Narrative:
The operator of Vehicle #1 stated that as he was attempting to make a left turn from Washington St, to Lewis St, he collided with Vehicle #2. He added that he believed Vehicle #2 was travelling at a high rate of speed. The operator of Vehicle #2 stated that as he was travelling Eastbound on Washington St, he collided with Vehicle #1 as he attempted to make a left turn. The operator of Vehicle #3 stated that Vehicle #2 was travelling at a high rate of speed, and went through the red light, prior to the collision with Vehicle #1. As a result of the collision with Vehicle #2, Vehicle #1 struck Vehicle #3 with its rear driver's side. Vehicle #3 was damaged in the front bumper, as well as the driver's side quarter panel. The operator of Vehicle #1 was transported to Newton-Wellesley Hospital for an evaluation. Both Vehicles 1, and 2 were towed by Tody's Towing Co.

Narrative:
On Tuesday, July 23rd 2019, at approximately 5:24pm, I, Officer Brooks, responded to 444 Lowell avenue for a 2 car MVA. Upon my arrival I observed MV1(NH REG 4120041) with heavy front end damage. The vehicle was located in the driveway of 444 Lowell avenue, with the front end sticking out into the roadway. I spoke with the operator of MV1 who stated he was trying to exit the driveway and turn left onto Lowell avenue southbound, when he was struck by MV2(MA REG 3AP243) which had been traveling northbound on Lowell avenue. According to the operator of MV1, MV2 had been traveling at a high rate of speed and tried to swerve around him. There was airbag deployment in MV1 however the operator signed a patient refusal with Cataldo. MV1 was towed by Tody's. I then spoke with the operator of MV2 who stated she was traveling northbound on Lowell avenue and saw MV1 exiting the driveway. She assumed he was going to stop pulling out of the driveway, and when he did not she stated she attempted to swerve around him. MV1 made contact with the passenger side rear wheel well of MV2 causing MV2 to spin out. The operator was not injured and arranged for a private tow due to the damage in the wheel well area.

Operator #2 states he was pulling out of his driveway to turn right onto Auburn St when vehicle #1 hit the left driver's side of the vehicle. Operator #2 did not see vehicle #1 approaching. All parties declined medical attention but vehicle #2 had to be towed to Tody's due to moderate damage to the left driver's side door and bumper. Vehicle #1 sustained minor damage to the front right bumper but was driveable.

Operator #1 states he was traveling southbound on Cherry St when he observed vehicle #2 drive through the stop sign at the end of Harris Rd, resulting in a collision with the front end of his vehicle, vehicle #1. Operator #2 states she was traveling westbound on Harris Rd and did not see the stop sign as she approached Cherry St. She subsequently drove through the stop sign and made contact with vehicle #1. Her intention was to cross Cherry St and go down Cherry Place, which is directly across the street from Harris Rd. No injuries were reported. Vehicle #2 sustained damage to the right rear corner as well as the undercarriage and was towed by Tody's. Vehicle #1 was parked legally on Cherry St and the operator has his own tow coming, as the vehicle is a contracted Waltham school bus van. It should be noted that no students were on board the van at the time of the collision.

MV #1 was traveling northbound on Lagrange St. MV #2 was traveling southbound on Lagrange St. when the operator turned eastbound onto Corey St.(Boston). As MV#2 turned eastbound it collided with MV #1. Significant damage was observed to both MV#1 and MV#2's front bumpers and engine compartments. Both MV#1 and MV#2's airbags were deployed upon impact. The Operator of MV#1 reported she was uninjured while the Operator of MV#2 was transported to the hospital. Both vehicles were towed from the scene by Tody's Tow Service.

Operator #1 states he was driving northbound on Seminary Ave in MA reg 5LE114 (vehicle #1), when he was t-boned on the driver's side by MA reg 959SD7 (vehicle #2). Operator #2 states that he was attempting to turn left out of the Arnow Campus Center parking garage and did not see vehicle #1 approaching from the right. It should be noted that the physical address of the parking garage appears to be 131 Woodland Rd, however the collision occurred on the Seminary Ave side. All parties declined medical attention and no tows were needed. Vehicle #1 sustained significant damage along the driver's side doors. Vehicle #2 has moderate front end damage. Operator #2 was issued Ma citation T1268432 in hand for Failure to Yield to Oncoming Traffic. 131 Woodland Rd will be the address used in documenting this crash.

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

Mv#1 operator stated he came to a full stop at the end of Pleasant St N/B. #1 stated he pulled forward to turn left W/B on Homer St. At that time, #1 passenger side front end collided with Mv#2 driver's side front end which was travelling on Homer St W/B. #1 stated he did not see #2 on Homer St. Significant damage sustained to #1 front end and towed by Tody's. #1 all occupants reported no injuries. #2 operator stated he was travelling on Homer St W/B at approximately 30mph. #2 stated he saw #1 emerge from Pleasant St. #2 stated he did not have enough time to stop at which time the collision occurred with #1. Heavy damage sustained to #2 front end and towed by Tody's. After initial questioning of #2 operator, #2 later stated he could feel some pain in the right leg from getting jammed into the console area. #2 refused a medical evaluation. A beginning of a skid mark was left behind by #2 in the middle of the intersection of the Homer St W/B travel lane on the Kenwood Ave side, where the collision also occurred. A bystander witness who wished to remain anonymous stated #1 came to a full stop before proceeding left W/B on Homer St. The anonymous witness also stated #2 was travelling in excess of 30mph.

Narrative:
Mv#1 operator Feng Zhu stated he was travelling on Greenwood St W/B. Zhu stated as he approached Cynthia Rd, he slightly veered left to avoid a parked MV on Greenwood St. Zhu stated at that time Mv#2 suddenly emerged from Cynthia Rd without stopping turning right onto Greenwood into the path of #1. Zhu stated his driver's side door was struck by the front of #2. Zhu was not injured. #1 sustained significant driver's side door damage. Zhu stated #2 operator stopped after the crash and provided his name Boris Dubinsky, phone number 617-470-4874 and discussed the crash. Zhu took a photo of #2's plate (MA reg 19GA78) before #2 drove away prior to my arrival. Zhu was able to identify #2 as the same operator from an RMV photo query on scene of the plate provided. I responded to Dubinsky's address at 155 Cynthia Rd. I met a woman there who said he was not home. She called the number and said nobody was answering. I had the woman verify the number and she said it was 617-470-3874 (one digit different than above). I told the woman to have Dubinsky call me at HQ. He never did. I called him later from HQ myself. Dubinsky stated he came to a stop at the end of Cynthia Rd. He stated as he turned right, #1 struck him on his left front driver's side because #1 went to the other side of the road due to a truck parked on Greenwood St. I advised him Greenwood St has the right of way and he was required to yield from Cynthia Rd. Dubinsky stated after he gave his name and number, Zhu was on his phone for a period of time so he left. Dubinsky was advised to provide his license and registration to complete the proper exchange after an accident. When asked about the different phone number, he stated Zhu must have typed in the wrong number. I advised Dubinsky I would be citing him for Failure to Yield (Ch89/Sec9) to be sent in the mail. He was lastly advised his option to appeal if he so wishes.

On 10/02/2021 while assigned to marked cruiser N503 at approximately 20:46 hours I, Officer Vazhenin, responded to 601 Washington Street for a report of 2-car motor vehicle accident. Upon arrival on scene, I spoke to Mr. Jeffrey Yanfeng Xiao (Driver 1), who stated that he was driving eastbound on Washington Street when he observed another car (Vehicle 2) pulling out of 7/11 parking lot at 612 Washington street. Driver 1 said that in order to avoid collision with Vehicle 2, he had to swerve to the left in oncoming traffic lane crossing double yellow line. He said that while he was passing Vehicle 2, Vehicle 2 kept driving and crashed into the passenger side of his car (Vehicle 1). I then spoke to Ms. Shahrbanou Asadi, the operator of Vehicle 2, who stated that she was pulling out of 7/11 parking lot and was trying to make a left turn on Washington street. She stated that Vehicle 1 "showed up out of nowhere" and was traveling at high rate of speed. Ms. Asadi stated that she proceeded making her left turn when Vehicle 1 suddenly appeared in the front of her car and her car (Vehicle 2) crashed into passenger side of Vehicle 1. It should be noted that Ms. Asadi did not have her driver's license in possession. License plate inquiry of her car (Vehicle 2, MA Reg.#883MH8) revealed the fact that her driver's license was expired, which she failed to disclose to me when asked about status of her registration and driver's license. I ussued a Criminal Application to Ms. Shahrbanou Asadi on Massachusetts Uniform Citation #T1445734 for Ch.90 S.10 "Unlicensed Operation of Motor Vehicle", Ch.89 S.8 "Failure to yield", Ch.90 S.11 "License not in possession".

MINOR DAMAGE SUSTAINED TO MV 1 AND NO INJURIES WERE REPORTED. THE OPERATOR OF MV 2 DID NOT SPEAK ENGLISH (ONLY SPANISH SPEAKING), HOWEVER HER SIDE OF THE STORY WAS EXPLAINED TO ME THROUGH HER DAUGHTER WHO WAS USED AN INTERPRETER. SHE STATED THAT SHE WAS IN FRONT OF MV 1 MERGING ONTO CENTRE ST FROM CENTRE AVE, WHEN MV 1 CAME FROM BEHIND HER AND TRIED TO PASS HER ILLEGALLY ON HER RIGHT HAND SIDE CAUSING THE COLLISION. MINOR DAMAGE TO THE PASSENGER FRONT SIDE AND NO INJURIES WERE REPORTED. BASED ON MY INVESTIGATION IT APPEARS THAT MV1 WAS IN THE PROPER LANE WHEN MERGING AND MV 2 MAY HAVE BEEN AT FAULT FOR THE MINOR COLLISION. BOTH PARTIES SWAPPED INFORMATION AND WERE SENT ON THEIR WAY.

On Monday 3/14/2022 at approx 1259hrs, while assigned to N-496, I responded to the intersection of Union St and Beacon St for a 2 car MVA. Upon arrival all parties were out of their vehicles and stated they suffered no injuries. Operator of MV1 states that he was merging onto Beacon St from Union St (one way). He states that he was looking left just prior to the accident and when he saw no EB traffic on Beacon he continued going forward. When he looked back in front of him he immediately saw MV2 but it was too late to stop. Operator of MV2 states he was attempting to turn left into the Gulf Gas Station at 732 Beacon St from Beacon St WB. He said that he believed MV1 would stop at the yield sign for Union St when it approached Beacon St but failed to do so. MV2 had passenger side airbag deployment and both occupants were evaluated by medics. MV2 was towed by Tody's Towing service.
