OPMV#1 Was traveling Northbound on Grove St when she came to a stop at the intersection of Grove St and Woodland Rd. OPMV#1 Stopped and then proceeded into the intersection and was struck by the OPMV#2. OPMV#1 Stated she stopped at the stop sign and was waiting for the other vehicle to go but they didn't move. So she started to move into the intersection and that's when she was struck by the OPMV#2. OPMV#2 Was traveling Westbound on Woodland Rd when she came to a stop at Woodland Rd and Grove St. OPMV#2 Stopped and then proceeded into the intersection and was struck by the OPMV#1. OPMV#2 Stated she stopped at the stop sign as the other vehicle did and happened to be entering the intersection at the same time when she was struck by the OPMV#1. Vehicle#1 was towed due to extensive damage. There were no injuries due to this accident.

MV1 was traveling southbound on Walnut St when the operator, Saptarshi Ganguly, stated that he began to feel fatigued, believed he blacked out and then woke up after impact with MV2. Mr. Ganguly stated that he takes Atenolol for vasovagal syncope. I know this to be a condition that makes individuals feel like they are going to faint. He stated he was diagnosed with the disorder about 10 years ago. MV1 crossed over the double yellow lines on Walnut St and collided head-on with MV2 who was traveling northbound on Walnut St. Two witnesses (Kyle Fieleke and Edward Aybar) at the scene corroborated the story and said that MV1, in a 2015 BMW X535I, MA Reg: 668BG5 veered into the opposite lane and hit MV2 head-on, a 2018 Tesla Model3, MA Reg: EV585N. MV1 operator, Saptarshi Ganguly, was issued MA Uniform Citation T1440468 for 89/4a for Marked Lane Violation. A Request for Medical Evaluation form was filled out for Saptarshi Ganguly and faxed to the MA RMV at 857-368-0018 as I am unable to determine his driving ability. Both operators were transported to Newton Wellesley Hospital to be evaluated. Both vehicles had extensive front end damage and were towed by Tody's towing.

MV1 was traveling SOUTH on Cherry St through the intersection of Webster St. MV2 was traveling WEST on Webster St through the same intersection. MV2 collided with MV1 inside of the Cherry St/Webster St intersection causing heavy front end damage to MV2 and damage to the rear drivers side wheel and fender. MV1 operator, Leah Wilson, stated that she had a green light and proceeded through the intersection when she was hit by MV2. MV2 operator, Caroline Steiner, and her 12yo daughter (passenger/front seat) Lorelai Steiner, stated that they were proceeding through the green light and observed MV1 going at a high rate of speed through the intersection. No injuries reported. Both vehicles towed by Tody's. MV1 operator, Leah Wilson was issued in hand MA Uniform Citation T1440472 for 89/9 - Red Light/Fail. On scene, I observed debris surrounding the four-way intersection and MV1 laid to rest past the intersection and facing the wrong direction in traffic. The damage to both vehicles was severe and MV1 had air bag deployment. The reporting party was an NPD auxiliary officer who had responded prior to my arrival because he heard the crash from the backyard of the police station which is approximately 350-400ft. Based on my training and experience, I believed Leah Wilson to be traveling through the intersection while failing to yield to the traffic control device.

On 02/08/2019, I, Officer Conary, responded to 309 Tremont Street for a past hit and run involving a pedestrian. Upon arrival, I was met by FEBLOWITZ, Colleen, who stated at approximately 1745 hours, she was crossing in the crosswalk at Tremont at Hibbard when her foot was hit by a car. FEBLOWITZ explained to me that she was in the crosswalk and she saw a silver sedan, MV1, traveling Eastbound on Tremont Street approaching her but it was far away where she felt safe to walk across the street. She said she also raised her hand to draw attention to herself. FEBLOWITZ was halfway in the crosswalk when she felt severe pain in her right foot. She looked behind her and saw MV1 was very close to her. MV1 continued to drive down Tremont Street and not stop. There were no witnesses. FEBLOWITZ went to Partner's Urgent Care to be treated for minor injuries. The crosswalk is in a lighted roadway. FEBLOWITZ was unable to gain any other information on MV1. No further incident to report.

Narrative:
On Friday March 15th 2019 at approximately 0730 hours I responded to the intersection of Beacon Street and Herrick Road for a report of a hit and run involving a bicyclist. The weather at the time of the accident was cloudy and the road conditions were dry. Both streets are public ways in the City of Newton. Upon arrival I observed the victim identified as Glenn Butcher (MA OLN S36552335), inside the ambulance getting medical attention. Butcher was getting a bandage applied to his left leg and complained of leg, back and neck pain. Butcher appeared visibly shaken and in pain. Butcher stated he had the green traffic light and was travelling eastbound on Beacon Street crossing Centre Street and then continuing straight crossing Herrick Road. Butcher stated MV1 was travelling westbound on Beacon Street and took a left turn onto Herrick Road and collided into his bicycle. Butcher states upon impact he flipped over his bicycle and landed on the hood of MV1, before rolling off and landing on the ground. Butcher states he was wearing his helmet which was present. Butcher states his bicycle ended up partially under MV1. Butcher states the operator of MV1 pulled his vehicle to the side of the road and got out of his vehicle and spoke to several witnesses briefly. Butcher states the operator then left the scene without properly exchanging information and did not wait for the police to arrive. Butcher described the vehicle as a dark gray Honda, and described the operator as an older white male, 5'10 approximately 80 years old with white hair. Butcher was also able to provide an email address for another bicyclist who witnessed the accident. The witness left his information with Butcher prior to my arrival. Fire fighters on scene were able to get the registration number of the vehicle from the witness (MA REG 4CX148) and gave it to Officers on scene. Butcher was transported by Medic 1 to NWH with minor injuries. His bicycle had visible damage to the frame area and skid marks were present on both tires. 19 photographs of the scene and the bicycle were taken and sent to the IT dept. email. Officer Gabriel transported the bicycle to the station where it was logged into Property & Evidence for safekeeping. Dispatch was able to query the registration which came back to a gray, 2010 Honda Accord registered to a Matthew Sugarman (MA OLN S38601397), out of W. Roxbury. Dispatch contacted the Boston Police Dept. and had them send an Officer to the address to try to make contact with Sugarman. Sugarman was advised to contact the Newton Police Dept. and I was able to speak with him on the telephone. Sugarman states he was driving the vehicle today and was involved in an accident with a bicycle. Sugarman states he was travelling westbound on Beacon Street and was taking a left onto Centre Street not Herrick Road. While speaking with Sugarman he seemed confused as to his location during the accident and was adamant that he was turning left onto Centre Street and not Herrick Road. I advised Sugarman that I was on scene with the first responding Officers and the accident clearly occurred at the intersection of Beacon Street and Herrick Road based upon the final resting place of the bicycle. Sugarman went on to state that his vehicle was already turning when the bicycle struck his vehicle and he believed it was the bicyclists fault. I advised Sugarman it appears the bicyclist had the right of way since it was traveling straight and that because his vehicle was taking a left turn, he failed to yield for on coming traffic at an intersection. When asked why he did not stay at the scene of the accident Sugarman stated he got out of his vehicle and spoke to the bicyclist and he "seemed okay". Sugarman states he then left the scene while the victim was still sitting on the ground. I asked Sugarman if he had ever been involved in accident before and he replied "no." I advised Sugarman that I am currently looking at his drivers history and see nine accidents that he was involved in. Sugarman stated "he has never been in accident like this before." I then asked Sugarman what the law states when you are involved in an accident and he was not able to provide me with an answer. I advised him that the law states you must stop and exchange information before leaving the scene of an accident. Again he stated the bicyclist seemed okay and "he had somewhere to be so he left." Sugarman has an extensive drivers history including nine surchargeable accidents, reckless operation, two different charges of leaving the scene property damage and two immediate threat affidavits filed against him. I advised Sugarman that I will be mailing him a citation for Ch89 s8 Fail to yield to oncoming traffic, and Ch90 s24 Leaving the scene of personal injury. I also filed an immediate threat form with the registry. I emailed the witness using the information he provided to the Newton Fire Dept that was on scene and received a phone call back from the witness identified as Bruce Adams. See witness report for his statements. I contacted the Bank of America located at 808 Beacon Street in Newton, as they are the closest business to that intersection with video surveillance. I spoke with George Walsh (781-834-8275) who stated they do not have any working cameras on that side of the building to the bank. For this report the bicyclist is entered as a non-motorist for validation purposes. Witness Bruce Adams (MA OLN S96398108) stated that he was riding his bicycle travelling behind the victim about 100 feet away. Adams states he was watching the victim and had a clear view of him as he travelled behind him, through the green traffic light heading eastbound on Beacon Street. Adams stated he witnessed MV1 travelling westbound on Beacon Street take a sharp left turn onto Herrick Road which turns into Union Street. Adams stated that the vehicle turned left colliding into the bicycle and there was no possible way the bicyclist had time to stop to avoid the collision. Adams stated the victim flew over his handle bars and fell on top of MV1's hood and then landed on the ground. Adams stated the victim's bicycle was stuck under MV1's front bumper. Adams stated the operator of MV1 backed his vehicle up with the bicycle still attached to his vehicle bumper and pulled MV1 over to the side of the road. The operator then got out of his vehicle and walked up to the victim who was laying on the ground visibly in pain. Adams stated the operator asked the victim if he was okay to which the victim did not immediatley reply as he was visibly shaken up. Adams stated the operator then walked back to his vehicle and attempted to leave the scene and that is when Adams stood infront of MV1 to stop him from leaving the scene. Adams stated he then removed the bicycle from underneath the bumper of MV1 and placed it on the side of the road. Adams stated the operator got out of his vehicle again and walked up to the victim and asked him "what do you want to do?". Adams stated the victim did not respond and that is when the operator got back into his vehicle and drove away, leaving the scene before any emergency response arrived. Adams stated he was able to memorize MV1's registration and stayed with the victim until police, fire and medics arrived. He stated he gave the fire dept the registration and his contact information prior to leaving the scene.

On 03/19/19 at 14:57 hours, I responded to 220 Dedham Street for a past motor vehicle hit and run accident involving a parked car that occurred at 12:26 hours on this date. Dedham Street is a public way maintained by the City of Newton. The owner of M/V # 1 states her vehicle was legally parked in front of 220 Dedham Street facing northbound when she came out and found a note from a witness saying it was struck by a gray Honda Accord bearing MA Reg. # 8512HV. It appears that the operator of M/V # 2 side swiped M/V # 1 as it was passing by on Dedham Street. I observed marks to the driver's side rear door area and the side view mirror was broken. On 03/20/19 at 08:47 hours, I spoke to the witness, Kawtar Bhih, on the phone and she stated that she was parked on the opposite side of where the accident took place. Bhih observed a gray Honda Accord with an elderly female operating M/V # 2 on Dedham Street almost side swiped another parked car before side swiping M/V # 1. She says that the vehicle slowed down after and then continued to leave the scene of the accident. She then took a photo of the above listed registration plate. I attempted to contact the owner of M/V # 2 (Jali Medical Inc Waltham, MA) and the receptionist was not able to provide any information at the time. I was told to call back the next day. I gave the vehicle and registration info to the person inquiring if they could find a possible driver and call back. While working on this report, I observed that the owner of M/V # 2 was involved in a past accident in our system and the operator at the time had an address of 66 Country Club Rd. Newton. On 03/20/19 at approx. 09:30 hours, I went over to this address and observed M/V # 2 in the driveway with an operator inside. After inquiring about the accident, she stated she was driving down Dedham Street when she heard a noise coming from her passenger side view mirror as she had struck something. She then left the scene not thinking it was serious. I observed fresh damage to her passenger side mirror along with other marks along the doors which appeared to be older. Operator was issued in hand MA Uniform Citation # T1441710 for Ch. 90/S24 Leaving the scene of property damage.

VEHICLE #2 STATES SHE WAS TRAVELING S-BOUND ON CHERRY ST. AS SHE APPROACHED WEBSTER ST. SHE STATES SHE WAS BLINDED BY SUN GLARE AND CONFUSED THE LIGHTS AT CHERRY AND WASHINGTON AS BEING THE CORRECT LIGHTS ( THEY WERE GREEN AT THE TIME ). WHEN SHE REALIZED THE LIGHT AT CHERRY AND WEBSTER WAS RED SHE WAS UNABLE TO AVOID HITTING VEHICLE #1. VEHICLE #1 HAD EXTENSIVE REAR AND LEFT FRONT END DAMAGE AND WAS TOWED BY TODYS. VEHICLE #2 HAD EXTENSIVE FRONT END DAMAGE AND WAS TOWED BY TODYS. OPERATOR OF VEHICLE #2 WAS MAILED CITATION ( T1442287 ) FOR CHAPTER 89/9 FAILURE TO YIELD. VEHICLE #1 WAS MAILED CITATION ( T1442286 ) FOR PASSENGER RESTRICTION VIOLATIONS UNDER JUNIOR OPERATOR LICENSE. ALL PARTIES , ( WITH THE EXCEPTION OF WRIGLEY LOWE ) REPORTED NO INJURIES AND SIGNED PATIENT REFUSALS. CHRISTOPHER CAMPBELL AND SERGIO ALVAREZ WERE TURNED OVER TO THEIR PARENTS. The operator of vehicle 1 stated that with a green left turn arrow showing, and her left directional on she went to turn onto Waltham Street from Crafts Street when all of the sudden she hit a bicyclist. She stated she did not see the bicyclist at all prior to him hitting her car. The bicyclist stated does not remember anything from the accident at this time. The bicyclist was traveling southbound on Crafts Street and appears to have entered the intersection of Crafts and Waltham Street without stopping and while a red light was showing. He was wearing a helmet at the time of the crash, he was not wearing any reflective clothing(dark green jacket), and the bike did not have any lights. The bike was taken to support services for safe keeping by Ofc. J.March. Vehicle#1 was towed away by Tody's towing due to extensive damage. Bicyclist was transported to Beth Israel hospital for several trauma injuries. Pictures of scene was taken by me and sent to IT Bureau. Both witness's statements were the same. They stated that they were behind vehicle 1 in traffic and saw that she had on her left dirrectional signal and had a green left arrow signal when vehicle 1 went to turn left onto Waltham Street.

The operator of vehicle 1 stated she was traveling West on Mill Street when vehicle 2 went through a stop sign, she was unable to stop and hit vehicle 2 in the front passenger side panel and door. This caused moderate damage to the front of her vehicle. She was evaluated by Cataldo and signed a medical refusal and was able to drive her car away. The Operator of vehicle 2 stated she was traveling North on Bullough Park, she came to a stop at the stop sign and proceeded to cross over Mill Street towards Upland Road. She was then hit on the passenger side by vehicle 1. She stated she did not see vehicle 1 at all. As a result of the impact, vehicle 2 sustained extensive damage to the passenger side and had side air bag deployment. The passenger in vehicle 2 stated he did not see anything until impact. The Occupants of vehicle 2 stated they had no injuries or pain and were taken away from the scene by parent Niels Steenstrup. As a result of the accident vehicle 2 was towed by Tody's by Billy.

Narrative:
On April 24, 2019, while assigned to N496, I was called by Dispatch to respond to the area Beacon Street and Hammond Street for a motor vehicle crash. This was called in by Boston College Police Department. Upon arrival MV #1 was on Beacon Street just past Hammond Street at the Island with considerable vehicle damage. MV#1 stated while traveling westbound on Beacon St, while approaching the intersection of Hammond he stated he started to feel strange and not like himself. MV#1 stated he then struck the curb with his front right tire, not believing his accident was very serious at this time. He reversed his vehicle off the sidewalk and proceeded to travel westbound on Beacon St. After passing through the intersection he stated his car felt very off at which point he initiated his hazards and pulled over to the right. MV#1 stated he has a history of epilepsy and currently takes Oxcarbazepine twice a day for his condition. He stated he did not believe he had a seizure or lost consciousness but did not recall the incident very well. Boston College Ofc. Cadogan witnessed the incident, and stated the following, while conducting a directed patrol at the intersection of Beacon and Hammond St he saw MV#1 traveling westbound on Beacon St. Ofc. Cadogan stated MV#1 then took a sudden tight turn on to the curb striking the metal fence surrounding the BC facility. MV#1 was then seen / heard revving his engine while still in drive before reversing his vehicle off the curb and attempting to continue on Beacon St westbound. The fence surrounding the BC Facility sustained moderate damage. Tire marks in the grass show the path of MV#1 in direct line to the damaged property. While on scene, I spoke at length with Vanhorne. I did not detect or any alcohol from his breath as he spoke. Vanhorne reported he only consumed one beer tonight at roughly 2030hrs and smoked a little marijuana at 2200hrs. Vanhorne's eyes did not detect any signs of alcohol and he walked with no signs of any impairment. Sgt. Devine administered a horizontal nystagmus test which revealed no signs of impairment. Vanhorne reported he was at a work event and then visited a friend in Brookline. Vanhorne reported his last seizure from epilepsy occurred a little over two years ago. Pictures of the scene were taken and submitted to IT to be attached to this report. MV#1 operator stated he was not injured at this time. MV#1 was towed from the scene by Todys's due to heavy front end damage. Vanhorne was picked up from the scene by Paul Miller his step father. A towed MV form was filled out and placed in the traffic box. Witnesses Cassandra Williams and Bill Peirce observed the crash and stated identical narratives to Ofc. Cadogan's. An Immediate Threat form was filed with the RMV (via Fax and E-mail at approximately 0650hrs 4/24/19)due to his medical history and his explanation with this crash. All streets mentioned are public ways in the City of Newton. It should be noted that the road was wet from rain on this night. Update: on 4/27/19 Sgt.Devine was able to watch the video of the incident from Boston College Police. The video confirmed the statements given by BCPD and witnesses's. Due to the aforementioned statements and the video surveillance of the incident, MV#1 Operator will be cited with Marked Lanes Ch89/4a (MA Uniform Citation #T1441792. Vanhorne will be notified via phone he will be receiving the ticket in the mail.

HE STATES VEHICLE #1 STOPPED ABRUPTLY AND HE WAS UNABLE TO AVOID HITTING HER. VEHICLE #1 HAD EXTENSIVE REAR AND FRONT END DAMAGE AND WAS TOWED BY TODYS. VEHICLE #2 HAD EXTENSIVE FRONT END DAMAGE AND MULTIPLE AIR BAG DEPLOYMENT AND WAS ALSO TOWED BY TODYS. OPERATOR OF VEHICLE #1 WAS TRANSPORTED TO NEWTON WELLESLEY HOSPITAL FOR POSSIBLE WRIST INJURY. PASSENGER WAS ALSO TRANSPORTED TO CHECK FOR POSSIBLE INJURIES. OPERATOR AND PASSENGER OF VEHICLE #2 WERE EXAMINED BY MEDICS AND BOTH REFUSED MEDICAL TREATMENT. BOTH OCCUPANTS OF VEHICLE #2 HAD CUTS ON LEGS AND WRISTS ASSOCIATED WITH AIRBAG DEPLOYMENT. THEY WERE ESCORTED TO SCHOOL BY NEWTON NORTH STAFF TO BE EVALUATED BY THE SCHOOL NURSE. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES.

Operator of vehicle 3 states he was traveling westbound on Washington St and had stopped to turn left into #1860 when he was struck from behind by vehicle 2. Operator of vehicle 2 stated she saw vehicle 3 waiting to turn left and was slowing down as she approached him when she was struck from behind by vehicle 1 and pushed into vehicle 3. She stated she saw vehicle 1 coming up behind her in rear view mirror and it appeared he attempted to swerve at the last minute to avoid a collision but failed. He attempted to swerve right to avoid contact but struck the rear of vehicle 2 pushing it in to the rear of vehicle 3. No reported injuries. Vehicle 3 was towed due to extensive damage by Todys.

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

On Wednesday 6/17/2020 at approx 2116hrs, while assigned to N-494, I responded to the intersection of Walnut St and Comm Ave for MVA involving a bicyclist. The bicyclist, Aziz Boltayev, said he was traveling NB on Walnut St and attempted to cross Comm Ave. He stated that he was in the middle of the intersection when Comm Ave' light signal turned green. Contact was made to MV1s driver side front panel. Boltayev was transported to Beth Israel with serious injuries to his legs. Boltayev was not wearing a helmet nor wearing reflective clothing. His bicycle, a blue Schwinn, had no front head lamp attached in violation of MGL c85 s11B. A citation was issued and mailed to Boltayev. He was wearing a light colored shirt and shorts. Operator of MV1 states that he was on Comm Ave heading WB and stopped at a red light at the intersection of Walnut St. He states that he had a green signal when attempting to cross the intersection and did not see the bicyclist prior to the accident. Pictures were taken and submitted to IT. The Schwinn bike was placed in property and evidence for safekeeping.

Reportedly STB June 25 at 1500hrs and June 26 at 0700hrs, an unknown Toyota MV struck a large wooden fence structure and struck a large back up generator behind that fence and left the scene at the Adelaide of Newton Centre grounds at 157 Herrick Rd. A broken Toyota "Koito" taillight or headlamp case (part #SAE AIPP2 97 DOT 60-64) was found in front of the damaged area. Appears unknown MV was parked in an angle marked space in front of generator and either backed up into it or collided head on into it. Damage appears extensive with possible underground conduit damage connected to the generator power source. Eversource notified. No outdoor video. Adelaide staff will update NPD if they become aware of any MV involved. Canvass of the grounds was negative results.

On Wednesday, 7/1/2020, at 2230hrs, I was dispatched to the area of 856 Watertown St for a report of a possible car crash. Watertown St is a public way as it is owned and maintained by the City of Newton. Upon arrival, I met with operator, Henok Kiflit, who was out of the vehicle and very disorientated. I observed a gash on his head right above his eye and noticed all airbags had been deployed in his vehicle. I asked him to sit down and I radioed for an ambulance. Kiflit stated that he was driving EAST on Watertown St when he collided with a unoccupied parked car (a red 2015 Jeep Wrangler, MA Con R22519) which was parked on the street outside of 856 Watetown St. Kiflit was driving his girlfriend's car (a gray 2020 Nissan Sentra, MA Reg 1ANN57) and was on his way home in Watertown. When I asked for Kiflit's license, he told me that he only has a permit and has yet to take the test. He apologized repeatedly. Kiflit did not have any passengers in his vehicle. Kiflit said the roadway was so dark and he didn't see the parked Jeep. He claimed he was not speeding. There was minimal damage done to the drivers side rear bumper of the Jeep. The Nissan had extensive front end damage and was subsequently towed by Tody's Towing. Towed motor vehicle form filled out and filed, copy made for police prosecutor. Owner of the Jeep came out and took possession of the vehicle. Kiflit was taken to NWH for evaluation of his injuries. Kiflit's driver history shows that he had an surchargable accident recently 05/30/2020. Massachusetts Uniform Citation T2080927 was filled out and prepared to be mailed to Kiflit at his Watertown address. Henok Kiflit is being cited for: 90/10 - Operating without a License (Learner's Permit violation; Kiflit, operator, was not accompanied by any motorist)

MV1 was operating westbound on Watertown St (public way). MV2 was parked on Watertown St in front of 144 Bridge St (public way). MV1 operator struck and side swiped MV2. MV2 sustained extensive damage. MV1 continued westbound on Watertown St and struck a cross walk sign and Newton Fire Department box which both in front of Coletti-Magni Park, and both were destroyed. MV1 hit and hopped the curb of Watertown St in front of Coletti-Magni Park, drove onto the side walk, and hit the fence of Coletti-Magni Park and damaged much of it. MV1 then came at rest on top of the Coletti-Magni Park fence and sustained extensive damage. MV1 operator stated he was not injured at this time and sign a patient refusal with Newton Medics. MV2 was unoccupied. Both MV1 and MV2 were towed by Todys.

SHE STATES ON 7-31-20 AT APPROX. 2200HRS. SHE WAS TRAVELING W-BOUND ON CHESTERFIELD RD. SHE STATES SHE ACCELERATED BECAUSE SHE WAS TRAVELING UPHILL. STATED SHE LEFT THE VEHICLE OVERNIGHT.. SHE STATED SHE DID NOT REPORT THE INCIDENT BECAUSE OF EMBARRASSMENT. COMP. STATES SHE HAD JUST CONTACTED AAA AND THEY GAVE HER AN E.T.A. OF ONE HOUR. COMP REPORTS NO INJURIES. COMP. ADVISED TO CONTACT INSURANCE AGENCY AND THAT I WOULD BE WRITING A REPORT OF THE INCIDENT. ADVISED TO CONTACT THE POLICE IN FUTURE INCIDENTS REGARDLESS OF EMBARRASSMENT. CLEARED WITHOUT FURTHER INCIDENT.

BOTH PARTIES WERE EXAMINED BY MEDICS AND SIGNED PATIENT REFUSAL. WHILE CRASHING THE VEHICLE MOTION TOOK THEM ONTO THE SIDEWALK AT THE CORNER OF HARRINGTON AND WASHINGTON. OWNER OF 45 HARRINGTON REPORTS DAMAGE TO HIS FRONT BUSHES AND A SMALL TREE. THERE WAS A CITY STREET SIGN THAT GOT BENT DURING THE CRASH. PICTURES TAKEN BY N522 ( OFF MCSWEENEY ) AND TURNED OVER TO I.T. BUREAU. BOTH PARTIES WERE ADVISED TO CONTACT THEIR INSURANCE AGENCIES.

On Monday, August 10th 2020 at 0459 hours, I responded to the intersection of Brookline St at Vine St for a one vehicle accident into a City Of Newton Tree. The vehicle was a Black Honda Accord Mass Reg # 7DZ958. Upon arrival, Samuel Perez was outside the vehicle and no operation of the vehicle was witnessed. There was no drugs or alcohol inside the vehicle. The vehicle appeared to have been operated heading west on Vine St. The vehicle continued through the stop sign at Vine St straight into Brookline St where the vehicle collided into a City of Newton Tree. Pictures were taken and submitted to IT. The airbag was deployed. Perez complained of chest and head pain. Perez was taken by Ambulance to Newton Wellesley Hospital. There was extensive damage to the front of the vehicle. Tody's Towed the vehicle. A Towed Motor Vehicle Inventory form was completed. Perez was mailed Mass Uniform Citation #T1273442 in the amount of $ 105 for M.G.L Ch. 89/9 Failure to Stop at Stop Sign.

Operator of vehicle one, Agustine Velasco stated that on 08/10/2020 at 13:36 hours he was driving a city of Newton contacted Waste Management trash truck ( 2009 Atuo Xpedit ) northbound on Woodbine St. Woodbine St is a pubic way in the city of Newton. Velasco stated that as his vehicle passed a parked unoccupied northbound facing vehicle ( 2017 Rage Rover MA reg 5GD698 ) in front of 19 Woodbine St the mechanical arm that picks ups trash barrels was not stored in the fully closed position. Velasco stated that the mechanical arm's clamp struck the driver's side rear door and lower quarter panel of the Range Rover causing extensive damage. The truck's arm clamp was broken off in the crash. Owner of vehicle two Maura Brown stated her vehicle was parked unoccupied facing north in front of 19 Wood brine St prior to the crash. Neither vehicle was towed . Velasco stated that hen was not injured in the crash. I took photos of both vehicles and forward the disk to the NPD's IT Bureau for downloading. Based upon Velasco's statements to me about his truck's mechanical arm not being in the fully closed position, and my observations of the crash scene, I issued Velasco MA Citation T2015997 and cited him for a violation of city of Newton Ordnance 19/75, failure to use care while starting.

On Friday, August 28, 2020 MV1 was traveling Northbound on Walnut St. when it crashed into the rear end of MV2 that was stopped at the intersection of Walnut St. and Cabot St. MV2 stated she was stopped at a red light and was rear ended. MV1 stated she drove into the rear end of MV2 and that she did not see the vehicle stopped. The front end of MV1 showed severe damage and was towed from the scene by Tody's Towing. The rear end of MV2 showed damage as well, but mainly scratches. All persons involved were evaluated by Cataldo Ambulance Service and all refused treatment.
