Operator of vehicle one stated that while driving westbound on Adella Ave the rear driver's side of his vehicle struck the front driver's side mirror of vehicle two. There was no damage to either vehicle. Vehicle two was parked unoccupied in front of 30 Adella Ave at an angle.There was another unoccupied vehicle parked across from 30 Adella Ave further constricting the roadway. Both occupants of vehicle one stated that they were not injured. Photos of both vehicles were taken and the disk was downloaded by the N PD's IT bureau. Adella Ave is a pubilc way in the City of Newton.

On 1/21/19 I was working N498 when I responded to 17 Charles River Ter for a report of a Cab that had struck a wall. Upon arrival I met with Pema Jigmey and Miriam Jost. Jost stated that Pema who was driving for Metro Cab (or Photo Cab) picked her up at the airport and brought her home. Once at her home 17 Charles River Ter, she asked him to back into the driveway due to the snow and ice conditions. While backing up in the driveway of 17 Charles River Ter the vehicle struck the residencet causing damage. Pema stated the same thing but believed Jost was half to blame due to the fact she asked him to be dropped in the driveway. I informed Pema that he drove the vehicle into the house and so was at fault. The house only appeared to suffer minor cosmetic damages but could not be accurately examined. Pema and Jost exchanged information in the hopes of settling the damages without insurance. Both parties were satisfied and provided a report number. Both parties stated they were uninjured and refused medical attention.

Vehicle 1 was traveling east on Washington Street. The operator of vehicle 1 states he felt the steering wheel pull hard to the right. The operator of vehicle 1 then pulled the steering wheel to the left in attempt to correct the pulling to the right. At this time the vehicle began to pull harder to the right and ended up striking the curb and a concrete light pole. The concrete pole is owned by the City of Newton, pictures taken and submitted to IT. MED2 responded and obtained a patient refusal. Eversource was notified and responded to disconnect the power. After the power was disconnected Todys was able to remove the vehicle. I was able to move the disconnected light pole and some debris to make the sidewalk passable. Eversource placed a cone over the disconnected wires and I was able to tape off the downed pole. Dispatch notified the city for the pole and to clean up the debris.

On Thursday, February 21, 2019, while assigned to Traffic unit N526, I responded to the area of Ionia Street, Newton for a report of a motor vehicle accident involving a City of Newton owned vehicle. Ionia Street is a public way maintained by the City of Newton. The weather at the time of the accident was overcast. The road surface was icy and snow covered. The operator of MV1, Patrick Carresi (MA: S73958042), stated he was operating a City of Newton owned 2016 Ford F350 plow truck (MA MVN: M94756) turning left onto Ionia Street from Auburndale Av. Carresi stated as he took the turn, the rear passenger corner of the truck made contact with the drivers side front panel of MV2 which was parked on the side of the road facing southbound. I observed minor damage to the rear of MV1. MV2 received heavy front end damage with the entire front panel torn off. Carresi reported no injuries. Carresi works for the Utilities Department with the DPW. The operator of MV2, Donna Canavan (MA: S57637058), stated she was parked inside her 2005 Honda CRV (MA: 8ZEE40) facing southbound on Ionia Street. Canavan stated as the plow truck took the turn onto Ionia Street from Auburndale Av its rear corner of the truck struck the front panel of her parked vehicle ripping the panel off. I observed significant damage to the front panel area of MV2. Canavan reported no injuries and was able to drive her vehicle from the scene. Pictures were taken of both vehicles and submitted to the IT Bureau.

MV1 was backing into a snow covered parking space at 25 Chestnut St. The lot had not been plowed yet. As he was backing, the rear of the truck fishtailed slightly and struck parked MV2(city MV). There was a scratch to the drivers side mirror of MV2. The rear drivers tail light and quarter panel were damaged on MV1. I took pictures as did oper MV1. I transferred my pictures to IT.

The operator of MV#1 states that she was traveling north on Melrose St from Auburn St when she encountered a large flat bed truck blocking the roadway in front of #283 Melrose St, which is currently under construction. The operator of MV#1 states she then spoke with an individual whom she believes to be Steven Vona of Vona Construction who is currently developing the site at #283 Melrose St. According to operator #1, Mr. Vona told her she needed to turn around immediately, which she attempted to do when the front of her MV struck a raised sidewalk causing damage to the front of her MV. Operator #1 was very concerned that there was no signage or personnel from Vona Construction directing motorist where to go and feels he is responsible for the damage to her MV as he was the one informing her she needed to leave the "construction zone" immediately. She further states that no one assisted her in turning around, her view was obstructed because her MV is so low to the ground and that a construction truck was blocking the nearest driveway, making it even more difficult to maneuver. As I entered Melrose St from Comm Ave when I arrived I observed no signage and MV's were having to turn around. I informed Steven Vona that if he was going to temporarily block the street to unload equipment he needed to either use cones, signs or workers to assist drivers. In regards to the crash, Mr. Vona stated that as operator #1 attempted to turn her MV around the front of her MV struck the sidewalk. On Thursday 04/18/209 at approximately 1222 hours I responded to 81 Oak Street for a motor vehicle accident involving Newton Fire Ladder 2. At the time of the accident the weather was cloudy and road conditions were dry. Oak Street is a public way in the city of Newton. Upon arrival I spoke with the operator of MV1 who stated while trying to manuever through oncoming traffic on Oak Street heading southboud MV1 struck MV2's drivers side mirror while MV2 was parked. MV1 reported no damage and reported no injuries. MV2 reiterated the same events and reported very minor damage to the drivers side mirror. No injuries were reported. Several pictures were taken of both vehicles and submitted to the IT Bureau.

On Thursday 04/18/2019 at approximately 1400 hours I responded to 37 Elm Street for a single vehicle accident with City of Newton property damage. At the time of the accident the weather was cloudy and the road conditions were dry. Elm Street is a public way in the city of Newton. The operator of MV1 identified as Silton Barrett (S17437114) stated he parked his UHAUL van in front of 37 Elm Street to make a delivery. While attempting to pull MV1 back into the traffic lane he struck a fire hydrant knocking it over and completely detaching it from the base on the ground. MV1 sustained minor front end damage and was able to leave the scene. Several photographs were taken and submitted to the IT Bureau. City of Newton was notified for the property damage.

The operator of Vehicle #1, identified as Ms.Erzsebet Piros, stated that she took a left turn from Walnut St onto Washington St prior to the colliding with Ms. Kim Nguyen. She further stated that she did not see the pedestrian prior to hearing the collision. Her passenger, identified as Ms. Amy Torres, was sitting in the front passenger seat. She stated a similar account, and did not see the pedestrian prior to the collision. Neither party complained of injury, when asked. A witness, Ms. John Angelucci, was travelling in his vehicle, in the lane next to Ms. Piros. He stated that he only heard the collision, and couldn't see it due to vehicle lights, and the falling rain. The pedestrian, Ms. Kim Nguyen, stated that she attempted to cross the street, when she did not see any cars coming. She stated that she didn't see Vehicle #1 approaching until it was too late to get out of the way. It should be noted that Ms. Nguyen was not attempting to cross inside of a crosswalk, and was wearing dark colored/Navy Blue clothing. Ms. Nguyen was transported to Beth-Israel Hospital, and was communicating effectively prior to transport. Pictures of the scene, and damage to the vehicle were taken, and sent to IT for entry.

DISPATCHED NOTIFIED THE CITY ABOUT THE SINKHOLE. I TOOK PICTURES OF THE SINKHOLE AND VEHICLE #1 AND PLACED THEM IN I.T. DRIVER REPORTED NO INJURIES. ADVISED TO CONTACT INSURANCE COMPANY. On 05/10/19 I met with Luiz A. Lopes who stated his vehicle ( Mass Commercial Reg# T83695 White Toyota Tacoma P/ U) was struck while parked in the lot around 07:00. Lopes said he didn't call the Police right away because he wasn't sure his car was struck there. The truck sustained a cracked front bumper and the fog light was popped out. We went inside of National Lumber and spoke with a manager who said he would contact G.M. Greg Assarian and have him look at the security footage. On 05/14/19 at approximately 14:00 the Police Station received a call from Lopes who just got off the phone with Assarian who found some security footage of the incident. I then traveled over to National Lumber and met with Assarian. He showed me the video and it appears that a newer grey four door Chevrolet truck backed into Lopes truck around 06:58 of 05/07/19. The Massachusetts plate is unreadable but an older White heavy set male gets out of the truck and walks into the store. After parking the truck the male can be seen checking his own truck bumper for damage. The male is wearing a baseball hat and sun glasses and cannot be identified on the video. The male does however walk with a noticeable limp. Assarian will show his picture to his employees and contact me if the employees are able to identify the male driver. I called Lopes back and apprised him of my findings. ******************UPDATE**************************************** As of today 05/20/19 I have yet to hear back from anyone at National Lumber so I will end this investigation and label it a hit and run crash.

MV1 had just gotten out of her MV in the driveway of 111 Hammondswood Rd. While between the door and the MV the MV started rolling backwards down the sloped driveway. The operator grabbed a hold of the top of the door and was dragged backwards across the street into the yard of 104 Hammondswood Rd where it came to a stop. The operator initaially claimed minor injuries but signed a refusal. This accident was caused by operator error as it appears that the car was not completely in park. There was minor damage(tire track mark) to the lawn. I attempted to contact the homeowner with negative results. I took pictures of the damage and TOT IT.

Bike One was travelling Eastbound on Auburndale Ave when it appeared the operators front tire came in contact with a slightly raised manhole cover causing the operator to fall off his bike and impact the roadway. The cyclist was wearing a safety helmet but received a laceration around his eye and several lacerations to his arms and legs. He was transported to Beth Israel Boston. I spoke to the bicyclist who was unsure of how the crash occurred. I contacted his brother, Paul Colangelo (603 434-1251) who was advised of the incident. Photos were taken of the crash scene and emailed to the IT Bureau. I placed his bike into support services for safekeeping.

Bicyclist, Tasker Howell, was traveling SOUTH on Watertown St, entering the lane of traffic when MV1 collided with her handlebars and front tire. MV1 (MA Reg: 8SP529, 2018 Ford SUV) was traveling WEST on Watertown St when she collided with the Tasker on the bicycle. MV1 operator, Maria Forgione, stated that while she was traveling on Watertown St, she had a passed a large landscaping truck parked at 857 Watertown St which hindered her seeing Tasker pulling out into traffic on the bicycle. Tasker stated the same, that she saw traffic clear and believed it would be safe to pull out into traffic but the landscaping truck created a blindspot. Tasker was not wearing a helmet. There was damage to MV1 on the passenger front end. Tasker was transported to Newton Wellesley Hospital. Tasker's bicycle was left in her possession at her residence at 871 Watertown St. Pictures were sumbitted to IT.

Operator of MV#1 stated he was travelling westbound on Wolcott St at approximately 25mph. He saw Bicyclist #1 on the sidewalk to his right. He then saw Bicyclist #1 enter the roadway and swerved out of the way to avoid contact. Bicyclist #1 drove into the right side of MV#1. Minor damage to MV#1. Bicyclist #1 stated he was riding his Trek Multi-sport on the sidewalk on Wolcott St when he "made a mistake" and entered the roadway without seeing MV#1. He stated his handlebars made contact with MV#1 and he fell off his bicycle. Bicyclist#1 suffered minor injuries to his elbows. On Friday, August 30, 2019, while assigned to Traffic unit N525, I responded to Burr Park (142 Park Street, Newton) for a motor vehicle crash involving a City of Newton Fire truck. The operator of Newton Fire Ladder 3 (MA: MF453453), Michael Pohlman, stated while trying to exit Burr Park from the Waverly Street exit, the middle driver side tire area of Ladder 3 made contact with a fixed cement pole. The pole is located at the entrance/exit and has a chain attached to it used to restrict access to the area. I observed minor damage to the driver side middle tire area and no damage to the pole. No injuries were reported by the occupants of Ladder 3. Photos were taken and submitted to the IT Bureau. Ladder 3 stated they were at the Burr Park permorming an inspection.

On Wednesday, September 4th, 2019 while assigned to Traffic unit N526, I responded to 84 Paul Street, Newton for a report of a motor vehicle crash involving a City of Newton DPW vehicle. The weather at the time of the accident was clear and sunny. The road surface was dry. Paul Street is a public way maintained by the City of Newton. I met with the operator of MV1, Newton Associate Engineer John Daghlian. Daghlian stated he was operating Newton DPW Nissan Leaf (MA M99257) attempting to parallel park on Paul Street in an open space. Daghlian stated as he pulled forward into the space his front end made contact with the bumper of MV2 which was parked info of MV1 unoccupied. Both vehicles sustained very minor damage. No injuries were reported by the occupants of MV1. I spoke with the owner of MV2, Naomi Singer (2019 BMW 330XI, MA: 8SP372). She stated she was inside her dentist appointment when the accident occurred and was notified by Daghlian. Pictures of both vehicles were taken and submitted to the IT Bureau.

THE OPERATOR WAS UNINJURED AND THE VEHICLE HAD LITTLE TO NO DAMAGE TO THE FRONT RIGHT PASSENGERS SIDE. THE PEDESTRIAN, MICHAEL MCKEAN, STATED HE RAN OUT TO CROSS THE STREET AND DIDN'T SEE MV1. HE SUFFERED A MINOR RIGHT FOOT INJURY AND WAS TRANSPORTED TO ST. ELIZABETHS. BY MEDIC 1. ALL PASSENGERS OF MV1 REPORTED THAT THE OPERATOR WAS NOT SPEEDING AND THE PEDESTRIAN DARTED INTO THE STREET. THE OPERATOR HAD DASH CAM FOOTAGE OF THE ACCIDENT, AND IT SHOWED MV1 TRAVELING WESTBOUND ON BEACON ST APPROACHING THE VEHICLE PULLED TO THE SIDE OF THE ROAD. IT THEN SHOWED THE PEDESTRIAN RUN INTO THE STREET WITHOUT LOOKING, GETTING STRUCK BY THE FRONT PASSENGER SIDE OF MV1 AND FALLING TO THE GROUND. On 09/27/19 at 08:41 hours while assigned to Traffic Unit 523, I responded to a motor vehicle/bicyclist accident at 1330 Centre Street. The weather at the time was clear and sunny. The road surface was dry and Centre Street is a public way maintained by the City of Newton. Traffic conditions were at a heavy volume and Centre Street was congested at the time. There is no dedicated bicycle lane in this area due to the road being too narrow. I spoke to the bicyclist involved (Unit 2), Michael Hnatko (S10432772). Mr. Hnatko stated he was traveling northbound on Centre Street on the right side of the road on his 20 inch gray Trek bicycle. He further says that he was peddling very fast at the time and must have not been paying attention to MV1 turning right in to parking lot of 1330 Centre Street as he was approaching from behind. As MV1 was turning right, he then struck the passenger side rear quarter panel of the vehicle and fell forward off of his bicycle. He sustained minor injuries such as scrapes to his knuckles on both hands and a small cut to the bridge of his nose. He signed a patient refusal with Cataldo Ambulance. It appeared that the front alignment was now off on his Trek bicycle. I then spoke to the operator of MV1, Lonnie McGinnis (S66044083). He states that he was inching along in heavy traffic waiting to turn right in to the lot of 1330 Centre Street with his right turn directional signal on. As he was turning right, the bicyclist struck his vehicle on his passenger side rear quarter panel area. McGinnis thought it was clear to turn right at the time and never saw the bicyclist approaching from behind. No injuries reported by McGinnis and I observed minor dents and scratches to the passenger rear quarter panel area. Digital photos taken by Ofc. Gaudet.

On Wednesday October 23rd, 2019 at approximately 1306 hours I responded to 40 Beaconwood Road for a motor vehicle accident involving a city vehicle. At the time of the accident the weather conditions were sunny and the road conditions were dry. Beaconwood Road is a public way owned by the City of Newton. Upon arrival I spoke with the operator of the city vehicle (MV1) identified as Norayr Sargsyan ( S34138376). Norayr stated that he was traveling southbound on Beaconwood Rd operating a yellow 2009 Ford drwsup truck (MA COM REG; M2939A) and as he passed a landscaping truck that was parked, the rear drivers side corner of the city truck made contact with the rear drivers side of the landscaping trailer. Norayr stated there was a car parked facing the wrong direction directly across from the trailer making it hard for him to pass in between both vehicles. No damage to the city truck was visible, minor damage was present on the trailer. Photographs were taken of the damage and submitted to the IT bureau. No injuries were reported. I spoke with the operator of the landscaping truck (MV2) identified as Jose Nunez (S24348117). Jose stated MV2, a red 2008 Ford drwsup dump truck was unoccupied and parked with a red 2012 Brav utility trailer (MV3) attached to it. Jose reiterated the same sequence of events as Norayr. Both vehicles were able to leave the scene without further incident and I advised both parties a report would be submitted.

On 11/6/19 at approximately 0610 hrs I responded to 15 Needham St., the National Lumber for a MVA involving a pedestrian. Upon arrival I spoke to the pedestrian who was lying on the street who stated he landed on his back and couldn't feel if anything was hurting. I advised him to stay put until the medics responded. The pedestrian was wearing brown pants and a black jacket and stated he was listening to his head phones when he attempted to cross the street. Where he attempted to cross was down Needham St from the main intersection and there was no crosswalk. He attempted to cross from the National Lumber side crossing the S/SW travel lane first. I then talked to the operator of MV#1 who stated he turned right onto Needham St from Winchester St and didn't see anybody attempting to cross until the last second. At this time he stated he jammed on his brakes and saw pedestrian on the hood of his Pickup before he fell to the ground. There was minor damage to the hood of the vehicle on the passenger side. The pedestrian was evaluated by the Medics and transported to NWH for further evaluation. Pictures were taken on scene and turned over to the IT bureau.

On Wednesday, November 20, 2019, I met with Officer Lazarakis at 25 Chestnut Street, Newton for a report of damage to Newton Police unmarked cruiser 1325. Officer Lazarakis stated she was retrieving an item out of the front passenger side door of the 2017 Ford Fusion (MA: 7HD877) when she observed a medium size dent near the door. Officer Lazarakis stated she does not know when or where the damage occurred. The vehicle has been moved several times in the last few days. The vehicle is typically parked in the rear parking lot of 25 Chestnut Street. It appears the dent was caused by a vehicle backing into the side of the vehicle. I observed no paint transfer or any other physical evidence left by the unknown vehicle. Pictures were taken to document the damage to the front passenger side door and submitted to the IT Bureau. IT was asked to check the surveillance cameras at 25 Chestnut Street to see if the incident was captured. I checked the area for vehicles for damage consistent to the damage on 1325 with a negative result.

On 12/23/19, at 1919 hours, I responded to the area of 807 Washington St. for pedestrian struck by a motor vehicle. Upon arrival with Cataldo Medics and Newton Fire, I observed Catherine Chapman being treated in the westbound lane a few feet in front of a 2010 Hyundai Elantra (MA Reg. 4KH 743). The vehicle had a shattered windshield. Ms. Chapman appeared to be wearing a dark colored jacket and dark pants. The operator of the motor vehicle, Emily Shuman, stated she was traveling westbound in the outside lane when Ms. Chapman stepped in front of her car from the eastbound lane. I also spoke with a witness, Malcolm Robinson, who stated Ms. Chapman was crossing the street from the eastbound lanes with two other people. The other two people stopped in the middle of the street but Ms. Chapman kept walking and was struck. The other two people were Victor Polk and Halimah Polk, both of which confirmed the witness statement. Ms. Chapman was transported by Cataldo to Brigham & Womens Hospital with non-life threatening injuries. Todys towed the vehicle and pictures of the scene were submitted to IT.

On Thurs 7/9/20 at approximately 1911 hrs I responded to 111 Dedham St for a report of an accident involving a bicyclist. Upon arrival I observed the MV and the bicycle on the EB travel lane side of Dedham St with the bicycle on the sidewalk/driveway and the MV facing the driveway at 111 Dedham St. I spoke with the operator of MV#1 who stated that he was traveling EB on Dedham St and then turning right into his driveway when he collided with the bicyclist. He stated that he never saw her until the last second as the bike was in his blind spot. When contact was made he immediately got out of his car to see if the bicyclist was okay. The passenger in the vehicle stated that the bicyclist was traveling Eastbound on Dedham St on the sidewalk when MV#1 struck her. The bicyclist was riding a turquoise and black Bianchi Aria racing bike with black leggings and a white and blue tank top. The bicyclist was wearing a helmet which had little to no damage on it. Before being transported to BI by the medics, Ms. Loomis stated she was traveling EB on Dedham St in the shoulder/bike lane when she was struck by MV#1 who was attempting to turn into the driveway. Pictures were taken of the scene and the SIM card was turned over to the IT bureau to be uploaded into this report. The bike was turned over to the bicyclist's friend, Christine Long.

On 08/09/2020 at 1500 hrs, I was dispatched to the area of Chestnut St. and Gordon Rd. for a Motor Vehicle Accident. Dispatch stated a UHaul truck had collided into a city tree. Upon arrival, I observed a Uhaul truck, on the sidewalk in front of 458 Chestnut St. The truck was facing northbound on Chestnut St. and had heavy front end damage as it was driven into tree #2.The operator stated that tree #1 had fallen onto the road, causing her to swerve, hit the curb and then collide into tree #2.. There were tire marks leading from the curb tree #2. The Newton Forestry Department was contacted to remove the tree #2 from the roadway, and to evaluate tree #1. (Both city tree's). Tody's Tow service arrived on scene and removed the Uhaul truck from the sidewalk. The operator stated she was not injured and declined medical attention. The homeowner at 458 Chestnut St. was give instructions on how to obtain a copy of this report, as potential damage was done to her property. Pictures of the damaged Uhaul truck, and both tree's were taken on the (N493) camera and submitted to the Newton Police It Bureau.

On 8/18/20 at 1548 hrs I responded to Sutcliffe Pk for a Waste Management truck that had taken down a telephone pole. The truck was turning around at the top of Sutcliffe Pk and got caught in the wires and an overhanging tree. As the truck continued to progress it dragged more of the wires and snapped a telephone pole causing it to fall on the street. The operator states that the truck is 13' 6" high. 5 pictures were taken of the scene and were turned over to IT. No injuries reported on scene. Eversource responded and restored power. Eversource, Verizon, and Comcast all had wires damaged due to the accident.

On August 19th, 2020, while working in marked unit N526, I responded to a MVA vs. bicyclist in the area of the intersection of Crafts Street and Watertown Street, Newton. The weather at the time of the crash was clear and cloudy. The road surface was dry. Crafts Street and Watertown Street are both public ways maintained by the City of Newton. The operator of MV1, Cody Hjelm (NHL19517660), stated he was traveling westbound on Watertown Street and had just crossed over Crafts Street when he saw a bicyclist traveling in the same direction on Watertown Street. As MV1 approached the bicyclist he moved over to the left towards the double yellow lines to give the bicyclist room. MV1 states the bicyclist was no longer in front of his vehicle when he heard a thud type noise come from the side of his van. MV1 stated he pulled over and saw the bicyclist on the ground with a head injury. MV1 sustained moderate damage to the right passenger side of the vehicle. The operator of MV1 reported no injuries and signed a patient refusal. MV1 was able to leave the scene of the accident. The bicyclist identified as Harriet Fell (S04980771) was being treated by medics on scene at the time of my arrival. I spoke with her briefly inside the ambulance as she was being treated for a head injury. Harriet stated she has no recollection of the accident and does not remember what happened before or after the crash. Medics advised she was being taken to NWH for treatment. I spoke with Harriet on the telephone around 1630 hours. She stated she still does not remember what happened but stated she was just finishing her forty mile bike ride and was heading home. Harriet stated she has a hematoma on her head and recieved three stiches. Harriet also stated she has small abrasions on both hands. Harriet stated she was being released from the hospital shortly and will pick up her bicycle tomorrow. The bicycle was transported to the police station by Newton DPW and was secured for safekeeping inside Support Services by Officer Manouk. A property form was completed and submitted along with an Incident report # 20039014. I took photographs of the accident scene and submitted them to the IT Bureau. Update by Ofc. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of the vehicle involved in the crash, Dina Hudson (S33833807). Ms. Hudson stated she was sitting in her 2015 white Jeep Grand Cherokee (MA: 727SZ7) that was parked (E) in a metered parking spot in front of 780 Beacon Street. Ms. Hudson stated traffic in the Eastbound lane of Beacon Street to her left was stopped. There was a red light ahead at the intersection and traffic had built up past where her vehicle was parked. Ms. Hudson stated she opened her driver side door while she was looking for her debit card to go to the bank. Ms. Hudson stated at this time a bicyclist was traveling at a high rate of speed around the stopped traffic towards the intersection and crashed into her driver side door. Ms. Hudson stated the operator of the bicycle then traveled further up the roadway and crashed behind a vehicle stopped in traffic. Ms. Hudson stated she was unsure if the bicyclist crashed into the stopped vehicle or if it had landed on the roadway just before it. Ms. Hudson stated the bicyclist then got up from the roadway and began yelling at her that the crash was her fault. I observed a few small fresh scrapes on the outside of the driver side door area. When I initially arrived at the crash location, I observed Officer Lazarakis checking on his well-being of the injured bicyclist, Marc Lind. Lind was observed laying on the sidewalk in front of 780 Beacon Street. After being evaluated by Newton Medics, I spoke with Mr. Lind. Mr. Lind stated he was traveling on his Trek bicycle Eastbound on Beacon Street towards Langley Road. Mr. Lind stated while traveling through the area at this time, traffic in the Eastbound Lane was stopped and there was a red light ahead of him at the intersection. Mr. Lind estimated his speed at this time to be 10-12 miles per hour. Mr. Lind stated as he travelled past the Santander Bank at 780 Beacon Street, the operator of a parked white Jeep to his right abruptly opened their door into traffic. Mr. Lind stated he attempted to avoid the vehicles door and veered to the left. Mr. Lind stated the right end of his handlebar made contact with the door and as he traveled forward and applied his brakes his right side crashed into the rear of a vehicle stopped in traffic ahead of him. Mr. Lind stated he crashed to the roadway on his right side. Mr. Lind was able to move to the side of the roadway and wait for Newton PD, Fire, and Medics to respond. Mr. Lind reported injuries to his right arm, side, and leg area. Mr. Lind signed a patient refusal with Newton Medics and was transported away from the area by his wife with his bicycle. I observed no substantial damage to his Trek bicycle. At the time of the crash, Mr. Lind was wearing a white t-shirt, black shorts, black, shoes, and a gray helmet with sunglasses. I spoke with a witness to the crash, Steve Worthington. Mr. Worthington stated he was stopped in traffic on Beacon Street about 4 vehicles back from the red light. Mr. Worthington stated he looked out of his passenger door mirror and observed a bicyclist "whipping" down Beacon Street in between vehicles parked on the side of the road and vehicles stopped in traffic at the red light. Mr. Worthington estimated the speed of the bicycle to be approximately 20 miles per hour. Mr. Worthington stated the bicyclist had to "lean" by his vehicle to get by. Mr. Worthington stated a vehicle just ahead of him that was parked opened their door and the bicyclist made contact with the door. Mr. Worthington stated the bicyclist then went down in front of the car that was in front of him. Mr. Worthington stated the bicyclist was rolling around on the ground and screaming "It's your fault" at the woman that opened her door Photos were taken of the bicycle and the vehicle involved and submitted to the IT Bureau.

On November 16, 2020 at approx 1247 hours I responded to the area of 866 Beacon Street for a report of motor vehicle crash with City property damage. Upon arrival I spoke with the oper of vehicle 1 Mr. Quinn, he stated while traveling east bound on Beacon Street, he pulled over to park properly the right top corner of the dump body had contact with a City tree in the area of 866 Beacon Street. I observed the dump truck in the eastbound lane of Beacon Street with damage to the right side dump body, right side of truck cab, passenger side door, mirror and passenger side fender, there may have been more internal damage, but I could not asses due to the tree leaning against the truck. I looked around the crash site area, I observed the City tree was not trimmed properly and was too far into the roadway in the normal path of travel for all vehicles. Forestry arrived on scene cut up and disposed of the tree. I took pictures of the crash site area that I will download at the I.T. Bureau I advised Mr. Quinn of the process.

On 12/3/2020 at 1400hrs I investigated a past motor vehicle crash involving a pedestrian. The crash occurred on 12/03/2020 at approximately 1240hrs. The crash was located in the area of 483 Centre St. Centre St is a public way in the city of Newton. Vehicle 1 was traveling southbound on Centre St near the area of 483 Centre St. Pedestrian 1 was located on the southbound side of Centre St near 483 Centre St. Pedestrian entered the southbound lane of Centre St and was struck by Vehicle 1. As a result of the impact Pedestrian 1 sustained a minor injury to his left leg. Vehicle 1 had no known damage. No parties were transported. No tow was required. It should be noted that the crash was reported by the Operator of Vehicle 1 Daniel Zandman at approximately 1400hrs. Zandman states that he was operating his Vehicle southbound on Centre St just after the intersection at Church St. Zandman states that Pedestrian 1 unexpectedly ran in front of his vehicle. Zandman stated that he did not have enough reaction time to come to a complete stop and his front bumper made contact with Pedestrian 1. Zandman stated that after impact he immediately got out of his vehicle and checked on Pedestrian 1. Zandman described pedestrian on as a 10 year old white male identified as Dexter Jagoditish. Zandman stated that while on scene Dexter called his mother Jess Jagoditish who immediately responded to the scene. Zandman stated that he exchanged information with Jess Jagoditish who then took Dexter home. Zandman states that a witness Mia Martin was also on scene. At approximately 1425hrs I called and spoke to Dexter and his mother Jess Jagoditish. Dexter states that he was walking on the sidewalk near 483 Centre St. Dexter states that he wanted to cross Centre St and thought there were no cars coming. Dexter stated that he entered the southbound lane of Centre St and a car came out of no where and hit him. I then spoke to Jess Jagoditish who is Dexter's mother. Jess stated that she received a call from Dexter around 1240hrs stating he was just hit by a car. Jess stated that when she arrived on scene Dexter was with Zandman and a witness Mia Martin. Jess stated that both Zandman and Martin explained them what had happened. Jess stated that Dexter appeared to be ok so she decided to take him home. Jess states that at this time Dexter has a bruise on his left calf but otherwise appears to be ok. At 1551hrs I spoke with the witness Mia Martin. Martin states that just prior to the crash she was walking her dog behind Dexter. Martin states that "out of nowhere" Dexter sprinted into the southbound lane of Centre St. Mia states that she then observed Vehicle 1 attempt to slow down and swerve to the lest to avoid Dexter. Mia states that the passenger side front bumper then made contact with Dexter. Mia states at impact Vehicle 1 was traveling at a very slow speed. Mia states that Dexter was then knocked on to the ground but then immediately jumped up and started to run away. Mia states that both she and Zandman were able to get Dexter to comeback so that they could check to see if he was ok. Mia states that Dexter was then able to contact his mom who responded on scene. Due to this crash being reported after the fact, Fire, EMS or Police where not initially notified and did not respond to the scene. Pictures were also not taken of the crash scene. At this time this report is for documentation purposes. After speaking with all the parties involved it appears that the crash was caused by Pedestrian 1 entering the roadway in a manner which gave Vehicle 1 little or no time to react.

On 12/08/20 at 08:42 hours, I responded to 26 Cummings Road for a city motor vehicle accident involving a parked unoccupied motor vehicle. Cummings Road is a public way maintained by the City of Newton. The weather at the time was approximately 30 degrees and very cloudy. On arrival, I spoke to the operator of M/V # 1 Alphonse Secinaro MA DL# S88518110 who works for the City of Newton Dept. of Public Works Yard out of 74 Elliot Street. Mr. Secinaro stated at the time he was sanding or cleaning up icy spots on the roadways. He was operating a 2012 International 7 Series color orange (MA Official Reg# M87605) with a plow attached to it. While driving northbound on Cummings Road between number 26 and 34, Secinaro attempted to maneuver his large plow truck between two parked motor vehicles on opposite sides of the roadway. While trying to do so, he struck parked M/V # 2 (14' Honda Pilot MA # 7RT637) on the driver's side rear parking light area. This caused the brake light cover to break and a small dent to the vehicle to the left of that brake cover. I observed M/V # 1 to be parked on the right side of 26 Cummings Road partially on the grass where there is no sidewalk. The other vehicle was parked a little more southbound on the opposite side of the roadway which made it very difficult for the large plow truck to navigate through. I observed no parking restrictions for both sides of the street. Digital photos were taken of the scene and submitted to the IT Bureau.

Uninvolved owner of MV1 was believed to be parked facing southbound in front of 97 Hawthorn St. when a dark colored jeep wrangler struck MV1 head on and dragged the vehicle to 88 Hawthorn St. MV1 came to rest at an angle on the sidewalk and damaged a fence in front of 88 Hawthorn St. The front bumper of MV1 was left in the roadway in front of 97 Hawthorn St. along with debris from the impact. Based off where the debris and front bumper were located, it was believed the impact of the crash was in that area. A witness called the police and stated he heard two loud bangs outside and observed what he believed to be a dark colored jeep wrangler with heavy front end damage leaving the scene. I spoke to witness, Mr. Kenis who stated he observed the operator struggle to start the engine after the impact and took off before he could obtain a registration plate. Mr. Kenis stated he fled towards Watertown St. Units checked the area for the other vehicle believed to be involved, however had negative results. MV1 sustained heavy front end damage along with minor left rear end damage where the vehicle struck the fence. MV1 was unoccupied during the collision. No injuries. Pictures were taken on scene and placed into the IT Bureau's box. Property owner, Joe Russo of 88 Hawthorn St. was notified of the damage to his fence. Mr. Russo stated he was going to contact his insurance company and was satisfied. Owner of MV1 was also contacted and notified of the accident. Tody's responded and towed MV1. A towed motor vehicle form was filled out and placed on file.

On 04/20/21 at 11:46 hours, I (Traffic Unit 523) responded to the CVS parking lot located at 978 Boylston Street for a city motor vehicle hit & run accident. The city vehicle was struck when the operator was inside the store and the other unknown vehicle left the scene. The CVS parking lot is a thru-way for the public to access and park for the store. The weather at the time was partly cloudy and the road conditions were dry. On arrival, I met the driver of the city vehicle identified as William Bertrand (MA D/L# S24973075). The vehicle is a 2016 Ford Escape color black with MA registration # M2534A and is labeled as vehicle number 304. Mr. Bertrand stated he was parked in the lot and went inside the store. The Escape was parked in the seventh spot in from the parking stalls closet to the entrance of the store. He then came out and observed that the vehicle had been struck on it's rear driver's side quarter panel area. He says the other vehicle had left the scene and no note was left on his vehicle. I observed numerous scrape marks to that mentioned area and I took digital photos of the damage. I also observed two small specks of paint on the damaged area that were a bluish gray type of color from the suspect vehicle. It appears that due to the scrape marks that the unknown vehicle was pulling in to the parking space to the left of the city vehicle. As it was pulling in, it struck the city vehicle with it's front passenger quarter panel area. I spoke to Nick (CVS manager) on the phone and was inquiring about possible surveillance footage of the lot. He stated that he didn't believe the cameras cover that area of the lot but he would look for any possible footage and notify me if so.

On Thursday May 27th at approximately 0859 hours I responded to 14 Thornton Street for a motor vehicle accident involving a city contracted Waste Management truck. At the time of the accident the weather was sunny and the road surface was dry. Thornton Street is owned and maintained by the City of Newton. Upon arrival I spoke with the operators involved. Operator of MV1, MA REG 31X870 identified as Amado Lebron (S92772420) stated he was stopped behind the Waste Management truck as it was collecting trash. He stated he could not see around the truck to pass so he stayed behind it. Amado stated the truck started to back up and he started beeping to alert the driver of the truck. Amado stated the truck did not stop in time and his vehicle's front end was crushed by the truck. Amado stated he was not injured. The operator of the Waste Management truck was identified as Jose Lopes (S60186065). Jose stated he was picking up trash bins with the truck and started to back up to collect trash. Jose stated if a vehicle is too close behind him there is no way to see it. He stated he heard the beeping and thats when he stopped the truck and noticed he backed into a vehicle. There was no damage to the truck and Jose was not injured. Jose was standing by for his supervisor with the truck. Amado was waiting for a tow and his vehicle was safely off to the side of the road. Motor vehicle 1 (MV1) was traveling northbound on Myrtle Street when it crashed into a low hanging City of Newton tree in front of 12 Myrtle Street. As a result of the crash the tree completely uprooted and ripped out the telephone/internet services to 10 and 12 Myrtle Street, as well as 1560 and 1554 Washington Street. MV1 sustained moderate front/top passengers side damage. The City of Newton Forestry division was able to respond and remove the downed tree. Narrative:
On 06/24/21 at 09:20 hours, I (Traffic Unit 523) was dispatched to 140 Adams Street for a city motor vehicle accident involving a Waste Management truck and a parked motor vehicle. Waste Management of Mass Inc. is contracted through the City of Newton to conduct trash pickup for the residents. The weather at the time was sunny and partly cloudy with road conditions being dry. Adams Street is a public way owned and maintained by the City of Newton. On arrival, I spoke to the operator of the Waste Management Truck identified as Jose Lopes (MA D/L # S60186065). Mr. Lopes was operating (M/V # 1) a 2010 Mack Leu dump truck bearing MA. Commercial Reg. # N81671 at the time. It is a right side loading truck that picks up the trash using a double claw mechanism. Lopes states that he did a trash pickup in the driveway right before 140 Adams Street and proceeded to drive northbound on Adams Street. While doing so, the left side double claw retracted back outward after it was closed by him. The claw then struck parked M/V # 2 which was parked in a legal spot in front of 140 Adams Street. The claw struck the driver's side rear bumper/taillight area and scraped along until the driver's side rear passenger door. Mr. Lopes then stopped the truck after noticing. This caused scrapes and dents to the rear bumper, rear quarter panel, and the rear door to that driver's side area. After, I was showed that the truck's claw feature was having mechanical failure as mentioned above. The claw was manually pushed in and secured. The truck was then put out of service. M/V # 2 is a 2013 Jeep Compass color blue bearing MA reg. # 3055GB. The owner Joseph Cedrone (MA D/L # S50190711) was parked and inside his vehicle at the time of the accident. No injuries were reported by him. Digital photos taken of the scene.

On Monday, July 19, 2021, while assigned to Traffic unit N525, I responded to the area of Commonwealth Avenue and Lowell Avenue, Newton for a report of a MVA/Pedestrian crash. Commonwealth Avenue and Lowell Avenue are both public ways maintained by the City of Newton. The weather at the time of the crash was overcast with light rain. The road surface at the time of the crash was wet. Feeney Brothers Utilities has been performing gas repair services at this intersection for the past month and they have had a paid Newton Police Detail with them during their service. The detail presented itself as follows on my arrival. There was service being conducted in the northbound lane of Lowell Avenue in-between the Commonwealth Avenue carriage roads. A large trench was present in the northbound lane that made the roadway impassible. Traffic was permitted to travel down the southbound lane of Lowell Avenue towards Commonwealth Avenue for the detail. Access to Lowell Avenue from Commonwealth Avenue was prohibited. The northbound lane was blocked by orange construction cones and road closed signs. There was also no access to the Commonwealth Avenue carriage roads at this location. Both of these roadways were closed with orange construction cones and caution tape. There was also caution tape and orange construction cones restricting access to a walkway that traveled from Commonwealth Avenue to its carriage road just after Lowell Avenue (E). I also observed a "Sidewalk Closed" sign posted on the walkway. I spoke with Officer Gabriel of the Newton Police Department. Officer Gabriel stated he was working a paid detail for Feeney Brothers at this location. Officer Gabriel stated he had just assisted two female parties cross Commonwealth Avenue (E) at Lowell Avenue. Officer Gabriel stated a short time later, he heard a scream and observed two female parties laying on the roadway on the opposite side of Commonwealth Avenue in the carriage road. Officer Gabriel immediately contacted Newton Police Dispatch via his portable radio and requested aid for the injured parties. Prior to my arrival, Officer Gabriel stated one of the pedestrians was transported to Newton Wellesley Hospital with a head injury. I spoke with one of the pedestrians involved in the crash, Ms. April Stein. Ms. Stein stated she was walking in the carriage road of Commonwealth Avenue (E) towards Lowell Avenue with her friend Sarah Kish. Ms. Stein stated when they got to Lowell Avenue there was work being done in the roadway, and a Police Officer assisted them with crossing the area. Ms. Stein stated they then re-entered the carriage road when they crossed Lowell Avenue. Ms. Stein stated they were walking along the edge of the roadway when they were hit by a front loader. Ms. Stein stated they fell to the roadway due to the collision. Ms. Stein stated she did not hear or see the front loader backing up towards them and did not notice it because they were having an "intense conversation". Ms. Stein reported no injuries and signed a patient refusal with Fallon Medics. I asked Ms. Stein if she had transportation to her residence. Ms. Stein stated she would walk home. I spoke with the operator of the front loader involved in the crash, Mr. Mark Mannion (S91049325). Mr. Mannion is an employee of Feeney Brothers Utilities and stated he was operating a 2015 Case Loader (MA CON: R99118) at the time of the crash. Mr. Mannion stated he was backing up the loader down the Commonwealth Avenue carriage road (E) from Lowell Avenue. Mr. Mannion stated he was going to get a load of fill from a pile of dirt on the side of the road inside the marked off work area. Mr. Mannion stated as he was backing up, he was looking over his right should behind him to make sure there were no obstacles in his way. Mr. Mannion stated his emergency backing alarm was activated while he was backing up. Mr. Mannion stated he noticed something enter the roadway to his right side and immediately slammed on his brake. Mr. Mannion stated then heard a scream and saw 2 female parties on the ground. Mr. Mannion stated he exited the loader and checked on the welfare of the two women. Mr. Mannion's CDL License, Vehicle Registration, and Hoisting License are all active. I asked Mr. Mannion to put the vehicle in reverse to assess if the emergency backing alarm was functioning properly. Mr. Mannion was cooperative and while reversing the vehicle the emergency backing alarm was functioning properly in my presence. I asked Mr. Mannion if the loader had moved since it was brought to a stop due to the crash. Mr. Mannions stated it had not moved. I observed this vehicle to be within the parameters of the work zone that was taped off. The vehicle at this time was released back to Feeney Brothers. Photos were taken of the work site and loader and submitted to the IT Bureau. I responded to Newton Wellesley Hospital and spoke with Ms. Sarah Kish (S85088490) in the Emergency Room. Ms. Kish stated she was walking down the Commonwealth Avenue carriage road (E) towards Lowell Avenue with her friend April Stein. Ms. Kish stated a Police Officer assisted them with crossing Lowell Avenue via the crosswalk located on Commonwealth Avenue. Ms. Kish stated after they crossed Lowell Avenue, they walked to the carriage road. Ms. Kish stated as they were walking, she was hit from behind by a front loader in the roadway. Ms. Kish stated she did not hear or see the front loader and believes the bucket hit her in the head. Ms. Kish stated she fell to the roadway. Ms. Kish was transported to Newton Wellesley Hospital for her injuries. Ms. Kish stated at this moment, she had some lacerations to her back and head, and her left shoulder/neck/back area was sore. I asked Ms. Kish how often she walks that particular route. Ms. Kish stated 5 times a week. Ms. Kish stated there was nothing in place to restrict her access down to the carriage road after she crossed Lowell Avenue. I spoke with a witness to the crash via telephone, Ms. Megan Boitano. Ms. Boitano stated she was traveling on Lowell Avenue (N) towards Commonwealth Avenue. Ms. Boitano stated she was stopped at the light right next to the trench that was in the northbound lane of Lowell Avenue. Ms. Boitano stated she observed two women crossing Lowell Avenue at Commonwealth Avenue in front of her. Ms. Boitano stated she then observed a heavy piece of construction equipment approaching the trench to her left at a high rate of speed. Ms. Boitano stated it startled her as the front loader dumped dirt into the trench. Ms. Boitano stated she then observed the front loader back down the carriage road at a high rate of speed towards a dirt pile. Ms. Boitano stated she observed the two women that just crossed Lowell Avenue in the roadway behind the front loader. Ms. Boitano stated she lost sight of the women, and then observed the front portion of the front loader aggressively shake. Ms. Boitano stated she then observed one of the women walk around from the side of the front loader holding her head. As presented while investigating this crash, the Feeny Brothers work site appeared to be properly set up to mitigate a potential safety incident with ample cones, barriers, caution tape, and signage. There was also a police detail present to assist with pedestrian travel. The area of the work zone where the pedestrans entered from Commonwealth Avenue after crossing Lowell Avenue was taped off with caution tape and a sidewalk closed sign was present. The crash happened inside the work zone parameters.

Operator of vehicle one Newton FF Eric Howard stated that while getting into the drivers seat of Newton Medic Unit 3 a Fallon Ambulance ( 2021 Ford Econoline MA reg AMN 7715 ), that was parked in front of 19 Faxon St, he inadvertently stepped on the break and disengaged the gear shift so that the ambulance went from park to drive. FF Howard was getting ready to drive Newton Medic Unit 3 to Newton Wellesley Hospital while Fallon Medics attend to their patient in the rear of the ambulance. FF Howard stated that after the ambulance went into drive it drove south bound on Faxon St and it's front driver's side fender struck the rear passenger side bumper of vehicle two. FF Howard stated that he was not injured in the crash and was able to drive the ambulance to Newton Wellesley Hospital. Fallon Medics Jared Gaglio and Shane McMahon stated that they were in the rear of the ambulance treating their patient, Dominica Tramontozzi when they felt the ambulance move forward and strike vehicle two. Both Gaglio and McMahon stated that they were not injured in the crash. I was unable to take a statement from Tramontozzi. Operator of Vehicle two Newton FF Joseph Colella stated that he was sitting in the driver's sea of Newton Fire Dept Eng 8 ( 2004 E-One Pumper MA reg MF471 and Newton Fire LT Douglas Quinn was sitting in the front passenger seat when the crash occurred. Vehicle two was parked in front of 19 Faxon St. FF Colella stated that the front driver's fender of vehicle one stuck the rear passenger bumper of vehicle two. Both FF Colella and Fire LT Quinn stated that they were not injured in the crash. Vehicle two did not require a tow. Vehicle one was towed from Newton Wellesley Hospital after a tow was arraigned by a Fallon Ambulance Supervisor. I took photos of both vehicles and forwarded the disk to the NPD's IT Bureau for downloading. Faxon St is public way in the City of Newton.

On 07/27/2021, while assigned to N494, I, Officer Conary, spoke with DEALE, Cathy (Operator of MV1) over the phone about a past hit and run. DEALE explained to me that she parked Westbound on Washington Street in front of 513 by the fire hydrant. Between 1200-1400 MV1 was hit on the driver side, and the mirror broke off. Operator of MV1 did not see any other vehicle standing by, she left the area and notified Newton Police. Operator of MV1 sent me pictures to confirm the damage. I went back to the area of where the accident was reported and confirmed glass and a pieces of a mirror on the side of the road.

Operator of MV1 was driving northbound on Centre Street and was turning left onto Walnut Street. As he began to turn, he lost control of the vehicle and the front tires locked up and the rear tires began to spin out. He stated this was a rental vehicle that he picked up a few hours prior to the accident, and since it was picked up, it had the ABS and all wheel drive warning lights on. He stated several other dashboard warning lights came on as he was turning onto Walnut Street. Once he grained control of the vehicle, he struck several guardrail poles in front of 1200 Walnut Street, and crossed into the parking lot of 1194 Walnut Street. As the car slid into the parking lot, it took down three parking signs for Newton Highlands Wine and Spirits. The operator of MV1 and his two passengers were evaluated by Fallon Medic 4. All three signed a patient refusal. The vehicle sustained heavy damage with all front and side air bags deployed. David Donahue of Todys' Towing responded and towed the vehicle. 4 Photos were added to the incident report attached to this crash report showing the damage to City Property and Newton Highland Wine and Spirits.

MV#1 WAS FLOODED INSIDE AND WITH THE CURRENT IT WAS PUSHED UNDER THE FOOT BRIDGE WHICH CAUSED MAJOR DAMAGE TO IT. TRAFFIC RESPONDED AND TOOK PICTURES. TODY'S RESPONDED WITH THE FIRE DEPARTMENT TO LIFT MV#1 OUT OF THE BROOK. MV#1 WAS TOWED TO THE TOW YARD. MEDICS DID NOT RESPOND SO THERE WAS NO PATIENT REFUSAL.

Owner of Vehicle 1,Mary Malloy, stated that at approximately 1630hrs she heard a loud bang out in front of 45 William Street. Malloy stated she went out front and observed a blue dump truck driving southbound on William Street. Malloy said she was unable to see any writing on the vehicle. Malloy said she saw there was damage done to her vehicle. The front driver side bumper was hanging off and there was scratches starting from the rear driver door all the way up to the front driver side door causing the driver side mirror to be taken off as well. Malloy was visiting her friend, Barbara Harteln, who lives at 45 William Street. Harteln has a ring camera which catches the blue dump truck driving southbound on William Street. However Harteln informed me the camera did not pick up Malloy's vehicle being struck. I provided Harteln with my department email and advised her to send me the footage from her ring camera. I canvassed the neighborhood and was unable to find any other cameras near 45 William Street. I provided Malloy with the crash report number to this incident and advised her to contact her insurance company and to take photographs of her vehicle. Malloy said she was going to contact AAA to come and tow the vehicle to a dealership near her house in Hingham.

On Friday September 17th, 2021 at approximately 0849 hours I responded to the area of the Elm Street municipal lot for a motor vehicle accident involving a city owned vehicle. At the time of the accident the weather was cloudy and the road surface was dry. Elm Street is a public way owned and maintained by the City of Newton. Upon arrival I spoke with the operator of the City of Newton Parks and Recreation truck, identified as Benjamin Langkopf (S08711384). Benjamin stated his vehicle (MA REG; M3732A) was parked on the side of the road on Elm Street when a Ryder box truck pulled into the parking space in front of him and sideswiped his front drivers side area with the trucks rear passenger side. No injuries were reported and the city vehicle sustained minor damage. Photographs of the damage were taken and submitted to the IT Bureau. The operator of the Ryder Rental truck was identified as Hykeem Duane Wilkey (S76520284). Hykeem stated he was operating a Ryder Rental box truck (MA CON REG; T40779) and attempted to pull into a parking space on the street when his rear passenger side struck the front drivers side of the city vehicle. No damage was reported and the operator had no injuries.

Operator 1 stated she was traveling Southbound on Walnut St and tried to turn left into her driveway located at 926 Walnut St. As she tried to pull into her driveway, her vehicle got stuck in a ditch that was dug up by the city. There was also about an inch of water in the ditch, it is unknown if it was related to the excavation work. It should be noted that there was no cones placed around her driveway. There was minor damage to the front bumper of vehicle 1 and operator 1 had AAA towing removed the vehicle from the ditch and she was able to park it in a neighbors driveway for the night. On Sunday, October 10, 2021, at approximately 1601 hrs, while assigned to N494, I responded to 320 Washington St., the Four Points Sheraton, (rear Centre St. side) for a single motorcycle accident ( MV1- MA Reg 2S2136). Upon arrival, the fire and medics were treating the operator of the motorcycle, identified as BASS, Braxton Lee. BASS stated he came off the Mass Pike Eastbound exit, traveled three lanes to very left lane on the Washington St. rotary (EB), and he hit a pot hole in the road. He stated he then lost control of the motorcycle and was thrown over the handlebars crashing onto the sidewalk. The motorcycle skidded the length of the curb until the stair area of the hotel. BASS sustained injuries to his left shoulder, hands, left torso, knees, and the left side of his head. He was transported by Medic 5 to the BI Boston. The motorcycle sustained minor damage and was towed from the scene by Todys. I observed a dip in the left lane where BASS and witness on scene stated he initially lost control of the motorcycle. See attatched photos. Witness statement: MAZZIE, Zach stated he witnessed BASS crash his motorcycle. He stated BASS came off the Mass Pike exit, traveling Eastbound on Washington St., when he proceeded to the left lane, and was thrown from the motorcycle. He stated he saw him hit a bump in the road, at which point he lost control of the bike and crashed, landing on the curb into the fence. On October 28, 2021 at approximately 18:26 hours I, Officer Guarino, spoke with Craig Gimbrone over the phone, who reported that he was hit by a motor vehicle in the crosswalk at 1221 Centre St between 15:00 and 15:13 hours. Craig said that he was crossing the crosswalk at 1221 Centre St east to west and was struck by a black car traveling southbound in the far right lane of Centre St. Craig said that the vehicle struck him and caused him to fall over, Craig got up and continued to cross the street and enter CVS without speaking with the operator of the vehicle. Craig did not get a license plate, description of the vehicle or the operator. Craig could only recall that the vehicle was a black sedan. Craig was wearing blue jeans and a gray jacket at the time of the collision. Craig said that the crosswalk flashing lights were not activated prior to him crossing the roadway. Crag said that he did not need EMS but was starting to have pain in his right ankle and right shoulder. He emailed me photographs of his ankle which was forwarded to IT to be attached to this report. Craig was given the crash report number for his records.

On Friday November 5th 2021, at approximately 1304 hours I responded to the area of 36 Greylock Road for a two car motor vehicle accident involving a city contracted Waste Management receptacle truck. At the time of the accident the weather was sunny and the road conditions were dry. Greylock Road is owned and maintained by the City of Newton. Upon arrival I spoke with the operator of the garbage truck identified by RI License 9898223, as Norbin Cordon. Norbin stated he was operating the garbage truck and travelling westbound on Greylock Rd. Due to cars parked on both sides of the road he stopped to let another vehicle go by him coming from the opposite direction. The other vehicle stopped at the bend to let the garbage truck go by. Norbin stated it was a tight squeeze and when he tried to make his way through his rear drivers side tires made contact with the drivers side panel of the other vehicle. The only damage to the garbage truck was a smudge on the tire. Norbin reported no injuries. I then spoke with the operator of the other vehicle involved, identified by Mass Lic as Jane Lewis S79838916. Jane stated she was traveling eastbound on Greylock Road and stopped in the road to let the garbage truck pass. As the truck attempted to pass it struck her rear drivers side panel causing moderate body damage. Jane reported no injuries. Several photographs of both vehicles were taken and submitted to the IT Bureau. Jane's vehicle was able to drive and she left without further incident.

On Monday November 8th, 2021 at approximately 0855 hours I responded to the area of 182 Webster Street for a motor vehicle accident involving a City of Newton DPW truck. At the time of the accident the weather was sunny and the road surface was dry. Webster Street is owned and maintained by the City of Newton. Upon arrival I spoke with the operator of the city vehicle identified by Mass License as Michael Cannistraro S40721835. Michael stated he was travelling eastbound on Webster Street when the other vehicle involved backed out of the driveway and struck the front passenger side bumper of the truck. Michael reported no injuries and the vehicle was drive able. I then spoke with the operator of other vehicle involved, identified by Mass permit license as Sophia Mercado SA5660670. Sophia stated she was backing out the driveway and stopped, and that is when the DPW struck her rear end. Sophia reported no injuries and minor damage to the vehicle was reported. On 12/13/21 at 11:04 hours, I responded to the Mount Ida Campus of UMASS Amherst located at 777 Dedham Street for a one car crash involving a City of Newton Department of Public Works vehicle. The crash occurred at a y type walkway/driveway near the Shaw Building dining hall. The weather at the time was clear and approximately 50 degrees and the road conditions were dry. On arrival, I spoke to the operator of the DPW vehicle identified as William Bertrand (MA D/L # S24973075 ). Bertrand was operating a 2016 Ford Escape color black bearing MA Municipal registration # M2534A at the time of the crash. He states that he was at the Shaw campus building to conduct an inspection and was parked at a y type walkway/driveway outside of the building. When leaving, he attempted to make a three point turn to exit the driveway. While backing, he states that his foot slipped transitioning from the gas to the brake and he struck a light pole near the edge of the driveway. He says the back up sensors never alerted him of the pole either. The Ford Escape sustained minor scrape marks to the driver's side rear bumper area. As a result of the impact, the light pole cracked at the base and was knocked over on to the ground. Campus Security was on scene and will notify school administration regarding the damage to the pole. I took digital photos of the scene and submitted them to the I.T. Bureau.

Operator of MV1 stated they were coming down their very steep driveway at 40 Baldpate Hill Rd. when their vehicle slid across the street, and into the front exterior of 49 Baldpate Hill Rd. It should be noted it was snowing and there was snow on the driveway and roadway. There was front end damage to MV1 and damage to the exterior and garden of 49 Baldpate Hill Rd. Tody's responded to dislodge the vehicle from the home. MV1 was parked legally out front of 49 Baldpate Hill Rd. for the night. Both the operator and passenger of MV1 signed patient refusals. The homeowner of 49 Baldpate Hill Rd. was inside at the time of the crash but was unharmed. The homeowner took pictures of the damage, was made aware of the situation and advised to contact their insurance company regarding the damage to the home.

On Friday March 4th 2022 at approximately 1027 hours I responded to 90 Crafts Street the DPW City yard for a past report of a motor vehicle accident involving a city vehicle. The operator of the vehicle MA REG M98758, identified as Jeffrey Johnson S89595004 stated that on Thursday March 3rd 2022 around 1330 hours while driving westbound on Madison Ave he struck a parked vehicle with his passenger side mirror. Jeffrey stated he didnt realize there was any damage to his vehicle until he returned to the city yard. Jeffrey stated he went back to the area to try to locate the other vehicle involved but could not locate it. There is no vehicle information for the other vehicle involved. Pictures of the minor damage to the city vehicle were taken and submitted to the IT Bureau. Madison Ave is a public way owned and maintained by the City of Newton. No injuries were reported.

On Thursday March 24th 2022, at approximately 1126 hours i responded to the area of 380 Watertown Street for a minor motor vehicle accident involving a city vehicle. At the time of the accident the weather was rainy and the road surfaced was wet. Watertown Street is owned and maintained by the City of Newton. Upon arrival I spoke with the operator of the City of Newton DPW truck identified as Kelvin Sanders. Kelvin stated he was operating MAR REG M2526A travelling east on Watertown Street. Kelvin stated a parked vehicle on his right opened the drivers side door and struck the passenger side mirror of his truck causing minor damage. The operator of the parked vehicle identified as George Hoffman stated he was parked and opened his drivers side door and was unsure if he struck the DPW truck or the DPW truck struck his door. Minor damage to the drivers side door and panel were photographed as well as the minor damage to the DPW truck. No injuries were reported and all vehicles were able to leave the scene without further incident. Photos were sent to the IT Bureau.

On Tuesday April 12th 2022 at approximately 0915 hours I responded to the area of 197 California Street for a city of Newton fire truck that struck a fire hydrant while responding to an emergency. At the time of the accident the weather was cloudy and the road surface was dry. California Street is owned and maintained by the city of Newton. Upon arrival I spoke with the operator of Newton Fire Engine 1 identified as Joseph Frank. Joseph stated that while attempting to park the fire engine he struck the fire hydrant, causing the fire hydrant to be knocked over and inoperable. Minor damage to the front drivers side of the fire engine was photographed and submitted to the IT Bureau as well as the damaged fire hydrant. No injuries were reported. City of Newton was notified of the damaged hydrant.

On Monday May 16, 2022 at 0848 hours vehicle 1 was stopped in front of #188 Lexington St waiting to pick up a child. While parked vehicle 1s drivers side mirror was hit by a passing vehicle traveling southbound towards Commonwealth Ave with out stopping. The operator of vehicle 1, Claire Rice was driving for JSC Transportation and had one child aboard, Amir Ayden-Cronin who was being transported to Newton Early Childhood on Jackson Rd. Neither Rice or Ayden-Cronin reported any injuries and signed patient refusals with Fallon medics. Sahar Zaheer (617-943-8356) is the mother of Amir and I spoke to her over the phone. Zaheer was notified of the situation and gave consent for medics to evaluate her son and for him to continue to school for the day. I contacted Susan Downs (781-272-8555) of JSC to attempt to see if the vans dash camera captured any of the incident but have not heard back at this time. Rice had already spoken to her dispatch who did not believe the camera would have been clear enough to pick up anything to assist in identifying the other operator. Officer Gaudet took pictures of the damage to the vans mirror.

Owner of MV1 said she was standing in front of her house, 135 Randlett Park when she heard a loud bang. Owner of MV1 saw a landscaping company truck and trailer speeding away from her car. Owner of MV1 noted that there was a vehicle parked directly across the street making the street narrow but said trucks have been going up and down the street all day without any issues. Owner of MV1 said she could not make out the lettering on the truck but got the license plate which is 459567. Owner of MV1 said she's certain that this was the license plate but there is a chance she missed a number. I ran the license plate and it comes back cancelled to a 1988 American Command Pick Up truck and there is no contact information for the owner of the vehicle. It shows that the registration has been expired since 12/31/1989. Owner of MV1 showed me the damage that was done to the vehicle which was a flat tire on the rear direr side. Owner of MV1 made arrangements for AAA to come and take care of the vehicle. I provided Owner of MV1 with this accident report number. I canvassed the area for the landscaping truck with negative results. Owner of MV1 contacted me stating her neighbor has a picture of the truck off of their front door camera. Owner of MV1 emailed me the picture and I forwarded the picture to IT to be attached to this report. It should be noted you can not make out the lettering of the company in the photo.
