On Saturday, January 12th 2019, at approximately 4:12pm, I, Officer Brooks, responded to a single vehicle accident in the area of Cherry street and Derby street, both of which are public ways in the city of Newton. Upon arrival in the area I observed the fire department off with MV1(MA REG 9PAK20), in front of 171 Cherry street. The vehicle had a fire hydrant lodged under the front axle, and front end damage. The fire department was attempting to wake the operator who was unresponsive. The fire department ended up administering 2 doses of Narcan to the operator before he became responsive. The operator was transported to Newton Wellesley Hospital by Cataldo for further evaluation. Based on the accident scene, it appears the operator was traveling southbound on Cherry street, crossed over the center line into oncoming traffic, striking the fire hydrant in front of 154 Cherry street, and then crossed back over into the correct lane of traffic before coming to rest in front of 171 Cherry street. The vehicle was towed by Tody's towing, and an inventory form was filled out and filed. The operator was mailed MA Uniform Citations T0648078 and T0648079 charging him with MGL 89/4A Marked Lanes violation, 90/24 OUI Drugs, 90/24(2)(A) Operating to Endanger/Negligent operation, 90/11 No License in Possession, and 90/11 No Registration in Possession. Officer Donahue took pictures of the fire hydrant and crash scene, which have been submitted to the IT Bureau. The fire department was made aware of the broken hydrant.

Narrative:
On the above date/time I responded to Lexington Street / Freeman Street for a two motor vehicle crash with air bag deployment. Upon arrival on scene I checked all parties for injuries. Operator of vehicle one Ms. Farnese complained of head / neck injuries and Fire and Medics responded on scene. All parties in vehicle two did not have any injuries. I spoke with the operator of vehicle one Ms. Farnese in the ambulance she stated she was slowing traveling south on Lexington Street at the intersection of Freeman Street due to the traffic signal for Lexington Street was yellow. She stated as she was slowing she felt a heavy impact from in the rear by vehicle two. I spoke to the operator of two Ms. Heimbrock, she stated she is a pastor for the Salvation Army in New York and she was transporting two children Shamah Lapaix d.o.b. 4-18-14 and Grace Lapaix d.o.b. 1-10-11 to a Christian school in Wayland. She stated that she is staying with the children at 65 Falmouth Road Newton MA with the childrens grandmother due to the fact the parents are in New York doing a continuing education program until Friday January 18, 2019. She stated she is not familiar with the area, she was traveling south on Lexington Street approaching Freeman Street when she glanced at the map on her cell phone. She stated she did not observe the color of the traffic signal and did not observe the rear brake lights in vehicle one. All parties were evaluated in vehilce two by medics with no injury. I spoke with the childrens mother Sarah Lapaix 1-203-685-2809 who confirmed Ms. Heimbrock's statement about the care of the children. I did observe an expired inspection sticker as of 12/18 on vehicle one. Therefore due to operator of vehicle two's statement I checked the rear brake lights. When the brake pedal was pressed the left brake light and top(3rd) brake light was operational but the right rear brake light was not operational. Tody's towing arrived on scene and towed both vehicles top their lot. Ms. Heimbrock made arrangements for transport of herself and the children. Ms. Farnese was transported to Newton Wellesley Hospital for non life threatening injuries. I advised all parties of the process and gave them each others information. I checked the traffic signals in the intersection and they were fully functional. Therefore with all the above mentioned I am issuing citations to both parties Operator of vehicle one citation # T1440538 for Chapter 90 Section 20 EXPIRED INSPECTION STICKER $ 55.00 Operator of vehicle two citation # T1440539 for City Ordinance 19-75 FAILURE TO USE CARE IN STOPPING $ 20.00 Both citations were filled out after investigation and will be mailed to their license address.

Narrative:
At approximately 0110HRs on Sunday February 17, 2019, I was dispatched to the area of Dedham St. and Spiers Rd. (both public ways in the City of Newton), for a report of a motor vehicle vs pole accident with injury, airbag deployment and entrapment. Upon arrival, I exited my cruiser with my medical bag and AED and went to assist the motorist involved in the crash. MV1 (MA Passenger: 255ZA9) was completely off the roadway and on the shoulder of the road (facing Eastbound) with its nose contacting the light pole ( Verizon Pole #:103/80), which was now broken and listing to the side. Attempts to open the doors were unsuccessful and I attempted to communicate with the operator through a partially broken driver's window frame which was bent. The operator was slumped forward with his hand cradling his head and he was unable to speak, but was breathing. Newton Fire arrived and attempted to gain access to the vehicle via several manual tools with no success and instead deployed the Jaws of Life which was successful. Newton Fire also made entry into the rear passenger compartment because we viewed a child's car seat and checked with negative results for any child in the vehicle. Once the operator was cut out of his seatbelt and safely out of MV1 and on board Newton Medic 1, I entered the ambulance where he was identified as Mark Topham. It should be noted that Topham was having difficulty answering basic questions such as his name, date of birth, today's date, Etc. At one point when he was asked what today's date was, he gave his birth date of February 21, 1983 and it took him several attempts to say his last name, I had to run him over the air with a partial name to ascertain who he is. Also of note, in my presence and that of Sgt. Devine, Topham informed the medics that he had been consuming alcoholic beverages prior to the crash. Upon viewing the scene, I noted that the roadway was lit and I did not observe any skid marks on the dry, ice-free payment, indicating that there was unlikely any braking or swerving to avoid the collision. I spoke to the reporting party, Lindsay Giardina, she stated that she was traveling on Dedham St. when she heard what she believed to be a motor vehicle crash. When she came upon said crash, she attempted to assist the operator, as she was an off duty firefighter. She was unable to gain access to the inside of the vehicle, but communicated to the operator in the same manor as I did. She stated that she was able to smell the odor of an alcoholic beverage emanating from the operator. Topham was transported to Beth Israel of Boston, where he was assigned patient record number 3109921. I attempted to speak with him at the hospital, but was unable to do so because of his current state; I noted several injuries on Topham, such as a bloody lip, a wound to his lower left side and a wound to his left leg. Due to Topham's statements onboard of Medic 1 and Lindsay Giardina's observations, I will be charging Topham with one count of violating MGL Ch.90 S. 24 K OUI Liquor 2nd Offence, one count of violating MGL Ch.90 S. 24 E Negligent Operation of a Motor Vehicle and one count of violating MGL Ch. 89 S. 4A Marked Lanes Violation via MA Criminal Citations which I placed in the mail. Additionally, I will be seeking a subpoena for Topham's blood work from the hospital. Note: The crash occurred on the Newton/Boston line, Newton dispatch's GPS indicated that the crash was in Newton. I took several pictures of the scene and have forwarded them to IT to be attached to this report. I completed and filled an NPD Motor Vehicle Tow Inventory Form and Officer Boudrot stood by for the tow.

Narrative:
On Friday March 15th 2019 at approximately 0730 hours I responded to the intersection of Beacon Street and Herrick Road for a report of a hit and run involving a bicyclist. The weather at the time of the accident was cloudy and the road conditions were dry. Both streets are public ways in the City of Newton. Upon arrival I observed the victim identified as Glenn Butcher (MA OLN S36552335), inside the ambulance getting medical attention. Butcher was getting a bandage applied to his left leg and complained of leg, back and neck pain. Butcher appeared visibly shaken and in pain. Butcher stated he had the green traffic light and was travelling eastbound on Beacon Street crossing Centre Street and then continuing straight crossing Herrick Road. Butcher stated MV1 was travelling westbound on Beacon Street and took a left turn onto Herrick Road and collided into his bicycle. Butcher states upon impact he flipped over his bicycle and landed on the hood of MV1, before rolling off and landing on the ground. Butcher states he was wearing his helmet which was present. Butcher states his bicycle ended up partially under MV1. Butcher states the operator of MV1 pulled his vehicle to the side of the road and got out of his vehicle and spoke to several witnesses briefly. Butcher states the operator then left the scene without properly exchanging information and did not wait for the police to arrive. Butcher described the vehicle as a dark gray Honda, and described the operator as an older white male, 5'10 approximately 80 years old with white hair. Butcher was also able to provide an email address for another bicyclist who witnessed the accident. The witness left his information with Butcher prior to my arrival. Fire fighters on scene were able to get the registration number of the vehicle from the witness (MA REG 4CX148) and gave it to Officers on scene. Butcher was transported by Medic 1 to NWH with minor injuries. His bicycle had visible damage to the frame area and skid marks were present on both tires. 19 photographs of the scene and the bicycle were taken and sent to the IT dept. email. Officer Gabriel transported the bicycle to the station where it was logged into Property & Evidence for safekeeping. Dispatch was able to query the registration which came back to a gray, 2010 Honda Accord registered to a Matthew Sugarman (MA OLN S38601397), out of W. Roxbury. Dispatch contacted the Boston Police Dept. and had them send an Officer to the address to try to make contact with Sugarman. Sugarman was advised to contact the Newton Police Dept. and I was able to speak with him on the telephone. Sugarman states he was driving the vehicle today and was involved in an accident with a bicycle. Sugarman states he was travelling westbound on Beacon Street and was taking a left onto Centre Street not Herrick Road. While speaking with Sugarman he seemed confused as to his location during the accident and was adamant that he was turning left onto Centre Street and not Herrick Road. I advised Sugarman that I was on scene with the first responding Officers and the accident clearly occurred at the intersection of Beacon Street and Herrick Road based upon the final resting place of the bicycle. Sugarman went on to state that his vehicle was already turning when the bicycle struck his vehicle and he believed it was the bicyclists fault. I advised Sugarman it appears the bicyclist had the right of way since it was traveling straight and that because his vehicle was taking a left turn, he failed to yield for on coming traffic at an intersection. When asked why he did not stay at the scene of the accident Sugarman stated he got out of his vehicle and spoke to the bicyclist and he "seemed okay". Sugarman states he then left the scene while the victim was still sitting on the ground. I asked Sugarman if he had ever been involved in accident before and he replied "no." I advised Sugarman that I am currently looking at his drivers history and see nine accidents that he was involved in. Sugarman stated "he has never been in accident like this before." I then asked Sugarman what the law states when you are involved in an accident and he was not able to provide me with an answer. I advised him that the law states you must stop and exchange information before leaving the scene of an accident. Again he stated the bicyclist seemed okay and "he had somewhere to be so he left." Sugarman has an extensive drivers history including nine surchargeable accidents, reckless operation, two different charges of leaving the scene property damage and two immediate threat affidavits filed against him. I advised Sugarman that I will be mailing him a citation for Ch89 s8 Fail to yield to oncoming traffic, and Ch90 s24 Leaving the scene of personal injury. I also filed an immediate threat form with the registry. I emailed the witness using the information he provided to the Newton Fire Dept that was on scene and received a phone call back from the witness identified as Bruce Adams. See witness report for his statements. I contacted the Bank of America located at 808 Beacon Street in Newton, as they are the closest business to that intersection with video surveillance. I spoke with George Walsh (781-834-8275) who stated they do not have any working cameras on that side of the building to the bank. For this report the bicyclist is entered as a non-motorist for validation purposes. Witness Bruce Adams (MA OLN S96398108) stated that he was riding his bicycle travelling behind the victim about 100 feet away. Adams states he was watching the victim and had a clear view of him as he travelled behind him, through the green traffic light heading eastbound on Beacon Street. Adams stated he witnessed MV1 travelling westbound on Beacon Street take a sharp left turn onto Herrick Road which turns into Union Street. Adams stated that the vehicle turned left colliding into the bicycle and there was no possible way the bicyclist had time to stop to avoid the collision. Adams stated the victim flew over his handle bars and fell on top of MV1's hood and then landed on the ground. Adams stated the victim's bicycle was stuck under MV1's front bumper. Adams stated the operator of MV1 backed his vehicle up with the bicycle still attached to his vehicle bumper and pulled MV1 over to the side of the road. The operator then got out of his vehicle and walked up to the victim who was laying on the ground visibly in pain. Adams stated the operator asked the victim if he was okay to which the victim did not immediatley reply as he was visibly shaken up. Adams stated the operator then walked back to his vehicle and attempted to leave the scene and that is when Adams stood infront of MV1 to stop him from leaving the scene. Adams stated he then removed the bicycle from underneath the bumper of MV1 and placed it on the side of the road. Adams stated the operator got out of his vehicle again and walked up to the victim and asked him "what do you want to do?". Adams stated the victim did not respond and that is when the operator got back into his vehicle and drove away, leaving the scene before any emergency response arrived. Adams stated he was able to memorize MV1's registration and stayed with the victim until police, fire and medics arrived. He stated he gave the fire dept the registration and his contact information prior to leaving the scene.

On 4/14/2019 at approx 0923Hrs while assigned to 497 I responded to the area of Commonwealth Av and Ash St for a report of a three car crash. upon arrival I observed three cars off to the side of the road. i spoke with the operator of ma Reg T6T, Robert MURPHY who stated he had stopped for the red light at Commonwealth and Ash St when the operator of TN Reg 2N37P1, Jennifer BURNELL failed to stop striking the operator of Ma Reg 8CL834 Evan GUIDRY who in turn than ran into the back of MURPHY. I spoke with BURNELL who stated she was lost and had been looking down onto her phone looking for a map when she ran into the back of GUIDRY. BURNELL cited for violation MGL90/13 distracted Operation , GUIDRY transported to NWH by Cataldo Ambulance for neck and head pain. GUIDRY and BURN ELLS vehicles towed by Todys. NFD responded and cleared roadway or debris and fluids.

Narrative:
On Sunday, May 5th, 2019 at approximately 1315 hours while working the City of Newton detail for the Walk for Hunger I was standing at my assigned post at the intersection of Centre Street and Willow Street. At the time, the road surface was dry and the weather was cloudy. Traffic was particulary heavy as there were several streets in the area still blocked off for the Walk for Hunger and there were participants still walking the route in the area. My attention was drawn to the intersection of Centre Street and Bowen Street as there was a motor vehicle travelling southbound on Centre Street with their left turn signal on attempting to take a left turn on to Gibbs Street. Gibbs Street was temporarily blocked to non-residential motorists as there were several pedestrians in the road crossing over Gibbs Street. As the motor vehicle attempted to turn, there were several pedestrians in the crosswalk crossing over Gibbs Street. The vehicle had stopped halfway into the northbound lane on Centre Street blocking on coming traffic. The operator appeared to realize Gibbs Street was blocked and attemped to turn the vehicle back into the southbound lane to continue southbound on Centre Street. As the vehicle turned back into the southbound traffic lane, the vehicles rear end collided with Officer Richard Clements, who was operating NPD motorcycle unit 533. Officer Clements was traveling southbound on Centre Street and was passing the vehicles in the southbound traffic lane which were currently all at a standstill. Officer Clements had his emergency lights activated as he was on his way to remove traffic cones from the street to open up all the traffic lanes along the event route. Officer Clements was working the City of Newton detail for the Walk for Hunger assigned to the motorcycle to assist with traffic. Officer Clements front wheel, left side of the motorcycle and left leg collided with the rear end of the motor vehicle. The impact of the crash caused him to fall off the left side of the motorcycle and land on the street face first. At this time I immediately ran from my post to Officer Clements location and radioed dispatch that a motorcycle officer was down and that I needed assistance. As I approached Officer Clements he was face down, unconcious and breathing. I was able to roll Officer Clements into the recovery position and at this time Officer Clements regained conciousness and was able to speak and stated " What happened? What was that lady doing she turned right into me". I advised Officer Clements that he had just been involved in an accident and to stay still and that help was on the way. The operator of the motor vehicle identified as Helen Leibner (S52717988) stopped her vehicle immediatley after the crash and got out of her vehicle to offer her assistance. I advised Helen and several other witnesses and bystanders who were now crowding around Officer Clements and I to back away from the scene and to not move any of the vehicles involved. Officer Selig arrived on scene and was able to help crowd control until further help arrived. Helen and all witnesses were advised to stand by for police to take their statements. While tending to Officer Clements he was disoriented and made several attempts to move his body to which I had to physically hold him still to prevent possible further injuries. While speaking with Officer Clements I noticed blood coming from the inside of his mouth. Officer Clements was in a noticeable amount of pain. He complained of head, back and left leg pain. Officer Clements was wearing a yellow ANSI certified Newton Police reflective jacket and a Newton Police issued motorcycle helmet. Once the paramedics and fire personnel arrived I was able to speak to the operator of the motor vehicle and several other witnesses. Helen stated she was traveling southbound on Centre Street operating her blue 2014 Audi A4 (MA REG 213BC9). Helen states she was attempting to take a left turn onto Gibbs Street and activated her left turn signal. Helen states she had stopped halfway into the northbound lane as several pedestrians were crossing Gibbs Street in the crosswalk at the time. Helen states after the pedestrians cleared she saw the traffic cones blocking the street and realized the street was closed. Helen states she turned her vehicle back into the southbound traffic lane and did not see Officer Clements or the motorcycle until impact. Helen was not injured and her motor vehicle sustained moderate rear end damage. Her motor vehicle was able to drive away from the scene. Witness Adron Gordon (S90357161) stated he was participating in the Walk for Hunger and had just crossed over Gibbs Street when the crash occured. He stated he witnessed the Officer on the motorcycle with the emergency lights on traveling southbound on Centre Street behind the motor vehicle. The motor vehicle stopped in traffic attempting to turn left onto Gibbs Street. He states then the vehicle made a slight turn back into the southbound traffic lane and the motorcycle collided with the rear end of the motor vehicle. He states the Officer then fell off the motorcycle landing in the middle of Centre Street. Witness Elizabeth McCarthy (S00242346) and Katherine Regan (S43486601) stated they both were in the same vehicle together stopped in traffic in the northbound lane of Centre Street just before the set of lights at Centre Street and Bowen Street. Both witnesses stated they do not remember if the driver of the motor vehicle had the turn signal on but they did see her attempt to turn onto Gibbs Street. They stated they also saw the motorcycle with emergency lights activated travel southbound on Centre Street approaching behind the vehicle. Witnesses state the motor vehicle then suddenly turned back into the southbound lane and the motorcycle collided with the passenger side tailight of the motor vehicle. Both witnesses state the motorcycle attemped to turn away from the vehicle to avoid the crash but was unable to do so. Witness Nina Wang (S39460985) stated she was parked on the southbound lane of Centre Street just prior to the crash scene. Nina states she was in her City of Newton DPW marked vehicle with the hazards on as she was working to assist with traffic safety for the Walk for Hunger. Nina states she witnessed the motorcycle travel southbound on Centre Street and pass the vehicles stopped in the traffic lane. Nina states the motorcycle passed the traffic using the left side and had the emergency lights activated. Nina states the motorcycle was just passing through the green light at Centre Street and Bowen Street when the motor vehicle that was stopped at the lights attempting to take a left turn onto Gibbs Street. Nina states the vehicle then abruptly jerked her car to the right and collided with the motorcycles left side. Nina states she heard a loud crash and witnessed the Officer fall off the motorcycle and land face first onto the street. The motorcycle's final resting place was on its left side approximately two feet away from where Officer Clements was located. The motorcycle sustained moderate damage to its left side, including the front wheel well and undercarriage. The motorcycle left scuff marks on the street which were measured and photographed. Several photographs of the scene and damage to both vehicles were taken and submitted to the IT Bureau. The motorcycle was towed by Todys. Centre Street and Bowen Street are both public ways maintained by the City of Newton. The injured motorcyclist, Officer Clements, was transported to Saint Elizabeth Hospital with fire fighters on board. I was able to follow up with Officer Clements on the phone the next day who was admitted over night at the hospital. Officer Clements stated he has no recollection of the accident and can only remember driving down Centre Street to begin to pick up cones to open up all lanes of traffic. Officer Clements stated he suffered a brain bleed, head, back, left foot and right thumb injuries. A Massachusetts Uniform Citation was issued and mailed to Helen for Chapter 89 Section 7A, failure to yield to emergency vehicle, police motorcycle.

On 05/15/2019, while assigned to N494, I, Officer Conary, responded to a MVA at 109 Adam Street, a public way in the City of Newton. Upon arrival, I observed a utility pole broken in half, wires hanging in the street, and also a transformer on the ground. Fire and medics responded. Other police units were able to block the street from traffic. I spoke to witness, HARRIS, who stated that she was traveling Southbound on Adam Street when she saw MV1 in the wrong lane traveling toward her at a high rate of speed. HARRIS stated that MV1 then turned into the utility pole located outside of 109. MV1 took out the pole, side swiped MV2 and then hit MV3 head on. Operator of MV1 stated that he was traveling Northbound on Adam Street when he sneezed. At this point, he said his eyes were off the road and the next thing he could remember was being in the parking lot of 109 Adam Street after the accident. There was significant damage to the front of MV1 with airbag deployment. Operator of MV1 was checked out by the medics for a head injury and signed a refusal. Operator of MV1 was asked if he had consumed any alcohol or taken any medication today and he declined. Officer Spinney (N491) asked if he was willing to do a Field Sobriety Test, and he stated yes. Operator of MV1 did not show any signs of intoxication. Operator of MV2 stated that he had parked his car outside of 109 Adam Street and was sitting in the driver seat when he was hit by MV1 and also the utility pole. Operator of MV2 was able to exit his car. He was evaluated by the medics and signed a refusal. MV2 has significant damage to the right side and windshield. MV3 was parked in the lot of 109 Adam Street facing Southbound. MV1 hit MV3 head on. There was significant damage to MV3 front end. The electric meter attached to 109 Adam Street was ripped off the house. There are no signs of damage to the house to report at this time. MV1 was towed by Tody's. Eversource arrived on scene. Operator of MV1 was issued a citation for marked lanes violation. Pictures were taken of the scene and will be submitted to IT.

On 5/20/19 at about 1510hrs I was dispatched to 136 Evelyn Rd for an MVA. En route to the location with Ofc Gourdeau, it was determined by first arriving Ofc Gourdeau that a vehicle had crashed through a fence at the end of Troy Lane and came to rest in the backyard of 126 Evelyn Rd. On arrival, I located the vehicle operator, identified as Mr Stuart Phillips who was operating MA Reg 8VVP40, an '03 Jaguar S Type model. Mr Phillips was walking around the backyard after the collision and appeared coherent as to what had transpired. He sustained a laceration to the top of his head and was bleeding heavily. We sat him down in a lawn chair prior to medics arrival to evaluate. When asked what happened, he stated he "hit the gas instead of the brakes". I asked him where did the accident begin? He stated he was turning around at the end of Troy Lane (a dead end cul de sac) and crashed through the fence there. Mr Phillips was alert and speaking clearly. I asked him if he was on any medication? He stated he was taking Plavix only, a blood thinner. I asked about his medical history and he stated he was released from Spalding Rehab today for treatment from a stroke and was receiving occupational therapy. Medics arrived to evaluate and he was transported to NWH for treatment. An investigation of the crash path of the vehicle revealed the following based on tire tracks and damage left behind: The vehicle crashed through the fence at the end of the cul de sac belonging to 41A Helene Rd; Vehicle drove over that back lawn, clipped the back deck attached to the rear of the house; damaging the base of the deck and lattice below; Vehicle travelled over the patio, then crashed through shrubbery down an embankment leading to the backyard of 132 Evelyn Rd; Vehicle drove through backyard of 132 Evelyn, then continued along the side of the house where it drove over damaging three retaining walls and clipped a large tree; After driving over the third retaining wall, vehicle continued through the backyard of 126 Evelyn, causing significant lawn damage; Vehicle continued until it hit the back of the house at 126 Evelyn; there, it drove over the rear patio before it struck a ground level basement window and window well; It came to a rest there where the vehicle caught on fire under the hood. Witnesses helped extinguish the fire with a garden house (see incident report #19020129) until NFD arrived to knock it down. Significant front lawn damage to 126 Evelyn occurred as well as NFD was arriving on scene to what was first reported as a structure fire. Several photos of the area were taken by myself. The memory chip was TOT the I.T. bureau to be attached to this report. Tody's towing towed the vehicle out of the backyard by accessing the rear driveway of 132 Evelyn Rd. Mr Phillips cited for Negligent Operation and an Immediate Threat for License Suspension/Revocation faxed to the RMV. Troy Lane is a public way in the City of Newton.

On 06/08/2019 at 0150 hours I received a call from Sgt Lee in regard to a head on motor vehicle crash with entrapment that had occurred on Center St at the four way intersection with Cabot St and Sergeant St. Upon my arrival at the crash scene I was advised by Sgt Lee that all occupants of the involved vehicles had been transported to hospitals for treatment. I made the following observations of the crash scene , Center St is a two lane public way in the city of Newton that runs north to south. I observed a city of Newton contracted Cataldo ambulance stopped in the south bound lane of Center St at Cabot St. The ambulance was MA AMN registration AMB 370 a 2017 Ford Econo Line that had heavy damage to the front end and driver's side fender. I observed that the front tire of the ambulance was snapped off and was pushed back towards the driver's side door. I further observed that both of the front and side air bags of the ambulance were deployed. The front of the ambulance's final point of rest was 8'8" from a fixed overhead light pole on the northeast corner of Center St at Cabot St. The ambulance's front passenger side tire was 4'3" front the curb on the Cabot St side. The ambulance's rear passenger side tire was 2'3" from the Cabot St side curb. The ambulance's rear driver side tire was 5'1" from the double yellow line on Center St. The ambulance's front driver side tire was 6'3"from the Center St double yellow line. The width of Center St from the double yellow line to the Center St curb is 15'' and 16'" to the Sergeant St side. I observed a 2011 Mazda 3 with MA PAN registration 6KJ783 facing south bound in the north bound lane. The Mazda had heavy damage to it's front end, and I observed that it's front tires had snapped off. The Mazda's roof had been removed by Newton Fire to extract the occupant. The front of the Mazda's final point of rest was 40'3" from a fixed overhead light pole on the northeast corner of Center St and Sergeant St. The Mazda's front passenger tire was 4'6" from the double yellow line on Center St. The Mazda's rear passenger side tire was 7'6" from the Center St double yellow line. The Mazda's rear passenger side tire was 1'2" from the Center St double yellow line. The Mazda's front driver's side tire was on the curb on the Sergeant St side of Center St. There were no skid marks from the Mazda which indicates that no braking occurred. At the time of the crash, the Cataldo Ambulance was being operated by Paramedic Corey Bohan and Paramedic Tom White was in the front passenger seat. Bohan stated that he was operating his vehicle southbound on Center St towards Cabot St after clearing a medical call. There were no patients in the ambulance at the time of the crash, and Bohan estimated he was travelling 25 MPH . Bohan stated within less than two seconds he observed a vehicle traveling towards him northbound on Center St in the southbound lane and estimated it's speed to be approximately 80 to 100 MPH.Bohan stated he was able to slow to a near stop and had moved his vehicle towards the right shoulder on Center St before the Mazda struck his vehicle head on. Bohan stated that after forcing his way out of the driver's seat of his vehicle, he rendered medical aid to the operator and only occupant of the Mazda, Jessica Pondish. Bohan stated that Pondish speech was slurred and he detected an odor of an alcoholic beverage emanating from her breath and person. Bohan was later transported to Newton Wellesley Hospital for evaluation. The passenger in the ambulance, Tom White stated to officers that he was working on the ambulance's laptop computer and did not see anything leading up to the impact from the crash. White stated that he was able to force himself free of the passenger side of the ambulance, and rendered medical aid to Pondish. White stated that Pondish was " in and out of it " and did not appear coherent.White stated that he asked Pondish if she had consumed any alcoholic beverage and she nodded in the affirmative. White Stated that he was unsure if Pondish understood his questions. White stated that he did not detect any odor of an alcohol beverage emanating from Pondish's breath or person. White was transported to Newton Wellesley Hospital for evaluation. Pondish was freed from her vehicle after approximately 22 minutes by Newton Fire Department units by utilizing special extraction tools. Pondish was transported by Newton Medic Unit Two to the Brigham and Women's Hospital in Boston for treatment. From 06/08/2019 to 06/10/2019 Pondish had two surgeries and was unable to speak due to her condition. On 06/10/2019 myself and Officer Gaudet were able to speak with Pondish in her hospital ICU room at the Brigham and Women's Hospital. Pondish's parents Lorena and John Pondish were present while we spoke with her. Officer Gaudet asked Pondish about the events that led up to the crash. Pondish stated that on 06/07/2019 she was at a friend's residence in Norton, MA. Pondish stated that while at her friend's residence she consumed a bottle and a half of Riesling wine. After Pondish's admission myself and Officer Gaudet stopped our conversation with her. I read Pondish her rights directly from a Miranda card and explained them in full before allowing her to continue speaking with myself and Officer Gaudet about the events of that led to the crash. Pondish stated that she understood her rights. Officer Gaudet asked Pondish if she had consumed any alcoholic beverages prior to driving home to her residence in Newton. Pondish stated that she drank a bottle and a half of Riesling wine by herself between the hours of 1700 and 0000 hours on 06/08/2019. Officer Gaudet asked Pondish how large the bottles of wine was that she drank, and she stated that they were average size. Officer Gaudet asked Pondish how she perceived her state of being was after consuming a bottle and a half of wine. Pondish stated that she felt " buzzed " . Pondish stated that she left her friend's residence in Norton at approximately 0100 hours on 06/08/2019 to return to her residence in Newton. Pondish stated that she does recall the events leading up to the crash, except that she tried to make a phone call but the person did not pick up. Pondish stated that her seat belt was on and that she was not texting while operating her vehicle. Pondish stated that her injuries consisted of a broken left elbow and internal injures to her intestines. On 06/09/2019 I spoke to Cataldo Paramedic Robert Canning and Stephen Heirholcer who were the medic unit that transported Pondish to the Brigham and Women's Hospital. Canning stated that he rendered medical aid to Pondish for approximately 20 minutes while Fire units worked to free her. Canning stated that Pondish's legs were pinned under her vehicle's dashboard. Canning stated that during the approximately 20 minutes that he was in close proximity to Pondish he could detect a strong odor of an alcoholic beverage that was emanating from her breath and person. Canning stated that once Pondish was freed he continued to treat and monitor her during the transport to the Brigham and Women's Hospital. Canning stated that when he asked Pondish if she had consumed any alcoholic beverage she stated that she had. Paramedic Heirholcer stated that he did not detect any odor of an alcoholic beverage emanating from Pondish's breath or person. Based upon the physical evidence at the crash scene, and statements made to officers by all of the above mentioned persons, Pondish is being charged with violation of MGLs 90/24j OUI liquor, 90/24e negligent operation of a motor vehicle, 89/4a failure to stay within marks lanes, and 90/17 speeding. On 06/12/2019 at the conclusion of the crash investigation I issued MA Criminal Application T 1269996 in hand to Pondish at her hospital room and explained to her what action she needed to take with court. On 06/11/2019 a letter of preservation was sent to Brigham and Women's Hospital requesting that a sample of Pondish's blood, urine and medical records be saved. On 06/12/2019 Brigham and Women's Hospital confirmed that a sample of Pondish's blood had been preserved. As of 06/12/2019 a search warrant for Pondish's blood sample was pending. Photos of the involved vehicles and of the crash scene were taken and downloaded by the NPD's IT Bureau. Both involved vehicles were towed from the crash scene and towed motor vehicle forms were filled out. An Iphone and backpack belonging to Pondish were taken from her vehicle for safekeeping and returned to her on 06/11/2019. A copy of this report was faxed to the MA RMV Driver Control Unit to request an immediate threat suspension.

On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

MV#2 was traveling south on Waverly Avenue when it was struck from Franklin Street by MV#1. -Operator of MV#1 stated she was traveling east on Franklin Street and that she stopped at the intersection. She stated as she was about to enter Waverly Avenue from Franklin Street and after checking to ensure she was clear to enter, she collided into MV#2. She stated he was flying up the road. -Operator of MV#2 stated he was traveling south on Waverly Ave when he was struck by MV#1. Operator of MV#2 stated the collision spun his vehicle 180 degrees. He stated he was facing the direction he was just comming from. He stated he didn't think she (Operator of MV#1) stopped. - I observed the front of MV#1 sustained heavy damage. The hood was bent in, the bumper was cracked and hanging, and fluids were leaking from the vehicle's engine. I next observed the damage to the doors on the right side of MV#2. I observed the base of the doors were pressed in, those doors would not open and the tires were grinding against the flared edges of the wheelwells. - There were no injuries reported at the scene, however, both operators were checked by Ambulance 1. Newton Fire responded to scene to contain the Hazmat spill from MV#1. MV#1 was towed from the scene. OpMV#1 was cited (T2015541) for Failure to come to a Complete stop at stop sign. I transported Operator of MV#1 to Tody's towing from the scene.

On Thursday, August 8, 2019, while assigned to Traffic unit N525, I responded to the area of Centre Street and Mt. Ida Street for a report of a 3 car motor vehicle crash with entrapment. The weather at the time of the crash was clear and sunny. The road surface was dry. Centre Street and Mt. Ida Street are both public ways maintained by the City of Newton. On arrival, I was updated by Sgt. Chisholm that one of the vehicles (MV3) had a female operator that was entrapped as a result of the collision. She was freed from the vehicle by the Newton Fire Department and transported to Beth Israel Hospital. Another operator (MV2) was transported to Newton Wellesley Hospital with minor injuries. A third operator (MV1) remained on scene with no injuries. With the assistance of Officer Wade we conducted a survey of the crash scene. We observed a large debris field at the intersection of Centre Street and Mt. Ida Street with debris from all three vehicles. The point of impact was observed to be on Centre Street (N) before Mt. Ida Street. There were fresh scrapes in the pavement as a result of the colission. We also observed skid marks from MV1 measuring 57 feet 8 inches from the start of it's skid to the vehicles final rest MV1 crashed into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane of Centre Street and crash into MV3 as it was passing by. The impact from MV2's front end crashing into MV3's driver side resulted in the operator of MV3 becoming entrapped. MV2 and MV3 came to final rest on Wesley Street (W) at Centre Street (S). The driver door of MV3 was inaccessible. The collsion resulted in air bag deployment for MV2 and MV3. I observed moderate damage to MV1's front grill/bumper area. I observed substantial damage to the front and rear of MV2 and substantial damage to the front and driver side of MV3. Tody's responded and removed MV2 and MV3 from the roadway. Photos were taken of the scene and submitted to the IT Bureau. Officer Martin of the Watertown Police Department was requested by Sgt. Lee to respond to the scene and assist with a commercial truck inspection of MV1. Officer Martin contacted Trooper Grey of the Massachusetts State Police Truck Team who also responded to assist with the truck inspection. Sgt. Lee remained with Officer Martin and Trooper Grey during the inspection. MV1 was relocated to Richardson Street for the inspection. Trooper Grey determined the truck was in compliance with all required paperwork and it passed it's mechanical inspection. MV1 was removed from the roadway by a state tow. I spoke with the operator of MV1, William Tisbert (S24792655). Tisbert stated he was operating a 2006 KW W900 Semi Truck (NH SEMI: AP2638) Northbound on Centre Street. Tisbert stated as he was approaching Mt. Ida Street, a vehicle came out of no where and he hit it. Tisbert stated there was no one in front of him and then a vehicle came off of a side street (Mt. Ida Street) and attempted to turn left in front of him to return to the side street. Tisbert stated he could not avoid hitting the vehicle. Tisbert crashed into the rear of MV2. MV2 then crossed into the opposite lane and crashed into another vehicle. Tisbert remained on scene and reported no injuries. Tisbert stated he was wearing his seatbelt at the time of the accident and was not distracted. Tisbert later admitted to crashing into the rear of MV2. I spoke with the operator of MV2, Janet Vanbuskirk (S59829146), at Newton Wellesley Hospital. Vanbuskirk stated while operating her 2004 Toyota Highlander (MA: RS 529Y), she was stopped on Centre Street (N) at Mt. Ida Street. Vanbuskirk had her left directional on and was waiting for traffic in the Southbound lane to clear to take a left turn onto Mt. Ida Street. Vanbuskirk stated MV1 crashed into the rear of her vehicle. As a result of the impact, Vanbuskirk's vehicle traveled into the opposite lane and crashed into the driver side of MV3. Vanbuskirk reported minor injuries to her neck, left arm, and right hand. I spoke with the operator of MV3, Madeleine Karolian (S23979811), at Beth Israel Deaconess Medical Center in Boston. Karolian stated she was operating her 2016 Toyota Prius (MA: EV5746) Southbound on Centre Street. Karolian stated MV1 crashed into MV2, and MV2 crashed into her vehicle. Karolian stated she did not have time to avoid the collision. Karolian stated she had minor injuries to her neck and chest. Karolian's vehicle had a dash cam recording at the time of the accident. I viewed the footage with her through an app on a cell phone. The footage shows MV3 traveling Southbound on Centre Street on August 8, 2019 at 1308 hours. Ahead of MV3 in the Northbound lane, MV2 is seen stopped on Centre Street before Mt. Ida Street with it's left directional activated. I observed MV1 travel Northbound on Centre Street and crash into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane and crashed into the driver side of MV3. The footage then ends. A copy of the footage was emailed to my work email. The footage was then forwarded to IT Director Steve Smith to be added to the media for this report. As a result of my investigation, the operator of MV1, William Tisbert, will be cited for Newton City Ordinance Chapter 19, Section 71 (Following too closely). Massachusetts Uniform Citation T1270942 was completed and mailed to Tisbert's residence.

AT THIS POINT DRIVER STATES SHE ENTERED THE TRAFFIC LANE AND TURNED ONTO CRAFTS ST. SHE STATES HER VEHICLE WAS HIT IN THE LEFT SIDE BY VEHICLE #1. SHE STATES SHE NEVER SAW THE MOTORCYCLE UNTIL IT WAS HITTING HER VEHICLE. OPERATOR OF VEHICLE #1 WAS BEING ATTENDED TO BY THE FIRE AND MEDICS. OPERATOR OF VEHICLE #2 WAS GIVEN MOTOR VEHICLE CITATION ( T1445177, 89/9, FAILURE TO YIELD AT INTERSECTION ). OPERATOR SERVED CITATION IN HAND. I SPOKE TO A WITNESS ( TIMOTHY CHANGOGARRETT ). HE STATED HE WAS TRAVELING E-BOUND BEHIND THE MOTORCYCLE. HE SAW VEHICLE #2 ENTER THE TRAFFIC LANE AND VEHICLE #1 HIT HER VEHICLE. HE STATES THE OPERATOR OF VEHICLE #1 WENT OVER HIS HANDLE BARS UPON COLLISION. LANDED ON THE HOOD OF VEHICLE #2 AND THEN FELL TO THE GROUND. AT THIS POINT HE CALLED 911. OPERATOR OF VEHICLE #1 WAS TRANSPORTED TO BRIGHAM AND WOMEN'S HOSPITAL BY MEDIC2 FOR FURTHER TREATMENT. I WENT TO THE HOSPITAL AND SPOKE TO THE OPERATOR IN HIS ROOM. HE STATES HE WAS TRAVELING E-BOUND ON CRAFT INTENDING TO GO TO THE POST 440 ON CALIFORNIA ST. TO ATTEND AN AA ( ALCOHOLICS ANONYMOUS ) MEETING. HE STATES HE TOOK HIS SON'S MOTORCYCLE WITH THE INTENTION OF SEEING IF THERE WERE ANY REPAIRS NECESSARY BECAUSE HIS SON IS TRYING TO SELL IT. WHILE TRAVELING HE APPROACHED THE INTERSECTION. HE SAW VEHICLE #2 ENTER ONTO CRAFT ST. HE STATES HE COULD NOT DIVERT AND HE WAS UNABLE TO AVOID HITTING HER VEHICLE. OPERATOR HAS BROKEN BONES IN HIS LEFT ARM. CUTS ALONG HIS FACE AND NECK AS WELL AS LOWER BODY PAIN. HE STATED THE HOSPITAL INTENDS ON KEEPING HIM OVER NIGHT FOR OBSERVATION. I ASKED THE DRIVER IF HE WAS AWARE THAT HIS LICENSE TO OPERATE A MOTOR VEHICLE WAS REVOKED. HE STATED HE HAD A CHANCE TO BE REINSTATED BUT CHOSE NOT TO. DRIVER WAS ISSUED MOTOR VEHICLE CITATION ( T1445178, 90/23/C, LICENSE REVOKED AS HTO ). I INFORMED THE OPERATOR THAT I WOULD MAIL HIM THE CRIMINAL APPLICATION. ALL OTHER PARTIES REPORTED NO INJURIES. MV1 (MA Reg: 1LMD95; Honda Accord 2019) was traveling west on California St when it came to rest on the shoulder of the road. MV2, (MA Reg: 5YR125; Honda CRV 2019) helped MV1 come to a stop because it was parked. Minor damage to MV1 front driver bumper and MV2 scratch on the rear bumper. MV2 owner, Lourdes Meira, exited her residence at 313 California St to witness MV1 pressed against her vehicle. Meira called 911 because she observed the driver of MV1 unconscious or asleep. Fire arrived on scene and gave two doses of Narcan to MV1 operator, Keith Harrington. Harrington gained consciousness and medics transported to NWH. Harrington admitted to injecting heroin today prior to this incident. Harrington to be summons for 90/24G-OUI Drugs, 2nd offense and 90/24E-Negligent Operation (MA Uniform Citation:T2079021). Citation given in hand at NWH by Ofc. Guarinio. Tody's towed MV1, towed motor vehicle form filled out. Used syringe from MV1 center console safely disposed of in N492. Small silver container with white substance confiscated from center console beside used needle. Container was filed into property and evidence to be tested. An immediate threat was filed and faxed to RMV.

The operator of MV1 states that she was traveling northbound on Needham Street with flow of traffic when she struck a pedestrian with the right front corner of her vehicle. The operator of MV1 stated that she did not see the pedestrian in the cross walk until after she hit her and that there was poor lighting in the area. Through the help of coworker from Anthony's Coal Fired Pizza, the victim stated that she was crossing Needham Street in the crosswalk and was almost on the side walk when she was struck by MV1 from behind. MV1 sustained minor front end damage while the victim sustained noticeable injuries and was transported to Newton Wellesley Hospital by Cataldo. Photographs of the scene were taken and submitted to IT to be attached to the report. The operator of MV1 was issued a Massachusetts Uniform Citation (T2079284) for 89-11, failure to yield to a pedestrian in a crosswalk.

Based on the witness statement of Yehor Shumerko, the operator of MV1 rolled down Eliot Street subsequently backing her vehicle into the foundation of 330 Eliot Street. Both tenants of the home then stated that they exited the home and when attempting to speak with operator she appeared distraught and begged them not to call the police before then leaving the scene without providing any information. Both occupants of the home were uninjured. Fire responded and determined that the home was safe for the time being. Wellesley PD attempted to locate the owner of the vehicle at her residence with negative results. Photographs of the scene were taken and submitted to IT to be attached to this report. Massachusetts Uniform Citation T2079291 was issued via mail to the operator of MV1 for 90-24, Leaving the scene of property damage. Approximately 1 hour later Officer Guarino located the operator of MV1 several streets away on Columbia Ave. She stated that she was smoked a cigarette when he vehicle began to roll backwards and struck the house. The operator of MV1 also stated that she did not believe there was any damage to the home which is why she left despite major damage to her vehicle. Todys responded to the scene and towed the vehicle.

Operator of Vehicle #1 was traveling on Saw Mill Brook Parkway before turning right over a curb and onto Kappius Path. Kappius Path is a walking path not intended for vehicle traffic. While traveling on Kappius Path the operator struck a tree with the driver's side front of the vehicle. The vehicle then rolled onto the passenger side. In order to extract both occupants of the vehicle, Newton Firefighters used the Jaws of Life to remove the roof of the vehicle. Firefighters also had to remove several sections of a chain link fence belonging to the property owner of 19 Kappius Path. Newton Medics obtained patient refusals from both occupants. No injuries were reported. The vehicle was towed from the scene by Todys and an inventory sheet was completed by Officer Emmanuel. Five photographs of the scene were submitted to the IT Bureau. The operator of Vehicle #1 was subsequently arrested and charged with OUI, Negligent Operation and Marked Lanes.

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

On 3/16/20 I was dispatched to 127 Commonwealth Ave for a report of a MV(#1) crash into a fire hydrant. The operator left the scene and was shortly after identified as Mr Bai Lin Wu. Wu called NPD to report the crash once he arrived home. Ofc Ferguson took photos of the knocked over hydrant. City Water Dept was notified of the damage. I responded to Wu's residence. Wu stated he was travelling on the Commonwealth Ave W/B carriage road. Wu stated an unknown MV travelling behind him was trying to pass him on the left. Wu stated he turned to the right to allow the unknown MV to pass. Wu ran off the road to the right and struck the hydrant in front of #127 Comm Ave. Wu further stated he left because he was trying to get medication home as soon as possible to his ill wife. Wu's vehicle sustained moderate damage to the passenger side front end. Wu issued citation for CO Ch19/Sec75 failure to use care turning (went off road).

On 03/18/20 at approximately 13:01 I responded to the intersection of Winchester St and Heatherland Rd. for a two vehicle crash. Upon arrival I spoke with Newton Police Detail Officer Schaefer who was involved in the accident. Officer Schaefer was operating his own vehicle (Mass Reg#1JP123 Blue 2001 Cadillac Deville) and headed back to a paid Feeney Brothers detail. Officer Schaefer stated he was traveling due South down Winchester St. approximately 30mph when a light colored vehicle (later determined to be Mass Reg#9FG942- Grey 2010 Toyota Prius operated by Rosana Cristina Brasileiro) pulled out of the one-way Heatherland Rd, the wrong way. Officer Schaefer stated the Prius struck him on his drivers side front and rear doors causing the vehicle to crash into the curb and causing his side air bag to deploy. Officer Schaefer was wearing his seatbelt but struck his head off the interior of his vehicle. I attempted to speak with the operator of the other vehicle Rosana Brasileiro but she only spoke Portuguese. Newton Fire Department Engine #7 was also on scene and Fire Fighter Ian Moniz stated he spoke a different dialect but was able to communicate with her. Fire Fighter Moniz translated for me and I was able to ascertain that Brasileiro stated she was driving due West on Heatherland Rd(wrong way) when she didn't see the other vehicle and made contact with it. I asked Brasileiro for a License and she handed me a Brazilian Passport and stated she didn't have a license. She stated she was just visiting from Brazil and that she was operating her bosses vehicle. Brasileiro was handed Mass. Criminal Citation #T2080843 for Ch90/ Sec10- Unlicensed Operation, Ch90/ Sec 16-Operating on a one way the wrong way and Ch 89/ Sec 8-Failure to yield to the right of way. Both vehicles were towed from the scene while Officer Schaefer went with the Medics to Newton-Wellesley Hospital to get checked out. Sgt. McLean and Traffic Officer Wilson (N523) were also on scene and Officer Wilson took pictures. Sgt. McLean traveled to N.W.H. to check on Officer Schaefer. All roads mentioned/traveled on in this report are public ways in the City of Newton as they are serviced and maintained by this city.

On May 17th, 2020 at approximately 14:26 hours while working N491 I responded along with NFD and Medic One to a mv crash at the intersection of Watertown St @ CRAFT St. On my arrival both the Fire Dept. and Medics were assessing the operators of the two involved vehicles. Operator from vehicle #1 a 2014 black Jeep identified as Mark Mccullough was transported from the scene by ambulance to Newton Wellesley due to his injuries. He did give a statement prior to being transported.. He stated he was going N/B on Craft St when vehicle#2 suddenly made a left turn in the intersection towards Watertown St causing the crash. Operator of vehicle #2 reported going S/B on Craft St and was attempting to turn left onto Watertown St when vehicle #1 crashed into him in the intersection. Witness id as Eamen Keane stated he was behind vehicle #1 on Craft St when he observed vehicle #2 turn left in front of vehicle #1 causing the crash. I cited operator #2 for failing to yield at an intersection C89/S8.additionally charged him with operating his mv without a valid license C90/S10. He did possess a Guatemalan license. His vehicle was registered to him with an address of #59 Chester Ave Waltham Ma. e . He was additionally charged with operating his vehicle on a public way without being properly licensed.C90s10.Citation #T2014121. Both vehicles were towed by todys. Both drivers sustained injuries but only operator #1 went to the hospital. Operator #2 refused. All above mentioned streets are public ways and maintained by the city of Newton DPW.

Both Parker St and Hagen Rd are public ways in the City of Newton. Vehicle 1 was then rear ended by Vehicle 2 described as a 2018 Ford Van color white bearing MA Con Reg S35168 which was traveling southbound on Parker St. As a result of the impact Vehicle 1 sustained damage to the passenger side rear bumper and quarter panel. Vehicle 2 sustained damage to the drivers side front bumper and fender. The operator of Vehicle 1 identified as Chelsea Halsor stated that she was stopped on Parker St due to another uninvolved vehicle which was stopped in front of her vehicle due to a pedestrian crossing Parker St in a crosswalk. Halsor stated that while stopped she was struck from behind by Vehicle 2. Halsor stated that the force of the impact caused her to hit her head on the steering wheel. Fire and Medics responded to the scene and evaluated Halsor. Halsor was released with a patient refusal. The operator of Vehicle 2 identified as Christopher Benoit stated that he was traveling southbound on Parker St and struck Vehicle 1 which was stopped in traffic. Benoit stated that he tried to avoid the collision by swerving to the right but was unable to and as a result struck the passenger side rear of Vehicle 1. Benoit stated he was uninjured. No other witnesses on scene. Vehicle 1 was driven from the scene by Halsor. An RMV check of Vehicle 2 revealed that the registration was expired as of 12/31/2019. Tody's responded and removed the vehicle. A Towed MV Inventory Sheet was completed. An RMV check also revealed that Benoits MA Drivers License (MA DL# S9550481) was suspended. As a result Benoit was issued in hand MA Uniform Citation T1268483 and cited for Mgl 90/23 Operation of a Motor Vehicle After License Suspension, Mgl 90/9 Operating an Unregistered Motor Vehicle on a Public Way and City Ordinance Violation 19-71 Following to Close.

On my arrival with NFD and Medics I observed one vehicle Maryland license plate 9DF562, 2016 Ford Escape in the middle of the intersection with heavy front end damage. The other vehicle Ma commercial V42208, 2014 white transit van resting into the chain link fence to the property of #99 Jackson Rd with heavy damage to the passenger side front quarter. I first spoke with the operator of the Van, identified as David Dwyer. The Van which David was operating is owned by Porcelain Patch & Glaze Corp. #140 Watertown St, Watertown Ma. David stated he wasn't sure what happened. He reported going N/B on Jackson Rd passing thru the intersection of Pearl St when the crash occurred. I asked him if he possibly might have not stopped at the traffic light(red)? He responded, "he wasn't sure". I then spoke with the other operator identified as Raul Davila. He reported that he was going W/B on Pearl St crossing the intersection of Jackson Rd when vehicle #2 struck his vehicle in the intersection. He further stated his light was definitely green. Both vehicles were towed from the scene by Todys. Both operators signed patient refusals. Mr Davila did complain of dizziness but refused to go with the Medics. Operator #2, David Dwyer was cited with Ma citation #T2014122 for failing to stop for a red light on Jackson Rd. The citation was later left at his work place at #140 Watertown St, Watertown Ma. with a fellow employee. David had gone back out on the road to a job. The property to which vehicle #2 caused damage to, I later gave the owner, Fernarla Thomasis the insurance information from that vehicle.

On August 13th, 2020, while working in marked unit N526, I responded to a 2 car MVA involving pedestrians at the intersection of Beacon Street and Chestnut Street, Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street and Chestnut Street are both public ways maintained by the City of Newton. The operator of MV1, Maria Dinapoli (S92626906), stated she was operating her 2018 Toyota Corolla ( MA REG: 1KNW71) heading west on Beacon Street and had the green light at the intersection of Chestnut Street. Dinapoli stated she was traveling straight through the intersection when MV2 which was traveling east on Beacon Street at the time cut infront of her vehicle to take a left turn onto Chestnut Street. Dinapoli stated she applied her brakes but was unable to stop in time and made contact with MV2. The impact of the crash caused MV1 to drive over the curb and onto the sidewalk where it made light contact with a baby stroller with a 1 year old inside. MV1 also struck and damaged a Newton Fire Department box that was located on the sidewalk. The baby stroller was being operated by the mother identified as Anna Gilman (S11328154). The pedestrians reported no injuries and signed patient refusals with Medic 2. MV1 sustained moderate front end damage and was towed from the scene by Todys. Dinapoli also reported no injuries and signed a patient refusal with Medic 2. The operator of MV2, Leena Dolabany (S12410136), stated she was operating her 2018 Jeep Grand Cherokee (MA REG: 1TCZ14) heading east on Beacon Street and had the green light at the intersection of Chestnut Street. Dolabany stated she was in a line of cars in traffic waiting to take the left turn onto Chestnut Street. Dolabany stated as the cars infront of her continued through the intersection she took a left onto Chestnut Street and as she was already into the turn she was struck by MV1. MV2 sustained moderate damage to the passenger side panel. Dolabany reported no injuries and signed a patient refusal by Medic 2. A query of all vehicles and parties involved through dispatch determined that MV2 was revoked for insurance. After speaking with the operator I was informed that MV2 had recently switched insurance agencies and Dolabany was able to provide paperwork from Geico Insurance stating the vehicle was insured. Due to the registry not displaying a valid registration, MV2 was towed from the scene by Todys. I advised Dolabany to contact her insurance company and advise them to update their information with the registry so she can retrieve the vehicle. Photographs of the scene and damage to city property were taken by Officer Wilson and submitted to the IT Bureau. On August 14th, 2020 I spoke with the operator of MV2, Leena Dolabany and her father George Dolabany in the front lobby of the police station. They provided paperwork from Geico Insurance that stated the vehicle was infact insured at the time of the accident and that there was an error made by the Registry of Motor Vehicles. Sgt. Devine and I advised them that we have to use the Registry's database for our investigation and because it still states the vehicle is revoked for insurance we can not release the license plates. Sgt. Devine advised them they could get another tow company to come and take custody of the vehicle but the plates will be seized until the registry shows an active status on the registration. I issued Mass uniform citation T2017328 in hand to Leena and advised her she will be summoned for a hearing for the following violations; Ch 90 section 34J Operating MV without insurance and Ch 90 section 9 Unregistered MV.

On Friday, September 18th, 2020 at approximately 1619 hours I, Officer Newton, was dispatched to Brookline St and Spaulding Ln(both are public ways in the City of Newton) for a report of a vehicle that crossed over Brookline St and struck a tree and drove off with major front end damage. The RP was able to give a description of the vehicle as a White Audi with the license plate beginning with 8X. The RP also stated the vehicle was going down Dedham St and possibly turned down Marvin Ln. Sgt M. Wade located the vehicle, White Audi MA Reg 8XVP10, with major front end damage, parked outside of 45 Indian Ridge Rd and made contact with the operator, John Bigda. It should be noted that the house is 2 houses from Marvin Ln. Upon arrival I spoke to John. John stated he was driving his vehicle and got into an accident but it was not a big deal because he was able to drive home. John was not able to explain the details of his crash and was very evasive as to exactly what happened, just that he got into an accident because somebody cut him off. John continued to tell me multiple times nothing bad happened and everything was fine. John was very unsteady on his feet and was stumbling around and almost fell several times while I was trying to speak with him. John also had slurred speech and a strong odor of an alcoholic beverage coming from his person that I was able to smell even though I was wearing a mask. At this time I asked John to take field sobriety tests to which he refused by saying "I'm not taking the test." The Medics arrived on scene to evaluate John. John was not able to answer their questions and the medics decided he should go to the hospital for an evaluation. However, John refused to go to the hospital. NFD arrived on scene and stated they saw tire marks and debris at the location of the crash. Ofc. Guarino went to that location to take pictures that will be added to this report. Due to the above facts and because John was almost falling over while speaking with us, he was placed under arrest for Operating Under the Influence of Liquor. John was handcuffed (D/L) and searched prior to being placed in cruiser 499. Cruiser 499 was searched before and after transport for contraband with negative results. John was booked and read his Miranda rights by Sgt M. Wade and Ofc Donovan processed him. John was handed citation T2014721 for 89/4/A Marked Lanes Violation (Driving over the double yellow line and leaving the roadway to strike the tree on Brookline St), 90/24/C Leaving the Scene of Property Damage (Hitting a city tree and causing damage), and 90/24/K (Operating Under the Influence of Liquor, 2nd Offense.. 1st offense was 11/18/2004 OUI docket 0443CR2527A CWOF ). The citation was placed with John's property. The vehicle was towed by Todys.

On 09/21/20 at 14:46 hours, I (Traffic Unit # 523) responded to the parking lot of 88 Chestnut Street for a motor vehicle accident involving a pedestrian. The weather at the time was 60 degrees ( localconditions.com) and the sky was clear and sunny. The road surface was dry. The parking lot is a thru way that the public use to enter/exit the lot or park there. On arrival at the same time with the Fire and Medics, I observed a female laying in the middle of the lot facing down on her back. A small red and gray CVS like type shopping carriage was knocked over and resting on it's side in front of the victim's head. The carriage was moved to the side in order to tend to the victim I also observed a black Honda Civic bearing MA reg.# 3741TW that was involved in the accident. The Honda was out of the parking spot resting approximately two feet away from the pedestrian's legs. I then spoke to the pedestrian identified as Darla Gipson in the rear of the ambulance. She sustained an abrasion to her front left shoulder and arm area which I observed. She also complained of left knee and elbow pain. She stated that she walking through the middle of the parking lot pushing the carriage by the door entrance in a westerly direction towards Chestnut Street. She states that she never saw a vehicle backing up and then she was suddenly struck and believes she was pushed underneath the vehicle. She was then transported to Newton Wellesley Hospital by Cataldo Ambulance. I then spoke to the operator of the Honda Civic identified as Linda Burke (MA S70320414). Ms. Burke was parked in the first spot to the right of the walkway that leads to the entrance/exit doorway. I asked her what occurred and she stated that she was backing her vehicle up to exit the parking lot when she looked behind her not seeing anything in sight. She then started backing up and struck the pedestrian without realizing it. She says the driver of a green cab and his female customer who were no longer on scene were yelling at her to stop her vehicle because the pedestrian was underneath it. She then stopped her vehicle. I observed minor scrapes to the entire rear bumper of the vehicle so it is unknown where it actually struck the pedestrian/carriage. I observed no obvious signs of damage to the carriage. It should be noted that Ms. Burke's Honda does not have a back up camera or back up sensors. There are no surveillance cameras to the apartment complex and no witnesses came forward for the accident. As a result of the investigation, Ms. Burke will be issued Uniform Citation # T1271069 for Newton City Ordinance 19-75 Failure To Use Care in Backing. Digital photos were taken by me of the scene.

On Thursday 10/29/20 at approximately 0541 hours while assigned to marked unit n499 I was dispatched to the area of 130 Vine Street for a report of a single vehicle roll-over crash. Upon arrival myself and n496(Ofc. Howes) observed a white SUV on its roof facing westbound in the eastbound lane of Vine Street (a public way owned, operated and maintained by the City of Newton). Upon our arrival Newton Firefighters had extracated the operator, (FOSTER Isabelle), who was sitting on a large rock in the front yard of 130 Vine Street. FOSTER was visibly upset, but didnt appear to have any major injuries. I asked FOSTER what happened and she stated she was travelling from Westgate Rd in Boston on her way home to Wellesley. She took a right turn onto Vine Street from LaGrange Street and began travelling westbound. FOSTER stated that she was travelling too fast and didnt realize how the road curved causing her to veer off the road and strike a large tree in the front of 130 Vine Street. FOSTER was travelling at a speed fast enough to cause the vehicle to flip onto its roof in the opposite lane of traffic. FOSTER admitted to me she was travelling too fast on the road. Vine street is a winding road that is not very well lit. Speed limit on the road is 20mph and its estimated FOSTER was travelling at least 30-45mph. The speed coupled with FOSTER's lack of familiarity with the area and her inexperience driving were all contributing factors to the crash. FOSTER was issued Massachusetts Uniform Citation T2012909 with the following civil motor vehicle infractions; C90 s18 SPEEDING (Posted) C89 s4a Marked Lanes Violation. FOSTER was transported to Beth Israel hospital by the medics and I was unable to issue the citation in hand at the time of the crash. Due to FOSTER's age (17) I also wanted to notify her parents and meet with them and FOSTER upon her release from the hospital. At approximately 1700 Hours I met with Scott Foster(Father) in person, Jodi Foster (Mother) and Isabelle foster via cell phone (speaker phone). I discussed the charges with all parties and explained the citation to Isabelle. Citation was given in hand to her father, Scott Foster and brough the citation to Isabelle.

On, 11/19/2020 at approximately 1700 hrs, Newton Police and Fire were dispatched to Commonwealth Ave. and Prince St. for a reported head on motor vehicle collision with injuries. Upon arrival, I observed MV#1 ( 1WDR15) facing eastbound in the westbound travel lane. MV#2 (986KW1) was also facing eastbound, but in the eastbound travel lane. Both vehicles had significant front end damage, consistent with a head on motor vehicle collision. The operator of MV#1 was sitting on the grassy median upon my arrival. She stated she was traveling eastbound on Commonwealth Ave. when she became distracted by the radio, crossed over the double yellow line into the westbound lane of Commonwealth Ave. and collided with MV #2. The operator of MV#2 was entrapped in her vehicle, but was able to be extracted from the vehicle by Newton Fire personnel. Operator #2 was able to inform me that she was traveling westbound on Commonwealth Ave. She noticed MV#1 cross over the double yellow line at a high rate of speed, and collide into her vehicle. The operator of MV#2 was conscious and alert and transported to Tufts Medical Center Boston for an evaluation. The operator of MV#1 declined medical attention and was given a Massachusetts Uniform Citation (T2014484) for violating MGL. Ch. 89. Sec. 4(a) Marked Lanes Violation. Both vehicles were towed away from the scene by Tody's Towing Service. Photographs of this collision were taken on the (N492) camera and will be attached to this report.

On 12/05/2020 at approximately 22:00 hours I was called in to respond to a single vehicle crash with serious injuries in the area of 291 Nahanton St. Nahanton St is a pubic way in the City of Newton. Upon my arrival at the crash scene with Sgt Devine I was briefed by Officers Daly and Anderson, ( see Ofc Daly's report for further information CAD # 20054728 in regard to changes take out against the operator of the truck ) I made the following observations of the crash scene, Nahanton St is a public way in the City of Newton that is a two way roadway divided by an unprotected merdium ( double yellow line ) that runs east to west. The roadway surface was wet due snow and ice. The roadway was plowed and the snow was pushed to the shoulder, althought wet the roadway was icy. There was approximately 2 inches of snow on the shoulders of the roadway. The road was light with overhead head street lights. The posted speed limit is 25 MPH. I observed a 2013 Chevrolet Silverado pick up truck that had major front end damage. The Chevrolet was at an angle with it's front end facing west bound and resting on the grass shoulder and into a wooded area. The Chevrolet rear was sticking out into the shoulder of the east bound lane of Nahanton St. The passenger side front and rear doors of the Chevrolet had been removed by the fire department to extricate the driver and passenger. The driver and passenger side air bags were deployed . The front passenger side windshield of the Chevrolet was completely missing and the driver's side had completely shattered. The front tires, hood, and engine had been compacted on it's self. The front and rear driver's side doors had been mechanically open by the fire department. The speedometer needle had stopped at 54 MPH. The Chevrolet left the roadway and left tire marks in the snow in the eastbound bike lane of Nahanton St. At a 106.4 feet from the beginning of the tire marks the first point of impact was with two tree stumps that were 3.5 feet from the roadway, four feet tall and 3.5 feet in diameter. The tire marks continue in the east bound bike lane and exit the roadway and onto a dirt shoulder at 136.1 feet from the beginning of the tire marks. The truck continued to the right of a utility pole and then it's passenger side made contact with a tree at 170.8 feet from the begin of the tire marks. The tree was 12 feet from the bike lane approximately 5 feet in diameter. The impact with the tree caused the truck to turn so that the rear was facing north east on the dirt shoulder and bike lane. The final point of rest of the rear passenger side tire was 200 feet from the beginning of the tire marks. The truck's rear diver side tire final point of rest was 209 feet from the beginning of the tire marks. The front driver's side tire was compacted into the trucks engine and the passenger side front tire was located behind the tree approximately 20 feet from the roadway. Sgt Devine took photos of the crash area and of the truck and the disk was download by the NPD's IT Bureau. The truck was towed from the crash scene by Todys's Towing to their lot for storage, I fill out a towed motor vehicle form. There were no reported witness who observed the crash. The crash was reported by an unknown passerby. On 12/07/2020 Ofc Gaudet faxed a request for preservation of medical records, blood and urine samples of Schneider to Beth Israel Deconess Medical Center.

On 12/10/20 at approximately 13:23 myself along with other Police, Fire and Ambulance units, responded to the intersection of Chestnut St. and Elliot St. for a two vehicle crash. Upon arrival I spoke with the operator of veh#1 who stated she was driving on Elliot St. due West when she proceeded through the intersection and was struck on her passenger side by veh #2. The operators 2 young children were in back of her vehicle in child seats. Her son was behind the passenger and her daughter was behind her. Both front and side airbags were deployed in veh #1. All three people in the vehicle were taken to Newton-Wellesley Hospital to be checked out as a precaution. The operator stated she had pain in her right wrist. The operator of veh#2 stated he was traveling due South on Chestnut St. when he looked down as he approached the intersection. The operator stated he was prepared to take a right onto Elliot St. but when he looked up he had already entered the intersection and struck veh #1. The air bags went off in veh #2 as the operator had cuts on his left hand . He was checked out by the Medics and signed a patient refusal. The operator of veh #2 had left the scene after I initally spoke with him but while I was attending to the occupants of veh #1. Because of this I was unable to issue him a citation on scene. I mailed him Mass. Citation #T2080850 for Ch 89/ Sec 9 Failure to obey the Red Light. Both vehicles sustained heavy damage and were both towed by Bobby and David from Tody's towing.

The operator of MV#1 stated that he was traveling north on Cherry St when he struck the fire hydrant in front of #35 Cherry St. Upon further questioning, the operator Johnathon Benson stated that he has been diagnosed with a medical condition that sometimes affects his coordination. I asked if he should be driving due to his medical condition and he stated his doctor has suggested he not drive. I then asked if he thought it was his medical condition or another factor which caused the crash. Mr. Benson stated he believes his medical condition is likely what caused the crash. When Mr. Benson exited his MV I noticed a large wet spot on both the front and rear of his pants around the groin/buttocks area. I then asked Mr. Benson if he has had siezures in the past, to which he stated he has. I then asked Mr. Benson if its possible that he may have had a seizure while driving today and he stated it was. Mr. Benson did not sustain any injuries, his MV was towed by Tody's and the Newton Fire Dept. was notified. At this time due to the information Mr. Benson provided and my concern about his/others safety while he operates a MV, I am filing an Immediate Threat with the RMV. On 3/8/21 I mailed citation #T2080897 to Johnathon Benson for operating Tx. Reg. HVY7642, which was unregistered at the time of the crash.

Operator of vehicle one Ziting Liu stated on 05/14/2021 at approximately 17:38 hours she was driving west bound on Commonwealth Ave in her vehicle a 2015 Honda CRV ( MA reg 497yy1 ). Liu stated that she was driving into the setting sun and was wearing sunglasses at the time. Liu stated that as she approached the four way intersection with Chestnut St she observed that west and east bound traffic on Commonwealth Ave had a green light and that north and south bound traffic on Chestnut St had a red light. Liu slowed her vehicle to turn left onto the south bound lane of Chestnut St. It should be noted that both Commonwealth Ave and Chestnut St are both public ways in the City of Newton. Liu stated that she did not observed any " cars " driving east bound on Commonwealth Ave so she proceed to make the left turn onto Chestnut St. Liu stated that she realized that her vehicle has struck an unknown object as she was turning left. Liu stopped her vehicle and saw that her vehicle had struck a female bicyclist. Liu stated that the bicyclist was thrown from their bike and was on the ground. Liu stated that she was not injured as a result of the crash. Liu's vehicle had damage to the front passenger side hood . The front passenger side of Liu's windshield had an inward crack that was approximately 12 inches in length and 12 inches in width. The type of inward crack on the windshield is indicative of an outside force making contact with it. Liu's vehicle was towed by Tody's Services to their lot for storage. I filled out a towed motor vehicle form. Bicyclist Isabella Turinovic stated that she was ridding her bike , east bound on commonwealth Ave near Chestnut St. Turinovic stated that east bound traffic had a green light. Turinovic stated that as she was crossing over Chestnut St Liu's vehicle struck her causing her to hit the windshield. Turinovic was tossed from her bike and onto the roadway. Turinovic was wearing a helmet at the time of the crash. Turinovic stated that she did not lose consciousness and she was transported to Newton Wellesley Hospital via EMS from treatment . Turinovic agreed to have her bike taken by NFD Ladder One to be stored at NFD station Two. I took photo's of Liu's vehicle, Turinovic's bike and the disk was downloaded by the NPD IT Bureau. Witnees Alexander Delima stated that he was the first vehicle stopped a red light on Chestnut St in the south bound lane. Delima stated that Liu's vehicle did not yield to Turinovic as she turned left onto the south bound lane of Chestnut St from the west bound lane of Commonwealth Ave .A second witness Lindsay Bentis stated that she was the second vehicle stopped a the red light in the south bound lane of Chestnut St behind Delima. Bentis stated that she also saw Liu fail to yield to Turinovic as she turned onto Chestnut St. Based upon statements made to be by all involved parties, I concluded that Liu is at fault for this crash. I issued via U.S. Mail, Liu MA Uniform Citation T1446670 and cited her for a violation of MGL 90/14, failure to yield to oncoming bicyclist when turning left.

MV1 was stopped in front of 206 Elliot St. waiting to turn left onto Cottage St. when MV2 struck the rear end of his vehicle. I asked the operator of MV2 if he was following MV1 too close which he stated "no" that he had looked away for a second and when he looked back up he noticed that MV1 was stopped and that was when he tried to stop and struck the back of MV1. Newton Fire Department was on scene and witnessed the incident. Both operators did not want medical attention. Operator of MV2 was given a written warning for improper operation 90/12 (Citation #2014395). Operator of MV2 was not given a citation for following too close, due to admitting that he was not close, but had looked away from where he was driving. MV1 was operable and was able to leave the scene. MV2 sustained heavy front end damage and was towed away by Tody's Towing Service.

On Friday, July 16, 2021, while assigned to Traffic unit N525, I responded to the intersection of Commonwealth Avenue and Temple Street, Newton for a crash involving a City if Newton Fire Department vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. Commonwealth Avenue and Temple Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Mr. Jacob Forbes. Mr. Forbes stated he was at a stop sign on Temple Street and was attempting to cross Commonwealth Avenue (N) from Temple Street. Mr. Forbes stated he looked to his right, and then to his left and did not see any vehicle approaching. Mr. Forbes stated as the began to enter the intersection he looked to his right and saw a vehicle pulling over to the side of the road and a Newton Fire truck with it's lights on was heading towards him. Mr. Forbes stated he could not avoid the collision and the front end of MV2 crashed into his passenger side. Mr. Forbes reported no injuries and signed a patient refusal with Fallon Medics. I observed damage to the front passenger side area of MV1 and it was removed from the roadway by Tody's Towing. MV2 is a City of Newton Fire Department vehicle "C2". It is a 2019 Ford Expedition (MF455). The vehicle was responding to a Fire Alarm Activation call with emergency lights and siren activate at the time of the crash. The occupants of the vehicle were taken to Newton Wellesley Hospital for evaluation. I observed front end and driver side damage to MV2. There was air bag deployment inside the vehicle, on both the front driver and passenger side. Tody's Towing removed MV2 from the roadway. Photos were taken of the crash scene and submitted to the IT Bureau. I responded to Newton Wellesley Hospital and spoke with Newton Firefighter John Morris (Driver) and Deputy Chief Michael Mcnamara (passenger) in the emergency room. Both parties stated they were responding to a call with emergency lights and siren activated. FF Morris stated as he approached the intersection of Commonwealth Avenue (W) and Temple Street, he observed a black SUV exiting Temple Street onto Commonwealth Avenue. FF Morris stated as they were passing he believed MV1 tried to speed up to cross Commonwealth Avenue. MV2 then crashed into MV1. As a result of the crash, FF Morris suffered right hand/wrist injury. Both FF Morris and Deputy Chief Mcnamara stated they had slight respiratory issues from breathing in the powder from the air bag deployment. MV1 failed to yield to MV2's right of way as he attempted to cross Commonwealth Avenue from Temple Street. The operator of MV1, Mr. Jacob Forbes will be mailed Massachusetts Uniform Citation 071113AB for Chapter 89, Section 8 (Fail to Yield at Intersection).

Narrative:
I responded to 234 Langley Road for a two car M/V crash with entrapment. Upon arrival with other officers, Fire, and medics I saw the following. MV1 was flipped over on the road way and the operator was still in the vehicle, but the passenger was outside the vehicle. MV1 Hit a parked MV2 unoccupied. MV1 was traveling north on Langley Rd when MV1 swerved to its right and made contact with the left rear bumper of the MV 2. The impact of the two MV's caused MV1 to flip over and land on its roof. Based on my observation the scene indicated : day light, clear and there were no road defects. I took numerous pictures which were submitted to IT bureau so they can be attached to this report. Both MV1 driver and passenger were taken to Beth Israel Hospital in Boston. Tody's arrived on scene and took possession of both MV's. A canvas of the area resulted with one neighbor at 255 Langley who witnessed the accident. Mr. Vish Showdhary stated that he saw MV1 driving on Langley Rd when MV1 crashed into the parked MV. He stated that there were no other vehicles on the road and that MV1 drove right into the parked vehicle. The scene was cleaned and I went to Beth Israel Hospital to speak with the operator and passengers of MV1. The operator of MV1 stated that they have no recollection of what happened. She stated that she does not know how she hit the parked MV and that the last thing she remembers is being pulled out of the rolled over MV by the Fire Department. The passenger in MV1 stated that he does not remember how the crash occurred. He stated that he was unsure how MV1 crashed into MV2. He stated thathe was able to get out of the MV without a problem. After a review of the crash I am citing MV1 with the following. M.G.L 89/4A Marked lanes and M.G.L 90/24E Negligent Operation.(Citation # 1445881) Furthermore a request for immediate threat license revocation was filled and submitted to the RMV. The MV Citation was delivered to the operator of MV1 in hand at the Beth Israel Hospital.

Upon arrival, vehicle 1 was rolled over and facing Eastbound in the Westbound lane. There was extensive damage all over the vehicle and it was leaking fluids. It appears that vehicle 1 was traveling Westbound on Commonwealth Ave and struck a tree and then crashed into the guard rail causing it to roll over. There was damage to a city tree and damage to the guard rail. Operator 1 stated (through translation from Officer Acuna), he was traveling Eastbound on Commonwealth Ave at approximately 25 MPH when he was looking at his GPS and his brakes gave out causing him to crash. When asked again, his story changed and he was not sure what direction he was traveling in. Newton Fire was able to retrieve operator 1's cell phone in the vehicle which was on a loop playing videos on instagram. Operator 1 was transported to Beth Israel Hospital by Fallon Ambulance. Tody's towed vehicle 1 and pictures were taken of the damage to city property and will be submitted to the IT Bureau to be attached to this report. MV1 operator stated he was operating westbound on Watertown St by Dunstan St (both public ways). MV1 operator struck (side swiped) both the driver side's of MV2 and MV3 which were parked in front of 915 Watertown St. MV1 operator struck the curb in front of 919 Watertown St and left the roadway. After MV1 left the road way it hit and destroyed a fire hydrant owned and maintained by the City of Newton. MV1 operator then hit the private wall of 919 Watertown St and it was it's final resting location. MV1 sustained extensive and complete front end and under carriage damage, it was towed by Todys. MV2 sustained minor damage and paint transfer, no tow required. MV3 sustained extensive driver side wheel well damage, which will privately towed at a later time. MV1 operator stated he struck his head and was not feeling well; he was transported to Newton-Wellesley Hospital by Newton Medics. MV2 and MV3 were not occupied. The City of Newton was notified in regards to the fire hydrant.

On November 21st, 2021 at approximately 07:56 hours while working N491 I along with Officer Mike Boudreau responded with NFD and Medic One to a report of a MV crash with injuries in the area of Centre St @ Franklin St. On my arrival the Medics were evaluating two adult males who standing outside one of the involved vehicle from this crash. There appeared to be several involved vehicles to this crash however two of them had been parked prior to the crash. The vehicle which the two unidentified males were standing by was MA reg. 9HY939, a 1988 Black Ford F150 PU, listed registered owner was a Peter J. Jara-Castro, #28 Ridge Ave, Lynn Ma. Mr Castro was one of the two males standing by that truck. The above vehicle appeared to have been travelling N/B on Centre St when it skidded off the road, jumping the curb striking a 1 foot slate retaining wall to the property to #573 Centre St. It continued onto that property driving over shrubs, small trees, then onto the driveway(same address) which is actually on Newtonville Ave, hitting the homeowners 2007 Nissan Versa, Ma reg. 6BH269, forcing it into more shrubbery. The Truck continued across Newtonville Ave where it struck another parked vehicle, a Grey 2019 Volkswagon, Ma reg. 1NFC27 on the rear passenger side pushing it forward. The truck finally came to a stop after driving into a wooden fence to the property of #559 Centre St.(Fence is on Newtonville Ave.) I spoke with the owner of the truck after he was evaluated by the Medics, who spoke limited English. The owner of the truck Peter Jara-Castro finally said he was driving the truck prior to the crash. His statement was he was travelling N/B down Centre St when another vehicle came out from Newtonville Ave causing him to slam on his brakes. Mr Castro doesn't remember much after that, possibly from hitting his head. At this time I couldn't determine who the actual driver was. Both Mr Castro and the passenger, sustained injuries but both declined to go to the hospital. Both were later picked up by an UBER driver. A Firefighter heading to work reported seeing the truck pass him on Centre St N/B at high rate of speed but never saw the driver or the actual crash. Both property owners that sustained damage were given Trucks insurance info. Pictures were taken by both property owners. Vehicle #1 and vehicle #2 were towed from scene. Both sustained heavy damage. Vehicle #3 sustained minor damage I was unable to make contact with that owner at this time. However later in the day I was able to make contact with owner of vehicle #3. She was given all vehicle #1 information along with his insurance. A citation for speeding was later issued after investigation was completed and witness statement. Citation#t1444076 was later sent VIA mail.

On Sunday November 21st at approx. 1720 Hrs. I responded to 191 Jackson Rd for a motor vehicle accident roll over with entrapment. Upon arrival I witnessed MV1 on its side in the middle of Jackson Rd. The operator of MV1 was conscious and alert and was taken out of the MV by the Newton Fire Department. The operator of MV1 stated that she was driving S/B on Jackson Rd. and she did not see the parked car on the right side of the road. MV1 then tried to swerve around MV2 and clipped the left rear bumper, and left rear door of MV2 which caused MV1 to roll over onto the middle of the road. MV1 had major damage all around the vehicle and all airbags deployed. MV2 had major damage to the rear and to the left side of the MV. Due to the extensive damage and length of travel of MV1 it was estimated that MV1 was traveling at a speed greater than 35 MPH. Speed limit for Jackson Rd is 25 MPH. MV2 was pushed approx. 20 feet from where it was parked, and MV1 travelled approx 25 feet after on its drivers side after impact. The operator of MV1 was transported by EMS to NWH, and Todys's took possession of both motor vehicles. Pictures were taken and submitted to IT. The operator of MV1 was cited 90/17, speeding.

On Tuesday November 30, 2021 at approx. 0857 hours I responded to the area of Chestnut Street and Ellis Street for a report of a two motor vehicle crash with unknown injuries. Upon arrival on scene Medics and Fire personnel were rendering aid to all operators / occupants, due to the location of the crash additional units were requested on scene to divert traffic. Operator of vehicle one Ms. Kim, stated she stopped at the stop sign. She stated her vision was obstructed with a truck that was parked on the northeast corner of Chestnut Street / Winter Street.. She stated she accelerated forward looking south and north. She stated she was going through the intersection when vehicle two traveling southbound had struck her vehicle. Operator of vehicle two Mr. Lafaiete-Gheringer stated he was traveling south on Chestnut Street when vehicle one traveled west from Winter Street through the intersection and struck his vehicle. After Medic evaluations on Ms. Kim and her child Ariela Seok, she signed patient refusal's with no reported injuries at this time. Mr. Lafaiete-Gheringer was transported to Newton Wellesley Hospital for a minor laceration on his head due to impact, non life threatening. Todys towing of 1354 Washington Street arrived on scene and towed both vehicles to their tow lot. Towed inventory sheets were completed. All parties advised of the process and I advised them my contact information. After reviewing the crash site area, possible obstructions and a safer way for vehicular and pedestrian traffic, I am recommending a stop sign be placed on the southbound lane Chestnut Street just prior to Ellis Street. In addition Mr. Lafaiete-Gheringer has an active registration without a Massachusetts license with an effective date of August 16, 2021. He currently has an active California License, with an California address. Therefore I am issuing and mailing Mr. Lafaiete-Gheringer MA citation number T1445575(warning) for Chapter 90 Section 26a Failure to notify registry of his license address change.

On Thursday, December 16, 2021, while assigned to Traffic unit N525, I responded to the area of Crafts Street and Linwood Avenue, Newton for a report of a single car motor vehicle crash with entrapment. The weather at the time of the crash was cloudy. The road surface at the time of the crash was wet. Crafts Street and Linwood Avenue are both public ways in the City of Newton. On my arrival, I observed a 2007 Toyota Rav 4 (MA: 7565LR), facing North in the Northbound lane of Crafts Street past the Linwood Street intersection. The vehicle had extensive damage to it's passenger side and was resting on it's drivers side. The lone occupant, Mr. Frank Distefano (S00385929), was trapped in the drive seat of the vehicle and was unable to exit due to his legs being pinned under the steering wheel/dash area. First Responders were able to rescue a pug dog from the vehicle and care for it until a family member responded to take custody of it. The dog did not appear to be injured. Newton Fire responded and had to use stabilizing equipment to support the vehicle during their rescue efforts to make sure the vehicle did not roll over onto it's roof. Newton Fire used specialized extrication tools to remove the roof of the vehicle to gain access to Mr. Distefano. Mr. Distefano was removed from the vehicle by Newton Fire and Medics and transported to Brigham and Women's Hospital in Boston for his injuries. Tody's towing removed the vehicle from the roadway. While Newton Fire was attempting to removed Mr. Distefano from the vehicle, I spoke with a witness to the crash. Mr. John Devalpine (S65213095). Mr. Devalpine stated he was operating his vehicle behind Mr. Distefano on Crafts Street (N). Mr. Devalpine stated he observed Mr. Distefano's vehicle stopped at the light on Crafts Street (N) at Linwood Avenue. Mr. Devalpine stated he then observed Mr. Distefano's vehicle accelerate from it's stopped position and drive across the intersection towards the right side of the roadway. Mr. Devalpine stated the vehicle then crashed into a City of Newton electrical box on the sidewalk. The collision caused the vehicle to turn over and land on it's driver side. I responded to Brigham and Woman's Hospital and spoke with Mr. Distefano in the Emergency Department. His wife, Clara, was present at this time. Mr. Distefano stated he was operating his vehicle and the next thing he knew his airbags deployed and he was trapped inside his vehicle. Mr. Distefano could not recall any other information about the crash. As a result of this crash, Mr. Distefano was cited with Massachusetts Uniform Citation 230024AB for Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Due to Mr. Distefano not being able to recall the events that led to his crash, and his recent driver history, I completed and submitted an Immediate Threat with the Registry of Motor Vehicles for Mr. Distefano. This request was emailed to the Registry (DCUImmediateThreat@massmall.state.ma.us). Photos were taken of the crash and submitted to the IT Bureau.

On Saturday, January 22, 2022, at approximately 2012hrs, MV1 was traveling Southbound on Centre St. when it was struck head on at the intersection of Walnut St. and Centre St. MV2 was traveling Northbound on Centre St. and struck MV1 when attempting to take a left turn on to Walnut St.. The operator and passenger of MV1 stated they were traveling straight through the intersection when they witnessed MV2 cross into their lane to turn onto Walnut St.. Operator of MV1 stated she slammed on her brakes, but was still struck by the on coming vehicle. The operator of MV2 stated she was traveling Northbound and was preparing to take a left turn onto Walnut St. The operator stated she was not paying attention and did not see MV1 when turning. Newton Fire Department and Fallon Ambulance Service were on scene to assist. The operator and passenger of MV1 and the operator of MV2 were evaluated by paramedics and signed patient refusals. Both vehicles sustained heavy front end damage and were towed by Tody's Towing and removed from the scene. The operator of MV2 was issued in hand Citation #T2014399 for failure to yield when turning. Both operators were informed a report would be generated in regards to the incident.

On Tuesday, February 15th, 2022 at approximately 1908 hours the operator of Vehicle 1 was travelling Westbound on Auburn St when it crossed over the center double yellow lines, crossed the opposing lane of traffic, and went off the roadway before crashing through the vinyl privacy fence at #106 Auburn St. Vehicle 1 came to rest after colliding with the North-East corner of the residence. The operator of Vehicle 1 then attempted to reverse away from the crash and backed into and destroying more of the residence's privacy fence. The operator was then confronted by the occupants of the residence, including the homeowner. Fire and EMS arrived on scene and conducted patient care on the operator and formed the opinion that she was intoxicated. I arrived and interviewed the operator who stated that she had previously taken oxycodone. The operator was then transported to Newton Wellesley Hospital. The vehicle was towed from the scene by Tody's. The operator was charged with the following violations: C90ss24 OUI- Drugs C90ss24 Neg Op C89ss4A Marked Lanes An incident report was completed for this crash (See Report #22005833).

There was already existing damage to the stone wall and the fire hydrant outside of 919 Watertown St. There was no new damage from this accident to either wall of hydrant.

On Tuesday, June 28, 2022, while assigned to unit N496, I responded to the area of Langley Road and Hamlet Street, Newton for a report of a roll over crash with 3 injured parties. The weather at the time of this crash was clear and sunny. The road surface in the area of the crash was dry. Langley Road and Hamlet Street are both public ways maintained by the City of Newton. I approached the area of the crash coming from Boylston Street and when I arrived my attention was drawn to two locations on Langley Road. I observed a female party laying prone on the sidewalk area of Langley Road (E) at the Hamlet Street intersection. This female party, later identified as Ms. Melinda Conroy, was being attended to by passerby's in the area. My attention was then drawn to an overturned vehicle resting half in the roadway/half on the sidewalk (W) in front of 323 Langley Road. As I began to run over to the vehicle, I observed a woman standing next to the vehicle screaming frantically. This party identified herself as the driver of the lone vehicle involved, Ms. Natalya Kabanovsky (S65512051). There were parties at the back of the vehicle yelling to me that there was a woman trapped under the vehicle and that they were holding the vehicle up to get her out. I ran over to the rear of the vehicle and observed a female party, later identified as Ms. Annette Sooper, lying underneath the vehicle in the roadway. There was a large pool of blood on the roadway as the result of a laceration to what appeared to be her head/face area. Due to the unknown stability of the vehicle resting on its roof, and due to the passerby's stating they were holding the vehicle up so it did not roll over onto Ms. Sooper, I reached under the vehicle from the rear, grabbed Ms. Sooper's person, and dragged her away from the vehicle to a safe location on the sidewalk. At this time, I observed a large laceration to Ms. Sooper's head and she stated she had suffered a leg injury. Newton Fire and Medics along with additional Police units arrived on scene and took over rendering aid to the victims. I then started to conduct my investigation into the crash. I spoke with the operator of the vehicle, Ms. Kabanovsky. Ms. Kabanovsky stated she was operating her 2005 Lexus ES330 (MA: 293TAB) on Hamlet Street (W) towards Langley Road. Ms. Kabanovsky stated as she approached the intersection, a female party (Ms. Conroy) ahead of her was running across Langley Road (S) at Hamlet Street. Ms. Kabanovsky stated she attempted to hit her brakes, but instead hit her gas pedal. Ms. Kabanovsky's vehicle crashed into Ms. Conroy and then she panicked. Ms. Kabanovsky stated her car then accelerated across the street and crashed. Ms. Kabanovsky stated she was not wearing a seatbelt at the time of the crash and extricated herself from her vehicle after it rolled over onto its roof. I requested Ms. Kabonovsky's license while speaking to her, but she was too upset to located it. I queried Ms. Kabanovsky's license and registration status through Newton Dispatch. Ms. Kabanovsky's license stated is expired as of 12/18/2016. Ms. Kabanovsky registration is active. I observed substantial damage to the entire front end, driver, and passenger side of Ms. Kabanovsky vehicle. Ms. Kabanovsky was evaluated by Newton Medics and complained of right arm pain. Ms. Kabanovsky was also visibly distraught and expressed concern over the two pedestrians involved in the crash. Ms. Kabanovsky was transported to Brigham and Women's Hospital for treatment. I briefly spoke with Ms. Melinda Conroy as she was being attended to by Newton Medics and Fire. Ms. Conroy stated she was running on Langley Road (S) and observed MV1 traveling on Hamlet Street (W) towards Langley Road. Ms. Conroy stated she believed MV1 was slowing down and coming to a stop. Ms. Conroy stated she waved at the vehicle and as she crossed the intersection, MV1 accelerated and crashed into her left side. Ms. Conroy stated she flipped over the vehicle and landed on the roadway. Ms. Conroy was conscious and alert when speaking to Newton Medics and was transported to Beth Israel Deaconess Hospital in Boston. I later spoke with Ms. Conroy after her release from the hospital. Ms. Conroy stated she suffered some bumps and bruises had no major injuries. Officer Pohlman spoke with Ms. Sooper prior to her transport to Beth Israel Deaconess Hospital in Boston by Newton Medics. Ms. Sooper stated she was walking on the sidewalk in front of 323 Langley Road after returning from Newton Centre. Ms. Sooper stated as she was walking, she observed MV1 hit Ms. Conroy and then travel across the street in front of her into a tree. Ms. Sooper stated she does not remember what happened after that. I spoke with Ms. Sooper's son at NPD HQ and was advised Ms. Sooper was admitted to the hospital and suffered a fracture of her left hip and required 6 staples to close a laceration on her head. After speaking with all parties involved, Ms. Kabanovsky will be cited with Massachusetts Uniform Citation 494688AB in hand at her residence for Chapter 90, Section 10 (Unlicensed Operation), Chapter 90, Section 24 (Operate To Endanger), and Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Ms Kabanovsky was also cited with Massachusetts Uniform Citation 494800AB in hand at her residence for Chapter 90, Section 13A (Fail To Wear A Seat Belt). A Request for Immediate Threat License Suspension/Revocation was completed and submitted to the Registry of Motor Vehicles with supporting documentation via e-mail, DCUImmediateThreat@massmail.state.ma.us.

Narrative:
On July 4, 2022 at approximately 06:56 hours I, Officer Guarino, responded to the area of Commonwealth Ave at Centre St for a report of a motor vehicle crash. Upon arrival, I observed the fire and medics on scene with a two vehicle motor vehicle crash, one of which was a rollover. MV1 was on the roof of its car in the northbound lane of Centre St. MV2 was in the Eastbound Lane of Commonwealth Ave. I spoke with the operator of MV2, Annika Morris-Kelly who told me that she was traveling Eastbound on Commonwealth Ave when she stopped at the red light. The light turned green and she began to go, as she proceeded, she noticed MV1 traveling southbound on Centre St and tried to stop but crashed into MV1. I spoke with the operator of MV1, Luymar Bezerra, who said that he was traveling southbound on Centre St and did not see the traffic signals due to the trees on the side of the road and continued through the intersection causing the crash. I asked Luymar for his license and registration. He provided me with a passport and a CT registration for his vehicle. Luymar said that he did not have a license on his posession and did not have a valid license within the United States. He said that he had a Portugese driver's license and has been in the country for less than a year. I was unable to confirm this information and issued a criminal complain against Luymar Bezerra on MA Uniform Citation T1447651 for MGL 90/10 Unlicensed Operation of a motor vehicle. I explained the citation to Bezerra, who said that he understood. All partied involved were seen by medics and signed a patient refusal. Both vehicles were towed from the scene by Todys.
