On 02/07/2019, while assigned to N494, I, Officer Conary, responded to a MVA at Franklin Street and Waverly Ave. I met with the Operator of MV1 who stated that he was driving Northbound on Waverly Ave when MV2 came into his lane from Franklin Street and hitting him on the left side. Operator of MV2 stated that he was stopped at the stop sign at Franklin Street and proceeded to take a left turn onto Waverly Ave. At this time, MV2 thought MV1 had a stop sign and was going to stop. MV2 hit MV1. There is no stop sign on Waverly Ave at Franklin Street. Operators of each vehicle were offered and declined medical attention. Operator of MV2 was a Lyft driver and was driving three others in the vehicle. Two occupants were offered and accepted medical attention. Both were transported to Newton Wellesley hospital. The other occupant (Paige) declined medical attention. She had a air cast on from a previous injury. MV1 could not be driven from scene. It was parked legally and safely on the side of the road. Operator of MV1 will get a tow later on. MV2 was able to be driven from scene. Operator of MV2 continued to drive Paige home. No further incident to report.

Narrative:
On 03/07/19 at 08:29 hours, I responded to a two car motor vehicle accident at Needham & Oak Streets. The operator of M/V # 1 states she blacked out upon impact of the accident. She was immediately transported to Newton Wellesley Hospital by Cataldo Ambulance for a possible head injury. After at the hospital, Operator of M/V # 1 states she was traveling straight ahead (WB) on Needham Street at Oak Street with the green light signal in front of her. As she approached the intersection, M/V # 2 suddenly attempted a left turn from the opposite side of the road and she was not able to stop in time. M/V # 1 sustained passenger front end damage and the vehicle was towed by Tody's as the operator was transported to the hospital. Operator of M/V # 2 states she was in the left turn lane on Needham Street facing eastbound attempting to take a left turn on to Oak Street. She states that she thought it was clear to proceed and M/V # 1 came out of no where and struck her on her rear passenger side. At first, the operator told me the signal light was green and then after stated that the light was changing to red when she was proceeding through. The vehicle sustained rear passenger side rear wheel and axel damage and it was towed by Tody's. Based on both operator statements, it appears that the operator of M/V # 2 did not yield the right of way and attempted to turn left when it was not clear to proceed. The operator was issued and mailed MA Uniform Citation # T1441703 for Ch. 89/8.

Operator of MV1 states she was stopped at the northbound light on Chestnut St at the intersection of Elliot St. She states the light turned green and she proceeded northbound through the intersection. MV2 was heading southbound on Chestnut St. Operator of MV1 states MV2 did not yield, and turned in front of her. She did not have time to stop and the two vehicles collided. Operator of MV2 states he was heading southbound on Chestnut St, turning left onto Elliot St. He states he did not see MV1, and turned left. While turning, MV2 collided with MV1. Witness states she was behind MV2 at the light. She states MV1 continued into the intersection when the light turned green. She states MV2 turned into MV1 without yielding. Operator of MV2 was issued citation #T1444579, for 89/8, fail to yield while turning left. MV1 had airbag deployment. Operator of MV1 was transported to Newton Wellesley hospital with apparent minor injured. Operator of MV2 signed a patient refusal, as did the passenger of MV1. Both vehicles were towed by Todys.

On 8/28/2019 at approx 1058hrs while assigned to 497 I responded to the area of 2014 Washington St for a report of a 2 car crash w/o injuries. Upon arrival I observed the vehicles on opposite sides of Washington St with NFD Lt Bourgeois standing by with his emergency lights activated, Lt Bourgeois stated both operators were shaken up but not injured. I spoke with the operator of Ma Reg 1XM680 Elizabeth CHISOLM who stated she was attempting to take a left onto Washington St, a vehicle in the right hand eastbound lane was turning right into the parking lot, CHISOLM stated she did not see Ma Reg EV3452 being operated by Julien GRANT in the left eastbound travel lane until it was too late colliding with him. I spoke with GRANT who stated he was traveling EB in the left travel lane of Washington St when struck by CHISOLM exiting the hospital.

Narrative:
On Saturday, September 28, 2019, I responded to the intersection of Crafts Street and Albemarle Road, Newton with Sgt. Lee for a report of a motor vehicle crash involving a motorcycle with serious injury to it's operator. The weather at the time of the crash was clear and sunny. The road surface was dry. Crafts Street and Albemarle Road are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jussara Pontes (S92930243). Pontes stated she was operating a 2017 Toyota Sienna (MA: 4GX777) at the time of the crash. Pontes stated she was traveling North on Albemarle Road and came to a stop at the stop sign located at the Crafts Street intersection. Pontes stated she looked both ways and began to inch out into the roadway. Pontes stated she stopped the vehicle again and was assisting a child sitting in the 2nd row of the vehicle. Pontes stated she then turned her attention back to the roadway. Pontes stated she looked to her right and left again and did not see any vehicles in either direction. Pontes stated she proceeded to cross the intersection to continue North on Albemarle Road. Pontes stated the front bumper area of MV1 crashed into the left side of a passing motorcycle. Pontes stated the motorcycle and it's operator fell to the roadway and slid across the street to the side of the road. Pontes stated she pulled her vehicle over onto Albemarle Road. Pontes stated she did not see the motorcycle prior to the crash and was not distracted. The operator of the motorcycle (MV2), Irene Pistoftzian (S06099449) was transported to Brigham and Women's Hospital with a serious left leg injury prior to my arrival. I observed a red 1990 Kawasaki ZR550 (MA MCN: 1V1063) resting on it's right side on the NE sidewalk area of Albemarle Road at Crafts Street with it's front wheel/handlebar facing Crafts Street. The motorcycle had moderate damage to it's left side from the impact of MV1 crashing into it. The right side of the motorcycle had damage from sliding on the roadway and the crashing into a fence on the sidewalk on Crafts Street (W) at Albemarle Road. There was also damamge to the front headlight/right handlebar area. The motorcycle and Toyota Sienna were towed from the scene by Tody's towing at Sgt. Lee's request. Pictures were taken of the scene and submitted to the IT Bureau. I spoke with Pistoftzian at Brigham and Women's Hospital in Boston on Monday, September 30, 2019. Pistoftzian stated she was operating her motorcycle on Crafts Street (W). Pistoftzian stated as she approached the intersection of Crafts Street and Albemarle Road, she observed a mini van to her left out of her peripheral. Pistoftzian stated she remembers being hit on her left side and then laying on the roadway looking up at the sky on the sidewalk with her head facing Albemarle Field. Pistoftzian stated as a result of the crash she has a significant injury to her left leg and also has an injury to her left wrist/arm. Pistoftzian stated she was wearing a helmet at the time of the crash. I spoke with Kymberley Mitchell via telephone who stated she witnessed the crash. Mitchell stated she was traveling behind the motorcycle involved in the crash on Craft Street (W) at North Street. Mitchell stated they were stopped at a red light at this time. Mitchell stated there were no vehicles in front of the motorcycle at the light. Mitchell stated the light turned green and they continued on Crafts Street at a low rate of speed. Mitchell stated she could see ahead at the intersection of Albemarle Street and Crafts Street, traffic was stopped on Crafts Street (E) letting traffic exit Albemarle Road (N). Mitchell stated vehicles were taking left and right turns out of Albemarle Road before the motorcycle reached the intersection. Mitchell stated a mini van was crossing past the double yellow line of Crafts Street to continue straight when it crashed into the left side of the motorcycle. I spoke with Cheryl Wermer via telephone who stated she witnessed the crash. Wermer stated she was traveling on Crafts Street (E) towards Albemarle Road. Wermer stated traffic ahead of her was stopped to allow vehicles to turn off of Albemarle Road. Wermer stated she observed a motorcycle traveling towards her on Crafts Street (W). Wermer stated she then saw a vehicle travel straight from Albemarle Road across Crafts Street and crash into the motorcycle. Wermer stated the operator of the motorcycle flew off the bike. Wermer stated the car then came to a stop and the driver got out and stated "Oh my god! I didn't see you!" Wermer stated both vehicles were traveling at a low rate of speed prior to the crash. I spoke with Bill Frabizio via telephone who stated he witnessed the crash. Frabizio stated he was behind the mini van (MV1) on Albemarle Road prior to the crash. Frabizio stated the vehicle in front of him began to cross the intersection as he was monitoring the traffic on to take a right turn onto Crafts Street. Frabizio stated he heard a loud "Boom" and saw a motorcycle slide across the street with its operator. Frabizio stated the motorcycle crashed into a fence post and the operator came off the motorcycle. Frabizio stated he pulled over and assisted operator of the motorcycle. After concluding my investigation, the operator of MV1, Jussara Pontes, was mailed Massachusetts Uniform Citation T2016424 for Newton City Ordinance Chapter 19, Section 75 (Failure To Use Care).

AT THIS POINT DRIVER STATES SHE ENTERED THE TRAFFIC LANE AND TURNED ONTO CRAFTS ST. SHE STATES HER VEHICLE WAS HIT IN THE LEFT SIDE BY VEHICLE #1. SHE STATES SHE NEVER SAW THE MOTORCYCLE UNTIL IT WAS HITTING HER VEHICLE. OPERATOR OF VEHICLE #1 WAS BEING ATTENDED TO BY THE FIRE AND MEDICS. OPERATOR OF VEHICLE #2 WAS GIVEN MOTOR VEHICLE CITATION ( T1445177, 89/9, FAILURE TO YIELD AT INTERSECTION ). OPERATOR SERVED CITATION IN HAND. I SPOKE TO A WITNESS ( TIMOTHY CHANGOGARRETT ). HE STATED HE WAS TRAVELING E-BOUND BEHIND THE MOTORCYCLE. HE SAW VEHICLE #2 ENTER THE TRAFFIC LANE AND VEHICLE #1 HIT HER VEHICLE. HE STATES THE OPERATOR OF VEHICLE #1 WENT OVER HIS HANDLE BARS UPON COLLISION. LANDED ON THE HOOD OF VEHICLE #2 AND THEN FELL TO THE GROUND. AT THIS POINT HE CALLED 911. OPERATOR OF VEHICLE #1 WAS TRANSPORTED TO BRIGHAM AND WOMEN'S HOSPITAL BY MEDIC2 FOR FURTHER TREATMENT. I WENT TO THE HOSPITAL AND SPOKE TO THE OPERATOR IN HIS ROOM. HE STATES HE WAS TRAVELING E-BOUND ON CRAFT INTENDING TO GO TO THE POST 440 ON CALIFORNIA ST. TO ATTEND AN AA ( ALCOHOLICS ANONYMOUS ) MEETING. HE STATES HE TOOK HIS SON'S MOTORCYCLE WITH THE INTENTION OF SEEING IF THERE WERE ANY REPAIRS NECESSARY BECAUSE HIS SON IS TRYING TO SELL IT. WHILE TRAVELING HE APPROACHED THE INTERSECTION. HE SAW VEHICLE #2 ENTER ONTO CRAFT ST. HE STATES HE COULD NOT DIVERT AND HE WAS UNABLE TO AVOID HITTING HER VEHICLE. OPERATOR HAS BROKEN BONES IN HIS LEFT ARM. CUTS ALONG HIS FACE AND NECK AS WELL AS LOWER BODY PAIN. HE STATED THE HOSPITAL INTENDS ON KEEPING HIM OVER NIGHT FOR OBSERVATION. I ASKED THE DRIVER IF HE WAS AWARE THAT HIS LICENSE TO OPERATE A MOTOR VEHICLE WAS REVOKED. HE STATED HE HAD A CHANCE TO BE REINSTATED BUT CHOSE NOT TO. DRIVER WAS ISSUED MOTOR VEHICLE CITATION ( T1445178, 90/23/C, LICENSE REVOKED AS HTO ). I INFORMED THE OPERATOR THAT I WOULD MAIL HIM THE CRIMINAL APPLICATION. ALL OTHER PARTIES REPORTED NO INJURIES. BOTH VEHICLES WERE TOWED BY TODYS. VEHICLE #1 HAD FRONT END DAMAGE AND FRAME DAMAGE. VEHICLE #2 HAD LEFT SIDE DAMAGE AND A BROKEN AXLE. WEST NEWTON SQUARE ( OFF L. MIKOLEIT ) ARRIVED ON SCENE AND TOOK 10 PICTURES. PICTURES PLACED IN I.T. BOX. I CLEARED THE AREA WITHOUT FURTHER INCIDENT.

On May 17th, 2020 at approximately 14:26 hours while working N491 I responded along with NFD and Medic One to a mv crash at the intersection of Watertown St @ CRAFT St. On my arrival both the Fire Dept. and Medics were assessing the operators of the two involved vehicles. Operator from vehicle #1 a 2014 black Jeep identified as Mark Mccullough was transported from the scene by ambulance to Newton Wellesley due to his injuries. He did give a statement prior to being transported.. He stated he was going N/B on Craft St when vehicle#2 suddenly made a left turn in the intersection towards Watertown St causing the crash. Operator of vehicle #2 reported going S/B on Craft St and was attempting to turn left onto Watertown St when vehicle #1 crashed into him in the intersection. Witness id as Eamen Keane stated he was behind vehicle #1 on Craft St when he observed vehicle #2 turn left in front of vehicle #1 causing the crash. I cited operator #2 for failing to yield at an intersection C89/S8.additionally charged him with operating his mv without a valid license C90/S10. He did possess a Guatemalan license. His vehicle was registered to him with an address of #59 Chester Ave Waltham Ma. e . He was additionally charged with operating his vehicle on a public way without being properly licensed.C90s10.Citation #T2014121. Both vehicles were towed by todys. Both drivers sustained injuries but only operator #1 went to the hospital. Operator #2 refused. All above mentioned streets are public ways and maintained by the city of Newton DPW.

On Monday, August 10th 2020 at 0459 hours, I responded to the intersection of Brookline St at Vine St for a one vehicle accident into a City Of Newton Tree. The vehicle was a Black Honda Accord Mass Reg # 7DZ958. Upon arrival, Samuel Perez was outside the vehicle and no operation of the vehicle was witnessed. There was no drugs or alcohol inside the vehicle. The vehicle appeared to have been operated heading west on Vine St. The vehicle continued through the stop sign at Vine St straight into Brookline St where the vehicle collided into a City of Newton Tree. Pictures were taken and submitted to IT. The airbag was deployed. Perez complained of chest and head pain. Perez was taken by Ambulance to Newton Wellesley Hospital. There was extensive damage to the front of the vehicle. Tody's Towed the vehicle. A Towed Motor Vehicle Inventory form was completed. Perez was mailed Mass Uniform Citation #T1273442 in the amount of $ 105 for M.G.L Ch. 89/9 Failure to Stop at Stop Sign.

The OP. of MV#1 states he was on the westbound lane of Washington Street ready to make a left turn into Newton-Wellesley Hospital. He had a green light and didn't see any oncoming traffic and proceeded to make the left turn. That's when MV#3 approached at a high rate of speed and they collided together. On Tuesday, October 6, 2020, I received a call at approximately 2158 hours from Sgt. Devine of the Newton Police Traffic Bureau. Sgt. Devine stated there was a motor vehicle crash with serious injury that had just occurred at the intersection of Crafts Street and Waltham Street in Newton. Sgt. Devine stated that one of the parties involved had suffered life threatening injuries and was transported to Brigham and Women's Hospital. I responded to the scene and conducted an investigation of the crash with the assistance of Sgt. Devine and Captain Boudreau. Upon arrival it was reported to me by officers on scene that a party operating a motorized scooter on Crafts Street (E) had been struck by a 2007 Chrysler Town and Country (MA: 5EE316). The Chrysler was attempting to make a left turn from Crafts Street (W) onto Waltham Street (S). The operator of the motorized scooter, identified as Nadhir Tchikou (DOB: 11/23/1993), had suffered serious injuries to his head and left leg area and was transported to Brigham and Women's Hospital. It was reported to me that Officers Conary, Boyle, and Donovan were the first to arrive at the crash and provided life-saving care to Mr. Tchikou prior to being transported to Brigham and Women's Hospital. Officer Donovan responded to Brigham and Women's to keep us updated on Mr. Tchikou's status. The update provided to me while on scene was that Mr. Tchikou was in critical condition and in surgery. Due to the seriousness of the crash and potential fatal injury, Sgt. Devine contacted the Massachusetts State Police to assist in the investigation. Sgt. Mahoney and Trooper Paul responded as part of the Massachusetts State Police Crash Reconstruction team and conducted a supplemental investigation of the crash (MSP report 2020-CAR-000285). The location where the crash took place is Crafts Street and Waltham Street. Crafts Street and Waltham Street are public ways maintained by the City of Newton. Crafts Street and Waltham Street are municipal roadways that support two way travel for motor vehicles and bicycles with an unprotected median in the middle of both roadways to separate the direction of travel. This intersection was recently re-paved within the last month and has newly painted street markings including crosswalks, dedicated bicycle lanes, and stops lines at the intersections. Crafts Street at this intersection features two travels lanes. I observed the road surface to be dry at the time of my arrival on scene. The north lane in the westbound direction of Crafts Street permits travel straight ahead and also permits motorists/bicyclist to take a right turn onto Harding Street. The south lane in the westbound direction of Crafts Street is a dedicated left turn only lane that coincides with a left turn only arrow on the traffic signal in the middle of the intersection. The opposite side of Crafts Street permits traffic traveling in the eastbound lane to travel straight or take a left turn onto Harding Street from is a single lane travel lane. The area is well lit featuring street lights bright enough to illuminate the intersection for motorist traveling through the area. The weather at the time of the crash appeared to be clear. In the intersection where the crash took place, I observed a large debris field comprised of parts from a 2020 Tao Tao motorized scooter (49CC) and a 2007 Chrysler Town and Country (MA: 5EE316). I observed the motorized scooter (VIN: L9NTCBAG1L1010287) resting on its side facing North in the northbound lane of Crafts Street in the intersection. The motorized scooter has significant damage to its front, right, and left side and was inoperable. The area in the immediate vicinity of the motorized scooter was saturated with gasoline. The rear lights and dash of the motorized scooter were illuminated while I was observing the crash scene. The headlights were not intact due to the impact from the crash and I was unable to determine if they were functioning. The motorized scooter was removed from the roadway by Tody's Towing. The intersection where the crash took place had numerous first responders present immediately after the crash rendering aid to Mr. Tchikou prior to his transport to the hospital. It was reported by officers on scene that some of the items and debris in the intersection had been moved from their initial place of rest after the crash took place. A black MMG helmet was observed resting on the ground outside the debris field from the crash before the westbound left turn only lane of Crafts Street. It is unknown if it was moved from its initial place of rest by a first responder. I observed the strap on the helmet that would be used to secure it to the operator to be in good operable condition. I did not observe it to be defected and the securing feature of the strap was in working order. I did not observe any blood, hair, or other apparent physical evidence inside the helmet that would be consistent with the head injury reported to the operator of the motorized scooter. I observed what appeared to be fresh markings on the front mouth/nose area of the helmet. I observed a white Iphone with a red phone case lying on the roadway a short distance away from the impact area. The Iphone was on and displaying travel directions from the Iphone application Waze. The Iphone and the helmet were seized pending the status of the operator of MV2. A property/evidence form was completed and the Iphone and helmet were secured in property locker number 9. I spoke with the operator of MV1, William Moran (S34946215). Moran stated he was operating his white 2007 Chrysler Town and Country (MA: 5EE316) on Crafts Street (N) as he approached Waltham Street. Moran stated he had a green light and activated his left blinker as he approached the intersection with the intention of taking a left turn onto Waltham Street. Moran stated as he proceeded to take his left turn he saw "orange" and was unable to avoid hitting the object in front of him. Moran stated the object came out of nowhere and he swerved to the left in an attempt to avoid the crash. Moran stated after the collision he accelerated away from the impact area and came to a controlled stop on Waltham Street (S) just past the intersection. Moran stated he exited his vehicle and looked back at the intersection he had just travelled through. Moran stated as he began walking towards the intersection he observed a party laying on the roadway in the intersection. Moran stated the party was not wearing a helmet at this time. Moran also added that he believes the moped involved in the crash did not have its headlights on prior to impact. Moran stayed on scene and cooperated fully with officers during the investigation. A query of Moran's license and registration status through Newton Police Dispatched showed they are both active. I asked Moran if he was using a cell phone, GPS, or any other type of hand held device prior to the crash. Moran stated he was not. I asked Moran if he was distracted by anything either inside the vehicle or something on the roadway prior to the crash. Moran stated he was not. I asked Moran if he was currently impaired in any way that would impede his ability to operate a vehicle safely. Moran stated he was not. I asked Moran if he takes any medications or has a medical condition that would impede his ability to operate a vehicle safely. Moran stated he does not. Moran reported no injuries as a result of the crash. I observed Moran's vehicle at its controlled rest location. The vehicle was parked on Waltham Street (S) in the southbound lane just past the intersection of Crafts Street. The 2007 Chrysler Town and Country (MA: 5EE316) had substantial damage to the front passenger side bumper, hood, fender, windshield, and passenger door area. There was physical evidence of what appeared to be hair, blood, and tissue presumed to be from the operator of MV2 present in the cracked windshield, and on the passenger side door/mirror area. I observed a blue Commonwealth of Massachusetts "R" inspection sticker with a date of 05/28/2020 affixed to the windshield. Moran stated the rejection sticker was a result of an emissions failure. The inspection sticker expires in 2021. I asked Moran if there were any other mechanical issues with the vehicle. Moran stated there were no issues to his knowledge. I observed no objects present inside the vehicle during my investigation that would have potentially impeded the operators ability to see clearly through the windshield. I observed glass debris from the windshield inside the passenger and driver area of the vehicle. There was no air bag deployment inside the vehicle. Subsequent to my investigation, the vehicle was seized pending the status of the operator of MV2. The vehicle was towed from the scene by Tody's towing and secured in the Newton Police Department garage. The vehicle was escorted to the garage by Officer Donovan. A towed motor vehicle form was completed by Officer Donovan and the key for the vehicle was secured in property locker number 9. A property/evidence form was completed. Subsequent to my investigation Moran's black Alcatel cell phone was seized pending the status of the operator of MV2. A property/evidence form was completed and the phone was secured in property locker number 9. Officer Conary stated to me that prior to my arrival on scene she interviewed a witness to the crash and provided me with the following information. Joshua Hamel was operating his vehicle (W) in the right lane of Crafts Street towards Waltham Street. Hamel stated that MV1 (MA: 5EE136) was in the left turn lane on Crafts Street (W) towards Waltham. Hamel stated the scooter was headed eastbound on Crafts Street from Waltham. Hamel stated MV1 began to take a left turn and there was a head on collision with the scooter. Hamel stated he believed the traffic light at the time of the crash was green. On Friday, October 9, 2020, I spoke with Hamel via telephone. Hamel corroberated the statement he gave to Officer Conary. I asked Hamel if he believed the headlights on the motorized scooter were activated at the time of the crash. Hamel stated he believes the headlights on the motorized scooter were on as it approached the intersection but was not postive. On Wednesday, October 7, 2020, I spoke with Mr. Tchikou in his ICU room at Brigham and Women's Hospital. Tchikou's wife, Yami Carrajza, was present in the room while I spoke with him. Tchikou stated he had a significant head injury, injuries to his nasal cavity, his back, and a significant injury to his left leg. I asked Tchikou about his recollection of the crash. Tchikou stated he had just left his friend's home on Hazelhurt Avenue in Newton and was traveling home to Brighton to bring his wife dinner. Tchikou stated he was traveling Eastbound on Crafts Street as he approached the intersection of Crafts Street and Waltham Street. Tchikou stated he had a green light and as he entered the intersection a white vehicle took a turn from the opposite side of the road in front of him. Tchikou stated he does not remember anything after that. I asked Tchikou if he was wearing a helmet at the time of the crash. Tchikou stated he was. I explained to Tchikou that I recovered a white Iphone with a red case at the scene of the crash. Tchikou identified the Iphone as his. I explained that the phone was displaying travel directions when I found it and I asked Tchikou if he was distracted by his phone prior to the crash. Tchikou stated he was not distracted by the phone and the phone was in a holder on the motorized scooter. I asked Tchikou if he remembers if his headlights were activated on his motorized scooter at the time of the crash. Tchikou stated they were on. I asked Tchikou if he could produce paperwork for the motorized scooter to prove he was the owner. Tchikou stated he had recently purchased the scooter and the paperwork for it was at his residence. Tchkou's wife e-mailed me a picture of the Certificate of Origin for the motorized scooter. I asked Tchikou if he could produce a license or permit that would allow him to operate the motorized scooter. Tchikou stated he did not have a Massachusetts license or permit, but had an Algerian license at this residence. I was able to observe a picture of the license on his wife's cell phone. The written text on the license was in Arabic. Tchikou did not have a translation of the license in English and did not possess an International Driver's Permit. As a result of my investigation, Mr. William Moran while operating MV1 (MA: 5EE316) entered the travel lane of MV2 (Tchikou) as he was making a left turn onto Waltham Street from Crafts Street. MV2 had the right of way and MV1 failed to yield to MV2's right of way causing the collision to take place. Moran was advised of my investigation and issued Massachusetts Uniform Citation T0115914 via mail by request for Chapter 90, Section 8 (Fail To Yield To The Right Of Way At An Intersection.) Mr. Nadhir Tchikou operated his motor scooter at the time of the crash without a valid license/permit. Tchikou is also in violation of operation of a motor scooter after dark (prohibited). The crash occurred at 2147 hours. Tchikou will be issued Massachusetts Uniform Citation T0115915 in hand at Brigham and Women's Hospital for Chapter 90, Section 1E (Unlicensed Operation Of A Motor Scooter) and Chapter 90, Section 1E (Operation Of A Motor Scooter After Sunset - Hours Restriction.)

The operator of vehicle #2 stated that he was travelling West bound on Washington St. attempting to make a left hand turn onto Lewis St. Operator #1 stated a vehicle heading in the opposite direction stopped to allow vehicle #2 to make a left hand turn. Vehicle #2 was then struck by vehicle #1 which was travelling in the right lane heading Eastbound. Vehicle #2 was spun around by the impact and struck Vehicle #3 which was stopped at a red light. Operator of vehicle #3 stated he was stopped at the red light at Lewis St. and Washington when he observed Vehicle #2 attempt to make a left turn onto Lewis St. Vehicle #2 was struck by vehicle #1 which spun vehicle #2 into vehicle #3. Vehicles #1 and #2 were towed away by tody's due to excessive damage. Occupant of vehicle#2 was taken to Newton Wellesley Hospital by paramedics for minor inuries.

On Thursday 4/8/21 at approximately 0728 hours while assigned to marked unit n499 I was dipatched to the parking lot of Seasons 52 (220 Boylston St) for a report of a motor vehicle crash with minor injuries. Upon arrival I spoke with the operator (KRAUSE Liza) of MV#1 (Mazda CX-5). KRAUSE states she pulled into the parking lot from rte9 EB and was travelling straight when she observed a dump truck about to enter her lane of travel from the left. KRAUSE states she blew her horn, but the dump truck coninued on striking her vehicle on the drivers side rear. Impact from the collision caused significant damage to the drivers side of the vehicle in addition to deploying the side air bags in her vehicle. KRAUSE was evaluated by medics and refused transport to the hospital. She states she is uninjured just shaken up. Secondly, I spoke with the operator (BENNETT Paul A Jr) of MV#2 (KW Dump Truck). BENNETT states he was following another dump truck through the parking lot to the back exit on Florence Street to deliver asphalt to a construction job. In front of Seasons 52 he states he stopped at the stop sign and proceeded through unaware that MV#1 was in front of him striking the MV on the drivers side. There was no damage to the dump truck other than some marks on the front bumper. Motor vehicle crash occurred in a parking lot containing many stores, restaurants and a supermarket (Wegmans). Where the crash occurred was in front of Seasons 52 at a stop sign. At the stop sign is an island angled in such a manner to push vehicles to stay to the right and not make any turns. However, there are no signs displaying turning restrictions. The dump truck attempted to cross over the main entrance from rte9 to the parking lot containing the tent for SoulCycle and exit through the back of the parking lot. Vehicles entering from rte 9 do not have a stop sign and have the right of way to travel straight upon entry.

Bicyclist Cory Small stated that on 06/10/2021 at 17:48 hours he was riding his Specialized carbon frame bike in the westbound bike lane on Beacon St near Garland Rd. Beacon St and Garland Rd are both public ways in the City of Newton. Small stated that westbound Beacon St traffic was stopped at the intersection of Walnut St. Small stated that he continued riding westbound in the bike lane while traffic was stopped. Small stated that as he was crossing over the intersection of Garland Rd. when a vehicle that was driving in the eastbound lane of Beacon St turned left onto the northbound lane of Garland Rd and into his path of travel. Small stated that when the vehicle turned onto Garland Rd it caused him to crash into the rear passenger side of it. As a result of the crash Small had lacerations to his left hand and leg. Small's left ankle became swollen due to the crash. Small was transported via EMS to Newton Wellesley Hospital for treatment. Small was wearing a helmet at the time of the crash. Operator of vehicle one ( 2012 Volvo XC70 MA reg 2FJK50 ) Matthew Ristuccia stated that he was driving eastbound on Beacon St and was turning left onto the northbound lane of Garland Rd when the crash occurred. Ristuccia stated that he did not see Small prior to the crash. Ristuccia stated that he and his two sons who were passengers were not injured. Ristuccia's vehicle had minor scrape marks on the rear and center passenger side. I took photos of Ristuccia's vehicle and the disk was downloaded by the NPD's IT Bureau. Small's bike had damage to the left hand brake, and possible frame damage. Based upon statements made to me by Small and Ristuccia and my observation of the vehicle and bike, I issued Ristuccia MA Uniform Citation T1447316 and cited him for a violation of MGL 90/14 , failure to yield to an oncoming bicyclist when turning left. I explained to Ristuccia his payment and appeal options.

On Friday, July 16, 2021, while assigned to Traffic unit N525, I responded to the intersection of Commonwealth Avenue and Temple Street, Newton for a crash involving a City if Newton Fire Department vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. Commonwealth Avenue and Temple Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Mr. Jacob Forbes. Mr. Forbes stated he was at a stop sign on Temple Street and was attempting to cross Commonwealth Avenue (N) from Temple Street. Mr. Forbes stated he looked to his right, and then to his left and did not see any vehicle approaching. Mr. Forbes stated as the began to enter the intersection he looked to his right and saw a vehicle pulling over to the side of the road and a Newton Fire truck with it's lights on was heading towards him. Mr. Forbes stated he could not avoid the collision and the front end of MV2 crashed into his passenger side. Mr. Forbes reported no injuries and signed a patient refusal with Fallon Medics. I observed damage to the front passenger side area of MV1 and it was removed from the roadway by Tody's Towing. MV2 is a City of Newton Fire Department vehicle "C2". It is a 2019 Ford Expedition (MF455). The vehicle was responding to a Fire Alarm Activation call with emergency lights and siren activate at the time of the crash. The occupants of the vehicle were taken to Newton Wellesley Hospital for evaluation. I observed front end and driver side damage to MV2. There was air bag deployment inside the vehicle, on both the front driver and passenger side. Tody's Towing removed MV2 from the roadway. Photos were taken of the crash scene and submitted to the IT Bureau. I responded to Newton Wellesley Hospital and spoke with Newton Firefighter John Morris (Driver) and Deputy Chief Michael Mcnamara (passenger) in the emergency room. Both parties stated they were responding to a call with emergency lights and siren activated. FF Morris stated as he approached the intersection of Commonwealth Avenue (W) and Temple Street, he observed a black SUV exiting Temple Street onto Commonwealth Avenue. FF Morris stated as they were passing he believed MV1 tried to speed up to cross Commonwealth Avenue. MV2 then crashed into MV1. As a result of the crash, FF Morris suffered right hand/wrist injury. Both FF Morris and Deputy Chief Mcnamara stated they had slight respiratory issues from breathing in the powder from the air bag deployment. MV1 failed to yield to MV2's right of way as he attempted to cross Commonwealth Avenue from Temple Street. The operator of MV1, Mr. Jacob Forbes will be mailed Massachusetts Uniform Citation 071113AB for Chapter 89, Section 8 (Fail to Yield at Intersection).

On 10/18/2021, while assigned to N496, I, Officer Conary, responded to Beacon St at Chestnut Hill Road for a MVA with airbag deployment. Upon arrival, I met with the Operator of MV1 who stated that he was traveling Westbound on Beacon Street when MV2 pulled out from Chestnut Hill Road onto Beacon Street right in front of him. Operator of MV1 stated that MV2 just pulled right out that he had no time to stop. The passenger present in MV1 stated that they were traveling Westbound on Beacon Street when MV2 cut in front of them and could not avoid a collision. Operator of MV2 stated that they were pulling out from Chestnut Hill Road turning left. They saw no other cars traveling on Beacon Street and continued to make the left turn when MV2 hit MV1. Witness who was running across the street heard MV1 attempt to stop (screeching tires) and then MV2 hit MV1. Witness only provided a phone number for contact. All parties were evaluated by medics and signed patient refusal. Operator of MV1 complained of a hand injury but was going to transport themselves to the hospital. Both vehicles were towed.

On April 8th, 2022 at approximately 08:56 hours while assigned to N491 I along with NFD and Fallon Ambulance all responded to the intersection of Watertown St @ Eddy St for a report of a vehicle into a pole. On our arrival we located two vehicles involved in this crash. Vehicle #1 was located on the property of #790 Watertown St in some shrubbery with heavy damage. That vehicle was a blue 2008 Toyota Highlander, Ma. reg. 1WLR79 was unoccupied. The operator was located on Eddy St checking on the other operator. He was identified as Bennett Anyanwu. He stated he was travelling E/B on Watertown St when vehicle #2 entered out into the intersection from Eddy St causing him to crash into that vehicle. The impact caused his vehicle to veer off the road onto the property of #790 Watertown St. Vehicle #2 was located on Eddy St on the sidewalk facing east to the property of #790 Watertown St with heavy damage also. Vehicle #2 was a 2013 White Infiniti FX Utility, Ma. Reg. 4RE825, operated by Daniel Giang. He stated he was on Eddy St facing S/B and was attempting to cross the intersection of Watertown St to get to the other side of Eddy St when his vehicle was struck by vehicle #1 in the intersection. The impact caused him to continue forward to the other side of Eddy St striking the stop sign then careening off a telephone pole and then coming to a stop on the sidewalk. Mr Giang further stated he never saw vehicle #1 coming E/B on Watertown St prior to entering out into the intersection. Both vehicle were towed by Tody. Operator of vehicle #2, Mr Giang complained of rib and neck pain. His mother came to scene and she transported him to the hospital. Fallon obtained a patient refusal. The property to #790 Watertown St sustained damage from vehicle #1 being forced onto the property. I made contact with the property owner Arthur Murray and gave him the insurance company for vehicle #2 and the crash number also.

Narrative:
On June 6th 2022, at approximately 1543hrs, I, Officer Dragone, responded in the area of Chestnut St and Beacon St for a report of a rollover motor vehicle crash. Upon arrival, vehicle 1 was parked on Chestnut St, just past the intersection at Pine Ridge Rd facing Southbound. The operator of vehicle 1 was accompanied by a juvenile passenger who was in the back seat. Vehicle 2 was rolled over onto the drivers side of the vehicle at the corner of Chestnut St and Pine Ridge Rd. There were yaw marks located in the southbound lane of Chestnut St that trailed behind vehicle 2. Inside vehicle 2, the operator was trapped inside and was not able to get out. There was heavy front end damage to vehicle 1 and heavy damage to the passenger side of vehicle 2 which was the point of impact. Both vehicles had airbag deployment. Operator 1 stated she was experiencing neck pain and the passenger stated she did not have any injuries. Operator 2 was bleeding from the hand and stated she was experiencing pain in her sternum, prior to being extricated by Newton Fire Department. Operator 1 was transported to Newton Wellesley Hospital and Operator 2 was transported to Brigham and Women's Hospital by Fallon EMS. I spoke to a witness, Agnes Gee, who stated she was traveling Northbound on Chestnut St and she stopped to let vehicle 2 cross the intersection from Pine Ridge Rd. She stated vehicle 1 was traveling Southbound on Chestnut St before the crash occurred. Operator 1 stated that she did not remember anything that happened prior to the crash or which direction she was traveling. She stated she is not familiar with the area and was traveling from her job in Brighton to her home in Swansea, MA. Operator 1 states the last thing she remembered following detour signs prior to getting into the crash. It should be noted that there are detour signs throughout Chestnut St in between Commonwealth Ave and Beacon St. Operator 2 stated she was on Pine Ridge Rd heading Westbound and was crossing over Chestnut St. She stated that a vehicle in traffic let her proceed into the intersection and vehicle 1 was traveling in the Southbound lane prior to colliding into the passenger side of her vehicle. Both vehicles were towed by Tody's Towing. Photographs from the scene were taken to be attached to this report. It is unknown if vehicle 2 caused damage to a utility pole located at the corner of Chestnut St and Pine Ridge Rd; however, photos of the damage were taken and notifications were made to have it assessed.
