On 07/16/19 at approximately 09:38 I responded to a two vehicle crash on Needham St. outside of #141. The Operator of Veh #1 stated she was turning left on Needham St when the South bound traffic stopped to left her turn. As she turned, Veh#2 drove South down the multi-turn lane and they collided nearly head on. Heavy damage and air bag deployment to Veh #1. The operator was checked out by Medics and signed a patient refusal. The Operator of Veh #2 stated she was driving down the multi-turn lane South on Needham St.. She was preparing to take a left onto Jaconnet St when Veh #1 pulled out of parking lot and collided with her vehicle. I observed moderate to heavy damage to Veh #2 and noted that the vehicle didn't have its directional on. The operator of Veh #2 complained to left leg pain and was transported to Newton-Wellesley Hospital By Ambulance #1. Both vehicles were towed by Tody's Towing. At the conclusion of my investigation it appears as if both vehicles were at fault during this crash; Veh#1 for not yielding to right of way and Veh #2 for traveling too soon in a turn lane, no directional and driving too fast for the heavy traffic conditions. Both operators were given verbal warnings and there will be no citations issued at this time.

Narrative:
On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

The operator of MV#1 stated she was travelling southbound on Centre St in the left side travel lane and attempted to switch lanes into the right side travel lane. The operator of MV#1 stated MV#2 was in her blind spot and made MV#2 swerved into MV#3. MV#1 never made contact with MV#2. There were no reported injuries to the operator of MV#1 or its passenger. There were no damages to MV#1. The operator of MV#2 stated he was travelling southbound on Centre St in the right side travel lane when MV#1 abruptly entered his travel lane causing him to swerve into MV#3. MV#2 sustained heavy front right side damages. There were no reported injuries to the operator of MV#2. MV#3 was unoccupied and parked in front of 1211 Centre St. MV#3 sustained moderate damages to its left side. The owner of MV#3 was advised of this accident. Based on the statements made to me, I will mail citation #T1268347 to the operator of MV#1 for violations of c89 4A marked lanes violation.

On 07/29/19 at 11:13 hours, I responded to the Dunkin Donuts parking lot located at 138 Needham Street for a three car motor vehicle accident. Operator of MV1 stated at first she could not recall what occurred during the accident due to lightheadedness at the time. After, she then stated after pulling in to the Dunkin Donuts lot she was attempting to back in to a parking spot in the back of the lot. While backing up, she attempted to hit the brake pedal but must have pushed the gas pedal instead. The vehicle then struck parked/unoccupied MV2 and MV2 was then pushed in to parked/unoccupied MV3. The operator was checked out by Cataldo Ambulance and signed a patient refusal. MV1 sustained moderate driver's side rear bumper area damage but it was still driveable. MV2 sustained damage along both sides of the vehicle and a rear broken axle and it was towed by Tody's. MV3 sustained scrapes and dents to the entire driver's side but the vehicle was driveable.

Operator of motor Vehicle # 2 stated that she was travelling Westbound on Brookline Street in Newton and wanted to take a left onto Dedham Street however due to the traffic detour she was forced to take a right onto Dedham Street (Northbound). Operator Motor Vehicle # 2 stated that she attempted to take a left turn into a driveway so she could turn around and thought she could make a U Turn instead. Operator of Motor Vehicle #2 stated that she took the left and did not see the vehicle that was in the left lane next to her and struck vehicle # 2. Motor Vehicle #2 sustained moderate drivers side, side damage, and motor vhehicle #1 sustained moderate front end damage. Both parties reported no injuries and signed medical refusal forms.

The operator of MV#1 stated he was stationary on Centre St northbound when he was struck from behind by MV#2. MV#1 sustained moderate damages to its rear bumper. There were no reported injuries to the operator of MV#1. The operator of MV#2 stated he was stationary on Centre St northbound when he was struck from behind by MV#3 which in turn pushed him into MV#1. MV#2 sustained moderate damages to its rear and front end. The operator of MV#2 reported non-incapacitating injuries to his back and right shoulder area. He was evaluated by Newton Paramedics and refused medical attention. The operator of MV#3 stated he was travelling northbound on Centre St when he looked to his right at a dump truck and took his vision off the street for a second when he struck MV#2 which in turn pushed MV#2 into MV#1. MV#3 sustained heavy front end damages and had both front airbags deployed. The operator of MV#3 stated both of his thumbs were hurting from the airbag deployment. He was evaluated by Newton Paramedics and refused medical attention. The operator of MV#2 does not have a driver's license and only provided a Brazilian passport. He was issued in hand MA uniform citation #T1268350 for violation of c90 s10 unlicensed operation. A licensed operator (MA license #S48386590) took possession of MV#2 and transported the operator of MV#2 back to his home. Based on the statements made to me I issued in hand to the operator of MV#3 MA uniform citation #T1268351 for violation of C.O 19-71 following too closely. MV#3 was towed by AAA towing and the operator of MV#3 was transported home by his parents.

MV1 was travelling behind MV2 when he attempted to pass her in a no passing zone. As he attempted to cross back to the correct side of the road MV1 made contact with the front drivers side of MV2. MV2 was travelling WB on Commonwealth ave when MV1 attempted to pass her and she was struck as he entered back into her lane. Operator 1 states operator 2 was making obscene gestures at him and he was attempting to separate himself from the situation and that is why he attempted to pass where it was illegal. Operator 2 states MV1 was tailgating her for approximately 3 blocks when she threw her hands up in the air and he then attempted to pass her. MV1 sustained minor rear end damage. MV2 sustained moderate front end damage. Both vehicles driven from the scene and no injuries were reported. Operator 1 issued MA Uniform Citation #T1445050 for marked lane violation.

Owner of Motor Vehicle #1 stated that he was exiting the gym at Planet Fitness and he noticed that someone was leaving a piece of paper on his front windshield. He was approached by the man who told him that he witnessed a Gray Dodge Ram bearing MA Reg 9JGD90 strike his Jeep Grand Cherokee that was parked and unoccupied causing moderate rear drivers side damage. MA Reg 9JGD90 came back to a 2018 Dodge Ram 1500 Gray in Color and registered to a Kenneth Doucette out of 41 Thaxter Street, Newton MA. I was able to locate several telephone numbers from our in house system however those numbers were no longer in service. I made several attempts to make contact with Mr. Doucette at his residents and had negative results. N491 Officer D. Healy also made an attempt to make contact with Kenneth Doucette later on in the evening and had negative results. I left a voicemail message to the witness and have yet to receive a return call for questioning. This will be further investigated on my next tour of Duty. **UPDATE Wednesday 09/04/19** At approximately 10:00 a.m. I went to 41 Thaxter Street to once again attempt to make contact with Kenneth Doucette and had negative results. I left another voicemail for the witness Claudio Silva and had negative results. I then spoke to Traffic Officer M. Gaudet to inform him about this incident and he stated that he will follow up with this investigation. Traffic Bureau update (Officer Gaudet): On September 4, 2019, I attempted to make contact with the witness to the crash, Claudio Silva, with a negative result. I made contact with the registered owner of MV2, Kenneth Doucette (S80920300). Doucette stated he was operating MA: 9JGD90 at the time of the accident. Doucette stated he pulled into a parking spot next to MV1 and stated he didn't think he "hit it hard". Doucette stated a short time later he left the area and when he got back to work he observed damage to his own vehicle. Doucette stated he had damage to the front passenger bumper area. Doucette stated he returned to the crash scene to see if the other vehicle had damage but it was gone. Doucette was advised and provided Massachusetts Uniform Citation T2016422 in hand for Chapter 90, Section 24 (Leaving the Scene of Property Damage, MV).

On 09/04/19 at 08:22 hours, I responded to 1052 Chestnut Street for a three car motor vehicle accident with minor injuries. All four parties involved were shaken up as a result of the accident but signed patient refusals with Cataldo Ambulance One. Operator of MV1 states she was traveling straight ahead (SB) by 1052 Chestnut Street when she observed several vehicles ahead start to proceed through the light. She then heard her phone ring and went to retrieve it by looking down for a second. When looking back up, the traffic directly in front of her did not move yet and she could not stop in time striking MV2 in the rear end. MV2 was then pushed in to MV3. Both MV2 and MV3 were stopped in traffic at the time awaiting to travel through Chestnut Street at Elliot Street. MV1 sustained moderate front end damage to the bumper and grille areas. MV2 sustained heavy rear end bumper and hatch area damage. MV3 sustained minor scratches to the rear bumper. All vehicles were still driveable.

OPERATOR OF MV 1 STATED THAT SHE WAS IN THE PARKING LOT BEHIND 999 BOYLSTON STREETAND WHILE SHE WAS IN A SPOT, SHE BELIEVES SHE ACCIDENTALLY STEPPED ON THE GAS PEDAL AND THE VEHICLE MOVED FORWARD. THE VEHICLE STRUCK A SECTION OF THE WOODEN FENCE WHICH FELL AND CAUSED MINOR DAMAGE TO THE VEHICLE PARKED NEXT TO THEM. MODERATE DAMAGE SUSTAINED TO MV 1 AND NO INJURIES WERE REPORTED. OWNER OF MV REQUESTED A PRIVATE TOW FOR THE VEHICLE. TWO OTHER VEHICLES WHO WERE PARKED CLOSE TO MV 1 MAY HAVE BEEN DAMAGED BY THE FALLING FENCE.

A BICYCLIST WAS TRAVELING WEST ON WASHINGTON STREET AND JUST AS HE WAS PASSING MV 1, THE OPERATOR OF MV 1 OPENED UP HIS DRIVERS SIDE DOOR. THE DOOR OPENED RIGHT IN THE PATH OF THE BICYCLIST AND A COLLISION OCCURRED, SENDING THE BICYCLIST TO THE GROUND. HE SUSTAINED MODERATE INJURIES AND SIGNED A PATIENT REFUSAL. THE OPERATOR OF MV 1 SUSTAINED NO INJURIES AND HIS DOOR SUSTAINED MINOR DAMAGE.

Operator of MV1 stated he was traveling Eastbound on Beacon St. and stopped for a pedestrian in the crosswalk located in front of 281 Beacon St. The operator of MV1 further stated as he came to a complete stop at the crosswalk MV2 rear ended him. Operator of MV2 stated she was traveling Eastbound on Beacon St. when she stopped behind MV1 in traffic. Operator of MV2 further stated as she was stopped in traffic MV3 rear ended her with enough force resulting in her vehicle colliding with MV1. The operator of MV3 stated she was traveling Eastbound on Beacon St. when she observed MV2 stopped in traffic, however she attempted to stop to avoid a collision but rear ended MV2. It should be noted the operator of MV3 attempted to turn her vehicle away from MV2 but she stated it was too late. No injuries. All involved parties signed patient refusals. MV1 sustained minor rear end damage. MV2 sustained moderate front and rear end damage. MV3 sustained heavy front end damage. Tody's responded and towed MV3. A towed motor vehicle form was filled out and placed on file.

On 10/5/19 at 1430 I responded to the parking lot of the Marshalls Dept store for a past hit and run MVA. Upon arrival I spoke with the owner/operator of MV#1 who stated she parked her vehicle, went into the store and when she came outside her front driver side bumper was hit. There was moderate damage to the vehicle but she was able to drive it home. There were no injuries. I checked the department stores for surveillance, which neither the CVS or Marshalls had any exterior cameras facing the parking lot. There were no witnesses on scene and the owner of MV#1 did not see who hit her.

Vehicle #1 was parked on the right side of the road and was unoccupied at the time of the crash. Vehicle #2 was attempting to parallel park behind Vehicle #1 and struck Vehicle #1 in the left rear with the right rear of Vehicle #2. A witness stated they watched Vehicle #2 hit Vehicle #1 in the attempt to park and then drove off after the accident leaving no information. The Witness left a note with Vehicle #2's information along with his information in regards to the accident. Vehicle #1 had moderate damage to the left rear and left side. Vehicle #2's damage is unknown at this time. I attempted to contact the owner of Vehicle #2 via the Boston Police with negative results. I was able to contact a relative of Vehicle #2 who refused to give me the drivers information but stated he would pass the message along to the driver. Ms.CORIOLAN, Roodline contacted me back and stated she was involved in an minor accident while attempting to park but stated there was no to little damage done. Ms. CORIOLAN was advised of the accident report that was filed. I contacted the owner of Vehicle#1 and advised him that I obtained all of Vehicles #2's information and informed him that it would all be in the accident report.

Vehicle #1 was parked facing south in front of 7 Spaulidng La. Vehicle #1 had a trailer with an excavator attached to it. Vehicle #1 was unoccupied but had one occupant in the excavator on the trailer. The operator/owner of vehicle #1 was moving the excavator off the trailer when the trailer and truck started to roll southbound down the hill on Spaulding La. The operator of vehicle #1 jumped out of the excavator onto the roadway. Vehicle #1 along with its trailer rolled down and veered off the road and over the curb. The rear driver side end of the of vehicle #1 collided with the rear end of vehicle #2 bringing the trailer and truck to a stop. The rear end of vehicle #2 had moderate damage. The rear end of vehicle #1 had minor damage. Vehicle #2 was parked facing southbound just south of the driveway of 7 Spaulding La. Vehicle #2 was parked on the side of the road on Spaulding La. and was crashed into by the unoccupied truck and trailer vehicle #1. The occupant of vehicle #2 was not injured. On Thursday, November 14, 2019, while assigned to Traffic unit N525, I responded to the rear parking lot of the Angier Elementary School located at 1697 Beacon Street for a motor vehicle crash involved a City of Newton owner vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. The operator of MV1, Jessica Moore, stated she was backing out of a parking spot in the rear of the Angier Elementary School while operating a 2007 Chrysler Town and Country (MA:395KR9). Moore stated she did not see MV2 passing by as she was backing out and the rear driver side bumper area of her vehicle crashed into the rear driver side of MV2. Moore reported no injuries. I observed moderate damage to the rear driver side bumper area of MV1. The operator of MV2, Patrice Lys, stated he was traveling Westbound in the parking lot of the Angier Elementary school in a 2018 Ford EcoVan (MA MVN: M2546A). Lys stated a vehicle on his driver side backed out of a parking spot and the rear end of MV1 crashed into the rear driver side of MV2. Lys reported no injuries. I observed minor damage to the rear driver side area of MV2. The operator of MV1 was cited with Massachusetts Uniform Citation T2016428 for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care, Backing). Photos were submitted to the IT Bureau.

Operator #1 states he was driving northbound on Seminary Ave in MA reg 5LE114 (vehicle #1), when he was t-boned on the driver's side by MA reg 959SD7 (vehicle #2). Operator #2 states that he was attempting to turn left out of the Arnow Campus Center parking garage and did not see vehicle #1 approaching from the right.
