Narrative:
On 07/25/19 at 10:28 hours, I responded to Centre Street and Boylston St off ramp (WB) for a two car motor vehicle accident with no injuries reported. Operator of MV1 states he was traveling straight ahead on Centre Street (NB) when he observed MV2 inching out of the Boylston Street off ramp. He was not able to stop in time and the two vehicles struck one another. I observed heavy damage to the passenger front bumper and tire area and the vehicle was towed by Tody's. Operator of MV2 states he was exiting the Boylston Street off ramp (WB) and was going to attempt a left turn on to Centre Street after stopping for the stop sign. He then slowly pulled out into the intersection due to National Grid trucks blocking his view and that is when he came in contact with MV1 on his driver's side front bumper. I observed moderate damage to the entire front bumper but the vehicle was still driveable. I did observe National Grid trucks parked at the intersection setting up to do construction work. One truck was parked on the left side of the off ramp and the other was parked on the side where the crash occurred. This did obstruct the operator of MV2's view when pulling out.

At approximately 1640 HRs on Wednesday, July, 31, 2019, I responded to the intersection of Centre St. at Hollis St. for a two vehicle crash. The operator of motor vehicle 1 stated she was traveling eastbound on Hollis St. while motor vehicle 2 was attempting to enter Centre St. southbound after stopping at the stop sign controlling the intersection. At this point, motor vehicle 2 reversed and struck motor vehicle 1 at the stop sign. The operator of motor vehicle 2 stated that he was entering the intersection in an attempt to travel southbound on Centre St. when traffic already southbound on Centre St. was flowing. In an attempt not to block northbound traffic, he reversed and accidentally struck motor vehicle 1. motor vehicle 1 sustained damage to its left fender and driver's side door areas. motor vehicle 2 sustained damage to its right rear bumper area and had preexisting scrapes to its right rear quarter and sliding door.. Neither party were injured and both vehicles left under their own power. I cleared without incident.

On Friday, August 16th 2019, at approximately 6:40pm, I, Officer Brooks, responded to Dunstan street at Washington street for a MVA pedestrian. Upon my arrival I spoke with the operator of MV1(294NZ5), who stated she was traveling southbound on Dunstan street and was attempting to pull out onto Washington street, when she made contact with a bicyclist traveling westbound on Washington street. The operator stated she never saw the bicyclist, and that she was traveling less than a mile per hour when she made contact. It should be noted that the sun was setting and the operators vision would have been impaired by the sun at the location of the accident. MV1 had a minor scuff mark on the front bumper. I spoke with the bicyclist who stated he was traveling westbound on Washington street and was crossing over Dunstan street when he was struck by MV1. He stated he was knocked off the bike, scraped his arm, and had minor back pain. The mirror on the right side of his bike was broken. The bicyclist was wearing greenish gray clothing, and was not wearing protective equipment such as a helmet. The bicyclist signed a refusal from Cataldo and was given a copy of the report number. Pictures were taken of the scene and all parties were sent on their way.

Valentine Baron stated on she was bicycling in the marked crosswalk at Center Ave at Center St from the north side walk to the south side walk. Baron stated that she activated the crosswalk light and had entered the crosswalk when the light was activated. Baron stated that while she was in the marked crosswalk vehicle one began to make a right turn on red . Baron stated that vehicle one struck her on her left side , knocking her off her bike . Baron stated that she had a helmutt but was not wearing it the time of the crash. Baron stated that she did not lose consciousness as result of the crash. Baron was transported via EMS to Mt Auburn Hospital for treatment for possible internal injuries. Operator of vehicle one Sara Ross stated that she was stopped at a red light on Center Ave at Center St. Ross stated that she was waiting to turn right on a red light and look to her left and then her right and drove forward. Ross stated that she did not see Baron in the marked crosswalk. Charles Hannum was a front seat passenger in vehicle one and stated that he did not see Baron in front of the vehicle until the crash occurred. I took photos of vehicle one and forwarded to the NPD IT bureau. I issued Ross M Citation T21065501 and cited her for a violation of CO 19/74 , failure to use care while turning.

Narrative:
The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave making a left turn onto Morton St when he was struck by MV#2. The operator of MV#1 stated he was unsure of where MV#2 came from. There was a strong solar glare facing westbound on Commonwealth Ave during this accident. MV#1 sustained heavy driver's side front end damages. Both front airbags were deployed. The operator of MV#1 stated he was not injured. The front passenger of MV#1 was not injured and had already left the scene to attend school. The driver's side rear passenger sustained a bleeding nose from this accident. She was unsure what caused her injuries because it happened too quickly. She was evluated by Newton Paramedics and signed a patient refusal of treatment. The initial point of impact for both vehicles was in the middle intersection of Commonwealth Ave at Morton St. There were broken vehicle parts and tire skid marks starting from the point of impact and onto the berm on Commonwealth Ave on the carriage lane side. MV#2's final resting spot was directly accross from 808 Commonwealth Ave. MV#2 sustained heavy front end damages to its driver's side wheel (Broken axle) and flat front right side tire. MV#2's driver's side airbag was deployed. The operator of MV#2 was transported by Newton Paramedics to the Boston Medical Center. I spoke to the operator of MV#2 via telephone and she stated she was travelling westbound on Commonwealth Ave when she was struck by MV#1 and caused her swerve onto the carriage lane berm. The operator of MV#1 is 6.5 months into her pregnancy and sustained neck, back, bruises and left hand injuries. She was transported to Boston Medical Center and was later discharged the same day. She was provided with this accident report number and was advised to contact her insurance provider. I spoke to the witness who was at the intersection of Cedar St and Commonwealth Ave. The witness stated MV#1 was travelling eastbound on Commonwealth Ave and had stopped, attempting to make a left turn onto Morton St. The witness stated she then observed MV#2 travelling westbound on Commonwealth Ave and struck MV#1. The witness stated she believed MV#2 was travelling at a high rate of speed because it continued to travel onto the berm after striking MV#1. I took digital photographs of the damaged berm on Commonwealth Ave. Based on the statements made to me, I issued in hand to the operator of MV#1 citation #T1268357 for violation of c89 s8 failure to yield when turning left. Both vehicles were towed by "Tody's Towing".

On Wednesday, October 16, 2019, I, Patrolman Hatfield, was assigned to marked cruiser N 495 to Division 5 for the 1530 to 2330 hour shift. At approximately 1742 hours, FTO Guarino and I were dispatched to Commonwealth Ave at Cedar St for a motor vehicle accident with minor injuries. FTO Guarino and I arrived on scene and noticed two MVs with damage. I spoke with the RO/operator of MV2, Janet Haines (Janet). Janet stated she was not suffering from injuries. Janet said she was operating southbound on Cedar St crossing Commonwealth Ave when she had a collision with MV2. Janet stated prior to crossing Commonwealth Ave, an unknown MV stopped at the intersection to allow her to cross. Janet then stated she proceeded accross when she crashed into MV1, who drove around the stopped MV. Janet said her brakes did not work and she continued to drive forward and ended on top of the curb of Commonwealth Ave facing eastbound. MV2 suffered damage to the whole front bumper and wheel wells. FTO Guarino spoke with the operator of MV1, Madeleine Richard (Madeleine). Be advised both front and side driver airbags were deployed, and the front driver's side door was unable to be opened. I then spoke with Madeleine. Madeleine stated she was operating eastbound on Commonwealth Ave before being involved in the collision. Madeleine stated she drove around the MV infront of her that was attempting to take a left from Commonwealth Ave onto Cedar St, and she did not see MV2 prior to the accident. MV1 was struck on the driver's side by MV2, and ended facing westbound in the middle of Commonwealth Ave. EMTs and fire arrived on scene. Janet stated she did not need to be transported. Madeleine was transported to Newton Wellesley Hospital for an evaluation. Both MVs were towed by Todys.

On Wednesday October 23, 2019, I, Patrolman Hatfield, was assigned to marked cruiser N495 in full uniform to Division 5 for the 1530 to 2330 hour shift. At approximately 2040 hours, myself and Officer Selig were traveling northbound on Centre St (a public way) in Newton Center. We were behind MA reg 9KT873 (2004 Toyota Corolla grey). We observed the motor vehicle drive around a pedestrian in the crosswalk who was attempting to cross at Centre St and Pelham St. I activated my emergency blue lights and attempted to conduct a motor vehicle stop of the Toyota Corolla. The operator continued to drive northbound on Centre St before pulling over just before the crosswalk at Centre St and Pleasant St. The operator then continued driving forward and struck a pedestrian crossing in the crosswalk Officer Selig and I saw the pedestrian being struck before landing on top of the Toyota Corolla hood, and falling forward onto the street. Officer Selig and I spoke with the two parties involved. I spoke with the victim, who was identified as Vivian Rubinstein (Vivian). Vivian crawled towards the sidewalk of Centre St while clutching her lower back. Initially Vivian denied being injured and refused medical aid. The Toyota Corolla operator was identified as the register owner Elise Ruben (Elise). Elise stated to Officer Selig that her front windshield was foggy and when she noticed the blue lights, she attempted to pull over. Elise stated she did not see the pedestrian in the crosswalk. Elise appeared very upset and refused medical treatment. Officer Selig was able to speak to two witnesses of the accident. The witnesses were identified as Stephan Aparicio (Stephan) and Mikaela Rice (Mikaela). Stephan said he was behind the victim in the same crosswalk. Stephan stated he saw the Toyota stop in front of the crosswalk and then drove forward hitting the female. Mikaela stated she was parked in her motor vehicle on Centre St facing northbound (facing the crosswalk). Mikaela stated she saw the female walking halfway across the crosswalk and the Toyota continuing drive forward before hitting the female. Officer Siegel arrived on scene and took four photographs which was submitted with this report. Vivian was transported by Newton Medics to Faulkner Hospital for an evaluation. I issued written warning citation number T2079786 to Elise for failure to yield to a pedestrian in crosswalk (Centre St at Pleasant St). I also issued Elise citation number T2079787 for failure to yield to a pedestrian in crosswalk (Centre St at Pelham St). The vehicle was driven from the scene.

On 11/27/19 at 12:43 hours, I responded to the Petsmart located at 215 Needham Street for a motor vehicle bicyclist accident. The weather at the time was approximately 50 degrees, cloudy and the road conditions were dry. No injuries reported. Operator of MV # 1 (Barbara Cooper) states she was facing eastbound on Needham Street waiting in the center turn lanes to take a left turn in to the garage of 215 Needham Street. Next, she states that a large truck going westbound stopped for her and waved her on to turn. She then proceeded in to the garage and parked and that is when she was confronted by the bicyclist saying that she was struck with her vehicle. The operator was unaware of an accident occurring and was not able to visibly see past the truck while turning. I observed a minor scratch to the passenger rear quarter panel area of the vehicle. Operator of the bicyclist (Merissa Brousseau) states she traveling westbound in the bicycle lane on the right side near 215 Needham Street when she observed the large truck mentioned above suddenly stop. She states that she was also visibly blocked from seeing ahead due to the truck. She then kept peddling straight through and struck MV # 1 in the passenger side rear end as it was making its left turn. The operator was evaluated by Cataldo Ambulance but refused medical attention. I observed no apparent damage to the bicycle. Ms. Brousseau was wearing a bicycle helmet, black and red Boston University bicycle suit, and red BU sock covers over her shoes. On Wednesday, 12/4/2019, at approximately 2045 hours, Ofc Helms and I responded to a past hit and run at 27 Parsons St. Upon arrival, I spoke with the owner of MV 1 (Marc Rizza) who stated that he noticed damage to the left rear of his vehicle today when he came back from vacation to pick up his car. MV 1 was parked in the driveway of 27 Parsons St, (the brother in law of the vehicle owner). He further stated that he believed a plow may have struck his car when it was plowing the driveway after or during the last snow storm. Yellow streaks were visibly seen on the left rear bumper consistent with the damage. The owner of MV 1 further stated that he believed the house across the street, 28 Parsons St is the person that usually plows the driveway. I spoke with Jeffrey Timmins who told me that he in fact did plow the driveway of 27 Parsons St after the storm sometime on Tuesday 12/3/2019. Mr. Timmins admitted to being the operator of MV 2 and stated he did not notice if he hit MV 1 while plowing the driveway but he "could have". I advised both parties with an accident report number. No tows required or reported injuries by either party.

At approximately 0500HRs on Tuesday, December 17, 2019, I was dispatched to a 2 vehicle MVA in front of 38 Commonwealth Ave. Upon arrival, I spoke with the operator of MV1 (MA CON: S25585) who stated that he attempted to back up his vehicle (Westbound) and accidently struck MV2 because he was not able to see it in his mirror. Upon speaking to the operator of MV2 (MA CON: S13628), he stated he was traveling Eastbound as MV1 was backing up. He then stopped MV2 and when MV1 did not, he activated his horn which had no effect and MV1 backed into him. I observed MV1 to have no discernable damage. I observed MV2 to have sustained damage to its front bumper, grille, hood, passenger's side fender and both head lamps. MV1 had a single passenger who reports he did not witness event in question, as he was focused on eating at the time. No one involved reports any injuries. I cleared without incident.

The operator of MV#1 stated he was parked at the street bend of 64 Homer St when he was struck from behind by MV#2. MV#1 sustained heavy damages to its driver's side rear bumper area.
