On Saturday January 5th 2019 at 18:01, I was dispatched to Crafts St at Waltham St for a report of a 2 car accident. Operator of MV1 stated he was driving southbound on Waltham Street and approached the intersection as the traffic control changed to red. He slowed down and came to a complete stop. Approximately 4-6 seconds later, he was rear ended by MV2. MV1 sustained minor damage to the rear passenger side. MV1 had three other occupants in the vehicle at the time of the accident. The two passengers in the back seat complained of back pain and were evaluated by Cataldo Paramedic Basic 3 and both signed patient refusals. The operator of MV2 stated as he approached the intersection he "was sleeping." When I asked him to elaborate further he was unable to provide me any answers. He was evaluated by PB3 and transported to NWH. He was issued citation # T1441363 for Operating Under the Influence of Alcohol (2nd Offense) and Negligent Operation of a Motor Vehicle. See Report #19000596 for further details. MV2 was towed by Todys Towing due to the operator being transported to the hospital and the passenger stating he was too intoxicated to drive. A towed motor vehicle form was filled out.

On Saturday, January 12th 2019, at approximately 4:12pm, I, Officer Brooks, responded to a single vehicle accident in the area of Cherry street and Derby street, both of which are public ways in the city of Newton. Upon arrival in the area I observed the fire department off with MV1(MA REG 9PAK20), in front of 171 Cherry street. The vehicle had a fire hydrant lodged under the front axle, and front end damage. The fire department was attempting to wake the operator who was unresponsive. The fire department ended up administering 2 doses of Narcan to the operator before he became responsive. The operator was transported to Newton Wellesley Hospital by Cataldo for further evaluation. Based on the accident scene, it appears the operator was traveling southbound on Cherry street, crossed over the center line into oncoming traffic, striking the fire hydrant in front of 154 Cherry street, and then crossed back over into the correct lane of traffic before coming to rest in front of 171 Cherry street. The vehicle was towed by Tody's towing, and an inventory form was filled out and filed. The operator was mailed MA Uniform Citations T0648078 and T0648079 charging him with MGL 89/4A Marked Lanes violation, 90/24 OUI Drugs, 90/24(2)(A) Operating to Endanger/Negligent operation, 90/11 No License in Possession, and 90/11 No Registration in Possession. Officer Donahue took pictures of the fire hydrant and crash scene, which have been submitted to the IT Bureau. The fire department was made aware of the broken hydrant.

Narrative:
MA commercial reg M94754 (vehicle #1), a City of Newton pickup/sander truck, was located crashed over the curb in front of 1970 Beacon St. The vehicle was unoccupied, had the keys in the ignition, and was reported stolen just a few minutes prior to discovery. The reporting party of the stolen truck and uninvolved occupant is identified as City DPW worker George Sementelli. The witness/passerby who discovered the collision is also a DPW worker, identified as Donny Brandt. Pursuant to an investigation outlined in incident #19002213, the operator at the time of the crash was determined to be Michael Carter. It is believed that Carter stole the vehicle from 295 Auburn St, fled the area, and ultimately crashed in front of 1970 Beacon St. Corroborating facts outlined in the incident report put Carter as the operator. The vehicle itself was located partially on the sidewalk and partially on the road, therefore Carter is being cited with MGL 89/4A - Marked Lanes Violation (T1268423). Due to the fact it is believed that Carter fled the scene after the crash, MGL 90/24/C - Leaving the Scene of Property Damage was added as well as MGL 90/23/D Operating After a Suspended LIcense, and MGL 90/24/O - Reckless Operation of a Motor Vehicle. The crash occurred during busy weekday commuting about 0.7 miles from Angier Elementary School. The last 3 charges mentioned have been listed on a second citation (T1268422). Officer Brooks also issued T0648082 for OUI Drugs MGL 90/24/G. See the incident report for details about this charge. In total, there are (3) citations issued to Carter. The vehicle sustained heavy front end damage. It should be noted that the utility pole a couple of houses down from the crash site (Beacon/Varick Rd, pole #212) sustained fresh, minor superficial damage, consistent with the vehicle crashing into it before verring onto the sidewalk. Pictures have been taken and submitted to the IT bureau. Tody's responded and towed the vehicle to the police garage for processing due to the pending other criminal charges involved in this accident/incident. Michael Carter was subsequently placed under arrest and his citations have been placed into his property. Carter suffered a hand injury due to the crash an was transported to NWH Hospital for an evaluation after he was booked.

Narrative:
At approximately 0110HRs on Sunday February 17, 2019, I was dispatched to the area of Dedham St. and Spiers Rd. (both public ways in the City of Newton), for a report of a motor vehicle vs pole accident with injury, airbag deployment and entrapment. Upon arrival, I exited my cruiser with my medical bag and AED and went to assist the motorist involved in the crash. MV1 (MA Passenger: 255ZA9) was completely off the roadway and on the shoulder of the road (facing Eastbound) with its nose contacting the light pole ( Verizon Pole #:103/80), which was now broken and listing to the side. Attempts to open the doors were unsuccessful and I attempted to communicate with the operator through a partially broken driver's window frame which was bent. The operator was slumped forward with his hand cradling his head and he was unable to speak, but was breathing. Newton Fire arrived and attempted to gain access to the vehicle via several manual tools with no success and instead deployed the Jaws of Life which was successful. Newton Fire also made entry into the rear passenger compartment because we viewed a child's car seat and checked with negative results for any child in the vehicle. Once the operator was cut out of his seatbelt and safely out of MV1 and on board Newton Medic 1, I entered the ambulance where he was identified as Mark Topham. It should be noted that Topham was having difficulty answering basic questions such as his name, date of birth, today's date, Etc. At one point when he was asked what today's date was, he gave his birth date of February 21, 1983 and it took him several attempts to say his last name, I had to run him over the air with a partial name to ascertain who he is. Also of note, in my presence and that of Sgt. Devine, Topham informed the medics that he had been consuming alcoholic beverages prior to the crash. Upon viewing the scene, I noted that the roadway was lit and I did not observe any skid marks on the dry, ice-free payment, indicating that there was unlikely any braking or swerving to avoid the collision. I spoke to the reporting party, Lindsay Giardina, she stated that she was traveling on Dedham St. when she heard what she believed to be a motor vehicle crash. When she came upon said crash, she attempted to assist the operator, as she was an off duty firefighter. She was unable to gain access to the inside of the vehicle, but communicated to the operator in the same manor as I did. She stated that she was able to smell the odor of an alcoholic beverage emanating from the operator. Topham was transported to Beth Israel of Boston, where he was assigned patient record number 3109921. I attempted to speak with him at the hospital, but was unable to do so because of his current state; I noted several injuries on Topham, such as a bloody lip, a wound to his lower left side and a wound to his left leg. Due to Topham's statements onboard of Medic 1 and Lindsay Giardina's observations, I will be charging Topham with one count of violating MGL Ch.90 S. 24 K OUI Liquor 2nd Offence, one count of violating MGL Ch.90 S. 24 E Negligent Operation of a Motor Vehicle and one count of violating MGL Ch. 89 S. 4A Marked Lanes Violation via MA Criminal Citations which I placed in the mail. Additionally, I will be seeking a subpoena for Topham's blood work from the hospital. Note: The crash occurred on the Newton/Boston line, Newton dispatch's GPS indicated that the crash was in Newton. I took several pictures of the scene and have forwarded them to IT to be attached to this report. I completed and filled an NPD Motor Vehicle Tow Inventory Form and Officer Boudrot stood by for the tow.

On 04/08/2019, at 1620 hrs, I responded to the area of 2040 Commonwealth Ave for a three car MVA with injuries. Upon arrival, I observed all three vehicles facing westbound on Commonwealth Ave. MV1 had heavy front end damage and airbag deployment. MV2 also had heavy front end damage and airbag deployment. MV3 had minor rear end damage. The operator of MV1 was brought to Newton Wellesley Hospital by Cataldo medics. The operator of MV2 stated she was on Pamela Ct attempting to take a left onto Commonwealth Ave. There was heavy traffic headed westbound and a vehicle stopped to let MV2 go. As MV2 was taking a left onto Commonwealth Ave, MV1 was traveling westbound in the eastbound lane around traffic and struck MV2. MV2 then in turn struck MV3s rear end. The operator of MV3 stated that he wasn't sure what occurred because the accident was behind him. The operator of MV1 stated she was traveling westbound on Commonwealth Ave and was planning on taking a left into the Star Market parking lot which was approximately 250 feet down the street. She stated she thought there were two lanes in the westbound direction at that point in the roadway. MV2 then pulled out in front of her and they collided. Two witnesses who were traveling westbound in traffic a few vehicles behind MV3 both stated that MV1 drove erratically into the oncoming lane and around traffic heading westbound. One witness stated "the car was almost completely in the opposite lane" and the other witness agreed. A moment later they stated they heard the crash but did not see it. They also stated that she almost crashed into a car turning eastbound out of the Star Market exit prior to the accident. MV1 and MV2 were towed from the scene by Tody's. The operator of MV2 was brought to Newton Wellesley Hospital by her mother. The operator of MV1 was cited for C90 S23 operating after revoked license, C90 S23 operating after revoked registration, C90 S24 operating to endanger, and C89 S4a marked lanes violation. All aforementioned streets are public ways maintained by the City of Newton.

On 06/08/2019 at 0150 hours I received a call from Sgt Lee in regard to a head on motor vehicle crash with entrapment that had occurred on Center St at the four way intersection with Cabot St and Sergeant St. Upon my arrival at the crash scene I was advised by Sgt Lee that all occupants of the involved vehicles had been transported to hospitals for treatment. I made the following observations of the crash scene , Center St is a two lane public way in the city of Newton that runs north to south. I observed a city of Newton contracted Cataldo ambulance stopped in the south bound lane of Center St at Cabot St. The ambulance was MA AMN registration AMB 370 a 2017 Ford Econo Line that had heavy damage to the front end and driver's side fender. I observed that the front tire of the ambulance was snapped off and was pushed back towards the driver's side door. I further observed that both of the front and side air bags of the ambulance were deployed. The front of the ambulance's final point of rest was 8'8" from a fixed overhead light pole on the northeast corner of Center St at Cabot St. The ambulance's front passenger side tire was 4'3" front the curb on the Cabot St side. The ambulance's rear passenger side tire was 2'3" from the Cabot St side curb. The ambulance's rear driver side tire was 5'1" from the double yellow line on Center St. The ambulance's front driver side tire was 6'3"from the Center St double yellow line. The width of Center St from the double yellow line to the Center St curb is 15'' and 16'" to the Sergeant St side. I observed a 2011 Mazda 3 with MA PAN registration 6KJ783 facing south bound in the north bound lane. The Mazda had heavy damage to it's front end, and I observed that it's front tires had snapped off. The Mazda's roof had been removed by Newton Fire to extract the occupant. The front of the Mazda's final point of rest was 40'3" from a fixed overhead light pole on the northeast corner of Center St and Sergeant St. The Mazda's front passenger tire was 4'6" from the double yellow line on Center St. The Mazda's rear passenger side tire was 7'6" from the Center St double yellow line. The Mazda's rear passenger side tire was 1'2" from the Center St double yellow line. The Mazda's front driver's side tire was on the curb on the Sergeant St side of Center St. There were no skid marks from the Mazda which indicates that no braking occurred. At the time of the crash, the Cataldo Ambulance was being operated by Paramedic Corey Bohan and Paramedic Tom White was in the front passenger seat. Bohan stated that he was operating his vehicle southbound on Center St towards Cabot St after clearing a medical call. There were no patients in the ambulance at the time of the crash, and Bohan estimated he was travelling 25 MPH . Bohan stated within less than two seconds he observed a vehicle traveling towards him northbound on Center St in the southbound lane and estimated it's speed to be approximately 80 to 100 MPH.Bohan stated he was able to slow to a near stop and had moved his vehicle towards the right shoulder on Center St before the Mazda struck his vehicle head on. Bohan stated that after forcing his way out of the driver's seat of his vehicle, he rendered medical aid to the operator and only occupant of the Mazda, Jessica Pondish. Bohan stated that Pondish speech was slurred and he detected an odor of an alcoholic beverage emanating from her breath and person. Bohan was later transported to Newton Wellesley Hospital for evaluation. The passenger in the ambulance, Tom White stated to officers that he was working on the ambulance's laptop computer and did not see anything leading up to the impact from the crash. White stated that he was able to force himself free of the passenger side of the ambulance, and rendered medical aid to Pondish. White stated that Pondish was " in and out of it " and did not appear coherent.White stated that he asked Pondish if she had consumed any alcoholic beverage and she nodded in the affirmative. White Stated that he was unsure if Pondish understood his questions. White stated that he did not detect any odor of an alcohol beverage emanating from Pondish's breath or person. White was transported to Newton Wellesley Hospital for evaluation. Pondish was freed from her vehicle after approximately 22 minutes by Newton Fire Department units by utilizing special extraction tools. Pondish was transported by Newton Medic Unit Two to the Brigham and Women's Hospital in Boston for treatment. From 06/08/2019 to 06/10/2019 Pondish had two surgeries and was unable to speak due to her condition. On 06/10/2019 myself and Officer Gaudet were able to speak with Pondish in her hospital ICU room at the Brigham and Women's Hospital. Pondish's parents Lorena and John Pondish were present while we spoke with her. Officer Gaudet asked Pondish about the events that led up to the crash. Pondish stated that on 06/07/2019 she was at a friend's residence in Norton, MA. Pondish stated that while at her friend's residence she consumed a bottle and a half of Riesling wine. After Pondish's admission myself and Officer Gaudet stopped our conversation with her. I read Pondish her rights directly from a Miranda card and explained them in full before allowing her to continue speaking with myself and Officer Gaudet about the events of that led to the crash. Pondish stated that she understood her rights. Officer Gaudet asked Pondish if she had consumed any alcoholic beverages prior to driving home to her residence in Newton. Pondish stated that she drank a bottle and a half of Riesling wine by herself between the hours of 1700 and 0000 hours on 06/08/2019. Officer Gaudet asked Pondish how large the bottles of wine was that she drank, and she stated that they were average size. Officer Gaudet asked Pondish how she perceived her state of being was after consuming a bottle and a half of wine. Pondish stated that she felt " buzzed " . Pondish stated that she left her friend's residence in Norton at approximately 0100 hours on 06/08/2019 to return to her residence in Newton. Pondish stated that she does recall the events leading up to the crash, except that she tried to make a phone call but the person did not pick up. Pondish stated that her seat belt was on and that she was not texting while operating her vehicle. Pondish stated that her injuries consisted of a broken left elbow and internal injures to her intestines. On 06/09/2019 I spoke to Cataldo Paramedic Robert Canning and Stephen Heirholcer who were the medic unit that transported Pondish to the Brigham and Women's Hospital. Canning stated that he rendered medical aid to Pondish for approximately 20 minutes while Fire units worked to free her. Canning stated that Pondish's legs were pinned under her vehicle's dashboard. Canning stated that during the approximately 20 minutes that he was in close proximity to Pondish he could detect a strong odor of an alcoholic beverage that was emanating from her breath and person. Canning stated that once Pondish was freed he continued to treat and monitor her during the transport to the Brigham and Women's Hospital. Canning stated that when he asked Pondish if she had consumed any alcoholic beverage she stated that she had. Paramedic Heirholcer stated that he did not detect any odor of an alcoholic beverage emanating from Pondish's breath or person. Based upon the physical evidence at the crash scene, and statements made to officers by all of the above mentioned persons, Pondish is being charged with violation of MGLs 90/24j OUI liquor, 90/24e negligent operation of a motor vehicle, 89/4a failure to stay within marks lanes, and 90/17 speeding. On 06/12/2019 at the conclusion of the crash investigation I issued MA Criminal Application T 1269996 in hand to Pondish at her hospital room and explained to her what action she needed to take with court. On 06/11/2019 a letter of preservation was sent to Brigham and Women's Hospital requesting that a sample of Pondish's blood, urine and medical records be saved. On 06/12/2019 Brigham and Women's Hospital confirmed that a sample of Pondish's blood had been preserved. As of 06/12/2019 a search warrant for Pondish's blood sample was pending. Photos of the involved vehicles and of the crash scene were taken and downloaded by the NPD's IT Bureau. Both involved vehicles were towed from the crash scene and towed motor vehicle forms were filled out. An Iphone and backpack belonging to Pondish were taken from her vehicle for safekeeping and returned to her on 06/11/2019. A copy of this report was faxed to the MA RMV Driver Control Unit to request an immediate threat suspension.

At approximately 1825HRs on August 06, 2019, I was dispatched to 785 Centre St. (Newton Country Day School) in reference to a motor vehicle vs bicycle crash. Upon arrival, I located both parties on the sidewalk adjacent to the intersection of Centre St. and Colby Rd. The operator of the bicycle was complaining of feeling dizzy, although she stated she did not hit her head, and suffered several abrasions on her left leg and hands; the operator of MV1 stated she was uninjured. The operator of MV1 stated that as she was traveling northbound on Centre St., she attempted to give the operator of the bicycle room, but the bicyclist made an abrupt left hand turn and struck MV1. The operator of the bicycle stated she was traveling northbound on Centre St. in the Bicycle Lane. She then signaled a left hand turn and entered the Motor Vehicle Travel Lane to make a second left hand turn to travel eastbound on Colby Rd. Once she exited the northbound Motor Vehicle Travel Lane on Centre St. and began to cross over the southbound lane, MV 1 crossed over the solid yellow lines onto the southbound side of Centre St. and struck her bicycle. MV1 sustained damage to its right front bumper and I noted red paint transfer of the left side of the front bumper, which the operator of MV1 states was old damage. MV1 also sustained a flat left front tire after contacting the curb. The bicycle sustained damage to both tires and where the rear tire attaches to the frame appeared to be warped. I took several photos of the damage and have submitted them to be attached to this report. The operator of the bicycle was transported to the hospital by Newton Medics and the operator of MV1 signed a patient refusal. Todys Services arrived and took custody of MV1, for which I completed and filled an NPD Towed Motor Vehicle Form. All streets mentioned are public ways in the City of Newton. Due to the potential danger of serious bodily injury caused by the operator of MV1's reckless driving behavior (crossing the solid yellow road divider line and driving on the wrong side of the road against traffic), the actual damage caused (striking and causing the bicycle operator to be transported to the hospital and damage to the bicycle) and that the bicycle operator was clearly visible/in clear contrast with the road (riding a red bicycle and wearing a red shirt) I issued the operator of MV1 MA Criminal citation T1445010, in hand, for violating one count of MGL Ch.90 S. 24 (Operating To Endanger) I cleared without incident. Note: I logged the bicycle into property/evidence for safekeeping, as it could not be taken on the ambulance.

MV1 jumped a curb and collided with a parked vehicle MV2 (a 2004 GMC Yukon, MA Reg: 8EC792) in front of 185 Waltham St. MV1 operator, Angel Gonzales, was unconscious on scene in the driveway of 185 Waltham St. Waltham St is a public way that is owned and maintained by the City of Newton. A passerby, Joseph Anderson saw the aftermath of the crash and called 911. MV2 had damage to its drivers side door and rear side. MV1 was heavily damaged, on its side, and keys were in the ignition. MV1 was towed by towed by Tody's. Gonzales was transported to Beth Isreal in critical condition. When Gonazales was in the back of the ambulance, myself and medics smelled an odor of an alcoholic beverage emanating from Gonzales. All first responders thoroughly checked the surrounding area for a helmet but one could not be located. Due to Gonzales' critical condition and urgency to get to the hospital, and ID was not thoroughly looked for on his person. Ofc. Guarino, N491, retrieved an ID from Gonzales upon arrival at Beth Isreal Hospital. Ofc. Stake filled out a towed inventory form. Pictures taken and sent to IT. Sgt. Lee was notified and will be following up with this incident. MV1 operator, Angel Gonzales is being charged with the following (MA Uniform Citations T1442823 & T1442824): 90/24J - OUI Liquor (public way) 90/24E - Negligent Operation of a MV (operating recklessly at a high rate of speed) 89/4A - Marked Lanes 90/23 - Attaching Registration Plates (attaching registration plates to motorcycle to conceal the fact that it was unregistered. Registration plate on the motorcycle came back to a 2002 Honda VT750 motorcycle, MA Reg: 1T3988; VIN on the motorcycle came back to a 2005 Suzuki GSXR60, MA Reg: 1L4389) 90/9B - Unregistered MV (motorcycle 2005 Suzuki MA Reg: 1T3988 was confirmed unregistered by RMV) 90/23D - Operating MV with a Suspended License (RMV confirmed that Gonzales' license was suspended S94722414) (see Incident #19039806 for more details). For QED purposes, the charge 266/139 - Stolen Registration was added to this report because 90/23 - Attaching Registration Plates was not an option. Due to Gonzales' hospitalization, the citations will be mailed to his last known address. Officers were able to rise a Harold Curtis, the father of Anthony Curtis (owner of MV2, the GMC Yukon) at 185 Waltham St. He was notified of the incident and advised to call their insurance company. Curtis mentioned that he has cameras facing the street where the incident took place and will see if he can retrieve the footage for us. Police prosecutor to subpoena blood.

The operator of MV1 stated that he was texting and traveling at a high rate of speed when his vehicle lost control and crashed into parked vehicles 2 and 3. All three vehicles sustained major damage and were towed from the scene by Todys. The operator of MV1 had no apparent injuries but due to the high rate of speed of the vehicle at the time of the crash was transported to Newton Wellesley Hospital for precautionary reasons. Photographs of the crash as well as the open container of "Chum Churum" alcoholic beverage were taken and placed in IT's mailbox.

Operator of MV1 states she had the green signal when MV2, described as a dark colored pick up truck, ran the red light traveling northbound from the off ramp of Rt. 95 southbound/Quinobequin Rd. and crashed into MV1 head on. Operator of MV1 states MV2 fled the scene traveling northbound in the Do Not Enter lane traveling towards Grove St. Operator of MV1 states MV2 sustained front end damage, while MV1 sustained damage to the right front fender and was towed from the scene. Operator of MV1 sustained a wrist injury and was transported to Newton Wellesley Hospital. While investigating the scene, the license plate (MA Reg: 1YF845) from MV2 was left behind from the crash. The registration plate came back on a gray 2017 Honda Ridgleline pick up truck, which matches the description of the vehicle involved in the crash. The vehicle is registered to Andrew Wiggin out of 32 Marlborough Rd Salem, MA. Two witnesses, Lisa Duncan and Jennifer McQuillan, witnessed the crash. Ms. McQuillan states she was behind MV1 when she observed MV2 run the red light, crash into MV1 head on and fled the scene northbound in the Do Not Enter lane. Ms. Duncan states she was a few car lengths behind MV1 traveling on the right hand side of MV1. Ms. Duncan also states she observed MV2 run the red light, crash into MV1 head on and fled the scene northbound in the Do Not Enter lane. Both Ms. McQuillan and Ms. Duncan believe a male was operating the vehicle.

The operator of MV1, Nicole Laurin, sates that she was driving southbound on Sheridan Street when she lost control of her vehicle causing her to crash into MV2 which was parked in front of 107 Sheridan Street. Both vehicles sustained major damage and airbag deployment and were towed from the scene by Tody's. Due to the high rate of speed of the crash and air bag deployment, the operator of MV1 was transported to Newton Wellesley Hospital to be evaluated. It should be noted that prior to the reported crash there was a 911 call regarding a white sedan traveling at a high rate of speed going through numerous traffic signals and signs in a densely populated neighborhood with narrow streets. After the crash several more 911 calls were received stating that a vehicle traveling at an excessive rate of speed just crashed into a parked vehicle. Based on these calls and the nature of the crash, Ms. Nicole Laurin was issued in hand Massachusetts Unifrom Citation T2013231 for 90-24, Operating to Endanger, and 90-17, Speeding over the reasonable and proper rate. Sheridan Street is a public way in the city of Newton.

It should be noted MV1 failed to stop for police previously in a motor vehicle pursuit. MV1 sustained heavy front end damage. Operator sustained minor injuries and was transported to Newton Wellesley Hospital on a section 12. The vehicle was then towed by Tody's. A motor vehicle inventory form was filled out in regards. A message was left for the city to repair the street sign. Pictures were taken and placed into property and evidence. It should be noted a fence had to be removed at 4 Meredith Ave in order to removed the vehicle. I was able to get permission by the owner of the home to remove the fence.

On Friday, September 18, 2020, at 88update88approximately 0657 hrs, while assigned to N495, I was dispatched to the area of 1221 Centre St for a report of a MV accident. When I arrived on scene I observed a Motor Cycle (MC) laying on its side in the middle of the street. I observed a party laying on the ground, while medical personnel were tending to him. I also observed a white Honda (MV#1) pulled off to the side of Pelham St, and the operator standing next to his vehicle. Operator of MV#1 stated that as he was attempting to make a left turn off of Centre St, onto Pelham St. Operator of MV#1 stated that while he was in the middle of his turn, he observed a MC driving at a high rate of speed coming towards him. Operator of MV#1 stated that he was not able to get out of the way of the MC before it crashed into him I was not able to speak with the operator of the MC, due to him being transported to Brigham and Womens Hospital. Ofc McSweeney stated that he will be following up with the operator of the MC and will advise me. It should be noted that the operator of the MC appeared to be wearing headphones while operating, picture was taken and submitted. An anonymous party stated to me that he observed the MV traveling at a high rate of speed, heading North on Centre St. The party stated that he observed the MC weaving in and out of traffic. Multiple pictures were taken of MV#1 and MC and were submitted to IT to be attached to this report. Operator of MC was transported to Brigham and Womens. Operator of MV#1 was cleared with a patient refusal. Both vehicles were towed by Todys. **UPDATE** I spoke with the father of Scott Mahon, Keith Mahon. Keith stated that his son is still in the hospital, and has received several surgeries. Keith stated that the injuries his son received are a Open Tibia/Fibula fracture of the right leg, and a concussion. Keith stated that Scott has no recollection of the incident, as far as to not remembering leaving the house that morning.

On 09/30/2020 at approximately 19:32hrs members of the Newton Police Patrol Bureau responded to a motor vehicle crash near the intersection of Washington St and Eddy St. After an initial investigation of the crash the Patrol Bureau secured the crash scene and called in the Newton Police Traffic Bureau to investigate. At approximately 19:45hrs Sgt. Devine, Ofc Wilson and I (Ofc Schlegel) from the NPD Traffic Bureau responded to the scene to investigate. Upon arrival to the scene I was met by Ofc Kane of the NPD Patrol Bureau. Ofc Kane stated that the crash was located in the driveway of 74 Eddy St and that it involved a Ford F350 pickup striking two parked cars. Ofc Kane stated that the operator identified as Kyle Butler had already been taken to Newton Wellesley Hospital by Cataldo EMS for injuries. . Upon approaching the crash scene I observed the following: The crash occurred at night time hours at approximately 19:32EST near the intersection of Washington St and Eddy St. Both Washington St and Eddy St are public ways in the City of Newton. The weather conditions at the time of the crash were a dark and clear with a temperature of approximately 60 degrees F. Washington St is a four lane undivided roadway (2 westbound travel lanes and 2 eastbound travel lanes). Washington St lighted roadway which has concrete light poles on both the westbound and eastbound sides. Washington St was under construction at the time and had a base coat of asphalt applied. At the time of the crash Washington St did not have any painted lane markings due to ongoing construction. Washington St is separated from the road sides on both the eastbound edge and the westbound edge with fixed granite curbing. The roadway also had several orange traffic barrels and traffic cones located edges of the eastbound and westbound lanes. Affixed to the light poles and the traffic barrels were temporary no parking signs for the ongoing road construction. At the time of the crash no road construction activities were taking place. No major debris or other obstructions were observed in the roadway. Washington St is a major roadway in the City of Newton and travels west to East across the entire City of Newton. This area of Washington St where the crash occurred is in between West Newton Square and Newtonville Square, this area has a posted 35 MPH speed limit. Both West Newton Square and Newtonville Square are densely populated with several businesses and residential areas. The section of Washington St where the crash occurred is also traveled frequently by pedestrian, bicycle and motor vehicle traffic. At the time the crash occurred 19:32hrs This area of Washington St is still a heavily travelled roadway. I observed Vehicle 1 described as a 2003 Ford F350 color white (MA Reg 5DT664) located in the driveway of 74 Eddy St. 74 Eddy St is a multi-family dwelling house located on the corner of Washington St and Eddy St. The driveway of 74 Eddy St is on the Washington St side. Vehicle was faced easterly across the driveway. I observed Vehicle 1to have extensive damage to the entire vehicle including but not limited to the enclosed cab area, cab roof and all windows, front end, rear end, along with both driver and passenger side. I also observed city of Newton owned concrete traffic light resting on top of the cab area of Vehicle 1. Upon further observation of Vehicle 1 it appears that both front tires and rims were flat, heavily damaged and had little to no tread left. Both front tires were worn down to the interior steel belt. I also observed that Vehicle 1 did not have a valid inspection sticker. Located to the immediate front of Vehicle 1 and resting on the front end was a white fence which had damage consistent with the impact of a motor vehicle. Immediately to the driver's side of Vehicle 1 was Vehicle 2 described as a 2007 gray Subaru Forester (MA Reg 45D610. Vehicle 2 was facing westerly across the driveway. Vehicle 2 had extensive damage to the driver's side front end, rear end, in addition to the entire driver and passenger sides. At this time Vehicle 2's drivers door was in contact with Vehicle 1's drivers door. Both drivers doors could not be opened.The damage to Vehicle 2 appeared to be consistent with impact from another motor vehicle. To immediate passenger side of the Vehicle 2 I observed Vehicle 3 described as a 2011 Toyota Rav4 color black (MA Reg 47XB99). Vehicle was facing in an easterly direction across the driveway. I observed the Vehicle 3 to have extensive damage to driver's side rear quarter panel, passenger side front quarter panel and front end. Also observed was damage to south east corner of the dwelling of 74 Eddy St. This area of the dwelling is located adjacent to the drive way. The damage appeared consistent with the impact caused by a motor vehicle. Also observed were large amounts of debris located in the area of the crash which included the sidewalk, yard and the driveway of 74 Eddy St. The debris included pieces of concrete which appeared to have been dislodged from the concrete light pole. A city of Newton no parking sign was lying in the yard of 74 Eddy St. This parking sign would have been attached to the concrete light pole due to ongoing road construction on Washington St. I also observed two tire marks on a curb stone located next the westbound lane of Washington St. These tire marks were in front of the residence of 74 Eddy St and were to the immediate left of a storm drain. The tire marks were consistent with being struck by a motor vehicle. In between the tire marks and just after the curb there was a hole in the ground with wires sticking out of it. This hole appears to be the location the concrete light pole was located prior to impact. Tire marks were located on Washington St traveling from the east bound lane easterly across the westbound lane and ending at the tire marks on the curb. The total length of the tire marks was approximately 190 feet. After conducting an on scene investigation it appears that Vehicle 1 from the start of the tire mark in eastbound travel lane of Washington St traveled 190 feet in the easterly direct across the westbound lane of Washington St and impacted the curb in front of 74 Eddy St. The tire marks had lateral striations almost the entire length indicating that the front tires were rolling and sliding with little to no braking prior to impact with the curb. After impact with the curb Vehicle 1 exited the roadway, struck a concrete light pole. Vehicle 1 then traveled over a sidewalk, across the yard of 74 Eddy St and into the driveway of 74 Eddy St. Located in the driveway of 74 Eddy St were Vehicle 2 and Vehicle 3. Vehicle 2 was parked facing toward the rear of 74 Eddy St. Vehicle 3 located to the passenger side of Vehicle 2 and was parked facing towards Washington St. When Vehicle 1 entered the driveway it struck Vehicle 2. The force of the impact of Vehicle 1 striking Vehicle 2 caused Vehicle 2 to strike Vehicle 3. The force of the impact of Vehicle 2 striking Vehicle 3 caused Vehicle 3 to strike the southeast corner of the dwelling located at 74 Elliot St. Vehicle 1 continued across the drive way and struck a white fence before coming to final rest. Vehicle traveled a approximately 55 feet from when it initially struck to the curb till final rest. No other injuries were reported on scene. All involved vehicles were towed from the scene by Tody's Towing. After the investigation the operator of Vehicle 1 Kyle Butler was issued Citations T2015882 and T2015883 for: Mgl 90/24/O Recklass Operation of a Motor Vehicle Mgl 89/4B/A Failure to Operate in the Right Lane Mgl 90/20 Failure to Submit a Motor Vehicle for Inspection Mgl 90/7 Defective Equipment (worn front tire treads) Mgl 90/13A Failure to wear a seatbelt An immediate threat form was also filed with the MA RMV. Operator of MV1, Emily Stoller, states she was operating MV1 in the parking lot at 200 Boylston Street when her friend, Alvin Day, asked her if he could drive MV1, which she gave him permission to do and was under the impression Mr. Day had a valid driver's license. It should be noted this area is regularly used to get to commercial businesses and has the right of access to the public. Ms. Stoller states Mr. Day was only driving about 15mph when he lost control of the vehicle and crashed head on into a light pole. MV1 sustained heavy front end damage and was towed from the scene by Tody's towing. Mr. Day states he does not have a valid driver's license and asked Ms. Stoller if he could drive MV1. Mr. Day states he was "goofing off" driving around the parking lot at approximately 15mph when he "cut the steering wheel too much", lost control of the vehicle and crashed MV1 head on into the light pole causing the driver's side airbag to deploy. Mr. Day states he lost consciousness for a brief moment where he was assisted out of the vehicle by the rear passenger, Alex Kakooza. Mr. Day was evaluated by the medics and transported to Children's Hospital. Ms. Stoller and Mr. Kakooza were also evaluated by the medics where they obtained patient refusals. All three juveniles had an adult family member respond to the scene. Ms. Stoller's father, Gregory Stoller, Mr. Day's mother, Quonekuia Day and Mr. Kakooza's sister, Latifah Kakooza. Professional electrical contractors responded for the damaged light pole and mall security will be notifying the property owner.

On 10/22/2021, at approx 0752Hrs, while assigned to 497, I responded to 2071 Washington St for a report of a past hit and run with property damage. Upon arrival I observed the tire tracks leading from the right westbound lane leading over the curb , knocking over a chain link fence and post . There were tire ruts into the side yard of 2071 Washington St, an there was a End of Reduced Salt sign and post wedged into the ground of the homeowners side yard. I spoke with the property owner , Ali Shamser who stated at approx midnight he heard a loud bang, and looked out his window , saw nothing in the road but did not think to look down into his side yard. Ali's home is across the street from Newton Wellesley Hospital and next to Route 128 as well as on a very busy road so he is used to hearing crashes and sirens regularly. Ali stated at approx 0745Hrs when he left his home to take his children to school he noticed his damaged fence and yard. Ali has no cameras, across the street is Newton Wellesley Hospital and his neighbor adjacent on Walsingham St has his property surrounded by a 6 foot vinyl fence.

At approximately 0922HRs on Friday, October 29, 2021, I was dispatched to 265 Washington St. for a motor vehicle crash with minor injury. Upon arrival, I observed MA Pass: 1ZCM56 parked almost in the on-ramp to I90, causing a potential safety hazard for drivers attempting to enter/exit the highway. The vehicle's driver's side headlight was broken, its front bumper/driver's side fender was partially dislodged and there were scratches and dents in the general area of impact. Parked in front was MA Pass: 2CHV64, which displayed a large scrape on its passenger's side rear bumper, gouges missing from around the wheel well and scratches through the general area of impact. Both vehicles were owned by the operators. The first of which being operated by Marineyde Oliveira (OLN: no status/NRE) and the second of which being operated by Brittany Rivera (MA OLN: S95463618). With the translation assistance of Officer Calderon, I spoke with Marineyde, who stated she was traveling westbound on Washington St., by the entrance to I90. At this time, she attempted to change one lane to the left, when she struck Brittany's vehicle in the rear. Upon requesting her driver's license, Marineyde stated she did not have one.. Upon speaking to Brittany, she stated she was traveling westbound on Washington St., in the lane closest to the concrete island, just before the entrance of I90. It was at this time that Marineyde entered her lane, striking her vehicle in the rear. Brittany stated she then pulled over and exited her vehicle, past the on-ramp and it appeared Marineyde initially was going to drive away without stopping. Brittany then began to signal her to stop by waving both of her arms and Marineyde parked behind her. Brittany, a Spanish speaker herself, asked Marineyde for her driver's license in Spanish, after she watched Marineyde exit the driver's seat, but Brittany was told she did not have one. Because Brittany was concerned that Marineyde initially appeared as if she was going to hit-and-run and because Marineyde did not have a Driver's License, she elected to call The Newton Police. It should be noted that Brittany was complaining of neck pain and stated she would drive herself to Newton-Wellesley Hospital; she signed a patient refusal. Due to Marineyde admitting to me that she was driving without a license, Brittany witnessing said illegal operation, the vehicle being registered to Marineyde and no other party being in her vehicle, I initially placed her under arrest for violating one count of MGL Ch. 90 S. 10 (Operating With Out A License). She was handcuffed behind her back, double locked and placed in the cruiser for transport. Officer Calderon began to transport her to Newton Police Headquarters. However, after discussing the matter with Sergeant Poutas, we came to the belief that the "in the officer's presence" element of the crime would likely not be satisfied by the above, so Marineyde was released forthwith. She will be receiving MA Criminal Citation: T1445781 in the mail for the above offense, as she was already released from the station prior to my arrival. Officer Calderon explained everything pertaining to the initial arrest and that she will be receiving the criminal citation in the mail. This rotary/intersection is a particularly dangerous area, in which we respond to many crash reports. The area where MA Pass: 1ZCM56 was parked, was creating a potential hazard for motorists entering the highway, as well as the many motorists who often exit the ramp, where she was parked, to avoid accidentally entering the highway. Because of this, Todys Services removed the vehicle. Note: Washington St. is A Public Way in The City of Newton. I completed and filed an NPD Towed Motor Vehicle Form and cleared without further incident.

Opr of MV1 stated he was traveling south on Chestnut Street. At this time, he drove through a red light, attempted to step on the brake but could not slow down, and drove into a front yard. Opr of MV1 stated he was not speeding and did not see anyone else involved. MV1 was towed. MV1 suffered injuries and was transported to Brigham and Woman's Hospital. Opr of MV2 was visibly upset and shaking. Opr of MV2 stated she was waiting for the light to turn green at the intersection of Berkeley Street and Chestnut Street. Once the light turned green she attempted to drive straight. At this time, she saw MV1 traveling at a high rate of speed, travel through the red light, swerve, hit her car, then hit one utility pole, then hit a second utility pole, split in half and land on the front yard (260 Chestnut Street). Opr of MV2 suffered minor damage and was not injured. After further investigation Opr of MV1 was cited (See Incident Report# 21048544) Officer Wilson took pictures.

On November 21st, 2021 at approximately 07:56 hours while working N491 I along with Officer Mike Boudreau responded with NFD and Medic One to a report of a MV crash with injuries in the area of Centre St @ Franklin St. On my arrival the Medics were evaluating two adult males who standing outside one of the involved vehicle from this crash. There appeared to be several involved vehicles to this crash however two of them had been parked prior to the crash. The vehicle which the two unidentified males were standing by was MA reg. 9HY939, a 1988 Black Ford F150 PU, listed registered owner was a Peter J. Jara-Castro, #28 Ridge Ave, Lynn Ma. Mr Castro was one of the two males standing by that truck. The above vehicle appeared to have been travelling N/B on Centre St when it skidded off the road, jumping the curb striking a 1 foot slate retaining wall to the property to #573 Centre St. It continued onto that property driving over shrubs, small trees, then onto the driveway(same address) which is actually on Newtonville Ave, hitting the homeowners 2007 Nissan Versa, Ma reg. 6BH269, forcing it into more shrubbery. The Truck continued across Newtonville Ave where it struck another parked vehicle, a Grey 2019 Volkswagon, Ma reg. 1NFC27 on the rear passenger side pushing it forward. The truck finally came to a stop after driving into a wooden fence to the property of #559 Centre St.(Fence is on Newtonville Ave.) I spoke with the owner of the truck after he was evaluated by the Medics, who spoke limited English. The owner of the truck Peter Jara-Castro finally said he was driving the truck prior to the crash. His statement was he was travelling N/B down Centre St when another vehicle came out from Newtonville Ave causing him to slam on his brakes. Mr Castro doesn't remember much after that, possibly from hitting his head. At this time I couldn't determine who the actual driver was. Both Mr Castro and the passenger, sustained injuries but both declined to go to the hospital. Both were later picked up by an UBER driver. A Firefighter heading to work reported seeing the truck pass him on Centre St N/B at high rate of speed but never saw the driver or the actual crash. Both property owners that sustained damage were given Trucks insurance info. Pictures were taken by both property owners. Vehicle #1 and vehicle #2 were towed from scene. Both sustained heavy damage. Vehicle #3 sustained minor damage I was unable to make contact with that owner at this time. However later in the day I was able to make contact with owner of vehicle #3. She was given all vehicle #1 information along with his insurance. A citation for speeding was later issued after investigation was completed and witness statement. Citation#t1444076 was later sent VIA mail.

On 01/03/22 at approximately 1617 hours Officer Robert March and I, Officer Whitney Hyde, were assigned to N499 when we were dispatched to a 2 car MVA at 160 Needham St with no injuries. Both vehicles were waiting in the parking lot. On location I made contact with the operator of Vehicle 1, identified as Samantha LERNER. LERNER stated that the had been travelling southbound on Needham St and stopped in the center left turn lane to turn into the Farm Grill. When it was clear to turn, LERNER began to turn left when Vehicle 2 passed her on the left and made contact with the driver side front quarter panel of Vehicle 1. I observed scrapes and minor damage to Vehicle 1's front bumper and driver side front wheel rim. LERNER stated that the operator of Vehicle 2, later identified as Leanna LANDE, exited her vehicle at the time of the accident and exclaimed that her dog was dying and she was headed to the animal hospital down the road and they could exchange information there. LERNER proceeded to the animal hospital and was waiting for LANDE to come back out. I then entered the animal hospital and made contact with LANDE. LANDE was extremely upset as her 7 week old puppy had just died. LANDE stated that she had been in a panic because of her dying dog and passed LERNER when she shouldn't have, stating it was "stupid." Outside of the animal hospital I observed Vehicle 2 had minor damage along the length of the passenger side, including scrapes with white paint transfer. I issued LANDE MA Uniform Citation #T1448086 in hand for 89/4A Marked Lanes violation and explained the payment and appeal process. LANDE stated she understood. I provided LANDE and LERNER with the accident number and facilitated their exchange of information before clearing without further incident.

On Tuesday, February 15th, 2022 at approximately 1908 hours the operator of Vehicle 1 was travelling Westbound on Auburn St when it crossed over the center double yellow lines, crossed the opposing lane of traffic, and went off the roadway before crashing through the vinyl privacy fence at #106 Auburn St. Vehicle 1 came to rest after colliding with the North-East corner of the residence. The operator of Vehicle 1 then attempted to reverse away from the crash and backed into and destroying more of the residence's privacy fence. The operator was then confronted by the occupants of the residence, including the homeowner. Fire and EMS arrived on scene and conducted patient care on the operator and formed the opinion that she was intoxicated. I arrived and interviewed the operator who stated that she had previously taken oxycodone. The operator was then transported to Newton Wellesley Hospital. The vehicle was towed from the scene by Tody's. The operator was charged with the following violations: C90ss24 OUI- Drugs C90ss24 Neg Op C89ss4A Marked Lanes An incident report was completed for this crash (See Report #22005833).

MV 1 was backing out of her parking spot when she backed onto the sidewalk and into a parking meter. On 4/21/22, I was dispatched to the intersection of Crafts St./California St. for a motor vehicle collision involving a single vehicle into a tree. Upon arrival, I observed MV#1 had crashed into a tree in front of 315 Craft St. The operator stated the "road was slippery" and he veered off the roadway, hitting the curb, before traveling 20-25 yards on the sidewalk, hitting two tree's. MV#1 sustained heavy front end damage and air bag deployment consistent with colliding into a tree at a decent rate of speed. MV#1 was removed from the roadway by Tody's Tow Service. The operator of Mv#1 was transported to Newton-Wellesley Hospital with injuries. Charges pending at this time for OUI Liquor 2nd Offense, Marked Lanes Violation and Negligent Operation of a Motor Vehicle.

Narrative:
On Tuesday, June 28, 2022, while assigned to unit N496, I responded to the area of Langley Road and Hamlet Street, Newton for a report of a roll over crash with 3 injured parties. The weather at the time of this crash was clear and sunny. The road surface in the area of the crash was dry. Langley Road and Hamlet Street are both public ways maintained by the City of Newton. I approached the area of the crash coming from Boylston Street and when I arrived my attention was drawn to two locations on Langley Road. I observed a female party laying prone on the sidewalk area of Langley Road (E) at the Hamlet Street intersection. This female party, later identified as Ms. Melinda Conroy, was being attended to by passerby's in the area. My attention was then drawn to an overturned vehicle resting half in the roadway/half on the sidewalk (W) in front of 323 Langley Road. As I began to run over to the vehicle, I observed a woman standing next to the vehicle screaming frantically. This party identified herself as the driver of the lone vehicle involved, Ms. Natalya Kabanovsky (S65512051). There were parties at the back of the vehicle yelling to me that there was a woman trapped under the vehicle and that they were holding the vehicle up to get her out. I ran over to the rear of the vehicle and observed a female party, later identified as Ms. Annette Sooper, lying underneath the vehicle in the roadway. There was a large pool of blood on the roadway as the result of a laceration to what appeared to be her head/face area. Due to the unknown stability of the vehicle resting on its roof, and due to the passerby's stating they were holding the vehicle up so it did not roll over onto Ms. Sooper, I reached under the vehicle from the rear, grabbed Ms. Sooper's person, and dragged her away from the vehicle to a safe location on the sidewalk. At this time, I observed a large laceration to Ms. Sooper's head and she stated she had suffered a leg injury. Newton Fire and Medics along with additional Police units arrived on scene and took over rendering aid to the victims. I then started to conduct my investigation into the crash. I spoke with the operator of the vehicle, Ms. Kabanovsky. Ms. Kabanovsky stated she was operating her 2005 Lexus ES330 (MA: 293TAB) on Hamlet Street (W) towards Langley Road. Ms. Kabanovsky stated as she approached the intersection, a female party (Ms. Conroy) ahead of her was running across Langley Road (S) at Hamlet Street. Ms. Kabanovsky stated she attempted to hit her brakes, but instead hit her gas pedal. Ms. Kabanovsky's vehicle crashed into Ms. Conroy and then she panicked. Ms. Kabanovsky stated her car then accelerated across the street and crashed. Ms. Kabanovsky stated she was not wearing a seatbelt at the time of the crash and extricated herself from her vehicle after it rolled over onto its roof. I requested Ms. Kabonovsky's license while speaking to her, but she was too upset to located it. I queried Ms. Kabanovsky's license and registration status through Newton Dispatch. Ms. Kabanovsky's license stated is expired as of 12/18/2016. Ms. Kabanovsky registration is active. I observed substantial damage to the entire front end, driver, and passenger side of Ms. Kabanovsky vehicle. Ms. Kabanovsky was evaluated by Newton Medics and complained of right arm pain. Ms. Kabanovsky was also visibly distraught and expressed concern over the two pedestrians involved in the crash. Ms. Kabanovsky was transported to Brigham and Women's Hospital for treatment. I briefly spoke with Ms. Melinda Conroy as she was being attended to by Newton Medics and Fire. Ms. Conroy stated she was running on Langley Road (S) and observed MV1 traveling on Hamlet Street (W) towards Langley Road. Ms. Conroy stated she believed MV1 was slowing down and coming to a stop. Ms. Conroy stated she waved at the vehicle and as she crossed the intersection, MV1 accelerated and crashed into her left side. Ms. Conroy stated she flipped over the vehicle and landed on the roadway. Ms. Conroy was conscious and alert when speaking to Newton Medics and was transported to Beth Israel Deaconess Hospital in Boston. I later spoke with Ms. Conroy after her release from the hospital. Ms. Conroy stated she suffered some bumps and bruises had no major injuries. Officer Pohlman spoke with Ms. Sooper prior to her transport to Beth Israel Deaconess Hospital in Boston by Newton Medics. Ms. Sooper stated she was walking on the sidewalk in front of 323 Langley Road after returning from Newton Centre. Ms. Sooper stated as she was walking, she observed MV1 hit Ms. Conroy and then travel across the street in front of her into a tree. Ms. Sooper stated she does not remember what happened after that. I spoke with Ms. Sooper's son at NPD HQ and was advised Ms. Sooper was admitted to the hospital and suffered a fracture of her left hip and required 6 staples to close a laceration on her head. Tody's Towing responded to the scene and removed MV1 from the roadway. Traffic Officer Wilson took pictures of the scene and submitted them to the IT Bureau. After speaking with all parties involved, Ms. Kabanovsky will be cited with Massachusetts Uniform Citation 494688AB in hand at her residence for Chapter 90, Section 10 (Unlicensed Operation), Chapter 90, Section 24 (Operate To Endanger), and Newton City Ordinance Chapter 19, Section 75 (Fail To Use Care). Ms Kabanovsky was also cited with Massachusetts Uniform Citation 494800AB in hand at her residence for Chapter 90, Section 13A (Fail To Wear A Seat Belt). A Request for Immediate Threat License Suspension/Revocation was completed and submitted to the Registry of Motor Vehicles with supporting documentation via e-mail, DCUImmediateThreat@massmail.state.ma.us.
