On Monday March 25th, 2019 I responded to a hit and run motor vehicle accident at 115 Florence St near Louise Rd. Vehicle # 1 (Mass Reg #5TFK30 Honda Pilot operated by Sabryna Reyes) was traveling east on Florence St headed toward Route 9. According to Sabryna, Vehicle # 2 was taking a left hand turn onto Florence St from Louise Rd. Vehicle # 2 made contact with the left side of the vehicle of vehicle # 1. Vehicle # 2 drove away without staying on scene. Sabryna was unable to get a license plate of the vehicle driving away. There were no witnesses to the accident and no video cameras in the area. Sabryna described vehicle # 2 as a black honda CRV with white male passenger operator approximately in his 30s. Their were no injuries and Sabryna signed a patient refusal. Newton Fire responded because of the airbag deployment. Vehicle # 1 suffered minor damage to the left front side of the vehicle. Vehicle # 1 was towed by AAA Towing.

Narrative:
On Tuesday, October 1st 2019, at approximately 3:45pm, I, Officer Brooks, responded to Valentine street by Lenox street for a motor vehicle roll over accident with entrapment. Upon my arrival I observed MV1(MA REG SN227) laying on its passenger side with heavy damage to the entire vehicle, and all air bags deployed. The operator was trapped in the driver's seat but was concious and alert. The fire department arrived and extracted the operator, who signed a patient refusal with Cataldo medics. The operator stated she had been traveling southbound on Valentine street, looked down for a moment and struck a parked car with the passenger side of her vehicle. Her vehicle then spun out and rolled onto its side. I spoke with the owner of MV2(MA REG 9WG975) who stated he was alerted by the sound of the crash. His vehicle was legally parked on Valentine street. MV2 sustained heavy damage to the driver's side rear end. Both vehicles were towed by Tody's and inventory forms were filled out and filed.

The operator of MV1 states that she was traveling northbound on Needham Street with flow of traffic when she struck a pedestrian with the right front corner of her vehicle. The operator of MV1 stated that she did not see the pedestrian in the cross walk until after she hit her and that there was poor lighting in the area. Through the help of coworker from Anthony's Coal Fired Pizza, the victim stated that she was crossing Needham Street in the crosswalk and was almost on the side walk when she was struck by MV1 from behind. MV1 sustained minor front end damage while the victim sustained noticeable injuries and was transported to Newton Wellesley Hospital by Cataldo. Photographs of the scene were taken and submitted to IT to be attached to the report. The operator of MV1 was issued a Massachusetts Uniform Citation (T2079284) for 89-11, failure to yield to a pedestrian in a crosswalk.

On 12/4/19 at approximately 2000 hours, Ladder Truck E-1 (MF463) responded to 47 Charles St for a fire alarm. Mv2 drove next to me due to the alarm being at her residence. She stated she was going to park her car and speak to the firefighters. She had parked her vehicle directly behind the truck. After the scene was clear, Firefighter Leone was operating the truck and reversed. The vehicle was in the blind spot and he was unable to see her. He had reversed into her vehicle. Operator of vehicle one Cataldo Medic Elisle Gedansky stated on 12/24/2019 at 12:55 hours she was driving MA reg AMN 369 a 2016 Ford Ambulance east bound on Myrtle St with it's emergency lights and siren activated responding to a medical call at 52 Myrtle St. Gedansky stated the Newton Fire Eng 2 was parked in front of 52 Myrtle St and that vehicle two Ma reg 9940RB a 2000 VW was parked unoccupied across from 52 Myrtle St. Gedansky stated that she believed that there was enough room to drive between the fire truck and vehicle two. Gedansky stated that the front passenger side of vehicle one struck the front driver's side of vehicle two. Vehicle one had no damage and Gedansky was not injured. Vehicle two had damage to it's driver's side mirror and driver's side front bumper. I took photos of vehicle two and forwarded the disk to the NPD's IT bureau. Owner of vehicle two Aline Elliot of 46 Myrtle St was advised of this incident.

Sgt. McNeil, N495, and I responded to 474 Wodward St. for an auto pedestrian accident. Upon arrival the victim, Elizabeth Cantwell was laying in the street and receiving treatment from NFD and Cataldo. The operator of Ma Reg. 192HV0 was Margaret Meehan. Meehan was visibly upset and was hearing impaired, as a result Meehan was unable to communicate what happened in the accident. At this time we were able to contact Meehans daughter, Bernadette Conley 781-248-6222. Cantwell was transported to Beth Israel Hospital. Traffic Bureau was notified. I went to the BI hospital to get a statement from Cantwell. Cantwell states that her husband parked their vehicle across the street from Starbucks. Cantwell states that she stood next to her car to cross the street. Vehicle 1 came to a stop and Cantwell believed that the vehicle was going to let her cross the street (not in crosswalk). Cantwell states that when she went to cross the street vehicle 1 hit her, causing her to land on the hood and then rolled off onto the street. At this time Cantwell has a dislocated hip and head injury. I took photos of the scene. Meehans vehicle was parked in a legal spot and I transported her to NHQ where she was picked up by her daughter Bernadette Conley. I filled and immediate threat with the RMV and a copy of the report was attached with the form.

On 02/14/20 at approximately 14:45 I responded to a crash in the parking lot outside of #50 Winchester St- The Tile In Style store. Upon my arrival the Fire Department was already there and I was met by the owner of the building Bruno DiFazio. The vehicle that had struck the building had left the scene but he showed me security footage of the crash. The video showed a white S.U.V. (later determined to be Mass. Reg# 6GEL40 White 2017 Acura RDX owned and operated by Eleonora Revzin) pull slowly into a parking spot and then accelerate up and over the curb and into the building. The crash caused moderate damage to the brick facade and inside wall of the tile store. A short time later, Revzin drove back into the parking lot where I spoke with her. Revzin stated she didn't know why her car had accelerated into the building but she was having some personal issues and may have been distracted. She explained that she left the area because she was shaken up but then as she had time to clear her head, thought she should return to the store. There was minor damage to her vehicle and she stated she was not hurt and didn't want me to call the Medics for an evaluation. I issued Revzin Mass. Citation #T2013436 in hand for N.C.O. 19/75- Failure to use care in stopping and a verbal warning for leaving the scene. City of Newton Inspector Paul Nelson showed up and deemed the building to be structurally safe. N523 Officer Wilson arrived and took pictures of the scene.

Operator of vehicle 1 was parked half up on the sidewalk across from #241 Riverview Ave. He attempted to pull forward and onto the street and forgot there was a fire hydrant in front of him. the vehicle struck the hydrant knocking it from its base. Operator reported no injuries and the vehicle did not require a tow. Officer Gaudet took several digital images and the City was notified about the hydrant.

On Wednesday, May 6, 2020 while assigned to Traffic unit N525, I responded to the area of Harvard Street and Washington Park, Newton for a report of a construction vehicle that crashed into a City of Newton fire hydrant. The weather at the time of the crash was clear and sunny. The road surface was dry. Harvard Street and Washington Park are both public ways maintained by the City of Newton. I spoke with the operator of the vehicle involved in the crash, Michael Clavette (S36731553). Clavette is employed S and P Construction and was operating a 1998 Mack 700 (MA CON: S71054) while working as a sub contractor for Feeney Brothers on Washington Park. Clavette stated he was attempting to back down Washington Park (W) from Harvard Street. Clavette stated he pulled his vehicle forward to reposition and the front bumper area made contact with a fire hydrant in front of 82 Harvard Street. I observed minor damage to the front bumper area of MV1. I observed minor damage to the fire hydrant. Photos were taken of the damage and submitted to the IT Bureau. The City of Newton Water Department was notified.

Operator of vehicle #2 Henry Burke stated that on 08/07/2020 at 14:09 hours while parking his vehicle ( MA reg 80S 2008 Toyota SUV ) in front of 349 Watertown St the front bumper of his vehicle made contact with the rear bumper of the vehicle that was parked in front of him. Watertown St is a public way in the City of Newton. Vehicle #2 had no damage, and Burke stated that he was not injured . Operator of vehicle #1 Newton Fire Department Lt. Michael White stated that he was sitting in a NFD SUV ( MA reg MV N MF4653 2016 Ford SUV )was facing west bound when vehicle #2's front bumper stuck his vehicle's rear bumper. Vehicle #1 had small scuff marks to it's rear center bumper. White stated that he was not injured and neither vehicle required a tow, I took photos of both vehicles and turned the disk over to the NPD's IT Bureau for downloading.

Operator of MV1 stated she was pulling into a parking spot in front of 165 Needham Street-Shop n Shop. As she was pulling into the spot, she believes she accidentally hit the gas pedal instead of the brake. She struck a light pole and went up and onto the sidewalk area. The light pole was knocked to the ground and broke into two pieces. Operator of MV1 was not injured and signed a patient refusal. The vehicle was towed by David Donahue from Tody's Towing. The Manager on Duty at Stop n Shop was made aware of the damage. She notified the store's general manager and the property manager about pole that was knocked down and the damage to their storefront window frame. NFD Engine 7, taped off the area.

On 09/21/20 at 14:46 hours, I (Traffic Unit # 523) responded to the parking lot of 88 Chestnut Street for a motor vehicle accident involving a pedestrian. The weather at the time was 60 degrees ( localconditions.com) and the sky was clear and sunny. The road surface was dry. The parking lot is a thru way that the public use to enter/exit the lot or park there. On arrival at the same time with the Fire and Medics, I observed a female laying in the middle of the lot facing down on her back. A small red and gray CVS like type shopping carriage was knocked over and resting on it's side in front of the victim's head. The carriage was moved to the side in order to tend to the victim I also observed a black Honda Civic bearing MA reg.# 3741TW that was involved in the accident. The Honda was out of the parking spot resting approximately two feet away from the pedestrian's legs. I then spoke to the pedestrian identified as Darla Gipson in the rear of the ambulance. She sustained an abrasion to her front left shoulder and arm area which I observed. She also complained of left knee and elbow pain. She stated that she walking through the middle of the parking lot pushing the carriage by the door entrance in a westerly direction towards Chestnut Street. She states that she never saw a vehicle backing up and then she was suddenly struck and believes she was pushed underneath the vehicle. She was then transported to Newton Wellesley Hospital by Cataldo Ambulance. I then spoke to the operator of the Honda Civic identified as Linda Burke (MA S70320414). Ms. Burke was parked in the first spot to the right of the walkway that leads to the entrance/exit doorway. I asked her what occurred and she stated that she was backing her vehicle up to exit the parking lot when she looked behind her not seeing anything in sight. She then started backing up and struck the pedestrian without realizing it. She says the driver of a green cab and his female customer who were no longer on scene were yelling at her to stop her vehicle because the pedestrian was underneath it. She then stopped her vehicle. I observed minor scrapes to the entire rear bumper of the vehicle so it is unknown where it actually struck the pedestrian/carriage. I observed no obvious signs of damage to the carriage. It should be noted that Ms. Burke's Honda does not have a back up camera or back up sensors. There are no surveillance cameras to the apartment complex and no witnesses came forward for the accident. As a result of the investigation, Ms. Burke will be issued Uniform Citation # T1271069 for Newton City Ordinance 19-75 Failure To Use Care in Backing. Digital photos were taken by me of the scene.

The operator of MV#1 stated she was parked in front of 1664 Centre St arranging packages inside the cargo area of her van (Amazon delivery van) when her vehicle was struck by MV#2 causing her to fall inside the cargo area. The operator of MV#1 stated she was shaken up but was not injured. Newton Paramedics along with Newton Fire evaluated her and she signed a refusal of treatment. MV#1 sustained moderate damages to its driver's side door. The operator of MV#2 stated she was reversing out of her driveway (1665 Centre St) and did not see MV#1 parked there when she struck it. MV#2 sustained moderate damages to its passenger side rear bumper. The operator of MV#1 along with its front passenger were evaluated by Newton Paramedics and Newton Fire and both signed a refusal of treatment.

On 11/11/2020 I, Officer Mildner, was dispatched to 48 Windermere Rd. for a report of Gas Lamp being struck by a single MV. Operator of MV1 stated that he was traveling Northbound on Windermere Rd when he took a left and pulled into the driveway of 47 Windermere Rd. in an attempt to back out and turn around. Operator of MV1 stated that as he was backing out Eastbound, he hit a pole containing a gas lamp at the top, right in front of 48 Windermere Rd. Witness 1 stated he saw Operator of MV1 pull into the driveway of 47 Windermere and back out directly into a pole containing a gas lamp at the top. There were no injuries. No damage to MV1. There was minor damage to the light pole and gas lamp. Fire was successful in turning off the gas. Pictures were taken and submitted to the IT Bureau.

On Monday, December 21, 2020, while assigned to Traffic unit N525, I responded to 8 Cragmore Road, Newton for a report of a motor vehicle crash involving a City of Newton Fire apparatus. Cragmore Road is a public way maintained by the City of Newton. The weather at the time of the crash was cloudy. The road surface was wet from melting snow in the area. The operator of MV1, Jon Papia, stated he was operating City of Newton Fire Engine 7 (MA MVN: MF470 ) Southbound on Cragmore Road. As MV1 travelled past 8 Cragmore Road, it's passenger side made contact with the front driver side area of MV2 (MA: 1JTC19). The Honda Accord was parked and unoccupied at the time of the crash. The registered owner was made aware of the crash and advised. I observed no damage to the passenger side of MV1. I observed damage to the front driver side of MV2. Photos were taken and submitted to the IT Bureau. No injuries were reported by any of the involved parties.

MV1 stated she was attempting to manuever around vehicles to exit the street when "I stepped on the gas and not the brake" while in reverse subsequently going backwards, over a curb, across the lawn, and into the bushes of 17 St James Ter sustaining heavy damage throughout the vehicle. The homeowner of the residence reported damage to his curb, lawn, bushes and electrical conduit (Newton Fire checked the residence prior to clearing). Todys Towing responded and removed the vehicle from the lawn and leaving it on the street as the owner wanted AAA to respond. Due to airbag deployment, Fallon Ambulance responded to the scene, however the driver refused to be evaluated (also refused to sign a refusal).

BOX. VEHICLE #1 HAD MINOR REAR LEFT DAMAGE. DRIVER REPORTED NO INJURY AND WAS ADVISED TO CONTACT HIS INSURANCE COMPANY. CITY OF NEWTON NOTIFIED TO GET THE HYDRANT. CLEARED WITHOUT FURTHER INCIDENT. Operator of Motor Vehicle #1 stated that she was traveling Southbound on Centre and slowed down and pulled to side of the road when she saw a City Of Newton Fire Truck with the lights and sirens activated traveling Northbound on Centre Street (The opposite direction). Operator of Motor Vehicle #1 stated that she was then struck by motor vehicle #2 from behind. Operator of Motor Vehicle #2 stated that he was traveling Southbound on Centre Street and saw the City Of Newton Fire Truck traveling Northbound on Centre Street (The opposite direction) in the center lane with its lights and sirens activated however did not realize that the vehicle in front of him was going to stop and struck vehicle #1 from behind. Motor vehicle #1 sustained moderate rear end damage and motor vehicle # 2 sustained major front end damage. Both parties were evaluated by medics and signed medical refusals forms. Motor Vehicle # 2 was towed from the scene by Tody's Towing.

On March 29, 2021 at approximately 17:40 hours I responded to 33 Boylston St, Juice Pressed, for a report of a single vehicle crash into a building. Upon arrival, I observed a black Mercedes SUV (MV1) parked in front of Juice Pressed with glass on the hood of the vehicle. The window pane store front and right door to the "Juice Press" storefront were shattered and broken. The operator of MV1 said that she was stopped in the parking space in front of Juice pressed ordering her drinks via the mobile app for pickup when she thought the vehicle was in park she took her foot off of the brake and accidentally hit the gas peddle. The vehicle crashed into the front of the store traveling about 2-3 feet into the building, breaking the front glass and door. No patrons were within striking distance of glass or the vehicle. Two employees were inside the business and said they were 15-20ft away from where the vehicle made contact. Newton Fire were on scene, management for "the Street", and "Juice Pressed" were notified and responded and arranged for the storefront to be boarded up. Management and vehicle owners were given the accident report number and information on how to obtain the report.

MV1 was stopped in front of 206 Elliot St. waiting to turn left onto Cottage St. when MV2 struck the rear end of his vehicle. I asked the operator of MV2 if he was following MV1 too close which he stated "no" that he had looked away for a second and when he looked back up he noticed that MV1 was stopped and that was when he tried to stop and struck the back of MV1. Newton Fire Department was on scene and witnessed the incident. Both operators did not want medical attention. Operator of MV2 was given a written warning for improper operation 90/12 (Citation #2014395). Operator of MV2 was not given a citation for following too close, due to admitting that he was not close, but had looked away from where he was driving. On Thursday July 01, 2021 at 1413 hours while on patrol in N496 I responded to 1280 Centre St for a report of a motor vehicle that had crashed into the front stairs of the building. On arrival I saw vehicle 1 ( MA reg#271RM5) half way up the front concrete steps of the building and wedged on an iron fence. The operator was identified as Norma Greenberg and she was already out of the vehicle and stated she was uninjured. Greenberg stated she was in her vehicle parked in a parking spot attempting to pull out and her vehicle would not go forward. Greenberg stated she kept hitting the gas when the vehicle suddenly took off and went up the stairs. Greenberg reported no prior mechanical defects with the vehicle. It appears she was in neutral and then tapped it into drive as she pressed the accelerator. Newton Fire had to use a saw to take apart the iron railing due to it being wedged into the vehicle. The vehicle was towed from the scene by Todys. The area was roped off with caution tape due to the damaged railing and I notified the building manager, Jodie Zussman of the report number. Greenberg was transported home by N494 Officer Lazarakis.

On August 7th, 2021 at approximately 08:40 hours while working N491, I along with NFD and Fallon ambulance responded to #266 Nevada St for a report of a MV crash involving a pedestrian. On my arrival the fire and medics were speaking with the pedestrian that was struck. He identified himself as SEAN DAZET, DOB 04/3/81, #28 Ashmont Ave, Newton. He stated while walking on the sidewalk with his dog S/B on Nevada St he observed a vehicle backing out a driveway to #266 Nevada St. He said the vehicle was probably ten feet away from him when he yelled out to the driver but the vehicle was already on him. Mr DAZET unable to retreat out of the way put up his left hand to brace for the impact. Mr DAZET was then struck from the vehicles passenger side rear bumper. He was knocked back but never fell to the ground. Mr DAZET reported the vehicle backing out was going he estimated between 5/10 miles per hour prior to the crash. Mr DAZET reported to the medics he was experiencing left hip and left shoulder pain. However he signed a patient refusal. I spoke with the operator of involved vehicle identified as Patrick Alvarez. He stated while he was backing out from the driveway to #266 Nevada St his vehicle made contact with the pedestrian. He further stated he never saw the person walking past the driveway as he was backing up until he heard him yelling out. He immediately stopped his vehicle. The accident was a low impact crash. Mr DAZET was given the operator's information along with his insurance in case he had to seek further treatment for his non visible injuries. Finally Mr DAZET did indicate he probably will follow up with his primary care doctor on Monday. There was no damage to the involved vehicle.

On Thursday 2/17/22 at approximately 0920 hours while assigned to marked unit n498 I was dispatched to the area of 1 Saco St. for a report of a single vehicle crash into a tree. Upon arrival the actual address was on Sweet St. and the operator, XIAO Liangping, was being evaluated by Medic 4. XIAO stated to the medics that she was not hurt and denied any medical attention and signed a patient refusal. Upon speaking with XIAO she states she turned onto Sweet Street from Oak Street and maneuvered her vehicle on the right side of the road between 2 parked cars in order to back into her driveway at 1 Saco St. The driveway for 1 Saco St. is located on Sweet Street. XIAO states she accidentally left the vehicle in drive instead of reverse. When she accelerated it caused her to go forward over a curb and into a tree. Damage to the vehicle made it unsafe to drive and I asked XIAO if she wanted the vehicle towed and she stated she did. Upon arrival of Dave Donohue from Tody's she changed her mind and stated she wanted it left in her driveway. Dave stated to XIAO that would be fine, but he advised her to leave the vehicle in the driveway and not to put it in the garage out of an abundance of caution due to prior incidents of other Tesla vehicle catching fire. Dave left the vehicle in her driveway and as Dave and I left the scene XIAO pulled her vehicle into her garage against the recommendation of Dave.

On Tuesday 3/1/22 at approximately 1300 hours while assigned to marked unit n498 I was dispatched to the area of 980 Boylston Street for a report of a motor vehicle into a building. Upon arrival fire and medics were on scene treating the operator of motor vehicle #1 (EDMAN Brigitta). EDMAN's vehicle crashed into the front of Mr K's Liquor store smashing out the front window and doing minor damage to the inside of the store. Prior to striking the building she also slightly struck an Anton Cleaner's van doing very little to no damage to the van. EDMAN states she was attempting to park in the handicap parking spot in front of the store when she accidentally stepped on the accelerator instead of the brake, which caused her vehicle to lunge forward and crash into the front of the building. EDMAN was not trapped in the vehicle but had difficulty exiting the vehicle so she was assisted by Newton Fire. EDMAN was also evaluated and transported to Newton Wellesley Hospital by Medi 4. While on scene Newton Inspectional Services was notified and responded to determine the structural integrity of the building. After looking inside the building Inspectional Services requested the vehicle be removed at which time Tody's was notified. Upon removal Inspectional Services also inspected the outside of the structure and determined the structural integrity of the building was not compromised and Mr. K's was able to stay open for business. Due to the nature of the crash I requested through the Massachusetts Registry of Motor Vehicle an immediate threat license suspension / revocation for EDMAN's drivers license. Seven pictures were taken and forwarded to IT to be attached to this report.

Operator of MV1 was parking in a parking spot outside of Pressed Cafe located at 300 Needham St. Operator believed she was in Park but was actually in Drive. Operator mistakenly pressed on the gas and accelerated over the curb and through a large window and ended up inside of Pressed Cafe. Operator and her daughter, Brynn Bryant, were trapped in the vehicle and freed by NFD. Operator and Brynn were transported to Newton Wellesley Hospital. Operator of vehicle one stated that while passing parked vehicle two the right side of the his vehicle scraped up against vehicle two's front driver's side bumper. Vehicle two was a City of Newton DPW truck that was parked in front of 367 Albemarle Rd waiting to block off the street for the Fourth of July fireworks at Albemarle field. Vehicle one's operator stated that he could not pull to the side of the road after the crash because of heavy traffic due to the Fourth of July activities in the area. He did stop at his residence approximately two blocks from the crash area. Vehicle one had damage to the passenger side front and rear as well as the rear bumper. None of the occupants of vehicle one stated that they were injured. The owner of vehicle one had concerns about the amount of room that the DPW tuck took up on the roadway. It should be noted that a City of Newton DPW worker measured out the length of the roadway from the truck's front bumper to the grass line and it was 11 1/2 feet. According to the General Motors web site the width of vehicle one is 6 feet 7 inches. Operator of vehicle two stated that he was sitting in the driver side of the truck when vehicle one passenger side scraped up against it, and that he was not injured. Vehicle two front bumper had damage to it. It took photos of both vehicles and forward the disk to the NPD's IT Bureau for downloading. I issued operator of vehicle one MA Uniform Citation T 3045462 and cited him for a violation of City of Newton Ordnance 19/75 failure to use care while passing. Albemarle Rd is a public way in the City of Newton.
