Operator of MV1(Ma reg 866MF0) stated he was traveling EB on Washington St when his vehicle was struck in the rear. Operator of MV1 stated he was stopped at the red light at the intersection of Jackson Rd. Operator of MV2(AMB1406) stated he was traveling EB on Washington St when he struck the rear of MV1. Operator of MV2 stated he observed a vehicle coming down from Lewis Terrace and was concerned they were not gonna stop at the intersection. This distraction caused MV2 to strike MV1 at a slow rate of speed. Both vehicles sustained minor damage and did not require a tow. The passenger and driver of MV1 were observed by the medics. The passenger was transported in the ambulance for undisclosed minor injury. The driver refused to sign the medical release. 5 Photos were taken of both vehicles showing the damage. I put the memory card in an envelope and left it for the I.T. Bureau.

VEHICLE #2 STATES SHE WAS DRIVING E-BOUND ON WASHINGTON ST. SHE WAS IN THE RIGHT LANE AND WHILE DRIVING WAS HIT BY VEHICLE #1. On Wednesday March 31, 2021 at approx 1430 hours Officer Panica responded to the area of Washington Street and Jackson Road for a report of a tractor trailer (car carrier) with New York registration that knocked over a traffic signal and kept traveling east on Jackson Road with out stopping. Officer Nardelli and I responded to the area as dispatch was giving updates of tractor trailer information and direction of travel. I traveled west on Watertown Street from Chapel Street and observed a car carrier traveling west approx four blocks west ahead of me. I activated my emergency lights and made a motor vehicle stop at Watertown Street at Nevada Street. I observed a trailer NY reg # BH32997 which dispatch confirmed this reg was the one involved in the crash. The name of the comapny is DIVERISIFIED AUTOMOTIVE INC of 6 RAMLAND ROAD ORANGEBURG N.Y. 10962 N.Y. apportioned tractor reg 17833PB / trailer NY reg BH32997, the driver was later identified as MA lic # SA0310636 Januel Ortiz d.o.b. 5-17-2001 of 111 Country Club BLVD apt. 201 Worcester MA 01605. Officer Panica confirmed the crash site location and damage to a City of Newton Traffic signal/footings/ sidewalk/retaining wall/landscape. Officer Panica was also approached by witness Erin McGinty of 233 Jackson Road(see statement below) I approached the car carrier on the passenger side and observed fresh damage on the trailer that would be consistent with striking the traffic signal. I then approached the driver of the car carrier Mr. Ortiz. I advised him of the crash and he stated he was traveling west on Washington Street and turned right (north) onto Jackson Road. He stated while turning his trailer struck the traffic signal knocking it over. I asked him why didn't he stop to assess damage or notify anyone. He stated he was looking for a safe place to pull over. He stated he continued north on Jackson Road then left onto Pearl Street and then left onto Watertown Street to drop off vehicles at the 445 Watertown Street,which is where I Pulled him over. I advised him I am familiar with the travel route he went and the length of Jackson Street is not a Heavily traveled area since the school is closed from the crash site, and he traveled approx a mile and a half until I pulled him over. I advised him I would be doing a level III inspection and advising him of my findings. Witness Ms. McGinty stated she heard the crash and observed the car carrier with NY reg # BH32997 have contact with the traffic signal and continue to travel north on Jackson Street without stopping. Therefore after observing damage/pictures and statements I issued in hand MA criminal citation # T2017488 in hand to Mr. Ortiz for Chapter 90 Section 24 LEAVING THE SCENE OF CRASH WITH PROPERTY DAMAGE City Ordinance 19-75 FAILURE TO USE CARE IN TURNING. Mr. Ortiz was explained the process. I took pictures of the damage to the car carrier, and crash site area that I will have downloaded at the I.T. Bureau.

On 7/1/2021, at 1830 hrs, I spoke with REGAN, AMY in the lobby of NPD HQ regarding a past motor vehicle hit and run accident. Ms. Regan reports; at 1800 hrs this afternoon, she was traveling eastbound on Washington St. and was yielding to oncoming traffic as she attempted to turn northbound onto Jackson Rd. when her Honda CR-V was struck from behind by a construction Front Loader. The operator, described as a thin, white male, with a beard, grey hat and Irish accent, motioned for Ms. Regan to pull over onto Jackson Rd. which she assumed was to exchange information. As Ms. Regan pulled onto Jackson Rd. the operator of the Front Loader continued eastbound on Washington St. towards the Massachusetts Turn Pike rotary. I observed damage to the rear bumper and tailgate of Ms. Regan's Honda CR-V, but the vehicle is operable. I provided Ms. Regan a report number and instructions on how to obtain a copy of this report.

Upon arrival both vehicles were off to the side of Washington St. Operator 1 (Anna Gianfrancesco) stated she was traveling straight ahead and was turning right onto Adams St from Washington St when the accident occurred. She stated that Vehicle 2 came into her lane of travel. Operator 2 (Francisco De Jesus Pereira) stated that he was traveling Westbound on Washington St and the Operator 1 was in the right turn lane on Washington St at Jackson Rd. He stated that Operator 1 came into his lane and side swiped his vehicle. Upon speaking with Operator 1 again, she stated that the accident "may" have occurred at the intersection of Washington and Jackson Rd but stated it was not her fault. She repeatedly stated she was trying to turn right onto Adams St. I observed heavy damage on vehicle 1's driver side panel. Both parties declined medical attention and neither vehicle was disabled and needed a tow. Based upon the investigation, Operator 1 is at fault due to changing lanes when she did not have the right of way.

Motor vehicle 1 (MV1), was traveling eastbound in the left lane on Washington Street when motor vehicle 2 (MV2), which was also traveling eastbound on Washington St., attempted to merge from the right lane to the left lane in order to take a left hand turn onto Jackson Road. As MV2 attempted to merge over, it clipped the rear passengers side tire of MV1. As a result of the crash, MV1 sustained minor damage, while MV2 sustained moderate front end damage.

On March 28th, 2022 at approximately 09:13 hours while assigned to N491 I responded to the intersection of Washington St @ Jackson Rd for a reported MV crash. On my arrival I located both involved vehicle E/B on Washington St @ Jackson Rd. Vehicle #1 was a 2019 Black Tesla X SUV, Ma reg. EV502N, operated by Jeffrey Cook. He stated he had just stopped on Washington St E/B due to the vehicle in front of him stopping by Jackson Rd. His vehicle was then rear ended by vehicle #2. Vehicle #2 was a 2020 Black Chev. Silverado PU, Ma reg. 2NBN74, operated by David Haber. HE stated he was behind vehicle #1 on Washington St E/B when vehicle #1 suddenly stopped in front of him causing him to crash into the rear of the vehicle. Vehicle #1 sustained heavy rear end damage. Vehicle #2 sustained heavy front end damage. Neither vehicle required a tow and no reported injuries at this time.
