Narrative:
On Sunday May 19th 2019 at 03:32, I responded to a single car accident at the intersection of Washington Street and Park Street. Operator of MV1 stated she was alone in the car and was coming from the Newton Corner rotary attempting to turn onto Park Street. She stated that she misjudged the right turn and went up onto the first median knocking down a crosswalk signal pole and the pole with the traffic signal and Park Street sign. She then went up onto the second median taking down another crosswalk pole and a street sign pole. Operator of MV1 stated she was fatigued and was not paying attention when the accident occurred. She was evaluated by Cataldo Medic 1 and signed a patient refusal. David Donahue of Todys Towing removed the vehicle from the accident scene. Large debris and the poles were placed onto the median and out of traffic. 494 and myself placed cones over each of the bases of the poles. Dagle and the City of Newton were notified about the damage and for the debris to be picked up in the morning. A total of 12 pictures of the damage was taken and turned into IT.

Operator of MV1 states he was traveling northbound on Lowell Ave when he hit MV2 in front of 490 Lowell Ave. Operator of MV1 states he was coming from work and only got 2 hours of sleep last night and must have "dozed off" and hit MV2. Operator of MV1 had minor facial and hand injuries, but signed a patient refusal with the medics. MV1 sustained air bag deployment, heavy front end damage and was towed by Tody's. Operator of MV2 was home during the crash and was made aware of the damage to her vehicle. MV2 sustained heavy rear end damage and towed by Tody's.

Narrative:
MV1 operator states he was driving Southbound on Parker St when he believes he may have nodded off. This resulted in his vehicle hitting two light poles and causing damage to a residential fence. Damage to the vehicle was observed all in the front. No injuries were reported by MV1 operator. A medical refusal was signed. Tody's was notified and took possession of the vehicle after arriving. A call went out to Eversource and Verizion to notify about the damage done to the two poles. Eversource employee responded and stated the poles were not in any risk of falling over. Verizon was left a message incuding the two pole numbers. The home owner of 84 Ridge Ave where the fence was partially damaged was left a message to call the NPD. Samuel Nurko was issued a citation(T1442789) for 89/4a(marked lanes violation). I then cleared without further incident.

Operator of MV1 states she arrived at The Sinclair restaurant in Cambridge with a friend at approximately 11:30pm and had 1 glass of wine. Operator of MV1 states she left at closing at approximately 2am to drive her friend home located on Mass Ave. Operator of MV1 states after dropping her friend off she traveled home to 41 Hyde St in Newton. Operator of MV1 states she took Lake Ave westbound heading towards Hyde St when she was fighting off sleep and must have passed out at the wheel and struck a city tree on the eastbound side of Lake Ave in front of 160 Lake Ave. Operator of MV1 states she contacted AAA at 3:37am, but they never showed up so she abandoned the vehicle and walked home. Operator of MV1 states she didn't sustain any injuries and refused medical attention. MV1 sustained heavy damage to the left front fender and was towed by Tody's.

On Saturday 6/27/20 at approximatly 0700 hours while assigned to marked unit n499 I was dispatched to the parking lot of 188 Florence Street (Heathwood Rehab) for a report of a motor vehicle that backed into a pipe comng from the building. Upon arrival I observed the vehicle had backed from a parking spot at 188 Florence Street into the rear building of 160 Boylston Street striking the gas meter. I could hear and smell gas coming from the meter and immediately cleared the reporting party from the area and requested the assistance of Sgt. McNeil, Fire Dept, National Grid and Maintenence from the Avalon (160 Boylston Street). Fire arrived on scene and was able to slow the leak of the gas and I was able to speak with the reporting party identified as SANFORD Troy. Troy states that he is an employee at 188 Florence Street and got to work a little early this morning. He states that he stayed in his car awaiting the start of his shift and must have fallen asleep with his foot on the brakle and the transmission still in reverse. When he fell asleep he states his foot must have come off the brake causing him to back into the gas meter of 160 Boylston Street. The impact not only damaged the gas meter, but also did damage to the rear center of Troy's vehicle. Through my observations of Troy's demeanor and actions he did not appear to be impaired in any way. Troy stated that he did not get much sleep last night and was tired. He also stated that he did not need any medical attention Fire remained on location awaiting the arrival of National Grid. After interviewing Troy and confirming the area was secured I cleared the location without further incident

The operator of MV1, Mr. Tong Zhi, stated that he was traveling west bound on Needham street when he fell asleep causing him to lose control of the vehicle and crash into a Verizon utility pole 305/17. The motor vehicle sustained major front end damage and full airbag deployment. The utility pole was snapped, and was leaning as a result of the impact. Mr. Zhi stated that he had wrist and chest pain due to the crash and was transported to Newton Wellesley Hospital by Cataldo. Todys arrived on scene and towed the vehicle. Verizon was notified regarding their damaged utility pole. Photographs of the crash were taken and place in the IT Bureau's mailbox. Due to the facts that Mr. Zhi was asleep at the wheel while traveling on a major public way in the City of Newton, that he crossed the double yellow line into the on coming travel lane, and subsequently crashed into a utility pole causing heavy damage, Mr Zhi is being charged with 90-24 Operating to Endanger. A Massachusetts Uniform Citation, #T2080141, was completed back at the station for the offenses of 89-4A Marked Lanes violation and 90-24 Operating to Endanger. The violator's copy of the citation was placed in the Chiefs mailbox to be mailed to the offender while the criminal application portion was placed in the court prosecutors mailbox attached to this report.

Operator 1 states that he was coming home from work traveling NB on Waverley Ave. He states that a raccoon was in the road and had to swerve to avoid it. MV1 had heavy front end damage after crashing into the gated entrance of 254 Waverley Ave. The owner of the gate and fence was notified of the damage. Pictures were taken and submitted to IT. Operator 1 was transported to NWH with minor injuries and his vehicle was towed by Tody's towing service.

On 09/13/2021, while assigned to N494, I, Officer Conary, and Officer Kane (N492), responded to 885 Centre Street for a report of a single motor vehicle accident with airbag deployment. Upon arrival, I observed MV1 up on the curb with significant damage to the front and front right side. Operator of MV1 explained to me that he just got out of work and was driving Northbound on Centre Street when he fell asleep. Next thing he knew he was off the roadway. He explained to me that he had an early morning at work, and this hasn't happened before. Medics arrived on scene and he signed a patient refusal. Tody's towed MV1. The City of Newton was notified because the crosswalk sign was taken down during the accident. There was also damage to a City tree. Pictures were taken and submitted to IT to be uploaded accordingly.

MV1 operator stated he was operating westbound on Watertown St by Dunstan St (both public ways). MV1 operator struck (side swiped) both the driver side's of MV2 and MV3 which were parked in front of 915 Watertown St. MV1 operator struck the curb in front of 919 Watertown St and left the roadway. After MV1 left the road way it hit and destroyed a fire hydrant owned and maintained by the City of Newton. MV1 operator then hit the private wall of 919 Watertown St and it was it's final resting location. MV1 sustained extensive and complete front end and under carriage damage, it was towed by Todys. MV2 sustained minor damage and paint transfer, no tow required. MV3 sustained extensive driver side wheel well damage, which will privately towed at a later time. MV1 operator stated he struck his head and was not feeling well; he was transported to Newton-Wellesley Hospital by Newton Medics. MV2 and MV3 were not occupied. The City of Newton was notified in regards to the fire hydrant.

On February 15, 2022 at approximately 0404hrs, Mv1 was traveling Southbound on Chestnut St., when the operator of the vehicle fell asleep at the wheel, causing her vehicle to hop the curb and strike a telephone pole and Federal Mailbox in front of 619 Chestnut St. Operator stated she was returning home late from a friends house and did not realize how tired she was and fell asleep, while driving. Newton Fire Department and Fallon Ambulance Service responded to evaluate the operator. Operator stated she way okay and signed a patient refusal. The telephone pole did not appear to have sustained any damage, but National Grid was made aware in order to later inspect the pole. The U.S. Postal Service was also made aware of the damaged postal box in order to replace it. The vehicle sustained heavy front end damage, consisting also of deployed air bags.Todys Towing Service was notified and arrived on scene to tow the vehicle. Pictures were taken of the scene and have been sent to the IT Bureau to be attached to this report.
