Operator 1 states he was stopped in traffic on Centre St, when he was rear ended by vehicle 2. Operator 2 states she was in stop and go traffic and was traveling northbound while "changing the radio" when she collided into the rear of vehicle 1. There was minor damage to the rear bumper of vehicle 1, and significant damage to the back right wheel well. There was heavy front end damage to vehicle 2, and the front airbag was deployed. Vehicle 2 was towed by Tody's Towing. Both operator 1 and 2 signed patient refusals with Fallon Medics.

The operator of MV1 stated he was traveling westbound in the parking lot travel lane at Bloomingdale's's when MV2 drove out behind a dodge truck and struck the passenger side of his vehicle. The operator of MV2 stated he was driving across a parking spot, to another spot and as he entered the travel lane in the parking lot, he struck the passenger side of MV1 with the front of his vehicle. All occupants stated they were not injured. MV1 sustained significant damage to the passenger side of the vehicle. MV2 had major damage to the front of his vehicle. MV1 was able to drive away from the accident after proper paper work was gathered. MV2 called for a private tow, AAA.

Narrative:
On Tuesday October 12th at approximately 1252 hours I responded to the intersection of Rowe Street and Wolcott Street for a city vehicle accident involving a pedestrian. At the time of the accident the weather was sunny and the road conditions were dry. Rowe Street and Wolcott Street are both owned and maintained by the City of Newton. Upon arrival I spoke with the operator of the vehicle involved, identified as Parks and Recreation worker John Siratos (S50488498). John stated he was traveling southbound on Rowe Street and was stopped at the stop sign at the intersection of Wolcott Street. John stated he was looking straight ahead towards the direction he was attempting to travel and proceeded into the intersection to cross over Wolcott Street when he struck the pedestrian with his front bumper causing him to fall on the ground. John stated he was only traveling a couple miles per hour as he just accelerated from a stop. John stated he did not see the pedestrian because of the suns glare. I was able to speak to the pedestrian inside the ambulance receiving medical treatment. He was identified as Justin Anderson (S27026247). Justin stated he was jogging westbound on Webster Street and was crossing the intersection of Wolcott Street and Rowe Street, attempting to head westbound on Wolcott Street. Justin stated he was jogging on the sidewalk and entered the crosswalk to cross the street and was struck by the city vehicle in the right leg causing him to fall to the ground. Justin reported a minor thumb injury and a small scrape on his head. Justin stated his temperature was high and he was being transported to NWH by Medics as a precaution. Pictures of the city vehicle and the scene were taken and submitted to the IT Bureau. No damage to the city vehicle was observed or reported.

Narrative:
On 10/13/2021 at 18:33 hours I responded to 30 Myrtle St for a report of a single vehicle crash involving a motorcycle that had a driver and a passenger. Myrtle St is a public way in the City of Newton. Upon my arrival I spoke with Roberto Ventura who stated that he and his passenger Melissa Mendoza were ridding westbound on Myrtle St on his 2009 Yamaha Raider motorcycle. Ventura and Mendoza were both wearing DOT approved helmets at the time of the crash. Ventura stated that as he approached 30 Myrtle St he drove over a patch of compacted dirt and gravel that was approximately three feet wide and four feet long. The patch was not coved by asphalt and was dug out and filled by the City of Newton Utilities Department. The patch of dirt and gravel was not marked by any cones, barrels or barricades. There were no street lights in the area so the roadway was not lit. There was a similar patch approximately 10 feet east that had four cones around it. Ventura stated that he did not see the dirt patch until he drove over it. Ventura stated that after driving over the dirt patch that he did not see at approximately 20 MPH he lost control of the motorcycle. Both Ventura and Mendoza stated that he were then thrown from the motorcycle and landed on the roadway. Ventura complained about pain in his left shoulder and knee, and had road rash abrasions to his left leg and arm. Mendoza stated that she had road rash abrasions to her left arm and leg. Both Ventura and Mendoza declined medical treatment at the crash scene and singed patient refusal forms provided to them by Fallon medic. Ventura's motorcycle had damage to it's left side tail light, and handlebar, as well as scrapes on the left side. Ventura's motorcycle did not require a tow. I placed reflective cones around the dirt and gravel patch and had dispatch leave a message for Newton Utilities to pave over both patches.

Mv#1 operator stated she was travelling on Beacon St E/B in the area of #1200 when she stated she briefly looked down to touch a button on her front dash. At that time, #1 rear ended Mv#2 which in turn rear ended Mv#3. #1 operator signed a patient refusal. #1 sustained significant front end damaage and was able to be driven from the scene. #2 operator was slowing to a stop behind #3 when he was rear ended and pushed forward between both Mv's. #2 operator appeared to be significantly shaken up from impact and was transported by medics with unknown or any injuries. #2 sustained heavy rear end and front end damage, towed by Tody's. #3 operator stated he came to a stop on Beacon St E/B to allow a pedestrian to cross in a marked crosswalk when he was rear ended by #2. #3 operator signed a patient refusal. #3 sustained significant rear end damage and was able to be driven from the scene. Pedestrian/witness stated she was crossing in the crosswalk when the collision occurred. Pedestrian was not injured.

THEY STATED THEY HEARD THE VEHICLE CRASH BUT DID NOT ACTUALLY SEE THE ACCIDENT. VEHICLE #1 HAD EXTENSIVE FRONT RIGHT END DAMAGE AND WAS TOWED BY PRIVATE TOW. VEHICLE #2 HAD NO DAMAGE AND WAS STILL OPERATIONAL. ALL PARTIES REPORTED NO INJURIES. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES. CLEARED WITHOUT FURTHER INCIDENT.

On October16th, 2021 at approximately 10:13 hours while working N491 I responded to a MV crash in the area of Washington St @ Channing St. On my arrival I located both involved vehicles at the gas station at the corner of Washington St @ Thornton St. Vehicle #1 was a 2018 blue BMW X2, CT REG. AS41177 operated by a Narendra Roy. He stated he was going W/B on Washington St in the far right lane when vehicle #2 entered his lane causing the crash. Vehicle #2 was a blue 2018 Lexus LX570 MA plate HAYATI, operated by a Faten Akar. She stated she was travelling W/B on Washington St in the middle lane when she attempted to change lanes so as to get to a business at #341 Washington St when vehicle #1 struck her vehicle. She further stated prior to the crash she was already half in the other lane when vehicle #1 came from her rear and never saw him.

MV1 operator was driving the speed limit southbound on Grove St by the Recreation Road/Rt. 95 northbound on-ramp intersection. MV1 operator stated she observed MV2 stopped at the Rt.95 northbound off-ramp intersection's stop sign. MV1 operator stated as she was continuing on Grove St, she was "t-boned" on her driver side by MV2. MV1 operator said after she was struck by MV2, her MV struck the utility pole and landed on top of the southbound Grove St curb. MV1 operator reported no injuries at this time. MV1 passenger did not report any injuries but she and MV1 operator were transported to Newton-Wellesley Hospital for an evaluation by Newton Medics. MV1 sustained extensive driver side, passenger side, front end, and under carriage damage. All airbags were deployed and the front passenger door entrapped MV1 passenger. MV1 was towed by Todys. MV2 operator stated he had exited Rt.95 northbound at the Grove St exit. MV2 operator said he was attempting to drive straight on to Recreation Rd/Rt.95 northbound on-ramp. MV2 operator said he stopped at the stop sign at Grove St and looked both ways. MV2 operator stated he did observe MV1 driving southbound on Grove St at a reasonable speed. However, MV2 operator said he thought MV1 had stopped to let him go. MV2 operator said as he attempted to drive straight onto Recreation Road/Rt.95 northbound on-ramp, he struck the driver side of MV1. MV2 operator reported no injuries at this time. MV2 sustained extensive front end damage, and the driving wheel airbag was deployed. MV2 was towed by Todys.

On 10/18/2021, while assigned to N496, I, Officer Conary, responded to 22 Union Street for a MVA. Upon arrival, I met with Operator of MV1 who stated he was driving Eastbound on Union Street when MV2 pulled out and hit MV1. Operator of MV1 showed me only the rear damage but there was also damage to the right front bumper as well. Operator of MV2 stated that she was pulling out of a parking spot going Eastbound on Union Street when MV1 hit MV2. MV2 was about halfway out of the parking spot. Operator of MV2 said while she was pulling out she saw that MV1 was driving quickly down the street. A witness on scene said that he saw MV1 hit MV2. MV1 was driving quickly down the street before the accident. This witness attends Newton South. MV2 was towed by AAA. Both parties declined medical attention.

On 10/18/2021, while assigned to N496, I, Officer Conary, responded to Beacon St at Chestnut Hill Road for a MVA with airbag deployment. Upon arrival, I met with the Operator of MV1 who stated that he was traveling Westbound on Beacon Street when MV2 pulled out from Chestnut Hill Road onto Beacon Street right in front of him. Operator of MV1 stated that MV2 just pulled right out that he had no time to stop. The passenger present in MV1 stated that they were traveling Westbound on Beacon Street when MV2 cut in front of them and could not avoid a collision. Operator of MV2 stated that they were pulling out from Chestnut Hill Road turning left. They saw no other cars traveling on Beacon Street and continued to make the left turn when MV2 hit MV1. Witness who was running across the street heard MV1 attempt to stop (screeching tires) and then MV2 hit MV1. Witness only provided a phone number for contact. All parties were evaluated by medics and signed patient refusal. Operator of MV1 complained of a hand injury but was going to transport themselves to the hospital. Both vehicles were towed.
