Narrative:
OPMV#1 Was traveling Southbound on Chestnut St when she saw an object in the middle of the road and swerved into the Northbound lane of Chestnut St. OPMV#1 Drove up over the curb and onto the sidewalk colliding with a stone wall at 542 Chestnut St and a Utility pole. The vehicle then traveled Southbound on the sidewalk and stopped at the intersection of Chestnut St and Roslyn Rd. I observed major damage to front of the vehicle along with left and right side damage. Airbags were deployed on all sides. OPMV#1 Was evaluated by EMS and she signed a medical refusal. Vehicle was towed by Tody's Towing. OPMV#1 is charged with the following: M.G.L. C.89 S.4A Marked Lanes, M.G.L. C.90 S.24 Negligent Operation, M.G.L. C.90 S.24 OUI Alcohol. On Tuesday, July 23rd 2019, at approximately 5:24pm, I, Officer Brooks, responded to 444 Lowell avenue for a 2 car MVA. Upon my arrival I observed MV1(NH REG 4120041) with heavy front end damage. The vehicle was located in the driveway of 444 Lowell avenue, with the front end sticking out into the roadway. I spoke with the operator of MV1 who stated he was trying to exit the driveway and turn left onto Lowell avenue southbound, when he was struck by MV2(MA REG 3AP243) which had been traveling northbound on Lowell avenue. According to the operator of MV1, MV2 had been traveling at a high rate of speed and tried to swerve around him. There was airbag deployment in MV1 however the operator signed a patient refusal with Cataldo. MV1 was towed by Tody's. I then spoke with the operator of MV2 who stated she was traveling northbound on Lowell avenue and saw MV1 exiting the driveway. She assumed he was going to stop pulling out of the driveway, and when he did not she stated she attempted to swerve around him. MV1 made contact with the passenger side rear wheel well of MV2 causing MV2 to spin out. The operator was not injured and arranged for a private tow due to the damage in the wheel well area.

On Wednesday, October 16, 2019, I, Patrolman Hatfield, was assigned to marked cruiser N 495 to Division 5 for the 1530 to 2330 hour shift. At approximately 1742 hours, FTO Guarino and I were dispatched to Commonwealth Ave at Cedar St for a motor vehicle accident with minor injuries. FTO Guarino and I arrived on scene and noticed two MVs with damage. I spoke with the RO/operator of MV2, Janet Haines (Janet). Janet stated she was not suffering from injuries. Janet said she was operating southbound on Cedar St crossing Commonwealth Ave when she had a collision with MV2. Janet stated prior to crossing Commonwealth Ave, an unknown MV stopped at the intersection to allow her to cross. Janet then stated she proceeded accross when she crashed into MV1, who drove around the stopped MV. Janet said her brakes did not work and she continued to drive forward and ended on top of the curb of Commonwealth Ave facing eastbound. MV2 suffered damage to the whole front bumper and wheel wells. FTO Guarino spoke with the operator of MV1, Madeleine Richard (Madeleine). Be advised both front and side driver airbags were deployed, and the front driver's side door was unable to be opened. I then spoke with Madeleine. Madeleine stated she was operating eastbound on Commonwealth Ave before being involved in the collision. Madeleine stated she drove around the MV infront of her that was attempting to take a left from Commonwealth Ave onto Cedar St, and she did not see MV2 prior to the accident. MV1 was struck on the driver's side by MV2, and ended facing westbound in the middle of Commonwealth Ave. EMTs and fire arrived on scene. Janet stated she did not need to be transported. Madeleine was transported to Newton Wellesley Hospital for an evaluation. Both MVs were towed by Todys.

MV #1 was traveling northbound on Lagrange St. MV #2 was traveling southbound on Lagrange St. when the operator turned eastbound onto Corey St.(Boston). As MV#2 turned eastbound it collided with MV #1. Significant damage was observed to both MV#1 and MV#2's front bumpers and engine compartments. Both MV#1 and MV#2's airbags were deployed upon impact. The Operator of MV#1 reported she was uninjured while the Operator of MV#2 was transported to the hospital. Both vehicles were towed from the scene by Tody's Tow Service.

Operator #1 states he was driving northbound on Lexington St operating MA reg 47C440 (vehicle #1). Once at the green light at Lexington/Rumford Ave, operator #1 attempted to take a left turn onto Rumford when MA reg 4FR581 (vehicle #2) collided with him. Vehicle #2 was driving eastbound on Rumford and operator #1 states he did not see the car until the crash occurred. Operator #2 reports he was driving eastbound on Rumford and was attempting to take a right turn onto Lexington St at the intersection. Operator #2 acknowledges that he had a red light but states he stopped prior to turning (right on red is allowed). It should be noted that upon my arrival on scene, vehicle #2 was located partially in the intersection with airbags deployed and I am not able to conclude that vehicle #2 was in fact turning right. The point of impact does appear to be the left front area. Operator #2 states he doesn't know what happened, and that vehicle #1 may have oversteered the left turn. The damage to both vehicles is extensive and mostly contained to the front end/undercarriage areas. Both vehicles were towed by Tody's and all parties signed patient refusals from medics on scene.

I, Officer Sullivan, spoke with Margaret Wilkens MA OLN#S33922902 at Newton PD Headquarters at approximately 1745hrs. 11/14/2019. M. Wilkens was involved in an MVA in the area of Chestnut St.@Elliot St. (Newton) . M. Wilkens appeared upset as crying. I asked if she were injured. She stated her neck was sore but she'd be ok. I asked if she had hit her head or if there were airbag deployment to which she answered "no". I asked her if she wanted EMS to medically evaluate her to which she also stated "no". M. Wilkens stated she was reared ended by another woman who refused to give her any information. M. Wilkens stated after being rear ended she and the other woman were outside of their vehicles checking for visible damage. M. Wilkens stated it was dark and difficult to see. The other woman stated their was no damage to M. Wilkens car. M. Wilkens told her the vehicle (V1) she was driving was not hers and is in fact a rental. V1; MA Reg. 8ZA768, 2019 Lexus/NX300H color gray. M. Wilkens stated the other woman returned to her vehicle and drove away. M. Wilkens thought she was going to her vehicle to get her license and vehicle information. M. Wilkens also had recorded with her cell phone the brief interaction with the other operator. M. Wilkens described the other woman as in her mid 20's average build approx. 5'5'' with brown hair (in a pony tail), M. On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

I responded to 1641 Beacon St. for a report of an MVA/Hit &Run. I arrived on location and spoke with Alicia Ianiere MA OLN#S84312664. Alicia's vehicle was parked when the car was hit by a passing vehicle. Alicia was inside a restaurant picking up food. The vehicle was occupied by her daughter Katherine Cullen 23 Y.O., who was seated in the front passenger seat, at the time of the collision. Katherine stated she had some stiffness in her neck. EMS responded to the scene to evaluate her. There was no airbag deployment. Katherine was cleared by EMS, she was not transported. The vehicle that was hit was MA Reg. 1JXF31; 2018 Toyt/Camry sedan color brown. The vehicle was hit by an unidentified vehicle. RP Alicia Ianiere stated the vehicle struck the rear of her vehicle as it was parked in front of 1641 Beacon St. The vehicle continued Westbound failing to stop. Ianiere's Vehicle sustained damage to the rear driver's side. There were several people seated at an outside area of a nearby restaurant. Those I spoke with heard the crash but did not actually observe the collision. There was one witness identified who stated he observed the collision. witness was identified as Dylan Shuler 24 Y.O.. Mr. Shuler described the other vehicle as a "silver2 door Honda Civic. He stated the plate was either MA Reg. 6EJZ30 or 6ZJZ30. A query of the possible plates came back not matching the vehicle description. The other witnesses also described a silver 2 door Honda. Nobody else observed a plate number. Alicia Ianiere was advised to notify her Ins. company and that I would document the accident as a MVA Hit&Run.

On 9/1/20 while working N493, I responded with N497 and the Fire Dept to 40 Crescent St for a report of a car into the house. On arrival I observed MA 6CM450 in the backyard of #40 unit C. It was stationary against the large AC Compressor in the rear of the unit, and appeared to had run over several large shrubs before coming to a stop. I spoke with vehicle owner and operator Marion Cohen 8/27/46. Cohen stated that as she was parking in the rear lot of the complex, she accidentally mistook the brake for the gas pedal. She in turn ended up going over the curb and through some bushes before hitting the compressor. There was a passenger in the vehicle identified as Milton Kornfeld 3/10/42. Both occupants were checked out by medics and declined transport. The airbags did not deploy. There does not appear to be any structural damage to anything other than the compressor. NFD checked the integrity of the house, and the compressor was unplugged. I took pictures of the scene and submitted the memory card to IT. Homeowner Xiaozhi Zhao and his family were present inside at the time of the collision and are unharmed. Tody's responded and removed the vehicle from the property. It sustained heavy front end damage and was towed to their lot via flatbed. A m/v inventory form was filled out. This report number has been provided to all parties for their respective insurance companies.

Roselourdes Pierreadrien was operating vehicle #2 S/B on Chestnut St. Roselourdes states that she had a green light and was proceeding through the intersection and was struck on the right side by vehicle #1. Roselourdes vehicle rolled over and landed on its roof. Lourdes was able to free herself from the vehicle prior to my arrival. Stuart Schneller was operating vehicle #1 on Elliot St E/B. Stuart stated that he believed he had a green light when proceeding through the intersection. Both parties suffered injures and had a hard time recalling what had happened. Both parties transported to NWH and both vehicles towed by Tody's Off. Kane was on a detail nearby. Off. Kane states that she did not see the accident happen but heard it. Off. Kane then saw vehicle #2 roll onto its roof. Off. Kane then went over to the vehicle, broke the drivers side window, cut open the airbag and assisted Roselourdes from the vehicle. Off. Kane went to NWH for injuries to her hands where she was eventually discharged.

On Tuesday February 9th, 2021 at approximately 1334 hours I responded to the intersection of Ascentra Terrace and Highland Street for a two car motor vehicle accident. At the time of the accident the weather condition was heavy snow in the area and the roads were covered in snow. Both Ascentra Terrace and Highland Street are owned and maintained by the City of Newton. Upon arrival I met with the operator of MV1 identified as Elizabeth Carron (S53478024). She stated she was heading northbound on Highland Street when she lost control of her vehicle on the bend of the road and made contact with MV2. MV1 sustained heavy rear end damage and airbag deployment. The operator of MV2 identified as David Balaban (S09328852) stated he was traveling southbound on Highland Street and could not avoid MV1 when it lost control and collided with his vehicle. MV2 sustained heavy drivers side panel damage. The Fire department and Medic 2 arrived on scene and cleared with a patient refusal. Todys responded and towed MV1 as it was inoperable.

Operator #1 states that she was traveling northbound by 226 Lexington St when she observed vehicle #2, a white Ford Explorer, approaching at a high rate of speed from Lexington St southbound. Vehicle #2 then crossed the double yellow line into the northbound lane, subsequently sideswiping vehicle #1. Operator #1 states that two parties exited the Explorer and spoke briefly to her. Once she requested to exchange information, the two refused and drove away on Lexington St southbound. Operator #1 described the parties as a hispanic male and hispanic female, around 50 years old. The male party was balding/or bald, and the female was wearing a silver turtleneck. Operator #1 noted that it was the male who was operating the vehicle both during the collision and as the vehicle left the scene. While medics were documenting a patient refusal for operator #1, Ofc Ciccone was en route to NPD headquarters to report for duty in his p/v and heard the broadcast for the suspect vehicle. Ofc Ciccone observed a white Explorer with a flat tire traveling an estimated 50+ mph past him on South Ave by Park Rd in Weston heading towards Natick. This location is approximately 2.2 miles from the crash site and is accessible from 226 Lexington St by heading southbound, taking a right, and continuing straight on Rt 30. Weston Pd was immediately notified and the vehicle was located in a driveway at 7 High Meadow Rd, a residence directly on the corner of Rt 30. Sgt Rooney and Ofc Ciccone (who was able to take a spare cruiser and call into service as this incident unfolded) responded and met Weston PD. While on scene, it was explained to all officers by the resident of 7 High Meadow Rd, Helder Quesada, that he was walking his dog and observed the white Explorer broken down in an unsafe spot. Quesada offered up his driveway to park the vehicle as the male and female occupants waited for assistance. Quesada allegedly does not know Lopez and Albizureis, and all involved persons were outside in the driveway when police located the vehicle. The description of the male asociated with the Explorer matched that which operator #1 gave to me- a hispanic male balding in his 50's. He was in the presence of a hispanic female who was wearing a silver colored turtleck. The male was identified as Armando Lopez-Rubi DOB 1/1/73 and the female Ana Maria Albizureis DOB 11/27/73. The Explorer was MA reg 6GZ591, registered to a different party then all on scene. It had obvious damage to the left side doors consistent with an opposite direction sideswipe. With the translation assistance of Helder Quesada, Lopez-Rubi stated to Sgt Rooney and Officer Ciccone that he was in fact driving somewhere near Starbucks "on the corner" when he got into an accident (there is a large Starbucks located on the corner of Commonwealth Ave and Lexington St in Newton). Lopez originally said that the other involved vehicle left the scene, however it was pointed out by Sgt Rooney that the operator was currently stopped in Newton talking to me and was cooperative. Upon routine query it was discovered that Lopez-Rubi does not have a license to operate a motor vehicle. Pursuant to the investigation, Officer Ciccone issued in hand to Lopez MA citation R8006998 for the following: 89/4a- Marked Lanes Violation 90/24- Leaving Scene of Property Damage 90/10- Unlicensed Operation of a Motor Vehicle. Vehicle #2 was left in the driveway of 7 High Meadow Rd with Helder Quesada's permission. Quesada also volunteered to drive home both Lopez and Albizureis to Framingham, as he was feeling charitable. Vehicle #1 was towed by Tody's due to the amount of damage and airbag deployment. A m/v inventory form was filled out.

Operator of MV1 said she was in the far left lane at the intersection of Walnut Street at Washington Street preparing to turn left onto Washington Street. Operator of MV1 said the light turned green and she proceeded to turn left when she was struck by MV2. Operator of MV1 said MV2 was in the center lane preparing to continue straight on Walnut Street. Once the light turned green operator of MV1 said MV2 tried to turn left striking the front of her vehicle and MV2 immediately turned back to Walnut Street and continued driving. Operator of MV1 got the license plate of MV2 which is Arizona Registration AD50102. MV2 was a Uhaul Box Truck and I attempted to contact Uhaul to find out who was operating the truck yielding negative results. There was front end damage to MV1 and it is unknown if there is damage to MV2. The airbags in MV1 did not deploy and there was no injures on scene. MV 1 was able to drive away from the scene. Traffic Bureau update (Officer Gaudet): I contacted U-Haul in an attempt to get information on the party that rented this vehicle. I was advised that per U-Haul's policy, their company required a written request to their legal department for any information about their renters. This request was submitted on July 7, 2021 and it can take up to 10 days for a response. To be further investigated.

MV1 was heading Westbound on Newtonville Ave through the intersection of Harvard St when he collided with MV2. MV2 was heading Northbound on Harvard St through the same intersection when it collided with MV1. Both operators stated that they had the green light to proceed through the intersection. MV1 collided with MV2 and then rolled over to its top, laying to rest on Newtonville Ave up against a residential fence belonging to #355 Newtonville Ave. MV2 obtained extensive damage to the front and all airbags deployed. Both operators signed refusals with the medics. MV1 operator, Everton Roberto-Rosa was issued in hand MA Citation #T2080934 for 90/10 Unlicensed Operation. Both vehicles were totalled and towed by Tody's. Two witnesses saw the accident. The first witness, Jessica Foz, stated "wow, look at him, he is not stopping" and stated that MV1 was driving too fast and ran a red light. Another witness was Georgia and Harry Triantafilles who both stated that MV1 had the green light.

The operator of MV#1 stated she was travelling northbound on Winchester St at the intersection of the Boylston St off ramp when she was struck on the passenger side of her vehicle. The operator of MV#1 was unsure of what happened but stated that she believed MV#2 was travelling in front of her. The front passenger of MV#1 stated they were travelling northbound on Winchester St when they were hit on the passenger side. The front passenger also stated that MV#2 was travelling in front of them. The operator of MV#1 was visibly crying and was evaluated by Newton Paramedics with a patient refusal of treatment. The operator of MV#1 later complained of neck pain. She refused medical evaluation and was transported home by her mother. They were advised to seek medical assistant if need be. The front passenger of MV#1 complained of right side shoulder pain area. His eye glasses were also damaged during this accident. He was evaluated by Newton Paramedics with a patient refusal of treatment. MV#1 sustained heavy passenger side damages with both passenger side rear and front curtain airbags deployed. MV#1 was towed by Tody's Towing. The operator of MV#2 stated she was travelling northbound on Winchester St and stated, "next thing I know there was a bang". The operator of MV#2 was unsure of how the accident occurred. MV#2 sustained heavy front passenger side damages and was leaking fluids. MV#2 displayed MA registration 4GF124. A query of this registration showed that it was registered to a different vehicle. The paper registration that the operator provided me displayed registration #18717. Both registrations were active and registered to the operator of MV#2. It appeared that the operator of MV#2 owns 2 vehicles of the same make and model but with different colors. The operator of MV#2 stated the RMV were the people that attached her plates to her vehicles. MV#2 was towed by Tody's Towing. The operator of MV#3 stated she was stopped at the stop sign on the Boylston St off ramp (Westbound) at the intersection of Winchester St when she heard the accident involving MV#1 and MV#2. The operator of MV#3 stated MV#1 was then pushed into her vehicle causing minor damages to her front end. The operator of MV#3 stated she was unsure which vehicle caused the accident. The operator of MV#3 stated she was not injured. The point of impact between MV#1 and MV#2 was approximately 20 yards south of the Boylston St bridge. When Officers arrived on scene, the positions of MV#2 was directly underneath the Boylston St bridge and MV#1 was in front of MV#3 at the stop sign to the Boylston St off ramp. I could not determine who caused the accident.

On November 23rd, 2021 at approximately 14:08 hours while working N491 I responded to #25 Pearl St for a report of a MV crash with airbag deployment. On my arrival I located the involved vehicle a 2021 black Cadillac, Ma reg. K9195 with extensive damage on the property of a four unit condominium complex that abuts #275 Centre St(Plaza). The operator who was standing outside her vehicle was identified as Margaret Cloherty. She appeared uninjured from this crash. She reported the following: She first entered the parking lot to #275 Centre St which has several different businesses. As she drove through the parking lot her vehicle suddenly began to accelerate. She attempted to brake but her vehicle continued straight through the parking lot continuing to accelerate before finally crashing through a wooden fence and onto the property to #21 to #27 Pearl ST. Ms Cloherty was evaluated by the Medics but refused to by transported to the hospital. She signed a patient refusal form. Her vehicle which appeared to be totaled was towed from the scene by Todys. The property owner of the fence was identified as Steve Knapp 508-958-1906 was contacted by police. He will be provided a copy of this report of his insurance.

OPERATOR OF VEHICLE #2 STATES SHE WAS TRAVELING N-BOUND ON LOWELL AVE AND WAS STOPPED AT THE RED LIGHT. SHE DID NOT SEE VEHICLE #1 COMING AT HER UNTIL SHE WAS ACTUALLY HIT. THE OPERATOR OF VEHICLE #5 ORIGINALLY LEFT THE SCENE AND THEN RETURNED. HE STATED HE WAS TRAVELING W-BOUND ON WASHINGTON AND WAS HALFWAY COMPLETED TURNING LEFT ONTO LOWELL AVE. DRIVER STATES HE INTENDED TO COMPLETE THE TURN ONCE VEHICLE #1 HAD PASSED. HE STATES WHILE WAITING HE SAW VEHICLE #1 TURN RIGHT ABRUPTLY AND SEEMED TO LOSE CONTROL OF THE VEHICLE BEFORE SMASHING INTO VEHICLE #2 AND CAUSING SUBSEQUENT COLLISIONS. I responded to 275 Grove St for a report of a two car MVA/no injuries. I arrived on location and observed the two involved vehicles pulled into the parking lot of 275 Grove St. V1; MA Reg. 2JNS36, 2022 Hyun/Kona SUV color blue. The crash occurred on Grove St. directly in front of the entrance to 275 Grove St./Riverside Center and directly across from the Woodland Park Apt. complex. V2 operator stated he was exiting the apt complex turning left onto Grove St. (southbound). At that time V1 travelling southbound on Grove St. collided into the passenger side of V2. The roadway is two way divided by a median island. There is a traffic light that set on flash at the time of the crash. Both operators stated their visibility was obstructed by the snow bank on the median island. Neither of the operator's were injured as a result of the collision. there was no airbag deployment in either vehicle. Both vehicles were driveable and did not require towing services. The operators were advised and left the area in their vehicles without further incident.

Operator of MV1 states she was turning left eastbound from Sumner Street onto Commonwealth Avenue when she hit MV2. Operator of MV1 states she couldn't see over the snow pile on Commonwealth Avenue. Operator of MV1 stated she didn't see the car until the last second. MV1 sustained major damage to the front left of the vehicle and left side of the vehicle. All airbags were deployed. MV1 was towed by Todys's Service Inc as it was disabled. Operator of MV1 had no injuries and denied medical attention. Operator of MV2 states he was traveling straight ahead (Westbound) when he saw MV1's car lights and then all he remembers was crashing into the fence of 63 Sumner Street. MV2 sustained major damage to the front center and right side of the vehicle. Todys's Service Inc towed MV2 as all the air bags were deployed and the vehicle was disabled. Operator of MV2 had minor injuries and was transported by medics to Newton Wellesley Hospital to be evaluated. A section of 63 Sumner Street fence was damaged due to Motor Vehicle Accident. Owner (Sam Figler) cell phone: 650-269-4780 was informed of the damage. Witness states he saw MV1 come off of Sumner Street and get hit by MV2 on Commonwealth Avenue. Witness' vehicle did not sustain damage. Witness had no injuries. Several photos were taken of accident and turned into IT.

Officers were dispatched to the location of Commonwealth Avenue and Ash Street for a reported Motor Vehicle Accident. Officer Ciccone arrived on scene shortly before myself and stated there was no vehicle at this time at that location. Shortly after, I observed a red Ford Taurus traveling eastbound on Commonwealth Avenue turning onto Islington Road with a missing front bumper and airbags deployed (see incident #22005328). Operator of MV stated he was traveling westbound to a nearby hotel when he lost control of the vehicle and drove off the road. The MV sustained damage to the front of the vehicle and drove into the property of 58 Ash Street. The property had damage to the front lawn. All airbags were deployed. The Operator was taken to Newton Wellesley Hospital for injuries. The passenger had no injuries. The MV was towed by Tody's Service Inc. Due to the injuries I was unable to give the Operator (Anthony Earle) his Massachusetts Unifrom Citation (T1448313) for leaving the scene of an accident (property damage) Chapter 90 Section 24. I handed the citation to his girlfriend Kerry Materia.

On Monday 3/14/2022 at approx 1259hrs, while assigned to N-496, I responded to the intersection of Union St and Beacon St for a 2 car MVA. Upon arrival all parties were out of their vehicles and stated they suffered no injuries. Operator of MV1 states that he was merging onto Beacon St from Union St (one way). He states that he was looking left just prior to the accident and when he saw no EB traffic on Beacon he continued going forward. When he looked back in front of him he immediately saw MV2 but it was too late to stop. Operator of MV2 states he was attempting to turn left into the Gulf Gas Station at 732 Beacon St from Beacon St WB. He said that he believed MV1 would stop at the yield sign for Union St when it approached Beacon St but failed to do so. MV2 had passenger side airbag deployment and both occupants were evaluated by medics. MV2 was towed by Tody's Towing service.

Newton Police, Fire and Medics were dispatched to Commonwealth. Ave. and Dartmouth St. for Motor Vehicle Collision. Dispatch was advising a vehicle had collided into a poll. Upon arrival, Ofc. observed a Eversource street light poll laying on the grass carriage lane (westbound side). MV#1 was observed facing westbound in the eastbound lane of Commonwealth. Ave with front and side airbags deployed. The operator of MV#1 reported she was traveling westbound on Commonwealth Ave. prior to colliding into the light pole, but states she does not remember the moments prior to the collision.The operator of MV#1 reported she was uninjured, but was transported to Newton-Wellesley Hospital as a precaution. Newton Fire personnel determined the damaged light poll was not a hazard while Eversource was notified regarding their damaged poll. A witness to the collision reported they observed MV#1 traveling westbound on Commonwealth Ave. at approximately 20-25 mph, before it veered off the roadway and into the light poll. After colliding into the light poll MV#1 crossed over the double yellow line into oncoming traffic and hit the curb on the eastbound side of Commonwealth Ave. Pictures of MV#1 and the Eversource light poll were taken on the N493 camera and submitted to the NPD IT bureau. A Request for an Immediate Threat License Suspension/Revocation was faxed over to the Massachusetts RMV on behalf of the Operator of MV#1 due to the circumstances of this collision.
