I responded to the area of Washington St.@Quinobequin Rd. for a report of a two car MVA. I arrived in the area and located the MVA directly at the entrance to Quinobequin Rd. Both Operators and passenger were out of the vehicle upon my arrival with no reports of injury. V1 Operator; Cunniff, Erin MA OLN#S65270968. V1; MA Pan Reg. 6PW389, 2011 SUBA/STW color gray. V2 Operator; Murphy, Kelly RI OLN#3262011. V2; RI Pan Reg. 410986, 2011 Suzu/SX4 color red. V2 Passenger; Revoy, Antoine. V1 operating Eastbound on RT. 16 Washington St. attempted to turn right onto Quinobequin Rd. The operator of V1 traveled past the entrance, came to a stop and attempted to backup. V1 while reversing backed into V2 which was also traveling Eastbound on RT. 16 Washington St. and was attempting to turn right onto Quinobequin Rd. The V2 Operator and Passenger both stated that V2 Operator beeped its horn but V1 backed into V2 On Washington St., at the entrance to Quinobequin Rd. There was no air bag deployment in either vehicle. V1 sustained right rear end damage which did not require towing service. V2 sustained heavy left front end damage and left side damages to include the tire and wheel well. V2 did require towing service. Tody's Towing Inc. responded and removed V2 from the crash scene. I advised all parties involved. I also transported V2 Operator and Passenger To Tody's Towing located at 1354 Washington St., Newton.

On April 19, 2019 at approximately 15:45 Operator of MV1 walked into the Newton Police Station to file a motor vehicle crash report. Operator of MV1 said that the operator of MV2 had left the scene after the crash. Operator of MV1 said that she was on Cabot St driving westbound, turning right onto Harvard St northbound at the stop sign. She said that the operator of MV2 was turning left onto Cabot St from Harvard St and the vehicle's trailer struck front driver's side of her vehicle. Witness, Mike Horner (508)930-7375 followed MV2 and got the license plate information. I contacted the operator of MV2, who said that he was turning left on Cabot St and the operator of MV2 started to turn before he was done. He saw the operator of MV1 hit a wire of the vehicle's trailer, but did not believe that there was any damage done and thought that MV1 drove away.The operator of MV2 was compliant in the investigation and did not believe he left the scene of an accident.

ON MY ARRIVAL TO THE SCENE I SPOKE WITH THE OWNER OF VEHICLE #1 IDENTIFIED AS ROBERT RANKINGS. HE REPORTS THAT WHILE EATING LUNCH IN TOMMY DOYLES A WOMEN(WITNESS), LATER IDENTIFIED AS PATTY TUCKER ENTERED THE RESTAURANT ASKING IF ANYONE OWNED A RED TOYOTA PARKED OUT FRONT? HE REPLIED, "HE DID". WITNESS THEN TOLD HIM SHE WITNESSED ANOTHER VEHICLE HIT HIS CAR THEN DRIVE AWAY. THE FOLLOWING DAY,JUNE 7TH, 2019, AT 09:30HOURS I SPOKE WITH THE WITNESS, PATTY TUCKER. Operator of MV1 states she was driving westbound on Lincoln St. She states MV2 pulled out suddenly from a parking stall on the right side and crossed in front of her vehicle. She states she did not have time to avoid MV2, and the front of her vehicle collided with the side of MV2. Operator of MV2 states he was parked in the parking stalls, westbound on Lincoln St. He stated he did not see MV1, and pulled out in order to do a U-turn (which is prohibited in this area). When he pulled out, he collided with MV1. Witness stated she did not observe the crash as it happened but it appeared that MV2 was perpendicular to the roadway as if he was attempting a U-turn. No injuries were reported and no tows were needed.

MV1 operator reported general pain and shock from accident. Medics responded and a refusal was signed. MV2 operator states he was driving eastbound on Centre St when he missed his left turn to go over the bridge heading towards Watertown. At approximately 1825HRs on Tuesday, September 17, 2019, I was dispatched to the intersection of Washington St. at Court St. for a report of a two car crash with minor injuries. Upon arrival, I spoke with the operator of motor vehicle 1 (MA Passenger: 64G720), who stated through a third party interpreter, that he was driving "on the big road (Washington St. Westbound) and was hit." The operator of motor vehicle 2 (MA Passenger:8WH577), stated that she was traveling southbound on Court St. and stopped at the stop sign with the intent to make a left hand turn onto Washington St. (eastbound). At this time, she observed motor vehicle 1 traveling towards her with its right hand turn signal activated to take a right hand turn on Court St. (northbound). Motor vehicle 1 began to execute said turn, but as it was partially on Court St., changed directions and began to again attempt to travel westbound on Washington St., striking motor vehicle 2 as it was at the stop sign. Motor vehicle 1 sustained damage to its front right bumper area. Motor vehicle 2 sustained damage to its front left side bumper area. The operator of motor vehicle 1 was uninjured. The operator of motor vehicle 2 is pregnant and signed a patient refusal after speaking with Newton Medics. Both vehicles left the scene under their own power and I cleared without incident.

On November 20th, 2019 at 17:27 hours, a two vehicle motor vehicle accident took place at the intersection of Vine St and Lagrange St. There were no injuries. Vehicle 1 was being operated by registered owner Misha Brainin (Mass Reg #9BJ848, 2018 Jeep SUV Color Gray, Safeco Insurance). Vehicle # 2 was being operated by Christopher Moses (Mass Reg # 1YGT60, Niss Altima Color Red, USAA Casualty Insurance). Misha stated he had come to a complete stop at the stop sign on Vine St. Misha took a left hand turn from Vine St on to Lagrange St. Christopher stated he was on Lagrange St waiting to take a left hand turn from Lagrange St on to Vine St. Both parties took a left hand turn at the same time and the vehicles collided. There was minor damage to Misha's vehicle in the front left side. There was major damage to Christopher's vehicle in the left rear side. Tody's towed Christopher's vehicle. Both vehicles are insured.

MV1 was operating southbound on Parker St (public way) and the operator stated he had the green light. MV2 was operating northbound on Parker St, and the operator also said she had the green light. On Friday December 27th, 2019, at approximately 1348hrs, while assigned to N508 to assist with a funeral escort, I was notified by Dispatch that "MC1" was involved in a crash at the Whole Foods parking lot on Washington Street. Upon arrival I found that Newton Police Motorcycle #1 was involved in a crash while helping to assist a funeral that had just left the Our Lady Church property. Police MC #1 is MA MMC7219, a 2016 Harley Davidson motorcycle. The motorcycle was operated by Officer Rocco Marini. MV #2 was also located on scene and parked in the Whole Food parking lot. This vehicle is MA Reg. 7VN155 a 2017 Lexus ES350. This vehicle was operated by Diana Simkin. Officer Marini reported that he was traveling westbound on Washington Street when he was hit by MV #2 that was making a left (opposite direction) into the Whole Foods parking lot. Officer Marini was in the right hand lane escorting the long line of cars that was part of a funeral procession. These vehicles were in left westbound lane. Officer Marini reported he was in the right hand lane and had his emergency lights on when the collision occurred. Officer Marini did not see MV #2 prior to collision. Officer Marini reported that the front of his motorcycle hit the rear passenger side tire of MV #2. Officer Marini was taken from the scene by Ambulance to Newton Wellesley Hospital. I spoke to the operator of MV #2 Diana Simkin. Simkin reported she was traveling east on Washington Street and wanted to make a left into Whole Foods parking lot when the crash occurred. Simkin reported she was waved into the lot by an unknown motor vehicle headed in the opposite direction. Simkin was not sure which lane this person waved from. Simkin made the left into the lot and was not aware of the crash or did she see MC 1 until she was stuck in traffic in the parking lot and some woman knocked on her window. Simkin reported that she never saw an police emergency flashing lights either. There was minimal damage to Simkin's vehicle. There was scuff marks on the passenger rear tire. Simkin also did not report any injuries. MC #1 sustained some front damage to the wheel fender fairing. The motorcycle was taken from the scene by Tody's. Pictures were taken on scene by Officer McSweeney and given to IT for downloading purposes. While on scene Officer McSweeney and myself went to New England Diamond and Jewelry at 660 Washington Street. We met the owners Dave and Robert Alpert (617-243-0099) who reported that they have video of the collision. In the video I observed at 1346hrs the funeral begin to pass the Whole Foods entrance. At 1348hrs the video shows MV #2 come up in the easttbound lane and stop. The vehicle stops and waits until an unknown white SUV stops to allow MV #2 to make a left turn into the lot. Once this unknown SUV stops, the video shows that MV #2 makes the left and hits MC #1 as he is traveling straight ahead with his emergency lights on. It appears from the video that MC #1 had no warning and could not brake his motorcycle. The video shows a long line of cars in this funeral procession with their emergency flashing lights on. Due to the angle of the video and vehicles I am unable to tell if this SUV had their flashing lights on. The video was sent to Director Smith in IT for downloading purposes. I went to Newton Wellesley Hospital and spoke with Officer Marini. He was reporting soreness to his left elbow. Officer Marini was released from the Hospital and taken back to the station at about 1500hrs. Based on the facts of the investigation, I have issued MA Uniform Citation T0340444 to Diana Simkin for violating 89/8: failure to yield the right of way and for violating 89/7A: interference with an emergency vehicle. Simkin was notified of this citation via telephone at 1730hrs on this day and this citation was mailed to her address. Washington Street is a public way in the City of Newton.

OPERATOR OF MV 2 ACKNOWLEDGED THE ACCIDENT WAS HIS FAULT AND WAS ISSUED MA UNIFORM CITATION T2079858 FOR C.O. 19/75 FAILURE TO USE CARE WHILE TURNING. THERE WAS A CONSTRUCTION DETOUR IN PLACE WHICH HAD ALL TRAFFIC HEADING NORTHBOUND ON WAVERLY EITHER TURNING LEFT OR RIGHT AT FRANKLIN ST.

Operator of MV1 stated while turning left from Washington Street onto Washington Street at the I90 overpass, while in the left/ straight lane, MV2 which was in the no turn lane, heading South toward Centre St decided to turn left onto Washington Street. MV1 was side swiped on the passenger side of the vehicle. Operator of MV1 stated MV2 did not stop and continued on Washington Street. Operator of MV1 had damage to the passenger side of the vehicle. Traffic Bureau update (Officer Gaudet): On Thursday, February 27, 2020, I made contact with the registered owner of MV2, CRM Property Management Corp. A representative there stated they would find out who was driving the vehicle and have them contact me. On Friday, February 28, 2020, I spoke with the operator of MV2, Peter Ben-Simon (S48100671) at the Newton Police Station. Ben-Simon stated he recalls the incident but said his vehicle never came in contact with MV1. Ben-Simon stated MV1 was driving erratically and was following him for some time on Washington Street. Ben-Simon believes his interaction with MV1was a road rage incident but did not want to pull over at the time and report it. I observed no fresh damage to MA: T20208 that would be consistant with being in a crash with MV1. Ben-Simon's information has been added to this report to reflect him being the operator of MV2. No futher action will be taken at this time.

On Wednesday October 28, 2020 at approx. 0900 I responded to 24 Maguire Court for a report of a crash with property damage involving a tractor trailer. Upon arrival I met with the operator of the tractor trailer Mr. Williams, he stated he was traveling on Maguire Court delivering equipment to a local business, and while turning into the business parking lot he misjudged the turn and the trailer struck a fence,posts,wall belonging to 24 Maguire Court. I advised the owners son who was on the property of the crash and advised I would have a report. There were no reported injuries and all parties advised of the process. I also conducted a Level one inspection on the truck/trailer. On 11/28/2020 at approx 0917hrs while assigned to 497 I responded to the area of Chestnut St and Beacon St for a report of a 2 car crash w/o injuries. Upon arrival I found Ma Reg 3GEH51 operated by Patrick Bowe to the side, facing the wrong way, in front of 584 Chestnut St, Ma Reg P73262, a Clifford s Flower delivery Van, being operated by Rochelle Driscoll was in the driveway of 584 Chestnut St with no damage. I spoke with Driscoll who stated she was travelling SB on Chestnut St attempting to turn left into 584 Chestnut St, Bowe was behind her and he swerved when she turned, trying to avoid colliding and ran into the utility pole on the corner. I spoke with Bowe who stated he was travelling SB on Chestnut St behind Driscoll , she turned left ,than backed into the driveway of 585 Chestnut St, as he began to pull forward she pulled out of the driveway of 585 across the roadway into 584 causing him the brake and swerve to avoid hitting her instead glancing off the utility pole in front of 584 Chestnut St. No damage to pole, Bowes vehicle had significant front left damage , when he attempted to pull away it appeared his front left axle was broken, Bowe pulled safely onto a side St and elected to call AAA for a tow.

On Wednesday March 31, 2021 at approx 1430 hours Officer Panica responded to the area of Washington Street and Jackson Road for a report of a tractor trailer (car carrier) with New York registration that knocked over a traffic signal and kept traveling east on Jackson Road with out stopping. Officer Nardelli and I responded to the area as dispatch was giving updates of tractor trailer information and direction of travel. I traveled west on Watertown Street from Chapel Street and observed a car carrier traveling west approx four blocks west ahead of me. I activated my emergency lights and made a motor vehicle stop at Watertown Street at Nevada Street. I observed a trailer NY reg # BH32997 which dispatch confirmed this reg was the one involved in the crash. The name of the comapny is DIVERISIFIED AUTOMOTIVE INC of 6 RAMLAND ROAD ORANGEBURG N.Y. 10962 N.Y. apportioned tractor reg 17833PB / trailer NY reg BH32997, the driver was later identified as MA lic # SA0310636 Januel Ortiz d.o.b. 5-17-2001 of 111 Country Club BLVD apt. 201 Worcester MA 01605. Officer Panica confirmed the crash site location and damage to a City of Newton Traffic signal/footings/ sidewalk/retaining wall/landscape. Officer Panica was also approached by witness Erin McGinty of 233 Jackson Road(see statement below) I approached the car carrier on the passenger side and observed fresh damage on the trailer that would be consistent with striking the traffic signal. I then approached the driver of the car carrier Mr. Ortiz. I advised him of the crash and he stated he was traveling west on Washington Street and turned right (north) onto Jackson Road. He stated while turning his trailer struck the traffic signal knocking it over. I asked him why didn't he stop to assess damage or notify anyone. He stated he was looking for a safe place to pull over. He stated he continued north on Jackson Road then left onto Pearl Street and then left onto Watertown Street to drop off vehicles at the 445 Watertown Street,which is where I Pulled him over. I advised him I am familiar with the travel route he went and the length of Jackson Street is not a Heavily traveled area since the school is closed from the crash site, and he traveled approx a mile and a half until I pulled him over. I advised him I would be doing a level III inspection and advising him of my findings. Witness Ms. McGinty stated she heard the crash and observed the car carrier with NY reg # BH32997 have contact with the traffic signal and continue to travel north on Jackson Street without stopping. Therefore after observing damage/pictures and statements I issued in hand MA criminal citation # T2017488 in hand to Mr. Ortiz for Chapter 90 Section 24 LEAVING THE SCENE OF CRASH WITH PROPERTY DAMAGE City Ordinance 19-75 FAILURE TO USE CARE IN TURNING. Mr. Ortiz was explained the process. I took pictures of the damage to the car carrier, and crash site area that I will have downloaded at the I.T. Bureau.

At approximately 1620 HRs on Monday, June 07, 2021, I was dispatched to a report of a fallen traffic light pole at the intersection of Oak St. and Chestnut St. Upon arrival, I observed said light pole down and observed what appeared to be a fresh side-swipe mark on one of the green light lenses, as if it had been in contact with a large vehicle. While on scene, I was contacted by passing motorist, Witness 1, who stated that they were traveling South Bound on Oak. St., when they observed the offending tractor/trailer attempt to make a right hand turn from Chestnut St. (East Bound) onto Oak St. (South Bound), striking the traffic light. Witness 1 additionally provided me with a picture of said trailer (ME Semi Plate: 273605C (Trailer #6673), which has been attached to this report. I contacted Amber, the company which is registered as leasing the trailer and spoke with their Risk Assessment Member, Elizabeth, who stated she would attempt to determine who the driver was. Additionally, I left a message for Mike of Attic RRG, the insurance company for the trailer. At the conclusion of writing this report, I am still awaiting a call from both. ***update*** As of June 13, 2021, all attempts to identify the operator of the tractor/trailer have been met with negative results. Traffic Bureau update (Officer Gaudet): I have made several attempts to contact the registered owner of the trailer involved in this crash over the past week with a negative result. I contacted the Maine Motor Transport Association that handle the registrations for trailers in the state. An agent there stated the trailer is a long term rental to a company called Mesilla Valley Transportation (MVT) Leasing out of New Mexico. I contacted MVT and spoke with Angelica from their Risk Management. Angelica was able to identify the operator of the vehicle as Mr. Hayes Hall Jr. Anglica stated Mr. Hall was in the area making a delivery on the date of the crash. Angelica also stated Mr. Hall did not report being involed in a crash on June 7, 2021. I contacted Mr. Hall to inquire about the crash. Mr. Hall stated he was operating that vehicle on the date of the crash. Mr. Hall stated he was unaware he hit the light pole, and if he knew he made contact with it he would have stopped and reported it. Mr. Hall's information was added to this report to reflect his as the operator of MV1. Mr. Hall reported no new damage to his vehicle that could have resulted from crash with the light pole. It is conceivable that due the to size for the vehicle Mr. Hall was driving, he was unaware of the pole he hit. Mr. Hall will not be cited for leaving the scene of the crash.

The operator of MV#1 stated he was travelling westbound on Beacon St when he saw an available open parking spot and made a u-turn onto the eastbound lane striking MV#2. MV#1 sustained heavy damages to its driver's side front and rear doors with driver's side front and rear airbag curtain deployments. The operator of MV#1 sustained injuries to his left side ear and left side bruising of his ribs area. The operator of MV#1 was evaluated by Newton Paramedic 1 and signed a patient refusal or treatment. MV#1 was towed by Tody's Towing. The operator of MV#2 stated she was travelling westbound on Beacon St attempting to make a left turn onto Hammond St when she was struck by MV#1. The operator of MV#2 stated MV#1 was to her right side when MV#1 made an abrupt left turn into her lane striking her vehicle on her front passenger side and pushing her into the eastbound travel lane. MV#2 sustained heavy front end damages. The operator of MV#2 sustained injuries to her left side torso area. The operator of MV#2 was evaluated by Newton Paramedic 1 and signed a patient refusal of treatment. It should be noted that the point of impact for both vehicles occurred on Beacon St westbound by the Boston College "McElroy Commons" (325 Beacon St at the St Jude Gate entrance) and there are no intersections.

On September 20th, 2021 at approximately 12:22 hours while working N491 I responded with Fallon Ambulance and NFD to a report of a MV crash on Washington St @ Park St with injuries. On my arrival I located both involved vehicles on Washington St near Park St. Vehicle #1 was a 2015 Grey Toyota RAV4, MA REG. 5AVE80, operated by a Marineide Lopes. She stated she was going E/B on Washington St @ Park St in the far left lane when vehicle #2 who was on her right, next lane over attempted to turn left in front of her causing the crash. The impact forced her vehicle to the left onto an island causing further damage to her vehicle. Vehicle #2 was a 2016 White Chev.Cruz MA REG. 8NWJ30, operated by a Michael Faraji. HE stated he was going E/B on Washington St when his GPS indicated to make a left turn (north)towards the Centre St Bridge. He then attempted to make the left turn striking vehicle #1 as it was passing the area that was open for only N/B traffic coming from Park St. There is no legal left turn for E/B traffic on Washington St Both vehicles were towed from the scene by todys. Driver to vehicle #1 was transported by Fallon Ambulance. Both driver and passenger from vehicle #2 were also transported by another Fallon Ambulance to Newton Wellesley.

Mv#1 operator stated he was travelling on Chase St E/B and then turned right onto Ripley St making a U-turn reversing his direction intending to turn left back onto Ripley St W/B. As #1 was making the U-turn, #1 struck the passenger side rear end side of Mv#2. Mv#2 operator stated he was travelling on Chase St E/B behind #1. #2 stated #1 suddenly turned right onto Ripley St making a U-turn, then struck #2 passenger rear end side as #2 was passing by. The impact pushed #2 sideways and #2 crashed over the sidewalk into a city tree and stone wall at 40 Chase St. Both operators were uninjured. Both Mvs sustained significant damage; #1 was able to be driven away with driver's side front end damage; #2 was inoperable and towed by Tody's. Photos of city tree knocked over and stone wall were submitted to the I.T. bureau. City forestry notified. Based on the following observations and statements, #1 was issued citation #T2080655 for Ch90/Sec14 Improper Turn; #1 was making a simultaneous turn and U-turn prior to colliding with #2 on its rear end side indicating #2 had already passed through most of that intersection; most of crash debris was found in proper travel lane of #2 indicating #2 was suddenly struck by #1 not allowing enough reaction time for #2 to avoid being struck.

On 10/6/2021 at approx 0759Hrs while assigned to 497 I responded to the area of Washington St and 128N Off Ramp for a report of a two car crash without injuries. Upon arrival I observed Ma Con Reg R14581 a large tree removal truck fully in the southern eastbound lane of Washington St. Ma Reg 6PX766, a white VW Jetta straddling the fog lane and partially into the southern Eastbound lane.I spoke with the operator of the Jetta, Jordan Glickman who stated he was stopped in traffic when the tree truck struck him. I spoke with the operator of the tree truck, Leobardo Carrillo Reyes, who stated he was travelling east on Washington St when the Jetta came up the off ramp, to avoid eastbound traffic backed up on the exit he came up the left turn lane from the exit, took and illegal right turn onto Washington St and tried to cut into the trucks lane causing the collision. Leobardo also gave me the card of a witness , Michael Minkoff who had stopped and corroborated this version of events.

Bicyclist Winter Douglass stated that they were riding their bike east bound in the bike lane on Beacon St near Union St on 11/01/2021 at 18:20 hours. Douglass stated that an SUV ( MA reg 2ZVE39 2019 Chevrolet Tahoe ) that had taken a prohibited left turn from Union St onto Beacon St was blocking the bike lane. Douglass stated that they attempted to pass the SUV on the right but could not. Douglass stated that they crashed into the rear driver's side of the SUV. Douglass was wearing a helmet at the time of the crash. Douglass's bike was equipped with lights on the front and back. Douglass was transported to Newton Wellesley Hospital via EMS for a left shoulder injury. Susan Hynds the operator of the SUV stated that they turned left from Union St on to Beacon St and were stopped in traffic when the crash occurred. Hynds stated that they did not see the posted sign prohibiting left turns from Union St onto Beacon St. Hynds stated that she and her son, Sean Hynds ( DOB 07/12/2010 ) who was a passenger in the SUV were not injured. Hynds's SUV had a dent in the rear driver's side and it did not require a tow. I took photo's of the SUV and the posted no left turn sign and turned the disk over to the NPD's IT Bureau. Douglass's bike had no visible damage and it was transported to the station and secured at Support Services. I issued Hynds MA Uniform Citation T2016744 for a violation of MGL 90/14 improper left turn.

Vehicle 1 and Vehicle 2 were headed eastbound on Auburn St when Vehicle 1 attempted to parallel park on the south side of Auburn St. Vehicle 2 attempted to drive around Vehicle 1 while they attempted to park, at which time Vehicle 1 decided to abandon their attempt to parallel park and instead took a wide left turn in an attempt to pull into the angled parking spaces across the street on the north side of Auburn St. When Vehicle 1 attempted this turn, the front driver's side quarter panel of Vehicle 1 made contact with the passenger side front and rear quarter panel of Vehicle 2. I observed minor scrapes to the side of Vehicle 2 and minor scrapes to the front quarter panel of Vehicle 1. The operator of Vehicle 1 stated that she was having difficulty turning the steering wheel of her vehicle following the accident, and I observed that her two front wheels were no longer parallel with each other and instead pointed at an inward angle. Both parties denied injuries. Vehicle 2 was able to drive away from the scene while Vehicle 1 requested a tow from AAA.

Ms Lima stated she was parked facing W/B on Washington St near #721 when she attempted to make a U-Turn to go E/B when she was struck by vehicle #2. Vehicle #2 was a 2016 Jeep Compass, MA. reg 298XH9, operated by a Jevon D. Foster. He stated he was going W/B on Washington St when vehicle #1 pulled out in front of him from a parked position causing him to crash into her vehicle. Both vehicles were towed by Todys and there were no reported injuries.
