On 01/11/2019, I responded to 1466 Chestnut St for a four vehicle accident. On arrival, I spoke with the operator of MV1 who stated that she was driving westbound on Commonwealth Ave and was unsure what happened but thought the vehicle in front of her came to a sudden stop. MV1 stated she attempted to stop but could not, and collided with the rear of MV2. The operator of MV2 stated that he was stopped on Commonwealth Ave behind MV3 when he was hit from behind by MV1 and pushed into the rear of MV3. The operator of MV3 stated she was stopped behind MV4 when he was rear ended by MV3, and pushed into the rear of MV4. The operator of MV4 stated that she was stopped on Commonwealth Ave due to traffic turning into the driveway of 1466 Commonwealth Ave. MV4 stated that she was stopped, with her left turn signal on for some time before she heard the sounds of a car accident behind her. MV1 sustained heavy front end damage and was towed by Tody's. MV2 sustained rear end and front end damage and was towed by AAA. MV3 sustained rear damage and minor front end damage. MV4 sustained minor scratches to the rear bumper. All parties involved declined medical attention.

Operator of MV1 stated he was traveling Eastbound on Commonwealth ave in his lane of travel when he observed MV2 parked on the right hand portion on Commonwealth ave. Operator of MV1 stated as he was approaching MV2 he attempted to slow down in an effort to avoid colliding with MV2, but stated he could not stop due to the slippery surfaces on the road and collided with MV2. MV2 was parked and unoccupied at the time of the accident. I spoke to Owner of MV2 who shorty arrived on scene after the accident who stated he parked in front of 676 Commonwealth ave for a while to shovel snow in his driveway. No injuries. MV1 was towed by AAA and MV2 was driven off by owner to his nearby residence. MV1 sustained heavy right front end damage. MV2 sustained heavy rear end damage.

On 01/31/19 at 10:39 hours, I responded to the parking lot of the Jewish Community Center located at 333 Nahanton Street for a motor vehicle hit and run accident. The parking lot is accessible to the public and designed for vehicles to travel to and from the JCC. Owner of M/V 1 states she was parked on the side of the handicapped parking stall thinking it was ok to park there. She then ran in to the JCC for approximately one minute and when she returned she observed her driver's side front door was struck. I observed significant damage to the door frame and the door would not close. The owner called for a private tow to remove the vehicle. The staff at the JCC stated that a Martha Bixby gave her number and stated that she observed a black colored car with MA reg. # 934HH4 strike M/V 1. I attempted to call the number and left a message. The listing for that plate comes back to a 2010 Lexus ES350 color gray. I was then showed the video surveillance from the JCC's cameras and it shows an older white female exiting the lobby wearing a winter hat and carrying a bag with purple flower designs on it. She then walks through the lot and enters a gray Lexus ES350. The Lexus then starts backing out of the spot. The Lexus then continues backwards in to the other side of the parking stalls and strikes M/V 1 on the driver's side front door. The Lexus then pulls forward and leaves the scene without stopping or providing her information. As the Lexus is leaving, you can see there is a dent to the driver's side rear bumper. The video is not clear enough to see the plate on the Lexus or clearly see the operator's face. After, I located a number for the owner of the Lexus M/V 2 (Tania Lefman). I spoke to Lefman on the phone and she stated that she was backing out of the JCC parking lot at that time and felt a tiny bump to her rear end. She says that she could not stop in time due to the icy parking lot and then left the area thinking that nothing was wrong, it was only minor. I advised her there was significant damage to the other car. She was issued and mailed MA Uniform Citation T1441054 for Ch. 90/24/C Leaving the scene of property damage.

FOUND A SECOND NUMBER ( 860-301-1336 ) FOR THE METROWEST PACKAGING COMPANY AND LEFT ANOTHER VOICE MAIL.WILL TRY TO MAKE CONTACT AGAIN BEFORE PURSUING OTHER OPTIONS. Traffic Bureau Update (Officer Gaudet): On Monday, February 4, 2019 I made contact with the President of Metrowest Packaging, Brian Hersh. Hersh stated his company does fulfillment deliveries for Amazon. Hersh was advised that a vehicle registered to his company was allegedly involved in a hit and run accident. Hersh was provided information on the vehicle involved. Hersh stated an employee named Alexis was operating MA CON:T65665 on the date of the accident and provided a phone number for this party. On Tuesday, February 5, 2019 I spoke with Alexis Laporte-Soto (S84358654). Laporte-Soto stated he was operating a 2017 Mercedes Sprint van (MA CON:T65665) in Newton for Metrowest Packaging on the date of the accident. I explained to Laporte-Soto the vehicle he was operating was alleged to have been involved in a hit and run accident on Highland Street in Newton. Highland Street is a public way maintained by the City of Newton. Laporte-Soto stated the vehicle he was operating hit the driver door mirror of a parked card on Highland Street. Laporte-Soto reported the accident as mirror (MV2 passenger side) to mirror (MV1 driver side). Laporte-Soto stated after his passenger side door mirror made contact with MV1's driver side door mirror, he stopped his vehicle 15 feet down the road. Laporte-Soto stated he walked up to the car he hit and saw the mirror was bent back but there was no damage. Laporte-Soto stated he waited near the car for 10 minutes and the owner never showed up. Laporte-Soto also stated a "random guy" took a picture of his license plate. Laporte-Soto stated he said "hi" to this party but the party ignored him and left the area. I asked Laporte-Soto why he did not call Newton Police to assist him in finding the owner of the vehicle he hit. Laporte-Soto stated he didn't know. I also advised Laporte-Soto that the name of the owner of the vehicle he hit was printed on the driver side door next. The owner's phone number and email address were also printed on the driver door. Laporte-Soto stated he didn't see that. I spoke with the owner of MV1, Jason Correria. Correria stated a co-worker observed the entire accident and observed MV2's passenger side mirror hit MV1's driver side mirror. Correria stated his co-worker then observed MV2 travel down Highland Street with out stopping and pull into the Santander parking lot located at 1314 Washington Street. Correria stated his co-worker saw the operator of MV2 exit his vehicle, check his passenger side door mirror for damage, and leave the area. Laporte-Soto will be mailled Massachusetts Uniform Citation T1443121 for Chapter 90, Section 24 (Leaving the scene of property damage).

On 2/3/19, I was dispatched to 33 Austin St (Star Market) for an accident in the parking lot. On arrival I met with the operator of MV2 (MA Reg: 819VR6). MV2 stated that she was turning into the Star Market parking lot while someone was exiting the lot in the wrong direction. MV2 stated that due to the other vehicle going out the wrong way, she had to move over and collided with MV1. The operator of MV2 stated that her car came in contact with the drivers side front bumper of MV1. The operator of MV1 (MA Reg: 977GE4) was not on scene at the time of the accident. Owner of vehicle #1 reports that a waste management driver alerted her that he observed a blue F250 pickup truck back up from the driveway across from her and hit her vehicle on the rear drivers side. operator of suspect truck then pulled back into the driveway before backing out again heading east towards jasett st. witness gave possible ma registration of 969-wr4 but the plate didn't match the description of the vehicle. I will check with residents of property of #15 jasett to see if they know anyone who operates a blue F250. There was no answer at the door at this time. On February 10th, i went back to #15 jasett st and spoke with Jesse Grillo and told her about this accident. i asked her if she new anyone who drove a blue ford f250 that might have came to her home looking for her or her husband? She stated no.

On Monday, February 11th, 2019, I responded to a two vehicle MV accident in the parking garage at Lifetime Fitness Center at 300 Boylston St. There were no injuries. Vehicle # 1 (Bearing Mass Reg # 16KK69, Toyota Rav4, owned by Sasha Bordett) was unoccupied and parked in a marked spot facing in. Vehicle # 2 ( Bearing Mass Reg #875LD0, Toyota SUV, owned/operated by Cassandra Spittel) was parked backed in to the spot next to vehicle # 1. Cassandra stated she took a right hand turn out of the parking spot and heard a noise. She stopped her vehicle and inspected the damage and saw scratches to the right front side of her vehicle. She also noticed a dent to the right rear of Bordett's vehicle. Since the other vehicle was unoccupied, Cassandra called Newton Police and stated she had been in an accident and left her info to myself to make an accident report. I contacted the owner of the Toyota Rav4 and informed her of the accident and informed her their would be an accident report on file.

The owner of MV#1 stated her vehicle was parked in the rear parking lot (Located on Rice St) of 1066 Centre St and was informed by 2 witnesses that stated her vehicle was struck by MV#2. I spoke to the 2 witnesses and they stated they were on the second floor of 1066 Centre St in suite #212 and had a bird's eye view of the rear parking lot on Rice St. The witnesses stated that MV#1 was parked while MV#2 was parked at an angle at a very close proximity to MV#1. The witnesses stated the operator of MV#2 was cleaning the snow off her vehicle, got into her vehicle then backed out of her parking space striking MV#1. The witnesses took a digital photograph of MV#2's registration (MA REG 763YC9). They described the operator of MV#2 as an Asian female in her 40's, 5'1" in height, operating a black colored "Buick" S.U.V type vehicle. MV#1 sustained minor damages to its front passenger door and bumper. A query of MA REG 763YC9 showed that it was registered to an Asian female to a black colored "Buick Rendezvous". The owner of MV#1 stated the owner of MV#2 also has a business in the same building at 630 Commonwealth Ave (Move2Boston Group). I spoke to the registered owner of MV#2 via telephone and she stated she was parked in the rear parking lot on Rice St but denied making contact with any vehicles. I could not observe MV#2 as it was not at the scene. At 14:40 I returned to 1066 Centre St and was able to observe MV#2. MV#2 displayed minor scrapes to its front driver's side bumper which was consistent relative to where both vehicles were parked. Based on the witnesses statements I will document this accident and provide this report number to the vehicle owners.

On 03/12/2019, I responded to the area of 279 Chestnut St for two car motor vehicle accident. On arrival, I spoke with the operator of MV1 (MA Reg: 824WA6) who stated that he was driving northbound on Chestnut St when he was rear ended by MV2. I spoke with the operator of MV2 (MA Reg: 4DN621) who stated that she was travelling northbound on Chestnut St behind MV1. MV2 stated that they were in stop and go traffic and misjudged the distance and rear ended MV1. MV1 sustained moderate damage to its rear end. MV2 sustained moderate damage to the front end and had airbags deployed. All parties were evaluated by medics and signed patient refusals. MV2 was towed from the scene by Tody's Towing.

On 03/19/19 at 14:57 hours, I responded to 220 Dedham Street for a past motor vehicle hit and run accident involving a parked car that occurred at 12:26 hours on this date. Dedham Street is a public way maintained by the City of Newton. The owner of M/V # 1 states her vehicle was legally parked in front of 220 Dedham Street facing northbound when she came out and found a note from a witness saying it was struck by a gray Honda Accord bearing MA Reg. # 8512HV. It appears that the operator of M/V # 2 side swiped M/V # 1 as it was passing by on Dedham Street. I observed marks to the driver's side rear door area and the side view mirror was broken. On 03/20/19 at 08:47 hours, I spoke to the witness, Kawtar Bhih, on the phone and she stated that she was parked on the opposite side of where the accident took place. Bhih observed a gray Honda Accord with an elderly female operating M/V # 2 on Dedham Street almost side swiped another parked car before side swiping M/V # 1. She says that the vehicle slowed down after and then continued to leave the scene of the accident. She then took a photo of the above listed registration plate. I attempted to contact the owner of M/V # 2 (Jali Medical Inc Waltham, MA) and the receptionist was not able to provide any information at the time. I was told to call back the next day. I gave the vehicle and registration info to the person inquiring if they could find a possible driver and call back. While working on this report, I observed that the owner of M/V # 2 was involved in a past accident in our system and the operator at the time had an address of 66 Country Club Rd. Newton. On 03/20/19 at approx. 09:30 hours, I went over to this address and observed M/V # 2 in the driveway with an operator inside. After inquiring about the accident, she stated she was driving down Dedham Street when she heard a noise coming from her passenger side view mirror as she had struck something. She then left the scene not thinking it was serious. I observed fresh damage to her passenger side mirror along with other marks along the doors which appeared to be older. Operator was issued in hand MA Uniform Citation # T1441710 for Ch. 90/S24 Leaving the scene of property damage.

On 03/20/19 at 09:43 hours, I responded to the Avalon Apartments at Newton Highlands located at 99 Needham Street for a past hit and run that occurred at the east end garage exit on 03/19/19 at 14:10 hours. I spoke to Senior Community Manager Karen Schueler who showed me video surveillance footage from yesterday's accident. In the video, you can see a blue Subaru Forester attempting to exit the east end of the garage to the property. The operator of the Subaru is identified by Ms. Schueler as a resident of the building as Gertrude Weinstein. After Weinstein scans her garage pass to exit, she then over steers towards the right and strikes the overhead garage door opening device. She then continues to exit the garage scraping the entire passenger side of her car along the device that is attached to the wall. I viewed the garage door and there was significant damage as it became detached from the wall. The door had to be placed out of service. After I spoke to Ms. Weinstein who admitted to striking the side of the garage door but only thought there was damage to her vehicle. No citation issued as the underground garage to the building is private property and not a public way.

On Monday March 25th, 2019 I responded to a hit and run motor vehicle accident at 115 Florence St near Louise Rd. Vehicle # 1 (Mass Reg #5TFK30 Honda Pilot operated by Sabryna Reyes) was traveling east on Florence St headed toward Route 9. According to Sabryna, Vehicle # 2 was taking a left hand turn onto Florence St from Louise Rd. Vehicle # 2 made contact with the left side of the vehicle of vehicle # 1. Vehicle # 2 drove away without staying on scene. Sabryna was unable to get a license plate of the vehicle driving away. There were no witnesses to the accident and no video cameras in the area. Sabryna described vehicle # 2 as a black honda CRV with white male passenger operator approximately in his 30s. Their were no injuries and Sabryna signed a patient refusal. Newton Fire responded because of the airbag deployment. Vehicle # 1 suffered minor damage to the left front side of the vehicle. Vehicle # 1 was towed by AAA Towing.

On Tuesday May 7th, 2019 at approximately 13:15 hours I responded to the area of 30 Langley Road for an auto vs. pedestrian accident. At the time of the accident the weather was sunny and the road conditions were dry. Langley Road is a public way maintained by the City of Newton. Upon arrival I first spoke to the pedestrian who was inside the ambulance getting medical attention. The pedestrian identified as Donna Kalikow (S24151638) stated she was standing in an open parking space for a few seconds, attempting to walk across the street to enter J.P. Licks. Donna stated she was standing in the marked parking space waiting for traffic to stop to safely cross the street. Donna states the cars stopped to let her cross and she proceeded into the roadway. She states MV1 was attempting to back into the parking space that she was standing in and struck her right side and knocked her to the ground. Donna states the operator of MV1 stopped the vehicle when he realized he had struck her and got out to contact 911. Donna appeared to have minor injuries and complained of head pain. Medics transported Donna to NWH to be further evaluated. The operator of MV1 identified as Christopher Moos (S24090966) stated he was attempting to parallel park into an empty parking space and never saw the pedestrian standing in the space. Christopher stated as he was backing up into the space he was notified by a bystander that he just struck a pedestrian. Christopher stated he immediately stopped the vehicle and got out to assist the pedestrian and contact 911. Christopher stated he was not travelling no more than 5mph at the time of the accident. MV1 did not have any damage to the vehicle and was able to leave the scene. Christopher reported no injuries. Several photographs were taken of the scene and submitted to the IT Bureau. I mailed the operator a Massachussetts Uniform Citation for a City of Newton Ordinance violation for Section 19-75 Failure to use care when backing.

Tipruthai Thangthong stated that on 05/21/2019 at approximately 16:36 hours she was pulling into the driveway of her sisters home at 53 Rangeley Rd while operating her vehicle MA reg 2LVL61 a 2018 Honda Accord. Thangthong stated that her three old son Mighty Mettanant was a passenger in her vehicle. Thangthong stated that she was driving approximately 5 MPH as she drove into the driveway. The driveway runs east to west and Thangthong's vehicle was heading west into the driveway as the sun was in the Western sky creating a glare. Thangthong stated that she did no see anything in the driveway prior to her pulling into it. Thangthong stated that she felt the front of her vehicle strike what she described as " something " and stopped her vehicle. Thangthong stated that she put her vehicle in reverse and then into park and exited her vehicle to see what she had struck. Thangthong stated that she saw her two nieces, Kalla Mongkhonlertsirkul age three and Kelley Mongkhonlertsirkul age seven laying in the driveway in front of her vehicle. Thangthong stated that she then realized that her vehicle had stuck her two nieces as they were in the driveway. Both of the Mongkhonlertsirkul sisters were conscious and alert and were able to walk to an ambulance for transport to Newton Wellesley Hospital for treatment. Thangthong stated that she and her son were not injured in the crash. I observed no damage to Thangthong's vehicle. I responded to the Newton Wellesley Hospital and spoke with the Mongkhonlertsirkul sisters and their mother Piyada Kellum. Kellum stated that her oldest daughter Kelly is a student at the Franklin Elementary School . Kellum stated that her daughters told her that they were playing in their driveway when their aunt's vehicle entered the driveway and did not stop. I spoke with both of the Mongkhonlertsirkul sisters who told me that they tried to get out of the way of their aunt's vehicle but could not in time. The Mongkhonlertsirkul sisters stated that they were both struck in the back and fell to the ground between the two front tires of their aunt's vehicle. Kalla Mongkhonlertsirkul had scrapes to her right knuckles. Kelly Mongkhonlertsirkul had lacerations to her lower left arm and the left side of her face near her eye. I spoke with hospital staff who stated that there did not appear to be any broken bones or other internal injures. I took photos of the crash area and forwarded the disk to the NPD's IT bureau for downloading.

The operator of vehicle 1 stated he was traveling North on Parker Street when he began to brake and slow down due to traffic stopped ahead of him. His brakes started pulsing and he lost control of the vehicle and could not stop. This caused him to crash into vehicle 2 which was stopped in traffic and then caused vehicle 2 to crash into the rear of vehicle 3. This caused moderate damage to vehicle 1 which had to be towed. It caused major damage to vehicle 2 however due to the location of the damage it was able to be driven away, and minor damage to the rear of vehicle 3 which was also able to be driven away. The operator of vehicle 2 stated she was sitting in traffic on Parker Street when vehicle 1 rear ended her, causing her to then rear end vehicle 3. The operator of vehicle 3 stated he was sitting in traffic on Parker Street when vehicle 2 rear ended him. The operator of vehicle 3 stated he had minor neck pain and was evaluated by Cataldo medics and signed a patient refusal.

On Wednesday, June 5th 2019, at approximately 12:12am, I, Officer Brooks, responded to 163 Lexington street for a report of a 2 car MVA. Upon my arrival I observed that MV1(MA REG 574PF5) had struck MV2(FL REG CWWE22) which was parked legally in front of 163 Lexington street. I spoke with the operator of MV1 who stated he was traveling southbound on Lexington street, and had reached over to his passenger side to grab a Burger King bag. When he reached for the bag he drifted to the right and struck MV2. MV1 had heavy damage to the passenger side and airbag deployment. The operator was checked out by Cataldo and signed a patient refusal. MV1 was towed by Tody's. The operator was issued a citation for an expired registration. I spoke with the owner of MV2 who was inside of 163 Lexington street at the time of the accident. His vehicle had been legally parked when struck. MV2 sustained major damage to the driver's side. MV2 was towed by Tody's. Both operators were provided the report number.

Operator of vehicle one, Benjamin Beaver stated that while driving south bound near 321 Walnut St his vehicle ( MA reg 551DK8 2014 Lexus ) made contact with a pedestrian in a marked crosswalk. Walnut St is a public way in the city of Newton. Beaver stated that he did not see the pedestrian prior to the crash and that his vehicle was travailing approximately 5 to 10 MPH. Beaver stated that the pedestrian was knocked to the ground but got up and walked around. The pedestrian, NNHS student Benjamin Donovan stated that he was crossing east to west in the marked crosswalk in front of 321 Walnut St. Donovan stated that he saw Beaver's vehicle approaching him at approximately 5 to 10 MPH. Believing that Beaver was stopping to let him cross , Donovan stated that he held up his left hand to signal to Beaver that he was going to cross. Donovan stated that Beaver's vehicle struck him on his left side and knocked him to the ground. Donovan stated that he had minor scrapes on his right side as a result of falling to the ground. Donovan stated that he did not lose consciousness and was transported via EMS to Newton Wellesley Hospital for treatment. I took photos of Beaver's vehicle and forward the disk to NPD IT Bureau. I issued Beaver MA Citation T 1269995 and cited him for a violation of MGL Ch 89 C 11, failure to yield to a pedestrian in a crosswalk. On 06/12/2019 I was forwarded an E-mail from Capt Anastasia of a video of the crash that was taken from a camera that was mounted on the roof of 321 Walnut St. The video shows Donovan walking into the marked crosswalk from the east side of Walnut St and hold up his left had to Beaver's on oncoming vehicle. Beaver vehicle appears to be travailing faster than the 5 to 10 MPH that Beaver believed that he was travailing. The video shows that Beaver did not brake until Donovan was struck and landed on his vehicles hood. Donovan was thrown approximately 10 feet from Beaver's vehicle and landed on his right side on the roadway. The video shows Beaver stopping and pulling to the side of the roadway after the crash.

On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

On 07/29/19 at 11:13 hours, I responded to the Dunkin Donuts parking lot located at 138 Needham Street for a three car motor vehicle accident. Operator of MV1 stated at first she could not recall what occurred during the accident due to lightheadedness at the time. After, she then stated after pulling in to the Dunkin Donuts lot she was attempting to back in to a parking spot in the back of the lot. While backing up, she attempted to hit the brake pedal but must have pushed the gas pedal instead. The vehicle then struck parked/unoccupied MV2 and MV2 was then pushed in to parked/unoccupied MV3. The operator was checked out by Cataldo Ambulance and signed a patient refusal. MV1 sustained moderate driver's side rear bumper area damage but it was still driveable. MV2 sustained damage along both sides of the vehicle and a rear broken axle and it was towed by Tody's. MV3 sustained scrapes and dents to the entire driver's side but the vehicle was driveable.

On 08/02/19 I responded to single vehicle crash across the street from #33 Dedham St. The operator of veh #1 stated he was driving due North on Dedham St. when his car veered into the city tree. There was airbag deployment and heavy damage to veh #1. Operator of veh #1 had some cuts and scrapes to legs but was checked out by the Medics and signed a patient refusal. The vehicle was towed by Tody's towing and the operator was issued Mass Citation #T1445105 in hand for CH89/ Sec4a Marked Lanes violation (traveling outside of breakdown lane) and Ch 90/ Sec 18 Speeding (based upon the significant amount of damage to the vehicle and how far the debris was found from the crash site) . On 08/06/19 at 12:34 hours, I responded to Industrial Place by The Lakeshore Learning Store at the request of Newton Police Officer Justin March who witnessed an accident in the parking lot of 300 Needham Street. Officer March was working a road construction detail at the time in the City of Newton. He states that at approximately 12:20 hours he observed MV1 exiting the parking lot of 300 Needham Street( a drive way to a building that the public has access to). There is an island in the middle that separates the entrance and exit of the lot as it goes out to Needham Street. As MV1 was exiting, he drove up on the island and pushed two large stone builders out of place. There was also damage to numerous different flowers planted in the ground. MV1 then exited the lot and went eastbound on Needham Street. Officer March then followed in his vehicle and kept beeping his horn to alert the driver to stop. The operator turned on to Industrial Place and pulled over. Ofc. March attempted to speak to the operator, Gelvin Perez-Ramirez but there was a language barrier. Next, Ofc. Calderon responded and assisted with Spanish translation. He asked the operator what occurred and he stated that he never saw the island. Due to the fact that the operator left the scene of the accident and never reported it, he was issued MA Uniform Citation # T1445351 for Ch. 90/24/C. MV1 sustained damage to the two exhaust pipes on the passenger side of the truck. There is also possible undercarriage damage. Ofc. M. Wade took photos of the scene.

On 08/13/19 at 14:53 hours, I responded to a hit and run motor vehicle accident located at 244 Needham Street. The operator of MV1 states she was parked in a parking spot to the right of the Splash business at 244 Needham Street. The parking spot is located on Charlemont Street. After parking, she was sitting in her motor vehicle when she heard a bump to her driver's side rear end from MV2 backing up from a parking spot across from her. She says the operator of MV2 turned and looked at her and kept proceeding (NB) on Charlemont towards Needham Street. She then exited her vehicle to try and get the operator's attention to no avail. She took a photo of MV2 which was a black Toyota Highlander bearing MA reg.# R85045. She described the operator as a white female in her forties with possibly dark curly hair. I observed minor paint damage and scratches to the driver's side rear quarter panel area. The operator is unsure if that damage is from the accident. The owner of MV2, Steven Masterson has a residential address of 40 Wild Acres Rd in North Attleboro, MA and a mailing address of 34 Central Street Foxborough, MA. A telephone search revealed a phone number of (269)857-7814 for him out of Aiken, SC and the Foxborough address but the number is not in service. UPDATE: I was provided a phone number for Masterson from N. Attleboro PD. On 08/16/19 at 12:45 hours, I spoke to Sheila Masterson who stated she was driving MV2 at the time of the reported accident. She stated that she was unaware that she struck MV1 and saw the other operator looking at her but thought nothing of it. Due to the accident being very minor or possibly no damage at all, no further action will be taken. Masterson was provided with the other operator's information.

MV1 stated he was travelling westbound on Needham St and was attempting to turn left into the Verizon store when MV2 who was behind him passed him subsequently side swiping him. MV1 further stated that the operator "made eye contact with me after the crash but kept going. MV1 took a photo of MV2's trailer (MA REG C98747). I left a VM with the owner of MV2 requesting a call back. MV1 sustained minor passenger side damage. It is unknown what damage was caused to MV2 and whether it would be to the truck or the trailer. Traffic Bureau update (Officer Gaudet): On September 4, 2019, I made contact with the owner of LPS Enterprises, Tony Lisa. Lisa confirmed MA TRN: C98747 is registered to his company. Lisa stated he will look into who was using the trailer on 8/30/19 and will contact me with their information. I spoke with the operator of MV2, John Subilosky (S89884850). Subilosky stated he was operating a 2011 GMC Sierra (MA CON: T58930) with MA TRL:C98747 attached to it on the afternoon of the accident. Subilosky stated he was not aware he was involved in an accident and stated he would have stopped had he known. Subilosky stated there was a bobcat in the trailer at the time of the crash. Subilosky stated there is no damage to the trailer. Due to the size and weight of the trailer attached to MV2, it is reasonable to believe if MV2 made minor contact with MV1, the operator of MV2 would not have been aware. Subilosky has been added to this report as the operator of MV2.

Owner of Motor Vehicle #1 stated that he was exiting the gym at Planet Fitness and he noticed that someone was leaving a piece of paper on his front windshield. He was approached by the man who told him that he witnessed a Gray Dodge Ram bearing MA Reg 9JGD90 strike his Jeep Grand Cherokee that was parked and unoccupied causing moderate rear drivers side damage. MA Reg 9JGD90 came back to a 2018 Dodge Ram 1500 Gray in Color and registered to a Kenneth Doucette out of 41 Thaxter Street, Newton MA. I was able to locate several telephone numbers from our in house system however those numbers were no longer in service. I made several attempts to make contact with Mr. Doucette at his residents and had negative results. N491 Officer D. Healy also made an attempt to make contact with Kenneth Doucette later on in the evening and had negative results. This will be further investigated on my next tour of Duty. **UPDATE Wednesday 09/04/19** At approximately 10:00 a.m. I went to 41 Thaxter Street to once again attempt to make contact with Kenneth Doucette and had negative results. I left another voicemail for the witness Claudio Silva and had negative results. I then spoke to Traffic Officer M. Gaudet to inform him about this incident and he stated that he will follow up with this investigation. Traffic Bureau update (Officer Gaudet): On September 4, 2019, I attempted to make contact with the witness to the crash, Claudio Silva, with a negative result. I made contact with the registered owner of MV2, Kenneth Doucette (S80920300). Doucette stated he was operating MA: 9JGD90 at the time of the accident. Doucette stated he pulled into a parking spot next to MV1 and stated he didn't think he "hit it hard". Doucette stated a short time later he left the area and when he got back to work he observed damage to his own vehicle. Doucette stated he had damage to the front passenger bumper area. Doucette stated he returned to the crash scene to see if the other vehicle had damage but it was gone. Doucette was advised and provided Massachusetts Uniform Citation T2016422 in hand for Chapter 90, Section 24 (Leaving the Scene of Property Damage, MV).

On 09/04/19 at 08:22 hours, I responded to 1052 Chestnut Street for a three car motor vehicle accident with minor injuries. All four parties involved were shaken up as a result of the accident but signed patient refusals with Cataldo Ambulance One. Operator of MV1 states she was traveling straight ahead (SB) by 1052 Chestnut Street when she observed several vehicles ahead start to proceed through the light. She then heard her phone ring and went to retrieve it by looking down for a second. When looking back up, the traffic directly in front of her did not move yet and she could not stop in time striking MV2 in the rear end. MV2 was then pushed in to MV3. Both MV2 and MV3 were stopped in traffic at the time awaiting to travel through Chestnut Street at Elliot Street. MV1 sustained moderate front end damage to the bumper and grille areas. MV2 sustained heavy rear end bumper and hatch area damage. MV3 sustained minor scratches to the rear bumper. All vehicles were still driveable.

On 9/08/19 Kevin Coston came into NPD Headquarters to report a MV/Ped with Injury that had occurred on 8/23/19 at about 06:15 in the parking lot of Whole Foods (916 Walnut Street). The RP stated that he had left work (Whole Foods) at the back receiving door and was walking to his car, he bent over to take his knee pads off, while he was doing this he was struck by a delivery van that backed into him. The driver took out his headphones and said sorry before driving away. RP stated that he told the driver that he had been hit but the driver just drove away. RP went to hospital (Faulkner) on his own for lower back pain and has not been to work since. RP was able to take picture of the side of the delivery van. The van belonged to Shortstop Distribution Inc (508-655-5948). RP left a message that morning. He was texted by Gedeon (786-352-6695) who stated there was a language barrier and he didn't understand what had happened. RP advised Gedeon to talk to his lawyer or the police. Exact location of accident unknown. Diagram is a estimate. Report printed out and placed in traffic box for follow up. Traffic Bureau Update (Officer Gaudet): On Monday, September 9, 2019 I attempted to make contact with the operator if MV1, Gedeon Duarte, with a negative result. A Hit and Run motor vehicle inquiry was mailed to Duarte. I spoke with the injured party, Kevin Coston, via telephone on September 9, 2019. Coston stated he is an employee at the Whole Foods located at 916 Walnut Street, Newton. Coston stated on August 23, 2019 a black van backed into him as he was kneeling on the ground taking off his knee pads in the loading area of the store. Coston stated he approached the driver who remained in the vehicle to speak with him after he was hit. Coston described the driver as a heavy set Spanish male. Coston stated the operator of the vehicle rolled down his window, took his headphones out of his ear, said sorry, and left the area. Coston stated he contacted the Newton Police to report the accident but was told he needed to come in person to file the report. Coston stated he was unable to come to the station to file the report until September 8, 2019. On Friday, September 13, 2019, I spoke with Gedeon Duarte at the Newton Police Department. Duarte stated he spoke Portuguese and did not speak English. With the assistance of David Sargent from the Language Line (1-800-225-5254) translating, I was to speak with Mr. Duarte in the Traffic Bureau. Duarte stated he was making a delivery at Whole Foods and was waiting for another truck to leave so he could leave the area. Duarte stated a male party (Coston) came up to his driver door and was yelling him. Duarte stated that due to language barrier he was unable to understand what the person was saying to him, but stated the male party was yelling, "Fuck you! Fuck you!" Duarte stated he did not know what was going on and stated he left the area to allow the male party to calm down. Duarte stated he returned a minute later the male party did not want to speak with him. Duarte stated he returned a second time later in the morning and Coston was not there. Duarte stated he received a call from his employer stating Kevin Coston had called for him and he was provided with Coston's phone number. Duarte stated he texted Coston to find out what happened that morning. Duarte showed me a text message exchange from August 23, 2019 at 9:03 A.M. between himself and a party listed as "Kevin Whole". In the text messages, Duarte stated he is the driver of the van and that his supervisor called him. The other party responded and stated "Talk to the police and my lawyer". Duarte stated that he did not understand what happened and that he returned to whole foods. Duarte ended the text exchange with "927535425 my number of policy..." Duarte stated he had no idea he made contact with Coston and stated at the time of the alleged accident he did not hear/see/feel anything otherwise he would have stopped. Duarte's information has been updated in this report to reflect him as the operator/vehicle involved. Duarte also submitted pictures of the rear of his vehicle, a 2008 black Ford E350 (MA: 8NY171), from the day of the accident. Photos were submitted to the IT Bureau. Officer Wade went to the Whole Foods located at 916 Walnut Street, Newton in an attempt to view surveillance footage of the accident. Officer Wade stated the surveillance camera did not capture the incident and the van not viewed at all.

On Tuesday, September 10, 2019 while assigned to Traffic unit N525, I responded to the area of Beacon Street and Roslyn Road, Newton for a report of a motor vehicle/bicycle crash. The weather at the time of the crash was overcast. The road surface was dry. Beacon Street and Roslyn Road are both public ways maintained by the City of Newton. I spoke with the bicyclist involved in the crash, Steven Greenzang (S96615185). Greenzang stated he was traveling Westbound on Beacon Street in the shoulder lane on his black Specialized bicycle. Greenzang had a yellow reflective vest on and was wearing a helmet at this time. Greenzang stated a vehicle in the travel lane to his left drove by him and hit the mirror located on his left handlebar. I observed damage to the mirror. Greenzang report no injuries. I spoke with the operator of MV1, Sarah Bonfanti (S66811372). Bonfanti stated she was parked in front of 1411 Beacon Street waiting to pick up a co-worker that resides at that address. Bonfanti stated a bicycle traveling on the sidewalk to her right crashed into the passenger side door mirror of her 2008 Toyota Scion (MA: 681YH1). Bonfanti stated the bicyclist was acting "crazy" during her interaction with him and he was trying to take her picture. Bonfanti stated she got back into her vehicle and moved up the roadway to wait for police to arrive on scene. I canvassed the area where the accident occurred with Sgt. Dempsey. There was glass from the passenger side mirror of MV1 and Greenzang's bicycle in the main travel lane of the roadway and the shoulder lane where Greenzang stated he was traveling. We also observed a large tree and a telephone pole on the sidewalk next to the roadway where the accident is alleged to have occurred. After speaking with all parties involved, observing the location of the glass in the roadway from the crash, and the location of the tree and telephone pole on the sidewalk where he accident occurred; Greenzang would not have been able to operate his bicycle on the sidewalk and be close enough to the roadway to make contact with MV1. Prior to clearing the accident, I asked Bonfanti if she had any questions and if she wanted to report any injuries. Bonfanti stated she was hit by the bicyclist during her exchange with him and began grabbing her left leg. I asked Bonfanti why she did not inform the officers on scene that she was assaulted. Bonfanti stated she said something to our Dispatch when she called. I asked Bonfanti if her leg was injured. Bonfanti stated I was putting words in her mouth and that she was just scratching her leg. Bonfanti then stated she was hit in the right shoulder. I asked Bonfanti if she would like an ambulance to respond and evaluate her. Bonfanti declined and stated she was more concerned about her mirror. I advised Bonfanti if she wanted to pursue charges against Greenzang she could do so in Newton District Court. I listened to the 9-1-1 recording Bonfanti made in regards to this incident. Bonfanti stated a pedestrian busted out her window. Bonfanti then stated to our dispatcher that the bicyclist lost control of his bicycle and smashed into her car. During her exchange with the our dispatcher, I did not hear Bonfanti say she was physically assaulted by Greenzang.

Operator of MV1 stated he was driving southbound on Lake Ave and looked down for a brief moment and did not see the bend in the road and struck the stone wall in front of 219 Lake Ave. The passenger of MV1 stated the operator was traveling southbound and did not see the turn and went up on the side walk and collided with the stone wall. The operator of MV1 was evaluated by Medic 1, the passenger reports no injuries. Cataldo received two parental refusals for both parties. A witness (Baptiste, Franck Jean) stated he witnessed the accident and believed the vehicle was travelling at approximately 40mph and lost control around the bend and struck the wall. David Donahue from Tody's Towing towed the vehicle. The homeowner at 219 Lake Ave was home and notified about the damage to their property. Six digital photos of the accident scene were taken and submitted to IT. Update 9/28/19: Upon further review and discussion with Sgt. Dempsey, Operator of MV1- Gallivan, Chase was issued citation T1445308- 89/4A- Marked Lane Violation and 90/8-JOL Passenger Restriction. Citation was issued in hand.

On Tuesday, October 1st 2019, at approximately 3:45pm, I, Officer Brooks, responded to Valentine street by Lenox street for a motor vehicle roll over accident with entrapment. Upon my arrival I observed MV1(MA REG SN227) laying on its passenger side with heavy damage to the entire vehicle, and all air bags deployed. The operator was trapped in the driver's seat but was concious and alert. The fire department arrived and extracted the operator, who signed a patient refusal with Cataldo medics. The operator stated she had been traveling southbound on Valentine street, looked down for a moment and struck a parked car with the passenger side of her vehicle. Her vehicle then spun out and rolled onto its side. I spoke with the owner of MV2(MA REG 9WG975) who stated he was alerted by the sound of the crash. His vehicle was legally parked on Valentine street. MV2 sustained heavy damage to the driver's side rear end. Both vehicles were towed by Tody's and inventory forms were filled out and filed.

The Operator of vehicle 1 stated he was driving North on Columbus Street, he misjudged how close a parked Vehicle 2 was to him as he approached and ended up sideswiping it. Katerina Iliades stated that on 10/23/2019 at 11:56 hours she was standing in front of the rear driver's side door of her vehicle MA reg 7BM656 2019 Land Rover while it was parked facing north in front of 316 Walnut St. Walnut St is a public way in the city of Newton. Iliades stated that she was placing her daughter Melina Bichelen in her car seat and when she observed MA reg 8EP212 a white Subaru Outback driving north bound on Walnut St near the marked crosswalk at 312 Walnut St. Iliades stated that she observed the Subaru was being operated by an elderly white male. Iliades stated that the she observed the Subaru's front drivers side strike several orange plastic three foot poles that are affixed to the roadway around the crosswalk. Iliades stated that after the Subaru struck the poles at the crosswalk it veered towards her. Iliades stated that she leaned back into the back seat of her vehicle and pulled the door towards herself as the Subaru's passenger side mirror made conact with her vehicle. Iliades stated that the Subaru continued north bound on Walnut St without stopping or slowing. Iliades complained of lower leg pain and refused medical aid or transport to a hospital at the crash scene. I observed no damage or markings on Iliades vehicle. MA reg 8EP212 is registered to Harilaos Papadogiannis ( DOB 01/02/1941 ) of 134 Apt 4 North St , Newton. I went to 134 North St and found MA reg 8EP212 a 2014 Subaru parked in the lot of 134 North St. I observed that the Subaru's passenger side mirror was lose from it's mounting and was turned down and was facing the wrong way. I further observed that the out side casing of the Subaru's mirror had scuff marks consistent with it forcefully making contact with an other object. I spoke with Papadogiannis who stated that he was driving his Subaru north bound on Walnut St at 11:56 hours on 10/23/2019. Papadogiannis stated that he could only recall striking the plastic poles near the crosswalk and nothing else regarding the crash. I as spoke with Papadogiannis he was confused and I had to repeat myself and my questions to him several times. I instructed Papadogiannis what steps he needs to take regarding filing out an operator crash report and contacting his insurance provider , but I believe that he did not fully comprehend what I was conveying to him. Based upon my observations at the crash seen and my conversations with Iliades and Papadogiannis , I believe that Papadogiannis was at fault for the crash. Based upon my observations of Papadogiannis I filed an Immediate Threat Request form and faxed it to the MA RMV Driver Control Unit. Photos of both vehicles were taken and downloaded by the NPD's IT Bureau.

Upon arrival I spoke with the operator of vehicle 1. He stated as he was heading towards the exit, vehicle 2 suddenly stopped in front of him. Vehicle 2 then backed into him and started moving forward again. The operator of vehicle 1 then exited the car to stop vehicle 2. The Operator of vehicle 2 then got out of her vehicle. The operator of motor vehicle 2 stated she was driving and she stopped to back up, as she backed up she thought she hit a curb but then the operator of vehicle 1 was at her window saying she hit him. As a result of the crash vehicle one sustained very minor front end damage and vehicle 2 had no noticable damage.

I was dispatched to 854 Beacon St, Newton, MA for an overturned motor vehicle accident. I arrived on scene and saw Vehicle #1 (MA reg 9AS213) overturned on its driver's side with the female operator/registered owner trapped inside. Newton Fire arrived on scene and mechanically removed the operator out of the motor vehicle. Prior to where Vehicle #1 was overturned I observed Vehicle #2 (MA reg 615FF6) with rear bumper damage and a snapped rear axle. Vehicle #2 was unoccupied. Operator of Vehicle #1 was transported to Newton-Wellesley Hospital for treatment by Newton Medics. Both motor vehicles were towed by Tody's and inventory sheets were completed and filed. I spoke with the Operator of Vehicle #1 at Newton-Wellesley Hospital. Operator stated that she does not remember what happened before the crash and that she only remembers waking up in her car. I told her that her car was towed and a report is on file. Ofc Selig spoke with the registered owner of Vehicle #2 and notified her of the incident. Based on my investigation it appears that Vehicle #1 struck the rear of Vehicle #2 and the operator over steered causing the vehicle to overturn onto the driver's side.

On 11/12/2019 at 12:26 hours, I responded to the Star Market parking lot located at 2040 Commonwealth Avenue for a motor vehicle pedestrian accident. The parking lot is a public/thru way that motorists use to access the Star Market along with other businesses. The weather at the time was cloudy and rainy and the parking lot was saturated. Prior to my arrival, the pedestrian involved left the scene and did not want to speak to the officer nor wanted medical attention. Ofc. Claflin then cancelled Cataldo Ambulance. The pedestrian, Mary Alexander, left her information with a store employee prior to leaving. I spoke to the operator of MV # 1 (Joshua Randel MD Lic.# R534440603244) who stated that he was travelling through the third row of the Star Market parking lot at an appropriate speed. He was then attempting to make a left turn near the entrance/exit of the store. He says that there was a large white van parked in the end handicapped spot towards the left of him. He says the van was sticking out in the spot limiting his visibility. As he was half way through his left turn and coming around the corner, he was not able to see a pedestrian pushing a shopping carriage. He then struck the carriage that was being pushed by Mary Alexander. He further says that the cart jolted Ms. Alexander and she fell down to the ground. At the time, Alexander was walking through the lot near the store entrance heading back towards the parked cars. She was not in the crosswalk at the time which was approximately ten feet down from her location. After, I attempted to go to Ms. Alexander's residence to follow up. I spoke to her friend who was bringing up Alexander's groceries and she stated that she decided to go to Newton Wellesley Hospital for treatment. I then spoke to Ms. Alexander in the lobby of the hospital. She stated that she was exiting the store through the lot with her shopping carriage. She believes she was approximately eight feet away from the door when all of a sudden she only saw red coming at her. She believes MV # 1 was speeding at the time. I asked her if she was struck by the vehicle or was the shopping carriage only struck. She replied that she did not know and only remembers falling to the ground after. She sustained a large bruise to the inside palm area of her right hand and her left knee was swollen. Digital photos of the scene were taken and pictures of Ms. Alexander's injuries were taken at the hospital. As far as possible video surveillance, I was told that the Loss Prevention Department has to access the video. On 11/13/19 at approximately 14:20 hours, I went in to the Star Market and spoke to the manager, Nidal Rajeh, inquiring about possible video surveillance. He said there is coverage in the parking lot and he was able to show me the video. A copy of the video would have to be done by Loss Prevention which I requested. I observed at the 12:12pm mark in the video, MV # 1 was parked five spots up from the third row of the lot that he was exiting. I also observed a white transport van parked in the end handicapped spot mentioned by the operator but it was parked correctly and not limiting visibility. Next, MV # 1 backs out of the spot and drives forward going at a reasonable speed. The pedestrian (Alexander) has already exited the store with her carriage and is walking at an angle toward the same third row of parked cars. As the pedestrian is walking towards the direction of MV # 1, it appears that the pedestrian should be visible to him. MV # 1 then starts to take the left turn around the corner without slowing or stopping and strikes the carriage head on. Due to the impact, Alexander then loses grasp of the carriage and it goes past her twisting her body around. She then hits and rolls up on the hood of the vehicle on her left side. MV # 1 then suddenly stops and immediately gets out and provides assistance to the pedestrian. Due to the video surveillance, Mr. Randel will be issued/mailed MA Uniform Citation # T2012347 for Ch. 90/14/A Failure to slow for a pedestrian.

On Thursday, November 14, 2019, while assigned to Traffic unit N525, I responded to the rear parking lot of the Angier Elementary School located at 1697 Beacon Street for a motor vehicle crash involved a City of Newton owner vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. The operator of MV1, Jessica Moore, stated she was backing out of a parking spot in the rear of the Angier Elementary School while operating a 2007 Chrysler Town and Country (MA:395KR9). Moore stated she did not see MV2 passing by as she was backing out and the rear driver side bumper area of her vehicle crashed into the rear driver side of MV2. Moore reported no injuries. I observed moderate damage to the rear driver side bumper area of MV1. The operator of MV2, Patrice Lys, stated he was traveling Westbound in the parking lot of the Angier Elementary school in a 2018 Ford EcoVan (MA MVN: M2546A). Lys stated a vehicle on his driver side backed out of a parking spot and the rear end of MV1 crashed into the rear driver side of MV2. Lys reported no injuries. I observed minor damage to the rear driver side area of MV2. The operator of MV1 was cited with Massachusetts Uniform Citation T2016428 for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care, Backing). Photos were submitted to the IT Bureau.

On 11/30/2019, while assigned to N494, I, Officer Conary, and other units, responded to Charlesbank Road at Nonantum Road for a single motor vehicle crash into a tree. Upon arrival, I observed a white mini van MA registration 5DV184 with heavy front end damage into a tree on Charlesbank Road, a public way in Newton. The operator of the motor vehicle, CORDERO, Ericka, was sitting in the back seat of the vehicle and was talking on the phone. CORDERO's primary language was Spanish. I asked CORDERO if she was okay and she said yes and I asked CORDERO if she was driving the car and she said yes. CORDERO also told me that she did not have a license. MA State Police were on scene and were able to asist with translating. Medics arrived and CORDERO was transported to Saint Elizabeth's Hospital. CORDERO's son and husband arrived on scene and I was able to confirm her information. Both stated that CORDERO did not have an active license. Newton Dispatch ran all information for CORDERO and confirmed she was unlicensed. CORDERO will be charged with MGL Ch.90/10 Unlicensed Operation of a Motor Vehicle and 89/4A Marked Lanes Violation, Uniform MA Citation T2079476. Due to CORDERO being transported to the hospital, the citation will be mailed to her active address. Pictures of the incident were taken and uploaded to IT accordingly. No further incident to report.

On Tuesday, December 3, 2019, while assigned to Traffic unit N525, I responded to 364 Woodward Street, Newton for a crash involving a City of Newton owned vehicle. The weather at the time of the accident was light snow. The road surface was wet. Woodward Street is a public way maintained by the City of Newton. I spoke with the operator of MV1, Brad Butera (S94383997). Butera stated he was operating a City of Newton owned 2011 Volvo Front Loader (MA MVN: M88537) on Woodward Street while clearing the roadways during a declared snow emergency. Butera stated he was backing up Southbound across Woodward Street in the area of 364 Woodward Street after clearing some snow. Butera stated he looked and did not see any vehicles behind him. Butera stated he was backing the Front Loader up and the rear back of his vehicle crashed into the front hood area of MV2. I did not observe any obvious damage to the bucket area. Butera reported no injuries. Butera stated the operator of MV2 stated "My fault" to him during their exchange after the crash. Butera is an employee of the City of Newton DPW. I spoke with the operator of MV2, Marco Caceres (S67223904). Caceres stated he was operating his 2006 Mercedes E350 (MA: 26P270) Westbound on Woodward Street. Caceres stated he saw a Front Loader ahead of him clearing snow in the roadway. Caceres stated he stopped his vehicle in the roadway and then began backing into the driveway of his residence (Southbound) located at 364 Woodward Street. Caceres stated as he was backing into his driveway, MV1 began backing up towards him. Caceres stated he was honking his horn numerous time. Caceres stated the rear of MV1 crashed into the front hood area of his vehicle. I observed damage to the front driver side hood area. Caceres reported no injuries. I spoke with a witness to the crash, Emily Frederick. Frederick stated she was clearing snow off of her car in the driveway of 364 Woodward Street. Frederick stated she heard a car honking and looked up into the roadway. Frederick stated she observed MV1 crashed into the front end of MV2. Photos were taken of both vehicles and submitted to the IT Bureau.

Izaguirre stated the front bumper of his vehicle crashed into the rear of MV2 at a low speed while they were in traffic. No injuries were reported. Pictures were taken and submitted to the IT Bureau. Sgt. McNeil, N495, and I responded to 474 Wodward St. for an auto pedestrian accident. Upon arrival the victim, Elizabeth Cantwell was laying in the street and receiving treatment from NFD and Cataldo. The operator of Ma Reg. 192HV0 was Margaret Meehan. Meehan was visibly upset and was hearing impaired, as a result Meehan was unable to communicate what happened in the accident. At this time we were able to contact Meehans daughter, Bernadette Conley 781-248-6222. Cantwell was transported to Beth Israel Hospital. Traffic Bureau was notified. I went to the BI hospital to get a statement from Cantwell. Cantwell states that her husband parked their vehicle across the street from Starbucks. Cantwell states that she stood next to her car to cross the street. Vehicle 1 came to a stop and Cantwell believed that the vehicle was going to let her cross the street (not in crosswalk). Cantwell states that when she went to cross the street vehicle 1 hit her, causing her to land on the hood and then rolled off onto the street. At this time Cantwell has a dislocated hip and head injury. I took photos of the scene. Meehans vehicle was parked in a legal spot and I transported her to NHQ where she was picked up by her daughter Bernadette Conley. I filled and immediate threat with the RMV and a copy of the report was attached with the form.

On January 3rd, 2020 at approximately 13:10 hours while working N491 I responded to #191 Watertown St for a past Hit and Run accident. On my arrival I spoke with the R/P, identified as Faith Hanson. She reports that while her 2017 red Lexus ma reg. 1LZR81 was parked in the parking lot for #191 Watertown St another vehicle struck her car. She found a note left on her windshield from a witness, later identified as Keith Forget, telephone #603/475/3843, stating he observed a black Silverado pickup truck, ma reg. 5RT997 hit her car after attempting to back out from a parking space next to her car. I queried the plate with the RMV that the witness provided and it came back registered to a Derrick Curran, #22 Carter St, Newton Ma. I along with officer Mark Hagopian went to Mr Currans residence and spoke with him. He first denied ever leaving his house today. However, Mr Curran, after I informed him someone witnessed his truck hit the other car and could identify the operator he reluctantly took responsibility. He further stated if I did hit the other car I didn't realize it. I provided both Mr Curran and MS Hanson a copy of each ones information for their insurance companies.

The operator of V2 Mr. AKSU stated he is driver for UBER and was traveling in the right lane of westbound lane of Washington St. He stated he activated his left turn signal to turn into the left lane. He stated he did not observe V1 until impact. Passengers AMANDA SHORE, MARYSA MASSOIA, BRIANNA URENA, MARISSA DELAURENTIS, KERI LEHTONEN, CAROLINE SIPPIN all stated the operator of V2 all of a sudden turned left into V1. There were no reported injuries at this time, all parties advised if anything changes to see a doctor. All parties advised of the process. The passengers contacted another ride share company that arrived and transported the passengers.

Operator of vehicle one , Allan Wint stated on 01/27/2019 at 09:40 hours he was driving his employer's pick up truck south bound on Lexington St near number 227. Lexington St is a public way in the city of Newton. Wint is a driver for Auto Zone auto parts located at 140 Lexington St , Waltham, MA ( 781 894-0656 ) . Wint was driving a 2018 Chevrolet Colorado pick up tuck ( MA reg 8CA823 ) that Auto Zone leases from Lease Plan USA Ltd. Wint stated that he was headed to Webster St in Newton to pick up a delivery. Wint stated that as he approached the area of 277 Lexington St he believed that the traffic light that is at the marked crosswalk was green for south and north bound traffic. Wint stated that he did not see any pedestrians in the crosswalk, until he heard the sound of an object impacting into the front driver's side of his vehicle. Wint stated that he stopped his vehicle approximately ten yards past the crosswalk and got out. Wint stated it was then he realized that his vehicle had stuck a female and a dog. Wint stated that he was not operating any electronic devices at the time of the crash, and I observed none in his vehicle. Wint stated he was not injured in crash. Wint's vehicle had damage to the driver's side mirror and lower molding of the driver's side front door. Wint's vehicle did not require a tow. Witness Bessy Shanks stated that she was driving in her vehicle behind Wint's vehicle south bound on Lexington St. Shanks stated that Wint's vehicle was driving slow and swerving approximately two blocks prior to the crash. Shanks stated that she believed that Wint may of been distracted. Shanks stated that south and north bound traffic was stopped because that cross walk light was red and that a female and a dog were crossing from the east to west in the marked crosswalk in font 227 of Lexington St. Shanks stated that the front driver's side of Wint's vehicle srtuck the female and the dog as they were approximately halfway into the crosswalk. Shanks stated that the female and dog both fell to the ground after the crash. Witness Elizabeth Keegan stated that she was driving in her vehicle south bound on Lexington St two vehicle behind Wint's vehicle. Keegan stated that she was not sure what color the traffic light was at the time of the crash, but traffic was stopped both not and south bound traffic was stopped. Keegan also stated that Wint's vehicle " didn't even slow down ". Keegan stated that Wint's vehicle struck the female pedestrian and the dog as they were in the marked crosswalk in front of 277 Lexington St. Witness Henry Taylor stated that his vehicle was the first one stopped at the red crosswalk light at 227 Lexington St in the north bound lane. Taylor stated that north and south bound traffic was stopped and that Wint's vehicle did not stop and stuck a female pedestrian and a dog in the crosswalk. Witness Jacqueline Deos stated that her vehicle was stopped behind Taylor's north bound on Lexington St. Deos stated that north and south bound traffic was stopped for the red light . Deos stated that Wint's vehicle did not stop and that it struck a female pedestrian and a dog in the marked crosswalk. The pedestrian in the crosswalk was identified as Xiaoping Sun. Sun stated that she and her dog stopped on the south bound side of the marked crosswalk across from 227 Lexington St. Sun stated that she activated the crosswalk light and waited until the light was red for north and south bound traffic, and that the walk single was activated. Sun stated that she looked to her left and saw traffic north bound traffic was stopping. Sun stated that she turned to her right and entered the marked crosswalk. Sun stated that she and her dog had crossed over the north bound side of Lexington St and she then looked to her right and saw Wint's vehicle not stopping and " going very fast ". Sun stated that the front driver's side mirror of Wint's vehicle stuck her in the face . Sun stated that as she fell to the ground she was struck on the left leg by Wint's driver's side. Sun stated that she did not lose consciousness after she had fallen to the ground. Sun was transported via EMS to The Brigham and Women's Hospital for treatment for head and neck injuries and abrasions to her legs. Sun's dog died as a result of being struck by Wint's vehicle. Photo's of the crash site and of Wint's vehicle were taken and the disc was downloaded by the NPD's IT Bureau. Before leaving the crash site, I tested the crosswalk lights and they were in working order. Based upon my observations of the crash site, Wint's vehicle, and the statements made to me, I concluded that Wint was at fault for this crash. I issued Wint MA Citation T 2013958 and cited him for a violation of MGL 89/11 , failure to yield to a pedestrian in a crosswalk.

The operator of MV1 states that she was backing out of the driveway of 384 Ward street and backed into a vehicle parked in front of Temple Emmanuel. The operator of MV1 was unable to locate the owner of MV2 but wrote down the registration and came to the Newton Police Headquarters to report the crash. MV1 sustained no damage and MV2 sustained minor damage to the drivers side door. The operator of MV1 was uninjured. I have been unable to contact the owner of MV2 thus far.

On 01/28/2020, at 1634 hours, I responded to the intersection of Centre St and Washington St, just before the exit 17 on ramp to the Mass Pike (WB), for a two car MVA. Upon arrival, I observed MV1 to be facing westbound on Washington St in front of MV2. MV1 had minor damage to the rear passenger side. MV2 was stopped on Centre St facing southbound towards the Mass Pike entrance with damage to the front end. Through an interpreter, the operator of MV1 stated he was in the lane to the left of MV2 and traveling straight onto the Mass Pike when he somehow collided with MV2. The operator of MV2 stated he was in the lane to the right of MV1 attempting to travel straight onto the Mass Pike when MV1 turned right onto Washington St in front of him causing the accident. No injuries were reported.

On 2/3/2020 at approx 1114hrs while assigned to 497 I responded to 2370 Commonwealth Av, The Speedway Gas Station for a report of a two car crash in the parking lot w/o injuries. Upon arrival I met with the owner of Ma Reg 25WR69, Anthony LUONI who stated he was stopped at the gas pump when the operator of Ma Reg 3672310 , Joshua RICHARDSON who was trailering u-haul trailerTX Reg AO37011 backed into him . RICHARDSON admitted he was attempting to back up and align with the gas pump when he accidently struck LUONIS car. RICHARDSON additionally admitted to having an expired license and registration out of NH and no insurance, RICHARDSON also had a valid rental agreement for the UHAUL trailer TX reg AO37011 with insurance . RICHARDSON was able to re register his van on scene and provided me with an emailed copy of confirmation of registration renewal and his passenger Brendan SEIGAL who had a valid NH license was able to drive . RICHARDSON issued a criminal application on Mass Uniform Citation T2015308 for violation of MGL 90/10 Unlicensed Operation and 90/9 Operating Unregistered MV .

DRIVER OF VEHICLE #2 STATES HE WAS TRAVELING E-BOUND ON OTIS ST. WHILE DRIVING HE STATES VEHICLE #1 ENTERED FROM A PARKED POSITION AND HIT HIM IN THE FRONT RIGHT QUARTER PANEL. VEHICLE #1 HAD EXTENSIVE FRONT END DAMAGE AND WAS TOWED BY PERFECTION. VEHICLE #2 HAD RIGHT FRONT QUARTER PANEL DAMAGE AND SCRAPES ALONG THE RIGHT SIDE OF THE VEHICLE. VEHICLE WAS STILL OPERATIONAL AND DID NOT REQUIRE A TOW. ALL PARTIES REPORTED NO INJURIES. ALL PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES.

On Friday, February 7, 2020 while assigned to Traffic unit N525, I responded to the rear parking lot of 1 Boylston Street, Newton for a report of a hit and run motor vehicle crash. I spoke with the operator of MV1, Hong Zhang (S39408212). Zhang stated while operating her 2019 Volkswagon Tiguan (MA: 6YJF90), she was attempting to back out of a parking spot in the parking lot behind 25 Boylston Street, Newton. Zhang stated while backing out of the spot she crashed into an unknown vehicle traveling Westbound through the parking lot. Zhang stated the only information she had for the other vehicle was that it was a large red and black truck. I observed damage to the rear trunk window of MV1. Zhang reported no injuries. Zhang stated she was unsure if she hit the vehicle or if something fell off of it as it was passing by. I went to several local businesses in the area in an attempt to view surveillance footage of the area where the accident occurred with a negative result. While canvassing the area for the other vehicle involved, I observed a red dump truck (MA CON: T71619) removing snow on the property next to Hammond Pond Parkway. The operator stated he has been working down that end of the parking lot for most of the morning and stated he was not involved in a crash. I observed no fresh damage to the vehicle consistent with a crash.

MV1 stated she was parked in her space when MV2 "backed into me" causing minor damage. After the collision MV2 "sped off" e/b on Needham St. A witness confirmed this chain of events. A partial Mass Reg of 21D81 was provided. A message was left for the owner of (MA REG: 2LD812) as that is the closest plate and vehicle is registered in close proximity to where the incident occurred. Dunkin's cameras do not cover this area of the parking lot. Traffic Bureau update (Officer Gaudet): On Monday, February 10, 2020 I made contact with the registered owner of MV2, Michael Ricci. Ricci stated he is currently a resident of San Diego, California and he was in Lake Tahoe over the past weekend. Ricci stated his vehicle (2009 Honda Accord, CA: 7XAS230) was with him at this time so he could not have been involved in this crash. Ricci's registration of MA: 2LD812 was cancelled as of 2/24/2017.

On 02/14/2020, at 0658 hours, I responded to the area of 935 Washington St for a hit and run. MV1 was parked facing westbound, with the owner inside, when MV2 struck it mirror to mirror. MV1s mirror was torn off. The operator of MV2 stopped momentarily but denied an accident occurred then drove away. The operator of MV1 stated that after stopping the driver got out and ran around to the passenger side of his vehicle to do something then returned to the drivers seat. He was described as a dark skinned male wearing a hat and a beaded necklace. The operator of MV1 stated there were scrape marks on MV2s passenger side mirror. The vehicle is registered to REM Services Transportation Services LLC in Bedford. A voice message was left on the company's line. On 02/15/2020 I was able to speak with the driver of MV2. He stated he was traveling west bound on Washington St when the driver of MV1 began honking his horn at him. Both vehicles pulled over and he stated that the operator of MV1 accused him of striking his mirror. The operator of MV2 stated he did not strike anything and that the scuff marks were already on the mirror. Both parties were advised.

On 02/14/20 at approximately 14:45 I responded to a crash in the parking lot outside of #50 Winchester St- The Tile In Style store. Upon my arrival the Fire Department was already there and I was met by the owner of the building Bruno DiFazio. The vehicle that had struck the building had left the scene but he showed me security footage of the crash. The video showed a white S.U.V. (later determined to be Mass. Reg# 6GEL40 White 2017 Acura RDX owned and operated by Eleonora Revzin) pull slowly into a parking spot and then accelerate up and over the curb and into the building. The crash caused moderate damage to the brick facade and inside wall of the tile store. A short time later, Revzin drove back into the parking lot where I spoke with her. Revzin stated she didn't know why her car had accelerated into the building but she was having some personal issues and may have been distracted. She explained that she left the area because she was shaken up but then as she had time to clear her head, thought she should return to the store. There was minor damage to her vehicle and she stated she was not hurt and didn't want me to call the Medics for an evaluation. I issued Revzin Mass. Citation #T2013436 in hand for N.C.O. 19/75- Failure to use care in stopping and a verbal warning for leaving the scene. City of Newton Inspector Paul Nelson showed up and deemed the building to be structurally safe. N523 Officer Wilson arrived and took pictures of the scene.

On Tuesday, February 18th 2020 at 1908 hours, I responded to the area of 94 Vine St for a single motor vehicle accident. The vehicle (Mass Reg # 5FRT20, 2013 ELANTRA SEDAN COLOR BLUE) was being operated by William Lake. Lake was headed east on Vine St toward West Roxbury when he crashed into a City of Newton Tree. The road conditions were wet with heavy rain. Lake stated the wet conditions caused him to lose control of the vehicle. There was major front end damage to the vehicle and airbag deployment. Lake suffered a laceration to his left eye and complained of neck pain. Lake was transported to Newton Wellesley Hospital by the Medics. The vehicle was towed by Tody's. I took pictures of the accident and submitted them to the IT Bureau to be uploaded.
