While turning around in the parking garage at The Towers of Chestnut Hill at 250 Hammond Pond Pkwy, Vehicle 1 backed into a drainpipe for the building's sprinkler system causing minor damage to the pipe. On 5/2/22 staff at the Towers provided security footage confirming that Vehicle 1 had caused the damage.

Vehicle 1 was in the left turn only lane and attempting to make a left turn while Vehicle 2 was in the adjacent straight only lane to the right. When Vehicle 1 attempted to navigate the turn, he first swung wide to the right causing his front passenger side quarter panel to make contact with Vehicle 2's driver side rear quarter panel. I observed minor dents and scratches to both vehicles and no one was injured. Both operators agreed that they would have the damage repaired without going through insurance and wished me to document the incident for liability reasons only.

On 4/22/22 I responded to 101 Charlemont St. for a past MVA that occurred on 4/21/22 at 0700. Operator of MV1 stated they were backing out of their driveway and onto Charlemont St. when MV2 was traveling Eastbound and struck the back of their vehicle. MV1 sustained damage to the rear end. Operator of MV2 stated they were traveling Eastbound on Charlemont when they didn't notice MV1 backing out of their driveway. The front of MV2 hit the rear of MV1. MV1 sustained damage to the front end. MV2 was issued MA Uniform Citation #T1447649 for Ch. 90 Sec. 9 Unregistered Motor Vehicle (Straight Expiration). It should be noted MV2 was down at an auto body shop in Scituate at the time I took the report. As of 4/28/22 the vehicle's registration is now active.

MV1 was traveling WESTBOUND when the operator, Gabriel Jabbar, found himself falling asleep behind the wheel. Mr. Jabbar had been driving to Boston from Missouri for two days straight. MV1 sustained heavy front end damage as it collided head on with a tree on the median inbetween the Commonwealth Ave carriage road and Commonwealth Ave. Tody's towed the vehicle. Mr. Jabbar signed a refusal with the medics.

On Monday, May 9, 2022, while assigned to Traffic unit N525, I responded to the area of 25 Berkshire Road, Newton, for a report of an MVA involving a City of Newton owned street sweeper and a City of Newton contracted school bus. The weather at the time of the crash was clear and sunny. The road surface was dry. Berkshire Road is a public way maintained by the City of Newton. The operator of MV1, Mr. Norman Fryar (S20854611), stated he was operating a City of Newton owned 2017 Elgn Pelican street sweeper (MA: MVN M96170) on Berkshire Road. Mr. Fryar stated as he was sweeping the street he was in the Eastbound lane traveling North. Mr. Fryar stated there was a vehicle parked ahead of him and to avoid it he began to change lanes to his left to go around it. Mr. Fryar stated as he was chaning lanes his phone was ringing and as he reached for the phone the driver side of his sweeper crashed into the front passenger side of a passing school bus. Mr. Fryar reported no injuries. I observed damage to the driver side of the sweeper. I spoke with the operator of MV2, Ms. Ana Mendes (S72311538). Ms. Mendes stated she was operating a City of Newton contracted school bus (MA: BU44536) on Berkshire Road with 25 students on board with a final destination of Bigelow Middle School. Ms. Mendes stated she was traveling North on Berkshire Road when she observed a street sweeper in front of her. Ms. Mendes stated she entered the Westbound land of Berkshire Road to go around the street sweeper. Ms. Mendes stated as she was passing the street sweeper, it made an abrupt left turn and it's driver side crashed into the front passenger side door/mirror of the school bus. Ms. Mendes reported no injuries. I observed moderate damage to the passegner side of MV2. After the initial crash, it was reported by both parties that MV1 attempted to back up to allow students to exit the bus through the passenger side door. As MV1 was backing up, it crashed into the middle passenger side of the school bus. There were 25 Bigelow Middle School students on the bus at the time of the crash. Ms. Mendes state she contacted her dispatch for further instructions on what to do with the students. Ms. Mendes stated to Sgt. Wade that the Eastern Bus Company dispatch instructed her to keep the students on the bus and wait for Police and EMS to arrive. Ms. Mendes stated after speaking with her dispatch, another school bus arrived at the scene. The decision was made by Ms. Mendes to have the students exit her school bus and be transported to Bigelow Middle School prior to my arrival. Ms. Mendes was advised in the future to follow the policy set forth by her Eastern Bus Company, the City of Newton, and the Newton Police Department. After clearing Berkshire Road, myself and Sgt. M. Wade responded to the Bigelow Middle School to check on the welfare of the students that were on the bus at the time of the crash. We were greeted by the school's Asssitant Principle Mr. Lucas Mogensen. Mr. Mogensen stated all of the students that were on the bus at the time of the crash were staged on the bus when it arrived at the school. Mr. Mogensen stated the school nurse Ms. Shannon Loughnane evaluated each student as they exited the bus and there were no injuries reported at this time.

The operator of MV#1 stated he was travelling eastbound on Beacon St attempting to make a left turn onto Walnut St when he was struck by MV#2 pulling out of the driveway of 1094 Beacon St. MV#1 sustained heavy front end damages. There were no reported injuries to the operator of MV#1. The operator of MV#2 stated he was pulling out of the driveway of 1094 Beacon St attempting to make a left turn onto Beacon St westbound when he struck MV#1. MV#2 sustained heavy front end damages. There were no reported injuries to the operator of MV#2. MV#1 is registered in New Brunswick Canada (New Brunswick REG JSO184. VIN #5UXTR9C54JLD89371) and the operator has a New Brunswick Canadian driver's license #1192493. I had to enter a random state information for the vehicle/ license for the purpose of validating this report.

On Saturday 5/14/2022 at approx 1252hrs, while assigned to N-496, I responded to 1672 Washington St in Newton for a MVA hit & run. There I met the operator of MV1 who states that a gray vehicle had just struck his. He could not provide any additional information but said he will try to get footage of the accident from his dash cam. He sates he was traveling EB on washington St in the left lane when an emergency vehicle ( possibly police) traveling WB, came into his lane to get around stopped traffic. He said he then changed lanes and went in front of the car that was directly to his right but another car drove around that car from the bike lane and struck the passenger side of his. He said the gray vehicle continued EB on washington St. I observed sideswipe damage to MV1's passenger side doors.
