Narrative:
On 01/31/19 at 08:56 hours, I responded to Dedham and Walnut Streets for a two car motor vehicle accident with no injuries. Operator of M/V 1 states he was stopped for the stop sign facing northbound on Dedham St towards Walnut St. He then proceeded straight through and observed M/V 2 coming at him so he tried to speed up but M/V 2 ended up striking him on the driver's side rear quarter panel area. He did state that M/V 2 did stop for the stop sign after him but did not understand why they were attempting to come through the intersection. The vehicle sustained minor damage to the driver's side rear quarter panel area and had a flat tire. The operator waited for Triple AAA for the flat. Operator of M/V 2 states she was stopped at the stop sign facing eastbound on Dedham St attempting to take a left turn on to Walnut St. Due to the bright sun and glare, she did not observe M/V 1 coming through the intersection and struck him. The vehicle sustained front end damage to the grille and bumper but the vehicle was still driveable. At the time, I did observe the sun to be blinding at the accident location and there was a lot of glare.

Operator of MV1 (Mass reg. 8928AN) states he was traveling south bound on Waverly Ave when MV2 went through the stop sign and entered the intersection causing him to collide with MV2. Operator of MV2 (Mass reg. 7SH894) states she does not have much recollection from the accident. She states she had glare in her eyes when she was traveling east bound on Franklin Street and stopped at the intersection with Waverly Ave and proceeded through the intersection and collided with MV1. All parties were evaluated by Cataldo paramedics, operator of MV2 was transported to Saint Elizabeth's Hospital with minor injuries. Tody's towed both motor vehicles.

ON 3-26-19 AT APPROX. 1054HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT THE INTERSECTION OF WEBSTER AND CHERRY ST. I S[POKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES HE WAS TRAVELING E-BOUND ON WEBSTER AND HAD THE GREEN LIGHT. WHEN HE WENT THROUGH THE INTERSECTION HE WAS HIT IN THE LEFT SIDE BY VEHICLE #2. VEHICLE #1 STATES AFTER BEING HIT HIS VEHICLE SPUN AND HIS RIGHT REAR SIDE HIT A EVERSOURCE UTILITY POLE ( 453/7 ). AFTER HITTING THE UTILITY POLE HIS VEHICLE CONTINUED TO MOVE ACROSS THE STREET TO 35 WEBSTER WHERE HE HIT A ROCK WALL CAUSING DAMAGE TO HIS LEFT FRONT. VEHICLE #2 STATES SHE WAS TRAVELING S-BOUND ON CHERRY ST. AS SHE APPROACHED WEBSTER ST. SHE STATES SHE WAS BLINDED BY SUN GLARE AND CONFUSED THE LIGHTS AT CHERRY AND WASHINGTON AS BEING THE CORRECT LIGHTS ( THEY WERE GREEN AT THE TIME ). Tipruthai Thangthong stated that on 05/21/2019 at approximately 16:36 hours she was pulling into the driveway of her sisters home at 53 Rangeley Rd while operating her vehicle MA reg 2LVL61 a 2018 Honda Accord. Thangthong stated that her three old son Mighty Mettanant was a passenger in her vehicle. Thangthong stated that she was driving approximately 5 MPH as she drove into the driveway. The driveway runs east to west and Thangthong's vehicle was heading west into the driveway as the sun was in the Western sky creating a glare. Thangthong stated that she did no see anything in the driveway prior to her pulling into it. Thangthong stated that she felt the front of her vehicle strike what she described as " something " and stopped her vehicle. Thangthong stated that she put her vehicle in reverse and then into park and exited her vehicle to see what she had struck. Thangthong stated that she saw her two nieces, Kalla Mongkhonlertsirkul age three and Kelley Mongkhonlertsirkul age seven laying in the driveway in front of her vehicle. Thangthong stated that she then realized that her vehicle had stuck her two nieces as they were in the driveway. Both of the Mongkhonlertsirkul sisters were conscious and alert and were able to walk to an ambulance for transport to Newton Wellesley Hospital for treatment. Thangthong stated that she and her son were not injured in the crash. I observed no damage to Thangthong's vehicle. I responded to the Newton Wellesley Hospital and spoke with the Mongkhonlertsirkul sisters and their mother Piyada Kellum. Kellum stated that her oldest daughter Kelly is a student at the Franklin Elementary School . Kellum stated that her daughters told her that they were playing in their driveway when their aunt's vehicle entered the driveway and did not stop. I spoke with both of the Mongkhonlertsirkul sisters who told me that they tried to get out of the way of their aunt's vehicle but could not in time. The Mongkhonlertsirkul sisters stated that they were both struck in the back and fell to the ground between the two front tires of their aunt's vehicle. Kalla Mongkhonlertsirkul had scrapes to her right knuckles. Kelly Mongkhonlertsirkul had lacerations to her lower left arm and the left side of her face near her eye. I spoke with hospital staff who stated that there did not appear to be any broken bones or other internal injures. I took photos of the crash area and forwarded the disk to the NPD's IT bureau for downloading.

The operator of MV1 stated he was taking a left onto Waldorf Rd. Westbound from Canterbury Rd. when he saw a child bicyclist traveling Eastbound on Waldorf Rd. in the street. He stated that immediately after, the solar glare from the sun impaired his vision and he could no longer see the child, and at that time the two collided. The left driver's side door area struck the bicyclist, as well as the possible front left tire. The operator and passenger stated they were uninjured. MV1 sustained little to no damage. The bicyclist was identified as CHO, Elena, and she stated she was traveling Eastbound on Waldorf Rd. approaching Canterbury Rd. She stated MV1 took a left onto Waldorf Rd. towards her and at that time she collided with the left side of MV1 and fell off her bike. The rear wheel of the bike was bent due to the collision. Elena stated that she was ok but had right hip pain. She was wearing a helmet, green jacket, blue shirt, black pants, and was riding a pink and white Diamondback bicycle (Serial # A1504026648). Her mother KIM, Kyung arrived on scene and signed a patient refusal with AMB1, and transported her to Harvard Vanguard Wellesley for an evaluation. I took photos of the scene and submitted them to the IT bureau to be attached to this report.

Operator of vehicle one, Christopher Sermak, stated that while turning north bound in the parking lot of 175 Boylston St he was blinded by solar glare. Sermak stated that the diver's side visor was down and the he was not wearing sun glasses. Sermak stated that he heard a loud bang coming from the front driver's side of his vehicle and realized that his vehicle had stuck something. Sermak stated that he stopped his vehicle and saw a male identified as John Baker laying on his back on the ground. Baker stated that he had step off the side walk in front of 175 Boylston St and was walking to his car when he saw Sermak's vehicle approaching him and realized it was not slowing, Baker sated that the front driver's side of Sermak's vehicle struck his right arm and shoulder causing him to be knocked to the ground on his back. Baker stated that his head struck the ground but he did not lose consciousness. Baker was transported to Newton Wellesley Hospital via EMS for treatment. While on scene I observed that the rear passenger side brake light of Sermak's vehicle was out. I issued Sermak MA Citation T 2012527 and cited him for violations of City of Newton City Ordnance 19-75, failure to use care while tuning , and MGL 90/7 , defective lights.

At that time vehicle 2 struck vehicle 1 with the front of vehicle 2. Medics responded due to airbag deployment and transported the operator of vehicle 2 to NWH for an evaluation. It should be noted at the time of the accident the glare while traveling east on Washington Street makes is difficult to see. Todys towed both vehicles.

On 9/10/2020 I was working N491 when I responded to the area of Adams St. for a report of an MVA unknown injuries. Upon arrival I spoke with Hasibullah Wahab who stated he was traveling southbound on Adams St. when an unknown vehicle traveling northbound on Adams St. approached with "blinding" headlights. Wahab stated he "was unable to see due to the headlights and swerved" striking Veh 2. Wahab was evaluated by Newton medics and signed a patient refusal. I located the Owner of Veh 2 Tim White. White requested his vehicle be towed off the street due to the fact it was leaking fluids and he did not feel safe driving it. Both Vehicles were towed by Todys. Both parties were provided accident report numbers.

On Wednesday, October 7th 2020 while in N526 I responded to 791 Walnut Street the Newton Cemetery Corp for a motor vehicle accident involving a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. The accident occurred inside the cemetery on the public way by the entrance. Upon arrival I spoke with the pedestrian identified as Tova Goldring. Tova stated she was walking in the roadway when she was struck by MV1. Tova stated MV1 was turning left onto the road that leads to the exit and when she realized the operator was not stopping she put out both her hands and made contact with the hood of MV1. Tova stated the impact caused her to fall to the ground and roll into a grassy area next to the road. Tova had minor scrapes from the accident and signed a patient refusal. The operator of MV1 identified as Antonio Domenico Ricci was on scene and his primary language was Italian. I was able to contact his daughter, Daniela Boccia who arrived on scene and translated Antonio's statements. Antonio stated he was turning left onto the main road that leads to the exit of the cemetery and had his left signal activated. Antonio states the sun was glaring into his eyes and made it hard for him to see. Antonio stated he took the left turn and never saw the pedestrian. Antonio states he heard a thud on his hood and stopped the vehicle and that is when he realized he had struck a pedestrian. Antonio was not injured and there was no damage to his vehicle. Pictures of the scene and Tova's injuries were taken by Officer Wilson and submitted to the IT Bureau. Daniela took possession of MV1 and drove Antonio to his residence.

On Thursday, October 15, 2020, while assigned to Traffic unit N525, I responded to the Whole Foods parking lot entrance located at 916 Walnut Street, Newton for a report of a motor vehicle/bicycle crash. Walnut Street is a public way maintained by the City of Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with the operator of MV1, Tyrone Powell. Powell stated he was operating his 2016 Toyota Highlander (MA: VTT188) while traveling across Walnut Street (E) from Carthay Circle. Powell stated there was a strong sun glare at this time on his passenger side. Powell stated as he entered the Whole Foods parking lot entrance, the passenger side of his vehicle collided with a passing bicyclist that was traveling northbound on Walnut Street. Powell reported no injuries. I observed damage to the passenger side hood/fender of MV1. I spoke with the bicyclist involved in the crash, Scott Chamberlain. Chamberlain stated he was operating his bicycle on Walnut Street (N) in the marked bicycle lane near the entrance to the Whole Foods located at 916 Walnut Street. Chamberlain stated he had a bicycle helmet on at this time. Chamberlain stated that he observed a vehicle to his left travel across Walnut Street in front of him into the Whole Foods parking lot. Chamberlain stated he was unable to avoid crashing into him. Chamberlain reported minor injuries and signed a patient refusal with Newton Medics. I observed no substantial damage to his red Specialized bicycle. Photos were taken and submitted to the IT Bureau. While on scene, Officers did notice a strong sun glare coming from the Southeast direction of Walnut Street. This glare would have shined on the passenger side area of MV1 as it crossed Walnut Street.

Operator of vehicle one ( 2016 Infinity MA reg 3YD266 ) Miram Qualter, stated on 11/05/2020 at 10:28 hours while driving south bound on Waverley Ave near the intersection of Tremont St " the light was turning " as her vehicle entered the intersection. Both Tremont St and Waverly Ave are public ways in the City of Newton. Qualter stated that she was driving into solar glare and did not see any traffic in the intersection as she proceeded into it. Qualter stated that she believed that she still had the right of way and that traffic on Tremont St had a red light. Qualter stated that she did not see vehicle two until the time of impact . Qualter stated that she was not injured in the crash. Qualter's vehicle had major damage to the front driver's side windshield and front driver's side quarter panel. Operator of vehicle two Keith Ward stated that he was riding his Itraxx Cruze motor scooter west bound on Tremont St near Waverley Ave on 11/05/2020 at 10:28 hours. Ward stated that west bound traffic had a green light and he proceeded into the intersection. Ward stated that as he was approximately halfway into the intersection when vehicle one crashed into the right front of his vehicle. Ward stated that he was ejected from his motorcycle and onto the hood of vehicle one. Ward was wearing a helmet and his head had contact with vehicle one's driver side windshield causing substantial damage. Ward was evaluated by Newton Medic Unit 3 and refused medical treatment. Ward signed a patient refusal form provided by the medics. Ward had visible head lacerations that caused minor blood loss. Witness Jose David stated that he was riding on a motor scooter behind Ward. David stated that west bond traffic on Tremont St had a green light and that vehicle one did not stop at a red light. Based upon statements made to me and my observations of the involved vehicles, I issued Qualter MA Citation T2016100 and cited her for a violation of MGL 89/9 , failure to stop at a red light. I took photos of both vehicles and the disk was downloaded by the NPD's IT Bureau. Both vehicles were towed by Tody's Towing, and I filled out towed motor vehicle forms. During my investigation of this crash I discovered that the motor scooter that Ward was operating is classified as a motorcycle, because it's cylinder capacity exceeded 50cc. Ward's vehicle speedometer indicted that it's top speed was 50 MPH, and it's VIN tag stated it was a motorcycle. I observed that there was no license plate on Ward's vehicle and he stated to me that it was unregistered. A check of the MA RMV had no record of Ward's vehicle and also revealed that Ward's driver's license was suspended on 12/04/2018. It was also revealed from a of the MA RMV data base that witness Jose David's driver's license statue is expired non renewable as of 01/23/2017. David stated to me that he did not have a driver's license. I also discovered that David's motor scooter was classified as a motorcycle, because as with Ward's vehicle it also had a cylinder capacity exceeding 50cc. David stated to me that his vehicle was unregistered. I issued David MA Criminal Application T2015901 and cited him for violations of MGLs 90/10, operation of a motor vehicle with out a license, 90/34j uninsured motor vehicle and 90/9 , unregistered motor vehicle. I issued Ward MA Criminal Application T2015902 and cited him for violations of MGLs 90/23 , operating a motor vehicle after suspension, 90/34j, uninsured motor vehicle and 90/9 unregistered motor vehicle. I filled out an Operating After Suspension Affidavit and faxed it to the MA RMV Driver Control Unit.

The bicyclist, Martin Pleynet, states he was riding his bicycle eastbound on Commonwealth Ave traveling towards the Boston College area when he was struck by MV1. Mr. Pleynet states he doesn't have recollection of the accident since he lost consciousness for a brief time after being struck. Mr. Pleynet was transported to Brigham and Women's Hospital where he sustained a fractured left shoulder, abrasion on his right shin and abrasion on his forehead. The bicycle sustained damage to the front wheel and was placed into property and evidence for safekeeping. Pictures of the scene were captured and the SIM card was submitted to the IT bureau.

Operator of vehicle one Ziting Liu stated on 05/14/2021 at approximately 17:38 hours she was driving west bound on Commonwealth Ave in her vehicle a 2015 Honda CRV ( MA reg 497yy1 ). Liu stated that she was driving into the setting sun and was wearing sunglasses at the time. Liu stated that as she approached the four way intersection with Chestnut St she observed that west and east bound traffic on Commonwealth Ave had a green light and that north and south bound traffic on Chestnut St had a red light. Liu slowed her vehicle to turn left onto the south bound lane of Chestnut St. It should be noted that both Commonwealth Ave and Chestnut St are both public ways in the City of Newton. Liu stated that she did not observed any " cars " driving east bound on Commonwealth Ave so she proceed to make the left turn onto Chestnut St. Liu stated that she realized that her vehicle has struck an unknown object as she was turning left. Liu stopped her vehicle and saw that her vehicle had struck a female bicyclist. Liu stated that the bicyclist was thrown from their bike and was on the ground. Liu stated that she was not injured as a result of the crash. Liu's vehicle had damage to the front passenger side hood . The front passenger side of Liu's windshield had an inward crack that was approximately 12 inches in length and 12 inches in width. The type of inward crack on the windshield is indicative of an outside force making contact with it. Liu's vehicle was towed by Tody's Services to their lot for storage. I filled out a towed motor vehicle form. Bicyclist Isabella Turinovic stated that she was ridding her bike , east bound on commonwealth Ave near Chestnut St. Turinovic stated that east bound traffic had a green light. Turinovic stated that as she was crossing over Chestnut St Liu's vehicle struck her causing her to hit the windshield. Turinovic was tossed from her bike and onto the roadway. Turinovic was wearing a helmet at the time of the crash. Turinovic stated that she did not lose consciousness and she was transported to Newton Wellesley Hospital via EMS from treatment . Turinovic agreed to have her bike taken by NFD Ladder One to be stored at NFD station Two. I took photo's of Liu's vehicle, Turinovic's bike and the disk was downloaded by the NPD IT Bureau. Witnees Alexander Delima stated that he was the first vehicle stopped a red light on Chestnut St in the south bound lane. Delima stated that Liu's vehicle did not yield to Turinovic as she turned left onto the south bound lane of Chestnut St from the west bound lane of Commonwealth Ave .A second witness Lindsay Bentis stated that she was the second vehicle stopped a the red light in the south bound lane of Chestnut St behind Delima. Bentis stated that she also saw Liu fail to yield to Turinovic as she turned onto Chestnut St. Based upon statements made to be by all involved parties, I concluded that Liu is at fault for this crash. I issued via U.S. Mail, Liu MA Uniform Citation T1446670 and cited her for a violation of MGL 90/14, failure to yield to oncoming bicyclist when turning left.

Upon arrival, vehicle 1 was on Commonwealth Ave and vehicle 2 was in the middle of the grass median. Operator 1 was able to move his vehicle off to the side of the road into the carriage lane. Operator 1 stated that he was on Commonwealth Ave facing eastbound and was trying to turn left on to Irving St. He stated that "the sun got into his eyes" when he was making the turn and crashed into vehicle 2 who was traveling westbound on Commonwealth Ave. Operator 2 stated that she was traveling westbound on Commonwealth Ave when vehicle 1 tried to turn left, directly in front of her path of travel. After the collision, operator 2 stated that she almost crashed into a tree and turned to avoid it, and her final resting spot was on the grass median. I observed heavy front end damage on the front driver's side bumper and wheel on vehicle 1. On vehicle 2 there was damage to the front driver's side bumper and the front wheel was bent. The right front tire of vehicle 2 was also flat. Both operators stated they were not injured and declined medical attention. Both operators also called AAA who came and towed the vehicles. A witness, Simon Welsby, left his phone number with operator 2 and I gave him a call from Newton Police Headquarters. Mr. Welsby stated that he was traveling behind vehicle 1 as they were turning left onto Irving St. Mr. Welsby stated vehicle 2 had the right of way and that vehicle 1 made an improper turn.

John stated he was looking straight ahead towards the direction he was attempting to travel and proceeded into the intersection to cross over Wolcott Street when he struck the pedestrian with his front bumper causing him to fall on the ground. John stated he was only traveling a couple miles per hour as he just accelerated from a stop. John stated he did not see the pedestrian because of the suns glare. I was able to speak to the pedestrian inside the ambulance receiving medical treatment. He was identified as Justin Anderson (S27026247). Justin stated he was jogging westbound on Webster Street and was crossing the intersection of Wolcott Street and Rowe Street, attempting to head westbound on Wolcott Street. Justin stated he was jogging on the sidewalk and entered the crosswalk to cross the street and was struck by the city vehicle in the right leg causing him to fall to the ground. Justin reported a minor thumb injury and a small scrape on his head. Justin stated his temperature was high and he was being transported to NWH by Medics as a precaution. Pictures of the city vehicle and the scene were taken and submitted to the IT Bureau. No damage to the city vehicle was observed or reported.

DRIVER LOST SIGHT OF THE ROAD AND CRASHED INTO A LANDSCAPING TRAILER THAT WAS PARKED IN FRONT OF 258 DERBY ST. I SPOKE TO THE OWNER OF VEHICLE #2. THEY STATED THEY HEARD THE VEHICLE CRASH BUT DID NOT ACTUALLY SEE THE ACCIDENT. VEHICLE #1 HAD EXTENSIVE FRONT RIGHT END DAMAGE AND WAS TOWED BY PRIVATE TOW. VEHICLE #2 HAD NO DAMAGE AND WAS STILL OPERATIONAL. ALL PARTIES REPORTED NO INJURIES. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES. CLEARED WITHOUT FURTHER INCIDENT.

On January 23, 2022 at approximately 1458 hours, I responded to the area of 325 Beacon St for a report of a 4 vehicle MVA with possible injury. Upon arrival with the Fire Department and EMS, I met with all the drivers. No injuries were reported on scene. MV1 operator states she was driving WB on Beacon St when MV2 came to a sudden stop and due to the sun's glare, did not see MV2 very well. MV1 operator then rear-ended MV2. I did not observe any damage to the front of MV1 or the rear of MV2. MV2 operator states she was driving WB on Beacon St when MV1 suddenly collided with her rear end. MV2 subsequently rear-ended MV3. I did observe damage to the front of MV2 and to the rear of MV3. MV3 operator states he was stopped, waiting for the cars to go since the light had just turned green, when he was suddenly rear-ended by MV2. MV3 subsequently rear-ended MV4. I did observe damage to the front of MV3 and the rear of MV4. MV4 operator states she was stopped in traffic when she was suddenly rear-ended by MV3. No vehicles were towed. No citations were issued.

On 01/27/2022 while working a detail for Feeney Utility Co, Lt Healy called over the air that he had just witnessed a motor vehicle pedestrian accident at his location and that no one was injured. On arrival I spoke to the pedestrian involved, Jinny Lin who stated he was walking northbound on Lowell Ave and was crossing Highland Ave in the marked crosswalk when he was struck by vehicle 1. Lin stated he was more than halfway across the street when he was struck. Lin said he was only bumped by the vehicle and was never knocked over. Lin had already notified his mother, Tina Chan who arrived on scene shortly after. Lin was seen by Fallon medics and signed a patient refusal. I than spoke to the operator of vehicle, Ali Roozbehani1 who stated he was traveling southbound on Lowell Ave and saw that it was blocked off and detoured down Highland Ave due to road construction. Roozbehani said he was slightly blinded due to solar glare as he made a left turn onto Highland Ave and as he did so struck a pedestrian. Roozbehani had no damage to his vehicle and was uninjured. I took several digital images of the scene and submitted them to IT to be attached to this report. Roozbehani was issued MA Uniform Citation #T2016735 for 89/11 pedestrian in a crosswalk.

Operator of MV1 (MA reg 2RBG35) was traveling EB on Pearl Street when she stopped for a vehicle pulling out of a driveway. While stopped the Operator of MV2 (MA reg 2YDP98) also traveling EB on Pearl Street rear ended MV1. Operator of MV2 stated the solar glare obstructed her vision and by the time she observed MV1 stopped it was too late. MV1 sustained minor damage to the rear of the vehicle and was able to drive away safely. MV2 sustained heavy front end damage and was leaking fluid. Air bags were also deployed from MV2. Medic 5 arrived on scene for an evaluation. Operator of MV2 was claiming a possible head injury and signed a medical refusal. Tody's arrived on scene and towed the vehicle.

At approximately 0933HRs on Wednesday, March 02, 2022, I was dispatched to 151 Sargent St. for a report of a two car MVA. Upon viewing the vehicles, I observed MV1 (MA Pass: 1HLE75) parked in front of 163 Sargent St. with damage to its passenger's side (fender, wheel/tire, front and rear doors). And I observed MV2 (MA Pass: 6ELY50) in the driveway of 151 Sargent St. with damage to its rear (tow hitch and bumper). Upon speaking with the operators, both of whom reported no injuries, the operator of MV1 stated they were traveling Eastbound on Sargent St., when MV2 attempted to exit the aforementioned driveway and they two vehicles made contact. The operator of MV2 stated that as they exited said driveway, there was solar glare which came from not only the sun in their eyes, but from sun reflected off of an uninvolved vehicle which passed on the opposite side of the road. Being that they were unable to see MV1 approaching, MV2 continued to travel out of the driveway, backing up Southbound, when the two vehicles made contact. Both operators exchanged information prior to my arrival. MV1 and its operator remained in a safe area on scene to await a tow from AAA and MV2 left under its own power. I cleared without further incident.

Vehicle #1 was traveling northbound on Centre St. and in light traffic when she was approaching the stopped cars ahead when she was blinded by the glare coming over the hill. She saw Vehicle #2 too late due to the glare and was unable to stop in time and struck Vehicle #2 in the rear. Vehicle #2 stated he was traveling northbound on Centre St. and was stopped in traffic at the light when he was struck from behind by Vehicle #1. Vehicle #1 had moderate damage to the front of the vehicle and was unable to be driven and was towed away by Tody's towing. Vehicle #2 had minor damage to the rear bumper and was able to be driven. All parties were asked if they were injured in the accident and all stated they were unhurt and refused any medical treatments.
