Narrative:
Operator of MV1 (MA reg. 328XVF) states he was driving eastbound on Church Street and stopped at a red light. He realized he was in the left turn only lane and began turning into the right lane to proceed straight through the intersection. Operator of MV1 does not believe there was any contact between the two vehicles. Operator of MV2 (MA taxi reg. TA477C) stated he was in his lane when MV2 turned into him. I did not see any new damage to either vehicle. Operator of MV2 requested to be evaluated by paramedics. Cataldo paramedics responded and transported operator 2 to Saint Elizabeth's Hospital. MV2 was parked in Eaton Mac Kay Funeral Home parking lot and Veterans Taxi was notified. Veterans Taxi was unable to verify weather the damage on MV2 was new or old. They do not have a log of damage to the vehicle.

Narrative:
MV1 was operating southbound on Parker St (public way) and the operator stated he had the green light. MV2 was operating northbound on Parker St, and the operator also said she had the green light. MV2 attempted to take a left hand turn onto the Boylston St overpass westbound, but struck MV1 in a head on crash. Both vehicles had front and side airbags deployed. Passenger of MV1 said they had minor injuries and was transported to Newton-Wellesley Hospital. MV1 operator did not report injuries. MV2 operator and passenger did not report injuries at this time. Both MVs sustained heavy front end damage and both were transported by Tody's. MV2 operator was issued MA Uniform citation # T2013392 for Ch.89 s8, failure to yield for approaching MV.

On Friday December 27th, 2019, at approximately 1348hrs, while assigned to N508 to assist with a funeral escort, I was notified by Dispatch that "MC1" was involved in a crash at the Whole Foods parking lot on Washington Street. Upon arrival I found that Newton Police Motorcycle #1 was involved in a crash while helping to assist a funeral that had just left the Our Lady Church property. Police MC #1 is MA MMC7219, a 2016 Harley Davidson motorcycle. The motorcycle was operated by Officer Rocco Marini. MV #2 was also located on scene and parked in the Whole Food parking lot. This vehicle is MA Reg. 7VN155 a 2017 Lexus ES350. This vehicle was operated by Diana Simkin. Officer Marini reported that he was traveling westbound on Washington Street when he was hit by MV #2 that was making a left (opposite direction) into the Whole Foods parking lot. Officer Marini was in the right hand lane escorting the long line of cars that was part of a funeral procession. These vehicles were in left westbound lane. Officer Marini reported he was in the right hand lane and had his emergency lights on when the collision occurred. Officer Marini did not see MV #2 prior to collision. Officer Marini reported that the front of his motorcycle hit the rear passenger side tire of MV #2. Officer Marini was taken from the scene by Ambulance to Newton Wellesley Hospital. I spoke to the operator of MV #2 Diana Simkin. Simkin reported she was traveling east on Washington Street and wanted to make a left into Whole Foods parking lot when the crash occurred. Simkin reported she was waved into the lot by an unknown motor vehicle headed in the opposite direction. Simkin was not sure which lane this person waved from. Simkin made the left into the lot and was not aware of the crash or did she see MC 1 until she was stuck in traffic in the parking lot and some woman knocked on her window. Simkin reported that she never saw an police emergency flashing lights either. There was minimal damage to Simkin's vehicle. There was scuff marks on the passenger rear tire. Simkin also did not report any injuries. MC #1 sustained some front damage to the wheel fender fairing. The motorcycle was taken from the scene by Tody's. Pictures were taken on scene by Officer McSweeney and given to IT for downloading purposes. While on scene Officer McSweeney and myself went to New England Diamond and Jewelry at 660 Washington Street. We met the owners Dave and Robert Alpert (617-243-0099) who reported that they have video of the collision. In the video I observed at 1346hrs the funeral begin to pass the Whole Foods entrance. At 1348hrs the video shows MV #2 come up in the easttbound lane and stop. The vehicle stops and waits until an unknown white SUV stops to allow MV #2 to make a left turn into the lot. Once this unknown SUV stops, the video shows that MV #2 makes the left and hits MC #1 as he is traveling straight ahead with his emergency lights on. It appears from the video that MC #1 had no warning and could not brake his motorcycle. The video shows a long line of cars in this funeral procession with their emergency flashing lights on. Due to the angle of the video and vehicles I am unable to tell if this SUV had their flashing lights on. The video was sent to Director Smith in IT for downloading purposes. I went to Newton Wellesley Hospital and spoke with Officer Marini. He was reporting soreness to his left elbow. Officer Marini was released from the Hospital and taken back to the station at about 1500hrs. Based on the facts of the investigation, I have issued MA Uniform Citation T0340444 to Diana Simkin for violating 89/8: failure to yield the right of way and for violating 89/7A: interference with an emergency vehicle. Simkin was notified of this citation via telephone at 1730hrs on this day and this citation was mailed to her address. Washington Street is a public way in the City of Newton.

Opr of V1 stated he was stopped and attempting to make a left hand turn into a driveway to turn around. Opr of V1 stated he believed he was clear to make the turn. Opr of V1 stated he started to turn and that is when V2 made contact with his vehicle on the left side of the vehicle. V1 was crossing over the double-yellow line to make the turn, but was clear from on coming traffic. V1 was towed privately. Opr of V1 sustained injuries and was transported to Mt Auburn Hospital. Opr of V2 stated he was traveling straight on California. Opr of V2 stated he observed V1 stopped to the right. Opr of V2 stated he thought he was clear to pass him, but was not due to V1 abruptly making the turn. I spoke to a witness who wishes to stay anonymous. Witness stated he was cleaning his car. While cleaning his car, he heard a bang, turned around, and observed the vehicles rolling off to the side. Witness stated he observed V1 and V2 in the middle of the road and not in the lane of traffic. Based on his observations he believes the collusion happened at the yellow lines. Witness stated he heard a car driving fast before the bang. He did not see the collision, but states V2 could have been driving too fast and tried passing on the left.

On Sunday June 13, 2021 at approximately 1933 hours I was dispatched to 2322 Washington for a two car MVA with no injuries. MV1 operator stated she was operating eastbound on Washington St (a public way) driving the speed limit. MV1 operator stated she observed MV2 on the westbound side of Washington St attempt to turn left into Starbucks (2322 Washington St). MV1 operator stated by the time she saw MV2 cut across in front of her and into her lane, it was too late and MV1 struck the passenger side of MV2. MV2 operator stated the same story. MV2 operator said she was also going the speed limit on Washington St westbound and attempted to turn left into Starbucks but misjudged MV1's speed, causing the MVA. MV1 sustained extensive total front end damage and was towed by Tody's. MV1 operator requested an evaluation by Newton Medics and was transported to Newton-Wellesley Hospital. MV2 sustained extensive and total passenger side damage and was towed by Tody's. MV2 operator reported no injuries.

Operator 1 stated he was traveling Northbound on Dedham St. He stated he saw vehicle 2 at the stop sign at the intersection of Rosalie Rd and Dedham St. Operator 1 stated that vehicle 2 turned out of Rosalie Rd and tried to turn left onto Dedham St and he tried to brake but was unable to stop. Vehicle 1 collided into vehicle 2 in a perpendicular angle. Operator 2 stated that she was stopped at the intersection of Rosalie Rd and Dedham St and was going to take a left onto Dedham St. After one car passed her going straight on Dedham St, she thought she had enough time to take a left. Operator 2 stated as she proceeded to try and turn left she saw vehicle 1 approaching, and tried to hit her brakes but could not stop in time. The front of vehicle 1 collided into the drivers side door of vehicle 2. There was minor damage to the front bumper of vehicle 1. On vehicle 2, there was heavy damage to the drivers side door leaving it unable to be opened. Vehicle 2 was towed by Tody's towing. Operator 1 was evaluated by Fallon medics and signed a patient refusal. Operator 2 stated she may have hit her head, and was transported to Newton Wellesley Hospital by Fallon.

Narrative:
Bicyclist Winter Douglass stated that they were riding their bike east bound in the bike lane on Beacon St near Union St on 11/01/2021 at 18:20 hours. Douglass stated that an SUV ( MA reg 2ZVE39 2019 Chevrolet Tahoe ) that had taken a prohibited left turn from Union St onto Beacon St was blocking the bike lane. Douglass stated that they attempted to pass the SUV on the right but could not. Douglass stated that they crashed into the rear driver's side of the SUV. Douglass was wearing a helmet at the time of the crash. Douglass's bike was equipped with lights on the front and back. Douglass was transported to Newton Wellesley Hospital via EMS for a left shoulder injury. Susan Hynds the operator of the SUV stated that they turned left from Union St on to Beacon St and were stopped in traffic when the crash occurred. Hynds stated that they did not see the posted sign prohibiting left turns from Union St onto Beacon St. Hynds stated that she and her son, Sean Hynds ( DOB 07/12/2010 ) who was a passenger in the SUV were not injured. Hynds's SUV had a dent in the rear driver's side and it did not require a tow. I took photo's of the SUV and the posted no left turn sign and turned the disk over to the NPD's IT Bureau. Douglass's bike had no visible damage and it was transported to the station and secured at Support Services. I issued Hynds MA Uniform Citation T2016744 for a violation of MGL 90/14 improper left turn.
