Narrative:
MV#1 stated while traveling westbound on Commonwealth Ave he closed his eyes for a few moments causing his vehicle to veer to the right. MV#1 drove up on the curb and struck a overhanging street light. MV#1 sustained heavy front end damage as well as uprooting the overhanging street light. Operator of MV#1 stated he had minor injuries and signed a patient refusal. Operator of MV#1 was given the following roadside tests, due to the time and circumstances of the incident. Horizontal Gaze Nystagmus - Pass One Leg Stand - Pass Walk And Turn - Pass Operator of MV#1 showed no signs of impairment. Through my training and experience I calculated MV#1 had speeds of approximately +50MPH. Tody's towed the MV, a towed MV inventory form was completed. Pictures taken on scene submitted to IT.

Narrative:
On April 24, 2019, while assigned to N496, I was called by Dispatch to respond to the area Beacon Street and Hammond Street for a motor vehicle crash. This was called in by Boston College Police Department. Upon arrival MV #1 was on Beacon Street just past Hammond Street at the Island with considerable vehicle damage. MV#1 stated while traveling westbound on Beacon St, while approaching the intersection of Hammond he stated he started to feel strange and not like himself. MV#1 stated he then struck the curb with his front right tire, not believing his accident was very serious at this time. He reversed his vehicle off the sidewalk and proceeded to travel westbound on Beacon St. After passing through the intersection he stated his car felt very off at which point he initiated his hazards and pulled over to the right. MV#1 stated he has a history of epilepsy and currently takes Oxcarbazepine twice a day for his condition. He stated he did not believe he had a seizure or lost consciousness but did not recall the incident very well. Boston College Ofc. Cadogan witnessed the incident, and stated the following, while conducting a directed patrol at the intersection of Beacon and Hammond St he saw MV#1 traveling westbound on Beacon St. Ofc. Cadogan stated MV#1 then took a sudden tight turn on to the curb striking the metal fence surrounding the BC facility. MV#1 was then seen / heard revving his engine while still in drive before reversing his vehicle off the curb and attempting to continue on Beacon St westbound. The fence surrounding the BC Facility sustained moderate damage. Tire marks in the grass show the path of MV#1 in direct line to the damaged property. While on scene, I spoke at length with Vanhorne. I did not detect or any alcohol from his breath as he spoke. Vanhorne reported he only consumed one beer tonight at roughly 2030hrs and smoked a little marijuana at 2200hrs. Vanhorne's eyes did not detect any signs of alcohol and he walked with no signs of any impairment. Sgt. Devine administered a horizontal nystagmus test which revealed no signs of impairment. Vanhorne reported he was at a work event and then visited a friend in Brookline. Vanhorne reported his last seizure from epilepsy occurred a little over two years ago. Pictures of the scene were taken and submitted to IT to be attached to this report. MV#1 operator stated he was not injured at this time. MV#1 was towed from the scene by Todys's due to heavy front end damage. Vanhorne was picked up from the scene by Paul Miller his step father. A towed MV form was filled out and placed in the traffic box. Witnesses Cassandra Williams and Bill Peirce observed the crash and stated identical narratives to Ofc. Cadogan's. An Immediate Threat form was filed with the RMV (via Fax and E-mail at approximately 0650hrs 4/24/19)due to his medical history and his explanation with this crash. All streets mentioned are public ways in the City of Newton. It should be noted that the road was wet from rain on this night. Update: on 4/27/19 Sgt.Devine was able to watch the video of the incident from Boston College Police. The video confirmed the statements given by BCPD and witnesses's. Due to the aforementioned statements and the video surveillance of the incident, MV#1 Operator will be cited with Marked Lanes Ch89/4a (MA Uniform Citation #T1441792. Vanhorne will be notified via phone he will be receiving the ticket in the mail.

Narrative:
The operator of MV1 stated he was taking a left onto Waldorf Rd. Westbound from Canterbury Rd. when he saw a child bicyclist traveling Eastbound on Waldorf Rd. in the street. He stated that immediately after, the solar glare from the sun impaired his vision and he could no longer see the child, and at that time the two collided. The left driver's side door area struck the bicyclist, as well as the possible front left tire. The operator and passenger stated they were uninjured. MV1 sustained little to no damage. The bicyclist was identified as CHO, Elena, and she stated she was traveling Eastbound on Waldorf Rd. approaching Canterbury Rd. She stated MV1 took a left onto Waldorf Rd. towards her and at that time she collided with the left side of MV1 and fell off her bike. The rear wheel of the bike was bent due to the collision. Elena stated that she was ok but had right hip pain. She was wearing a helmet, green jacket, blue shirt, black pants, and was riding a pink and white Diamondback bicycle (Serial # A1504026648). Her mother KIM, Kyung arrived on scene and signed a patient refusal with AMB1, and transported her to Harvard Vanguard Wellesley for an evaluation. I took photos of the scene and submitted them to the IT bureau to be attached to this report.

On Friday, August 16th 2019, at approximately 6:40pm, I, Officer Brooks, responded to Dunstan street at Washington street for a MVA pedestrian. Upon my arrival I spoke with the operator of MV1(294NZ5), who stated she was traveling southbound on Dunstan street and was attempting to pull out onto Washington street, when she made contact with a bicyclist traveling westbound on Washington street. The operator stated she never saw the bicyclist, and that she was traveling less than a mile per hour when she made contact. It should be noted that the sun was setting and the operators vision would have been impaired by the sun at the location of the accident. MV1 had a minor scuff mark on the front bumper. I spoke with the bicyclist who stated he was traveling westbound on Washington street and was crossing over Dunstan street when he was struck by MV1. He stated he was knocked off the bike, scraped his arm, and had minor back pain. The mirror on the right side of his bike was broken. The bicyclist was wearing greenish gray clothing, and was not wearing protective equipment such as a helmet. The bicyclist signed a refusal from Cataldo and was given a copy of the report number. Pictures were taken of the scene and all parties were sent on their way.

On Monday, November 4th, 2019 at approximately 1602 hours I, Officer Newton and Officer Healy responded to a motor vehicle accident at 22 Randlett Park. Upon arrival we spoke to the operator of vehicle 1. He stated the accident occurred on Watertown st. He was stopped at the traffic light on Watertown St at Walnut St when he was rear ended by vehicle 2. He attempted to pull over and speak to the operator of vehicle 2 but vehicle 2 proceeded to keep driving westbound on Watertown St. Vehicle 1 followed vehicle 2 until vehicle 2 drove to his residence at 22 Randlett Pk. When speaking to the operator of vehicle 2 he seemed confused. He could not answer questions regarding the accident until he was asked multiple times. He stated he was driving from Newtonville to his residence. He stated that he did not realize he was involved in an accident so he proceeded to drive to his residence. It should be noted that in our observations of Mr. Usher, it did not appear that he had a chemical impairment at this time, although he displayed confusion, and shaky hands as we spoke to him. Mr. Usher's mother stated that he had been released from a rehab facility recently, and has "not been himself". As a result of my investigation, Mr Usher was issued Citation#T2079939 for a violation of M.G.L.Ch.90/24-Leaving the Scene Property Damage. Based on the totality of the circumstances, an Immediate Threat Request was faxed to the RMV, see Incident number 19047444.

Sgt. McNeil, N495, and I responded to 474 Wodward St. for an auto pedestrian accident. Upon arrival the victim, Elizabeth Cantwell was laying in the street and receiving treatment from NFD and Cataldo. The operator of Ma Reg. 192HV0 was Margaret Meehan. Meehan was visibly upset and was hearing impaired, as a result Meehan was unable to communicate what happened in the accident. At this time we were able to contact Meehans daughter, Bernadette Conley 781-248-6222. Cantwell was transported to Beth Israel Hospital. Traffic Bureau was notified. I went to the BI hospital to get a statement from Cantwell. Cantwell states that her husband parked their vehicle across the street from Starbucks. Cantwell states that she stood next to her car to cross the street. Vehicle 1 came to a stop and Cantwell believed that the vehicle was going to let her cross the street (not in crosswalk). Cantwell states that when she went to cross the street vehicle 1 hit her, causing her to land on the hood and then rolled off onto the street. At this time Cantwell has a dislocated hip and head injury. I took photos of the scene. Meehans vehicle was parked in a legal spot and I transported her to NHQ where she was picked up by her daughter Bernadette Conley. I filled and immediate threat with the RMV and a copy of the report was attached with the form.

On Saturday 6/27/20 at approximatly 0700 hours while assigned to marked unit n499 I was dispatched to the parking lot of 188 Florence Street (Heathwood Rehab) for a report of a motor vehicle that backed into a pipe comng from the building. Upon arrival I observed the vehicle had backed from a parking spot at 188 Florence Street into the rear building of 160 Boylston Street striking the gas meter. I could hear and smell gas coming from the meter and immediately cleared the reporting party from the area and requested the assistance of Sgt. McNeil, Fire Dept, National Grid and Maintenence from the Avalon (160 Boylston Street). Fire arrived on scene and was able to slow the leak of the gas and I was able to speak with the reporting party identified as SANFORD Troy. Troy states that he is an employee at 188 Florence Street and got to work a little early this morning. He states that he stayed in his car awaiting the start of his shift and must have fallen asleep with his foot on the brakle and the transmission still in reverse. When he fell asleep he states his foot must have come off the brake causing him to back into the gas meter of 160 Boylston Street. The impact not only damaged the gas meter, but also did damage to the rear center of Troy's vehicle. Through my observations of Troy's demeanor and actions he did not appear to be impaired in any way. Troy stated that he did not get much sleep last night and was tired. He also stated that he did not need any medical attention Fire remained on location awaiting the arrival of National Grid. After interviewing Troy and confirming the area was secured I cleared the location without further incident

At approximately 1650HRs on Thursday, July 23, 2020, I was dispatched to the area of 1221 Centre St. for a report of a three car motor vehicle crash. Upon speaking with the operator of MV 1 (MA Pass:376Y90), he stated he was traveling NB on Centre St. during a heavy downpour and in stop-and-go traffic conditions. He stated due to the impaired visibility caused by the weather, he was unable to see MV2. When he was able to see how close they were, he applied his breaks, but MV1 continued forward on the wet pavement and made contact with the rear of MV2. Upon Speaking with the operator of MV2 (MA Pass:1GWR81), she stated she was traveling NB on Centre St., when the rear of her vehicle was contacted by MV1. This caused her vehicle to continue forward on the wet pavement, despite appliying her brakes, and make contact with the rear of MV3. Upon speaking with the operator of MV3 (MA Pass: 5DB384), she stated she was traveling NB on Centre St., when the rear of her vehicle was contacted by MV2. Everyone involved then pulled to the side of the road to wait for police assistance. Upon my arrival, I noted that the road was wet, due to a heavy downpour which had just occurred. I also noted that there was a decent amount of traffic using the road, which is typical for this area at that time of day and would likely be exacerbated by the poor weather conditions. All parties involved reported that they were not injured and I assisted them in gathering each others pertinent information, so they could begin the process with their respective insurance companies. I noted the following damage: MV 1 sustained minor damage to its front bumper on the passenger's side, by and including its tow hook. MV 2 sustained minor damage to its lift gate on the driver's side and below its rear reflector on the same side. Additionally, MV2 sustained minor damage to its front bumper on the lower driver's side and the bumper cover access panel on the same side. MV 3 sustained minor damage to its lift gate on the passenger's side and to its rear bumper, just to the left and up from its tow hitch.

Narrative:
On Tuesday, October 6, 2020, I received a call at approximately 2158 hours from Sgt. Devine of the Newton Police Traffic Bureau. Sgt. Devine stated there was a motor vehicle crash with serious injury that had just occurred at the intersection of Crafts Street and Waltham Street in Newton. Sgt. Devine stated that one of the parties involved had suffered life threatening injuries and was transported to Brigham and Women's Hospital. I responded to the scene and conducted an investigation of the crash with the assistance of Sgt. Devine and Captain Boudreau. Upon arrival it was reported to me by officers on scene that a party operating a motorized scooter on Crafts Street (E) had been struck by a 2007 Chrysler Town and Country (MA: 5EE316). The Chrysler was attempting to make a left turn from Crafts Street (W) onto Waltham Street (S). The operator of the motorized scooter, identified as Nadhir Tchikou (DOB: 11/23/1993), had suffered serious injuries to his head and left leg area and was transported to Brigham and Women's Hospital. It was reported to me that Officers Conary, Boyle, and Donovan were the first to arrive at the crash and provided life-saving care to Mr. Tchikou prior to being transported to Brigham and Women's Hospital. Officer Donovan responded to Brigham and Women's to keep us updated on Mr. Tchikou's status. The update provided to me while on scene was that Mr. Tchikou was in critical condition and in surgery. Due to the seriousness of the crash and potential fatal injury, Sgt. Devine contacted the Massachusetts State Police to assist in the investigation. Sgt. Mahoney and Trooper Paul responded as part of the Massachusetts State Police Crash Reconstruction team and conducted a supplemental investigation of the crash (MSP report 2020-CAR-000285). The location where the crash took place is Crafts Street and Waltham Street. Crafts Street and Waltham Street are public ways maintained by the City of Newton. Crafts Street and Waltham Street are municipal roadways that support two way travel for motor vehicles and bicycles with an unprotected median in the middle of both roadways to separate the direction of travel. This intersection was recently re-paved within the last month and has newly painted street markings including crosswalks, dedicated bicycle lanes, and stops lines at the intersections. Crafts Street at this intersection features two travels lanes. I observed the road surface to be dry at the time of my arrival on scene. The north lane in the westbound direction of Crafts Street permits travel straight ahead and also permits motorists/bicyclist to take a right turn onto Harding Street. The south lane in the westbound direction of Crafts Street is a dedicated left turn only lane that coincides with a left turn only arrow on the traffic signal in the middle of the intersection. The opposite side of Crafts Street permits traffic traveling in the eastbound lane to travel straight or take a left turn onto Harding Street from is a single lane travel lane. The area is well lit featuring street lights bright enough to illuminate the intersection for motorist traveling through the area. The weather at the time of the crash appeared to be clear. In the intersection where the crash took place, I observed a large debris field comprised of parts from a 2020 Tao Tao motorized scooter (49CC) and a 2007 Chrysler Town and Country (MA: 5EE316). I observed the motorized scooter (VIN: L9NTCBAG1L1010287) resting on its side facing North in the northbound lane of Crafts Street in the intersection. The motorized scooter has significant damage to its front, right, and left side and was inoperable. The area in the immediate vicinity of the motorized scooter was saturated with gasoline. The rear lights and dash of the motorized scooter were illuminated while I was observing the crash scene. The headlights were not intact due to the impact from the crash and I was unable to determine if they were functioning. The motorized scooter was removed from the roadway by Tody's Towing. The intersection where the crash took place had numerous first responders present immediately after the crash rendering aid to Mr. Tchikou prior to his transport to the hospital. It was reported by officers on scene that some of the items and debris in the intersection had been moved from their initial place of rest after the crash took place. A black MMG helmet was observed resting on the ground outside the debris field from the crash before the westbound left turn only lane of Crafts Street. It is unknown if it was moved from its initial place of rest by a first responder. I observed the strap on the helmet that would be used to secure it to the operator to be in good operable condition. I did not observe it to be defected and the securing feature of the strap was in working order. I did not observe any blood, hair, or other apparent physical evidence inside the helmet that would be consistent with the head injury reported to the operator of the motorized scooter. I observed what appeared to be fresh markings on the front mouth/nose area of the helmet. I observed a white Iphone with a red phone case lying on the roadway a short distance away from the impact area. The Iphone was on and displaying travel directions from the Iphone application Waze. The Iphone and the helmet were seized pending the status of the operator of MV2. A property/evidence form was completed and the Iphone and helmet were secured in property locker number 9. I spoke with the operator of MV1, William Moran (S34946215). Moran stated he was operating his white 2007 Chrysler Town and Country (MA: 5EE316) on Crafts Street (N) as he approached Waltham Street. Moran stated he had a green light and activated his left blinker as he approached the intersection with the intention of taking a left turn onto Waltham Street. Moran stated as he proceeded to take his left turn he saw "orange" and was unable to avoid hitting the object in front of him. Moran stated the object came out of nowhere and he swerved to the left in an attempt to avoid the crash. Moran stated after the collision he accelerated away from the impact area and came to a controlled stop on Waltham Street (S) just past the intersection. Moran stated he exited his vehicle and looked back at the intersection he had just travelled through. Moran stated as he began walking towards the intersection he observed a party laying on the roadway in the intersection. Moran stated the party was not wearing a helmet at this time. Moran also added that he believes the moped involved in the crash did not have its headlights on prior to impact. Moran stayed on scene and cooperated fully with officers during the investigation. A query of Moran's license and registration status through Newton Police Dispatched showed they are both active. I asked Moran if he was using a cell phone, GPS, or any other type of hand held device prior to the crash. Moran stated he was not. I asked Moran if he was distracted by anything either inside the vehicle or something on the roadway prior to the crash. Moran stated he was not. I asked Moran if he was currently impaired in any way that would impede his ability to operate a vehicle safely. Moran stated he was not. I asked Moran if he takes any medications or has a medical condition that would impede his ability to operate a vehicle safely. Moran stated he does not. Moran reported no injuries as a result of the crash. I observed Moran's vehicle at its controlled rest location. The vehicle was parked on Waltham Street (S) in the southbound lane just past the intersection of Crafts Street. The 2007 Chrysler Town and Country (MA: 5EE316) had substantial damage to the front passenger side bumper, hood, fender, windshield, and passenger door area. There was physical evidence of what appeared to be hair, blood, and tissue presumed to be from the operator of MV2 present in the cracked windshield, and on the passenger side door/mirror area. I observed a blue Commonwealth of Massachusetts "R" inspection sticker with a date of 05/28/2020 affixed to the windshield. Moran stated the rejection sticker was a result of an emissions failure. The inspection sticker expires in 2021. I asked Moran if there were any other mechanical issues with the vehicle. Moran stated there were no issues to his knowledge. I observed no objects present inside the vehicle during my investigation that would have potentially impeded the operators ability to see clearly through the windshield. I observed glass debris from the windshield inside the passenger and driver area of the vehicle. There was no air bag deployment inside the vehicle. Subsequent to my investigation, the vehicle was seized pending the status of the operator of MV2. The vehicle was towed from the scene by Tody's towing and secured in the Newton Police Department garage. The vehicle was escorted to the garage by Officer Donovan. A towed motor vehicle form was completed by Officer Donovan and the key for the vehicle was secured in property locker number 9. A property/evidence form was completed. Subsequent to my investigation Moran's black Alcatel cell phone was seized pending the status of the operator of MV2. A property/evidence form was completed and the phone was secured in property locker number 9. Officer Conary stated to me that prior to my arrival on scene she interviewed a witness to the crash and provided me with the following information. Joshua Hamel was operating his vehicle (W) in the right lane of Crafts Street towards Waltham Street. Hamel stated that MV1 (MA: 5EE136) was in the left turn lane on Crafts Street (W) towards Waltham. Hamel stated the scooter was headed eastbound on Crafts Street from Waltham. Hamel stated MV1 began to take a left turn and there was a head on collision with the scooter. Hamel stated he believed the traffic light at the time of the crash was green. On Friday, October 9, 2020, I spoke with Hamel via telephone. Hamel corroberated the statement he gave to Officer Conary. I asked Hamel if he believed the headlights on the motorized scooter were activated at the time of the crash. Hamel stated he believes the headlights on the motorized scooter were on as it approached the intersection but was not postive. On Wednesday, October 7, 2020, I spoke with Mr. Tchikou in his ICU room at Brigham and Women's Hospital. Tchikou's wife, Yami Carrajza, was present in the room while I spoke with him. Tchikou stated he had a significant head injury, injuries to his nasal cavity, his back, and a significant injury to his left leg. I asked Tchikou about his recollection of the crash. Tchikou stated he had just left his friend's home on Hazelhurt Avenue in Newton and was traveling home to Brighton to bring his wife dinner. Tchikou stated he was traveling Eastbound on Crafts Street as he approached the intersection of Crafts Street and Waltham Street. Tchikou stated he had a green light and as he entered the intersection a white vehicle took a turn from the opposite side of the road in front of him. Tchikou stated he does not remember anything after that. I asked Tchikou if he was wearing a helmet at the time of the crash. Tchikou stated he was. I explained to Tchikou that I recovered a white Iphone with a red case at the scene of the crash. Tchikou identified the Iphone as his. I explained that the phone was displaying travel directions when I found it and I asked Tchikou if he was distracted by his phone prior to the crash. Tchikou stated he was not distracted by the phone and the phone was in a holder on the motorized scooter. I asked Tchikou if he remembers if his headlights were activated on his motorized scooter at the time of the crash. Tchikou stated they were on. I asked Tchikou if he could produce paperwork for the motorized scooter to prove he was the owner. Tchikou stated he had recently purchased the scooter and the paperwork for it was at his residence. Tchkou's wife e-mailed me a picture of the Certificate of Origin for the motorized scooter. I asked Tchikou if he could produce a license or permit that would allow him to operate the motorized scooter. Tchikou stated he did not have a Massachusetts license or permit, but had an Algerian license at this residence. I was able to observe a picture of the license on his wife's cell phone. The written text on the license was in Arabic. Tchikou did not have a translation of the license in English and did not possess an International Driver's Permit. As a result of my investigation, Mr. William Moran while operating MV1 (MA: 5EE316) entered the travel lane of MV2 (Tchikou) as he was making a left turn onto Waltham Street from Crafts Street. MV2 had the right of way and MV1 failed to yield to MV2's right of way causing the collision to take place. Moran was advised of my investigation and issued Massachusetts Uniform Citation T0115914 via mail by request for Chapter 90, Section 8 (Fail To Yield To The Right Of Way At An Intersection.) Mr. Nadhir Tchikou operated his motor scooter at the time of the crash without a valid license/permit. Tchikou is also in violation of operation of a motor scooter after dark (prohibited). The crash occurred at 2147 hours. Tchikou will be issued Massachusetts Uniform Citation T0115915 in hand at Brigham and Women's Hospital for Chapter 90, Section 1E (Unlicensed Operation Of A Motor Scooter) and Chapter 90, Section 1E (Operation Of A Motor Scooter After Sunset - Hours Restriction.)

On Wednesday, October 21st 2020 at 0341 hours, Myself N-499 and Ofc Crowe N498 responded to 524 Parker St for a single vehicle accident into a city tree. Upon arrival, I identified Mass Vet #VT36122, 2016 Dodge Sedan, Color Black had crashed into Verizon Pole number 340/50A. The airbags had been deployed. The operator was no longer in the vehicle. The operator was sitting down on the ground nearby . He was identified as Joshua Stomberg. Stomberg admitted he had operated the vehicle. Stomberg stated he was driving south on Parker St toward the VA Hospital for his work shift. Stomberg stated he was tired and did not feel alert. Stomberg also stated there was a heavy fog that made the road difficult to see. Stomberg's vehicle veered off the side of the road and collided with the pole causing heavy front end damage. Stomberg did not show any signs of being impaired by alcohol. An inventory of Stomberg's vehicle showed no discovery of any alcohol. It should be noted that on my way to the accident scene, I observed a very heavy fog on Parker St that made it difficult to see the road in front of me. Stomberg suffered an injury to his back and was transported to the VA Hospital by Cataldo Ambulance. Tody's towed the vehicle. Stomberg was mailed MA Uniform Citation # T2013041 for the amount of $ 105 for M.G.L Ch 89/S4A Marked Lanes Violation. Pictures were taken of the accident and submitted to Newton IT Bureau.

Operator of MV1 states he just left Wegman's where he works the overnight shift and was traveling home to 223 River Street. Operator of MV1 states he was traveling westbound on Commonwealth Ave and was going to take a right hand turn onto Lowell Ave when he doesn't know what happened and struck the concrete street light pole dislodging it from its base where it laid across MV1 and part of the roadway. It should be noted while speaking with the operator of MV1 there was a slight language barrier and he couldn't elaborate much on the events leading up to the crash. MV1 sustained heavy front end damage and air bag deployment. Medics were requested for an evaluation and found no signs of fatigue, impairment or injuries and cleared with a signed patient refusal. Coviello Electric was notified for the street light pole and the Newton Fire Department capped off the exposed wires from the base and it was taped off with caution tape. Tody's towing responded and removed the vehicle from the roadway. The operator of MV1 insisted on walking home from the scene.

Operator of MV1 was traveling Westbound of Wachusett Rd. and collided with a utility pole in front of 48 Wachusett Rd. Operator of MV1 left the scene of the accident and was located nearby at the intersection of Hammondswood Rd. at Intervale Rd. Operator of MV1 stated he left the scene of the accident because he was scared. During questioning, Operator of MV1 admitted to consuming alcohol, appeared impaired and under the influence of alcohol on scene. Operator of MV1 completely left his lane of travel and was operating in a negligent manner. Operator of MV1 sustained injuries to his head and left ankle. He was then transported to Newton Wellesley Hospital for evaluation. MV1 sustained heavy front end damage due to impact. MV1 was unable to be towed due to live power lines underneath the vehicle. A voicemail was left with the traffic bureau to follow up on when it was safe to tow the vehicle involved. Operator of MV1 was mailed a criminal application, citation #T2012668 for the following offenses: 1. Ch. 90 Sec. 24/C (Leaving the scene of property damage). 2. Ch. 90 Sec. 24/E (MV, Negligent operation). 3. Ch. 90 Sec 24/J (OUI, Liquor). See incident #21016286 for further review.

The operator of MV3 stated he was stopped at the traffic light Westbound on Nahanton St. at the intersection of Winchester St. He stated he took his foot off the break to proceed straight, and due to the glare of the sun impairing his vision, he rear ended MV2. MV3 sustained minor front end damage and the operator was able to drive away from the scene. As a result of being rear ended by MV3, MV2 then rear ended MV1. The operator of MV1 left prior to my arrival after exchanging information. MV2 sustained minor damage to the front and rear end. The operator and passenger of MV2 reported no injuries. The operator of MV2 was given Massachusetts Uniform Citation # T2012850 in hand for Ch. 90 Sec. 10 Unlicensed Operation. MV2 was driven from the scene by a licensed operator. MV1 was driving EASTbound on Otis St when the operator, Philip Dare, drove into a fence belonging to 242 Otis Street. Operator showed signed of impairment and was arrested for OUI Liquor, Marked Lanes, and Negligent Operation. MV1 was towed by Tody's. MV1 sustained heavy front end and undercarriage damage. No injuries were reported by Dare. Medics and Fire arrived at the scene and Fire made the scene safe with yellow caution tape and cleared debris. Driver of vehicle#1 stated he was driving northbound on Hancock St when he fell asleep behind the wheel.
