Narrative:
On 4/14/2019 at approx 0923Hrs while assigned to 497 I responded to the area of Commonwealth Av and Ash St for a report of a three car crash. upon arrival I observed three cars off to the side of the road. i spoke with the operator of ma Reg T6T, Robert MURPHY who stated he had stopped for the red light at Commonwealth and Ash St when the operator of TN Reg 2N37P1, Jennifer BURNELL failed to stop striking the operator of Ma Reg 8CL834 Evan GUIDRY who in turn than ran into the back of MURPHY. I spoke with BURNELL who stated she was lost and had been looking down onto her phone looking for a map when she ran into the back of GUIDRY. BURNELL cited for violation MGL90/13 distracted Operation , GUIDRY transported to NWH by Cataldo Ambulance for neck and head pain. GUIDRY and BURN ELLS vehicles towed by Todys. NFD responded and cleared roadway or debris and fluids.

Narrative:
Operator #1 states that he was driving his work van (Patriot Mechanical) westbound on Commonwealth Ave near Ash St, when his vision became blinded by the sun which caused him to rear end vehicle #2. Vehicle #2 was stationary at the red light and subsequently hit vehicle #3 upon impact in a "domino effect". All operators were spoken to separately and their version of events are the same. It should be noted that while responding to this call, I did take note of the extreme solar glare on Commonwealth Ave westbound. All parties declined medical attention. Damage visible to me were as follows: -Vehicle #1 sustained heavy front end damage, towed by Tody's -Vehicle #2 had moderate rear end damage, towed by Tody's -Vehicle #3' s right rear passenger side above the wheel well had minor damage, possible tire damage as well. Vehicle was moved off to the side of the road and owner has their own tow coming.

On arrival the operator of MV2 stated that the operator of MV1 had already left the scene after swapping papers because he was in a rush to pick up his daughter. The operator of MV2 stated she was stopped at a red light on Commonwealth Ave when she was struck from behind by MV1. MV2 sustained damage to the rear bumper and drivers side brake light area but was able to drive away from the scene. MV1 has unknown damage as it had already left the scene on my arrival. A query of the operator of MV1's MA Drivers License shows status suspended.

Narrative:
Officers were dispatched to the location of Commonwealth Avenue and Ash Street for a reported Motor Vehicle Accident. Officer Ciccone arrived on scene shortly before myself and stated there was no vehicle at this time at that location. Shortly after, I observed a red Ford Taurus traveling eastbound on Commonwealth Avenue turning onto Islington Road with a missing front bumper and airbags deployed (see incident #22005328). Operator of MV stated he was traveling westbound to a nearby hotel when he lost control of the vehicle and drove off the road. The MV sustained damage to the front of the vehicle and drove into the property of 58 Ash Street. The property had damage to the front lawn. All airbags were deployed. The Operator was taken to Newton Wellesley Hospital for injuries. The passenger had no injuries. The MV was towed by Tody's Service Inc. Due to the injuries I was unable to give the Operator (Anthony Earle) his Massachusetts Unifrom Citation (T1448313) for leaving the scene of an accident (property damage) Chapter 90 Section 24. I handed the citation to his girlfriend Kerry Materia.
