On 01/04/19 I responded outside of #54 Needham St for a two vehicle crash. Veh #1 was a delivery box van who was driving due North on Needham St. The operator of veh #1 stated he was attempting to take a left into #55 Needham St. when veh #2 came from behind him on his left and he struck veh #2 as he turned. The operator of veh # 1 was taken by Ambulance #1 to Newton-Wellesley Hospital for an evaluation after complaining of pain in his chest area. The passenger of veh #1 stated everything happened "so fast" he didn't see what transpired. Moderate damage to veh #1. The operator of veh #2 stated he was traveling due North on Needham St. and was behind veh #1 for a long stretch of the road. The operator of veh # 2 stated veh #1 put his right directional on three different times but didn't turn. He stated veh #1 had his right directional on and was in the right lane when he went to pass him. Veh #1 then turned into his rear passenger side as he passed. Moderate damage to veh # 2. The operator of veh #2 stated the operator of veh #1 was walking around and yelling about the accident for a period of time and then eventually grabbed his chest and fell to the ground complaining of pain. From the result of my investigation it appears as if veh # 1 attempted to turn from the wrong lane and veh # 2 was passing in the "suicide lane", also wrong lane. There will be no citations issued at this time as both operators were given verbal warnings.

MV1 was parallel parked in front of Bank of America on Walnut St when he began to pull out, MV2, ( suspected to be MA Reg:8WX995) hit MV1 on his driver's side door. MV2 backed up in the road and then sped off without stopping to give his information. MV1 operator, Marcus Klevan, came into the police station to report this hit and run which occurred around 1430hrs today. MV1 operator and passenger were able to identify the license plate of the vehicle as it left the scene. The plate came back to a 1997 Toyota Tercel, blue, registered to John Twinomujuni out of Waltham. Waltham PD was notified and was unable to locate the vehicle at this time. UPDATE: Waltham PD was requested to check again for this vehicle at it's registered address tonight, 5/1/2019. This check resulted in negative results for the vehicle. No citation will be issued at this time until there is verification that this vehicle was indeed involved in this accident.

On the above date and time Mary Lacamera (operator # 1) came to NPD Headquarters to report a traffic accident that occurred on 07/14/2019 in the parking lot of Brae Burn Country Club, 326 Fuller St. Lacamera stated that she found a note on her M/V stating that the note writer had witnessed a black Buick Terraz (Ma Reg 9LW783), back into Lacamera's car, and leave the scene. The author of the note did not leave any contact information, or name. Ma Reg 9LW783 came back as a 2006 Black Buick Terraz, registered to Mirna Espinal, of 12 Delford St. Roslindale Ma. 02131. I attempted to contact Espinal with negative results. Citation #T1442113 was mailed to Espinal for MGL 90/24 Leaving The Scene?Property Damage. Lacamera stated that there was damage to the right rear of her M/V. (See NPD Incident Report #19029166)

M/V#1 parked in the parking lot of Stop N Shop on Needham St. She stated that she went shopping, and returned to her vehicle, she observed damage to the left rear. She also found a note from a Kathleen Bateman. The note stated that Kathleen Bateman witnessed an white Mercedes SUV, back into M/V#1, then drive away. She included a MA. REG for the Mercedes (MA REG 56J140). The operator of M/V#2 was charged with MGL 90/24 Leavung The Scene (Property Damage) citation #T2015402. See NPD incident report #19035938.

Operator #1 states he was driving eastbound on Auburn St in MA reg 905RRM (now known as vehicle #1), when a rabbit ran in front of his car which caused him to swerve. This resulted in vehicle #1 sideswiping MA reg 445ZE2 (now known as vehicle #2), which was parked/unoccupied on Auburn St in front of the Auburndale Commuter Lot. Vehicle #1 sustained damage to the entire right side and vehicle #2 had minor damage visible near the front driver's side wheel well. Upon querying both vehicles it was discovered that vehicle #1 had an expired registration. Vehicle #1 was subsequently towed by Tody's, and operator #1 was issued MA citation T1444941 in hand. A note was left on vehicle #2 referencing this crash report. It should be noted that vehicle #2 was parked in a posted No Parking area on Auburn St. Auburn St is a public way in the City of Newton. Operator #1 declined medical attention.

Operator #1 states he was driving northbound on Seminary Ave in MA reg 5LE114 (vehicle #1), when he was t-boned on the driver's side by MA reg 959SD7 (vehicle #2). Operator #2 states that he was attempting to turn left out of the Arnow Campus Center parking garage and did not see vehicle #1 approaching from the right. It should be noted that the physical address of the parking garage appears to be 131 Woodland Rd, however the collision occurred on the Seminary Ave side. All parties declined medical attention and no tows were needed. Vehicle #1 sustained significant damage along the driver's side doors. Vehicle #2 has moderate front end damage. Operator #2 was issued Ma citation T1268432 in hand for Failure to Yield to Oncoming Traffic. 131 Woodland Rd will be the address used in documenting this crash.

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

Operator 1 of MV 1 stated he was traveling E/B on Pine Ridge Rd when somebody was crossing the street and caught him off guard. Operator 1 swerved off the roadway taking out a fence and into the yard of 197 Pine Ridge Rd where it flipped over and landed on the hood of the vehicle. Operator #1 states that she was traveling northbound by 226 Lexington St when she observed vehicle #2, a white Ford Explorer, approaching at a high rate of speed from Lexington St southbound. Vehicle #2 then crossed the double yellow line into the northbound lane, subsequently sideswiping vehicle #1. Operator #1 states that two parties exited the Explorer and spoke briefly to her. Once she requested to exchange information, the two refused and drove away on Lexington St southbound. Operator #1 described the parties as a hispanic male and hispanic female, around 50 years old. The male party was balding/or bald, and the female was wearing a silver turtleneck. Operator #1 noted that it was the male who was operating the vehicle both during the collision and as the vehicle left the scene. While medics were documenting a patient refusal for operator #1, Ofc Ciccone was en route to NPD headquarters to report for duty in his p/v and heard the broadcast for the suspect vehicle. Ofc Ciccone observed a white Explorer with a flat tire traveling an estimated 50+ mph past him on South Ave by Park Rd in Weston heading towards Natick. This location is approximately 2.2 miles from the crash site and is accessible from 226 Lexington St by heading southbound, taking a right, and continuing straight on Rt 30. Weston Pd was immediately notified and the vehicle was located in a driveway at 7 High Meadow Rd, a residence directly on the corner of Rt 30. Sgt Rooney and Ofc Ciccone (who was able to take a spare cruiser and call into service as this incident unfolded) responded and met Weston PD. While on scene, it was explained to all officers by the resident of 7 High Meadow Rd, Helder Quesada, that he was walking his dog and observed the white Explorer broken down in an unsafe spot. Quesada offered up his driveway to park the vehicle as the male and female occupants waited for assistance. Quesada allegedly does not know Lopez and Albizureis, and all involved persons were outside in the driveway when police located the vehicle. The description of the male asociated with the Explorer matched that which operator #1 gave to me- a hispanic male balding in his 50's. He was in the presence of a hispanic female who was wearing a silver colored turtleck. The male was identified as Armando Lopez-Rubi DOB 1/1/73 and the female Ana Maria Albizureis DOB 11/27/73. The Explorer was MA reg 6GZ591, registered to a different party then all on scene. It had obvious damage to the left side doors consistent with an opposite direction sideswipe. With the translation assistance of Helder Quesada, Lopez-Rubi stated to Sgt Rooney and Officer Ciccone that he was in fact driving somewhere near Starbucks "on the corner" when he got into an accident (there is a large Starbucks located on the corner of Commonwealth Ave and Lexington St in Newton). Lopez originally said that the other involved vehicle left the scene, however it was pointed out by Sgt Rooney that the operator was currently stopped in Newton talking to me and was cooperative. Upon routine query it was discovered that Lopez-Rubi does not have a license to operate a motor vehicle. Pursuant to the investigation, Officer Ciccone issued in hand to Lopez MA citation R8006998 for the following: 89/4a- Marked Lanes Violation 90/24- Leaving Scene of Property Damage 90/10- Unlicensed Operation of a Motor Vehicle. Vehicle #2 was left in the driveway of 7 High Meadow Rd with Helder Quesada's permission. Quesada also volunteered to drive home both Lopez and Albizureis to Framingham, as he was feeling charitable. Vehicle #1 was towed by Tody's due to the amount of damage and airbag deployment. A m/v inventory form was filled out.

On 06/05/21 at approximately 09:47 I responded outside of #28 Indiana Terrace for an accident that happened yesterday 06/04/21 at approximately 14:30. The owner of veh#1 had his vehicle parked on the street in front of #28 Indiana Terrace and stated Witness #1 (his wife), saw the owner of veh #2 get into her car and attempt a u-turn. Witness said she saw veh#2 strike the front drivers side fender of her husbands parked vehicle-veh#1. The owner of veh#2 arrived back on scene and stated she did make a u-turn yesterday in the area of parked veh #1 but stated she didn't believe she struck it. There is damage to veh #2 consistent with the damage to veh #1 in the area which it was struck but the owner of veh #2 stated it was old damage. Owner stated she bought the car used with that damage and showed me pictures as evidence dated last year. Both owners were advised to file an accident report. Based on conflicting information no citations were issued at this time.

On August 27th 2021, Miriam Nkuba DOB 6/30/56 came to Newton Police Headquarters to report a past hit and run. Nkuba states that around 1500 hrs today, she was shopping in Star Market at 1 Boyston St. When she exited the store and returned to her vehicle in the lot (MA reg 7CNJ50), there were security workers cleaning debris from around her car. Immediately apparent was damage to the rear left tail light/bumper area. The security workers then directed Nkuba to witness Kate Sweeney, who was able to provide a picture of MA reg 4AX435, a black Chevrolet Suburban. Sweeney told Nkuba that the Suburban had hit her car and left the scene. I called Sweeney and spoke with her directly. She relayed that she was in a nearby store and heard a loud crash. Her attention was directed outside to a black Suburban slowly driving with obvious front passenger side damage. The vehicle then stopped in a parking space, and a heavy set woman exited from the driver's seat to inspect the damage on the front right side. The woman then got back into the vehicle and drove away in an unknown direction. 4AX435 comes back active on a 2012 Chevrolet Suburban color black to Maria Dasilva Alves out of West Roxbury. I requested Boston PD E-5 try and raise Alves and have her contact me. It should be noted that Alves has an expired/non renewable license and appears from her license photo to be heavy set. Charges pending. *Update, 8/28/21- Boston PD had negative results locating Alves or the vehicle. Based on corroborating statements by the witness, MA uniform citation T1446618 will be mailed to Alves for 90/24 Leaving the Scene of Property Damage and 90/10 Unlicensed Operation.

On 10/02/2021 while assigned to marked cruiser N503 at approximately 20:46 hours I, Officer Vazhenin, responded to 601 Washington Street for a report of 2-car motor vehicle accident. Upon arrival on scene, I spoke to Mr. Jeffrey Yanfeng Xiao (Driver 1), who stated that he was driving eastbound on Washington Street when he observed another car (Vehicle 2) pulling out of 7/11 parking lot at 612 Washington street. Driver 1 said that in order to avoid collision with Vehicle 2, he had to swerve to the left in oncoming traffic lane crossing double yellow line. He said that while he was passing Vehicle 2, Vehicle 2 kept driving and crashed into the passenger side of his car (Vehicle 1). I then spoke to Ms. Shahrbanou Asadi, the operator of Vehicle 2, who stated that she was pulling out of 7/11 parking lot and was trying to make a left turn on Washington street. She stated that Vehicle 1 "showed up out of nowhere" and was traveling at high rate of speed. Ms. Asadi stated that she proceeded making her left turn when Vehicle 1 suddenly appeared in the front of her car and her car (Vehicle 2) crashed into passenger side of Vehicle 1. It should be noted that Ms. Asadi did not have her driver's license in possession. License plate inquiry of her car (Vehicle 2, MA Reg.#883MH8) revealed the fact that her driver's license was expired, which she failed to disclose to me when asked about status of her registration and driver's license. I ussued a Criminal Application to Ms. Shahrbanou Asadi on Massachusetts Uniform Citation #T1445734 for Ch.90 S.10 "Unlicensed Operation of Motor Vehicle", Ch.89 S.8 "Failure to yield", Ch.90 S.11 "License not in possession".

MV 1 WHO INITIALLY STOPPED ON WASHINGTON ST IN ORDER TO LET A MV TURN LEFT OUT OF THE DRIVEWAY OF THE WARREN HOUSE, LEFT THE SCENE PRIOR TO EXCHANGING INFORMATION WITH THE OTHER INDIVIDUALS INVOLVED. A WITNESS IDENTIFIED AS KATE VELASQUEZ HELLER WAS ABLE TO TAKE A PICTURE OF MV1 WHICH INCLUDED THEIR LICENSE PLATE (MA REG 6NH642) AND IT SHOULD BE NOTED THEIR REAR WINDSHIELD WAS BLOWN OUT DUE TO THE IMPACT. THIS PICTURE, WHICH SHE TOOK ON HER CELL PHONE WAS EMAILED TO ME AND ATTACHED BY THE IT BUREAU TO THIS REPORT. THE VEHICLE BELONGS TO A FEMALE PARTY OUT OF BRIGHTON MA AND THE OWNER DOES NOT APPEAR TO HAVE A MA LICENSE. I HAD DISPATCH CALL BPD TO SEE IF THEY COULD GO BY THE ADDRESS TO SEE IF THE INVOLVED VEHICLE WAS THERE AND TO SEE IF THEY COULD RAISE ANYBODY AT THAT ADDRESS AND I NEVER GOT A RESPONSE REGARDING THIS ACCIDENT. IT IS UNCLEAR FROM THE PICTURE AND VIDEOS I VIEWED WHO IS OPERATING MV1. ALL THREE VEHICLES SUSTAINED MODERATE TO HEAVY DAMAGE. MV2 WAS TOWED BY AAA AND MV 3 WAS TOWED BY TODYS TO THEIR LOT. THE OPERATOR OF MV2 WAS TRANSPORTED TO NWH BY MEDICS TO BE FURTHER EVALUATED AND NO OTHER INJURIES WERE REPORTED. THE OPERATOR OF MV2 ALSO HAS A DASH CAM WITH 2 VIDEOS AND STATED THAT SHE WOULD EMAIL ME THE VIDEOS WHEN THEY HAVE A CHANCE. I VIEWED BOTH DASH CAM VIDEOS WHICH WERE EMAILED TO ME, AND IT CONFIRMS THAT MV 3 CAUSED THE ACCIDENT BY NOT STOPPING AND REAR ENDING MV 2. THESE VIDEOS WERE ALSO ATTACHED TO THIS REPORT BY THE IT BUREAU. UPDATE: ON 11/12/21 I RECEIVED A CALL FROM SALINA LOPES AND HER FRIEND WHO HELPED TRANSLATE FOR HER DUE TO A LANGUAGE BARRIER. SALINA CONFIRMED THAT SHE WAS THE PERSON DRIVING HER VEHICLE AT THE TIME OF THE CRASH AND SHE DIDN'T STAY ON SCENE BECAUSE SHE WAS NERVOUS. SHE WAS ADVISED SHE WOULD BE ISSUED A CITATION VIA MAIL FOR CH. 90 S.10 UNLICENSED OPERATION.

MV1 operator states she was pulling out of the Sunoco gas station when MV2 began backing up and struck the front driver side door of MV1. I observed damage to MV1 driver side door. MV2 operator states he was going to pull onto Rt.9 but due to size of vehicle, needed to back up to create more space. While backing vehicle, MV2 struck MV1. No injuries reported on scene. No tows needed. Operator of MV2, Luciano Pinheiro was cited for Unlicensed Operation c90s10. See incident report 21048567. A complaint app was filed and submitted. The parking lot is a location in which the public has access as invitees.

On January 23, 2022 at approximately 1458 hours, I responded to the area of 325 Beacon St for a report of a 4 vehicle MVA with possible injury. Upon arrival with the Fire Department and EMS, I met with all the drivers. No injuries were reported on scene. MV1 operator states she was driving WB on Beacon St when MV2 came to a sudden stop and due to the sun's glare, did not see MV2 very well. MV1 operator then rear-ended MV2. I did not observe any damage to the front of MV1 or the rear of MV2. MV2 operator states she was driving WB on Beacon St when MV1 suddenly collided with her rear end. MV2 subsequently rear-ended MV3. I did observe damage to the front of MV2 and to the rear of MV3. MV3 operator states he was stopped, waiting for the cars to go since the light had just turned green, when he was suddenly rear-ended by MV2. MV3 subsequently rear-ended MV4. I did observe damage to the front of MV3 and the rear of MV4. MV4 operator states she was stopped in traffic when she was suddenly rear-ended by MV3. No vehicles were towed. No citations were issued.

MV1 stated he was travelling "slow" down California St when a "black truck (MV2) tried to pass me, hit my bumper forcing me to hit that parked car" (MV3). No reported injuries. OP1 of MV 1 was traveling n/b on Wells ave when he was taking a right into 70 Wells ave. The Street was made a one way due to the bike race detail going on. OP1 had to cross over a set of cones to take a right into the parking lot and did not see the bicycle traveling. The bicycle his the passenger side of MV1. Minor damage to MV1. OP1 was not injured. Bicycle operator was traveling around Wells ave N/B when he saw the vehicle turn right, he was unable to stop in time and hit the side of MV1. Bicycle operator had a slight injury to his hand but refused medical treatment. At this time no citations issued at this time.
