Mv#1 was attempting to maneuver between an illegally parked moving truck (Rare Moving & Trucking 617-825-7273) and a legally parked car (Mv#2) on Jefferson St E/B in front of #51A. #1 operator stated moving truck workers were trying to help direct him through. On Monday, July 1st 2019 at 0740 hours, I responded to a single car Motor Vehicle Accident outside the area of 140 Vine St. The vehicle was a 2016 Black Merz Mass Reg # 5AX136 being operated by 17 year old Eleanor Grace Bomberg. The vehicle was headed north on Vine St when it veered off the side of the road and collided with a city tree outside of 140 Vine St. The vehicle flipped over and landed in the middle of the street. The vehicle suffered extensive damage to the front end and right side. The operator suffered minor injuries to her arm. Eleanor's father, Patrick Bomberg, signed the patient refusal. Todys's towed the vehicle. Pictures were taken and submitted to the IT Bureau.

OPMV#1 Was traveling Eastbound on Washington St when they were struck by the OPMV#2 exiting the NWH at 2014 Washington St. OPMV#1 Stated he was traveling straight on Washington St heading home and noticed a vehicle pulling out into his lane then stop. He slowed down to let them go but the vehicle didn't move so he proceeded to travel straight and the vehicle pulled out in front of him colliding with his vehicle. OPMV#2 Was traveling Northbound on the NWH exit/entrance roadway attempting to make a left turn onto Washington St traveling Eastbound when he was struck by the OPMV#1. OPMV#2 Stated he thought he could make a left turn until he was suddenly struck by the OPMV#1. Both vehicles were towed by Tody's Towing. EMS evaluated all parties with no injuries.

The owner of MV1 (MA Reg: 673GR1) was parked on the side of the road in the area of 320 Crafts St. MV1 was not in the vehicle, and came outside when she heard a collision. MV1 was parked legally facing southbound on Crafts St. The operator of MV2 (MA Reg: 228YM9) stated she is unfamiliar with the area and was trying to figure out where to turn while driving down Craft St. MV2 stated that she looked at her GPS, came out of the travel lane, and sideswiped MV1. MV1 sustained damage to the rear and drivers side. MV2 sustained damage to the front bumper passengers side. MV2 was evaluated by medics and signed a patient refusal. MV2 was towed by Tody's Towing. Mv#1 was parked unoccupied in a marked space in front of 1353 Washington St W/B. An anonymous witness reported #1 was sideswiped there by a light colored MV (#2) bearing partial NH reg 333. I later located Mv#2 a white Subaru NH reg 333 3334, parked in the West Newton Municipal lot directly behind 1353 Washington St. I also met the #2 operator who stated, while pulling out forward, #2 front passenger side bumper sideswiped #1 driver's side rear bumper. Both vehicles sustained minor damage. All parties were advised and satisfied with this investigation and documentation of this collision. No further police action at this time.

Operator of vehicle 1 states she was sitting inside her parked vehicle at a metered space, when she was struck in the left rear by vehicle 2 as it was parallel parking. The operator of vehicle 1 states she got out and talked to the operator of vehicle 2 but then got back into her vehicle to call a friend as she did not know what to do in a situation like this. While the operator of vehicle 1 was on the phone, the operator of vehicle 2 left the area. I was able to get in contact with the company vehicle 2 is registered to. I spoke to the operator of vehicle 2 who states he was in Newtonville today and did strike another vehicle while parallel parking The operator of vehicle 2 states he spoke to the other operator thought they agreed neither vehicle had damage and went their separate ways. Both operators were informed to contact their respective insurance companies. No injuries reported.

On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

RP David Alessi who is an employee for the chestnut hill mall stated he was alerted to a light post down outside of the mall in the parking lot. David stated he met with operator of MV#1 that stated he was backing up did not see the light poll behind him. He struck the light poll causing it to uprooted and fall over. MV#1 was not on scene I gathered the information from RP David. MV#1 was operating a garbage truck for company JRM whom is subcontracted out with the mall. I was not able to reach the drive, he will not be issued a citation. I then cleared with no further incident.

On 07/29/19 at 11:13 hours, I responded to the Dunkin Donuts parking lot located at 138 Needham Street for a three car motor vehicle accident. Operator of MV1 stated at first she could not recall what occurred during the accident due to lightheadedness at the time. After, she then stated after pulling in to the Dunkin Donuts lot she was attempting to back in to a parking spot in the back of the lot. While backing up, she attempted to hit the brake pedal but must have pushed the gas pedal instead. The vehicle then struck parked/unoccupied MV2 and MV2 was then pushed in to parked/unoccupied MV3. The operator was checked out by Cataldo Ambulance and signed a patient refusal. MV1 sustained moderate driver's side rear bumper area damage but it was still driveable. MV2 sustained damage along both sides of the vehicle and a rear broken axle and it was towed by Tody's. MV3 sustained scrapes and dents to the entire driver's side but the vehicle was driveable.

On 08/02/19 I responded to single vehicle crash across the street from #33 Dedham St. The operator of veh #1 stated he was driving due North on Dedham St. when his car veered into the city tree. There was airbag deployment and heavy damage to veh #1. Operator of veh #1 had some cuts and scrapes to legs but was checked out by the Medics and signed a patient refusal. The vehicle was towed by Tody's towing and the operator was issued Mass Citation #T1445105 in hand for CH89/ Sec4a Marked Lanes violation (traveling outside of breakdown lane) and Ch 90/ Sec 18 Speeding (based upon the significant amount of damage to the vehicle and how far the debris was found from the crash site) . The operator was picked up on scene by his daughter. Traffic Officer McSweeney arrived and took pictures of the damage to the city tree.

Operator of vehicle 1 states she was traveling westbound on Washington St when the vehicle to her right began to enter her lane and then struck her passenger side pushing her onto the raised island. As a result vehicle 1 suffered a flat front driver side tire. Vehicle 2 did not stop and continued westbound on Washington St towards Wellesley. Operator of vehicle 1 was able to follow the other vehicle and got a license plate of MA reg#149DE7 and described the vehicle as a black sedan. AAA responded and fixed vehicle 1s flat tire and she was able to go on her way. I ran the license plate that was given for the other vehicle and it came back to a 2013 black Kia Rio sedan owned by a Dan Kuksin of 24 Bryon Rd apt 2 in Boston. I was unable to get a phone number for Kuksin. Dispatch requested Boston send a unit by to try and make contact and as of finishing this report I have not heard back. Traffic Bureau Update (Officer Gaudet): On Tuesday, July 6, 2019, I attempted to make contact with the registered owner of MV2, Daniel Kuksin (S61105670), with a negative result. A Hit and Run inquiry was mailed to the Kuksin. On 08/07/2019 the owner of MA reg#149DE7 came into headquarters to discuss what happened. With him was his father, Arkady Kuksin who was operating the vehicle at the time of the incident. Kuksin admitted to operating the vehicle in the area at the time of the incident but stated that someone had thrown a rock at his vehicle causing damage. He then stated that it was not thrown but kicked up from his tire. Myself and Officer Gaudet went outside with Kuskin and examined his vehicle and saw obvious damage that corresponded with the damage to vehicle 1. There were obvious signs of fresh paint transfer. I informed Kuksin that I would be updating my report and he would be receiving a summons in the mail for 90/24 leaving the scene property damage. i mailed Kuksin MA uniform citation T1444589.

On 08/06/19 at 12:34 hours, I responded to Industrial Place by The Lakeshore Learning Store at the request of Newton Police Officer Justin March who witnessed an accident in the parking lot of 300 Needham Street. Officer March was working a road construction detail at the time in the City of Newton. He states that at approximately 12:20 hours he observed MV1 exiting the parking lot of 300 Needham Street( a drive way to a building that the public has access to). There is an island in the middle that separates the entrance and exit of the lot as it goes out to Needham Street. As MV1 was exiting, he drove up on the island and pushed two large stone builders out of place. There was also damage to numerous different flowers planted in the ground. MV1 then exited the lot and went eastbound on Needham Street. Officer March then followed in his vehicle and kept beeping his horn to alert the driver to stop. The operator turned on to Industrial Place and pulled over. Ofc. March attempted to speak to the operator, Gelvin Perez-Ramirez but there was a language barrier. Next, Ofc. Calderon responded and assisted with Spanish translation. He asked the operator what occurred and he stated that he never saw the island. Due to the fact that the operator left the scene of the accident and never reported it, he was issued MA Uniform Citation # T1445351 for Ch. 90/24/C. MV1 sustained damage to the two exhaust pipes on the passenger side of the truck. There is also possible undercarriage damage. Ofc. M. Wade took photos of the scene.

On 08/13/19 at 14:53 hours, I responded to a hit and run motor vehicle accident located at 244 Needham Street. The operator of MV1 states she was parked in a parking spot to the right of the Splash business at 244 Needham Street. The parking spot is located on Charlemont Street. After parking, she was sitting in her motor vehicle when she heard a bump to her driver's side rear end from MV2 backing up from a parking spot across from her. She says the operator of MV2 turned and looked at her and kept proceeding (NB) on Charlemont towards Needham Street. She then exited her vehicle to try and get the operator's attention to no avail. She took a photo of MV2 which was a black Toyota Highlander bearing MA reg.# R85045. She described the operator as a white female in her forties with possibly dark curly hair. I observed minor paint damage and scratches to the driver's side rear quarter panel area. The operator is unsure if that damage is from the accident. The owner of MV2, Steven Masterson has a residential address of 40 Wild Acres Rd in North Attleboro, MA and a mailing address of 34 Central Street Foxborough, MA. A telephone search revealed a phone number of (269)857-7814 for him out of Aiken, SC and the Foxborough address but the number is not in service. UPDATE: I was provided a phone number for Masterson from N. Attleboro PD. On 08/16/19 at 12:45 hours, I spoke to Sheila Masterson who stated she was driving MV2 at the time of the reported accident. She stated that she was unaware that she struck MV1 and saw the other operator looking at her but thought nothing of it. Due to the accident being very minor or possibly no damage at all, no further action will be taken. Masterson was provided with the other operator's information.

Traffic Bureau update (Officer Gaudet): On September 4, 2019, I made contact with the owner of LPS Enterprises, Tony Lisa. Lisa confirmed MA TRN: C98747 is registered to his company. Lisa stated he will look into who was using the trailer on 8/30/19 and will contact me with their information. I spoke with the operator of MV2, John Subilosky (S89884850). Subilosky stated he was operating a 2011 GMC Sierra (MA CON: T58930) with MA TRL:C98747 attached to it on the afternoon of the accident. Subilosky stated he was not aware he was involved in an accident and stated he would have stopped had he known. Subilosky stated there was a bobcat in the trailer at the time of the crash. Subilosky stated there is no damage to the trailer. Due to the size and weight of the trailer attached to MV2, it is reasonable to believe if MV2 made minor contact with MV1, the operator of MV2 would not have been aware. Subilosky has been added to this report as the operator of MV2.

Owner of Motor Vehicle #1 stated that he was exiting the gym at Planet Fitness and he noticed that someone was leaving a piece of paper on his front windshield. He was approached by the man who told him that he witnessed a Gray Dodge Ram bearing MA Reg 9JGD90 strike his Jeep Grand Cherokee that was parked and unoccupied causing moderate rear drivers side damage. MA Reg 9JGD90 came back to a 2018 Dodge Ram 1500 Gray in Color and registered to a Kenneth Doucette out of 41 Thaxter Street, Newton MA. I was able to locate several telephone numbers from our in house system however those numbers were no longer in service. I made several attempts to make contact with Mr. Doucette at his residents and had negative results. N491 Officer D. Healy also made an attempt to make contact with Kenneth Doucette later on in the evening and had negative results. I left a voicemail message to the witness and have yet to receive a return call for questioning. This will be further investigated on my next tour of Duty. **UPDATE Wednesday 09/04/19** At approximately 10:00 a.m. I went to 41 Thaxter Street to once again attempt to make contact with Kenneth Doucette and had negative results. I left another voicemail for the witness Claudio Silva and had negative results. I then spoke to Traffic Officer M. Gaudet to inform him about this incident and he stated that he will follow up with this investigation. Traffic Bureau update (Officer Gaudet): On September 4, 2019, I attempted to make contact with the witness to the crash, Claudio Silva, with a negative result. I made contact with the registered owner of MV2, Kenneth Doucette (S80920300). Doucette stated he was operating MA: 9JGD90 at the time of the accident. Doucette stated he pulled into a parking spot next to MV1 and stated he didn't think he "hit it hard". Doucette stated a short time later he left the area and when he got back to work he observed damage to his own vehicle. Doucette stated he had damage to the front passenger bumper area. Doucette stated he returned to the crash scene to see if the other vehicle had damage but it was gone. Doucette was advised and provided Massachusetts Uniform Citation T2016422 in hand for Chapter 90, Section 24 (Leaving the Scene of Property Damage, MV).

On 09/04/19 at 08:22 hours, I responded to 1052 Chestnut Street for a three car motor vehicle accident with minor injuries. All four parties involved were shaken up as a result of the accident but signed patient refusals with Cataldo Ambulance One. She then heard her phone ring and went to retrieve it by looking down for a second. On 9/08/19 Kevin Coston came into NPD Headquarters to report a MV/Ped with Injury that had occurred on 8/23/19 at about 06:15 in the parking lot of Whole Foods (916 Walnut Street). The RP stated that he had left work (Whole Foods) at the back receiving door and was walking to his car, he bent over to take his knee pads off, while he was doing this he was struck by a delivery van that backed into him. The driver took out his headphones and said sorry before driving away. RP stated that he told the driver that he had been hit but the driver just drove away. RP went to hospital (Faulkner) on his own for lower back pain and has not been to work since. RP was able to take picture of the side of the delivery van. The van belonged to Shortstop Distribution Inc (508-655-5948). RP left a message that morning. He was texted by Gedeon (786-352-6695) who stated there was a language barrier and he didn't understand what had happened. RP advised Gedeon to talk to his lawyer or the police. Exact location of accident unknown. Diagram is a estimate. Report printed out and placed in traffic box for follow up. Traffic Bureau Update (Officer Gaudet): On Monday, September 9, 2019 I attempted to make contact with the operator if MV1, Gedeon Duarte, with a negative result. A Hit and Run motor vehicle inquiry was mailed to Duarte. I spoke with the injured party, Kevin Coston, via telephone on September 9, 2019. Coston stated he is an employee at the Whole Foods located at 916 Walnut Street, Newton. Coston stated on August 23, 2019 a black van backed into him as he was kneeling on the ground taking off his knee pads in the loading area of the store. Coston stated he approached the driver who remained in the vehicle to speak with him after he was hit. Coston described the driver as a heavy set Spanish male. Coston stated the operator of the vehicle rolled down his window, took his headphones out of his ear, said sorry, and left the area. Coston stated he contacted the Newton Police to report the accident but was told he needed to come in person to file the report. Coston stated he was unable to come to the station to file the report until September 8, 2019. On Friday, September 13, 2019, I spoke with Gedeon Duarte at the Newton Police Department. Duarte stated he spoke Portuguese and did not speak English. With the assistance of David Sargent from the Language Line (1-800-225-5254) translating, I was to speak with Mr. Duarte in the Traffic Bureau. Duarte stated he was making a delivery at Whole Foods and was waiting for another truck to leave so he could leave the area. Duarte stated a male party (Coston) came up to his driver door and was yelling him. Duarte stated that due to language barrier he was unable to understand what the person was saying to him, but stated the male party was yelling, "Fuck you! Fuck you!" Duarte stated he did not know what was going on and stated he left the area to allow the male party to calm down. Duarte stated he returned a minute later the male party did not want to speak with him. Duarte stated he returned a second time later in the morning and Coston was not there. Duarte stated he received a call from his employer stating Kevin Coston had called for him and he was provided with Coston's phone number. Duarte stated he texted Coston to find out what happened that morning. Duarte showed me a text message exchange from August 23, 2019 at 9:03 A.M. between himself and a party listed as "Kevin Whole". In the text messages, Duarte stated he is the driver of the van and that his supervisor called him. The other party responded and stated "Talk to the police and my lawyer". Duarte stated that he did not understand what happened and that he returned to whole foods. Duarte ended the text exchange with "927535425 my number of policy..." Duarte stated he had no idea he made contact with Coston and stated at the time of the alleged accident he did not hear/see/feel anything otherwise he would have stopped. Duarte's information has been updated in this report to reflect him as the operator/vehicle involved. Duarte also submitted pictures of the rear of his vehicle, a 2008 black Ford E350 (MA: 8NY171), from the day of the accident. Photos were submitted to the IT Bureau. Officer Wade went to the Whole Foods located at 916 Walnut Street, Newton in an attempt to view surveillance footage of the accident. Officer Wade stated the surveillance camera did not capture the incident and the van not viewed at all.

On Tuesday, September 10, 2019 while assigned to Traffic unit N525, I responded to the area of Beacon Street and Roslyn Road, Newton for a report of a motor vehicle/bicycle crash. The weather at the time of the crash was overcast. The road surface was dry. Beacon Street and Roslyn Road are both public ways maintained by the City of Newton. I spoke with the bicyclist involved in the crash, Steven Greenzang (S96615185). Greenzang stated he was traveling Westbound on Beacon Street in the shoulder lane on his black Specialized bicycle. Greenzang had a yellow reflective vest on and was wearing a helmet at this time. Greenzang stated a vehicle in the travel lane to his left drove by him and hit the mirror located on his left handlebar. I observed damage to the mirror. Greenzang report no injuries. I spoke with the operator of MV1, Sarah Bonfanti (S66811372). Bonfanti stated she was parked in front of 1411 Beacon Street waiting to pick up a co-worker that resides at that address. Bonfanti stated a bicycle traveling on the sidewalk to her right crashed into the passenger side door mirror of her 2008 Toyota Scion (MA: 681YH1). Bonfanti stated the bicyclist was acting "crazy" during her interaction with him and he was trying to take her picture. Bonfanti stated she got back into her vehicle and moved up the roadway to wait for police to arrive on scene. I canvassed the area where the accident occurred with Sgt. Dempsey. There was glass from the passenger side mirror of MV1 and Greenzang's bicycle in the main travel lane of the roadway and the shoulder lane where Greenzang stated he was traveling. We also observed a large tree and a telephone pole on the sidewalk next to the roadway where the accident is alleged to have occurred. After speaking with all parties involved, observing the location of the glass in the roadway from the crash, and the location of the tree and telephone pole on the sidewalk where he accident occurred; Greenzang would not have been able to operate his bicycle on the sidewalk and be close enough to the roadway to make contact with MV1. Prior to clearing the accident, I asked Bonfanti if she had any questions and if she wanted to report any injuries. Bonfanti stated she was hit by the bicyclist during her exchange with him and began grabbing her left leg. I asked Bonfanti why she did not inform the officers on scene that she was assaulted. Bonfanti stated she said something to our Dispatch when she called. I asked Bonfanti if her leg was injured. Bonfanti stated I was putting words in her mouth and that she was just scratching her leg. Bonfanti then stated she was hit in the right shoulder. I asked Bonfanti if she would like an ambulance to respond and evaluate her. Bonfanti declined and stated she was more concerned about her mirror. I advised Bonfanti if she wanted to pursue charges against Greenzang she could do so in Newton District Court. I listened to the 9-1-1 recording Bonfanti made in regards to this incident. Bonfanti stated a pedestrian busted out her window. Bonfanti then stated to our dispatcher that the bicyclist lost control of his bicycle and smashed into her car. During her exchange with the our dispatcher, I did not hear Bonfanti say she was physically assaulted by Greenzang.

A witness (Baptiste, Franck Jean) stated he witnessed the accident and believed the vehicle was travelling at approximately 40mph and lost control around the bend and struck the wall. David Donahue from Tody's Towing towed the vehicle. The homeowner at 219 Lake Ave was home and notified about the damage to their property. Six digital photos of the accident scene were taken and submitted to IT. Update 9/28/19: Upon further review and discussion with Sgt. Dempsey, Operator of MV1- Gallivan, Chase was issued citation T1445308- 89/4A- Marked Lane Violation and 90/8-JOL Passenger Restriction. Citation was issued in hand.

On Tuesday, October 1st 2019, at approximately 3:45pm, I, Officer Brooks, responded to Valentine street by Lenox street for a motor vehicle roll over accident with entrapment. Upon my arrival I observed MV1(MA REG SN227) laying on its passenger side with heavy damage to the entire vehicle, and all air bags deployed. The operator was trapped in the driver's seat but was concious and alert. The fire department arrived and extracted the operator, who signed a patient refusal with Cataldo medics. The operator stated she had been traveling southbound on Valentine street, looked down for a moment and struck a parked car with the passenger side of her vehicle. Her vehicle then spun out and rolled onto its side. I spoke with the owner of MV2(MA REG 9WG975) who stated he was alerted by the sound of the crash. His vehicle was legally parked on Valentine street. MV2 sustained heavy damage to the driver's side rear end. Both vehicles were towed by Tody's and inventory forms were filled out and filed.

Based on observations and statements made, the following occurred. Katerina Iliades stated that on 10/23/2019 at 11:56 hours she was standing in front of the rear driver's side door of her vehicle MA reg 7BM656 2019 Land Rover while it was parked facing north in front of 316 Walnut St. Walnut St is a public way in the city of Newton. Iliades stated that she was placing her daughter Melina Bichelen in her car seat and when she observed MA reg 8EP212 a white Subaru Outback driving north bound on Walnut St near the marked crosswalk at 312 Walnut St. Iliades stated that she observed the Subaru was being operated by an elderly white male. Iliades stated that the she observed the Subaru's front drivers side strike several orange plastic three foot poles that are affixed to the roadway around the crosswalk. Iliades stated that after the Subaru struck the poles at the crosswalk it veered towards her. Iliades stated that she leaned back into the back seat of her vehicle and pulled the door towards herself as the Subaru's passenger side mirror made conact with her vehicle. Iliades stated that the Subaru continued north bound on Walnut St without stopping or slowing. Iliades complained of lower leg pain and refused medical aid or transport to a hospital at the crash scene. I observed no damage or markings on Iliades vehicle. MA reg 8EP212 is registered to Harilaos Papadogiannis ( DOB 01/02/1941 ) of 134 Apt 4 North St , Newton. I went to 134 North St and found MA reg 8EP212 a 2014 Subaru parked in the lot of 134 North St. I observed that the Subaru's passenger side mirror was lose from it's mounting and was turned down and was facing the wrong way. I further observed that the out side casing of the Subaru's mirror had scuff marks consistent with it forcefully making contact with an other object. I spoke with Papadogiannis who stated that he was driving his Subaru north bound on Walnut St at 11:56 hours on 10/23/2019. Papadogiannis stated that he could only recall striking the plastic poles near the crosswalk and nothing else regarding the crash. I as spoke with Papadogiannis he was confused and I had to repeat myself and my questions to him several times. I instructed Papadogiannis what steps he needs to take regarding filing out an operator crash report and contacting his insurance provider , but I believe that he did not fully comprehend what I was conveying to him. Based upon my observations at the crash seen and my conversations with Iliades and Papadogiannis , I believe that Papadogiannis was at fault for the crash. Based upon my observations of Papadogiannis I filed an Immediate Threat Request form and faxed it to the MA RMV Driver Control Unit. Photos of both vehicles were taken and downloaded by the NPD's IT Bureau.

The operator of motor vehicle 2 stated she was driving and she stopped to back up, as she backed up she thought she hit a curb but then the operator of vehicle 1 was at her window saying she hit him. As a result of the crash vehicle one sustained very minor front end damage and vehicle 2 had no noticable damage.

I was dispatched to 854 Beacon St, Newton, MA for an overturned motor vehicle accident. I arrived on scene and saw Vehicle #1 (MA reg 9AS213) overturned on its driver's side with the female operator/registered owner trapped inside. Newton Fire arrived on scene and mechanically removed the operator out of the motor vehicle. Prior to where Vehicle #1 was overturned I observed Vehicle #2 (MA reg 615FF6) with rear bumper damage and a snapped rear axle. Vehicle #2 was unoccupied. Operator of Vehicle #1 was transported to Newton-Wellesley Hospital for treatment by Newton Medics. Both motor vehicles were towed by Tody's and inventory sheets were completed and filed. I spoke with the Operator of Vehicle #1 at Newton-Wellesley Hospital. Operator stated that she does not remember what happened before the crash and that she only remembers waking up in her car. I told her that her car was towed and a report is on file. Ofc Selig spoke with the registered owner of Vehicle #2 and notified her of the incident. Based on my investigation it appears that Vehicle #1 struck the rear of Vehicle #2 and the operator over steered causing the vehicle to overturn onto the driver's side.

On 11/12/2019 at 12:26 hours, I responded to the Star Market parking lot located at 2040 Commonwealth Avenue for a motor vehicle pedestrian accident. The parking lot is a public/thru way that motorists use to access the Star Market along with other businesses. The weather at the time was cloudy and rainy and the parking lot was saturated. Prior to my arrival, the pedestrian involved left the scene and did not want to speak to the officer nor wanted medical attention. Ofc. Claflin then cancelled Cataldo Ambulance. The pedestrian, Mary Alexander, left her information with a store employee prior to leaving. I spoke to the operator of MV # 1 (Joshua Randel MD Lic.# R534440603244) who stated that he was travelling through the third row of the Star Market parking lot at an appropriate speed. He was then attempting to make a left turn near the entrance/exit of the store. He says that there was a large white van parked in the end handicapped spot towards the left of him. He says the van was sticking out in the spot limiting his visibility. As he was half way through his left turn and coming around the corner, he was not able to see a pedestrian pushing a shopping carriage. He then struck the carriage that was being pushed by Mary Alexander. He further says that the cart jolted Ms. Alexander and she fell down to the ground. At the time, Alexander was walking through the lot near the store entrance heading back towards the parked cars. She was not in the crosswalk at the time which was approximately ten feet down from her location. After, I attempted to go to Ms. Alexander's residence to follow up. I spoke to her friend who was bringing up Alexander's groceries and she stated that she decided to go to Newton Wellesley Hospital for treatment. I then spoke to Ms. Alexander in the lobby of the hospital. She stated that she was exiting the store through the lot with her shopping carriage. She believes she was approximately eight feet away from the door when all of a sudden she only saw red coming at her. She believes MV # 1 was speeding at the time. I asked her if she was struck by the vehicle or was the shopping carriage only struck. She replied that she did not know and only remembers falling to the ground after. She sustained a large bruise to the inside palm area of her right hand and her left knee was swollen. Digital photos of the scene were taken and pictures of Ms. Alexander's injuries were taken at the hospital. As far as possible video surveillance, I was told that the Loss Prevention Department has to access the video. On 11/13/19 at approximately 14:20 hours, I went in to the Star Market and spoke to the manager, Nidal Rajeh, inquiring about possible video surveillance. He said there is coverage in the parking lot and he was able to show me the video. A copy of the video would have to be done by Loss Prevention which I requested. I observed at the 12:12pm mark in the video, MV # 1 was parked five spots up from the third row of the lot that he was exiting. I also observed a white transport van parked in the end handicapped spot mentioned by the operator but it was parked correctly and not limiting visibility. Next, MV # 1 backs out of the spot and drives forward going at a reasonable speed. The pedestrian (Alexander) has already exited the store with her carriage and is walking at an angle toward the same third row of parked cars. As the pedestrian is walking towards the direction of MV # 1, it appears that the pedestrian should be visible to him. MV # 1 then starts to take the left turn around the corner without slowing or stopping and strikes the carriage head on. Due to the impact, Alexander then loses grasp of the carriage and it goes past her twisting her body around. She then hits and rolls up on the hood of the vehicle on her left side. MV # 1 then suddenly stops and immediately gets out and provides assistance to the pedestrian. Due to the video surveillance, Mr. Randel will be issued/mailed MA Uniform Citation # T2012347 for Ch. 90/14/A Failure to slow for a pedestrian.

On Thursday, November 14, 2019, while assigned to Traffic unit N525, I responded to the rear parking lot of the Angier Elementary School located at 1697 Beacon Street for a motor vehicle crash involved a City of Newton owner vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. The operator of MV1, Jessica Moore, stated she was backing out of a parking spot in the rear of the Angier Elementary School while operating a 2007 Chrysler Town and Country (MA:395KR9). Moore stated she did not see MV2 passing by as she was backing out and the rear driver side bumper area of her vehicle crashed into the rear driver side of MV2. Moore reported no injuries. I observed moderate damage to the rear driver side bumper area of MV1. The operator of MV2, Patrice Lys, stated he was traveling Westbound in the parking lot of the Angier Elementary school in a 2018 Ford EcoVan (MA MVN: M2546A). Lys stated a vehicle on his driver side backed out of a parking spot and the rear end of MV1 crashed into the rear driver side of MV2. Lys reported no injuries. I observed minor damage to the rear driver side area of MV2. The operator of MV1 was cited with Massachusetts Uniform Citation T2016428 for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care, Backing). Photos were submitted to the IT Bureau.

On 11/30/2019, while assigned to N494, I, Officer Conary, and other units, responded to Charlesbank Road at Nonantum Road for a single motor vehicle crash into a tree. Upon arrival, I observed a white mini van MA registration 5DV184 with heavy front end damage into a tree on Charlesbank Road, a public way in Newton. The operator of the motor vehicle, CORDERO, Ericka, was sitting in the back seat of the vehicle and was talking on the phone. CORDERO's primary language was Spanish. I asked CORDERO if she was okay and she said yes and I asked CORDERO if she was driving the car and she said yes. CORDERO also told me that she did not have a license. MA State Police were on scene and were able to asist with translating. Medics arrived and CORDERO was transported to Saint Elizabeth's Hospital. CORDERO's son and husband arrived on scene and I was able to confirm her information. Both stated that CORDERO did not have an active license. Newton Dispatch ran all information for CORDERO and confirmed she was unlicensed. CORDERO will be charged with MGL Ch.90/10 Unlicensed Operation of a Motor Vehicle and 89/4A Marked Lanes Violation, Uniform MA Citation T2079476. Due to CORDERO being transported to the hospital, the citation will be mailed to her active address. Pictures of the incident were taken and uploaded to IT accordingly. No further incident to report.

The weather at the time of the accident was light snow. The road surface was wet. Woodward Street is a public way maintained by the City of Newton. I spoke with the operator of MV1, Brad Butera (S94383997). Butera stated he was operating a City of Newton owned 2011 Volvo Front Loader (MA MVN: M88537) on Woodward Street while clearing the roadways during a declared snow emergency. Butera stated he was backing up Southbound across Woodward Street in the area of 364 Woodward Street after clearing some snow. Butera stated he looked and did not see any vehicles behind him. Butera stated he was backing the Front Loader up and the rear back of his vehicle crashed into the front hood area of MV2. I did not observe any obvious damage to the bucket area. Butera reported no injuries. Butera stated the operator of MV2 stated "My fault" to him during their exchange after the crash. Butera is an employee of the City of Newton DPW. I spoke with the operator of MV2, Marco Caceres (S67223904). Caceres stated he was operating his 2006 Mercedes E350 (MA: 26P270) Westbound on Woodward Street. Caceres stated he saw a Front Loader ahead of him clearing snow in the roadway. Caceres stated he stopped his vehicle in the roadway and then began backing into the driveway of his residence (Southbound) located at 364 Woodward Street. Caceres stated as he was backing into his driveway, MV1 began backing up towards him. Caceres stated he was honking his horn numerous time. Caceres stated the rear of MV1 crashed into the front hood area of his vehicle. I observed damage to the front driver side hood area. Caceres reported no injuries. I spoke with a witness to the crash, Emily Frederick. Frederick stated she was clearing snow off of her car in the driveway of 364 Woodward Street. Frederick stated she heard a car honking and looked up into the roadway. Frederick stated she observed MV1 crashed into the front end of MV2. Photos were taken of both vehicles and submitted to the IT Bureau.

Narrative:
On Tuesday, December 10, 2019, while assigned to Traffic unit N525 I responded to the area of Walnut Street and Lake Street, Newton for a report of a motor vehicle crash involving a City of Newton vehicle. The weather at the time of the crash was overcast. The road surface was wet. Walnut Street and Lake Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Joseph Ratta (S82871504). Ratta was operating a City of Newton owned 2017 Ford F350 (MA MVN: M99236) Southbound in the area of 1141 Walnut Street. Ratta stated while in Traffic, MV2 crashed into the rear of his vehicle. I observed minor damage to the rear bumper area of MV1. No injures were reported. Ratta is employed by the Utilities Division. I spoke with the operator of MV2, Bryan Izaguirre (S64158637). Izaguirre stated he was operating a 2016 Honda Civic (MA: 8ZP172) Southbound in the area of 1141 Walnut Street, Newton. Izaguirre stated the front bumper of his vehicle crashed into the rear of MV2 at a low speed while they were in traffic. No injuries were reported. Pictures were taken and submitted to the IT Bureau. Izaguirre was issued Massachusetts Uniform Citation T2012561 for Newton City Ordinance Violation Chapter 19, Section 75 (Failure to Use Care).

Operator of mv1 states he was changing lanes to his right to go north on Galen St. when he was struck in the right door area of mv2. Operator of mv2 states he was changing lanes to the left to go west on Washington St. when mv 1 was also making a lane change. mv2 struck mv1 in the passenger side door area of mv1 with the front drivers side of mv2. Both vehicles were in the newton corner rotary. No injuries reported.

MV#1 was traveling south in the right hand lane through the parking lot of 232 Boylston St. MV#2 was also traveling south in the left hand lane of the lot and turned right (westbound) into the parking lot adjacent to the roadway, striking MV#2. Damage was observed to MV#1's front end while MV#2 sustained a flat tire and damage to its passenger side panel. No injuries were reported and Tody's was contacted to remove MV#1 while MV#2 was able to be driven from the scene.

MV#1 was parked facing northbound on Waban St. MV#2 traveled northbound around MV#1, sideswiping MV#1. Damage was observed to MV#1's driver's side front end, while MV#2 had damage to its passenger side, side panel. No injuries were reported. MV#1 was unoccupied at the time of collision and MV#2 was able to be driven away from the scene.

MV1 was traveling SOUTH on Lexington St when it collided with MV2 while it was backing out of a driveway ( 244 Lexington St). MV2 sustained damage to its rear driver's side. MV1 did not have any damage as it was suited with a plow mounted in front. MV2 operator was transported to NWH due to head/facial injuries from the steering wheel. Conditions of the road were extremely icy. Tody's towed MV2 from the driveway of 244 Lexington St.

Oper of v1 stated he was traveling west on Wolcott St, while traveling near the bend in the road he looked down for a second and crash into a City tree. I took pictures of the crash site and will have them downloaded at the I.T. Bureau. Todys towing arrived on scene and transported the vehicle to their tow lot. The operator had a laceration to his nose due to the impact of the tree Air bag deployment was on the front and right side. Medics arrived on scene, oper signed patient refusal Oper of v1 was advised of the process.

Operator of vehicle one Cataldo Medic Elisle Gedansky stated on 12/24/2019 at 12:55 hours she was driving MA reg AMN 369 a 2016 Ford Ambulance east bound on Myrtle St with it's emergency lights and siren activated responding to a medical call at 52 Myrtle St. Gedansky stated the Newton Fire Eng 2 was parked in front of 52 Myrtle St and that vehicle two Ma reg 9940RB a 2000 VW was parked unoccupied across from 52 Myrtle St. Gedansky stated that she believed that there was enough room to drive between the fire truck and vehicle two. Gedansky stated that the front passenger side of vehicle one struck the front driver's side of vehicle two. Vehicle one had no damage and Gedansky was not injured. Vehicle two had damage to it's driver's side mirror and driver's side front bumper. I took photos of vehicle two and forwarded the disk to the NPD's IT bureau. Owner of vehicle two Aline Elliot of 46 Myrtle St was advised of this incident.

Owner of vehicle #1 reports he parked his car on the street across from #12 Lombard St when vehicle #2 struck his car as it was backing out from his driveway at #12 Lombard St. Operator #2 vehicle was backing out of his driveway to his home #12 Lombard St when he backed up into vehicle #1 while it was parked across from his residence. Note operator#2 left scene prior to my arrival but owner of parked vehicle who was working in the street and heard the crash was able to get #1's information before he left the scene.
