On 01/03/19 at 07:26 hours, I responded to a two car motor vehicle accident at Boylston Street and Ellis Street with one reported minor injury (headache). Operator of MV # 1 states she was exiting the Boylston Street (RT 9EB) off ramp at Ellis Street when she states that she came to a stop for the stop sign. She then inched out and thought it was clear to proceed straight through and struck MV # 2 on both passenger side doors. Operator also stated that she did not think traffic would be allowed to come from the left of her. She was advised that there is a sign below the stop sign that states that the oncoming traffic does not stop. Operator was issued MA Uniform Citation T1441046 for Ch. 89/9 Stop Sign Violation. MV # 1 sustained front end damage to the entire front but was still driveable and the operator reported no injuries. Operator of MV # 2 states she was operating straight through the Boylston Street overpass (SB) on to Ellis Street when she states that MV # 1 never stopped for the stop sign. MV # 1 then proceeded out and struck her vehicle on the passenger side. MV # 2 sustained moderate damage to both passenger side doors and was towed by Tody's towing. The operator complained of a headache but signed a patient refusal with Cataldo Ambulance One.

OPERATOR OF MV 1 STATED HE WAS TRAVELING NORTHBOUND ON PEARL ST AND AS HE APPROACHED WASHBURN STREET, MV 2 ENTERED THE INTERSECTION ATTEMPTING TO CROSS PEARL ST AND CONTINUE STRAIGHT ON WASHBURN STREET. MV 1 AND MV 2 COLLIDED WHICH RESULTED IN MV 2 STRIKING MV 3 WHICH WAS ALSO STOPPED AT THE OTHER STOP SIGN AT WASHBURN AND PEARL ST. MV 1 SUSTAINED FRONT END DAMAGE AND NO INJURIES REPORTED. MV 2 SUSTAINED MODERATE SIDE DAMAGE AND BOTH OCCUPANTS OF THE VEHICLE SIGNED PATIENT REFUSALS FROM MEDICS. MV 3 SUSTAINED FRONT END DAMAGE AND NO INJURIES REPORTED. OPERATOR OF MV 2 WAS ISSUED MA UNIFORM CITATION T1441116 FOR CH. 89 S. 9 FTC STOP SIGN.

Operator of MV#1 stated he was stopped southbound on Walnut St at Otis St waiting at the red light when he was rear ended by MV#2. MV#2 stated he was travelling southbound on Walnut St when he witnessed MV#1 turn left from Claflin Place onto Walnut St. MV#2 stated he rear ended MV#1. MV#2 admitted fault. MV#1 had no damage. MV#2 had moderate damage to the front of the vehicle. Both vehicles driven from the scene.

On 01/15/2019, at 1919 hrs, I responded to the intersection of Washington St and Centre St for a two car MVA. Upon arrival, I observed heavy damage to the front end of MV1 and moderate damage to the rear end of MV2. The operator of MV2 complained of head/neck injuries and was transported to Newton Wellesley Hospital by Cataldo for an evaluation. The operator of MV1 stated she was attempting to merge onto Washington St from Centre St when she rear ended MV2. The operator of MV2 stated when she slowed to merge onto Washington St from Centre St she was rear ended by MV1. Both vehicles were towed by Tody's. Towed MV inventory forms were filled out and filed.

Vehicle #1 stated she was stopped at the stop sign at the intersection of Goddard St. and Winchester St. when she had the right of way, she proceeded to drive through the intersection and was struck in the left driver side door by Vehicle #2. Vehicle #2 stated she believed she had stopped at the stop sign and entered the intersection when Vehicle #1 drove in front of her and she was unable to stop, striking Vehicle #1 in the side with the front of her vehicle. Vehicle #1 had moderate damage to the front left corner and driver side, however it was able to be driven away and did so. Vehicle #2 had moderate damage to the front of the vehicle and was unable to be driven and towed by Tody's Towing. Both drivers were evaluated on scene by medics and stated they were uninjured and declined any further medical treatment.

On 1/19/19 I responded to 25 Normandy Rd for a report of a past hit and run. I spoke with the Operator of MV#1 who stated the accident had taken place in front of Tom's Pizza, 423 Lexington St, at approximately 1830 hrs. Operator of MV#1 stated she parked her vehicle in a parking spot directly in front of the restaurant and went inside. When she returned approximately 5 minutes later she realized her vehicle had been rear ended. Two witnesses provided her with information about the accident and the plate number. (See witness narratives.) MV#1 suffered moderate damage to the rear and was driven from the scene. Waltham PD responded to the address listed for MV#2 and were not able to locate the vehicle. At approximately 2000 hrs, I was contacted at the police station by the owner of MV#2's wife. After further investigation (see incident report #19002425) it was determined that the operator of MV#2 at the time of the accident was the vehicle owner's brother. I spoke with Operator of MV#2 on the phone and he stated that the road was icy and slippery due to snow and he crashed into the back of MV#1. He stated he was nervous and thought he had only hit the sidewalk curb. Operator of MV#2 was issued citation T1440626 by mail for 90/24C Leaving the Scene of Property Damage, 90/10A Unlicensed Operation and 90/24 Use of MV Without Authority. He stated the vehicle suffered minor damage to the front.

Vehicle #1 stated she was traveling south bound on Grant Ave. Vehicle #1 stated that in the area where Grant Ave. bends to the left the roadway is dark and she had trouble seeing the road. Vehicle #1 stated at the last second she saw the road bending left and attempted to turn but skid on the snow and struck a small boulder in the front lawn of 173 Grant Ave. moving the rock a few feet. Vehicle #1 then skidded and came to a stop in the middle of the lawn of 173 Grant Ave. Vehicle # 1 then pulled off the lawn and to the side of the road. The owner of the property of 173 Grant Ave. was also present and was notified of the accident. Vehicle #1 had moderate damage to the front of the vehicle but was still able to be driven and was driven home. Both the operator and passenger in Vehicle #1 stated they were uninjured in the accident and refused any medical treatment.

On 01/22/19 I responded to a three vehicle crash on Needham St. in front on #241- Marshalls Plaza. The operator of veh #1 stated she was driving due West on Needham St. when she looked right at a car waiting to pull out of the parking lot. When she looked back to Needham St., the traffic was stopped and she hit her brakes and snow slid covered her windshield. Operator of veh #1 never saw the impact, but veh #1 struck stopped veh #2 which was pushed into stopped veh #3. Moderate damage to veh #1 and 2 but no damage at all to veh #3. Operator of veh #2 said she was going "2-3 MPH " when she was pushed into veh #3. Operator of veh #3 claimed she was dizzy from the accident and signed a patient refusal from Ambulance #1. Engine #7 was also on scene assisting. Operator of veh #1 was cited in hand Mass citation #T1441211 for Ch 90/ Sec 13- failure to remove snow from vehicle . All vehicles were driven away from the scene.

On Friday 01/25/2019 at appx. 1:30 PM while on patrol in N 491 I was dispatched to 74 Bowers St for a report of a past hit and run. On arrival I spoke to Jamie Juskiewicz who stated she parked her vehicle legally around 11:30 AM. When she came back to her vehicle around 1:30 PM she realized that an unknown person struck her vehicle causing moderate damage to the drivers side and bumper. I did not observe any surveilance cameras and there were no witnesses. The other operator did not leave a note.

OPERATOR OF MV 1 STATED SHE WAS TRAVELING NORTHBOUND ON CRAFTS ST WHEN SUDDENLY MV 2 FAILED TO STOP AT THE STOP SIGN LOCATED AT WALNUT ST AND CRAFTS ST, STRIKING HER VEHICLE ON THE FRONT DRIVERS SIDE CAUSING MODERATE DAMAGE. OPERATOR AND PASSENGER OF MV 1 STATED THAT MV 2 NEVER STOPPED TO SEE IF THEY WERE INJURED OR EXCHANGE INFORMATION AND CONTINUED ON CRAFTS ST EVENTUALLY TURNING RIGHT ONTO CALIFORNIA ST. MV 2 COULD ONLY BE DESCRIBED AS A WHITE COMCAST VAN WITH A LADDER ON THE ROOF. THERE WAS ALSO A WITNESS TO THE HIT AND RUN ACCIDENT IDENTIFIED AS RYAN WALSH WHO STATED HE WAS TRAVELING SOUTHBOUND ON CRAFTS ST WHEN MV 2 FAILED TO STOP AT THE STOP SIGN, CAUSING HIM TO SWERVE OUT OF THE WAY TO AVOID A COLLISION. WALSH ALSO STATED THAT THE VEHICLE WHO BLEW THE STOP SIGN AND CAUSED THE ACCIDENT WAS A COMCAST VEHICLE. I CONTACTED COMCAST SUPERVISOR BRIAN DELANO (617 756 6293) WHO WORKS OUT OF THE NATICK OFFICE AND IS IN CHARGE OF EMPLOYEES IN NEWTON. HE STATED HE LOOKED AT GPS RECORDS AND STATED ONLY ONE OF HIS EMPLOYEES WAS IN THE AREA AT THE TIME OF THE CRASH AND HE WAS STATIONARY AT ADDRESS ON MELVILLE AVE. I ALSO CALLED JOHN MCGUINNESS WHO IS THE SUPERVISOR FOR COMCAST WORKING IN WESTON, WALTHAM AND WATERTOWN AREAS. I LEFT A VOICEMAIL FOR HIM TO CALL ME BACK. I RECEIVED A CALL BACK FROM JOHN MCGUINNESS WHO RELAYED TO ME THAT HE SENT A MESSAGE OUT TO ALL OF HIS EMPLOYEES INQUIRING ABOUT THE ACCIDENT IN QUESTION AND ONE OF HIS DRIVERS CALLED HIM AND TOLD HIM THAT HE WAS THE PERSON INVOLVED IN THE ACCIDENT. I MET WITH HIM WHO WAS IDENTIFIED AS CAMDEN DUQUETTE AND HE STATED THAT HE STOPPED AT THE STOP SIGN, PROCEEDED FORWARD AND DIDN'T SEE A TRUCK COMING ON HIS LEFT. HE STATED HE WAS ABLE TO AVOID THE FIRST CAR AND DIDN'T REALIZE HE STRUCK ANOTHER VEHICLE (STATED HE THOUGHT HE STRUCK THE CURB).THERE WAS DAMAGE ON HIS PASSENGER SIDE CONSISTENT WITH STRIKING A VEHICLE AND NOT THE CURB. AS A RESULT OF THE INVESTIGATION HE WAS ISSUED MA UNIFORM CITATION T1441118 FOR CH .90 S. 24 LEAVING SCENE OF AN ACCIDENT (PROPERTY DAMAGE) AND CH. 89 S. 9 FTC STOP SIGN.

On 2/9/19 I responded to 54 Ash St and met with Ms. Zardar Hovhannisyan in regards to a past hit & run to her vehicle. Hovhannisyan parked MA reg 1EC474 on Ash St (a northbound public one way street) just prior to Commonwealth Ave around 0800 hrs this morning. When she returned around 1000 hrs, she noticed moderate damage to the front left area of the vehicle. There was minor debris on the ground consistent with the damage I viewed. It should be noted that there are parking stalls parallel to where the vehicle was parked, with several residents of Norumbega Gardens coming and going quite frequently. It is not believed that there are cameras facing the specific area where the collision occurred. Hovhannisyan was advised to contact her insurance company.

OPER OF MV#1 STATED THAT SHE WAS STOPPED (EB) ON GIBBS ST ST, AT THE STOP SIGN, WHEN MV#2 EMERGED FROM SUMNER ST, STRIKING HER VEHICLE. OPER OF MV#2 STATED THAT HE WAS TRAVELLING (NB) ON SUMNER ST, WHEN MV#1 WENT THROUGH THE STOP SIGN ON GIBBS ST, STRIKING HIS VEHICLE. MV#1 WAS TOWED WITH MODERATE FRONT DAMAGE. MV#2 SUSTAINED A FRONT RIGHT FLAT TIRE AND MINOR FRNDER DAMAGE.

OPERATOR OF MV 1 STATED HE WAS TRAVELING SOUTH ON CRAFTS ST AND AS HE WAS PASSING THE ARS RESTORATION SPECIALISTS PARKING LOT WHICH WAS ON HIS RIGHT, HE WAS STRUCK BY MV 2 WHO WAS BACKING OUT OF THE PARKING LOT AND INTO TRAFFIC ON CRAFTS ST. MV 2 WAS PLOWING THE SNOW IN THE PARKING LOT AND AS HE WAS BACKING UP, HIS PASSENGER SIDE REAR BUMPER STRUCK MV 1 ON THE DRIVERS SIDE. BOTH VEHICLES HAD MODERATE DAMAGE AND NO INJURIES WERE REPORTED. I TOOK SEVERAL PHOTOS OF THE ACCIDENT AND PUT THEM IN IT'S BOX TO BE DOWNLOADED.

Mv#1 operator stated she was at a complete stop in traffic on Centre St N/B opposite #1269. #1 stated she was suddenly rear ended by Mv#2. No injuries reported. #1 appeared to sustain moderate damage and was driven away from the scene. #2 operator stated she was travelling straight ahead on Centre St N/B passing through the Beacon St intersection. #2 stated she was distracted by watching a pedestrian nearby and as a result rear ended #1 ahead of her. No injuries. #1 sustained heavy front damage and was towed by AAA towing.

The operator of MV1 was traveling Eastbound on Beacon St. when she stopped at a crosswalk for a pedestrian to cross and was struck in the rear by MV2. The operator of MV2 was traveling Eastbound on Beacon St. when MV1 came to a stop and she struck MV1 in the rear. She stated she didn't have enough time to react but pressed her brakes down all the way until they locked up and she ultimately struck the vehicle. All parties reported no injuries. MV1 sustained minor damage and was able to be driven away from the scene safely. MV2 sustained moderate/heavy front end damage and was towed from the scene by Tody's.

On 3/1/19 while working N493, myself and N492 Officer Selig were sent to the area of 1354 Washington St for a motor vehicle accident with reported injuries. On arrival, I spoke to Matthew Digregorio DOB7/2/74 ( hereby referred to as operator #1) who was standing on the sidewalk next to MA reg 134TR9 (vehicle #1 ). Vehicle #1 had significant damage to the front and right quarter panels. It was straddled between the travel lanes and did not have the keys in the ignition. It was immediately apparent that operator #1 suffered an injury to his head, as there was a moderate amount of blood and small pieces of glass stuck in his forehead area. I observed operator #1 to be unsteady on his feet, have bloodshot eyes and slurred speech. I could detect a moderate odor of alchoholic beverage coming from operator #1 as I was approaching him, which was apparent from about an arms length away (it should be noted that medics were already en route to the scene at this time). Prior to the medics arrival, I asked operator #1 for a brief synopsis of what transpired. Operator #1 stated that he had just left Paddy's Pub and was driving westbound on Hunter St (which is approximately 1/8 mile from our location) when he collided head on with vehicle #2. Vehicle #2 was not immediately visible upon police arrival, however was pointed out as being located at the intersection of Hunter and Putnam St. I asked operator #1 where he was heading, as Hunter St is a back road not directly in the vicinity of Paddy's. Operator #1 could not answer the question and also remained silent when I asked him how much alcohol he had consumed this evening. At this time the medics arrived on scene. I asked operator #1 for the car keys to reliquish to the tow truck operator, which is standard operating practice. Operator #1 reached into his pocket and then provided me with the keys. Due to concern over operator #1's obvious head trauma, he was then transported to NWH to be evaluated while the investigation continued on scene. Two other parties approached myself and Officer Selig. One was the operator of vehicle #2, MA reg 1RD999. Operator #2 is identified as Luke Ferdinands DOB 5/11/80. The other party who approached is identified as Douglas Dasilva-Pimenta DOB 6/2/86 (Dasilva-Pimenta will now be referred to as operator #3). Operator #2 states that he was taking a right onto Hunter St from Putnam St, when he observed vehicle #1 on the wrong side of the road approaching him head on. Operator #2 states vehicle #1 then collided head on with him, took a right onto Putnam St, and continued onto the Washington St overpass near 1354 Washington St. Operator #3 states he was inside of #5 Hunter St when he heard a loud crash coming from outside. He looked out the window and saw vehicle #1 traveling at a high rate of speed on Hunter St towards Putnam St. Operator #3 observed damage to the right side of his parked vehicle (now referred to as vehicle #3) MA reg 7CAK80. He immediately got in his vehicle and proceeded to follow vehicle #1, which then crashed into vehicle #2 head on. Operator #1 did not stop until it was obvious that operator #3 was following close behind. This ultimately landed vehicle #1 at it's final location near 1354 Washington St. Vehicle #1 and vehicle #2 were not driveable and were towed to Tody's. It should be noted that there was a passenger in vehicle #2 at the time of the collision. Airbags were deployed and medics checked out both operator and passenger. Both declined transport. The passenger is identified as Elizabeth Ferdinands DOB 5/22/53.Ferdinands' version of events coincide with that of operator #2. Upon speaking with Officer Selig it was learned that operator #1 dropped his cell phone several times as he was holding it on scene. Both Officer Selig and operators #2 and #3 observed this. Operators #2 and #3 both expressed concern over the obvious odor of alcohol eminating from operator #1, and the fact that he was unsteady on his feet. I responded to NWH to follow up with operator #1 however he did not wish to speak further. He did express concern over the well being of all parties involved. Operator #1 was issued in hand the following citations: T1268426 - 90/24 OUI Alcohol, 90/24 Leaving the Scene (property damage, vehicle #2), and 90/24 Leaving the Scene (property damage, vehicle #3). T1268425 - 89/4a Marked Lanes violation. T1269216 - 90/24 Negligent Operation of a M/V (mailed after consultation with superiors) All streets mentioned are public ways in the City of Newton. 1354 Washington St will be the address used on this report and subsequent citations due to multiple locations being involved. Citations and paperwork have been placed in the prosecutor's mailbox. An immediate threat form has been filled out and faxed to Mass DOT. *Attention Police Prosecutor for blood work subpoena, if applicable*

The operator of MV#1 stated he was travelling northbound on Florence St when he was struck from behind by MV#2. MV#1 sustained moderate damages to its rear end. There were no reported injuries to the operator of MV#1. The operator of MV#2 stated he was travelling southbound on Florence St and could not stop in time (due to the snowy road conditions) and struck MV#1. MV#2 sustained minor front end damages. There were no reported injuries to the operator of MV#2.

Operator of MV1 was traveling southbound on Centre Street. She stated there was heavy stop and go traffic. MV2 struck the rear of her vehicle when she came to a complete stop as she passed 785 Centre Street. Operator of MV2 said he did not how close he was to MV1 and rear ended the vehicle. Both parties were uninjured and declined medical attention. MV1 had minor rear end damage and MV2 had moderate front end damage. Both vehicles were able to be driven from the scene.

On 03/09/2019, at 0117 hrs, I responded to Washington St. at Church St. for a two car MVA. Upon arrival, I observed both vehicles facing the same direction in the outer eastbound lane of Washington St. MV1 had moderate damage to the front end and MV2 had heavy damage to the rear end. The operator of MV1 stated that as he was approaching MV2 he attempted to slow down but the vehicle accelerated, rear ending MV2. The operator of MV1 stated that moments after the traffic light turned from red to green, MV2 rear ended her. The passenger in MV2 sustained a bruised lip but all involved parties signed patient refusals with Cataldo. Tody's towed MV2.

OPERATOR OF MV 1 STATED HE WAS STOPPED ON WILTSHIRE RD AT THE INTERSECTION OF JACKSON RD. BEFORE TURNING RIGHT ONTO JACKSON RD HE LOOKED LEFT AND DID NOT SEE ANY VEHICLES COMING SO HE BEGAN TO TURN RIGHT AND AS HE TURNED HE SAW MV 2 TRAVELING SOUTHBOUND ON JACKSON RD. OPERATOR OF MV 1 BELIEVES MV 2 WAS PARKED IN FRONT OF THEIR HOUSE AND BEGAN TRAVELING SOUTH AT A HIGH RATE OF SPEED JUST AFTER HE LOOKED LEFT. AS HE TURNED, THE FRONT DRIVERS SIDE OF MV 1 STRUCK THE PASSENGER SIDE OF MV 2. MODERATE DAMAGE SUSTAINED TO MV 1 AND NO INJURIES WERE REPORTED. OPERATOR OF MV 2 STATED HE WAS TRAVELING SOUTHBOUND ON JACKSON RD AND WAS STRUCK BY MV 1, WHO WAS TURNING RIGHT FROM WILTSHITRE RD ONTO JACKSON RD. MODERATE DAMAGE SUSTAINED TO THE PASSENGER SIDE OF MV 2 AND NO INJURIES REPORTED.

On Wednesday, April 10, 2019 while assigned to Traffic unit N525, I responded to the intersection of Centre Street and Wesley Street, Newton for a report of a motor vehicle accident involving a bicycle. It was also reported that the parties involved were arguing with each other. The weather at the time of the accident was cloudy and the road surface was dry. Centre Street and Wesley Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Abdulaziz Stewart (S04075169). Stewart stated he was operating his 2017 Toyota Camry (MA: 669FM3) at the time of the accident. Stewart stated he came to a stop on Wesley Street (E) at Centre Street and was attempting to take a left turn onto Centre Street. Stewart stated there were no vehicles in the Southbound lane of Centre Street and there was a vehicle stopped in the Northbound lane of Centre Street motioning to him to take his left turn. Stewart stated as he began to start his left turn, a bicyclist crashed into the front passenger area of his vehicle. Stewart stated the bicycle had been traveling Northbound on Centre Street in the Southbound Lane. Stewart stated after the collision the bicyclist was yelling at him and called him a "Nigger". Stewart and his occupant, Miriam Stewart, reported no injuries. Miriam is a student at Bigelow Middle School. I observed minor damage to the front passenger quarter panel area of MV1. I spoke with the bicyclist involved in the accident, Timothy Murtagh (S49233464). Murtagh stated he was traveling Northbound on Centre Street on his Cannondale bicycle in the Southbound lane. Murtagh was not wearing any safety equipment while operating his bicycle (helmet) and was wearing a dark red jacket. Murtagh stated as he approached Wesley Street, MV1 "jammed on the gas" and hit him in the roadway. Murtagh stated he then fell to the ground and landed in the middle of the street. While speaking with Murtagh I told him it was reported to me by the operator of MV1 that he used a racial slur during their interaction. Murtagh acknowledged that he did and stated, "I called him a nigger". I observed moderate damage to the front wheel/rim of Murtagh's bicycle. Murtagh reported no injuries. Murtagh was advised and issued Massachusetts Uniform Citation T1443441 for Chapter 85, Section 11B (Fail to ride on the correct side of the roadway). Photos were taken at the scene and submitted to the IT Bureau. Due to the language used by Murtagh during his exchange with Stewart, Officer Rosenbaum of the Community Services Bureau was made aware of the incident for follow up.

Operator 1 states she was traveling Eastbound on Hammond St when vehicle 2 pulled out of Hammondswood Rd and struck vehicle 1. Operator 2 states she stopped at the stop sign on Hammondswood Rd. Operator two made sure both ways were clear and then pulled out of Hammondswood. Vehicle 1 was traveling at a high rate of speed and struck vehicle 2. Both vehicles sustained moderate damage and vehicle 2 was towed by Tody's. Operator 1 complained of back pain; but, refused medical attention. Operator 2 was not injured. Operator 2's license was suspended and she was issued citation #T0646633 for 89/9 failure to comply stop sign, 90/23/D operating on a suspended license, and 90/23 Unregistered M/V.

On Thursday, April 11, 2019 while assigned to Traffic unit N525, I responded to the area of 1302 Washington Street, Newton for a report of a motor vehicle accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Washington Street is a public way maintained by the City of Newton. The operator of MV1, Jacqueline Dupre (S29256883), stated she was parked in front of 1302 Washington Street (E) in her 2018 Honda CRV (MA: 5054RF). Dupre stated she was attempting to exit her street parking spot and did not see any vehicles traveling towards her as she entered Washington Street. Dupre stated MV2's passenger side hit her front driver side area as she was attempting to enter the lane. I observed minor damage to the front driver side quarter panel/headlight area. Dupre reported no injuries at this time. The operator of MV2, Donnell Stoute (S53698210), stated he was operating a 2017 Porsche Macan (MA: 7ZC447) East on Washington Street. Stoute stated as he was traveling by 1302 Washington Street, MV1 pulled out of her parking spot and hit the passenger side of the vehicle he was operating. I observed moderate damage to the passenger side of MV2. Stoute reported no injuries at this time.

The operator of veh #1 was driving due South on Ellis St. when veh #2 drove from his right without stopping for the stop sign. Veh#1 collided with veh#2 causing slight damage to veh #1 bumper. The operator of veh#2 stated he was driving off the Rt 9 (Boylston St.) East off ramp when he "spaced out and didn't fully stop" for the stop sign at the end of the ramp and veh #2 collided with veh#1. Moderate to heavy damage to veh#2 drivers side. Operator of veh#2 issued Mass. Citation #T1444223 in hand for Ch89/ Sec9 Stop Sign violation.

Mv#1 operator was travelling on Pleasant St E/B (one way). Near the end of Pleasant St, #1 reversed backing down Pleasant St the wrong way in order to reach the Pelham St Municipal Parking Lot driveway ( instead of continuing onto Centre St and turning onto Pelham St directly to reach the parking lot). While backing the wrong way, #1 veered into Mv#2 that was legally parked in a space on Pleasant St. #2 was sitting in her MV, not operating, at the time of the collision. #1 passenger side rear struck #2 driver's side front. Both MVs sustained moderate damage. No injuries. #1 was issued citation #T1443634 for CO Ch19/Sec 85 one way violation and Ch90/Sec 11 no license in possession.

The operator of vehicle 1 stated she was traveling West on Mill Street when vehicle 2 went through a stop sign, she was unable to stop and hit vehicle 2 in the front passenger side panel and door. This caused moderate damage to the front of her vehicle. She was evaluated by Cataldo and signed a medical refusal and was able to drive her car away. The Operator of vehicle 2 stated she was traveling North on Bullough Park, she came to a stop at the stop sign and proceeded to cross over Mill Street towards Upland Road. She was then hit on the passenger side by vehicle 1. She stated she did not see vehicle 1 at all. As a result of the impact, vehicle 2 sustained extensive damage to the passenger side and had side air bag deployment. The passenger in vehicle 2 stated he did not see anything until impact. The Occupants of vehicle 2 stated they had no injuries or pain and were taken away from the scene by parent Niels Steenstrup. As a result of the accident vehicle 2 was towed by Tody's by Billy.

On Wednesday, April 24, 2019 while assigned to unit N525, I responded to the area of Centre Street and Richmond Street, Newton for a 3 car motor vehicle accident. The weather at the time of the accident was clear. The road surface was dry. Centre Street and Richmond Street are both public ways maintained by the City of Newton. The operator of MV1, Irina Gorolevici (S18897891) stated she was stopped in traffic on Centre Street (N) at a red light while operating her 2017 Toyota Highlander (MA: 8JY370). Gorolevici stated while stopped in traffic MV2 crashed into the rear of her vehicle. I observed damage to the rear bumper area of MV1. Gorolevici reported no injuries and signed a patient refusal with Newton Medics. The operator of MV2, Wiranya Thatwisai Partosan (S27096641) stated she was stopped in traffic on Centre Street (N) at a red light while operating her 2016 Honda Civic (MA: 5VA815). Thatwisai Partosan stated while stopped in traffic MV3 crashed into the rear of her vehicle. The force from MV3 crashing into the rear of her vehicle caused MV2's front bumper area to crash into the rear bumper area of MV1. I observed moderate to the front and rear bumper areas of MV2. The vehicle was removed from the roadway by Tody's towing. Thatwisai Partosan reported no injuries and signed a patient refusal with Newton Medics. The operator of MV3, Sofija Garcevic (S74039706) stated she was traveling North on Centre Street towards Cabot Street. Garcevic stated she looked up and observed MV2 in front of her. Garcevic stated she did not have time to stop and crashed into the rear of MV2. I observed moderate to the front bumper area of MV3. The vehicle was removed from the roadway by Tody's towing. Garcevic reported no injuries and signed a patient refusal with Newton Medics. As a result of the accident, Garcevic was provided with Massachusetts Uniform Citation T1443443 in hand for Newton City Ordinance Chapter 19, Section 75 (Fail to use care, stopping).

Mv#1 operator was travelling on the Boylston St W/B off ramp at the Chestnut St intersection. #1 front seat passenger spoke for #1 operator who spoke limited English. #1 passenger stated #1 operator came to a stop at the posted stop sign, prior to attempting to cut straight across the intersection further W/B on the ramp. After stopping, #1 proceeded straight ahead into the intersection and collided with Mv#2 which was travelling straight ahead on Chestnut St. #1 driver's side front struck #2 passenger side front. No injuries. #1 appeared to sustain moderate damage and did not require towing. #2 stated he was travelling straight ahead on Chestnut St N/B (right of way). #2 stated he was travelling at a low speed. #2 stated when he arrived at the off ramp intersection, #1 drove out straight ahead from his right and crashed into his passenger side front. No injuries. #2 appeared to sustain moderate damage, was leaking fluids and required towing. Tody's towed #2 to their storage yard without incident. Witness was walking on the Chestnut St sidewalk. Witness stated he observed #1 edging out (stop and go fashion) into the intersection before he observed #1 fully proceed forward colliding with #2.

Operator 1 of MV 1 was traveling on Hammond St when MV 1 was struck on the drivers side by MV 2. No injuries to Operator 1 and moderate damage to MV1. Operator 2 of MV 2 was stopped at Hammondswood Rd and looked right, then left and did not look right and proceeded forward and did not see MV 1. Moderate damage to MV 2 and no injuries to Operator 2 or the passenger. Both vehicles were able to be driven from the scene.

Mv#1 was stopped at the center intersection red light on Grant Ave S/B at Commonwealth Ave. (intersection has two lights, other light prior to carriage rd). Mv#2 was travelling on Grant Ave S/B and rear ended #1. #1 operator stated, after the crash, #2 operator told him the brakes failed. #2 operator told him he would back up, pull over and get out of the way so information could be exchanged. At that time, #2 drove away on Commonwealth Ave W/B. Witness stated he observed the crash while he was at the red light from the opposite direction on Grant Ave N/B. Witness observed #2 rear end #1 and observed #2 drive away. Witness observed and wrote down license plate of #2, MA reg 8SVH20. #1 sustained moderate damage to the driver's side rear that did not require a tow. #1 stated he could feel possible knee and side (oblique) pain. Medics arrived for an evaluation. #1 signed a patient refusal. #2 operator (no injury) was later located on Newtonville Ave (incident report #19017194). #2 sustained heavy front end damage with airbag deployment. Mv#2 was towed from 148 Sumner St by Tody's. #2 issued citation for 90/24 leaving the scene of property damage and 90/10 unlicensed operation.

Vehicle 1 was travelling southbound on Nahanton St. The operator of vehicle 1 lost control to the vehicle, due to the wet road conditions, and struck the telephone pole (#6/4) in front of 23 Nahanton St. There was no damage to the telephone pole. There was moderate damage to the front of vehicle 1. I observed air bag deployment to the front and passenger side of the vehicle. No one was injured, and the vehicle was towed by Tody's to their lot.

On Sunday, May 5th, 2019 at approximately 1315 hours while working the City of Newton detail for the Walk for Hunger I was standing at my assigned post at the intersection of Centre Street and Willow Street. At the time, the road surface was dry and the weather was cloudy. Traffic was particulary heavy as there were several streets in the area still blocked off for the Walk for Hunger and there were participants still walking the route in the area. My attention was drawn to the intersection of Centre Street and Bowen Street as there was a motor vehicle travelling southbound on Centre Street with their left turn signal on attempting to take a left turn on to Gibbs Street. Gibbs Street was temporarily blocked to non-residential motorists as there were several pedestrians in the road crossing over Gibbs Street. As the motor vehicle attempted to turn, there were several pedestrians in the crosswalk crossing over Gibbs Street. The vehicle had stopped halfway into the northbound lane on Centre Street blocking on coming traffic. The operator appeared to realize Gibbs Street was blocked and attemped to turn the vehicle back into the southbound lane to continue southbound on Centre Street. As the vehicle turned back into the southbound traffic lane, the vehicles rear end collided with Officer Richard Clements, who was operating NPD motorcycle unit 533. Officer Clements was traveling southbound on Centre Street and was passing the vehicles in the southbound traffic lane which were currently all at a standstill. Officer Clements had his emergency lights activated as he was on his way to remove traffic cones from the street to open up all the traffic lanes along the event route. Officer Clements was working the City of Newton detail for the Walk for Hunger assigned to the motorcycle to assist with traffic. Officer Clements front wheel, left side of the motorcycle and left leg collided with the rear end of the motor vehicle. The impact of the crash caused him to fall off the left side of the motorcycle and land on the street face first. At this time I immediately ran from my post to Officer Clements location and radioed dispatch that a motorcycle officer was down and that I needed assistance. As I approached Officer Clements he was face down, unconcious and breathing. I was able to roll Officer Clements into the recovery position and at this time Officer Clements regained conciousness and was able to speak and stated " What happened? What was that lady doing she turned right into me". I advised Officer Clements that he had just been involved in an accident and to stay still and that help was on the way. The operator of the motor vehicle identified as Helen Leibner (S52717988) stopped her vehicle immediatley after the crash and got out of her vehicle to offer her assistance. I advised Helen and several other witnesses and bystanders who were now crowding around Officer Clements and I to back away from the scene and to not move any of the vehicles involved. Officer Selig arrived on scene and was able to help crowd control until further help arrived. Helen and all witnesses were advised to stand by for police to take their statements. While tending to Officer Clements he was disoriented and made several attempts to move his body to which I had to physically hold him still to prevent possible further injuries. While speaking with Officer Clements I noticed blood coming from the inside of his mouth. Officer Clements was in a noticeable amount of pain. He complained of head, back and left leg pain. Officer Clements was wearing a yellow ANSI certified Newton Police reflective jacket and a Newton Police issued motorcycle helmet. Once the paramedics and fire personnel arrived I was able to speak to the operator of the motor vehicle and several other witnesses. Helen stated she was traveling southbound on Centre Street operating her blue 2014 Audi A4 (MA REG 213BC9). Helen states she was attempting to take a left turn onto Gibbs Street and activated her left turn signal. Helen states she had stopped halfway into the northbound lane as several pedestrians were crossing Gibbs Street in the crosswalk at the time. Helen states after the pedestrians cleared she saw the traffic cones blocking the street and realized the street was closed. Helen states she turned her vehicle back into the southbound traffic lane and did not see Officer Clements or the motorcycle until impact. Helen was not injured and her motor vehicle sustained moderate rear end damage. Her motor vehicle was able to drive away from the scene. Witness Adron Gordon (S90357161) stated he was participating in the Walk for Hunger and had just crossed over Gibbs Street when the crash occured. He stated he witnessed the Officer on the motorcycle with the emergency lights on traveling southbound on Centre Street behind the motor vehicle. The motor vehicle stopped in traffic attempting to turn left onto Gibbs Street. He states then the vehicle made a slight turn back into the southbound traffic lane and the motorcycle collided with the rear end of the motor vehicle. He states the Officer then fell off the motorcycle landing in the middle of Centre Street. Witness Elizabeth McCarthy (S00242346) and Katherine Regan (S43486601) stated they both were in the same vehicle together stopped in traffic in the northbound lane of Centre Street just before the set of lights at Centre Street and Bowen Street. Both witnesses stated they do not remember if the driver of the motor vehicle had the turn signal on but they did see her attempt to turn onto Gibbs Street. They stated they also saw the motorcycle with emergency lights activated travel southbound on Centre Street approaching behind the vehicle. Witnesses state the motor vehicle then suddenly turned back into the southbound lane and the motorcycle collided with the passenger side tailight of the motor vehicle. Both witnesses state the motorcycle attemped to turn away from the vehicle to avoid the crash but was unable to do so. Witness Nina Wang (S39460985) stated she was parked on the southbound lane of Centre Street just prior to the crash scene. Nina states she was in her City of Newton DPW marked vehicle with the hazards on as she was working to assist with traffic safety for the Walk for Hunger. Nina states she witnessed the motorcycle travel southbound on Centre Street and pass the vehicles stopped in the traffic lane. Nina states the motorcycle passed the traffic using the left side and had the emergency lights activated. Nina states the motorcycle was just passing through the green light at Centre Street and Bowen Street when the motor vehicle that was stopped at the lights attempting to take a left turn onto Gibbs Street. Nina states the vehicle then abruptly jerked her car to the right and collided with the motorcycles left side. Nina states she heard a loud crash and witnessed the Officer fall off the motorcycle and land face first onto the street. The motorcycle's final resting place was on its left side approximately two feet away from where Officer Clements was located. The motorcycle sustained moderate damage to its left side, including the front wheel well and undercarriage. The motorcycle left scuff marks on the street which were measured and photographed. Several photographs of the scene and damage to both vehicles were taken and submitted to the IT Bureau. The motorcycle was towed by Todys. Centre Street and Bowen Street are both public ways maintained by the City of Newton. The injured motorcyclist, Officer Clements, was transported to Saint Elizabeth Hospital with fire fighters on board. I was able to follow up with Officer Clements on the phone the next day who was admitted over night at the hospital. Officer Clements stated he has no recollection of the accident and can only remember driving down Centre Street to begin to pick up cones to open up all lanes of traffic. Officer Clements stated he suffered a brain bleed, head, back, left foot and right thumb injuries. A Massachusetts Uniform Citation was issued and mailed to Helen for Chapter 89 Section 7A, failure to yield to emergency vehicle, police motorcycle.

OPER OF MV#1 STATED THAT HE HAD STOPPED FOR A PEDESTRIAN CROSSING THE ROAD, WHEN MV#2 STRUCK HIM FROM BEHIND. OPER OF MV#2 COULD NOT STOP IN TIME FOR A STOPPED MV#1, AND STRUCK THE VEHICLE FROM BEHIND. MV#1 SUSTAINED MODERATE REAR END DAMAGE AND WAS TOWED (PRIVATE). MV#2 SUSTAINED MODERATE FRONT DAMAGE AND WAS TOWED (PRIVATE)

OPER OF MV1 STATED THAT HE WAS PULLING OUT FROM A BOSTON COLLEGE DRIVEWAY (ON TO BEACON ST), WHEN MV2 DROVE AROUND ANOTHER STOPPED VEHICLE, CAUSING THEM TO COLLIDE. OPER OF MV#2 STATED THAT HE WAS TRAVELLING WB ON BEACON ST. OPER OF MV#2 STATED THAT HE DROVE AROUND A STOPPED VEHICLE (LEFT SIDE) AND ENTERED A LEFT TURN LANE. WHILE DOING SO, MV#1 EMERGED FROM A DRIVEWAY (WB SIDE) AND THE VEHICLES COLLIDED. MV1 SUSTAINED MINOR FRONT DAMAGE. MV2 SUSTAINED MODERATE PASSENGER SIDE DAMAGE.

Operator of M/V # 1 was operating straight ahead exiting the Boylston Street (EB) off ramp at Ellis Street. The operator states that she never saw the stop sign at the end of the off ramp and proceeded through the intersection when she was struck by M/V # 2 on her driver's side rear door. M/V # 1 sustained moderate damage to the door but the vehicle was still driveable. Operator was issued MA Uniform Citation # T1444908 for Ch. 89/S9 Stop Sign Violation. Below the stop sign there is also a sign that states that oncoming traffic does not stop. Operator of M/V # 2 was operating straight ahead through the above listed intersection attempting to go southbound on to Ellis Street when M/V # 1 suddenly came through the intersection. The operator could not avoid M/V # 1 striking it on the driver's side rear door. The vehicle sustained moderate damage to the right side of the front bumper area but it was still driveable. The operator had her dog, Coconut, in the rear of the vehicle at the time but the dog appeared to be fine. No injuries reported by either operators.

The operator of MV#1 stated he was travelling westbound on Watertown St when MV#2 rolled backwards from its parking spot and struck him. MV#1 sustained moderate damages to its front right side bumper and its rear right side doors. There were no reported injuries to the operator of MV#1. The tow truck operator ("Todisco Towing" truck with a ramp bed MA Repair plates 98BD) stated he was loading up MV#3 (An unregistered red colored BMW sedan being retrieved for auction bearing VIN WBABS33403PG88984) onto his inclined ramp bed when the tow hook on MV#3 broke causing MV#3 to roll backwards striking MV#2 which in turn pushed MV#2 into MV#1. Both MV#3 and MV#2 were unoccupied at the time of this accident and both vehicles were being towed for auction. There were no visible damages to the rear end of MV#3. There were minor damages to the rear left side of MV#2.

Based on statements and observations the following occurred: Mv#1 was travelling straight ahead on Oak St S/B and rear ended Mv#2 that was stopped in traffic at #149 Oak St. #1 operator wasn't paying attention to traffic stopped ahead of him, as a result wasn't able to stop in time, and rear ended #2. No injuries reported. #1 sustained significant front end damage. It was towed by Tody's. #2 appeared to sustain moderate rear end damage. #2 did not require towing and was driven away by the operator.

On Wednesday, May 29, 2019 while assigned to Traffic unit N525, I responded to the area of Centre Street and Washington Street, Newton for a report of a motor vehicle crash involving a school bus. The weather at the time of the crash was overcast and the road surface was dry. Centre Street and Washington Street are both public ways maintained by the City of Newton. The operator of MV1, Norma Cedeno (S60008138), stated she was operating a 2016 Dodge Caravan (MA SPN: 104893) around the Washington Street/Centre Street rotary. Cedeno stated as she was traveling in the left turn lane on Centre Street, she attempted to travel straight (N) through the intersection at Washington Street towards 255 Washington Street. Cedeno stated the front passenger side of her vehicle crashed into the driver side of MV2. MV2 was attempted to continue left around the rotary and was located on the passenger side of her vehicle. Cedeno reported no injuries. I observed minor damage to the front passenger side tire area of MV1. Cendeno's passenger, Cristina Velez (S04276767), reported not injuries. The operator of MV2, Mary Sanker (S62890912), stated she was operating her 2016 BMW 328XI around the Washington Street/Centre Street rotary. Sanker stated as she was continuing through the rotary in her lane, the vehicle traveling in the lane to her left (MV2) entered her travel lane in an attempt to travel straight towards 255 Washington Street and crashed into the driver side of her vehicle. I observed moderate damage to the front and rear driver side doors of MV1. Sanker reported no injuries. The operator of MV1 was cited for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care)

OWNER OF MV 1 STATED SHE HAD PARKED HER CAR AROUND 9 AM IN FRONT OF 18 BARNES ROAD. WHEN SHE LEFT HER HOUSE AROUND 1:45 PM, SHE NOTICED THE REAR DRIVERS SIDE OF HER VEHICLE WAS STRUCK BY ANOTHER UNKNOWN VEHICLE ( POSSIBLE LANDSCAPER TRUCK) CAUSING MODERATE DAMAGE. NO WITNESSES TO THE CRASH.

On Thursday, June 6, 2019 while assigned to Traffic unit N525, I responded to the intersection of Waverly Avenue and Franklin Street for a report of an accident involving a City of Newton school bus. The weather at the time of the accident was clear and sunny. The road surface was dry. Waverly Avenue and Franklin Street are both public ways maintained by the City of Newton. The operator of MV1, Dorothy Wensink (S40227061), stated she was operating her 2010 Toyota Prius (MA: 538EV6) Eastbound on Franklin Street towards Waverly Avenue. Wensink stated she came to a stop at the stop sign on Franklin Street (E) at Waverly Avenue. Wensink stated as she began to travel through the intersection, a school bus traveling on Waverly Avenue (S) crashed into the rear driver side door area of her vehicle. Wensink reported no injuries on scene. I observed moderate damage to the rear driver side area of MV1. Wensink was able to driver her vehicle away from the scene. MV2 was a City of Newton contracted yellow school bus (MA BUN: 39173) transporting students attending the Ward Elementary School. The bus was returning to the Ward School from a field trip. The operator of MV2, Antonio Goncalves (S54275856), stated he was operating the school bus Southbound on Waverly Avenue towards Franklin Street. Goncalves stated MV1 abruptly accelerated across the intersection from Franklin Street. Goncalves stated he was unable to avoid making contact with MV1 and crashed into the rear driver side of the vehicle. I observed minor damage to the front middle and passenger side bumper area of MV2. The following Ward Elementary School student, teachers, and parents were traveling on the school bus at the time of the accident and reported no injuries. Students: Avishai Arbili, Eleanor Chen, Maeve Donovan, Esat Efendigil, Mark Elentuck, Julian Ismay, Rhea Khanna, Griffin Kirby, Ori Layani, Priscilla Le-Ngo, Callum McLaughlin, Carolina Muniz, Emily Ney, Hrithik Ponduru, Levi Rosenberg Van Gameren, Zachary Schwarzberg, Ava Smoller, Matthew Somers, Lillian Welford, William Balerna, Alexandra Bogdanova, Randi Green, Christopher Iatrou Johnson, Charlotte Nichols, Henry Pozen, Sofia Shadah Alcalde, and Zoey Steinberg. Teachers: Katherine Goldman and Lauren Ludman. Parents: Enrique Shadah, Brian Rosenberg and Xinxin Wang. The Principal of the Ward Elementary School, Becca Brogadir, responded to the crash scene and signed a patient refusal for all of the students with Newton Medics. The students, teachers, and parents, then walked back to the Ward Elementary School with Principal Brogadir. The school bus was able to drive away from the scene. Once the students arrived at the Ward Elementary School, the nursing staff there evaluated each child before they could return to class. The school staff stated they would make notifications to the parents of the children involved in the accident. After speaking with all parties involved, the operator of MV1 was cited in hand for Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care). Pictures were taken of both vehicles and submitted to the IT Bureau.

Operator of MV1 was driving northbound on Cypress Street and was trying to apply his brakes to slow down in traffic. His brakes failed and he turned left onto Paul Street, striking a curb in front of #17 to stop his vehicle. He was evaluated by Cataldo and signed a patient refusal. The vehicle sustained moderate damage to the front passenger wheel and front bumper. It was towed by Bobby Donahue of Tody's Towing. Operator of MV1 stated he recently had his vehicle in the repair shop to get new brakes installed.

Operator of MV1 parked his vehicle outside his residence from 1100-1600hrs today. When he returned home at 1630, he came outside and noticed someone had struck his rear drivers side bumper. The vehicle had moderate damage and did not require a tow. The vehicle was unoccupied at the time of the accident and no note or other party information was left. Operator of MV1 requested the accident to be documented for insurance purposes.

Vehicle #1 stated they were attempting to turn around on beacon St. and turned into the driveway at 865 Beacon St. Vehicle #1 stated they were reversing out into the road and was stopped and shifting into drive when they were struck from the rear by Vehicle #2. Vehicle #2 stated she was traveling westbound on Beacon St. when she lost sight in front of her due to the glare from the sun. Vehicle #2 stated she never saw Vehicle #1 until she struck the vehicle. Vehicle #1 had minor damage to the rear and was able to be driven away. Vehicle #2 had moderate damage to the front and was towed by Tody's Towing. Operator of Vehicle #1 stated she was not hurt in the crash and refused any medical treatment. Operator of Vehicle #2 stated she had chest pain and was evaluated by Medic 1. Operator of Vehicle #2 was cleared by the medics and refused any further treatment.

Operator of MV#1 stated she was travelling westbound on Crafts St when MV#2 took a left hand turn in front of her causing a collision. MV#1 suffered moderate damage to the front and right side. MV#2 suffered damage to the passenger side. Both operators exited the vehicles and agreed to move out of the roadway, park and exchange information. Operator of MV#2 then left the scene without providing any information. Operator of MV#1 responded to the station to report the accident. Operator of MV#2 was described as an older white woman possibly in her late 60s early 70s driving a white sedan. No further description or plate given. No cameras in the area.

Vehicle #1 stated they were stopped on beacon st. facing east bound waiting for the light to turn green when she was struck in the rear by Vehicle #2. Vehicle #2 stated they saw Vehicle #1 slowing down and stop but was unable to fully stop in time and struck Vehicle #1 in the rear. Vehicle #1 had minor damage to the rear and was able to be driven away. Vehicle #2 had moderate damage to the front and was unable to be driven away. Vehicle #2 was towed by Tody's Towing. Bother operators were asked if they were injured and both stated they were not hurt in the crash and refused medical treatment.

On Monday, July 1, 2019 while assigned to Traffic unit N525, I responded to the area of 802 Walnut Street, Newton for a report of a motor vehicle accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Walnut Street is a public way maintained by the City of Newton. The operator of MV1, Dayana Martinez (S96710554), stated she was operating her 2019 Toyota Rav 4 (MA: 934RJ9), Northbound in the area of 802 Walnut Street, Newton. Martinez stated as she was coming around the bend the sensor system for her vehicle activated and she attempted to pull her vehicle away from the double yellow line. Martinez stated the vehicle locked up and she made contact with MV2. Martinez reported no injuries on scene. I observed moderate damage to the front driver side area of MV1. The operator of MV2, Gerard Kenneally (S75917793), stated he operating his 1999 Ford E150 (MA: 3AL821), Southbound in the area of 802 Walnut Street, Newton. Kenneally stated MV2 was traveling Northbound on Walnut Street and crossed over the double yellow line and side swiped the driver side of his vehicle. Kenneally reported no injuries on scene. I observed moderate damage to the driver side of MV2 from the driver door to the rear bumper area. I spoke with a witness to the accident, Bryan Stewart, via telephone. Stewart stated he was traveling Southbound on Walnut Street behind MV2 at the time of the accident. Stewart stated he observed MV1 drive over the double yellow line and into their travel lane. Stewart stated 3/4 of the vehicle was in the Southbound lane while it was traveling Northbound. Stewart stated MV1 crashed into MV2. Stewart stated if MV2 was not in front of him then MV1 would have crashed into him. After speaking with all parties involved, Martinez was mailed Massachusetts Uniform Citation T1272998 for Chapter 89, Section 4A (Marked Lanes).

On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

The operator of MV#1 stated she was travelling southbound on Centre St in the left side travel lane and attempted to switch lanes into the right side travel lane. The operator of MV#1 stated MV#2 was in her blind spot and made MV#2 swerved into MV#3. MV#1 never made contact with MV#2. There were no reported injuries to the operator of MV#1 or its passenger. There were no damages to MV#1. The operator of MV#2 stated he was travelling southbound on Centre St in the right side travel lane when MV#1 abruptly entered his travel lane causing him to swerve into MV#3. MV#2 sustained heavy front right side damages. There were no reported injuries to the operator of MV#2. MV#3 was unoccupied and parked in front of 1211 Centre St. MV#3 sustained moderate damages to its left side. The owner of MV#3 was advised of this accident. Based on the statements made to me, I will mail citation #T1268347 to the operator of MV#1 for violations of c89 4A marked lanes violation.

On 07/25/19 at 10:28 hours, I responded to Centre Street and Boylston St off ramp (WB) for a two car motor vehicle accident with no injuries reported. Operator of MV1 states he was traveling straight ahead on Centre Street (NB) when he observed MV2 inching out of the Boylston Street off ramp. He was not able to stop in time and the two vehicles struck one another. I observed heavy damage to the passenger front bumper and tire area and the vehicle was towed by Tody's. Operator of MV2 states he was exiting the Boylston Street off ramp (WB) and was going to attempt a left turn on to Centre Street after stopping for the stop sign. He then slowly pulled out into the intersection due to National Grid trucks blocking his view and that is when he came in contact with MV1 on his driver's side front bumper. I observed moderate damage to the entire front bumper but the vehicle was still driveable. I did observe National Grid trucks parked at the intersection setting up to do construction work. One truck was parked on the left side of the off ramp and the other was parked on the side where the crash occurred. This did obstruct the operator of MV2's view when pulling out.

On 07/28/19 at approximately 12:26 I responded to a two vehicle crash at the intersection of Needham St and Tower Rd. The operator of veh #1 stated she was driving Southbound on Needham St. when veh #2 pulled out of Tower Rd. from her right and collided with her car. The side airbags went off in veh #1 but the driver stated she wasn't hurt. She was checked out and signed a patient refusal with the Medics. Moderate to heavy damage to veh #1 as it was towed by Tody's towing. The operator of veh#1 has a License and Registration issued by the Department of The State (Brazilian Consulate) and isn't in our in house system to plug into this report. The operator of veh #2 stated she was pulling out of Tower Rd attempting to go left (Northbound) on Needham St. and didn't see veh #1 passing as she turned. Minor damage to veh #2 as the operator was cited in hand Mass. #T1271259 for N.C.O. Failure to Use Care in Turning. All roads traveled/ mentioned in this report are public ways in the City of Newton as they are serviced and maintained by this city.

Operator of Motor Vehicle #1 stated that she was traveling Northbound in the left lane on Dedham Street getting ready to turn left onto Nahanton Street when the car next to her all of a sudden took a left crossing over the double yellow line in an attempt to turn around. Operator of motor Vehicle # 2 stated that she was travelling Westbound on Brookline Street in Newton and wanted to take a left onto Dedham Street however due to the traffic detour she was forced to take a right onto Dedham Street (Northbound). Operator Motor Vehicle # 2 stated that she attempted to take a left turn into a driveway so she could turn around and thought she could make a U Turn instead. Operator of Motor Vehicle #2 stated that she took the left and did not see the vehicle that was in the left lane next to her and struck vehicle # 2. Motor Vehicle #2 sustained moderate drivers side, side damage, and motor vhehicle #1 sustained moderate front end damage. Both parties reported no injuries and signed medical refusal forms.

Operator of MV1 stated she was traveling Westbound on Beacon St. then turned Northbound onto Sumner St. Operator of MV1 then stated as she turned onto Sumner St. MV2 collided with her vehicle in front of 212 Sumner St. Operator of MV2 stated he was attempting to turn out of his parking spot in front of 212 Sumner St. when he stated he looked, waited for a vehicle to pass him then proceeded to nose out into Sumner St. but observed MV1 at the last second and collided with MV1. No injuries. MV1 sustained moderate front and rear passenger door damage. MV2 sustained moderate left front end damage. No vehicles towed.

On Thursday, August 8, 2019, while assigned to Traffic unit N525, I responded to the area of Centre Street and Mt. Ida Street for a report of a 3 car motor vehicle crash with entrapment. The weather at the time of the crash was clear and sunny. The road surface was dry. Centre Street and Mt. Ida Street are both public ways maintained by the City of Newton. On arrival, I was updated by Sgt. Chisholm that one of the vehicles (MV3) had a female operator that was entrapped as a result of the collision. She was freed from the vehicle by the Newton Fire Department and transported to Beth Israel Hospital. Another operator (MV2) was transported to Newton Wellesley Hospital with minor injuries. A third operator (MV1) remained on scene with no injuries. With the assistance of Officer Wade we conducted a survey of the crash scene. We observed a large debris field at the intersection of Centre Street and Mt. Ida Street with debris from all three vehicles. The point of impact was observed to be on Centre Street (N) before Mt. Ida Street. There were fresh scrapes in the pavement as a result of the colission. We also observed skid marks from MV1 measuring 57 feet 8 inches from the start of it's skid to the vehicles final rest MV1 crashed into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane of Centre Street and crash into MV3 as it was passing by. The impact from MV2's front end crashing into MV3's driver side resulted in the operator of MV3 becoming entrapped. MV2 and MV3 came to final rest on Wesley Street (W) at Centre Street (S). The driver door of MV3 was inaccessible. The collsion resulted in air bag deployment for MV2 and MV3. I observed moderate damage to MV1's front grill/bumper area. I observed substantial damage to the front and rear of MV2 and substantial damage to the front and driver side of MV3. Tody's responded and removed MV2 and MV3 from the roadway. Photos were taken of the scene and submitted to the IT Bureau. Officer Martin of the Watertown Police Department was requested by Sgt. Lee to respond to the scene and assist with a commercial truck inspection of MV1. Officer Martin contacted Trooper Grey of the Massachusetts State Police Truck Team who also responded to assist with the truck inspection. Sgt. Lee remained with Officer Martin and Trooper Grey during the inspection. MV1 was relocated to Richardson Street for the inspection. Trooper Grey determined the truck was in compliance with all required paperwork and it passed it's mechanical inspection. MV1 was removed from the roadway by a state tow. I spoke with the operator of MV1, William Tisbert (S24792655). Tisbert stated he was operating a 2006 KW W900 Semi Truck (NH SEMI: AP2638) Northbound on Centre Street. Tisbert stated as he was approaching Mt. Ida Street, a vehicle came out of no where and he hit it. Tisbert stated there was no one in front of him and then a vehicle came off of a side street (Mt. Ida Street) and attempted to turn left in front of him to return to the side street. Tisbert stated he could not avoid hitting the vehicle. Tisbert crashed into the rear of MV2. MV2 then crossed into the opposite lane and crashed into another vehicle. Tisbert remained on scene and reported no injuries. Tisbert stated he was wearing his seatbelt at the time of the accident and was not distracted. Tisbert later admitted to crashing into the rear of MV2. I spoke with the operator of MV2, Janet Vanbuskirk (S59829146), at Newton Wellesley Hospital. Vanbuskirk stated while operating her 2004 Toyota Highlander (MA: RS 529Y), she was stopped on Centre Street (N) at Mt. Ida Street. Vanbuskirk had her left directional on and was waiting for traffic in the Southbound lane to clear to take a left turn onto Mt. Ida Street. Vanbuskirk stated MV1 crashed into the rear of her vehicle. As a result of the impact, Vanbuskirk's vehicle traveled into the opposite lane and crashed into the driver side of MV3. Vanbuskirk reported minor injuries to her neck, left arm, and right hand. I spoke with the operator of MV3, Madeleine Karolian (S23979811), at Beth Israel Deaconess Medical Center in Boston. Karolian stated she was operating her 2016 Toyota Prius (MA: EV5746) Southbound on Centre Street. Karolian stated MV1 crashed into MV2, and MV2 crashed into her vehicle. Karolian stated she did not have time to avoid the collision. Karolian stated she had minor injuries to her neck and chest. Karolian's vehicle had a dash cam recording at the time of the accident. I viewed the footage with her through an app on a cell phone. The footage shows MV3 traveling Southbound on Centre Street on August 8, 2019 at 1308 hours. Ahead of MV3 in the Northbound lane, MV2 is seen stopped on Centre Street before Mt. Ida Street with it's left directional activated. I observed MV1 travel Northbound on Centre Street and crash into the rear of MV2. The impact caused MV2 to enter the Southbound travel lane and crashed into the driver side of MV3. The footage then ends. A copy of the footage was emailed to my work email. The footage was then forwarded to IT Director Steve Smith to be added to the media for this report. As a result of my investigation, the operator of MV1, William Tisbert, will be cited for Newton City Ordinance Chapter 19, Section 71 (Following too closely). Massachusetts Uniform Citation T1270942 was completed and mailed to Tisbert's residence.

Operator of Motor Vehicle #1 stated that he was traveling Eastbound on Jackson Road and preceded straight through the green light at the intersection of Jackson Road and Pearl Street and was struck by motor vehicle #2 that ran the red light. Operator of Motor Vehicle #2 stated that he was traveling Northbound on Pearl Street and the glare of the sun was in is eyes and did not see the red light until it was to late and struck motor vehicle #1 which caused it to roll over. Operator of motor vehicle #2 stated that he helped to operator of motor vehicle #1 get out of the car and stated that he had a few scratches on his knee. Motor vehicle #1 sustained major damage throughout the entire vehicle. Operator of Motor vehicle #1 sustained minor injuries and was transported to Newton Wellesley Hospital. Motor vehicle #2 sustained moderate front end damage. Tody's Towing towed motor vehicle #1. A citation was issued to the operator of motor vehicle #2 for failure to stop at red light..

The operator of MV1 stated that he was traveling southbound on Chestnut Street, stopped at the stop sign, and then continued straight on Chestnut Street towards Needham. He then stated that MV2 came off the off ramp of Boylston Street and did not stop completely at the stop sign. MV1 subsequently t boned MV2. The operator of MV2 stated that he was exiting Boylston Street in the Eastbound off ramp. He stated that he came to a complete stop at the stop sign and began inching out when he was struck by MV1. He also stated that he observed MV1 stop at the first stop sign but believed MV1 was going to stop at another stop sign under the bridge which is why he inched out so far. Both operators were uninjured and signed patient refusals but the passenger of MV1 complained of arm and neck pain and was transported to Newton Wellesley Hospital by Cataldo. Todys arrived on scene and towed MV1 as both headlights were disabled and the operator accompanied his wife to the hospital. Overall, MV1 sustained moderate damage to the front end and was towed while MV2 sustained moderate damage to the left drivers side but was able to be driven from the scene.

Vehicle 1 was travelling southbound on Centre St, behind Vehicle 2. Vehicle 2 started to stop because of traffic ahead. Vehicle 1 attempted to slow down, and hydroplaned into the left rear of vehicle 1. I observed minor damage to the left rear of vehicle 2, and moderate damage to the right front of vehicle 1. No one was injured due to the accident.

Operator #1 states he was traveling westbound on Auburn St when he collided with vehicle #2, which was pulling out of the driveway of #3 Auburn St. Operator #1 states he did not see vehicle #2 until contact was unavoidable. Operator #2 states he was pulling out of his driveway to turn right onto Auburn St when vehicle #1 hit the left driver's side of the vehicle. Operator #2 did not see vehicle #1 approaching. All parties declined medical attention but vehicle #2 had to be towed to Tody's due to moderate damage to the left driver's side door and bumper. Vehicle #1 sustained minor damage to the front right bumper but was driveable.

On 8/26/19 at 1640 hrs I responded to the intersection of High St and Rockland Pl for a MVA with no injuries. Upon arrival I spoke with both operators who confirmed that there were no injuries. I first spoke with the operator of MV#1 who stated she was driving southbound on High St when MV#2 came out of Rockland PL and struck her driver side door and rear driver side door. MV#1 sustained moderate damage to the driver side of the vehicle. I then spoke with the operator of MV#2 who stated that it was her fault. She was coming down Rockland PL, westbound and didn't see MV#1 and attempted to turn left onto High St but struck MV#1. MV#2 sustained moderate damage to the front of the vehicle.

Vehicle owner states he parked MA reg 3DH161 on the opposite side of the road from 297 Derby St around 0100 hrs this morning. Around 1530, he came out and noticed moderate damage to the front left corner of the vehicle. No tow required at this time, as the vehicle is parked legally and owner has his own tow coming. Area canvassed with negative results.

OPERATOR OF MV 1 STATED SHE WAS TRAVELING OVER THE NEWTON CORNER ROTARY IN THE OUTER LANE TO CONTINUE WEST ON WASHINGTON STREET WHEN MV 2, (WHO WAS TRAVELING IN THE MIDDLE LANE ON MV 1'S LEFT) ENTERED THE OUTER LANE STRIKING MV 1 ON THE DRIVERS SIDE CAUSING MODERATE DAMAGE. NO INJURIES REPORTED AND NO TOW. OPERATOR OF MV 2 STATED SHE WAS TRAVELING IN THE MIDDLE LANE OF THE NEWTON CORNER ROTARY ALSO TRAVELING WEST ON WASHINGTON STREET. OPERATOR OF MV 2 STATED THAT MV 1 COLLIDED WITH HER VEHICLE AND SHE DIDN'T BELIEVE THAT HER VEHICLE LEFT HER TRAVEL LANE. NO INJURIES REPORTED AND HER VEHICLE WAS TOWED PRIVATELY. IT APPEARS AS THOUGH MV 2 DRIFTED INTO MV 1'S LANE DUE TO WHERE THE DAMAGE WAS ON BOTH VEHICLES AND WHERE THE POINT OF IMPACT WAS SHOWN TO ME.

On 08/29/19 at 10:58 hours, I responded to a three car motor vehicle accident at 320 Needham Street with no injuries reported. Operator of MV1 states she was heading straight ahead (WB) on Needham Street towards Christina Street. She states that she attempted to stop for the stopped traffic ahead of her by pushing on her brakes hard to no avail. She then rear ended MV2 and MV2 was pushed in to MV3. MV1 sustained moderate front end damage to the grille and hood area. MV2 sustained moderate rear end damage and MV3 sustained a minor dent/scratch to the rear end. All vehicles were still driveable.

Vehicle #1 was traveling westbound on Boylston St. and was attempting to take a right turn onto dudley rd. Vehicle #1 was going too fast to make the turn and struck a rock wall belonging to 25 dudley rd. Vehicle #1 then traveled down the road a short ways and pulled over. The owner of the rock wall was notified and stated he was not home at the time of the crash. Vehicle #1 had moderate damage to the front of the vehicle and was towed by AAA. Both the operator and passenger of Vehicle #1 stated they were not injuried and declinded any medical attention.

Operator of MV1 was traveling westbound on River Street. Operator of MV2 was backing out of her driveway at 303 River Street. She stated she looked to her left, then her right and then the left again and saw it was clear to continue backing out. She did not see MV1 coming up River Street when she collided with him. MV1 sustained moderate front end damage and front and side airbag deployment. The vehicle was towed by Bobby Donahue from Tody's Towing. The operator of MV1 signed a patient refusal with Catalado AMB2. The operator of MV2 was not injured and was able to pull her vehicle back into her driveway. She sustained moderate damage to her rear passenger side.

Mvs# 1,2,3 were travelling west on Dedham St (at Arnold Rd) at approx 5 mph. As mv#1 slowed in traffic, Mv#3 collided into mv#2 which in turn got pushed into mv#1. Mv#1 sustained minor rear damage Mv#2 sustained minor rear/front damage Mv#3 sustained moderate front end damage mv#3 was towed away by AAA and there were no injuries due to this accident.

On Saturday, September 28, 2019, I responded to the intersection of Crafts Street and Albemarle Road, Newton with Sgt. Lee for a report of a motor vehicle crash involving a motorcycle with serious injury to it's operator. The weather at the time of the crash was clear and sunny. The road surface was dry. Crafts Street and Albemarle Road are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jussara Pontes (S92930243). Pontes stated she was operating a 2017 Toyota Sienna (MA: 4GX777) at the time of the crash. Pontes stated she was traveling North on Albemarle Road and came to a stop at the stop sign located at the Crafts Street intersection. Pontes stated she looked both ways and began to inch out into the roadway. Pontes stated she stopped the vehicle again and was assisting a child sitting in the 2nd row of the vehicle. Pontes stated she then turned her attention back to the roadway. Pontes stated she looked to her right and left again and did not see any vehicles in either direction. Pontes stated she proceeded to cross the intersection to continue North on Albemarle Road. Pontes stated the front bumper area of MV1 crashed into the left side of a passing motorcycle. Pontes stated the motorcycle and it's operator fell to the roadway and slid across the street to the side of the road. Pontes stated she pulled her vehicle over onto Albemarle Road. Pontes stated she did not see the motorcycle prior to the crash and was not distracted. The operator of the motorcycle (MV2), Irene Pistoftzian (S06099449) was transported to Brigham and Women's Hospital with a serious left leg injury prior to my arrival. I observed a red 1990 Kawasaki ZR550 (MA MCN: 1V1063) resting on it's right side on the NE sidewalk area of Albemarle Road at Crafts Street with it's front wheel/handlebar facing Crafts Street. The motorcycle had moderate damage to it's left side from the impact of MV1 crashing into it. The right side of the motorcycle had damage from sliding on the roadway and the crashing into a fence on the sidewalk on Crafts Street (W) at Albemarle Road. There was also damamge to the front headlight/right handlebar area. The motorcycle and Toyota Sienna were towed from the scene by Tody's towing at Sgt. Lee's request. Pictures were taken of the scene and submitted to the IT Bureau. I spoke with Pistoftzian at Brigham and Women's Hospital in Boston on Monday, September 30, 2019. Pistoftzian stated she was operating her motorcycle on Crafts Street (W). Pistoftzian stated as she approached the intersection of Crafts Street and Albemarle Road, she observed a mini van to her left out of her peripheral. Pistoftzian stated she remembers being hit on her left side and then laying on the roadway looking up at the sky on the sidewalk with her head facing Albemarle Field. Pistoftzian stated as a result of the crash she has a significant injury to her left leg and also has an injury to her left wrist/arm. Pistoftzian stated she was wearing a helmet at the time of the crash. I spoke with Kymberley Mitchell via telephone who stated she witnessed the crash. Mitchell stated she was traveling behind the motorcycle involved in the crash on Craft Street (W) at North Street. Mitchell stated they were stopped at a red light at this time. Mitchell stated there were no vehicles in front of the motorcycle at the light. Mitchell stated the light turned green and they continued on Crafts Street at a low rate of speed. Mitchell stated she could see ahead at the intersection of Albemarle Street and Crafts Street, traffic was stopped on Crafts Street (E) letting traffic exit Albemarle Road (N). Mitchell stated vehicles were taking left and right turns out of Albemarle Road before the motorcycle reached the intersection. Mitchell stated a mini van was crossing past the double yellow line of Crafts Street to continue straight when it crashed into the left side of the motorcycle. I spoke with Cheryl Wermer via telephone who stated she witnessed the crash. Wermer stated she was traveling on Crafts Street (E) towards Albemarle Road. Wermer stated traffic ahead of her was stopped to allow vehicles to turn off of Albemarle Road. Wermer stated she observed a motorcycle traveling towards her on Crafts Street (W). Wermer stated she then saw a vehicle travel straight from Albemarle Road across Crafts Street and crash into the motorcycle. Wermer stated the operator of the motorcycle flew off the bike. Wermer stated the car then came to a stop and the driver got out and stated "Oh my god! I didn't see you!" Wermer stated both vehicles were traveling at a low rate of speed prior to the crash. I spoke with Bill Frabizio via telephone who stated he witnessed the crash. Frabizio stated he was behind the mini van (MV1) on Albemarle Road prior to the crash. Frabizio stated the vehicle in front of him began to cross the intersection as he was monitoring the traffic on to take a right turn onto Crafts Street. Frabizio stated he heard a loud "Boom" and saw a motorcycle slide across the street with its operator. Frabizio stated the motorcycle crashed into a fence post and the operator came off the motorcycle. Frabizio stated he pulled over and assisted operator of the motorcycle. After concluding my investigation, the operator of MV1, Jussara Pontes, was mailed Massachusetts Uniform Citation T2016424 for Newton City Ordinance Chapter 19, Section 75 (Failure To Use Care).

Operator of MV1 stated he was traveling Eastbound on Beacon St. and stopped for a pedestrian in the crosswalk located in front of 281 Beacon St. The operator of MV1 further stated as he came to a complete stop at the crosswalk MV2 rear ended him. Operator of MV2 stated she was traveling Eastbound on Beacon St. when she stopped behind MV1 in traffic. Operator of MV2 further stated as she was stopped in traffic MV3 rear ended her with enough force resulting in her vehicle colliding with MV1. The operator of MV3 stated she was traveling Eastbound on Beacon St. when she observed MV2 stopped in traffic, however she attempted to stop to avoid a collision but rear ended MV2. It should be noted the operator of MV3 attempted to turn her vehicle away from MV2 but she stated it was too late. No injuries. All involved parties signed patient refusals. MV1 sustained minor rear end damage. MV2 sustained moderate front and rear end damage. MV3 sustained heavy front end damage. Tody's responded and towed MV3. A towed motor vehicle form was filled out and placed on file.

Mv#1 came to a stop at the traffic light at the end of Hobart Rd S/B at the intersection of Beacon St. All traffic lights were on flash mode. #1 had a flashing red light. #1 stated she proceeded straight ahead after she believed traffic was clear. #1 middle/passenger side front end struck Mv#2 passenger side rear quarter panel area which was travelling through the intersection on Beacon St W/B. #2 stated he had a flashing yellow light and was negotiating his way through the intersection on Beacon St W/B. #2 stated at that time he was struck on the passenger side rear quarter panel area. Both vehicles sustained moderate damage. Observations upon arrival was heavy rush hour traffic with the intersection completely congested as vehicles were trying to get through it on flashing lights. City of Newton traffic light contractor Dagle Electric was notified to respond.

OPERATOR OF MV 1 WAS TRAVELING WEST ON CRAFTS STREET WHEN HE STATED HE LOST CONTROL OF HIS VEHICLE WHICH STRUCK MV2 IN THE REAR CAUSING SIGNIFICANT DAMAGE TO THE VEHICLE. DUE TO THE IMPACT, MV 2 WAS PUSHED INTO MV 3 CAUSING MODERATE DAMAGE TO THE REAR OF MV 3. BOTH MV'S 2 AND 3 WERE LEGALLY PARKED AND UNOCCUPIED AT THE TIME OF THE ACCIDENT. WE WERE ABLE TO NOTIFY BOTH OWNERS OF THE PARKED VEHICLES WHO WERE INSIDE F.A. DAY ATTENDING THE CHINESE LANGUAGE SCHOOL. MV1 AND MV 2 WERE TOWED FROM THE SCENE BY TODYS. OPERATOR OF MV 1 WAS ISSUED IN HAND MA UNIFORM CITATION T2079841 FOR 89/4A MARKED LANES.

MV#1 operator stated she was in traffic on Centre St W/B opposite Aberdeen St. #1 stated she had slowed and then proceeded straight ahead thinking traffic ahead of her was moving forward. At that time, #1 rear ended MV#2 and #2 then rear ended MV#3. #2 and #3 operators both stated they were stopped in traffic when they were rear ended. #1 and #3 were not injured. #2 stated she had whiplash symptoms from impact. All were evaluated and signed patient refusals with medics. #1 sustained moderate front end damage; #2 appeared to sustain light front and rear end damage; #3 appeared to sustain damage limited to the rear bike rack apparatus attached.

OPER OF MV1 WAS TRAVELLING EB ON LAGRANGE ST, WHEN MV2 PROCEEDED (NB) FROM BRYON RD, COLLIDING WITH HER VEHICLE (PASS SIDE). MV2 WAS TURNING ON TO LAGRANGE ST, FROM BRYON RD, WHEN SHE STRUCK THE PASSENGER SIDE OF MV1. OPER OF MV2 STATED THAT MV1 HAD THEIR RIGHT TURN INDICATOR ON, AND BELIEVED THAT MV1 WAS GOING TO TURN. MV1 SUSTAINED MODERATE PASS SIDE DAMAGE. MV2 SUSTAINED MINOR FRONT DAMAGE.

Vehicle #1 was parked facing south in front of 7 Spaulidng La. Vehicle #1 had a trailer with an excavator attached to it. Vehicle #1 was unoccupied but had one occupant in the excavator on the trailer. The operator/owner of vehicle #1 was moving the excavator off the trailer when the trailer and truck started to roll southbound down the hill on Spaulding La. The operator of vehicle #1 jumped out of the excavator onto the roadway. Vehicle #1 along with its trailer rolled down and veered off the road and over the curb. The rear driver side end of the of vehicle #1 collided with the rear end of vehicle #2 bringing the trailer and truck to a stop. The rear end of vehicle #2 had moderate damage. The rear end of vehicle #1 had minor damage. Vehicle #2 was parked facing southbound just south of the driveway of 7 Spaulding La. Vehicle #2 was parked on the side of the road on Spaulding La. and was crashed into by the unoccupied truck and trailer vehicle #1. The occupant of vehicle #2 was not injured. The occupant of vehicle #1 was transported to Beth Israel Boston by Newton Amb. 1. There were no tows needed. Reference incident report #19046951 for further details on this crash.

The operator of MV1 states that he was traveling Southbound on Chestnut street when MV2 failed to come to a complete stop on the Ellis Street and subsequently crashed into him. The operator of MV2 states that he came to a complete stop at the Ellis Street stop sign, made eye contact with the operator of MV1 who then waved him to pull out, but then drove forward causing the two vehicles to crash. Both operators reported no injuries and both vehicles were able to be driven from the scene as MV1 sustained moderate passenger side damage and MV2 sustained moderate front bumper damage.

Operator 1 of vehicle 1 was on Falmouth Rd heading W/B and was going too quick around the corner, lost contol and jumped the curb colliding with a tree in front of 101 Falmouth Rd. Moderate damage to the front of vehicle 1. No damage to the tree. Minor Injuries to operator 1 and passenger 1. Vehicle towed by todys towing. Operator was issued citation (T2015498) for c90s17 Speeding and c89s4A Marked Lanes violation.

On Wednesday, November 20, 2019, while assigned to Traffic unit N525, I responded to the area of Chestnut Street and Avalon Road, Newton for a report of a motor vehicle crash involving a Cataldo ambulance. Chestnut Street and Avalon Road are both public ways maintained by the City of Newton. The weather at the time of the accident was light rain. The road surface was wet. I spoke with the operator of MV1, Corey Remblad (S29346684). Remblad stated he was operating his 2007 Ford Ecoline (CT: 53CC04) Southbound on Chestnut Street towards Avalon Road. Remblad stated there was construction ahead of him on Chestnut Street and there was a police officer working a detail that was waving him on to continue down Chestnut Street. Remblad stated MV2 exited Avalon Road and it's front driver side crashed into the driver side of his vehicle. I observed moderate damage to the middle driver side of MV1. Remblad and his passenger Thomas Solecki report no injuries. I spoke with the operator of MV2, Richard Yunker (S70658898). Yunker stated he was operating 2019 Ford F150 (MA AMN: 5921) Westbound on Avalon Road towards Chestnut Street. Yunker was operating the vehicle in his capacity as a Field Supervisor Paramedic for Cataldo Ambulance. Yunker stated he approached the intersection and came to a stop before taking a right turn onto Chestnut Street. Yunker stated as he made his right turn his front driver side crashed into the driver side of MV1. I observed moderate damage to the front driver side of MV2. Photos were taken of both vehicles and submitted to the IT Bureau.

Operator #1 states he was driving northbound on Seminary Ave in MA reg 5LE114 (vehicle #1), when he was t-boned on the driver's side by MA reg 959SD7 (vehicle #2). Operator #2 states that he was attempting to turn left out of the Arnow Campus Center parking garage and did not see vehicle #1 approaching from the right. It should be noted that the physical address of the parking garage appears to be 131 Woodland Rd, however the collision occurred on the Seminary Ave side. All parties declined medical attention and no tows were needed. Vehicle #1 sustained significant damage along the driver's side doors. Vehicle #2 has moderate front end damage. Operator #2 was issued Ma citation T1268432 in hand for Failure to Yield to Oncoming Traffic. 131 Woodland Rd will be the address used in documenting this crash.

OPER OF MV#1 WAS STOPPED ON DEDHAM (EB), MAKING A LEFT TURN ONTO COUNTRY CLUB ROAD (SB). OPER OF MV#1 STATED THAT SHE HAD HER LEFT TURN INDICATOR ON. OPER OF MV#1 STATED THAT SHE WAS THEN STRUCK IN THE REAR BY MV#2. OPER OF MV#2 STATED THAT HE WAS TRAVELLING EB ON DEDHAM ST (AT COUNTRY CLUB ROAD), WHEN A CAT RAN ACROSS THE ROAD IN FRONT OF HIS VEHICLE. OPER OF MV#2 STATED THAT CAT DISTRACTED HIM, AND HE THEN STRUCK THE REAR OF MV#1. MV#1 SUSTAINED MODERATE REAR DAMAGE. MV#2 SUSTAINED MINOR FRONT DAMAGE. NEITHER VEHICLE WAS TOWED AND THERE WERE NO INJURIES.

Motor vehicle 1 (MV1) was traveling westbound on Derby St. when it side swiped motor vehicle 2 (MV2 ) which was legally parked in front of 267 Derby St. As a result of the crash MV1 sustained moderate passengers side damage, while MV2 sustained minor rear drivers side damage.

Veh #1 stated he was traveling due East on Dedham St when veh #2 pulled out of the "left turn only" lane and struck his drivers side. This pushed him into the curb flattening his passenger side front tire. Slight/moderate damage to veh #1 drivers side. Veh #2 stated he was traveling East on Dedham St when he realized he was in the wrong lane .He then tried to pull into the right lane when he collided with veh # 1. Slight/ moderate damage to veh #2 passenger side . Operator of veh#2 was cited in hand Mass #T2013424 for Ch89/ Sec4a-Unsafe lane change. Both vehicles were driven from the scene.

Operator of MV1 stated while traveling Northbound on Walnut Street MV2 was traveling close to his vehicle. Operator of MV1 stated as he approached the crosswalk to let a pedestrian cross, MV2 was in the left turn only lane onto Austin Street. Operator of MV1 stated MV2 then passed him on the left, striking the driver side of his vehicle and continued on Walnut Street toward Washington Street. Operator of MV1 stated as MV2 passed he was able to get a License Number of that vehicle. After speaking with the Operator of MV1, I contacted the Operator of MV2. The Operator of MV2 stated while traveling Northbound on Walnut Street, MV1 was attempting to look for a parking spot by stopping at times and turning his turn signal on and off. Operator of MV2 stated as MV1 turned his turn signal on and stopped after multiple times of breaking, he went around MV1. Operator of MV2 stated he did not know he struck the other vehicle and that is why he continued on Walnut Street. MV1 had moderate damage to the driver side of the vehicle. MV2 had moderate scratches to the passenger side of his vehicle.

OPERATOR OF MV 1 WAS ON FRANKLIN ST, STOPPED AT THE STOP SIGN LOCATED AT THE INTERSECTION OF WAVERLY AVE WHEN MV WAS TURNING LEFT ONTO FRANKLIN ST FROM WAVERLY AVE. AS MV TURNED LEFT HE TOOK THE TURN TOO QUICK AND STRUCK MV 1 WHO WAS STILL STOPPED AT THE STOP SIGN. MV 1 SUSTAINED MODERATE FRONT END DAMAGE, NO INJURIES REPORTED, AND THE VEHICLE WAS TOWED BY TODYS TO THEIR LOT. MV 2 SUSTAINED MODERATE DAMAGE, NO INJURIES REPORTED AND THE VEHICLE WAS NOT TOWED. OPERATOR OF MV 2 ACKNOWLEDGED THE ACCIDENT WAS HIS FAULT AND WAS ISSUED MA UNIFORM CITATION T2079858 FOR C.O. 19/75 FAILURE TO USE CARE WHILE TURNING. THERE WAS A CONSTRUCTION DETOUR IN PLACE WHICH HAD ALL TRAFFIC HEADING NORTHBOUND ON WAVERLY EITHER TURNING LEFT OR RIGHT AT FRANKLIN ST.

Operator 1 stated that he was traveling Northbound on Hyde Ave behind vehicle 2. Operator 2 suddenly stopped short and put his van in reverse, backing into vehicle 1. Operator 2 stated that he was traveling Northbound on Hyde Ave and missed the turned onto George St. Operator 2 looked in his side view mirror and in appeared to be all clear behind him. Operator 2 put the truck into reverse and started to back up, striking the front of vehicle 1. Vehicle 1 sustained moderate damage and vehicle 2 sustained minor damage. There were no tows and no injuries.

Mv#1 operator stated he was travelling on Cloverdale Rd E/B. #1 stated he began veering to the far left to avoid a parked MV on the right side of the road. #1 stated he saw Mv#2 on Cloverdale Rd on the left side of the road but believed #2 was parked as well. #1 attempted to drive between the middle of those two MVs. As a result, #1 driver's side front end struck #2 driver's rear passenger side. #2 operator stated he was not parked but also travelling on Cloverdale Rd W/B. #2 stated he saw #1 veering into his lane of travel. #2 stated he started to brake and move to his right and was forced onto the grassy birm by #1 approaching from the wrong side when #2 was struck by #1. No injuries reported. #1 was towed by Tody's with heavy front end damage. #2 had moderate damage not including rim damage to the driver's side rear wheel which #2 was able to change the flat to a spare on scene and able to drive the MV away.

On Wednesday 02/19/2020, at approximately 1500 hrs, operator of MV1 stated she was traveling eastbound on Newtonville Ave, right before Walnut Street, a dark colored pick up truck was traveling behind her. Operator of MV1 stated she was driving below speed limit due to the approaching intersection at Walnut street when MV2 went into the eastbound traveling lane attempting to go around her. While attempting to go around MV1, MV2 side swiped MV1 causing the driver side mirror to break off causing moderate damage to the driver side of her vehicle. Operator of MV1 stated the pick up truck then stopped in front of her and the operator, an adult male began to yell at her then drive off. Operator of MV1 stated she got back in her car and did not get any information and did not have her cell phone to call the police. Operator of MV1 stated she was able to drive her vehicle home that day. No further description of the pick up truck was provided.

The operator of MV#1 stated he was travelling southbound on Cypress St when MV#2 came out of Paul St and struck him. MV#2 continued to leave the area and was last seen travelling northbound on Cypress St. There were no reported injuries to the operator of MV#1. MV#1 sustained moderate damages to its rear passenger side door and wheel. There was a piece of MV#2's bodywork embedded in the rear rim and tire of MV#1. The bodywork is a black colored plastic and did not display any identifying markings. MV#2 was described as a black colored S.U.V type of vehicle with no further information on its make, model or operator. The damage to MV#2 should be to its front passenger side bumper area.

OPER OF MV#1, WAS PULLING OUT OF THE WEGMANS PARKING LOT ON TO ROUTE 9 (EAST), WHEN MV#2 STRUCK HER FROM BEHIND. OPER OF MV#2 WAS PULLING OUT OF THE WEGMANS PARKING LOT, ON TO RT 9 (EAST), WHEN SHE COLLIDED WITH THE REAR OF MV#1. MV#1 SUSTAINED MINOR REAR DAMAGE. MV#2 SUSTAINED MODERATED FRONT END DAMAGE AND WAS TOWED BY AAA.

Vehicle #1 was traveling southbound on Langley Rd. and was attempting to turn left onto Beacon St. when they struck the traffic light pole on the traffic island. Vehicle #1 stated they don't remember hitting the traffic light post. Vehicle #1 had moderate damage to the front and front right of the vehicle and was unable to be driven. Vehicle #1 was towed by Tody's towing. Both the operator and passengers were asked if they were injured in the crash to which they stated they were not hurt and refused any medical treatment. This accident report is in regards to incident report #20011285. Please see incident report for further details.

Operator of MV1 stated he was traveling Westbound on Washington St. attempting to merge Northbound on Washington St. and as he approached the intersection, he didn't see MV2 and collided with MV2. Operator of MV2 stated he was traveling Eastbound on Washington St. attempting to merge Northbound onto Washington St. into the center lane to travel over the bridge and through the intersection, however didn't see MV1 and collided with MV1. It should be noted MV2 had a yield sign when merging. M1 had a turning vehicles yield to pedestrian sign before entering the bridge. MV1 sustained moderate left front end damage. MV2 sustained heavy passenger side damage. Passenger in MV2 was transported to Newton Wellesley Hospital for shoulder pain. Tody's responded on scene and towed MV2. A towed motor vehicle form was filled out and placed on file.

On 3/16/20 I was dispatched to 127 Commonwealth Ave for a report of a MV(#1) crash into a fire hydrant. The operator left the scene and was shortly after identified as Mr Bai Lin Wu. Wu called NPD to report the crash once he arrived home. Ofc Ferguson took photos of the knocked over hydrant. City Water Dept was notified of the damage. I responded to Wu's residence. Wu stated he was travelling on the Commonwealth Ave W/B carriage road. Wu stated an unknown MV travelling behind him was trying to pass him on the left. Wu stated he turned to the right to allow the unknown MV to pass. Wu ran off the road to the right and struck the hydrant in front of #127 Comm Ave. Wu further stated he left because he was trying to get medication home as soon as possible to his ill wife. Wu's vehicle sustained moderate damage to the passenger side front end. Wu issued citation for CO Ch19/Sec75 failure to use care turning (went off road).

The operator of MV1 states that he was traveling northbound on Winchester Street when he stopped in front of 170 Winchester Street to allow a pedestrian to cross the road. While stopped, MV1 was struck from behind by MV2. The operator of MV2 stated that he saw the MV1 stop for a pedestrian but did not have enough time to stop and subsequently crashed into MV1. Both vehicles sustained moderate damage and were towed from the scene (MV1 by Todys and MV2 by AAA). Both operators were evaluated by medics due to air bag deployment and signed patient refusals.

Motor vehicle one (MV1) was traveling eastbound on Suffolk Rd. when it crashed into the rear of motor vehicle 2 (MV2), which was parked across from 217 Suffolk Rd. As a result of the crash MV1 sustained significant front end damage while MV2 sustained moderated rear end damage.

OPERATOR OF MV 1 WHICH IS A USPS MAIL TRUCK STATED HE HAD JUST DROPPED OFF A PACKAGE AT A HOUSE ON RIVER ST AND HAD CONTINUED STRAIGHT TRAVELING EAST ON RIVER STREET WHEN MV 2 (WHO WAS TRAVELING IN THE OPPOSITE DIRECTION) TURNED LEFT FROM RIVER ST ONTO ELM ST CUTTING DIRECTLY IN FRONT OF HIM. HE STATED THAT DUE TO THE SUDDEN TURN HE WAS UNABLE TO STOP IN TIME AND HIS FRONT BUMPER COLLIDED WITH HER PASSENGER SIDE. MV 1 SUSTAINED MINOR DAMAGE AN D NO INJURIES REPORTED. OPERATOR OF MV 1 ALSO STATED HE HAD SLOWED DOWN DUE TO A PEDESTRIAN WHO WAS CROSSING THE STREET ON ELM ST. OPERATOR OF MV 2 STATED THAT SHE WAS TRAVELING ON RIVER STREET AND TURNED LEFT TRYING TO GO SOUTH ON ELM ST AND MV 1 CAME OUT OF NOWHERE AND STRUCK HER PASSENGER SIDE. OPERATOR OF MV2 STATED SHE BELIEVED MV 1 WAS SPEEDING. MODERATE DAMAGE SUSTAINED TO THE PASSENGER SIDE, THE VEHICLE WAS TOWED BY AAA AND NO INJURIES REPORTED. I ADVISED THE OPERATOR OF MV 2 THAT SHE NEEDED TO BE SURE TRAFFIC WAS CLEAR BEFORE TURNING LEFT.

Operator of MV1 stated she was traveling Westbound on Tremont Street and could not see well due to a sun glare. Operator of MV1 further stated when she approached the intersection of Tremont Street at Hibbard Road she struck a pedestrian riding a bicycle. I was able to speak with two witnesses on scene who were friends with Ms. Shokri. Both were approximately 10 feet behind her in the crosswalk before she was struck by MV1. Witness statements are documented in this report. Both parents responded on scene and were entered into this report as non-motorists. Both followed medics to Newton Wellesley Hospital. MV1 sustained moderate front end damage. Pedestrian involved suffered minor injuries and was transported to Newton Wellesley Hospital. Operator of MV1 was evaluated on scene and signed a patient refusal. Due to the bicycle being moved prior to my arrival, pictures were not taken on scene. The bicycle was left off the roadway for her friends to pick up. Ms. Skokri was wearing a pink shirt, black shorts and was riding a black bicycle. Ms. Shokri was not wearing any protective equipment.

Operator of MV1 stated she was driving Northbound on Belmont St. when she struck MV2 that was parked. Operator of MV1 stated she attempted to drive around MV2, however stated she couldn't see out of one of her eyes due to a partial obstruction. MV2 was parked with no operators inside. Owner notified of accident. MV1 sustained moderate passenger side damage. MV2 sustained minor left rear damage. No injuries. No vehicles towed.

Uninvolved owner of MV1 stated his vehicle was struck while parked between the hours of 1400-1700 hrs on Thursday 07/16/2020. No information could be provided about MV2. MV1 sustained moderate damage to the drivers side passenger door. No injuries. MV1 was not towed. Operator requested an accident report for insurance purposes.

Operator 1 of MV 1 was traveling N/B on Chestnut St by Elliot St when MV 2 was attempting to turn left onto Elliot St and collided with MV1. Moderate damage to MV1, no injuries to Operator 1. Operator 2 was on Chestnut St facing S/B attempting to turn left onto Elliot St and did not see MV1 coming straight. MV2 collided with the front of MV1. Moderate damage to MV2. Passenger of MV2 was not injured. Both vehicles were towed by Todys towing. Operator 2 was issued citation # T2014675 for c90s23 Attaching wrong MV plates, c90s9 Unregistered MV and c89s9 Failure to yield onto oncoming traffic.

On August 13th, 2020, while working in marked unit N526, I responded to a 2 car MVA involving pedestrians at the intersection of Beacon Street and Chestnut Street, Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street and Chestnut Street are both public ways maintained by the City of Newton. The operator of MV1, Maria Dinapoli (S92626906), stated she was operating her 2018 Toyota Corolla ( MA REG: 1KNW71) heading west on Beacon Street and had the green light at the intersection of Chestnut Street. Dinapoli stated she was traveling straight through the intersection when MV2 which was traveling east on Beacon Street at the time cut infront of her vehicle to take a left turn onto Chestnut Street. Dinapoli stated she applied her brakes but was unable to stop in time and made contact with MV2. The impact of the crash caused MV1 to drive over the curb and onto the sidewalk where it made light contact with a baby stroller with a 1 year old inside. MV1 also struck and damaged a Newton Fire Department box that was located on the sidewalk. The baby stroller was being operated by the mother identified as Anna Gilman (S11328154). The pedestrians reported no injuries and signed patient refusals with Medic 2. MV1 sustained moderate front end damage and was towed from the scene by Todys. Dinapoli also reported no injuries and signed a patient refusal with Medic 2. The operator of MV2, Leena Dolabany (S12410136), stated she was operating her 2018 Jeep Grand Cherokee (MA REG: 1TCZ14) heading east on Beacon Street and had the green light at the intersection of Chestnut Street. Dolabany stated she was in a line of cars in traffic waiting to take the left turn onto Chestnut Street. Dolabany stated as the cars infront of her continued through the intersection she took a left onto Chestnut Street and as she was already into the turn she was struck by MV1. MV2 sustained moderate damage to the passenger side panel. Dolabany reported no injuries and signed a patient refusal by Medic 2. A query of all vehicles and parties involved through dispatch determined that MV2 was revoked for insurance. After speaking with the operator I was informed that MV2 had recently switched insurance agencies and Dolabany was able to provide paperwork from Geico Insurance stating the vehicle was insured. Due to the registry not displaying a valid registration, MV2 was towed from the scene by Todys. I advised Dolabany to contact her insurance company and advise them to update their information with the registry so she can retrieve the vehicle. Photographs of the scene and damage to city property were taken by Officer Wilson and submitted to the IT Bureau. On August 14th, 2020 I spoke with the operator of MV2, Leena Dolabany and her father George Dolabany in the front lobby of the police station. They provided paperwork from Geico Insurance that stated the vehicle was infact insured at the time of the accident and that there was an error made by the Registry of Motor Vehicles. Sgt. Devine and I advised them that we have to use the Registry's database for our investigation and because it still states the vehicle is revoked for insurance we can not release the license plates. Sgt. Devine advised them they could get another tow company to come and take custody of the vehicle but the plates will be seized until the registry shows an active status on the registration. I issued Mass uniform citation T2017328 in hand to Leena and advised her she will be summoned for a hearing for the following violations; Ch 90 section 34J Operating MV without insurance and Ch 90 section 9 Unregistered MV.

Operator 1 states that he came to a complete stop on Herrick Rd at Braeland Ave. and then proceeded forward. Operator 2 was traveling Westbound on Braeland Ave and did not stop at the stop sign. Vehicle 2 pulled in front of vehicle 1 and operator 1 did not have time to stop striking vehicle 2. Operator 2 states she was traveling Westbound on Braeland Ave. She did not see the stop signs because they were covered by tree branches and leaves and proceeded forward. While traveling through the intersection vehicle 2 was struck by vehicle 1. Both vehicle sustained moderate damage and vehicle 1 was towed by Tody's. There were no injuries.

On August 19th, 2020, while working in marked unit N526, I responded to a MVA vs. bicyclist in the area of the intersection of Crafts Street and Watertown Street, Newton. The weather at the time of the crash was clear and cloudy. The road surface was dry. Crafts Street and Watertown Street are both public ways maintained by the City of Newton. The operator of MV1, Cody Hjelm (NHL19517660), stated he was traveling westbound on Watertown Street and had just crossed over Crafts Street when he saw a bicyclist traveling in the same direction on Watertown Street. As MV1 approached the bicyclist he moved over to the left towards the double yellow lines to give the bicyclist room. MV1 states the bicyclist was no longer in front of his vehicle when he heard a thud type noise come from the side of his van. MV1 stated he pulled over and saw the bicyclist on the ground with a head injury. MV1 sustained moderate damage to the right passenger side of the vehicle. The operator of MV1 reported no injuries and signed a patient refusal. MV1 was able to leave the scene of the accident. The bicyclist identified as Harriet Fell (S04980771) was being treated by medics on scene at the time of my arrival. I spoke with her briefly inside the ambulance as she was being treated for a head injury. Harriet stated she has no recollection of the accident and does not remember what happened before or after the crash. Medics advised she was being taken to NWH for treatment. I spoke with Harriet on the telephone around 1630 hours. She stated she still does not remember what happened but stated she was just finishing her forty mile bike ride and was heading home. Harriet stated she has a hematoma on her head and recieved three stiches. Harriet also stated she has small abrasions on both hands. Harriet stated she was being released from the hospital shortly and will pick up her bicycle tomorrow. The bicycle was transported to the police station by Newton DPW and was secured for safekeeping inside Support Services by Officer Manouk. A property form was completed and submitted along with an Incident report # 20039014. I took photographs of the accident scene and submitted them to the IT Bureau. Update by Ofc. On 08/21/20 at 13:32 hours, I responded to Boylston & Chestnut Streets for a two car motor vehicle accident involving no injuries. On arrival, both vehicles were pulled over to the right side of the road by 917 Chestnut Street. Operator of M/V# 1 states she was traveling straight ahead on the westbound off ramp of Boylston Street at Chestnut Street. She states she stopped for the stop sign and was attempting to proceed straight through the intersection. Next, she states she was looking towards the right for oncoming traffic and thought it was clear to proceed both ways when she made contact with M/V # 2 in the intersection. I observed moderate damage to the driver's side front quarter panel area/bumper but the vehicle was still driveable. Operator of M/V# 2 states he was traveling straight ahead northbound on Chestnut Street at Boylston Street. He says that he believes M/V # 1 stopped for the stop sign but was inching out in to the intersection. He says that once she came out he attempted to apply his brakes to no avail and the two made contact. I observed moderate damage to the front passenger side quarter panel/bumper area but the vehicle was still driveable. As a result of the accident and the fact that the operator entered the intersection when it was not clear to proceed, I issued the Operator of M/V# 1 MA Uniform Citation# T2015936 for Ch. 89/9 Failure to Stop for Stop Sign.

The operator of veh #1 stated he was driving due North on Elliot St when veh #2 turned in front of him suddenly and without a directional. Moderate damage to veh #1 but he pulled into CVS parking lot and stated he had his own tow on the way. The operator of veh #2 stated he was driving South on Elliot St when he attempted to take a left into Negoshian's Gas Station and veh #1 drove down the street too fast and they collided. Moderate damage to veh #2 as it was towed from the scene by David from Tody's Towing. The operator of veh #2 was cited in hand for Ch 89/ Sec 8 Failure to Yield to right of way.
