Mv#1 (work truck) was parked unoccupied along the curb facing E/B in front of 1248 Beacon St. Truck was parked at this location for construction work being performed on above property. Workers on site heard a crash but were unable to observe Mv#2 (unknown) that was travelling on Beacon St E/B passing by and sideswiped #1. Crash debris left behind ripped off from #2 after sideswiping #1 was found on the street, a black passenger side mirror with "Honda" embossed inside the mirror. Also found inside the mirror was a sticker with possible QR code with the following characters: 15-10-17-043227 L13 4130 47088RC #1 sustained significant damage to the driver's side rear utility box compartment door. Video obtained from 1248 Beacon was negative results.

ON 8-31-21 AT APPROX. 0908HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT 309 WALTHAM ST. I SPOKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES HE WAS TRAVELING S-BOUND ON WALTHAM ST. HE STATES A TRACTOR TRAILER WAS BACKING INTO FAIRWAY DR. AND BLOCKING THE ROAD. DRIVER DECIDED TO TURN AND HEAD NORTH ON WALTHAM TRAVELING A DIFFERENT ROUTE. WHILE TURNING DRIVER STATES HE MISJUDGED WHERE HE WAS AND BACKED INTO THE RAMP OF VEHICLE #2 THAT WAS PARKED IN FRONT OF 309 WALTHAM ST. DRIVER #1 HAD HIS VEHICLES REAR WINDSHIELD SMASHED AS A RESULT OF HIS CRASH. VEHICLE #2 IS A RYDER BOX TRUCK THAT WAS PARKED IN FRONT OF 309 WALTHAM ST. I SPOKE TO PIECE BY PIECE MOVERS FOREMAN ( PABLERIO RENAS ). HE STATES HIM AND HIS CREW WERE NOT IN THE VEHICLE AT THE TIME OF CRASH. HE SAW THAT VEHICLE #1 HAD BACKED INTO THE RAMP AND HAD REAR WINDSHIELD DAMAGE. RENAS STATES THERE IS NO DAMAGE TO VEHICLE #2. BOTH PARTIES STATED THERE WAS A TRACTOR TRAILER BACKING INTO FAIRWAY DR. THAT HAD BLOCKED THE ROADWAY AT THE TIME OF THE CRASH. UPON MY ARRIVAL THERE WAS NO TRACTOR TRAILER IN THE AREA. ALL PARTIES REPORTED NO INJURIES. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES. CLEARED WITHOUT FURTHER INCIDENT.

Narrative:
On 08/31/2021 at 17:02 hours I was dispatched to Hammond St at Lawrence Rd for a report of a MVA involving a van and a bike. Hammond St and Lawrence Rd are both public ways in the City of Newton. Upon my arrival I spoke with the operator of vehicle one Evandro Ribeiro. Ribeiro stated that he was driving his van ( MA commercial registration W13024 2016 Ford Transit van ) south bound on Hammond St near Lawrence Rd. Ribeiro stated that as he was turning right onto the west bound lane of Lawrence Rd a bicyclist struck the front passenger side door of his van. Ribeiro stated that he activated the right turn signal of his van before turning right onto Lawrence Rd. Ribeiro stated that he did not see the bicyclist prior to the crash. Ribeiro stated that he was not injured in the crash and his van had no damage to it. I also spoke with the bicyclist Kevin Recor who stated that he was riding his bike south bound on Hammond St near Lawrence Rd when Ribeiro's van turned to the right in front of him. Recor stated that he did not recall if Ribeiro's van had it's right turn signal activated before it turned. Recor stated he did not have enough time to stop before the crash. Recor suffered minor lacerations to his left arm and left leg as a result of the crash. Recor was evaluated by Medics at the crash scene and refused further medical aid. I took photos of Ribeiro's van and Recor's bike and the disk was downloaded by the NPD's IT Bureau. Based upon statements made to me by Ribeiro and Recor I issued Ribeiro MA Uniform Citation T2016946 and cited him for a violation of MGL 90/14, failure to yield to a bicyclist when turning right.

Operator of MV1 stated they were heading Westbound on California St. when they fell asleep at the wheel and veered off and struck a tree on the eastbound lane sidewalk. MV1 rolled over.The final resting place of MV1 was positioned Southbound in front of 268 California St. MV1 sustained major damage to the front of the vehicle. Both the front and side air bags were deployed. Operator of MV1 sustained minor injuries and was transported to St. Elizabeths. Passenger of MV1 signed a patient refusal. Photos were taken and submitted to the IT Bureau. Tody's towed the vehicle. A towed motor vehicle inventory form was filled out. It should be noted the vehicle was a Zipcar. Zipcar was notified of the accident. The insurance company is Sedgwick Insurance (866)-345-2474.
