On 01/09/2019, at 1806 hrs, I responded to Washington St. on the Mass Pike overpass for a two car MVA. Upon arrival, I observed MV1 to have damage to the front passenger side and MV2 to have damage to the drivers side. No injuries were reported. The area of Washington St where the accident occurred is five lanes across in the same direction. The operator of MV1 stated he was attempting to change lanes to his right when he collided with MV2. The operator of MV2 stated he was attempting to change lanes to his left when he collided with MV1.

Narrative:
On the above date and time I responded to the area of Washington Street at Centre Street for a two motor vehicle crash. Upon arrival I met with the operator of vehicle one Mr. Kramer, he stated he was traveling north on Centre St at Mass pike over pass bridge. He stated he thought the traffic signal was maybe yellow and he went through the intersection and struck vehicle two causing damage to the left rear bumper and left rear wheel. I spoke to the operator of vehicle two Mr. Bidwell, he stated he was stopped at the red light facing west on Washington Street mass pike off ramp. He stated the traffic signal turned green, as per his work policy he waited 3 full seconds before accelerating. He stated he proceeded through the intersection and was struck by vehicle one in the left side rear. Due to the left rear wheel being struck I notified Tody's towing of 1354 Washington Street arrived on scene and towed vehicle two to their tow lot. Neither party had any injuries and they were both advised. I checked the traffic signals in the intersection and they were fully functional.

On 01/15/2019, at 1919 hrs, I responded to the intersection of Washington St and Centre St for a two car MVA. Upon arrival, I observed heavy damage to the front end of MV1 and moderate damage to the rear end of MV2. The operator of MV2 complained of head/neck injuries and was transported to Newton Wellesley Hospital by Cataldo for an evaluation. The operator of MV1 stated she was attempting to merge onto Washington St from Centre St when she rear ended MV2. The operator of MV2 stated when she slowed to merge onto Washington St from Centre St she was rear ended by MV1. Both vehicles were towed by Tody's. Towed MV inventory forms were filled out and filed.

On the above date/time I responded to the area of Washington Street/Centre St for a report crash involving two motor vehicles. Upon arrival I met the operator of v1, she stated she was traveling straight (east) when vehicle two crossed over lanes and struck her vehicle in the right rear. I spoke with the operator of v2, he stating he was merging lanes and did not observe vehicle one until impact. He stated the left front of his bumper had contact with vehicle two right rear. No reported injuries, all parties advised,all information was swapped.

MV1 stated he was attempting to "turn left" when the car to his left "went straight instead of left" subsequently striking his vehicle. MV1 was spun sideways and MV2 continued into Watertown. I then made contact with the operator of MV2 who admitted to striking MV1, however thought they weren't going to stop so he kept going as well. MV2 was cited for Leaving the Scene of Property Damage & Marked Lanes See incident report 19014042 for further information.

Elaine Forsythe was operating vehicle #2. Elaine took the exit 17 off ramp and attempted to merge onto the Centre Ave rotary. Elaine states that at this time vehicle #1 struck the back of her vehicle. Brian Radzwill was operating vehicle #1. Brain states that while trying to merge onto the rotary he bumped into vehicle #2. Both vehicles had very minor damage. Forsythe was transported to NWH. Her vehicle was parked at the Richardson St lot, meter #4711. Parking control notified for consideration.

On 01/02/2020, at 1813 hours, I responded to the area of Washington St and Centre St for a two car MVA. Upon arrival, I observed MV1 to have minor damage to the front drivers side wheel well. MV2 had minor damage to the passenger side.The operator of MV1 stated he merged onto Washington St from Centre St. While attempting to change lanes he did not see anyone and collided with MV2. The operator of MV2 stated he was traveling westbound on Washington St when MV1 changed lanes into his and collided with his passenger side. MV 2 stated he did not get into an accident, and appeared to be confused about what we were talking about (no visible damage). Two witnesses confirmed MV 1's account of the incident. MV1 was heading southbound coming from Centre Street. MV1 stated that he had the green light and began entering into the intersection heading towards the on-ramp of the Massachusetts Turnpike. MV2 stated that she was getting off the exit ramp of the Massachusetts Turnpike and had a yellow light heading westbound on Washington Street. MV2 drove through the yellow light trying to beat the light. MV2 struck into the drivers side of MV1. There was damage to the driver side door, and rear driver side door of MV1. There was damage to the front left bumper and front center bumper to MV2. There were no injuries from either party invovled. Both vehicles drove away from the scene.

On 5/31/2021, Newton Police responded to the corner of Centre St. and Washington St. for a motor vehicle collision involving two vehicles. Upon arrival, Mv#1 and Mv#2 were both parked facing eastbound on Washington St. MV#1 had significant rear end damage, while Mv#2 had damage to its front bumper. The damage to both vehicles was consistent with a rear end motor vehicle collision. The operator of Mv#1 stated he had yielded before attempting to merge eastbound onto Washington St. from Centre St. when Mv#2 struck the rear of the vehicle. The operator of Mv#2 stated he lost sight of Mv#1 as he attempted to merge eastbound onto Washington St. and collided into the rear of Mv#1. No injuries were reported and Mv#2 was able to be driven from the scene, while Mv#1 was towed away by Tody's Tow Service. No injuries reported.
