On 7/9/2019 at approx 1406Hrs while assigned to 497 I responded to level 4A of the Newton Wellesley parking garage at 2014 Washington St for a report of a minor collision. Upon arrival I spoke with the operator of RI Reg FV705, Malorie CURLEY who stated she had parked her new work vehicle in the parking stall was exiting her vehicle to see if she was within the lines when the operator of Ma Reg 69EL83 Laurence HENDY pulled into the spot next to her, opened his door striking her truck, CURLEY stated as she was rounding the car HENDY proceeded to grab his door and slam it into her car several more times. CURLEY stated she confronted HENDY who refused to exchange information with her for the damage to her car and fled. CURLEY took pictures which she will provide in her own crash report. The front bumper of CURLEYS car had several spots where the white paint was peeled back , HENDYS vehicle had the matching white paint on the edge of his drivers side door. Although the damage was minor CURLEY requested a report for documentation as it was her work vehicle. i attempted to contact HENDY but no phone number is listed.

On Saturday 7/13/19 at approximately 1719 hours I was dispatched to the parking lot of 210 Nahanton Street for a report of a past hit and run. Upon arrival I spoke to the operator, Rachel Namutebi, who states that she parked her car at 3pm on 7/10/19 and when she came out on 7/11/19 at 11am she noticed that the passengers side door had been struck by another vehicle causing damage to her car. She states she spoke with building security and they stated that the video surveillance does not video that part of the parking lot.

On Friday, August 30, 2019, while assigned to Traffic unit N525, I responded to Burr Park (142 Park Street, Newton) for a motor vehicle crash involving a City of Newton Fire truck. The operator of Newton Fire Ladder 3 (MA: MF453453), Michael Pohlman, stated while trying to exit Burr Park from the Waverly Street exit, the middle driver side tire area of Ladder 3 made contact with a fixed cement pole. The pole is located at the entrance/exit and has a chain attached to it used to restrict access to the area. I observed minor damage to the driver side middle tire area and no damage to the pole. No injuries were reported by the occupants of Ladder 3. Photos were taken and submitted to the IT Bureau. Ladder 3 stated they were at the Burr Park permorming an inspection.

Owner of Motor Vehicle #1 stated that he was exiting the gym at Planet Fitness and he noticed that someone was leaving a piece of paper on his front windshield. He was approached by the man who told him that he witnessed a Gray Dodge Ram bearing MA Reg 9JGD90 strike his Jeep Grand Cherokee that was parked and unoccupied causing moderate rear drivers side damage. MA Reg 9JGD90 came back to a 2018 Dodge Ram 1500 Gray in Color and registered to a Kenneth Doucette out of 41 Thaxter Street, Newton MA. I was able to locate several telephone numbers from our in house system however those numbers were no longer in service. I made several attempts to make contact with Mr. Doucette at his residents and had negative results. N491 Officer D. Healy also made an attempt to make contact with Kenneth Doucette later on in the evening and had negative results. I left a voicemail message to the witness and have yet to receive a return call for questioning. This will be further investigated on my next tour of Duty. **UPDATE Wednesday 09/04/19** At approximately 10:00 a.m. I went to 41 Thaxter Street to once again attempt to make contact with Kenneth Doucette and had negative results. I left another voicemail for the witness Claudio Silva and had negative results. I then spoke to Traffic Officer M. Gaudet to inform him about this incident and he stated that he will follow up with this investigation. Traffic Bureau update (Officer Gaudet): On September 4, 2019, I attempted to make contact with the witness to the crash, Claudio Silva, with a negative result. I made contact with the registered owner of MV2, Kenneth Doucette (S80920300). Doucette stated he was operating MA: 9JGD90 at the time of the accident. Doucette stated he pulled into a parking spot next to MV1 and stated he didn't think he "hit it hard". Doucette stated a short time later he left the area and when he got back to work he observed damage to his own vehicle. Doucette stated he had damage to the front passenger bumper area. Doucette stated he returned to the crash scene to see if the other vehicle had damage but it was gone. Doucette was advised and provided Massachusetts Uniform Citation T2016422 in hand for Chapter 90, Section 24 (Leaving the Scene of Property Damage, MV).

There was a newborn in veh #1 so as a precaution, the baby and its mother was transported by Medics to Newton-Wellesley Hospital to get checked out. Passenger #1 complained of headache after Medics left but didn't want me to call for a transport. She stated she would take some Tylenol and see how she felt later on. Very minor damage to veh #1. The operator of veh#2 stated she was merging onto Rt. 9 East when she didn't realize veh #1 in front of her didn't proceed and that's when she struck the rear of the car. Upon arrival I spoke with the owner/operator of MV#1 who stated she parked her vehicle, went into the store and when she came outside her front driver side bumper was hit. There was moderate damage to the vehicle but she was able to drive it home. There were no injuries. I checked the department stores for surveillance, which neither the CVS or Marshalls had any exterior cameras facing the parking lot. There were no witnesses on scene and the owner of MV#1 did not see who hit her.

On 11/12/2019 at 12:26 hours, I responded to the Star Market parking lot located at 2040 Commonwealth Avenue for a motor vehicle pedestrian accident. The parking lot is a public/thru way that motorists use to access the Star Market along with other businesses. The weather at the time was cloudy and rainy and the parking lot was saturated. Prior to my arrival, the pedestrian involved left the scene and did not want to speak to the officer nor wanted medical attention. Ofc. Claflin then cancelled Cataldo Ambulance. The pedestrian, Mary Alexander, left her information with a store employee prior to leaving. I spoke to the operator of MV # 1 (Joshua Randel MD Lic.# R534440603244) who stated that he was travelling through the third row of the Star Market parking lot at an appropriate speed. He was then attempting to make a left turn near the entrance/exit of the store. He says that there was a large white van parked in the end handicapped spot towards the left of him. He says the van was sticking out in the spot limiting his visibility. As he was half way through his left turn and coming around the corner, he was not able to see a pedestrian pushing a shopping carriage. He then struck the carriage that was being pushed by Mary Alexander. He further says that the cart jolted Ms. Alexander and she fell down to the ground. At the time, Alexander was walking through the lot near the store entrance heading back towards the parked cars. She was not in the crosswalk at the time which was approximately ten feet down from her location. After, I attempted to go to Ms. Alexander's residence to follow up. I spoke to her friend who was bringing up Alexander's groceries and she stated that she decided to go to Newton Wellesley Hospital for treatment. I then spoke to Ms. Alexander in the lobby of the hospital. She stated that she was exiting the store through the lot with her shopping carriage. She believes she was approximately eight feet away from the door when all of a sudden she only saw red coming at her. She believes MV # 1 was speeding at the time. I asked her if she was struck by the vehicle or was the shopping carriage only struck. She replied that she did not know and only remembers falling to the ground after. She sustained a large bruise to the inside palm area of her right hand and her left knee was swollen. Digital photos of the scene were taken and pictures of Ms. Alexander's injuries were taken at the hospital. As far as possible video surveillance, I was told that the Loss Prevention Department has to access the video. On 11/13/19 at approximately 14:20 hours, I went in to the Star Market and spoke to the manager, Nidal Rajeh, inquiring about possible video surveillance. He said there is coverage in the parking lot and he was able to show me the video. A copy of the video would have to be done by Loss Prevention which I requested. I observed at the 12:12pm mark in the video, MV # 1 was parked five spots up from the third row of the lot that he was exiting. I also observed a white transport van parked in the end handicapped spot mentioned by the operator but it was parked correctly and not limiting visibility. Next, MV # 1 backs out of the spot and drives forward going at a reasonable speed. The pedestrian (Alexander) has already exited the store with her carriage and is walking at an angle toward the same third row of parked cars. As the pedestrian is walking towards the direction of MV # 1, it appears that the pedestrian should be visible to him. MV # 1 then starts to take the left turn around the corner without slowing or stopping and strikes the carriage head on. Due to the impact, Alexander then loses grasp of the carriage and it goes past her twisting her body around. She then hits and rolls up on the hood of the vehicle on her left side. MV # 1 then suddenly stops and immediately gets out and provides assistance to the pedestrian. Due to the video surveillance, Mr. Randel will be issued/mailed MA Uniform Citation # T2012347 for Ch. 90/14/A Failure to slow for a pedestrian.

On Sunday 11/24/19 at approximately 1730 hours while working in marked unit n499 I was dispatched to the parking lot on the rt9 side of 225 Boylston Street (Bloomingdales) for a report of a minor motor vehicle crash. Upon arrival I spoke with the operator of motor vehicle 2, GIRARD Kathleen, who states that she was driving from the 2nd aisle of parking spots into the main travel lane to attempt to exit the mall. When she attempted to turn left she didnt see motor vehicle 1 already in the lane of traffic striking the side of the vehicle. After speaking with GIRARD I spoke with the operator of motor vehicle 1, SONG Guang min. SONG did not speak english, but he corroborated what GIRARD had stated by using a translation app on his cell phone. GIRARD had damage to the front of her vehicle and SONG had damage to the drivers side of his vehicle. Both vehicles were still operable and were driven from the scene. Neither party was injured

Witness said between 0930 to 1430 hours this date she was out of the hotel running errands. Witness stated her husband's MV (MV1) was parked unoccupied within the lines of 2345 Commonwealth Ave (Marriott Hotel) between those hours. Witness stated when she came back at 1430 hours she observed new damage done to MV1. Witness said during that time period an unknown MV sideswiped MV1 and left the scene without giving information. Witness believes the unknown MV must have scratched their car while attempting to back up from their space. Witness said the minor damage (scratch to the rear passenger side) was not there before today. Witness does not know who may have caused the damage. No injuries reported, no tow required, and no security camera recordings. Witness was advised for the registered owner to contact their insurance.
