On Saturday March 9, 2019, at approximately 0230 hours, I, Officer Brandon, responded to the Weston/Newton line on Commonwealth Ave to assist Weston PD with field sobriety. On arrival Weston PD had concluded field sobriety and determined that the party, Glenn Matot, was not under the influence. Weston PD then explained that they had been contacted by AAA stating that a single vehicle accident occurred in Weston's jurisdiction. After further review, Weston PD and I determined that the accident was in fact in Newton. I then spoke with the operator of MV1, Glenn Matot, who stated that he was on his way home from work traveling westbound on Commonwealth ave in front of the Charles River Canoe and Kayak when he became distracted by a text message. Mr. Matot further explained that while texting, his car drifted to the right causing him to drive up on the snow bank and up onto the guard rail before then falling off back into the road. MV1 sustained major damage to both the passenger side and undercarriage. Mr. Matot did not sustain any injuries and denied any medical attention. AAA arrived on scene and removed the vehicle from the road and Mr. Matot took an Uber home. Two pictures were taken of the damaged City of Newton guard rail and submitted to IT to be attached to the report.

The owner of vehicle 1 arrived to the parking lot of the Boston Sports Club at 135 Wells Ave, at approximately 09:30 hours. He left the gym at approximately 15:00 hours, at which point he noticed that his vehicle (vehicle 1) had a white paint mark on the rear left side. There was a note left on vehicle 1 from an unknown witness, which provided the license plate of vehicle 2. The note stated that the person witnessed vehicle 2 hit vehicle 1. I was able to locate vehicle 2 at the owners residence. I asked them if they were aware that they hit a vehicle today at Boston Sports Club. They stated that they did not hit any vehicle. I asked if I could see the vehicle in their garage, and they stated I could. I noticed green paint on the right rear side of vehicle 2. The owner of vehicle 2 acted surprised, and stated that they must have hit a vehicle backing up. She stated her radio was on very loud, and did not hear or feel any collision. I advised her if she hits a car that she needs to leave her information or to locate the owner, and she stated she would have if she had known. I supplied the owner of vehicle 1 with all the information he needed to file a claim with his insurance if he would like. He was satisfied.

On Monday 4/8/2019 at approx. 1706 hrs, while assigned to N-494, I responded to the intersection of Park St and Tremont St for a 2 car MVA. On scene both operators said they were uninjured. Operator of vehicle #1 states that he was traveling WB on Tremont St and attempted to take a left onto Park St while observing a green light. He said that he saw vehicle #2 not moving with the operator "looking down" and "probably looking at her phone." He then began to turn left but then he said that vehicle #2 suddenly moved forward and they collided. Operator of vehicle #1 believes he had his left turn signal activated. Operator of vehicle #2 states that she was on Park St, facing SB, about to turn left onto Tremont St. She said that prior to the accident that she was looking at her "Waze" app for directions on her phone just prior to the accident. She believes that she was moving "slowly" before the collision.

On 4/14/2019 at approx 0923Hrs while assigned to 497 I responded to the area of Commonwealth Av and Ash St for a report of a three car crash. upon arrival I observed three cars off to the side of the road. i spoke with the operator of ma Reg T6T, Robert MURPHY who stated he had stopped for the red light at Commonwealth and Ash St when the operator of TN Reg 2N37P1, Jennifer BURNELL failed to stop striking the operator of Ma Reg 8CL834 Evan GUIDRY who in turn than ran into the back of MURPHY. I spoke with BURNELL who stated she was lost and had been looking down onto her phone looking for a map when she ran into the back of GUIDRY. BURNELL cited for violation MGL90/13 distracted Operation , GUIDRY transported to NWH by Cataldo Ambulance for neck and head pain. GUIDRY and BURN ELLS vehicles towed by Todys. NFD responded and cleared roadway or debris and fluids.

I responded to the intersection of Crescent Ave.@Beacon st for a two car MVA at approx. 1343 hrs 04/25/2019. V1; MA reg 8738ES, 2010 toyt/Camry color gray operated by Kuller, Robert A. MA OLN S40421474. V1 operator was attempting to turn left onto Beacon St from Crescent Ave. There was on coming traffic on Beacon St. V1 operator who had been trying to make the turn was sticking out onto Beacon st. V1 operator put his vehicle in reverse and proceeded to back up for the oncoming traffic. V1 collided into V2 which was directly behind when he backed up. V2; MA Reg 6PA747, 2017 Jeep/Util. color white operated by Mesen, Alvarado MA OLN S31551122. V2 Operate's account of the collision was consistent with V1 operator without dispute. There was minor damage to both vehicles. Neither operator was injured as a result of the collision. No airbag deployment. Neither vehicle required towing service.

MV1, a 1999 Ford Explorer, MA Reg: 7ZT136, collided with a tree on city property just outside of 134 Mt Vernon St. Operator, Jarrod Stomberg, immediately told me that he looked down at his phone to make a phone call and in that moment, ran off the road and hit the tree. There was airbag deployment. Jarrod signed a refusal with the medics and was driven home by a family member. He was issued in hand MA Citation T1441780 for 90/13B - Texting While Driving. The vehicle was towed by Tody's Towing. Pictures were taken of the scene and placed into ITs box.

Operator of MV1(Ma reg 866MF0) stated he was traveling EB on Washington St when his vehicle was struck in the rear. Operator of MV1 stated he was stopped at the red light at the intersection of Jackson Rd. Operator of MV2(AMB1406) stated he was traveling EB on Washington St when he struck the rear of MV1. Operator of MV2 stated he observed a vehicle coming down from Lewis Terrace and was concerned they were not gonna stop at the intersection. This distraction caused MV2 to strike MV1 at a slow rate of speed. Both vehicles sustained minor damage and did not require a tow. The passenger and driver of MV1 were observed by the medics. The passenger was transported in the ambulance for undisclosed minor injury. The driver refused to sign the medical release. 5 Photos were taken of both vehicles showing the damage. I put the memory card in an envelope and left it for the I.T. Bureau.

Mv#1 operator was travelling on Commonwealth Ave E/B in the area of #32. #1 operator was distracted by his ringing cell phone on the front seat floor. While reaching down to retrieve it, #1 rear ended Mv#2 (postal truck) that was parked near the side of the road while operator was conducting mail business inside the truck. No injuries reported by #1 or #2. #1 sustained front end damage that required tow service by Tody's. #2 did not require a tow.

Operator of MV#1 stated he was driving southbound on Woodland St when a bug flew in his eye. He lost control of the vehicle and swerved into a telephone pole. Operator declined medical attention. Major damage to the front and right side of the vehicle. Vehicle towed by David from Tody's. Verizon notified for pole 475/7.

MV1 was parked in front of #96 McCarthy Rd. MV2 is a City of Newton Forestry truck that was occupied and parked behind MV1 waiting to move up to a work site. MV2 began moving forward towards the site and struck MV1 from behind. Operator states that he was distracted by repositioning his seat and forgot that MV1 was parked in front him. There was a scratch to MV2 and damage to the rear of MV1. Pictures were taken by me and TOT IT.

Based on the witness statement of Yehor Shumerko, the operator of MV1 rolled down Eliot Street subsequently backing her vehicle into the foundation of 330 Eliot Street. Both tenants of the home then stated that they exited the home and when attempting to speak with operator she appeared distraught and begged them not to call the police before then leaving the scene without providing any information. Both occupants of the home were uninjured. Fire responded and determined that the home was safe for the time being. Wellesley PD attempted to locate the owner of the vehicle at her residence with negative results. Photographs of the scene were taken and submitted to IT to be attached to this report. Massachusetts Uniform Citation T2079291 was issued via mail to the operator of MV1 for 90-24, Leaving the scene of property damage. Approximately 1 hour later Officer Guarino located the operator of MV1 several streets away on Columbia Ave. She stated that she was smoked a cigarette when he vehicle began to roll backwards and struck the house. The operator of MV1 also stated that she did not believe there was any damage to the home which is why she left despite major damage to her vehicle. Todys responded to the scene and towed the vehicle.

At approximately 0147 hours I was dispatched to the area of 250 Waltham St for report of an MVA. Upon my arrival I spoke with the involved parties. The operator of vehicle 1 stated that as she was traveling southbound on Waltham St she noticed a vehicle approaching at a high rate of speed from her rear. This vehicle tailgated her for a short time, until she attempted to pull over to the side of the road to let the vehicle pass. As she was signaling her desire to do so, she was struck in the rear by vehicle 1. The operator of vehicle 2 stated a similar account, and added that he was not paying attention prior to striking vehicle 1 in the rear. All parties declined medical attention, and both vehicles were able to drive away from the scene safely.

The operator of MV#1 stated that she was traveling west on Washington St in the area of #993 when she was distracted by her phone and struck MV#2. MV#2 was parked and unoccupied in front of #993 Washington St. MV#1 towed by Todys, no injuries.

Operator of MV1 (Mass. reg. 523SE3) states she was traveling north bound on Dedham Street and was stopped in the left turn lane waiting for the traffic light to turn green. Operator of MV1 states she was rear ended by MV2 while waiting for the light to change. Operator of MV2 (Penn. reg. JZE8629) states she was traveling north bound on Dedham Street behind MV1. Operator of MV2 states she looked at her phones GPS for directions when she rear ended MV1.

OPER OF MV#1 WAS STOPPED ON DEDHAM (EB), MAKING A LEFT TURN ONTO COUNTRY CLUB ROAD (SB). OPER OF MV#1 STATED THAT SHE HAD HER LEFT TURN INDICATOR ON. OPER OF MV#1 STATED THAT SHE WAS THEN STRUCK IN THE REAR BY MV#2. OPER OF MV#2 STATED THAT HE WAS TRAVELLING EB ON DEDHAM ST (AT COUNTRY CLUB ROAD), WHEN A CAT RAN ACROSS THE ROAD IN FRONT OF HIS VEHICLE. OPER OF MV#2 STATED THAT CAT DISTRACTED HIM, AND HE THEN STRUCK THE REAR OF MV#1. MV#1 SUSTAINED MODERATE REAR DAMAGE. MV#2 SUSTAINED MINOR FRONT DAMAGE. NEITHER VEHICLE WAS TOWED AND THERE WERE NO INJURIES.

Mv#1 was travelling on Kenwood Ave S/B, veered off the road to the right and struck a city light gas pole in front of #11 Kenwood. #1 operator stated she got distracted by admiring the blooming flowers on a tree across the street, taking her eyes off the road resulting in the collision with the pole. Pole broke in half near the middle of it. NFD was able to shut the gas off. National Grid was notified to respond. City was notified of the pole damage. Mv#1 appeared to sustain light damage to the front passenger side front end. I took photos of pole damage. Camera memory chip with photos placed in the IT bureau mail slot.

On May 15, 2020 at approximately 22:26 hours Officer Stake and I responded to 163 Suffolk Rd for a hit and run that had just occurred. Upon arrival, we observed MV1,a black Cadillac Escalate with heavy rear end damage parked facing westbound in front of 163 Suffolk Rd. MV1 also was had damage to the passenger side due to it being pushed into a utility pole. MV1 was parked and unoccupied at the time of the crash. I spoke with the residents of 170 Suffolk Rd, who said that they heard the crash and went outside and saw a dark colored Jeep Wrangler leaving the scene at a high rate of speed. They described the Jeep Wrangler as a "newer" model, possibly a four door with a hard top. The residents of 163 Suffolk Rd have a camera system and will view the footage at a later time. Units checked the area for the Jeep Wrangler but were unable to locate it. Todys's responded and removed MV1 from the scene. **Update by Ofc. Hamann** At 0600 hours, Dispatch advised me that a male party that was involved in an accident last night (5/15/20). Brett Catlin stated that he was driving home from CVS last night and admitted to looking down at his phone and then had crashed. He stated after he crashed he drove to his home, which is 4 houses down. He stated he had called the owner of the vehicle and apologized and stated he was going to report it in the morning. I advised him that there will charges pending.****

Operator of MV1 states he was driving northound on Walnut Street when he approached the intersection with Linwood Avenue when MV2 failed to stop at the stop sign and collided into MV1. Operator of MV2 states he was driving eastbound on Linwood Avenue when he approached the intersection of Walnut Street. He states he thought it was a four way stop sign. Operator of MV2 states he was distracted and proceeded through the inersection without yeilding to MV1.

On June 13, 2020 Officers responded to 129 Parker St for a report that a vehicle crashed into a parked car. Upon arrival we spoke with the operator of MV1 who was being evaluated by medics. The operator of MV1 declined further medical attention for leg and back pain. The operator of MV1 informed me that he was traveling southbound on Parker St. when he became distracted by his cell phone GPS falling on the floor. He went to retrieve the item and struck the rear of MV2 which was parked in front of 129 Parker St. The vehicle owner of MV2 was on scene and was advised of the situation. Due to the damage, both vehicles were towed from the cent by Tody's towing.

On Sunday 8/2/20 at approximately 701 hours while assigned to marked unit n499 I was dispatched to the rear of the Lifetime Center (300 Boylston Street) for a single vehicle motor vehicle crash. Upon arrival I observed a Black Honda CRV with significant front end damage next to a dumpster in the shoulder of Florence Street behind the Lifetime Center. Dumpster was placedt there for the ongoing construction at the Lifetime Center. I spoke with the operator of the CRV, LYSGAARD Ingrid, who states she was frustrated by all the noise being made by the construction workers. Ingrid states she drove to the site to take pictures of the workers whom she believed to be working in violation of Newton's city noise ordinance. She pulled over to the side of the road to take a picture with her cell phone, but didnt realize her foot came off the brake pedal causing her to strike the dumpster. Fire and Medics also responded to the crash and ingrid signed a patient refusal after being evaluated by the medics Tody's responded and towed the vehicle. I stayed on scene until the vehicle was removed and Ingid's son came to transport her hiome.

ON 8-4-20 AT APPROX. 1307HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT. UPON ARRIVAL AT THE INTERSECTION OF WASHINGTON ST AND HARRINGTON ST. I SPOKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES HE WAS TRAVELING W-BOUND ON WASHINGTON ST. HE LOOKED AT HIS G.P.S. AND WHEN HE LOOKED UP HE WAS UNABLE TO AVOID HITTING VEHICLE #2. DRIVER OF VEHICLE #2 STATES HE WAS STOPPED AT THE STOP SIGN AT HARRINGTON AND WASHINGTON. HE STATES WHILE STOPPED HE WAS HIT IN LEFT FRONT SIDE BY VEHICLE #1. VEHICLE #1 HAD EXTENSIVE FRONT END DAMAGE AND WAS TOWED BY TODYS. VEHICLE #2 HAD EXTENSIVE FRONT END DAMAGE AND WAS ALSO TOWED BY TODYS. DRIVER #1 REPORTS NO INJURIES. DRIVER #2 HAD A BLOODY NOSE. BOTH PARTIES WERE EXAMINED BY MEDICS AND SIGNED PATIENT REFUSAL. WHILE CRASHING THE VEHICLE MOTION TOOK THEM ONTO THE SIDEWALK AT THE CORNER OF HARRINGTON AND WASHINGTON. OWNER OF 45 HARRINGTON REPORTS DAMAGE TO HIS FRONT BUSHES AND A SMALL TREE. THERE WAS A CITY STREET SIGN THAT GOT BENT DURING THE CRASH. PICTURES TAKEN BY N522 ( OFF MCSWEENEY ) AND TURNED OVER TO I.T. BUREAU. BOTH PARTIES WERE ADVISED TO CONTACT THEIR INSURANCE AGENCIES.

MV#1 was traveling southbound on Walnut St. when the operator of MV#1 reported she became distracted while retrieving an item from the passenger seat. This inattention caused MV#1 to collide into the rear of MV#2 which was parked (unoccupied) facing southbound on Walnut St. MV#1 sustained heavy damage to its front end, while MV#2 sustained heavy damage to its rear end. Both vehicles were towed from the scene by Tody's Towing Service. The operator of MV#1 declined medical attention.

MV/1 stated she was stopped on Washington St. waiting to turn left into Woodland Golf Country Club. While waiting to turn left MV/1 was struck by MV/2. Mv/2 had struck the right rear bumper of MV/1. The driver of MV/2 stated that he was heading East bound on Washington St. when he was cut off by another driver causing him to stear to the left and strike MV.1 with the right front corner of his bumper. Both MV/1 and MV/2 stated there was heavy amounts of traffic.

The witness stated she was travelling southbound on Lexington St a few vehicles behind MV#1 and observed MV#1 veered towards the northbound travel lane and striking the city tree and retaining wall of 106 Lexington St. The witness then pulled over and assisted the operator of MV#1. The operator of MV#1 stated she was travelling southbound on Lexington St and was "multi tasking" inside her vehicle when she veered off into the northbound travel lane, jumped the curb onto the sidewalk and got wedged in between a city tree and the retaining wall of 106 Lexington St. The operator of MV#1 was not injured but could not exit her vehicle because the doors were wedged in between the tree and retaining wall. MV#1 sustained heavy front end damages, broken front passenger window, front passenger wheel and deployed driver's side airbag. The operator of MV#1 stated she was not injured and was evaluated by Newton Paramedics and signed a patient refusal. The operator of MV#1 was coherent and appeared to be of sound mind and health. I transported the operator of MV#1 to her home and Tody's Towing took possession of her vehicle. Traffic unit 535 took photographs of the damaged city tree.

On, 11/19/2020 at approximately 1700 hrs, Newton Police and Fire were dispatched to Commonwealth Ave. and Prince St. for a reported head on motor vehicle collision with injuries. Upon arrival, I observed MV#1 ( 1WDR15) facing eastbound in the westbound travel lane. MV#2 (986KW1) was also facing eastbound, but in the eastbound travel lane. Both vehicles had significant front end damage, consistent with a head on motor vehicle collision. The operator of MV#1 was sitting on the grassy median upon my arrival. She stated she was traveling eastbound on Commonwealth Ave. when she became distracted by the radio, crossed over the double yellow line into the westbound lane of Commonwealth Ave. and collided with MV #2. The operator of MV#2 was entrapped in her vehicle, but was able to be extracted from the vehicle by Newton Fire personnel. Operator #2 was able to inform me that she was traveling westbound on Commonwealth Ave. She noticed MV#1 cross over the double yellow line at a high rate of speed, and collide into her vehicle. The operator of MV#2 was conscious and alert and transported to Tufts Medical Center Boston for an evaluation. The operator of MV#1 declined medical attention and was given a Massachusetts Uniform Citation (T2014484) for violating MGL. Ch. 89. Sec. 4(a) Marked Lanes Violation. Both vehicles were towed away from the scene by Tody's Towing Service. Photographs of this collision were taken on the (N492) camera and will be attached to this report.

On 12/10/20 at approximately 13:23 myself along with other Police, Fire and Ambulance units, responded to the intersection of Chestnut St. and Elliot St. for a two vehicle crash. Upon arrival I spoke with the operator of veh#1 who stated she was driving on Elliot St. due West when she proceeded through the intersection and was struck on her passenger side by veh #2. The operators 2 young children were in back of her vehicle in child seats. Her son was behind the passenger and her daughter was behind her. Both front and side airbags were deployed in veh #1. All three people in the vehicle were taken to Newton-Wellesley Hospital to be checked out as a precaution. The operator stated she had pain in her right wrist. The operator of veh#2 stated he was traveling due South on Chestnut St. when he looked down as he approached the intersection. The operator stated he was prepared to take a right onto Elliot St. but when he looked up he had already entered the intersection and struck veh #1. The air bags went off in veh #2 as the operator had cuts on his left hand . He was checked out by the Medics and signed a patient refusal. The operator of veh #2 had left the scene after I initally spoke with him but while I was attending to the occupants of veh #1. Because of this I was unable to issue him a citation on scene. I mailed him Mass. Citation #T2080850 for Ch 89/ Sec 9 Failure to obey the Red Light. Both vehicles sustained heavy damage and were both towed by Bobby and David from Tody's towing.

The operator of MV1 (Ma reg NETH42) stated she was traveling WB on Beacon Street when she approached the stop light at the intersection of Chestnut Street. Operator of MV1 stated she was stopped waiting for the light to turn when her vehicle was bumped from behind. The operator of MV2 (ma reg 6EGK30) stated to the operator of MV1 that she was sorry but had an appointment to get to. When I arrived on scene only MV1 was still at the intersection. The operator of MV1 got the driver's license information and vehicle information before she left the scene. The operator of MV1 informed me that the operator of MV2 wanted to pay for the damage but was late for an appointment. Medic Two was called to the scene for Operator one and the passenger. Both parties were complaining about minor injuries. Operator 1 stated she had back, head, neck and knee pain. Passenger in MV1 stated he had back pain. Both parties signed a medical release form. The damage to MV1 was minor and only on the rear bumper. Since MV2 was not on scene I was unable to determine the extent of damage or if the driver suffered any injury. I made contact with the owner of the vehicle who stated he let the operator of MV2 borrow his vehicle while he was working on her vehicle. The owner of MV2 owns an auto body shop. He provided me with the driver's telephone number where I was only able to leave a voicemail. After further investigation I cited (MA Citation T2080370) the operator of MV2 with Newton City Ordinance Failure to use care while stopping. I citation was mailed to the operator.

Vehicle #1 was stopped at the red light at the intersection of Washington St. where it meets Quinobequin Rd. and the Rt. 95 off Ramp. Vehicle #1 was in the middle westbound travel lane on Washington St. Vehicle #2 was traveling in the middle westbound travel lane on Washington St. when it failed to break in time and collided with the rear end of vehicle #1. Vehicle #1 sustained no visible damage to the rear end. Vehicle #2 sustained minor damage to its front end but was leaking large amounts of fluid. Vehicle #2 was towed from the scene by Tody's. No injuries were sustained in the MVA.

On February 1st, 2021 at approximately 07:53 hours while assigned to N491 I responded to MV crash in front of #41 Bridge St. On my arrival I observed a large white box style truck crashed into a gray SUV both vehicles with extensive damage blocking the street completely. I first identified the operator of the box truck, Maurizio Didonna. The truck belonged to Russo's & Sons out of Pleasant St Watertown Ma. Mr Didonna reported driving S/B on Bridge St when he temporarily got distracted before crashing into a parked vehicle in front of #41 Bridge St. The owner of the parked vehicle was not on scene but a male construction worker who was building a new house at #41 Bridge St identified the vehicles owner as Jose Ramirez. He was able to make contact with him and informed him about the accident. Both vehicles were towed from the scene. One by Todys. Russo's & Sons truck was towed by Pro Tech out of Watertown. Mr Didonna had a minor injury but refused any medical treatment. Note:The parked SUV had a lot of construction equipment in the rear but was removed by the construction workers from the job site at #41 Bridge St. The parked car was affiliated with the construction site. The vehicles owner was notified to where his vehicle was being towed to. .

On Monday May 31, 2021 at approximately 2152 hours I came upon a single MVA on Waltham St (a public way ). MV1 operator, who has an active license from Guatemala, stated he was operating southbound on Waltham St at Llewellyn Rd (public way). MV1 operator said after he passed Llewellyn Rd he may have been on his phone. MV1 operator said he struck the curb of the southbound side of Waltham St, and then struck the public tree. MV1's airbags were deployed. MV1 operator stated he was injured, and he was visibly bleeding from the nose. Newton medics transported MV1 operator to Newton-Wellesley Hospital. MV1 sustained extensive and total front end damage. MV1 was towed by Todys. The tree struck by MV1 did not appear to have sustained major damage and is not a safety hazard at this time.

The operator of MV1 states she was traveling southbound on Lowell Ave., following her GPS. Operator of MV1 states she must have looked at her GPS then struck a parked vehicle, MV2. Thus causing MV1 to roll over onto its roof, deploying both front and side airbags. The operator of MV1 was treated for minor cuts and scrapes then transported to NWH for an evaluation. Todys towed MV1. MV2 was legally parked and unattended, Owner of MV2 to be notified.

MV1 was traveling South bound on Winchester Rd. when an unknown vehicle of an unknown make, model and color passed MV1 on a double solid yellow line. Due to being distracted from the passing vehicle, MV1 side swiped the left side of a parked vehicle (MV2) in front of 115 Winchester Rd. The owner of MV2 was not in the vehicle at the time of the incident. The driver of MV1 stated she was okay and declined medical attention. Gas from MV2 was leaking from the vehicle and Newton Fire Department responded to clean the spill. Owners of MV1 and MV2 contacted their own Triple A and had their vehicles towed, due to both vehicles not being operable.

Vehicle #1 stated he was traveling southbound on hammond st. Vehicle #1 stated he was stopped at the red light and once the light turned green, he proceeded through the intersection when he witnessed Vehicle #2 speed into the intersection and he was unable to stop. Vehicle #1 struck Vehicle #2 in the rear right side. Vehicle #2 stated they were traveling westbound on commonwealth ave when they stated they had a green light and drove through the intersection. Vehicle #2 stated they were struck from the side by Vehicle #1. A witness who was in Vehicle #3 behind Vehicle #1 stated they were stopped at a red light on hammond st. and when the light turned green she witnessed Vehicle #2 blow the red light and was struck by Vehicle #1. The witness stated she and Vehicle #1 had a solid green light at the time they entered the intersection. At the time of the crash there were four parties inside Vehicle #2. After the crash Vehicle #2 left the scene after they thought Vehicle #1 had left and dropped off two passengers before returning to the scene of the accident. I spoke with the two passengers who were not present at the scene who both stated they were inside Vehicle #2 at the time of the crash and Vehicle #2 had a clear red light which they failed to stop for. One of these passengers was complaining of a head injury and was currently at a hospital being evaluated. Vehicle #1 had moderate damage to the front of the vehicle and was able to be driven away. Vehicle #2 had moderate damage to the right rear side and was able to be driven away. All drivers and passengers on scene were asked if they were injured which they stated they were not and declined any medical attention. Due to multiple witnesses stating Vehicle #2 failed to stop for the red light, a citation was issued to the driver for MGL 89/9 Fail to stop/red light on MA Uniform Citation T2080447

On 7/26/21 I was working N495 when I was traveling northbound on Chestnut St. at Pineridge Rd. I observed a 2 car MVA. I spoke with both parties who stated they were uninjured. The operator of Veh 1 Steven Pierce stated he was traveling southbound on Chestnut St. when a pedestrian attempted to cross the road. Pierce stated he slowed down to allow the pedestrian to cross when he was rear ended by Veh 2. I spoke with the operator of Veh 2 Rielly Fitzgerald who stated he was traveling southbound on Chestnut St. when he reached down to get his water. Rielly stated when he looked back on the road the vehicle in front of him had come to stop. Reilly attempted to come to a stop but collided with Veh 1. Both parties were provided a report number. Veh 2 was towed by Todys.

On August 15th, 2021 at approximately 09:55 hours I along with NFD and Fallon Ambulance responded to a report of a single car crash into a stone wall at #144 Chapel St. On my arrival I observed a 2006 Brown Honda Accord, MA REG. 93ES83 crashed into a wall at #144 Chapel St. The operator was located outside the vehicle sitting in a chair inside the parking lot adjacent to #144 Chapel ST. She identified herself as Cleausa Cobden, resident of #145 Chapel St,(across the street from accident scene). She stated she had just turned onto Chapel St heading S/B from Watertown St, heading home when she got distracted for a moment taking her eyes briefly off the road and the next thing she remembers was crashing into the wall. She sustained minor injuries to her ankle and some minor cuts to her arm. She later signed a patient refusal for Fallon Ambulance. Tody responded and towed the vehicle. The property owner for the wall was later determined to be the property of the US Postal Office, #326 Watertown St. The wall sustained no damage from the impact that I could see.

On September 14th, 2021 at approximately 09:22 hours while working N491 I responded to the area of #227 California St for a report of a MV crash. On my arrival I located both involved vehicles. Vehicle #1 was a 2000 Toyota Avalon XL, Ct reg AK76968, operated by a Molly Parrott. She stated she was going W/B on California St when she came to a flashing yellow light and slowed/stopped because she thought it was about to turn red. A few seconds later she was rear ended by vehicle #2. Vehicle #2 was a 2011 Brown Chevrolet. Malibu, Ma reg. 9JB522, operated by a Dion Tribbett. He reported travelling W/B on California St when he had to go around onto the wrong side of the road due to a truck blocking the west bound side of California St. Due to that distraction of him looking at the truck as he went around it he didn't see that vehicle #1 had stopped just a little further down the street. His vehicle then drove into the rear of vehicle #1 that had stopped by Jasset St/ Forte Park entrance. Both operators sustained minor injuries and both refused to be transported to the hospital. Vehicle #2 was towed by Todys.

On Monday 9/27/2021 at approx 1434hrs, while assigned to N-494, I responded to 127 Waverley Ave in Newton for a 2 car MVA. Operator of MV1 stated that he was traveling straight on Waverley Ave NB when MV2 came into his lane and struck his vehicle. When I asked the operator of MV2 how the accident happened he replied, "It was my fault... people were calling me and I looked down at my phone." He states he did not answer his phone. I observed damage to MV1 driver side wheel and substantial damage to the front driver side of MV2. Both parties reported no injuries. Both vehicles were towed by Todys Towing Service.

Operator 1 states he was stopped in traffic on Centre St, when he was rear ended by vehicle 2. Operator 2 states she was in stop and go traffic and was traveling northbound while "changing the radio" when she collided into the rear of vehicle 1. There was minor damage to the rear bumper of vehicle 1, and significant damage to the back right wheel well. There was heavy front end damage to vehicle 2, and the front airbag was deployed. Vehicle 2 was towed by Tody's Towing. Both operator 1 and 2 signed patient refusals with Fallon Medics.

Operator of MV1 states she was operating MV1 east on Washington Street in the West Newton rotary, near the off ramp from the Mass Pike. MV1 was in the 2nd travel lane. While operating MV1, the rear passenger, a child, was eating. The rear passenger was eating something spicy and the operator of MV1 thought they were choking. The operator of MV1 turned around to check on the rear passenger. MV1 then jumped the median, striking a traffic light pole, ultimately coming to rest in the 3rd travel lane. No air bag deployment and no reported injuries. Todys towed MV1. Mass DOT notified for th etraffic light and pictures taken and submitted to IT.

On 10/19/21 at approximately 14:55 I responded to a two vehicle crash on Commonwealth Ave near Lowell Ave. The operator of veh #1 stated he was headed due West and stopped on Commonwealth Ave at a red light when veh #2 crashed into the rear of his vehicle. Minor to moderate damage to veh #1. The operator of veh#2 stated he traveling due West on Commonwealth Ave when he looked down at his phone for a split second and struck veh #1. The front air bag went off in veh #2. Both operators were checked out by Medic#1 and signed patient refusals. Officer Gabriel spoke with the mother of operator #1 as he was a minor. He heard the patient refusal given by the mother over the cell phone to the Medics. Moderate to heavy damage to veh #2 as it was towed by Tody's Towing (Bobby). The operator of veh #2 was cited in hand Mass. #T2080858 for Ch90/Sec13B Using a cell phone while operating a m.v..

On Tuesday 10/26/2021, at approx 1225hrs, while assigned to N-498, I responded to 38 Winchester St for a 2 car MVA. There, I observed both vehicles involved pulled over to the side of the road. MV1 had heavy rear-end damage and MV2 had minor front end damage. Operator of MV1 states he was stopped in traffic when he was suddenly hit from behind by MV2. Operator of MV2 states that while operating his vehicle NB on Winchester St, he looked down at his phone for directions. When he looked up from his phone he noticed that MV1 had stopped but did not have time to stop his vehicle before crashing into the rear of MV1. Operator of MV1 was transported to NWH with minor injuries. MV1 was towed by Todys Towing service. Operator of MV2 stated he had no injuries.

Upon arrival, vehicle 1 was rolled over and facing Eastbound in the Westbound lane. There was extensive damage all over the vehicle and it was leaking fluids. It appears that vehicle 1 was traveling Westbound on Commonwealth Ave and struck a tree and then crashed into the guard rail causing it to roll over. There was damage to a city tree and damage to the guard rail. Operator 1 stated (through translation from Officer Acuna), he was traveling Eastbound on Commonwealth Ave at approximately 25 MPH when he was looking at his GPS and his brakes gave out causing him to crash. When asked again, his story changed and he was not sure what direction he was traveling in. Newton Fire was able to retrieve operator 1's cell phone in the vehicle which was on a loop playing videos on instagram. Operator 1 was transported to Beth Israel Hospital by Fallon Ambulance. Tody's towed vehicle 1 and pictures were taken of the damage to city property and will be submitted to the IT Bureau to be attached to this report. Operator 1 was served in hand, Massachusetts Uniform Citation: T1445611 for the following: Ch. 89 s. 4A - Marked Lanes Violation Ch. 90 s.13B- Electronic Device Use While Operating a Motor Vehicle

At approximately 0039 hours on November 6, 2021 Officer Guarino and I, Officer Joyce, were dispatched to a report of a hit and run. At the scene we spoke to operator of MV1. He stated he heard a loud bang and came out of his house and observed his car damaged. Officer Guarino and I observed multiple pieced of MV2 on the road. The parts came back to a blue Hundai Elantra. The operator of MV1 will be emailing me survalence video from his home security system of the incident. Multiple units checked the area and did not find a vehicle matching the description. UPDATE 11/6/2021 1730HOURS: Officer Nardelli responded to Spiers Rd. The operator of MV2 Stanley Joseph, returned to the scene and exchanged information with the opperator of MV1 Elton Matos. Stanley Joseph who was working for Uber Eats, admitted he was looking at his GPS before the collision. Stanley Joseph said he left the scene because he did not want his car towed because he needs it for work. He did not intend to avoid exchanging information.

At approximately 1425HRs on Wednesday, November 24, 2021, I was dispatched to a motor vehicle crash, which took place on Nahanton St., between number 293 and Winchester St. Upon arrival, I observed MV1 to be facing eastbound in the westbound travel lane. It had its passenger's side front bumper contacting the passenger's side front bumper of MV3 and MV2 was off of the roadway, to the left of MV1's front driver's side. MV1: I noted damage to its front driver's side bumper, head light, fender, hood area and flat tire. On MV1's front passenger's side, I noted damage to its bumper, head light, fender and hood area. MV2: I noted damage to its front driver's side bumper, head light, fender, hood area and flat tire. MV3: I noted damage to its front passenger's side bumper. Upon speaking with the operator of MV1, they stated they were traveling eastbound on Nahanton St. At this time, they began to retrieve something from their bag on the passenger's seat. They then crossed MV 1 over the center yellow divider lines and struck MV2 and MV3. Upon speaking with the operator of MV2, they stated the last thing they remember is that they were traveling westbound on Nahanton St. and then that their vehicle was traveling backwards; They do not remember the crash. Upon speaking with the operator of MV3, they stated they were traveling westbound on Nahanton St. when MV1 crossed over the center yellow divider lines at a high rate of speed. They observe MV1 crash into MV2, pushing it backwards and to the side. MV1 then continued forward and impacted MV3. The operator of MV1 received a ride from their father, after signing a patient refusal. Newton Medics transported the operator of MV2 to the hospital. And the operator of MV3 was able to drive their vehicle away, after signing a patient refusal. Both MV1 and MV2 were removed by the scene by Todys Services (NPD Towed Motor Vehicle Form Filled). I cleared without further incident.

MV1 was traveling westbound on Randlett Park and struck MV2 which was parked in front of 135 Randlett Park. There was damage to the front passenger side of MV1 and there was damage to the driver side front and rear bumper along with damage to both doors on the driver side. Operator of MV1 admitted to texting and driving. Both MV1 and MV2 were towed away from the scene and operator of MV1 was handed Massachusetts Uniformed Citation T1446550 in hand. Fallon Medics arrived on scene and operator of MV1 signed a patient refusal. MV1 was revoked for insurance I mailed Massachusetts Uniform Citation T1446551 to Jason Markle at 25 Linnet Street, West Roxbury. Jason Markle is the registered owner of MV1.

Operator of MV1 while proceeding W/B on Nahanton Street distracted by cell phone rear-ended MV2 while stopped at red light at Nahanton Street and Winchester Street. Operator MV1 and Operator MV2 both stated they were uninjured and declined EMS. MV1 and MV2 sustained no damage and were driven away. Operator MV1 cited on Massachusetts Uniform Citation T1447838 for 90/10-Unlicensed Operation on public way. Owner of MV1 Flavia Costa (S74550628) arrived on scene and took possession of MV1.

On March 13th, 2022 at approximately 15:10 hours while assigned to N491 I responded to the intersection of Washington St @ Hovey St for a report of a crash . On my arrival I located both involved vehicles parked on Hovey St. Vehicle #1 was a Red Subaru Legacy, Ma Reg. 1KH957, operated by a CHUN HE. He stated he was stopped for the traffic light(RED) at the intersection of Washington St @ Hovey St when he was rear ended by vehicle #2. Vehicle #2 was a 2012 Red Honda CRV Sport, Ma Reg. 52SH40, operated by a Nathaniel Baker. He stated he was stopped behind vehicle #1 on Washington St when his foot slipped off the brake and his vehicle drove into the rear of vehicle #1. There was little to no damage to either vehicle but both the operator and passenger of vehicle #1 complained of neck pain. Both refused to have an ambulance respond to the scene. Chun He and his passenger Lei ZHOA are the two injured parties.

The operator of MV#1 was traveling south bound on Centre Street coming up to the intersection at Ward Street when she rear ended MV#2 who was pushed into MV#3. MV#2 and MV#3 were stopped at the red light. There was major damage to the front of MV#1 and rear end damage to MV#2 and MV#3. MV#1 was towed by Tody's towing. The operator of MV#1 and passenger signed a medical refusal. There was no airbag deployment.

On May 30th, 2022 at approximately 10:33 hours, while assigned to N491 I responded to #276 Church St, the YMCA parking lot for a report of a MV crash. On my arrival I located both involved vehicles in front of the main entrance doors to the YMCA. Vehicle #1 was a White 2019 Toyota RAV4, Ma. plate 2VKZ97. Owner was identified as Tamer M. Tahoun. He stated he was sitting in his parked vehicle in front of the YMCA when vehicle#2 struck his vehicle as it was backing into a parking space behind him. Vehicle #1 sustained damage to its drivers side rear quarter. Vehicle #2 was a 2018 Green Subaru Forester, Ma plate, 433NT9, operated by a Ralph Gilbert. He stated he was attempting to back into a parking space behind vehicle #1 when he struck vehicle#1. His vehicle sustained damage to its passenger side front quarter. There were no reported injuries and neither vehicle required a tow.

On Sunday 6/19/22 at aproximaytely 11:48 hours while assigned to marked unit N-498 Officer Soloman was on a special assignment to assist with a procession at Mary Immaculate of Lourdes Church (280 Elliot Street) beginning at noon. While setting up a road closure by placing traffic cones on Linden Street Officer Soloman inadvertently struck a parked motor vehicle in front of 43 Linden Street. While attempting to go around a traffic cone the passenger side fender above the wheel well of N-498 struck the drivers side tail light of the parked motor vehicle. The collision caused the tail light to crack on the parked motor vehicle and there were only minor scratches to N-498. Officer Soloman left the location to assist with the event at the church, Sgt. Dempsey was notified. At the conclusion of the event Officer Soloman went back to 43 Linden Street to attempt to speak with the owner of the motor vehicle. The owner was not home, but Officer Soloman was able to speak with her husband, DWYER Michael, and inform him of what happened. Officer Soloman gave him all the pertinent information that he would need. Pictures were taken of the damage to the motor vehicle and N-498 and forwarded to IT.

Operator of MV1 (MA reg. 54W780) states she was traveling eastbound on Washington Street when she approached the traffic light which was yellow and turning red. Operator of MV1 states she came to a stop at the light and a short moment later was struck from behind by MV2. Operator of MV2 (MA reg. 673YZ4) states he was traveling eastbound on Washington Street behind MV1. Operator of MV2 states he was distracted looking back at the rear passenger in his vehicle. When Operator 2 looked back at the road he was unable to stop and struck MV1. All parties stated they were not injured at the time. I issued all motorists involved the proper information for insurance purposes. Operator of MV2 was issued MA. uniform citation (T0644896) for following too closely.
