Narrative:
I responded to the area 107 Dartmouth St. at approx. 0345 on 04/28/9019 along with N493 Officer Kelly Donahue and N508 Sgt. Devine for a report of an MVA. The MVA was reported to be single vehicle crash. The vehicle left the roadway and was on the front lawn of 107 Dartmouth St. I arrived on location (first on scene ). I observed tire tread marks across the front lawn of 107 Dartmouth St. I then observed a vehicle in the bushes toward the left front side of the house. The vehicle; NY Reg. JHE8252, Porsche color blue. The RMV info on the plate came back registered to Porsche;of ;South. Registration plate type: Dealer Assignment. 185 W Sunrise HWY; Freeport,NY (no further info). The vehicle was unoccupied and secured. I felt the hood of the car which was still warm from the engine. I checked the immediate area surrounding the crash scene for an operator but found no one. I radioed to other responding units to canvas the neighborhood and surrounding area for the operator on foot. I spoke to the homeowners at 107 Dartmouth St., identified as Anthony and June Limberis. Mrs. Limberis stated she heard a load noise and a car engine revving. She stated when she looked out the window she observed a W/M tall, thin, wearing tan khakis and a light colored plaid shirt walking across the front lawn. He proceeded up Dartmouth St. (northbound) toward Commonwealth Ave. Newton Fire Dept arrived on location to check the crash scene and vehicle for possible leaking fluids and any other hazards. As I was talking with the firefighters we were approached by two men. One of the men was intoxicated. He was unsteady on his feet, had glassy bloodshot eyes, and was slurring his speech. The man was a W/M tall/thin approx. 6'2" and was wearing tan khakis and a light colored plaid shirt. He was identified as Eric Adams DOB 09/18/1969. The second man with him was identified as John Adams DOB 02/07/1936; An elderly W/M approx. 5'6" also wearing tan khakis and a light colored plaid shirt. Eric Adams stated his father John was the operator of the vehicle. Eric Adams had a strong odor of Alcoholic beverage emanating from his person and breath. He continued to slur his speech and was unsteady on his feet as he spoke. He was swaying back and forth and side to side as he spoke to me. Officer K. Donahue and Sgt. Devine were on scene at this time and also made the same observations. Multiple times we had to grab hold of Eric Adams to keep him from losing balance and falling. We spoke with Eric and John Adams for several minutes as their accounts of what took place were not consistent. I asked Eric Adams where the keys to the vehicle were. Eric Adams immediately pulled the starter key to the vehicle from his front right pocket. Eric Adams became defensive and belligerent when questioned about who was operating the vehicle. John Adams appeared tired (as if he just woke up) and confused as to the situation. Sgt Devine and I both spoke with Eric Adams and John Adams. Although both men claim John Adams was the operator of the vehicle, a clear determination was uncertain. A query of both mens driver's license status on NY Licenses came back active. Eric Adams has two prior DUI's on his RMV record. Officer Donahue and Sgt. Devine escorted the two men to 93 Dartmouth St. Eric Adams stated they were staying at that residence which is his sister's house. I stayed at the crash scene to document information and assist with clearing the MV from the property with Tody's Towing Service. As they were walking away I observed Officer Donahue and Sgt. Devine continue to have to assist Eric Adams with staying on his feet. Approximately ten minutes later as I was speaking with the aforementioned homeowners Mr.&Mrs. Limberis. Officer Donahue and Sgt. Devine returned to the crash scene. As they were updating me we observed Eric Adams returning to the crash scene. He was stumbling and swaying as he walked and repeatedly almost fell. Officer Donahue and Sgt. Devine immediately intercepted and assisted him before he fell. As they were assisting him again I continued speaking with the homeowners. Suddenly I heard Officer Donahue and Sgt. Devine struggling with Eric Adams who was now on the ground and resisting being handcuffed. I ran to assist them. Eric Adams continued to resist and be uncooperative until he was appropriately restrained in handcuffs. Eric Adams was placed under arrest by officer Donahue for Disorderly Conduct and Resisting Arrest. Reference Officer Donahues Newton Police Incident Report #19016484. The vehicle NY Reg. JHE8252 was removed from the property of 107 Dartmouth St. and towed from the crash scene by Tody's Towing Service.

On 6/13/19 I responded to 19 Sutcliffe Pk for a report of a MVA. Upon arrival I spoke with the operator of MV#1 who stated he turned onto Sutcliffe Pk to turn around and when he came back down the hill his brakes wouldn't' work when he struck MV#2 which was legally parked on the street. The owner of MV#2 was on scene and was not inside the vehicle at the time. There was heavy rear end damage and the owner stated he would call for his own tow first thing in the morning. MV#1 had air bag deployment and heavy front end damage. NFD and medics responded to evaluate the operator of MV#1. The operator had no injuries and signed a patient refusal. Tody's towing responded and towed MV#1. MV#2 had a dealer plate DUKE, but did not come back to anything through the registry. The vehicle was a 2006 black Bentley Flying Spur, VIN# SCBBR53W96C034217. John Bortone owns Velocity Motors out of 14 Hawthorne St in Newton and stated he recently purchased the vehicle out of Florida.

On August 1, 2019 at approximately 17:34 hours I, Officer Guarino, responded with Newton Fire and Medics for a report of a single vehicle crash into the building of 335 Boylston St. Upon arrival, the operator and sole occupant of the vehicle was out of the vehicle and had minor injuries. MV1 struck a stop sign and was up against 335 Boylston St, which appeared to have a broken window and minor structural damage. Operator of MV1 said that he was traveling in the right lane of Boylston St (Westbound) when a white Mercedes sedan (unknown model or registration number) turned into his lane and almost struck his vehicle. Operator of MV1 swerved to the right to avoid a collision and as a result crashed into the stop sign on John St. and the side of 335 Boylston St. Witness 1, who was parked at 325 Boylston St. saw the incident and corroborated MV1's story. A second witness, who I was unable to make contact with stopped and gave MV1 his phone number of (508)215-4951 and texted MV1 that he caught up with the possible vehicle, MA Reg 9BT373, which comes back to DESOUZA PLASTERING SERVICES INC. I will attempt to make contact with an operator during their business hours. The owner of 335 Boylston St, Onir Spiegel, was on scene, was given the incident number and was advised to contact her insurance company. MV1 was towed from the scene by Tody's and the operator signed a patient refusal from the Medics. Pictures were taken at the scene of the damage and submitted to IT to be attached to this report. Sergeant McLean notified the DPW for the stop sign and glass in the roadway. Sergeant McLean also left a message for Newton Inspectional Services to come inspect damage to the building at a later time.

Operator of MV1 stated she was traveling westbound on Washington Street passing the area of 719 Washington Street. As she was traveling in her lane, Newton Fire Engine 4 was in the lane next to her on her left. The engine veered to the right and went into her travel lane. Operator of MV1 said that she was forced to turn to the right to avoid colliding with the fire truck. She then struck MV2, which was parked on the side of Washington Street causing damage to his drivers side mirror and her passenger side mirror. The operator of MV2 was in his vehicle at the time of the accident. He stated that he also witnessed Engine 4 enter MV1's travel lane forcing her to collide into his vehicle. Neither party was injured and no contact was made with Newton Engine 4.

MV#1 stated while driving his truck and trailer Westbound on Nahanton St across from the golf course the right top side of the trailer struck a City of Newton tree. MV#1 stated after hearing and feeling the collision he immediately stopped his MV. The collision caused part of the tree to break off and fall on top of his truck as well as into the Eastbound lane. MV#1 sustained minor damage to the top passenger side of his trailer. MV#2 stated while traveling Eastbound on Nahanton St she witnessed MV#1 strike a tree on the top passenger side of his vehicle. MV#2 stated a tree branch then fell from the top of his MV into her Eastbound lane. The branches scratched the diver side of her vehicle while passing. MV#2 had scratch marks that was consistent to both operators statements. MV#2 sustained minor damage. Both parties stated the were not injured and refused medical attention at this time. Neither vehicle was towed from the scene due to minor damage. City of Newton Public works along with Newton Fire were able to safely remove the branches from the trailer as well as the road.

On 1/23/19 while working N493, I responded to 966 Watertown St for an MVA, car into a building. Business name is TIEM. The operator (operator #1) was the sole occupant of the vehicle (MA reg 4PW429, now known as vehicle #1). I was able to obtain a statement from operator #1 while medics and fire were en route. Operator #1 told me that she was driving eastbound on Watertown St when vehicle #2 (MA reg 1BSN69) pulled out to take a left from the Shell Gas Station at 979 Watertown St. Operator #1 had to swerve to the right in order to avoid a collision with vehicle #2. This caused vehicle #1 to crash into the exterior brick wall of 966 Watertown St. There was airbag deployment and visible structural damage to the business, however it did not appear severe. The operator of vehicle #2 was still on scene and confirmed the events that operator #1 spoke of. Operator #2 stated she did not see vehicle #1 as she was pulling out due to heavy traffic flow and parked cars. Vehicle #2 did not make contact with any other car or structure. Vehicle #1 sustained heavy front end damage and was towed by Tody's. Operator #1 was checked out by the medics but declined transport. The owner of the business affected was notified and responded to the scene. He was provided this report number for insurance purposes. No emergency accomodations or notifications needed be made in regards to the damage. It should be noted that a witness remained on scene and verified what operator #1 said transpired. His contact information was provided and is listed in the appropriate field, along with the business owner info for TIEM. Operator #2 was issued in hand MA citation T2079187 and sent on her way.

Operator #1 states he was attempting to back up spare Engine 11 into the left bay at Fire Station #2, when he accidentally hit a city light pole during the maneuver. The pole is located on the island between the eastbound/westbound lanes of Commonwealth Avenue. The pole was knocked down however it landed completely within the grassy island and is still intact. There is no damage to Engine 11, and Capt Roche notified NPD dispatch to contact DPW about the light pole. Pictures were taken and submitted to IT by myself. No injuries were sustained.

On 9/1/20 while working N493, I responded with N497 and the Fire Dept to 40 Crescent St for a report of a car into the house. On arrival I observed MA 6CM450 in the backyard of #40 unit C. It was stationary against the large AC Compressor in the rear of the unit, and appeared to had run over several large shrubs before coming to a stop. I spoke with vehicle owner and operator Marion Cohen 8/27/46. Cohen stated that as she was parking in the rear lot of the complex, she accidentally mistook the brake for the gas pedal. She in turn ended up going over the curb and through some bushes before hitting the compressor. There was a passenger in the vehicle identified as Milton Kornfeld 3/10/42. Both occupants were checked out by medics and declined transport. The airbags did not deploy. There does not appear to be any structural damage to anything other than the compressor. NFD checked the integrity of the house, and the compressor was unplugged. I took pictures of the scene and submitted the memory card to IT. Homeowner Xiaozhi Zhao and his family were present inside at the time of the collision and are unharmed. Tody's responded and removed the vehicle from the property. It sustained heavy front end damage and was towed to their lot via flatbed. A m/v inventory form was filled out. This report number has been provided to all parties for their respective insurance companies.

On 9/10/2020 I was working N502 when I responded to a 2 car MVA at 1611 Centre St. which was called in by Sgt. Mlcean. Upon arrival I spoke with Rosa Malave first the driver of Veh 1. Rosa stated she felt she had a head injury and Newton fire and medics were called to respond. Rosa stated she was sitting stoped in traffic when she was struck by the operator of Veh 2 Barakatullo Ashurov from behind. Rosa was transported by Newton Medics to NWH and her car was towed by Todys. I then spoke with Ashurov the operator of Veh 2. Ashurov stated he was sitting stopped in traffic when the car in front of him "Veh 1" began to reverse and struck him at aprox 1 to 2 miles an hour. Ashurov stated he got out to see if the woman was alright and she stated that he had hit her and an argument insued. There was no damage observed on either vehicle. There were no witnesses and no cameras in the area. Both parties were provided with a report number.

On Monday September 21st, 2020 while in N526 I responded to Newton Fire Station #2 located at 1750 Commonwealth Ave for a past accident involving Newton Fire Ladder #1 and a Cataldo Ambulance. The accident is reported to have occurred on Thursday September 17th 2020 at 10:00am. At the time of the accident the weather was partly cloudy and the road surface was dry. Captain Roche stated he was working at the station that day when a Cataldo Ambulance truck MA AMR:AMB41 slowly backed up and struck the top of the ladder on the Ladder Truck#1 MA MVN: MF463. Ladder #1 was parked and unoccupied at the time in front of the Fire Station. At the time no damage was reported on either apparatus. Today damage was noticed on the top part of the ladder. Pictures were taken of a crack and some damage to the ladder. I spoke with the operator of the ambulance identified as EMT James Perry (S14785886 ) who stated on Thursday at around 10:00am he was delivering spare parts to another ambulance and was slowly backing up in front of the Fire Station. James stated he was going about 2MPH when he bumped into the front of Ladder #1. James stated there was no damage reported to either apparatus at the time. This minor accident occurred on City of Newton Property in the driveway of Fire Station #2. No injuries were reported. Pictures of Ladder #1 were submitted to the IT Bureau.

On Wednesday October 14, 2020 at approx. 0901 hours I responded to the area of 277 Elliot Street for a report of a two motor vehicle crash unknown injuries. Upon arrival on scene, Newton Fire personnel and medics were on scene for an evaluation. Both parties refused / advised treatment and signed patient refusals. I spoke with the oper of v1, she stated a vehicle in front of her stopped abruptly and she slammed on her brakes and was stopped when she felt impact from vehicle 2. Oper v2 stated that vehicle 1 stopped abruptly, he slammed on his brakes and due to the roadway being wet his vehicle slid into v1. He stated he had some irritation from the front air bag deploying. Witness stated a vehicle in front of him stopped abruptly to turn left, which caused him to stop abruptly, which caused chain reaction.

-Mv#2 was parked in the rear parking lot of 2 Wells Ave when it was struck my an unknown MV. -Ow Mv#2 reported approximately 0915 she discovered the right side bumper of MV#2 had been struck and heavily damaged. OwMv#2 stated she parked Mv#2 in the first stall by the right side loading dock facing the building approximately 0350. She stated Newton Fire Department responded to the area (CAD 21012488) and they were the only vehicle to come in or out of the parking lot this morning. - I conducted and inspection of MV#2 in the Cherry Street Parking lot since Mv#2 was still serviceable. I observed damage to the rear left side bumper, a cut in the bumper plastic revealing the metal bumper frame underneath it and black friction mark on the surface of the bumper. -There were no injuries or other property damage reported at the time of this interview. This report will be forwarded to the Traffic Bureau for investigation. Traffic Bureau Update (Officer Gaudet): On Monday, March 22, 2021, I contacted Ms. Linda Nieman (Director of MGPO Dermatopathology Associates) inquiring about surveillance cameras on the property located at 2 Wells Avenue, Newton. Ms. Nieman stated she was unaware if the property had cameras but has sent and request to the building managers. I contacted the building manager Ms. Madison Hebb. Ms. Hebb stated she is not sure if the area of the building where the alleged crash took place has cameras, but she is looking into it and will contact me when she finds out. I contacted the Newton Fire Department to try and speak with Lt. Foli who was the incident commander for the call at 2 Wells Avenue (CAD 21012488). Lt. Foli is not back to duty until this Friday. I will attempt to make contact with him during his tour of duty at Station 10 at this time. If it is determined that a Newton Fire vehicle made contact with MV1, I will update this crash report. I spoke with Ms. Danielle Subramaniam, the owner of the damaged vehicle (MV1). Ms. Subramaniam stated she did not observe a Newton Fire vehicle hit her vehicle while they responded to 2 Wells Avenue the morning her vehicle was damage. Ms. Subramaniam stated she believes the only vehicles that were in the parking lot near her vehicle during her shift were Newton Fire vehicles. Ms. Subramaniam stated when she left work after her shift that morning, she observed new damage to her vehicle. On Tuesday, March 23, 2021, Miss Hebb updated me that the side of the property where the alleged incident took place does not have any surveillance cameras present. At this time, I am unable to conclude that a Newton Fire Department vehicle caused the damage to Ms. Subramaniam's vehicle.

A grey older model trailblazer backed into utility pole 1/18, causing a power surge at 425 Watertown Street. Fire responded to the building for smoke inside, when they arrived the witness told them what had happened. Eversource was notified. There were no other witnesses. The vehicle involved had damage to the rear passenger side tail light. Unknown registration. Description provided was a grey older model chevy trailblazer. At approximately 2000hrs the owner of the vehicle contacted NPD. The operator stated he was backing out of the parking spot and struck the untility pole. He stated he observed his rear passenger side tail light cracked and did not know there was damage to any wires or the utility pole. He proceeded eastbound on Watertown Street. The owner of the building/property where the accident occured contacted NPD. I spoke to Mr. Franchi and provided him the accident number for insurance purposes.

There was a medical emergency at 2101 Washington Street (Care One) so Fire, Medics, and Police were all parked by the front entrance. The OP. of MV#1 states that she was trying to park correctly in her spot. She backed out and her attention was drawn to an ambulance and didn't realize there was a second ambulance behind her. That's when she collided with MV#2. MV#2 is an unoccupied ambulance with its emergency lights on while medics were out assisting a patient. Medic J. Spratt was nearby and observed his truck shake and walked around and noticed MV#1 pulling away from the ambulance with damage to the corner of the vehicle. Pictures have been taken of both MV#1 and MV#2. Ambulance MV#2 has some old damage not related to this current accident. There doesn't appear to be any new damage to MV#2 from this current accident.

I NOTICED A RED FIRE HYDRANT ACROSS THE STREET WAS DISLODGED ROM ITS BASE AND LAYING SIDEWAYS ON THE GROUND. I SPOKE TO A WITNESS WHO LIVES ACROSS THE STREET AT 222 AUSTIN ST. ( DALLAS PERKINS ). WITNESS STATES HE HAD HEARD THE CRASH AND SAW FROM HIS WINDOW A LARGE BOX TRUCK WITH A TRAILER ATTACHMENT THAT CONTAINED SEVERAL LANDSCAPING EQUIPMENT PIECES DRIVE AWAY FROM THE SCENE. WHEN HE NOTICED THE FIRE HYDRANT WAS KNOCKED OVER HE CALLED THE POLICE. WITNESS STATES FROM HIS VANTAGE POINT HE WAS UNABLE TO GET A PLATE OR OTHER IDENTIFIABLE MARKINGS. NEWTON PUBLIC WORKS WAS NOTIFIED TO REPLACE THE HYDRANT. N525 ( OFF GAUDET ) ARRIVED AND TOOK PICTURES OF THE SCENE AND THE DAMAGE. CLEARED WITHOUT FURTHER INCIDENT.

On January 23, 2022 at approximately 1458 hours, I responded to the area of 325 Beacon St for a report of a 4 vehicle MVA with possible injury. Upon arrival with the Fire Department and EMS, I met with all the drivers. No injuries were reported on scene. MV1 operator states she was driving WB on Beacon St when MV2 came to a sudden stop and due to the sun's glare, did not see MV2 very well. MV1 operator then rear-ended MV2. I did not observe any damage to the front of MV1 or the rear of MV2. MV2 operator states she was driving WB on Beacon St when MV1 suddenly collided with her rear end. MV2 subsequently rear-ended MV3. I did observe damage to the front of MV2 and to the rear of MV3. MV3 operator states he was stopped, waiting for the cars to go since the light had just turned green, when he was suddenly rear-ended by MV2. MV3 subsequently rear-ended MV4. I did observe damage to the front of MV3 and the rear of MV4. MV4 operator states she was stopped in traffic when she was suddenly rear-ended by MV3. No vehicles were towed. No citations were issued.

On February 16th, 2022 at approximately 10:09 hours myself and NFD responded to the area of #64 Craft St, GM Auto Body for a report of wires down possibly arcing. On our arrival we found numerous cable wires down along Craft St and Clinton St blocking both roads to traffic. NFD was able to remove the lines which were not electrical but belonging to Verizon and Comcast Both companies were notified to respond due to the large area affected. Two residences had damage to their houses from there cables being ripped away from their house. #'s 64 and 43 Clinton St. Both spoke with Verizon about filing claims. The cause of this accident appears to have been due to an unknown truck catching a low hanging cable line that went from pole to pole on Craft St causing a chain reaction ripping down numerous other lines.

At 0630 hrs.on 4/21/22, Newton Police and Fire were dispatched to the area of 1146 Chestnut St. for a motor vehicle collision into a tree. Upon arrival, MV#1 was observed with damage to its front end and cracked/broken front windshields. The operator of MV#1 stated he had worked all night and had fallen asleep causing the garbage truck he was operating to veer off of the roadway and into the city owned tree in front of 1146 Chestnut St. The tree itself had been knocked over and was across the roadway. A second tree on the property of 1146 Chestnut St. suffered damage. Further, several large tree limbs had fallen a top of MV#2 which was parked in the driveway of 1146 Chestnut St, but no visible damage was observed. The operator of MV#1 declined medical attention and contacted a private tow to remove MV#1 from the roadway. The operator of MV#2 as well as the homeowner of 1146 Chestnut St. were apprised of the situation. Pictures of the damage were taken on the N498 camera and attached to this report.

Narrative:
Newton Police, Fire and Medics were dispatched to Commonwealth. Ave. and Dartmouth St. for Motor Vehicle Collision. Dispatch was advising a vehicle had collided into a poll. Upon arrival, Ofc. observed a Eversource street light poll laying on the grass carriage lane (westbound side). MV#1 was observed facing westbound in the eastbound lane of Commonwealth. Ave with front and side airbags deployed. The operator of MV#1 reported she was traveling westbound on Commonwealth Ave. prior to colliding into the light pole, but states she does not remember the moments prior to the collision.The operator of MV#1 reported she was uninjured, but was transported to Newton-Wellesley Hospital as a precaution. Newton Fire personnel determined the damaged light poll was not a hazard while Eversource was notified regarding their damaged poll. A witness to the collision reported they observed MV#1 traveling westbound on Commonwealth Ave. at approximately 20-25 mph, before it veered off the roadway and into the light poll. After colliding into the light poll MV#1 crossed over the double yellow line into oncoming traffic and hit the curb on the eastbound side of Commonwealth Ave. Pictures of MV#1 and the Eversource light poll were taken on the N493 camera and submitted to the NPD IT bureau. A Request for an Immediate Threat License Suspension/Revocation was faxed over to the Massachusetts RMV on behalf of the Operator of MV#1 due to the circumstances of this collision.
