Narrative:
On 10/13/21 at 08:50 hours, I responded to Washington Street & Brookside Avenue for a motor vehicle accident involving a bicyclist. The weather at the time was clear and sunny (70 degrees) and the road conditions were dry. The accident location is a t type intersection with Brookside Avenue having a stop sign in order to turn onto Washington Street. Washington Street is a four lane road with a painted divider. There is no crosswalk at the intersection as it appears it was recently paved. On arrival, the accident scene was already clear as the bicyclist and his bicycle were waiting on the sidewalk. The motor vehicle involved was parked further back on Brookside Avenue away from the intersection. I spoke to the operator of the motor vehicle identified as Balarama Thavanati. He was operating a 2012 Toyota Camry color white bearing MA reg. # 44VN32. He states that he was operating southbound on Brookside Avenue and stopped at the stop sign at Washington Street. He was attempting to make a left turn onto Washington Street. He states that he inched out after because you can not see the vehicles approaching you. Next, a vehicle stopped on Washington Street to let him out and he thought it was clear never seeing the bicyclist on the right of him. He says the bicyclist approached him very fast and it appeared that he never stopped the bicycle prior to entering the street from the sidewalk. Once he attempted to pull out for the turn he then made contact with the bicycle on the left side. Thavanati pointed to the approximate location where the bicycle came to rest and it appears he was pushed forward approximately three feet from the impact. I observed black scrape marks on the right side of the front bumper which appear to be from the bicycle tires. At this time, the bicyclist, Jacob Harrison, was being tended to by Fallon Medics and Jacob's mother (Juliet Harrison) was already on scene. Jacob stated he was fine but since he was not wearing a helmet and hit his head on the pavement his mother had him transported to Newton Wellesley Hospital by Fallon Ambulance. After, I spoke to Jacob in the emergency room along with his parents. He sustained whiplash to his neck and a scrape to his right inner thigh area. I then asked him what had occurred during the collision. He states he was bicycling on the sidewalk eastbound on Washington Street. He says he stopped at Brookside Avenue and saw the Toyota Camry which was also stopped. He thought it was clear to proceed as if the vehicle was going to stay stopped. He then pedaled into the street and was struck by the Camry on the left side of his body. He fell down and he does not recall what else occurred as the incident happened so fast. The bicycle sustained damage to the front tire rim as it became bent. No other visible damage. At the time, Jacob was wearing black shorts and a white t-shirt with a red picture on it. He was not wearing a helmet and was going against the traffic on Washington Street. On scene there were three witnesses who were walking southbound on Brookside Avenue near Washington Street at the time of the crash. There names were Christine Butera, Marlene Austen, and Marta Travers. All three of them stated that the operator of the Toyota Camry was stopped on Brookside Avenue waiting to make the left turn. He then pulled out and in a split second the bicycle entered the intersection and was struck. They did not have the vantage point to see if the bicyclist ever stopped first before entering the roadway from the sidewalk. Digital photos were taken of the scene.

MV#1 was stopped and unoccupied in the driveway at #115 Rumford Ave when its passengers side rear quarter was stuck by MV#2. The operator of MV#2 stated that he was traveling east on Rumford Ave and because of solar glare did not see the rear end of MV#1 in the roadway when crash occurred. No injuries and MV#2 arranged for his own tow. Photographs of both MV's taken at the scene.

Vehicle 1 was parked in the lot at Extra Space storage at 137 Rumford Ave, at 1300 hours. Between 1430 and 1730, vehicle 2 struck the right side of vehicle 1. Extra Space storage stated that they have the incident on surveillance footage, but refused to share it, without a police report. The owner of vehicle 1 has already filled out their own accident report, and submitted it to their insurance company. Traffic Bureau update (Officer Gaudet): On Friday, October 22, 2021, I met with Mr. Brian Pendleton from Extra Space Storage. Mr. Pendleton was able to provide me with surveillance footage from the crash. A white passenger van facing (W) in the parking lot is observed backing out of a parking space. The van's rear passenger side bumper area is observed crashing into the rear driver side of MV1. MV1 is parked unoccupied on the opposite side of the parking lot (facing S). MV1 is observed briefly moving side to side as a result of the collision. MV2 is then observed leaving the parking lot area through the main entrance on Rumford Avenue. After reviewing the footage, I am unable to decipher the characters on the license plate of the fleeing vehicle due to the quality of the image. The van is a newer model passenger van with black lettering on the rear driver side area. The operator of the vehicls appears to be a white male with a dark colored baseball cap and a dark colored jacket. The media clip provided by Extra Space Storage was submitted to the IT Bureau to be attached to this report.

On Thursday October 21st, 2021 at approximately 1114 hours I responded to the area of Commonwealth Ave and Auburn Street for a motor vehicle accident involving a city vehicle. At the time of the accident the weather was partly cloudy and the road surface was dry. Commonwealth Ave and Auburn Street are both owned and maintained by the City of Newton. Upon arrival I spoke with the operator of the city DPW truck MA CON REG: M2940A, identified as Joseph Driscoll (S99322981). Joseph stated he was stopped at the red light in the Commonwealth Ave carriage lane waiting for the green light to cross over Commonwealth Ave onto Auburn Street. Joseph stated when the light turned green he began entering into Commonwealth Ave when his vehicle made contact with another vehicle. Joseph stated he had the green light and was traveling straight and did not see the other vehicle until contact was made. Joseph reported no damage to the DPW truck and reported no injuries. I spoke with the operator of the other vehicle involved, identified as Thomas Sullivan (S53107703). Thomas stated he was operating MA REG; W25387 and was traveling northbound on Auburn Street and had the green light to enter Commonwealth Ave. Thomas stated he was turning left onto Commonwealth Ave when his vehicle was struck in the rear passenger side area. Moderate damage was observed and reported on the rear and passenger side. Thomas reported no injures. Pictures of both vehicles were taken and submitted to the IT Bureau. Both vehicles were able to leave the scene without further incident.

MV#1 was stopped in traffic on Beacon St. MV#2 was moving forward in traffic and struck MV#1 in the rear bumper. Both operators stated that the traffic was heavy and that MV#2 struck MV#1. MV#2 was a school transport van with three children on board. Parents were called, arrived on scene, and signed patient refusals for all children. Both vehicles were driven off scene. No other injuries occurred.

MV#1 was parked and unoccupied at the rear of the Franklin School, #125 Derby St on 10/18/21. According to school staff, sometime around 12:30PM another MV that was parked in the rear lot attempted to back out of their parking spot and struck MV#1. School staff states the crash is on video and they are trying to determine the identity of the operator. The operator of MV#2 is believed to be a visiting nurse. Request this hit & run to be followed up by the Traffic Bureau. Traffic Bureau update (Officer Gaudet): On Tuesday, October 26, 2021 I made contact with the Franklin School to follow up with this crash. The staff there stated they have surmised the operator that left the scene with out exchanging information was a Covid tester that has been assisting with elementary schools in the city. The only information they had was that his name was Alan. The staff stated they were in the process of figuring out what school Alan was going to be at today and they would provide him with my contact information. A short time later, I received a phone call from a party named Alan Chua. Mr. Chua stated he was the operator involved in the crash on 10/22/21 at the Franklin Elementary School. Mr. Chua stated he was backing out of a spot in the parking lot behind the school when the rear of his vehicle crashed into MV1, which was parked and unoccupied in the lot. Mr. Chua stated he had an engagement he had to go to so he left the area and did not stop to exchange information or attempt to find the owner of MV1. Mr. Chua stated he is not sure if there is any damage to his vehicle as a result of the crash. Mr. Chua reported no injuries as a result of the crash. Mr. Chua was advised and will be mailed Massachusetts Uniform Citation 173173AB for Chapter 90, Section 24 (Leaving the Scene of Property Damage) and Newton City Ordinance Chapter 19, Section 75 (Failure to Use Care).

Narrative:
I responded to 234 Langley Road for a two car M/V crash with entrapment. Upon arrival with other officers, Fire, and medics I saw the following. MV1 was flipped over on the road way and the operator was still in the vehicle, but the passenger was outside the vehicle. MV1 Hit a parked MV2 unoccupied. MV1 was traveling north on Langley Rd when MV1 swerved to its right and made contact with the left rear bumper of the MV 2. The impact of the two MV's caused MV1 to flip over and land on its roof. Based on my observation the scene indicated : day light, clear and there were no road defects. I took numerous pictures which were submitted to IT bureau so they can be attached to this report. Both MV1 driver and passenger were taken to Beth Israel Hospital in Boston. Tody's arrived on scene and took possession of both MV's. A canvas of the area resulted with one neighbor at 255 Langley who witnessed the accident. Mr. Vish Showdhary stated that he saw MV1 driving on Langley Rd when MV1 crashed into the parked MV. He stated that there were no other vehicles on the road and that MV1 drove right into the parked vehicle. The scene was cleaned and I went to Beth Israel Hospital to speak with the operator and passengers of MV1. The operator of MV1 stated that they have no recollection of what happened. She stated that she does not know how she hit the parked MV and that the last thing she remembers is being pulled out of the rolled over MV by the Fire Department. The passenger in MV1 stated that he does not remember how the crash occurred. He stated that he was unsure how MV1 crashed into MV2. He stated thathe was able to get out of the MV without a problem. After a review of the crash I am citing MV1 with the following. M.G.L 89/4A Marked lanes and M.G.L 90/24E Negligent Operation.(Citation # 1445881) Furthermore a request for immediate threat license revocation was filled and submitted to the RMV. The MV Citation was delivered to the operator of MV1 in hand at the Beth Israel Hospital.

The operator of MV#1 states that he was traveling south on Washington St directly behind MV#2 in the middle lane approaching the Mass Pike off ramp. According to operator #1, MV#2 veered to the left at the island and then without warning turned to the right crashing into MV#1. Very minor damage to drivers side tire on MV#1. The operator of MV#2 states he was traveling south on Washington St in the middle travel lane and at the island used his directional to turn into the right hand lane when crash occurred with MV#1. Damage to front quarter on passengers side and MV was towed by Todys at owners request. The operator of MV#2 provided MA Reg. 7ZP982 as a witness. MV did not match description and owner stated to me when contacted that he did not witness a crash in Newton today. No injuries and no citations issued.

The operator of MV1 stated she was backing out of her fathers driveway at 73 Benningston St when she struck MV2 in the front driver side. When she struck MV2, the operator stated she got scared, put the vehicle in drive and stepped on the gas. When she stepped on the gas she went onto the neighbors lawn, through the bushes, striking MV3 that was parked. The vehicle rested behind MV3 at an angle, in between the stairs to the residence at 69 Benningston St. The operator stated she is pregnant and was checked out by the medics and signed a refusal. The vehicle was towed to Todys due to it being inoperable. The owners of MV2 and MV3 were home and notified of the accident. The operator of MV1 has her MA permit but was operating the motor vehicle with no licensed operator. She was cited for unlicensed operation and given the citation in hand.

On October 26th 2021 while assigned to the front desk, Hanna Yoon DOB 9/9/83 came in to report a minor past mva/pedestrian. Yoon states that around 0820 hrs today, she was pulling out her driveway at 357 Langley Rd (Ma reg 5HL884). Yoon did not see a pedestrian approach from the right, and contact was made at a slow rate of speed. Yoon states the pedestrian did not fall down. She believes that the front right part of the vehicle hit the person's left thigh. Yoon asked the pedestrian if she was ok, to which the woman stated she was fine. The woman asked for and was provided Yoon's cell phone number before walking away. Her clothing description is unknown. Around 1100hrs Yoon then received a text from 617-913-5141. The person texting identified them self as the woman who was hit, and stated she was in the hospital with leg pain. She then texted that the hospital would contact Yoon for more information. Yoon did not respond and has not been contacted by anyone since this text. A name was not provided by the texter. There are no entries in QED indicating that the unknown pedestrian has called to report this accident. I tried to call the number provided with negative results. At this time, there is no identifiable victim. Yoon was advised that I will document her statement, and in the future it is best to call police to the scene of a crash in order to avoid confusion. Update by Lt. Dangelo: On 10/27/21 at approximately 0830 hrs. the pedestrian from the accident, Woo Jin Seo came into HQ with her mother to report the accident. She stated that on 10/26/21 at approximately 0820 hrs. she was walking to Newton South High School from 383 Langley rd. when a vehicle backing out of a nearby driveway bumped her. She stated that she did not fall down, but did suffer an injury to her leg. She stated that the operator of the vehicle stopped and checked and left her phone number. Ms. Seo went to Beth Israel Lahey urgent care the same day for leg and shoulder pain. I contacted the operator Hanna Yoon and updated her.
