Owner of V1 stated he was parked in the parking lot, came outside, and noticed damage to his right rear door. After several minutes, I was able to make contact with the second vehicle involved. Opr of V2 stated she was attempting to park her vehicle. Opr of V2 stated she backed up to try and park and that is when she made contact of V1. Opr of V2 stated V1 was not in a valid parking spot. V2 sustained damage to her right rear bumper. V1 was not in a spot and it is unclear if it is a valid parking spot.

OPMV#1 Was traveling Southbound on Chestnut St when she saw an object in the middle of the road and swerved into the Northbound lane of Chestnut St. OPMV#1 Drove up over the curb and onto the sidewalk colliding with a stone wall at 542 Chestnut St and a Utility pole. The vehicle then traveled Southbound on the sidewalk and stopped at the intersection of Chestnut St and Roslyn Rd. I observed major damage to front of the vehicle along with left and right side damage. Airbags were deployed on all sides. OPMV#1 Was evaluated by EMS and she signed a medical refusal. Vehicle was towed by Tody's Towing. OPMV#1 is charged with the following: M.G.L. C.89 S.4A Marked Lanes, M.G.L. C.90 S.24 Negligent Operation, M.G.L. C.90 S.24 OUI Alcohol. On the above date and time Mary Lacamera (operator # 1) came to NPD Headquarters to report a traffic accident that occurred on 07/14/2019 in the parking lot of Brae Burn Country Club, 326 Fuller St. Lacamera stated that she found a note on her M/V stating that the note writer had witnessed a black Buick Terraz (Ma Reg 9LW783), back into Lacamera's car, and leave the scene. The author of the note did not leave any contact information, or name. Ma Reg 9LW783 came back as a 2006 Black Buick Terraz, registered to Mirna Espinal, of 12 Delford St. Roslindale Ma. 02131. I attempted to contact Espinal with negative results. Citation #T1442113 was mailed to Espinal for MGL 90/24 Leaving The Scene?Property Damage. On Tuesday, July 23rd 2019, at approximately 5:24pm, I, Officer Brooks, responded to 444 Lowell avenue for a 2 car MVA. Upon my arrival I observed MV1(NH REG 4120041) with heavy front end damage. The vehicle was located in the driveway of 444 Lowell avenue, with the front end sticking out into the roadway. I spoke with the operator of MV1 who stated he was trying to exit the driveway and turn left onto Lowell avenue southbound, when he was struck by MV2(MA REG 3AP243) which had been traveling northbound on Lowell avenue. According to the operator of MV1, MV2 had been traveling at a high rate of speed and tried to swerve around him. There was airbag deployment in MV1 however the operator signed a patient refusal with Cataldo. MV1 was towed by Tody's. I then spoke with the operator of MV2 who stated she was traveling northbound on Lowell avenue and saw MV1 exiting the driveway. She assumed he was going to stop pulling out of the driveway, and when he did not she stated she attempted to swerve around him. MV1 made contact with the passenger side rear wheel well of MV2 causing MV2 to spin out. The operator was not injured and arranged for a private tow due to the damage in the wheel well area.

On August 1, 2019 at approximately 17:34 hours I, Officer Guarino, responded with Newton Fire and Medics for a report of a single vehicle crash into the building of 335 Boylston St. Upon arrival, the operator and sole occupant of the vehicle was out of the vehicle and had minor injuries. MV1 struck a stop sign and was up against 335 Boylston St, which appeared to have a broken window and minor structural damage. Operator of MV1 said that he was traveling in the right lane of Boylston St (Westbound) when a white Mercedes sedan (unknown model or registration number) turned into his lane and almost struck his vehicle. Operator of MV1 swerved to the right to avoid a collision and as a result crashed into the stop sign on John St. and the side of 335 Boylston St. Witness 1, who was parked at 325 Boylston St. saw the incident and corroborated MV1's story. A second witness, who I was unable to make contact with stopped and gave MV1 his phone number of (508)215-4951 and texted MV1 that he caught up with the possible vehicle, MA Reg 9BT373, which comes back to DESOUZA PLASTERING SERVICES INC. I will attempt to make contact with an operator during their business hours. The owner of 335 Boylston St, Onir Spiegel, was on scene, was given the incident number and was advised to contact her insurance company. MV1 was towed from the scene by Tody's and the operator signed a patient refusal from the Medics. Pictures were taken at the scene of the damage and submitted to IT to be attached to this report. Sergeant McLean notified the DPW for the stop sign and glass in the roadway. Sergeant McLean also left a message for Newton Inspectional Services to come inspect damage to the building at a later time.

Operator of vehicle # 1 stated she was travelling Northbound on East side parkway. Operator #1 stated as she approached a curb bump out that narrowed the roadway, an oncoming unkown(no description) vehicle sped up around a parked vehicle and entered vehicle # 1's travel lane. Operator #1 was forced to swerve to the right to avoid a collision . The cargo portion of the truck collided with a large branch of a city tree overhanging the road which then uprooted the tree. Photos of the crash were taken by traffic. Eversource responded to the scene and the tree was removed by the city. The home owner of 152 East side Parkway was not home at the time of the incident. I spoke to the home owner Mr. Hans Hinteregger on 08/19/2019. Mr. Hinteregger stated that his residence sustained damage when the power line was pulled from the side of his house. The crash drawing and property information has been updated with the homeowners information.

On 8/7/2019 at approx 1140hrs while assigned to 497 I responded to 2014 Washington St, NWH west entrance for a report of a two car crash without injuries. Upon arrival I spoke with the operators of Ma Reg 2GF281 Kira WIRSIG and Ma Reg LV56933 Yevhen ROITBERH. WIRSIG stated she was travelling straight ahead in the right EB lane when ROITBERH turned right into her vehicle from the left EB lane causing the collision. ROITBERH stated he was turning right into NWH from the right EB lane when WIRSIG attempted to pass him on the right . WIRSIGS passenger Christopher BERRY agreed with her version. ROITBERH had photos on his phone of his vehicle in the right lane turning into NWH which he claims show he had the right of way.

Mv#1 operator states she was pulling out forward W/B from a parallel marked parking space in front of 843 Beacon St. #1 states she was about half way into the travel lane coming out of her turn to proceed straight ahead. #1 states at this time, she was struck on her driver's side front end that took off the bumper by Mv#2 that was travelling past her at a high rate of speed from behind straight ahead on Beacon St W/B. #1 operator further states #2 had crossed over the double yellow line to drive around her as she was out into the travel lane. Mv#2 operator states she was travelling on Beacon St W/B and was struck on her passenger side by #1 as #1 was pulling out of the parking space. I observed the initial contact damage on #2 on its passenger side rear door running down to the front of the passenger side wheel well and passenger side rear tire (tire wall partly sliced from impact). When #2 operator was asked if she crossed over the double yellow line, #2 replied only after the impact. #2 added prior to the collision, she briefly looked at her GPS. After statements were received from both operators and at the conclusion of this investigation, a witness identified herself on scene. The witness stated she was sitting in her driver's seat in her parked Mv directly in front of #1. Witness stated she observed #1 pull out from behind her into the travel lane. Witness stated #1's front end had almost fully reached her driver's side door before #2 made impact. Based on statements and witness statements and it appears #2 operator did not see #1 enter the roadway until the last second before impact by overcorrecting and entering the opposite lane of travel.

Rizza stated he waited for the crosswalk signal to turn on. When it finally turned on to walk (white walking person) he got on his bike to start peddling. At this time, he observed a black vehicle traveling southbound traveling towards him. Moments later, Rizza was struck by the vehicle. Rizza rolled over the vehicle and hit the pavement. Rizza stated the vehicle took off, but he was unsure where it went (Mass Pike, Newton Corner, or Washington Street). The medics looked at Rizza and he was alert and had a minor laceration to his head. Rizza's mother Domenica Rizza picked him up and brought him to NWH. I drove around the area and could not find a black Honda sedan with damage to the front of it. As of now, I do not have anymore information. This report is a hit and run. Traffic Update (Officer Gaudet): Myself and Officer Anderson have both canvassed the area of the crash for identifying physical evidence from the vehicle that fled the scene with a negative result. We have also reached out to local business's in an attempt to secure surveillance footage of the crash with a negative result. At this time we are unable to obtain any further information on the vehicle that left the scene of the crash.

Vehicle #1 was parked on the right side of the road and was unoccupied at the time of the crash. Vehicle #2 was attempting to parallel park behind Vehicle #1 and struck Vehicle #1 in the left rear with the right rear of Vehicle #2. A witness stated they watched Vehicle #2 hit Vehicle #1 in the attempt to park and then drove off after the accident leaving no information. The Witness left a note with Vehicle #2's information along with his information in regards to the accident. Vehicle #1 had moderate damage to the left rear and left side. Vehicle #2's damage is unknown at this time. I attempted to contact the owner of Vehicle #2 via the Boston Police with negative results. I was able to contact a relative of Vehicle #2 who refused to give me the drivers information but stated he would pass the message along to the driver. Ms.CORIOLAN, Roodline contacted me back and stated she was involved in an minor accident while attempting to park but stated there was no to little damage done. Ms. CORIOLAN was advised of the accident report that was filed. I contacted the owner of Vehicle#1 and advised him that I obtained all of Vehicles #2's information and informed him that it would all be in the accident report.

The OP. of MV#1 states he was in a parking space and trying to correct his position within the lines. He pulled out and then when he was about to drive forward, MV#2 suddenly reversed into him. The OP. of MV#2 states she was exiting the parking lot due to it being full. As she was driving forward, MV#1 pulled out of a spot and collided into her. There is supposedly a witness by the name of Jean Charles working for VPNE Valet that observed the accident. He has left for the day and does not have a cell phone or a house phone. Manager on duty states only corporate headquarters 617-451-1393 has information on him. I checked with hospital security for surveillance footage and observed MV#2 passing MV#1. MV#1 then started backing out of his space but MV#2 started reversing. Both vehicles ended up colliding with each other at an angle. MV#1 stated while driving his truck and trailer Westbound on Nahanton St across from the golf course the right top side of the trailer struck a City of Newton tree. MV#1 stated after hearing and feeling the collision he immediately stopped his MV. The collision caused part of the tree to break off and fall on top of his truck as well as into the Eastbound lane. MV#1 was traveling North on Fair Oaks Ave, and attempted to make a right turn onto California St. MV#2 was parked in front of 589 California St facing West. MV#1 appeared to have run over the curb at the intersection, hit a city tree in front of 55 Fair Oaks Ave (California St side), and then crashed into MV#2. When I arrived on scene I observed a male party with blood on his head, and another male party standing next to him, later identified as Elmer Lopez.. I asked Elmer which car he was driving, and he stated that he was in MV#1. I asked Elmer what happened, and he stated that he and the other man with him, identified as Ricardo Deras, were in the rear seat with another male party, and that there were two other parties in the front seat, one of them was driving, he was not able to provide me with a name of who was driving. I asked him where everyone else was, and Elmer stated that after the crash, the got out of the car and ran away. At this point, medics arrived on scene and began to tend to Ricardo. I next spoke with the owner of MV#2, Jim Silva. Jim stated that he was in his home when he heard a loud crash. Jim stated that he came out of the house in time to witness the two males I was speaking with looking at the car, and he also stated that he observed three parties walk away from the scene, two heading east on California St, and one heading north on Fair Oaks Ave. MV#1 and MV#2 were towed off scene by Todys. Due to the scene, and the location of the cars in a busy street, I was not able to do a MV inventory of MV#1 until after it was towed by Todys. The inventory was done at Todys Service station, and a MV inventory sheet was filled out and placed the in appropriate box. I took three pictures of the damaged tree and submitted them to IT to be attached to this report. While I was taking the pictures of the tree, two males were walking past the scene. I stopped both parties and questioned them about the incident. They stated that they had been in the car, but had not been driving. I asked them if the knew who the driver was, and they told me that they did not know the name of the driver. Due to no witnesses observing the driver and all parties stating that it was someone else, I was not able to determine a driver of the vehicle. Units checked the area for the fifth party in the vehicle with negative results. Ricardo was transported to NWH hospital by medics. All other parties were sent on their way. All units cleared without further incident. **UPDATE** On Nov 25, 2019 at approximately 1621 hrs, I contacted Westborough Police Department in an attempt to get a hold of Dora. Westborough informed me that they made an attempt to contact her with negative results. Westborough PD left my contact information for her to call me back.

On 11/26/19 at 14:11 hours, I responded to Newton South High School located at 140 Brandeis Road for a two car motor vehicle accident involving a school transport van for the City of Newton and another vehicle. No injuries were reported. The road conditions at the time were dry and the weather was a mix of sun and clouds. Traffic conditions were heavy due to the release of school at the time. Operator of MV# 1 states he was operating straight ahead eastbound on Brandeis Road awaiting the backed up traffic in front of him that was entering the right roundabout for the main entrance to the high school. While doing so, he states that MV # 2 came up and passed him on the left to go around the traffic. MV # 2 then clipped his vehicle on the driver's side front quarter panel while passing him. I observed no visible damage to the quarter panel area and scuff marks to the left front rim which possibly were from the accident. Operator of MV # 2 states she was also operating straight ahead eastbound on Brandeis Road awaiting the backed up traffic in front. She states that once there was an opening in the roadway ahead of her, she signaled her left directional and started to pass traffic on the left. While doing so, she says that MV # 1 suddenly pulled out to the left without signaling and struck her vehicle on the front passenger side. I observed minor scuff marks to the front passenger quarter panel area and slight damage to the metal grate footstep on the passenger side. The operator was also transporting two students for the City of Newton through JSC Transportation Services at the time. Both students attend Newton South High School. MV#1 was parked next to handicapped stalls by the front doors of 1 Boyston Street when it was struck by MV#2. -OpMV1 stated he left his vehicle next to the handicapped stalls by the front doors of 1 Boylston Street approximately 1630hrs. He stated he returned to the stall, approximately 1730, where he discovered his vehicle had been struck in the rear right panel. OpMv#1 showed me the pictures of the damage to the vehicle on his phone. He stated he wanted to get the video surveillance of the parking lot before reporting the incident. His vehicle was not towed from the scene and no other incidents matching his collision were reported to the police.

On 11/27/2019, at 1611 hours, I responded to the area of 69 Waltham St, Highlander Farms, for a two car hit and run accident. Upon arrival, I observed MV1 to have damage along the drivers side. The operator stated that she was traveling southbound on Waltham St when MV2, which was parked facing southbound in front of Highlander Farms, abruptly pulled out and hit her. The operator of MV2 then pulled a U-turn and fled northbound into Waltham. MV2 was described as a small light grey car with drivers side damage and a popped front right tire. The operator of MV2 is believed to be female. Waltham PD was notified and searched the area with negative results. There are no exterior cameras on any of the businesses in the area nor was I able to locate any witnesses. No injuries were reported and MV1 was driven from the scene.

The OP. of MV#1 was responding to a cardiac arrest emergency call. She was backing into an area close to the front door for the ease of loading patients. Emergency lights and back-up alarm were all in use. She didn't expect a vehicle to be illegally parked at a certain area while she was backing up, and also didn't see observe any lights on that vehicle. That's when MV#1 collided into the rear of MV#2. MV#2 was parked near the front door in order to help an elderly party out of the car and into the building. The owner, who was also the driver of MV#2 was not inside the vehicle when the crash occurred. Inside MV#2 were two passengers waiting for the owner to come back. Pictures were taken and left with IT Bureau.

Operator #1 states he was driving northbound on Seminary Ave in MA reg 5LE114 (vehicle #1), when he was t-boned on the driver's side by MA reg 959SD7 (vehicle #2). Operator #2 states that he was attempting to turn left out of the Arnow Campus Center parking garage and did not see vehicle #1 approaching from the right. It should be noted that the physical address of the parking garage appears to be 131 Woodland Rd, however the collision occurred on the Seminary Ave side. All parties declined medical attention and no tows were needed. Vehicle #1 sustained significant damage along the driver's side doors. Vehicle #2 has moderate front end damage. Operator #2 was issued Ma citation T1268432 in hand for Failure to Yield to Oncoming Traffic. 131 Woodland Rd will be the address used in documenting this crash.

MV1 stated his vehicle was parked in front of 192 Adams Street on 12/12/2019 unoccupied when it was possible sideswiped by a FLIXBUS. MV1 stated his dash cam recorded the incident showing a green and yellow colored Prevost bus with FLIXBUS in large lettering make contact with his vehicle. MV1 was unable to provide a plate number at the time but did state he saw what he believes to be the same bus the following day in the same area on 12/13/2019. MV1 was able to get a plate of MA APN REG:3799A, which is registered to United Leasing INC out of Indiana on a white 2017 PREO H345 58 passenger bus, insured by Safety Insurance. MV1 stated he already filed an accident report with the RMV and notified his insurance company. I attempted to make contact with the FLIXBUS company to get the drivers contact information but was only able to speak with a customer service representative in a foreign country who would not provide me with any information. I was able to locate an email address for the company and emailed a request for information but have not received a response. I advised MV1 of my investigation results and he was satisfied. MV1 stated the damage to his vehicle was minor.

Owner of MV 1 stated he parked his car legally on Commonwealth Ave across from the Post Office and left his vehicle to go into a store. Operator of MV 2 was parked directly behind MV1 and was sitting in her vehicle, when she states she observed MV3 park in the space directly in front of MV1. Operator of MV2 states she observed MV 1 shake as MV 3 tried parking and believes that the rear passenger end of MV 3 struck the front left bumper of MV 1. Minor damage sustained to MV1's front drivers side and no injuries reported. Owner of MV 1 did not observe the accident. The operator of MV 3 stated that he was traveling west on Commonwealth Ave and parked his MV in a legal parking spot. He and his passenger left his vehicle and when he came back, he was approached by the owner of the MV behind him, who stated that he believed his vehicle may have struck his while parking. Operator of MV3 stated he did not believe he struck MV2 because neither he or his passenger heard, saw or felt anything, but he couldn't be 100 percent sure. There was no damage to MV3.
