Cherokee (MA: 2XR747) behind MV1 on Elm Street in what she thought was a legal parking space. Ms. North stated when she returned to her vehicle, the owner of MV1 alleged the damage on his vehicle was caused by her. Ms. North stated she did not make contact with MV1 and attempted to leave the area. Ms. North stated Mr. Bhol would not let her leave so they contacted the Newton Police to assist. A query of Ms. North's license status showed it was expired/non renewable. I advised Ms. North of her license status and stated she could not drive her vehicle away from the area. Ms. North stated she understood and walked to her residence with her two dogs. Ms. North's vehicle was left legally parked in a metered parking spot on Elm Street. I observed no noticeable damage to the front bumper area of MV2. Ms. North report no injuries. As a result of Ms. North's license status she was issued Massachusetts Uniform Citation 306986AB in hand for Chapter 90, Section 10 (Unlicensed Operation) and Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care). It is unclear is the damage to MV1's rear bumper was caused by MV2.

On Tuesday 5/3/22 at approximately 0934 hours while assigned to marked unit n498 I was dispatched to the area of Rachel Rd and Upland Ave for a report of a minor motor vehicle crash. Upon arrival I spoke with the Operator, LUNA CARDOZO Ana Regina, of motor vehicle #1 (ma reg 53D960). She states she was travelling northbound on Upland Ave when she stopped in front of 355 Upland Ave. to let someone cross the street. She states as she was stopped she was struck in the rear by motor vehicle #2. Operator, LOUGHLIN William, of motor vehicle #2 (ma Reg 9470MR) states he was travelling behind motor vehicle #1 and mis judged the distance between the two vehicles and could not stop in time when motor vehicle #1 stopped to allow someone to cross the street. While interviewing both parties i obtained information that the operator of motor vehicle #1 did not have a drivers license. I Informed LUNA CORDOZO that she was going to be cited and issued Massachusetts Uniform Citation T2012916 c90 s10 Operating Without a DriversLicense. The vehicle was parked legally on Upland Ave and LUNA CORDOZO's husband, EIDERICK-COSTRA Wagner, a licensed driver, would be coming later in the day to pick up the vehicle.

MV 1 was traveling south bound on Walnut St in front of 331 Walnut when she followed MV 2 too closely and rear ended her. MV 1 sustained major damage to its front end and was towed by Tody's for it being disabled. The operator of MV 1, Ms. Perla Heredia, was cited for driving without a license, unregistered motor vehicle, and attaching incorrect plates on the vehicle (MA reg 2KYY53). A police incident report was also completed ( incident number 22019531) MV 2 was traveling straight southbound on Walnut St when she was rear ended. MV 2 sustained minor damage to her rear end. MV 2 was able to be driven away by the operator. MV 2 was advised of this crash report number. There were no injuries to either party.
