Narrative:
Driver #1 stated she was driving southbound in front of 437 Centre St in slow traffic when Driver #2 rear ended her. She stated Driver #2 got out of the vehicle, looked at her car and stated he did not see any damage. She asked for his license and registration which he declined to provide. Driver #2 then drove away. Vehicle #1 suffered minor damage to the rear and was driven from the scene. Driver #1 was able to provide me with a picture of the vehicle license plate and a description of Driver #2 which matches the RMV image of the registered owner of Vehicle #2. I attempted to locate a number for Driver #2 with negative results. I also contacted Waltham PD for information for Driver #2 and they were unable to provide anything further. I mailed Driver #2 Citation #2081383 for Chapter 90 Section 24 Leaving the Scene of Property Damage.

Operator of Motor Vehicle #1 stated that she was exiting the Newton Wellesley Hospital West entrance and was taking a right turn Eastbound onto Washington Street and did not see motor vehicle # 2. Motor vehicle #1 struck motor vehicle #2 at the rear passenger side door and rear passengers side quarter panel causing moderate damage. Motor vehicle # 1 sustained moderate front end damage. Operator of Motor Vehicle #2 stated that she was traveling Eastbound on Washington Street and was struck by motor vehicle #1 as she was attempting to turn on Washington Street. All parties signed a medical refusal forms and reported no injuries. All units cleared without further incident.

ON 6-11-21 AT APPROX. 0920HRS. WHILE WORKING N492 I TOOK A REPORT FOR HIT AND RUN PROPERTY DAMAGE. UPON ARRIVAL AT THE INTERSECTION OF CRAFT AND NORTH ST. I NOTICED THAT THE TRAFFIC SIGNAL POLE HAD BEEN KNOCKED DOWN. REPORTING PARTY STATED THAT AN UNIDENTIFIABLE MOVING TRUCK HAD TAKEN A RIGHT TURN FROM NORTH ONTO CRAFT AND TOOK THE POLE DOWN. MOVING TRUCK WAS NO LONGER IN THE AREA. DAEGLE WAS NOTIFIED TO RESET THE TRAFFIC SIGNAL POLE. CLEARED WITHOUT FURTHER INCIDENT.

Vehicle 1 was travelling westbound on Rowe St. Operator 1 had a medical emergency, possibly a diabetic issue. Operator 1 lost control of the vehicle, and crashed into Eversource pole 385/15. Operator 1 was transported to NWH by Newton Medic 2, with minor injuries. I observed heavy damage to the front left of vehicle 1. Vehicle 1 was towed by Tody's to their lot. Eversource was notified to fix the pole, and arrived on scene.

Operator of V1 stated she was traveling east on Washington Street, turned left onto Washington Terr and had contact with V2. Operator of V2 stated she just pulled out of the parking space on the westbound lane of Washington Street she was traveling straight and V1 turned into her vehicle. All parties advised of the process, and there were no reported injuries.

Narrative:
Operator of MV1 stated he was driving westbound on Homer Street when he looked down at his car radio and struck a MV2, which was parked unoccupied on the right side of the roadway in front of 301 Homer Street. The MV2 was located to the right of the fog line. MV1 sustained damage to the front passenger wheel and axle. MV2 sustained heavy rear end drivers side damage. Both vehicles were towed by David Donahue of Tody's Towing. Operator of MV1 declined medical attention. Operator of MV1 consented to perform Standardized Field Sobriety Tests. He stated he did not have any medical conditions that would prohibit him from completely the testing properly. He was taken to the side of the road on Homer Street that was well lit by a street light, and we used the fog line as a guide. Operator of MV1 completed the testing and it was determined that he was not under the influence. He did state that he had been up since 0630 and was fatigued while driving. Operator of MV1 was provided a ride home by N-493 and was issued Citation #T2079603 for 90/13-Impeded Operation (Using Radio while driving) and 89/4A-Marked Lanes Violation (Crossing fog line).
