The operator of Vehicle #1 stated that his vehicle was struck from behind by Vehicle #2. Operator #2 then stated, "there is no damage", and left the area. Operator #1 then took down the registration of Vehicle #2, and notified the police. I observed significant fresh damage to the rear bumper of Vehicle #1. Vehicle #2 is registered to a Ms. Jessica Gavell. I was later able to learn that the vehicle was being operated by her cousin, a Mr. Michael Gavell. Mr. Gavell was issued a citation for a violation of M.G.L. Ch.90/24-Leaving the Scene of Property Damage, and Ch.90/23-Operating MV after suspension due to OUI. (See Incident#19005785). None of the parties involved complained of injury, when asked.

Operator of vehicle one, Keith DeVincent stated that on 04/09/2019 at approximately 10:00 hours he parked his vehicle ( MA reg 9WM464 2015 Nissan ) facing north on Colonial Ave across from number 28. DeVincent stated that he observed a white city of Newton pick up truck towing a trailer hit his vehicle's driver side mirror with the truck's passenger side mirror. DeVincent stated that the truck that struck his vehicle did not stop. Vehicle one driver's side mirror had minor damage. I was able to track down the city Parks and Recreation truck. Operator of vehicle two Ralph Annese stated that he was driving a city of Newton Parks and Recreation truck ( MA reg MVN M 95256 ) north on Colonial Ave on 04/09/2009 at approximately 10:00 hours . Annese stated that he did not realize that his vehicle struck DeVincent's vehicle . Vehicle two had no damage. I took photos of both vehicles and forwarded the disk to the NPD's IT bureau.

Vehicle #1 stated they were driving northbound on Cypress St. when her passenger accidently bumped into the cars shifter and put the car into reverse. Vehicle #1 stated they didn't notice and hit the gas and went backwards striking Vehicle #2 in the front. Vehicle #2 stated they were driving northbound on Cypress St. when the car in front of them stopped and then proceeded to reverse into the front of their vehicle. Vehicle #1 had minor damage to the rear of the vehicle and was able to be driven away. Vehicle #2 had minor damage to the front of the vehicle and was able to be driven. All operators and passengers stated they were uninjured and denied any medical treatment.

Opr of V1 stated he was pulling out of the parking spot onto Washington Street. At this time, he stated he thought he had enough room but did not and struck V2. V1 suffered significant damage to the front right side of the vehicle. V2 was parked in front of V1. The uninvolved owner was not around during the crash , but came to the car eventually. V2 suffered significant damage to the rear right side of the vehicle. No cars were towed and there were no injuries.

Emery Mays states that he took his fathers vehicle Ma. Reg 8BL949, this morning. Emery states that he got lost and ended up in the parking lot at 333 Nahanton St which is only accessible by the Jewish Community Center's access road and Nahanton Street (public way in the City of Newton). Emery then proceeded to do "doughnuts" in the muddied lot. Emery then lost control of the vehicle and struck a parked storage container causing minor damage. Both airbags were deployed. Emery does not have a license and was transported to NWH (as he is a juvenile and a parent could not sign a refusal in a reasonable time period ). He attends the Fusion Academy in Newton. I spoke with Emery's father Randolph Mays. I told Randolph of the accident. Randolph stated that he did not give Emery permission to take his vehicle. Randolph stated that he would meet Emery at NWH. I told Randolph that Emery was going to be charged wit Ch.90 Sec.10 unlicensed operation of a m/v, citation # T1272114 (in the mail due to his being at the hospital). I also advised that he could take out his own charges against Emery for using without authority if he chooses to do so. Todys towed the vehicle. JCC staff were notified of the incident.

MV1, a City of Newton contracted school bus, was parked on the side of the road. MV2 was driving east past the school bus and misjudged how close his trailer(Ma Reg D43490) was and struck MV1. There were no students on the bus. I took pictures and TOT IT. Operator MV2 was unlicensed and cited with Ma Citation T1269943 for 90/10. 2 licensed operators showed up and too possession of MV2.

On 05/27/2019, while assigned to N494, I, Officer Conary responded to 101 Madison Ave for a MVA. Upon arrival, I met with Operator of MV1 who explained to me that she was attempting to make a three point turn at driveway. When she began to drive away, she was too close to MV2 and hit the side. MV2 was parked legally on the side of the road. Operator of MV1 was offered and declined medical attention. Both MV1 and MV2 did not need a tow. Operator of MV2 was not on scene at the time of the accident. He called Newton Police Dispatch and received all information pertaining to the accident. No further incident to report.

On 06/07/2019, while assigned to N494, I, Officer Conary, responded to Centre at Church for a MVA. Upon arrival, I met with the operator of MV1 who stated that he was stopped at the red light headed Southbound of Centre Street when he was hit by MV2 on the left side. Operator of MV2 explained to me that she was traveling Southbound on Centre Street and was about to turn left onto Church Street from the left turn only lane. Operator of MV2 stated that she saw another car coming toward her going Northbound Centre Street was too close to her lane a travel. This caused MV2 to move into MV1 lane and hit MV1. MV1 had minor damage to the tail gate. MV2 had damage to the entire right side. Both vehicles were able to be driven from the scene. Both operators were offered and declined medical attention. No further incident to report.

VEH 1 EXITED BERTUCCI'S PARKING LOT, TAKING A RIGHT ONTO CENTRE STREET SOUTHBOUND WHEN IT MADE CONTACT WITH VEH #2 WHICH WAS PARKED ON THE SOUTHBOUND SIDE OF CENTRE STREET FACING SOUTH UNOCCUPIED. DISPATCH WAS UNABLE TO FIND A CONTACT NUMBER FOR OWNER OF VEH 2. I CHECKED SEVERAL NEARBY BUSINESS'S IN THE AREA WITH NEGATIVE RESULTS.

On 7/9/2019 at approx 1406Hrs while assigned to 497 I responded to level 4A of the Newton Wellesley parking garage at 2014 Washington St for a report of a minor collision. Upon arrival I spoke with the operator of RI Reg FV705, Malorie CURLEY who stated she had parked her new work vehicle in the parking stall was exiting her vehicle to see if she was within the lines when the operator of Ma Reg 69EL83 Laurence HENDY pulled into the spot next to her, opened his door striking her truck, CURLEY stated as she was rounding the car HENDY proceeded to grab his door and slam it into her car several more times. CURLEY stated she confronted HENDY who refused to exchange information with her for the damage to her car and fled. CURLEY took pictures which she will provide in her own crash report. The front bumper of CURLEYS car had several spots where the white paint was peeled back , HENDYS vehicle had the matching white paint on the edge of his drivers side door. Although the damage was minor CURLEY requested a report for documentation as it was her work vehicle. i attempted to contact HENDY but no phone number is listed.

On 8/6/2019 at approx 1214hrs while assigned to 497 I responded to 10 Windsor Rdfor a report of a hit and run. upon arrival I met with Lilia LEVITINA who stated her car , Ma Reg 536PRR was hit while she was in shopping. LEVITINAS car had damage to the front left . The owner of The Waban Market came out while I was standing there and told me he had no cameras that cover the front of his store.

The owner of MV#1 stated he came out to his vehicle and found a note that someone had struck it. There was damage on his passenger rear door of MV#1. I spoke with the operator of MV#2 who stated that MV#1 was parked too close to her vehicle and she opened her drivers door and struck the passenger side door of MV#1.

OPERATOR #1 STATED SHE WAS TRAVELLING N/B ON CENTRE ST WHEN VEHICLE #2 WHICH WAS PARKED ON CENTRE ST, OPENED THE DRIVERS SIDE DOOR STRIKING HER VEHICLE'S PASSENGER SIDE MIRROR. OWNER OF VEHICLE #2 STATED HE PARKED HIS TRUCK(FORD F250) N/B ON CENTRE ST NEAR BELLEVUE ST WHILE HE WAS WORKING(LANDSCAPING). HE OPENED HIS DRIVERS SIDE DOOR AND HIS DOOR STRUCK VEHICLE #1'S MIRROR AS IT PASSED HIS TRUCK N/B. THERE WERE NO INJURIES DUE TO THIS ACCIDENT AND NEITHER VEHICLE WAS TOWED.

OPERATOR OF MV 1 STATED SHE WAS TRAVELING OVER THE NEWTON CORNER ROTARY IN THE OUTER LANE TO CONTINUE WEST ON WASHINGTON STREET WHEN MV 2, (WHO WAS TRAVELING IN THE MIDDLE LANE ON MV 1'S LEFT) ENTERED THE OUTER LANE STRIKING MV 1 ON THE DRIVERS SIDE CAUSING MODERATE DAMAGE. NO INJURIES REPORTED AND NO TOW. OPERATOR OF MV 2 STATED SHE WAS TRAVELING IN THE MIDDLE LANE OF THE NEWTON CORNER ROTARY ALSO TRAVELING WEST ON WASHINGTON STREET. OPERATOR OF MV 2 STATED THAT MV 1 COLLIDED WITH HER VEHICLE AND SHE DIDN'T BELIEVE THAT HER VEHICLE LEFT HER TRAVEL LANE. NO INJURIES REPORTED AND HER VEHICLE WAS TOWED PRIVATELY. IT APPEARS AS THOUGH MV 2 DRIFTED INTO MV 1'S LANE DUE TO WHERE THE DAMAGE WAS ON BOTH VEHICLES AND WHERE THE POINT OF IMPACT WAS SHOWN TO ME.

On Thursday, August 29, 2019 while assigned to Traffic unit N525, I responded to 837 Beacon Street, Newton for a report of a motor vehicle crash involving a City of Newton Fire truck. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I met with the operator of MV1, Newton Firefighter Keive Mullings. Mullings stated he was operating Newton Fire Ladder 2(MA MF4951) Eastbound on Beacon Street towards Centre Street. Mullings stated the rear passenger side bumper area of MV1 made contact with a unoccupied motor vehicle parked Eastbound on Beacon Street in a metered spot across from 837 Beacon Street. No injuries were reported by the occupants of Ladder 2, I observed minor damage to the rear passenger bumper area of MV1. I spoke with the owner of MV2, Pakizeh Motiee (2017 BMW X3, MA: 7NCC90). I observed minor damage to the front driver side quarter panel area of MV2. Motiee was advised. Pictures of both vehicles were taken and submitted to the IT Bureau.

OPERATOR#1 REPORTS THAT SHE WAS GOING S/B ON CRAFT ST ATTEMPTING TO MAKE A LEFT TURN ONTO NORTH ST WHEN VEHICLE #2 STRUCK HER VEHICLE IN THE INTERSECTION. VEHICLE #2 CONTINUED N/B ON CRAFT ST WITHOUT STOPPING. VEHICLE WAS DESCRIBED ONLY AS A SILVER SUV WITH FRONT PASSENGER SIDE DAMAGE. DUE TO THE SUN GLARE OPERATOR #1 WAS UNABLE TO GET A BETTER DESCRIPTION. OPERATOR THOUGHT SHE LATER SAW THE INVOLVED VEHICLE STOPPED ON CRAFT ST AFTER SHE HAD TURNED ONTO NORTH ST THEN PULLED ONTO ALBEMARLE RD. THERE WERE NO CALLS ABOUT THIS ACCIDENT LOCATION OTHER THAN FROM OPERATOR #1. WALTHAM PD WAS NOTIFIED AND GIVEN THE LIMITED INFORMATION PROVIDED. OPERATOR #1 REPORTED NO INJURY AND HER VEHICLE WAS NOT TOWED.

Vehicle 1 was parked facing eastbound in a parking stall located in the eastbound lane of Washington St in front of 1286 Washington St. Vehicle 2 was attempting to parallel park in a parking stall located mediately in front of Vehicle 1. While backing Vehicle 2 backed into the front bumper of Vehicle 1. The operator of Vehicle 2 then left the scene without leaving any information. As a result of the impact Vehicle 1 sustained minor damage to the center of its front bumper. The operator of Vehicle 1 stated he was sitting in Vehicle 1 while parked when Vehicle 2 backed into his front end. The operator of Vehicle 1 stated that after the collision Vehicle 2 left the area without leaving any information. The operator of vehicle 1 described Vehicle 2 as a red Honda Accord bearing MA Reg 365KH6. The operator of Vehicle 1 described the operator of Vehicle 2 as a white female approximately 70 years of age with blonde/white hair. The operator of Vehicle 1 states that Vehicle 2 had minor damage to the drivers side rear bumper. No injuries were reported on scene. No vehicles were towed. An RMV check revealed Vehicle 2's registered owner as Joan Goldman of 53 Bartlett Way Apt 205 Waltham, MA. I was able to contact Goldman over the phone. Goldman identified herself as the operator and owner of Vehicle 2. Goldman states that while she was backing into a parking stall she was going very slow. Goldman states she did not feel any impact and left the scene because she did not think she collided with Vehicle 1. Goldman states she was not injured. Goldman was provided with the information of Vehicle 1 and was advised to contact her insurance company. I also called and provided the owner of Vehicle 1 with the information of Vehicle 2.

On Friday, September 13, 2019 at approximately 16:19 hours, while assigned to N496, I responded to 162 Warren St. for a report that two boys were hit by a car earlier in the day. Once on scene I spoke with Wes Moore and Luca Toro (both attend Oak Hill Middle School), as well as their mothers Jamie Moore and Alyssa Toro. Wes and Luca explained that at about 15:15 hours, they were crossing at the crosswalk adjacent to the Bank of America in Newton Centre (808 Beacon Street), heading towards the Newton Centre municipal parking lot. The boys stated that as they crossed a large van, which was traveling westbound on Beacon Street in the left turn lane, stopped to let them cross. Wes and Luca stated that they did not realize that there was another westbound travel lane, and after passing the truck they were both hit by an unknown sedan. Wes and Luca stated that the driver, who they described as an approximately 50 year old white female, got out of her car and started yelling at them to be careful. Wes and Luca stated that the unknown female operator checked to see if they were hurt and then left the area heading westbound on Beacon Street. Wes and Luca stated that they both fell to the ground as a result of being hit by the unknown motor vehicle, but only sustained minor injuries. I offered to have an ambulance respond to evaluate Wes and Luca, however, Mrs. Moore and Mrs. Toro declined. I advised the boys and their mothers that I would be documenting this incident with a crash report, and that I would head to the area to see if Bank of America had and cameras facing the crosswalk, so that we could hopefully identify the operator of the unknown motor vehicle. I then responded to the Bank of America, however, the bank was closed for the day. Supplement by Officer Ferguson: On Monday 09/16/2019 at approximately 0900 hours I responded to the Bank of America and spoke with the employees who stated they do not have any cameras that record the street or sidewalk areas. The camera they have only records the front door and lobby area.

M/V 2 was stopped on Homer Street (East Bound) at the intersection of Homer and Centre Street, she was stopped at the stop sign. The uninvolved wittiness was stopped (North Bound) on Center Street with his left directional on waiting to make a left turn. Operator of M/V 1 traveling (South Bound) on Centre Street slowed while driving, the vehicle did not have any headlights on when the operator flashed her high beam twice indurating she was letting the stopped vehicles waiting to turn to proceed and make their turns. When the operator of M/V 1 flashed her high beams twice Operator of M/V2 thought the operator of M/V 1 signaled her to proceed and made the turn (North Bound) onto Centre Street. Operator of M/V 2 started into the intersection and was hit by M/V 1 who did not stop. While writing this report it was discovered that the operator of M/V 2 had an expired Drivers License (Straight Expiration) as of 09/15/2019. Citation was issued.

Mv#1 was travelling straight ahead on Centre St N/B in the left turn lane approaching Walnut St. #1 was passing Mv#2 and Mv#3 which were on Centre St N/B in the right lane preparing to proceed straight ahead. As #1 was passing, the leaf blower hanging off the side of it's trailer sideswiped both #2 and #3. Both #2 and #3 sustained driver's side mirror damage/contact and driver's side rear quarter panel damage.

MV#1 was parked and unoccupied at the time of the crash. The operator of MV#2 stated that she was attempting to back out of a parking space in front of #339 Lexington St when she accidently stepped on the gas instead of the brake. MV#2 then struck MV#1 causing significant damage to the front of MV#1. No tows, no injuries.

Mv#1 was travelling on Beacon St W/B (opposite #808) in the left lane. #1 started passing Mv#2 travelling in the right lane. #1 made a lane change into the right lane colliding with #2. #1 passenger side struck #2 driver's side front end. #1 wedged itself into #2's front end and had to be towed out by Tody's to avoid any more damage. Witness stated she observed #1 make a sudden lane change to the right crashing into the side of #2. #1 cited for 90/14 Improper lane change. Both vehicles could be driven away by the operators.

On Monday, November 4th, 2019 at approximately 1557 hours I, Officer Newton and Officer Healy responded to the municipal parking lot at 121 Adams St for a motor vehicle accident. Upon arrival I spoke to the operator of vehicle 1. She stated she was sitting in her parked car when vehicle 2 backed out of his spot and struck the front of her car. I observed fresh damage to the driver's side bumper. The operator of vehicle 2 told the operator of vehicle 1 he would be right back. Vehicle 2 then left the parking lot and never returned. It should be noted that this vehicle was involved in another collision after this one. The operator of vehicle 2 stated he did not remember this accident when we asked him. As a result of my investigation, Mr Usher was issued Citation#T2079940 for a violation of M.G.L.Ch.90/24-Leaving the Scene Property Damage. Based on the totality of the circumstances, an Immediate Threat Request was faxed to the RMV, see Incident number 19047444.

Operator #1 reports going N/B on Eastside Parkway when he struck a parked car on Eastside Parkway at Hilltop St. Operator further reported there was another truck coming from the opposite direction so he pulled a little to his right to make room but the rear of his truck caught the parked vehicle. Note anonyomous worker reported vehicle #2 was parked illegally partially blocking Hilltop and Eastside Parkway which probaby led the truck hitting that car. Vehicle #2 was not at that locaton when I arrived on scene. Operator #2 reports her vehicle was stopped/parked at the intersection of Eastside Parkway and Hilltop St when her vehicle was hit by vehicle #1. She was stopped attempting to locate a parking space. No tows and no injuries.

MV1 was turning left from Tamworth Rd onto Chestnut St, traveling North when MV 2 struck the right side of MV 1. The operator of MV 1 stated that the heavy congestion of traffic and rain obstructed her view when she took the turn. MV 2 was traveling Northbound on Chestnut St and struck MV 1's right side when MV 1 attempted to turn left onto Chestnut St. MV 1 was unable to see MV 2 when making the turn due to weather and the high congestion of traffic. There were no injuries to either party.

MV#1 was traveling south on Commonwealth Ave as it was it was merging it collided with MV#2. - OpMV#1 stated he was traveling south on Commonwealth Ave as the road was about to merge from two lanes into one. He was on the left most lane, when he observed a black car come up beside his and tried to get in front of him. OpMv#1 stated he did not know about the vehicle until it collided with his front tire. -OpMV#2 stated he was traveling south on Commonwealth Ave as the road was about to merge from two lanes into one. He was on the right most lane, where he observed a large white landscaping truck merging into him.OpMv#2 felt the collision with the truck's front tire. - I conducted an inspection of both vehicle. I observed no damage to the front tire or the right side of MV#1. I did observe small scratches on the right rear passenger door of MV#2 that resulted from the collision with MV#1. - Both Operators moved off the road in front of 18 Old Colony Rd to safely handle the collision matter. There were no injuries reported at the time of this report. Neither vehicle was towed from the scene and OpM#1 was cited for Operating After Suspension (T2079504). See report19050505 for criminal application. MV#1 was left legally parked in front of 18 Old Colony road waiting to be picked up by another company employee. OpMV#1 took an Uber home from the scene.

Based on observations and statements made, the following occurred. M/V#1 was parked facing south on Chestnut St. awaiting to pull a M/V from an unrelated accident scene, with his yellow hazard light bar activated. A Verizon truck was parked facing north in the northbound lane of Chestnut St. and through traffic was being routed up the westbound side of Amherst St. M/V#2 approached traveling south on Chestnut St., and stopped approx. 100 feet away from M/V#1. It then began to slowly drive up to M/V#1 and attempt to pass between the two parked trucks, at a slow speed, when the right fender of M/V#2 struck the towing rig of M/V#1. The right fender of M/V#2 had damage along the right front wheel well. I was unable to see the extent of the damage to the tow rig. I went to speak to the operator of the M+#2 (who was standing outside to the right of his car) and as I approached him, I observed that he was swaying while standing. I asked the operator (later identified as David Taube-Perrotta) if he was ok, and he only said yeah. I then asked him what had happened, and he just motioned to his car with his right hand. As I stood next to him, I observed his eyes to be glassy and bloodshot. I asked him for his license and registration, and he kept staring towards the car without saying anything. I had to request his license and registration three times before he produced his license from a hip pocket. I then asked again for his registration, and he took approx. five unsteady steps to his passenger side door, opened the door, and almost fell into his car as he bent from the waist. He then stood up, closed the door, and started to return to me, until I told him I needed his registration again. He returned to his door, and again, almost fell into the car as he bent over to open the glove box. As he returned to me he appeared unsteady on his feet. He handed me the registration in a plastic sleeve, and I asked him to remove it from the sleeve. He appeared to have trouble removing the registration. I asked him where he was coming from, he stated that he was coming from Cambridge. I asked where in Cambridge, and he did not reply. I asked him if he had any alcoholic beverages during the evening, he shook his head in an exaggerated manner and stated that he didn't drink tonight. As I spoke more with him, I observed that his speech was slurred. I held my pen up and asked him if he could focus on the tip of pen. Instead of answering he tried to reach out and touch it. I advised him, I just wanted to know if he could focus on it, he stated yes, then raised his finger in an attempt to touch his nose, but instead, touched between his upper lip and nose, then tried to touch the tip of the pen again. I asked him to keep his down, and without moving his head, follow the tip of the pen. As I moved the pen from left to right, he turned his head to follow the pen. I had to remind him several times, to keep his head still, and follow only with his eyes. I asked him if he would be willing to take a field sobriety test, and he declined. I had also observed a small scratch on his forehead, and requested medics to evaluate him. As I took digital pictures of the accident, Taube-Perrotta was seen in the back of a Cataldo Ambulance, while Sgt. Devine observed. When I entered the ambulance, Sgt. Devine informed me that Taube-Perrotta stated that he was at "The Lizzard Lounge" in Cambridge earlier in the evening, and admitted that he had a few drinks. In my presence Taube-Perotta stated that he had two drinks, much earlier in the evening. While in the ambulance, Taube-Perrotta had trouble answering common questions by the medics, such as what his middle name was and where he lived. The operator of M/V#2 was placed under arrest for OUI. (See NPD Incident Report # 19053361)

At approximately 2250HRs on Friday, December 13, 2019, I was dispatched to the area of 275 Washington St. for a report of a hit and run crash; Newton Officers and a State Police Officer canvassed the area with negative results. Upon speaking with the operator of MV1 (MA Pass:1CTB36), he stated that he was traveling Westbound on Washington St., when he observed a black Honda mini van or SUV, MV2, approach him in his passenger's side mirror and then attempt to enter his lane of travel, striking the rear of MV1. Upon speaking with the two witnesses who were traveling behind MV1 and MV2, they concurred with the operator of MV1's account. The witnesses described MV2 as a black Honda or Toyota mini van or SUV. I observed damage to the right rear quarter panel, trunk area and right rear tire of MV1. Todys Services arrived and took custody of MV1. I filled an NPD Towed Motor Vehicle Form. **Update as of 1800HRs on 12/15/2019** I was made aware that the operator of MV2 in this incident, now identified as MA Pass: 8HD155, submitted a Commonwealth of Massachusetts Motor Vehicle Crash Operator Report on 12/14/2019. In said report, the operator admits to striking MV1 and stated that he pulled over on Peabody St., where he remained for approximately 15 minutes awaiting MV1. When MV1 did not arrive, the operator of MV2 attempted to drive away, but realized the damage was too severe and returned to the same spot on Peabody St. I viewed MV2 at Clay Auto Body and observed damage to its left fender/wheel well area, as well as a deployed passenger's side airbag. Upon speaking with the operator of MV2, he reiterated the same information he stated in the above summary of his Commonwealth of Massachusetts Motor Vehicle Crash Operator Report. In the presence of his parents, I advised him on the proper steps to take when involved in an accident, such as the proper exchange of information and the potential ramifications of not doing so. Due to the operator of MV2 being 17 years old, being issued his driver's license 9 months ago and his evident remorse/confusion surrounding the incident, I will not be seeking any charges. I updated the operator of MV1 and provided both parties with an NPD Crash Data Exchange Form.

On Sunday, December 15th 2019, at approximately 3:35am, Officer Donovan and I, Officer Brooks, responded to Washington street at Saint James street, for a report of a single car MVA. Upon our arrival we observed MV1 (MA REG 1GHG66) facing westbound on the sidewalk of Saint James street. The vehicle had heavy damage to the front drivers side door, broken axles, and all of the vehicles airbags had deployed. The operator of the vehicle was sitting outside of the vehicle speaking with MA State Trooper Newman who was on scene prior to my arrival, and had witnessed the accident. Trooper Newman stated MV1 was traveling westbound on Washington street, drove over a traffic island on Saint James street, hit a light post on the sidewalk, and spun sideways off the fence that separates Saint James street from the MA Turnpike. The vehicle then struck a tree and came to rest facing westbound on Saint James street. The vehicle was only occupied by the operator. Medics were called to evaluate the operator and a patient refusal was signed. After being cleared from the medics, Officer Donovan initiated an investigation into the circumstances of the accident, and subsequently placed the operator under arrest for operating a MV under the influence of alcohol. Tody's towing was notified to remove the vehicle due to its inoperability on the sidewalk. Officer Helms took photos of the accident scene which were submitted to the IT bureau. Officer Howes completed a motor vehicle inventory form which has been filed.

Operator of MV1 stated she was attempting to turn left from Washington St. onto Walnut St. headed Southbound when MV2 hit her vehicle in the intersection. Operator of MV1 could not provide a registration plate, however stated it was a black SUV and possibly a Honda Pilot. Operator of MV1 stated the unidentified vehicle took off onto the highway and did not stop to exchange any information. MV1 sustained very minor right front end damage. No injuries.

Operator #1 was driving w/b on Washington St just past Waltham St when his rear passenger decided to open the passenger side door attempting to exit into traffic causing a crash with vehicle #2. The rear passenger door as it opened struck vehicle #2's passenger side mirror and door as it was passing vehicle #1. Operator #2 reports he was driving W/B on Washington St just passing Waltham St when vehicle #1s rear passenger door opened up into traffic striking his passenger side mirror and door.

Mv#1 was traveling north on Centre Street when it was involved in an argument with Mv#2, resulting in the motor vehicles colliding(ref inc 19055516). -OpMv#1 stated that after the confrontation Mv#2 cut him off striking his vehicle. -OpMv#2 stated that after the confrontation Mv#1 was trying cut him off striking his vehicle. - Mv#2 Pass1 and Mv#2 Pass2 statements coincide with those made by OpMv#2. -Wit#1 he observed Mv#1 and Mv#2the at the intersection of Commonwealth Street and Centre Street. Mv#1 on the right lane and Mv#2 on the left turning lane. He stated the parties exchanging words at the intersection and a few minutes both parties spat on each other's vehicles. When the traffic control light changed he observed Mv#2 speed off and Mv#1 suddenly changed lanes after it. They stated that they observed that when both vehicle were on Commonwealth Avenue Mv#2 cut off Mv#1, causing the vehicles collided. Mv#1 threw a cup of coffee at Mv#2. Wit#1 and Wit#2 stated they intervened and de-escalated the situation until police arrived. - None of the parties reported injuries to them at the scene not either vehicle was towed. Op MV#1 was cited for Reckless Operation Ch 90s24. (Cit#2079508) by suddenly changing lanes and cutting across the intersection to follow Mv#2. Op MV#2 (Cit#2079507) was cited because his driving caused the collision by shifting lanes with out signaling, cutting off MV#1.

Owner of vehicle reports that unk vehicle struck her vehicle while it was parked in her assigned parking space at #146 Walnut st, The hit and run occurred stb 19:30 hours and 21:30 hours last nite, 12/19/2019.

Based on observations, and statements made, the following occoured. M/V#1 was traveling south on Parker St. and was stopped with traffic, when M/V#2 rear ended M/V#1. The operater of M/V#2 stated that as she came over the hill, she saw traffic stopped, and rear ended M/V#1.

Based on observations, and statements made, the following occurred. The operator of M/V#1 stated that at approx. 21:30 hrs. he parked M/V#1 in a parking lot behind, and parallel to 2011/2013 Chestnut St., in a clearly defined parking spot. M/V#2 appeared to have struck the right rear of M/V#1 pushing it several feet to the left, causing M/V#1's left rear door to impact on a steel balcony support pole (of 2011/2013 Chestnut St. causing damage to M/V#1's left rear door, but no apparent damage to the pole). It appeared that M/V#2 attempted to pull forward, causing damage a long the entire right side of M/V#1, and pushing it a few feet forward, then struck a hand rail causing the hand rail to bend inwards. M/V#2 had left behind a large amount of debris, including MA License Plate Veteran RG9. NPD Dispatch contacted Boston Police (E-5) and requested a unit got to 142 Wren St. Apt/ R1 and make contact with the owner of M/V#2. At approx. 0220 hrs, I contacted BPD (E-5) and was informed that Boston PD confirmed that M/V#2 was at 142 Wren St. with heavy front end damage, missing the front license plate. I was also informed that they made contact with the registered owner and advised him to contact Newton P.D. (See NPD incident report #20003150) After speaking to Robert Godnio, he informed me that his son Wayne Godino had been driving the vehicle on 01/17/2020 at approx. 2100 Hrs. I spoke to Wayne Godino, who stated that someone hit his car behind The Builtmore, and took off, then he left. I advised him that the evidence showed that he struck a parked car. Wayne asked how do you know? there weren't any pieces" I advised him that I recovered the license plate to his fathers car, and he stated "whatever". Charges will be sought for MGL C90 S24 Leaving the scene property damage.

Based on observations, and statements made, the following occurred. M/V#1 was stopped at a red light, northbound on Parker St. when M/V#2 came off the Rte 9 (Boylston St. ) westbound ramp. M/V#2 attempted to turn left on to Parker St. (southbound) but cut the turn to short and the left front of M/V#2 stuck the front of M/V#1.

The operator of MV#1 stated she was travelling southbound on Centre St and had stopped to let MV#2 merge onto Centre St from Miller Rd. The operator of MV#1 stated once MV#2 got onto Centre St, her foot slipped from the pedal causing her to accelerate and striking MV#2 in the rear. MV#1 sustained moderate damages to its front end. There were no reported injuries to the operator of MV#1. The operator of MV#2 stated he was stationary on Miller Rd attempting to make a left turn onto Centre St. The operator of MV#2 stated MV#1 had stopped to let him merge onto Centre St and was struck from behind by MV#1. MV#2 sustained heavy damages to its rear end. There were no reported injuries to the operator of MV#2.

Motor vehicle 1 (MV1) took a right turn from the eastbound travel lane on Beacon St. onto Centre St. (southbound). At the same time, motor vehicle 2 (MV2) took a left turn from the westbound travel lane on Beacon St. onto Centre St. (also southbound). As the vehicles both turned onto Centre St. MV1 attempted to switch from the right lane to the left lane and crashed into MV2. As a result of the crash, MV1 sustained minor front end damage, while MV2 sustained moderate rear passengers side damage.

M.V. #1 Was attempting to park on Herrick Rd when the passenger side of her front bumper struck the drivers side rear bumper of parked veh #2. Inside parked veh#2 was on duty Newton Parking Control Officer Lorin Amendola who was unhurt. Very minor damage to both vehicles. There were no pictures taken due to the fact Amendola was in her personal vehicle.

MV#2 was turning left, from Beach Street to Court Street when it was struck by MV#1. - Op Mv#1 stated he had finished making a delivery on Court Street and was backing up to go up Beach street in order to finish his route. He stated MV#2 turned left from Beach street on to Court Street and that is when MV#1 struck MV#2. Op MV#2 stated he was returning from the store with groceries. He told me that he came down Beach Street with the intentions of parking by the sidewalk in front of his apartment complex and off load groceries; he would then go around the block and park in the lot behind the complex. He stated that as he was making the left turn MV#1 back up and hit him. -I observed no damage to the loading gate of MV#1. I observed heavy damage to the front bumper, side panel, and wheel well of MV#2. I also observed the No Left turn and One Way Only sign posted at the intersection of Court and Beach Street. There were no injuries reported at the scene and neither vehicle was towed. Op Mv#1 was cited (citT2079514) failure to use care and OpMv2 (cit#T2079515) making a left turn were prohibited.

Operator of veh#1 stated he was parked inside the lot of #300 Needham St. in a parking spot. When he attempted to back out, he forgot this vehicle was in drive when he gave it gas. He then struck some bushes and a metal fence surrounding the property. The operator then panicked and drove home. Once at home he realized he should go back to the scene and was on his way back when the Police contacted his father. The operator and father showed up on scene and gave me the proper information. I gave the operator of verbal warning for leaving the scene and advised him that if he had not come back on scene there would have been a criminal citation issued. He and his father thanked me and left without any further issues. I called the Property Manager- Paul (978) 884-3045 for this building and left a message asking him to call me back with the owners information.

On Saturday, May 23rd at approximately 1540 hours, I responded to 167 Parmenter Rd for a hit & run accident. Reporting person Bradley Bankman stated his daughter in law's vehicle has been parked out in front of his house since last night. This afternoon he noticed fresh damage to the front left end of the vehicle. I observed the vehicle (Mass Reg # 4520NG, 1994 Toyota Coroll Blue) parked in front of the house with a dent on the left front end of the vehicle. Bradley stated he doesn't know which vehicle caused the damage. There are no cameras and no one witnessed the hit & run.

On 07/14/2020 at 12:03hrs I responded to the area of 14 Austin St for a motor vehicle crash involving a City of Newton owned vehicle. Austin St is a public way in the City of Newton. Vehicle 1 a City of Newton owned vehicle, was parked on Austin St in front of 14 Austin St facing eastbound. The operator of Vehicle 1 opened the drivers side door into traffic striking Vehicle 2 which was traveling eastbound on Austin St. As a result of the impact Vehicle 1 sustained damage to the drivers side door. Vehicle 2 sustained damage to the passenger side right fender and front door. Both vehicles were driven from the scene by the operators. No injuries were reported on scene. No transports were required. Pictures were taken and submitted to the NPD IT Bureau.

MV#1 was parked facing south on Elm Street (a public way in the City of Newton) when it collided with MV#2. - OpMv#2 stated she discovered a note on her windshield from Wit#1 informing her that her vehicle had been struck and OpMv#1 left the area. OpMv#2 observed the damage to her vehicle and reported the incident to the police. - Wit#1 stated MV#1 pulling out of a street side parking spot on Elm Street, as the vehicle pulled out of the spot it struck the left side bumper of Mv#2. Wit#1 described OpMv#1 as white male in his twenties. He further stated he observed OpMv#1 attempt to find the owner of the vehicle, however, he did not find them and so he left. He added that OpMv#1 did not leave any contact information OpMV#2 to find and contact him regarding the collision. He stated he was the one who left the note for OpMv#2 informing her that her vehicle was struck. - I observed the damage to the left bumper of MV#2. I made several attempts to make contact with OpMv#1 via telephone on several occasion; contacted Framingham police to contact OpMv#1, to no avail. During my investigation of the incident I discovered that OpMv#1 has a suspended driver's license. I conducted an inspection of the area and did not observe cameras that covered the involved parking stalls. -There were no injuries reported at this time and neither vehicle was towed from the scene. Traffic Bureau Update (Officer Gaudet): On July 27, 2020, I attempted to contact the registered owner of MV1, Toghrul Mustafayev (SA0610793), with a negative result. A "Hit and Run" inquiry was mailed to Mustafayev. On Tuesday, August 4, 2020, I spoke with Mustafayev via telephone. Mustafayev stated he is the owner of MV1 and recalls being involved in an incident on July 24, 2020 in front of 115 Elm Street, Newton. Elm Street is a public way maintained by the City of Newton. Mustafayev stated he was operating his vehicle (2007 Toyota Prius, MA: 93A250), while leaving a parking spot in front of 115 Elm Street. Mustafayev stated as he left the parking spot his vehicle touched another vehicle. Mustafayev stated he exited his vehicle and observed no damage to the other vehicle involved. Mustafayev stated he looked around for the other owner but could not find them. A query of Mustafayev's license status shows it is suspended as of November 18, 2019. I asked Mustafayev if he was aware of his license status. Mustafayev stated he knew it was suspended but he was only changing parking spots when the crash occurred. Mustafayev was advised and will be mailed Massachusetts Uniform Citation T0115912 for Chapter 90, Section 23 (Operating MV After Suspension) and Chapter 90, Section 24(2) (Leaving The Scene of Property Damage). A report and affidavit of an operating after suspension was completed and e-mailed to the Registry of Motor Vehicles at DCUOperatingAfterSuspension@massmail.state.ma.us.

Vehicle 1 a city of Newton Fire Engine was parked in the driveway of Newton Wellesley Hospital 2014 Washington St. Vehicle 2 was located in the driveway directly behind Vehicle 1. Vehicle 2 collided with Vehicle 1 while attempting to pass Vehicle 1 on the left. As a result of the impact Vehicle 1 sustained minor damage to the drivers side. Vehicle 2 sustained damage to the passenger's side front bumper and fender. No injuries were reported. No tows were required. It should be noted that Vehicle 1 was on scene due to a fire alarm activation at Newton Wellesley Hospital. The operator of Vehicle 2 stated that while behind Vehicle 1 he noticed another Fire Engine approaching. The operator of Vehicle 2 stated that he collided with Vehicle 1 while trying to move out of the way of the approaching Fire Engine. Pictures were taken on scene and submitted to the NPD IT Bureau.

City of Newton DPW employee Matthew Dagostino stated that on 08/06/2020 at 10:13 hours he was driving a 2007 International 7400 dump truck south bound on Walnut St in the area of number 457. Walnut St is a public way in the city of Newton. Dagostino stated that the truck was equipped with a sander and it's gross weight was appropriately 26,000 lbs. Dagostino stated that his speed was approximately 25 MPH as he drove past 457 Walnut St. Dagostino stated that a vehicle that was travailing south bound in front of his truck MA reg 491DH5 a 2019 Subaru Impreza stopped short . Dagostino stated that the Subaru stopped to let a pedestrian cross the street. Dagostino stated that when the Subaru stopped abruptly it caught him off guard because there was no marked crosswalk in the area. Dagostino stated when the Subaru stopped short he applied the breaks to his truck and had to turn to the left in an attempt to avoid a crash. Dagostino stated that his truck's front passenger side bumper made contact with the Subaru's rear driver's side bumper and tail light. Dagostino's truck had small scrapes to the front passenger side bumper. Nether operator stated that they were injured and the vehicles were not towed.Lev's vehicle had damage to the rear driver's side bumper and tail light . I took photo's of both vehicle and this disk was tuned over to the NPD's It bureau for downloading. Operator of vehicle two Naomi Lev stated that she was driving south bound on Walnut St when she said she was attempting to turn left into Berwick Rd and stopped to let a pedestrian across over Walnut St . It should be noted that Berwick Rd is approximately 1/2 of a mile south of where the crash occurred. Lev stated that she did stop her vehicle to let a pedestrian across just prior to the crash.

On 8/25/2020 at approx 1245hrs while assigned to 497 I responded to NWH emergency ambulance entrance for a report of a past hit and run reported by Walpole Fire Dept personnel. Upon arrival I met with Firefighter Albert KING who stated he was travelling E/B on Washington St towards NWH with a patient and 2 other Firefighters on board when a black Toyota Prius being operated by a Asian female bumped into his front passenger side. FF KING stated he tried to communicate with the women to pull into the hospital and meet him at the ER but upon turning she continued straight on Washington St. KING stated he did not get a license plate number. Minor paint transfer to front passenger side as well as damage to front passenger wheel guard. Patient information not included for privacy, see Walpole Fire dept trip sheet if needed.

The operator of MV1, Sally Wynn, stated she was driving her vehicle on Prince St and her breaks failed her. MV1 stated that she began to roll backwards on Prince St, panicked, and accelerated onto the curb. MV1 came to rest on the property of 308 Prince St. The vehicle came in contact with the front yard wall and railing, damaging both. I took photos of the damage and submitted them to the IT Bureau. Sally Wynn was cited for MGL c90s23 Operating after Suspension, MGL c90s9 Operating an Unregistered Vehicle, MGL c90/34J Operating an Uninsured Vehicle MV1 stated she had no injuries. The vehicle was towed by Tody's.

Operator of Vehicle 1 states he was traveling west on Commonwealth Avenue when he got a flat tire in Boston, just prior to the Newton line. At this time operator 1 states he had difficulty maintaining control of vehicle 1. At that time vehicle 1 struck vehicle 2, pushing vehicle 2 up onto the sidewalk and into Vehicle 3. Vehicle 3 was also parked. Vehicle 3 was then pushed into vehicle 4. Medic 1 transported the operator of vehicle 1 to the hospital. Todys towed all 4 vehicles from the roadway. It should be noted the parked vehicles were all registered out of town and at the time of accident we were unable to notify the owners.

MV was travelling n/b on Walnut St, struck the curb causing major front end damage and came to rest in the oncoming traffic lane. Tody's Towing removed the vehicle & the operator was arrested for OUI drugs, negligent operation, & marked lanes.

OPER OF MV1, WAS PULLING INTO A PARKING SPOT OF #260 NEEDHAM ST, AND CONTINUED THROUGH THE SPOT STRIKING A LIGHT POLE. OPER OF MV1 STATED THAT SHE AND THE VEHICLE DID NOT BREAK IN TIME. THE POLE WAS KNOCKED TO THE GROUND. MV1 SUSTAINED MINOR FRONT DAMAGE. POLE OWNED BY NORTHLAND GROUP.

On 10/25/2020 at approximately 0330 hours, I responded to 173 Grant Ave for a report of a vehicle into the residence. Upon arrival, the operator of the vehicle was out of her vehicle and speaking with the medics. She stated is that she was driving southbound on Grant Ave when a car swerved at her causing her to swerve. As I was speaking with her, I smelt a strong odor of alcohol and she was slurring her speech. She was brought into the ambulance due to her possible head injuries. She was transported to Newton Wellesley for an evaluation. The residence had structural damage to front window and the door frame. The vehicle was removed and towed by Todys and a towed motor vehicle form was filled out and filed.

On November 19th, 2020 @ approximately 12:52 hours while assigned to N491 I responded to a report of a two car MV crash @ the intersection of Watertown ST@ Crafts ST. On my arrival I located both involved vehicles inside the parking area to the Fire Station. #191 Craft ST. Vehicle #1 was a red Hyundai Elantra Ma reg. 9le523. The operator was identified as Jennifer F. Coats. She reported travelling W/B on Watertown ST approaching the intersection of Craft ST when she began to brake due to the traffic ahead of her was stopped for the signal light(red). According to Ms Coats as she braked her vehicle failed to stop. Her vehicle subsequently crashed into the rear of vehicle #2. There was a possibility of mechanical failure her anti braking system. Vehicle #2 was a while chev. van ma reg. T79747 operated by a Joasua Eailey. He reported being stopped for the traffic light W/B on Watertown ST @ the intersection of Crafts ST when her was rear ended by vehicle #1. There were no injuries due to this accident and vehicle#1 was towed by Todys.

On 11/19/2020 at approximately 11:47hrs I responded to a motor vehicle crash involving a City of Newton vehicle. The crash occurred on Westland Ave near the intersecting way of Cherry St. Both Cherry St and Westland Ave are public ways in the City of Newton. Vehicle 1 was stopped in traffic in the eastbound lane of Westland Ave at the intersecting way of Cherry St. Vehicle 2 (City MV) was stopped in traffic on Westland Ave behind Vehicle 1. Vehicle 1 then backed up and struck Vehicle 2. As a result of the impact Vehicle 1 sustained minor damage to the driver's side rear bumper. Vehicle 2 sustained minor damage to the passenger side front bumper. No injuries were reported. No tows were required. Photos were taken and submitted to the IT Bureau. The operator of Vehicle 1 stated upon approaching the intersection of Cherry St, his visibility of on coming traffic on Cherry St was obstructed due to hedges located on the property of 271 Cherry St. The operator of Vehicle 1 stated that he then pulled forward and observed a pedestrian jogging southbound on Cherry St and a vehicle traveling southbound on Cherry St. The operator of Vehicle 1 stated that upon observing the pedestrian and vehicle he backed up and struck Vehicle 2.

ON 12-23-20 AT APPROX. 1426HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR EHICLE ACIENT. UPON ARRIVL AT 1231 WASHINGTON I SPOKE TO THE COMPLAINANT ( MARK DONATO, 1231 WASHINGTON ST., NEWTON,MA. DOB: 9-5-57 ). COMP. WORKS FOR HERITAGE CORPORATION. HE SHOWED ME THE REAR LOT OF 1231 WASHINGTON ST. WHERE HE HAD A LARGE HEAVY DUMPTRUCK. BELOQW THE REAR OF THE TRUCK WAS GLASS AND DEBRIS FROM A PAST ACCIDENT. COMP. SHOWED ME VIDEO FOOTAGE FROM 11-30-20 AT 2150HRS. IN THE VIDEO I VIEWED AND SUV IN THE REAR LOT IDLEING. THE SUV ABRUPTLY BACKS IN TO THE PARKED DUMPTRUCK AND THEN QUICKLY SPEEDS AWAY. VEHILE #1 HAS NO VISIBLE DAMAGE. UNABLE TO ASSESS THE DAMAGE TO VEHICLE #2 ADVISED COMP. TO CONTACT HIS INSURANCE COMPANY.

On January 30th, 2021 at approximately 13:26 hours while assigned to N491 I responded to #624 Washington St, Santander Bank parking lot, for a hit & run MV crash. On my arrival I spoke with the R/P, victim and owner of vehicle #1. She reported she is employed at the bank and arrived at the employees parking lot at approximately 08:30 hours. She parked her vehicle a gray, 2014 Honda Civic, Ma reg. 5fk-747 in the S/E corner of the parking lot facing the7 Eleven store. She came out from work around 13:15 hours and noticed her rear drivers side had been struck by an unknown vehicle. Vehicle #2, left scene without making itself known.

On Monday 2/22/2021 at approx 0849hrs, while assigned to N491, I responded to 33 Austin St in Newton for a MVA hit and run. There I was met by the owner of MV1 who states that his car was parked in the Austin St lot while getting coffee at Starbucks. He states that a witness to the accident gave him their information. Witness statement: Schreiner states that she had just left Starbucks and heard a crash. When she looked into the Austin St parking lot she could see MV2 make contact with MV1. She said that MV2 then scrapped the side of MV1 with the front passenger side of the vehicle. When MV2 left the parking lot she said that she was able to read the MA plate of 1NTE29. Schreiner said that MV2 was white and looked like a Toyota Highlander operated by a white female w/ blonde hair in her 40's. It is unclear at this time who was operating MV2(MA# 1NTE29). Dracut PD was contacted about the registered owner and I was able to get a phone number. However, the number was to a business she hasn't worked at in five years. Traffic Bureau update (Officer Gaudet): On Monday, February 22, 2021 I was able to make contact with the registered owner of MV2, Ms. Melinda Russo (S29162725). Ms. Russo stated that she was operating her white 2019 Toyota Highlander (MA: 1NTE29) in the Austin Street municipal parking lot this morning. Ms. Russo stated she was backing out of a parking spot when she observed her passenger side mirror collapse in. Ms. Russo stated after she exited the parking spot she pulled her vehicle over and observed no damage to the passenger door mirror area. Ms. Russo stated she fixed her mirror and then left the area. Ms. Russo stated she did not think there would be damage to any other vehicle because she did not have damage on her vehicle. Ms. Russo was advised that if she believes she was involved in a crash, she should make every effort to contact the owner of the other vehicle. Ms. Russo was also advised she can contact the police department for assistance. Ms. Russo will be mailled Massachusetts Uniform Citation T1446762 for Newton City Ordinance Chapter 19, Section 75 (Fail to Use Care). Austin Street is a public way maintained by the City of Newton.

On 03/08/2021 at 1249hrs I responded to 25 Armory St for a motor vehicle crash involving a city of Newton owned motor vehicle. Vehicle 1 described as city of Newton Fire Engine #4 (MA Fire Reg 444) backed into Vehicle 2 which was parked and unoccupied facing northbound on Armory St. As a result of the impact Vehicle 1 sustained little to no visible damage, Vehicle 2 sustained minor damage to the passenger side front bumper. No injuries were reported on scene. No tows were required. Photographs were taken and submitted to the NPD IT Bureau.

Motor vehicle 1 (MV1) was stopped/slowed in traffic facing northbound, with its left turn signal activated as if it was taking a left onto Ballpate Hill Rd., when motor vehicle 2 (MV2), which was also traveling northbound, attempted to pass on the right. As MV2 was about to pass MV1, MV1 made a right hand turn towards the driveway of 153 Brookline St. As a result, MV2 crashed into the passengers side of MV1. MV1 sustained moderated passengers side damage, while MV2 sustained moderate front end damage.

ON 4-7-21 AT APPROX. 1154HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT HIT-AND-RUN. UPON ARRIVAL AT WATERTOWN ST AND WALKER ST. I SPOKE TO THE OPERATOR OF VEHICLE #1. DRIVER STATES SHE WAS HEADED W-BOUND ON WATERTOWN ST. AND STOPPED AT THE RED LIGHT. DRIVER STATES VEHICLE #2 PULLED UP TO THE LEFT OF HER. AT THIS TIME THE PASSENGER IN THE REAR OF VEHICLE #2 OPENED THE RIGHT PASSENGER SIDE DOOR AND HIT HER VEHICLE. SHE STATES THE PASSENGER CLOSED THE DOOR AND VEHICLE #2 TOOK A LEFT TURN ONTO WALKER AND SPED AWAY. DRIVER STATES SHE EXITED HER VEHICLE AND NOTICED DAMAGE TO THE LEFT REAR QTR. PANEL OF HER VEHICLE. DRIVER STATES SHE WAS UNABLE TO GET A LICENSE PLATE OF VEHICLE #2. DRIVER OF VEHICLE #1 REPORTED NO INJURIES. SHE HAD DAMAGE TO THE LEFT SIDE QTR. PANEL OF HER VEHICLE. ADVISED TO CONTACT HER INSURANCE COMPANY. CLEARED WITHOUT FURTHER INCIDENT.

On April 19th, 2021 at approximately 12:48 hours while working N491 I responded to #146 Norwood Ave for a report of a past MV crash. On my arrival I spoke with the R/P, owner of the vehicle involved, Ann William. She stated she received a call from her daughter around 11:40 this morning that someone had backed into her vehicle(2019 Subaru wagon) which was parked in the driveway to the above address. The other vehicle involved had driven away after the crash. Ms William informed me of a witness identified as Wagner Calhau (978-413-3927). She stated she had already spoken with him and he emailed her a copy of the video he took of the other involved vehicle. The witness apparently was doing work across the street when he witnessed this accident. I located the witness who was still on scene across the street. He reported the following: A truck was dumping some gravel to his job site and had temporarily blocked the street. A pick up truck heading W/B became impatient waiting for the truck to move and began to turn around to head back E/B. The pick up proceeded to back up into the driveway of #146 Norwood Ave to turn around. The truck as it backed into the driveway backed into the rear drivers side to vehicle #1 causing damage. The witness then grabbed his cell phone but was only able to begin recording as vehicle #2 was driving away. Witness showed me the video footage he captured on his cell phone. The pick up truck appeared to be silver in color with a tarp in the back. The license plate I couldn't discern. The operator was not visible.from the video. I will have the video emailed to our IT bureau to see if they can enhance the video for the purposes of capturing the license plate for the pick up truck. I will further investigate if IT bureau is able to enhance the video that shows the suspects license plate.

On 4/24/2021 at approx 0756 I responded to NWH west parking lot for a report of a past hit and run. Upon arrival i was met by reporting party , Scott Dragonetti who stated he and his wife Jennifer Dragonetti had parked their car on Wednesday 4/21/2021 at approx 1315hrs to go have their baby. Upon returning to the car today Scott found front left damage to his car consistent with somebody backing out of the space next to him. The car had minor creasing and white paint transfer on the front left with damage to the lens covers. NWH security stated to Scott they would check for security footage and relay advise if they found anything. Scott was contacted by NWH security on 4/25/2021that they had footage of the hit and run. I contacted NWH upon my return to work on 4/27/2021 and left a message with NWH Security to forward the footage to my work email. Traffic Bureau update (Officer Gaudet): On Friday, April 30, 2021 I responded to Newton Wellesley Hospital to try and retrieve the survellance footage they have capturing the crash. Security Staff there stated there was a miscommunication and they actually do not have any footage of the crash. At this time, there is no information on the vehicle that left the scene.

ON April 26, 2021 at approximately 08:23 hours while working N491 I responded to the intersection of Washburn St @ Pearl St for a report of a crash involving two vehicles. On my arrival I located both involved vehicles. Vehicle #1 was a Eversource 2010 Ford Van Ma CON. N96307. The operator was identified as Eversource employee, James Fiore. He reported being parked on the corner of Washburn St @ Pearl St sitting inside when vehicle #2 struck his vehicle in the rear drivers side. I located vehicle #2 which was a little further east on Washburn St. The operator was identified as John Agathos.. He was operating a 2011 Ford F150, Ma plate 2pv865. He stated he was on Pearl St N/B and was turning right onto Washburn St when he struck the rear bumper of vehicle #1. There were no injuries from this crash and neither vehicle needed to be towed.

THE OPERATOR OF MV#1 WAS PARKING AT THE ARABIC BAPTIST CHURCH IN THE PARKING LOT. THE OPERATOR OR MV#1 DID NOT STOP PROPERLY AND CRASHED INTO A NO PARKING SIGN OVER THE CURB. THERE WAS DAMAGE TO THE FRONT OF MV#1 AND THE SIGN WAS KNOCKED OVER/ BROKEN. MV#1 WAS UNREGISTERED AND TOWED BY TODYS. MA UNIFORM CITATION (WARNING ) #T2014106 WAS ISSUES CHAPTER 90/9. UNREGISTERED MV. THERE WERE NO INJURIES AND THE PROPERTY MANAGER SINOTE IBRAHIM WAS NOTIFIED ABOUT THE ACCIDENT AND DAMAGE. A VEHICLE INVENTORY FORM WAS FILLED OUT.

Motor vehicle 1 (MV1) stopped in traffic facing southbound in front of 143 Bridge Street when motor vehicle 2 (MV2), which was also stopped in traffic behind MV1, let off the break and crashed into the rear of MV1. As a result of the crash MV1 sustained very minor rear end damage, while MV2 sustained very minor front end damage.

Owner of V1 stated he was parked on Beacon Street. Owner of V1 stated he attempted to get out of the car and opened the door. At this time, V2 was driving by him and struck his driver's side door in traffic. V1 suffered damage to his driver's side door. V1 was towed by Todys's. Opr of V2 stated she was traveling east on Beacon Street. Opr of V2 stated she did not see V1 opening the door and it was too late to stop once she realized. At this time, she struck the driver's side door. V2 suffered damage on the right front side of the vehicle. Both parties stated they were not injured.

At approximately 0558HRs on Monday, May 10, 2021, I responded to 303 Auburn St. for a report of a 3 car motor vehicle crash. Upon arrival, I observed the 3 vehicles in question, which included MV1 (MA Pass: 922WA6) in the roadway and MV 2 (MA Con:J95164)/MV3 (MA Pass:3CR718) which were angle parked. Upon speaking with the operator of MV1, they stated that they were traveling on Auburn St. (West Bound ), when their coffee cup fell out of the cup holder and as they attempted to catch it, they inadvertently caused MV1 veer out of the roadway, colliding with MV2 and push it into MV3 and a city tree in front of the above address. Upon processing the scene, I observed damage to the driver's side fender/hood area of MV1, where it struck MV2. MV2 sustained damage to its driver's side fender/hood area, center bumper/hood area and passenger's side fender/hood area; its rear bumper where it struck the tree did not show any signees of outward damage. MV3 sustained damage to its driver's side fender/bumper area. The operator of MV1 reported no injuries and the remaining 2 operators were inside a local eatery during the time of the crash. Todys Services arrived and removed MV1, while MV2/MV3 were driven away. Dispatch notified Forestry to examine the tree. And I cleared without incident.

On Thursday May 13th, 2021 at approximately 1353 hours myself and several units responded 55 Chapel Street for a motor vehicle that struck a pedestrian. At the time of the accident the weather was sunny and the road surface was dry. Chapel Street is a public way owned and maintained by the City of Newton. Upon arrival Fire and medical personnel were attempting to remove the pedestrian from underneath the vehicle as her right leg was stuck in the undercarriage. Fire and medics were able to successfully free the pedestrian from the vehicle and render medical attention. The pedestrian was identified as Beth Herr ( S58524067) and it was determined that she was also the owner of the vehicle involved. I spoke to witness Joe Shields who stated he was outside his house located at 56 Chapel Street when his attention was brought to a blue SUV that was unoccupied moving backwards down Chapel Street. Joe stated he first witnessed the operator of the motor vehicle pull up to the UPS drop box located on the sidewalk across the street from his house. He stated he saw the female operator step out of the vehicle and drop off a package. Next he saw the vehicle start to roll backwards and the female operator opened the drivers side door and attempted to pull the emergency break. He stated the vehicle overtook her and she fell to the ground with her foot stuck under the car. Joe states he heard her yell and saw her get dragged by the vehicle to the opposite side of the street where the vehicle was stopped by the curb. Joe stated he tried to assist the woman but she was yelling in pain and he did not want to cause more injury so he waited for help to arrive. Witness Robyn Preston stated she was walking on Chapel Street when she saw the blue SUV rolling backwards with no one inside the vehicle. She states she then saw a woman try to stop the vehicle but she got dragged underneath the vehicle to its final resting place. Robyn stated she stayed on scene until help arrived. I was able to speak with Beth inside the ambulance prior to being transported to the hospital. Beth was conscious and alert but in a considerable amount of pain and discomfort. I asked her what happened to which she stated she drove her blue Toyota Rav4 MA REG: 1FHM53 to the UPS drop box located in the area of 55 Chapel Street. She stated she thought she put the vehicle in park but must not have as it started to roll backwards when she got out of the car. She states she tried to stop the car by opening the drivers side door and attempting to pull the emergency break. The car over took her and she was dragged to the ground with her right foot stuck underneath the vehicle. She was dragged across the street where the vehicle was finally stopped by a curb. Beth sustained minor scrapes and road rash on various parts of her body and her right leg was injured. Fallon medics transported her to the Brigham & Womens Hospital to be treated. Her husband was contacted using Beth's cell phone and notified of the accident. Captain Boudreau and I were able to drive her vehicle to her residence at 50 Broadway Street, Newton. Her vehicle was left parked outside her house and her car keys were left with her husband who was inside the residence. The vehicle did not sustain any damage. Pictures of the scene were taken and submitted to the IT Bureau. I will be following up with Beth at the hospital to check on her status. On Friday May 14th at 0900 hours I spoke with Beth on the phone. She stated she is home from the hospital and doing much better. She states she has right leg pain and road rash from the accident.

ON 5-23-21 AT APPROX. 1422HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT HIT AND RUN. UPON ARRIVAL AT 100 HULL ST. I SPOKE TO THE OWNER OF VEHICLE #1. SHE STATES SHE PARKED HER VEHICLE ACROSS FROM 100 HULL ST. AT 1300HRS. WHEN SHE RETURNED TO HER VEHICLE AT 1410HRS. SHE NOTICED THERE WAS DAMAGE TO THE LEFT FT. QUARTER PANEL. THERE WERE NO OTHER PARTIES IN THE AREA WHO WITNESSED A CRASH. VEHICLE #1 HAD LEFT FT QTR PANEL DAMAGE AND SCRAPES. DRIVER REPORTED NO INJURIES. DRIVER WAS ADVISED TO CONTACT HER INSURANCE COMPANY. CLEARED WITHOUT FURTHER INCIDENT. VEHICLE #1 WAS NOT TOWED.

On Monday, July 12th 2021, at approximately 5:24pm, I, Officer Brooks, responded to the parking lot of the Barn, 229 Walnut street, for a car accident. I spoke with the owner of MV1(MA REG 9937PH), who stated she had been parked here all day, and when she came out to her car there was a note on the windshield stating "Sorry i hit your car, foot slipped on wet". Her vehicle sustained damage to the drivers side front bumper. The note contained the license plate for MV2 (MA REG 54542), and the registered owners name. Dispatch attempted to reach her by phone but was unable to speak with her. MV2 was still on scene and parked in front of MV1. MV2 had minor damage to the front bumper. It appears the operator of MV2 was pulling into the parking spot and struck MV1. There were no injuries and both vehicles were able to be driven from the scene. The report number was given to both involved parties.

Mv1 was parked facing eastbound resting on the Centre St bridge curb. MV1 had extensive damage to the front end of the vehicle as well as 3 tires popped. MV1 was involved in a single car motor vehicle accident. Operator 1's parent signed a patient refusal due to being a minor. Todys's towed MV1. Operator of MV1 was subsequently arrested for multiple motor vehicle violations.

ON 7-21-21 AT APPROX. 1229HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT HIT AND RUN. UPON ARRIVAL AT 112 ADENA RD. I SPOKE TO THE OWNER OF VEHICLE #1. SHE STATES SOMETIME BETWEEN 0030-0900HRS. SOMEONE DRIVING S-BOUND ON ADENA HAD HIT THE DRIVER SIDE REAR VIEW MIRROR ON HER VEHICLE. THE MIRROR WAS KNOCKED BACK AND THE GLASS WAS BROKEN. DRIVER STATED SHE DOES NOT KNOW WHO BROKE HER MIRROR. COMP. ADVISED TO CONTACT HER INSURANCE COMPANY. CLEARED WITHOUT FURTHER INCIDENT.

- MV#1 was traveling east on North Street when it Collided with MV#2. - Op MV#1 stated he was traveling east on North Street when he observed MV#2 stopped at the stop sign on Farwell Street. He stated that MV#2 pulled out in front of him and they collided. He stated he was very glad he was going very slow or it could have been worse. -Op MV#2 stated he was traveling south on Farwell Street preparing to go on to North Street. He stated he observed MV#1 approaching in the distance with his right directional indicator on. He stated he would use the opportunity created by MV#2 and enter the road way. He stated as he was looking right and entered the road he collided with MV#1. -While on scene neither party reported injuries and both vehicles were capable of being driven away. I observed the damage to the right front bumper of MV#1 and I observed damage to the bumper of MV#2.

ON 8-13-21 AT APPROX. 1220HRS. WHILE WORKING N492 I TOOK A REPORT FOR MOTOR VEHICLE ACCIDENT HIT AND RUN. UPON ARRIVAL AT 14 MT. IDA ST. I SPOKE TO THE LESSEE ( CHRISTIAN FALK ) OF VEHICLE #1. LESSEE STATES HE PARKED HIS CAR ON THE STREET IN FRONT OF 14 MT. IDA ST AND SOMETIME BETWEEN 0030-1200HRS. SOMEONE HIT HIS CAR AND SPED AWAY. VEHICLE #1 HAD LEFT REAR QTR. PANEL DAMAGE, LEFT REAR DOOR DENTS AND LEFT REAR TIRE SCRAPES. IT WAS DIFFICULT TO TELL FROM THE DAMAGE THE ANGLE AS WELL AS THE POSSIBLE VEHICLE INVOLVED. DRIVER ADVISED TO CONTACT HIS INSURANCE COMPANY. CLEARED WITHOUT FURTHER INCIDENT.

MV-1 stated she was driving northbound on Chestnut Street when she thought she saw something in the roadway and swerved. MV-1 struck City Tree (#7912) causing severe front end damage. MV-1 towed by Tody's. Photo of the tree submitted to the I.T bureau.

Motor vehicle 1 (MV1), was traveling eastbound in the left lane on Washington Street when motor vehicle 2 (MV2), which was also traveling eastbound on Washington St., attempted to merge from the right lane to the left lane in order to take a left hand turn onto Jackson Road. As MV2 attempted to merge over, it clipped the rear passengers side tire of MV1. As a result of the crash, MV1 sustained minor damage, while MV2 sustained moderate front end damage.

On 09/28/2021 @ approx. 0339 hrs. while on patrolin the area of 1717 Beacon St. I, Officer Sullivan, observed two motor vehicles pulled off to the side of the Beacon St. (public way). The rear vehicle MA Reg. W27879 had its hazard lights on with the operator, in the vehicle, in the driver's seat. The forward vehicle MA Reg. 3FHK99 also with its operator seated in the driver's seat. The vehicle was running and the operator was testing the operation of the vehicle by slowly moving forward by tapping the accelerator. I observed the vehicle to have tire/wheel damage to multiple tires. I activated my blue lights and exited my cruiser. As I approached the two vehicles I instructed the forward operator identified as Jiang, Xuan to stop the vehicle. The operator from the rear vehicle exited his vehicle. He was identified as Tao,Haichun. Mr. Tao stated his friend (Mr. Jiang) called him and stated he had a damaged tire. I later discovered through continued conversation that Tao is his boss. The two men work at IQ kitchen restaurant located at 1158 Beacon St, Newton. The location of the business is approximately 1 mile from the location of this incident. Jiang stated he was coming from work. while speaking with Jiang the odor of Alcoholic beverage was detected. further information in regards to this incident is documented. Reference Newton PD incident report #21041400. The location of a crash is unknown at this time. The vehicle sustained damage along the passenger side of the vehicle, mirror taken off and damage to three tires; two front and rear passenger. The vehicle MA Reg. 3FHK99 was towed from the area of 1717 Beacon St, Newton by Tody's Towing. The Incident/crash to be further investigated with pending MV violations.

On 10/5/2021 at approx 0735Hrs while assigned to 497 I responded to the area of Washington St and 128N Off Ramp for a report of a two car crash without injuries. Upon arrival I observed Ma Reg 3SW448 a black Audi disabled in the northern eastbound lane of Washington St. It had been struck on the passenger side by OH Reg JID2310 a white Mazda when exiting the highway. I spoke with the operator of the Mazda who stated she was attempting to take a left onto Washington St from the off ramp, the car in the southern eastbound lane of Washington St stopped to let her go, as she was crossing she struck the Audi. I spoke with the operator of the Audi who stated she was traveling eastbound when struck by the Mazda.

On 10/7/2021 at approx 1241 Hrs while assigned to 497 I responded to the area of 1631 Beacon St for a report of a hit and run that had just occurred, the complainant was stating to dispatch that a white Dodge Ram work van with ladders on the roof heading eastbound had struck his car and fled. At the time of the call I was on Beacon St near the Washington St intersection, 494, Officer Tocci radioed that he was at the intersection of Beacon and Walnut St. Officer Tocci and I travelled towards each other the length of Beacon St with no sign of the van. I spoke with the complainant, Daniel Bryan who stated he had grabbed lunch, at Stone L'oven pizza and was eating in his car while parallel parked on Beacon St. Two men, an older white male and a approximately 30-40 year old Hispanic male who had been sitting at the bar eating exited the restaurant , stared at him, got into the van parked in front of Daniel, the older white male getting into the driver seat , put the car in reverse, smashed into Daniels car damaging the front left bumper, than fled eastbound on Beacon St. Daniel states that he believed the males possibly felt he was parked too close to their van and rather than ask him to move , deliberately backed into his car intending to cause damage. Daniel has a dash camera on his car, had several photos of the van but none with a clear picture of the plate on the van. Daniel forwarded the photos to me which i submitted to IT to see if they could clean them up enough to get a plate number and attach to this report.

The operator of MV#1 stated she was stationary at the red light at 870 Walnut St (Southbound) when she was struck from behind by MV#2. MV#1 sustained minor damages to its rear end. The operator of MV#1 sustained unknown non-incapacitating injuries and was transported to Newton Wellesley Hospital by Newton Paramedics. MV#1 was secured and parked in the parking lot of "Walgrens" at 1101 Beacon St. The operator of MV#2 stated he was travelling southbound on Walnut St when his foot slipped off the brake pedal and struck MV#1. MV#2 sustained minor damages to its front end. There were no reported injuries to the operator of MV#2.

I responded to 234 Langley Road for a two car M/V crash with entrapment. Upon arrival with other officers, Fire, and medics I saw the following. MV1 was flipped over on the road way and the operator was still in the vehicle, but the passenger was outside the vehicle. MV1 Hit a parked MV2 unoccupied. MV1 was traveling north on Langley Rd when MV1 swerved to its right and made contact with the left rear bumper of the MV 2. The impact of the two MV's caused MV1 to flip over and land on its roof. Based on my observation the scene indicated : day light, clear and there were no road defects. I took numerous pictures which were submitted to IT bureau so they can be attached to this report. Both MV1 driver and passenger were taken to Beth Israel Hospital in Boston. Tody's arrived on scene and took possession of both MV's. A canvas of the area resulted with one neighbor at 255 Langley who witnessed the accident. Mr. Vish Showdhary stated that he saw MV1 driving on Langley Rd when MV1 crashed into the parked MV. He stated that there were no other vehicles on the road and that MV1 drove right into the parked vehicle. The scene was cleaned and I went to Beth Israel Hospital to speak with the operator and passengers of MV1. The operator of MV1 stated that they have no recollection of what happened. She stated that she does not know how she hit the parked MV and that the last thing she remembers is being pulled out of the rolled over MV by the Fire Department. The passenger in MV1 stated that he does not remember how the crash occurred. He stated that he was unsure how MV1 crashed into MV2. He stated thathe was able to get out of the MV without a problem. After a review of the crash I am citing MV1 with the following. M.G.L 89/4A Marked lanes and M.G.L 90/24E Negligent Operation.(Citation # 1445881) Furthermore a request for immediate threat license revocation was filled and submitted to the RMV. The MV Citation was delivered to the operator of MV1 in hand at the Beth Israel Hospital.

On Monday, November 29, 2021 at approximately 1733hrs, MV1 was travelling Eastbound on Beacon St. when he had struck a pedestrian (James Bird) and his dog, who had been crossing Beacon St. Operator of MV1 stated he did not see the pedestrian crossing the street and had stopped when he noticed Mr. Bird and his dog roll off the front driver side, hood of the vehicle. Bird stated that when he was crossing Beacon St. in the crosswalk with his dog he had been struck by a vehicle. I asked if he would like to be evaluated by paramedics, which he replied, "yes." Mr. Bird informed me that he was sore and had lost consciousness when he had been struck. Fallon Paramedics arrived on scene to evaluate Mr. Bird and decided to transport him to Newton Wellesley Hospital for further evaluation. The operator of MV1 has a license restriction requiring him to wear corrective lenses (Restriction B). I asked the operator of MV1 if he had been wearing his glasses like his license said, which he replied, "no." A witness, Stephanie Meyer, who had been driving behind MV1 stated that MV1 had pulled out of Hancock Ave onto Beacon St., cutting her off and causing her to slam on her breaks. As she traveled behind him heading Eastbound she witnessed him drive all over the road, crossing the yellow solid line and stated that when she saw the vehicle stop she noticed Bird and his dog laying in the middle of the road on the crosswalk. The vehicle of MV1 was towed by Tody's Towing, due to no one being able to pick up his vehicle. The operator of MV1 was issued in hand Citation #T2014398 and was informed that he was going to summonsed into court for the following violations: MGL Chapter 89/11 (Failure to yield for a pedestrian on the crosswalk) MGL Chapter 90/10 (Unlicensed Operation (License Restrictions B. for not wearing corrective lenses)) The operator of MV1 was informed that an Immediate Threat was going to be processed with the Registry of Motor Vehicles and that his licensed was to be suspended. The operator of MV1 stated to me that he understood that he was no longer allowed to drive. Photographs of the scene and the vehicle and have been sent to the IT Bureau to be attached to this report. It should be noted that Newbury St. and Beacon St. are public ways within the City of Newton. An incident report (#21049761) was also generated in regards to the incident. An Immediate Threat Form was filled and faxed over to the Registry Motor Vehicle in regards to the incident.

On Wednesday, February 2, 2022 while assigned to Traffic Bureau N525, I observed a single car motor vehicle crash in the parking lot of 28 Austin Street, Newton. I observed a 2019 Toyota Rav 4 (MA: 2LK212 ) entere the parking lot from Austin Street and attempt to make a left turn into parking spot 7063. The front passenger side headlight area of the vehicle made contact with the cement corner of the enclosed parking area. I observed no damage to the corner of the cement wall next to the parking spot. I observed minor damage to the front passenger side headlight/bumper area of MV1. The operator of the vehicle, Ms. Carline Bijou (S13761288), reported no injuries.

On 02/07/22 I was dispatched to Washington St. @ Center St. for a two car motor vehicle accident. Upon arrival the operator of MV1 stated that he was driving on the right lane of Washington St and turned right to merge onto Center St (S/B). The operator of MV1 stated that the operator of MV2 was in the middle lane when he tried to merge onto the right lane which caused MV1 to hit MV2 on the right side of the bumbler. As they continued driving straight MV2 stopped which caused MV1 to tap the rear bumper of MV2 Causing minor damages to the bumper. The operator of MV2 stated that he was the one driving on right lane when the operator of MV1 hit him in the rear bumper. The operator of MV2 stated that he had the right away which is why MV1 crashed into his bumper. I asked the operator of the of MV2 how did he get damage on the right side of his vehicle to which he replied he was not sure, and that it was there already. He then continued to say that he must of hit the snow bank which caused the damage but the snow bank was a lot lower than where the damage was. Based on my observation the damage is consistent with what the operator of MV1 stated. Medics arrived on scene and both operators denied medical attention.

MV1 Operator states he was at the intersection of Commonwealth Avenue and Centre Street traveling eastbound. MV1 Operator stated the green light for the left turn only was activated. The green light to go straight ahead for the middle lane was not activated. MV1 Operator states he was then rear ended by MV2. MV1 Operator stated the other vehicle drove away without stopping to exchange information. MV1 sustained damage to rear center. MV1 Operator had no injuries. Dispatch stated MV2 (MA REG #W63589) had no MA OLN on file for the vehicle. I was unable to find or contact anyone who owned MV2. No injuries, no tows. Traffic Bureau update (Officer Gaudet): MV2 is a black 2011 Mercedes E350 (MA CON: W63589) . This vehicle is registered to a LAJANSLIM Services Inc out of 346 Eastern Avenue in Lynn, Ma. I attempted to located a contact number for this company with a negative result. On February 15, 2022, I contacted the listed insurance carrier for MV2, Commerce Insurance. Commerce could not provide any contact information for the registered owner of the vehicle, but they were able to provide contact information for the vehicles insurance agent, Friendly Insurance. I contacted Friendly Insurance with an address out of Lynn, Ma. An agent there was able to provide contact information for the owner of MV2, Mr. Fritherson Jean-Baptiste (S50659798). I contacted Mr. Jean-Baptiste who confirmed he is the owner of the 2011 Mercedes E350 involved in the crash. Mr. Jean-Baptiste stated he was not operating his vehicle at the time of the crash and stated it was someone that works for him. Mr. Jean-Baptiste was not able to provide the name of the operator of his vehicle at the time of the crash, but stated he would look into it and contact me with that information. Mr. Jean-Baptiste contacted me later and the day and said he was half asleep when we first spoke. I explained to Mr. Jean-Baptiste again why I contacted him regarding his vehicle. Mr. Jean-Baptiste stated his girlfriend has COVID and he has been quarenteening with her in Quincy. Mr. Jean-Baptiste stated his car was parked in Cambridge and to his knowledge no one else has operated it. Mr. Jean-Baptiste stated he did not understand what was going on with his vehicle and stated he now does not know where it is. Mr. Jean-Baptiste stated he believes his vehicle may now be stolen. Mr. Jean-Baptiste was advised that if he believes his vehicle was stolen and he does not know who was operating it at the time of the crash on February 12, he needs to contact the Cambridge Police Department and report the vehicle stolen. I advised Mr. Jean-Baptiste that I would follow up with him about his vehicle and it's operator. Officer Perry spoke with a witness to the crash, Mr. Robert Drabiuk. Mr. Drabiuk stated he was at the intersection of Commonwealth Avenue and Centre Street on foot when he observed the crash. Mr. Drabiuk stated to Officer Perry he observed MV2 rear end MV1. Mr. Drabiuk also stated the operator of MV2 got out of the vehicle after the crash, put his hands in the air, and then drove away without exchange of information. Mr. Drabiuk stated he observed a black Mercedes (MA REG # W63589) drive off and stated there was a black male operating the motor vehicle. Traffic Bureau update (Officer Gaudet): As of February 28, 2022, I have attempted to contact Mr. Jean-Baptiste numerous times to follow up on his vehicle's involvement in this crash. A party answers the phone and after I identify myself they hang up. There is no record to date of Mr. Jean-Baptiste reporting his vehicle stolen. At this time, I am unable to indeifty Mr. Jean-Baptiste as the operator of the vehicle involved in the crash.

MV 1 was traveling westbound on Charlesbank RD when MV 2 crossed the double yellow line and struck MV 1. MV 1 sustained major damage to all areas and the right side air bags deployed. The operator of MV 1(Mr. Nathan Clark) sustained no apparent injuries and signed a patient refusal with Fallon medics. MV 1 was towed by Tody's Services due to it being disabled on a public way. MV 2 was traveling eastbound on Charlesbank Rd when she crossed the double yellow lines and struck MV 1. The operator of MV 2 (Ms. Monique Hartley) stated she had been up for too long and was beginning to feel weak while operating MV 2. The operator of MV 2's husband (Mr. Thomas Clark) arrived on scene. Thomas and Monique explained Monique is a stage 4 cancer patient and has been undergoing chemotherapy treatment. Monique had no apparent injuries from the accident and signed a patient refusal. Thomas further stated he was going to follow up with Moniques doctors and have her evaluated by them. MV 2 was towed by Tody's services due to it being disabled on a public way.

MV1 stated he was travelling straight ahead when his dog jumped in his lap causing him to swerve and rear end MV2 which was parked & unoccupied. Operator of MV1 was cited for Impeded Operation (Dog). Both vehicles were towed from the scene and no injuries were reported.

On Thursday April 7th, 2022 at approximately 1408 hours I responded to the area of 755 Dedham Street or a motor vehicle accident involving a school bus. At the time of the accident the weather was cloudy and the road surface was dry. Dedham Street is a public way owned and maintained by the city of Newton. Upon arrival I was informed that the school bus was not a city contracted bus and was from the town of Needham. There were no students on the bus at the time of the accident, just the bus driver identified as Charles Jerome and the bus monitor identified as Iola Karen Blake. Charles stated he was stopped in a line of traffic when the bus was struck from behind. No injuries were reported and there was minor damage to the rear end of the bus. The operator of the other vehicle involved was identified as Ariel Fengler. Ariel stated she was traveling behind the school bus and attempted to step on the brakes as the traffic was slowing down. Ariel stated she has a boot on her foot from a previous injury and the boot got stuck under the pedals and she panicked and could not stop her vehicle from hitting the school bus. Ariel stated she was not injured and her vehicle suffered heavy front end damage. Her vehicle was able to leave the scene and no tow was needed.

MV 1 was turning onto Auburn Street from Commonwealth Ave when it drove onto the front lawn of 1887 Commonwealth Avenue. Causing damage to the lawn and bushes.

The owner Vehicle #1 stated that he observed fresh damage to the rear of his vehicle. After viewing video he saw that it wa involved in a collision with a vehicle bearing MA Reg. 7MLK30 as it was backing out of a parking space. I was able to contact the operator of Vehicle #2. He stated that as he was backing out of a parking space he sneezed and was not sure if he made contact with Vehicle #1. When he checked the vehicle, he stated that he did not observe any damage. He was informed of Mr. William's claims, and agreed to cooperate.

Operator of MV#1 stated he was being chased by Operator #2 from Rt. 95 N onto Rt. 30 (Commonwealth Ave) .According to Operator #1 both vehicles exited 95N and proceeded eastbound on Commonwealth Ave. towards Auburn St. Operator of MV#2 stated MV#1 pulled in front of his vehicle, traveled a few yards before abruptly attempting to turn left into the westbound lane of Commonwealth Ave. This abrupt turn caused MV#2 turn collide into rear side of MV#1. Damage to both vehicles consistent with operator of MV#2's statement. Operator of MV#1 stated he was "trying to pick up his mother" from work, so he attempted to turn left into the westbound lane of Commonwealth Ave. Both operators were transported to Newton-Wellesley Hospital for evaluations. Tody's towed both vehicles from the scene. Witness to collision confirms statement made by operator #2, indicating operator of MV#1's attempted left turn caused collision. Further statement by witness contradicts operator #1's statements of being chased as she witnessed MV#1 in front of MV#2. Additional investigation by officers led operator of MV#1 being cited for Operating Under Influence of Liquor (Mgl. Ch. 90 Sec. 24J) and Negligent Operation of a Motor Vehicle (Mgl. Ch. 90. Sec. 24E).

Oper of V1 stated she was stopped at the red traffic signal on Lexington Street facing south when she felt impact from behind from V2. Oper of V2 stated he was stopped on Lexington Street facing south at the red traffic signal. He stated his foot slipped off the brake due to a slippery material on his sneaker, and he tried to swerve as around V1 but was unable to and had contact with the rear of V!. No injuries reported at this time, and all parties advised of the process. Neither vehicle was towed.

MV1 and MV2 backed into each other while exiting their driveways. MV1 unintentionally continued in reverse at high rate of speed into Mary Ellen RD. MV1 then shifted into drive and accelerated into a City of Newton tree on Evelyn RD. Driver MV1 transported to NWH with small cut to her lip and pain in her right ankle. Driver MV2 and occupant declined medical attention. City of Newton Tree inspected by Urban Forestry. Urban Forestry cautioned off the area but stated the tree was not expected to fall. An immediate threat report was filed with the Department of Motor vehicles for the operator of MV1

Vehicle 1 was parked in the parking lot of the Star Market at #33 Austin St when Vehicle 2 attempted to park in the adjacent parking space. While doing so, the front left corner of Vehicle 2 made contact with the driver's side rear door and rear quarter panel causing minor scratches to both vehicles. The operator of Vehicle 1 confronted the operator of Vehicle 2 who stated "You're crazy!" before going across the street to get coffee from Starbucks. When the operator of Vehicle 2 returned she apologized to the operator of Vehicle 1 but felt uncomfortable and left the area without providing her information to the other operator. The operator of Vehicle 2 was issued MA Uniform Citation #T1448098 for leaving the scene with property damage and an incident report was completed (See Report #22019777).There were no injuries due to this accident and neither vehicle was towed.

On 06/06/20222, while assigned to N494, I, Officer Conary, responded to 76 Eldredge Street for a report of a hit and run. Upon arrival, I met with Operator of MV1 who explained to me that they were working in the building when a co-worker notified them that MV1 was hit by a white Mercedes that had left the area. Operator of MV1 was able to get a license plate of the vehicle. His co worker and another worker said a female was driving. The registration came back to a male party out of Weymouth. Newton Dispatch called Weymouth Police with negative results. There was an address listed out of Boston. I called Boston Police and have not heard back from them. I spoke with workers at Riverside Community Care who share the parking lot, and one of the associates is going to send an email to the rest of the staff. They were advised to notify Newton Police. There was minor damage to MV1 on the door. There would be damage on MV2 rear bumper.

-Ped#1 was crossing Auburn Street southbound when she was struck by MV#1. MV#1 left the scene of the collision and did not leave any contact information. -Ped#1 stated she was crossing Auburn Street, after signaling the crosswalk. She stated she was in the crosswalk when MV#1 kept driving. Ped#1 stated she jumped back wards to avoid being struck,however, she clipped on the left ankle. She identified the driver as an old man. -I next spoke to Wit#1 and her statements regarding the collision coincided with Ped#1 Wit one went on to describe MV#1 as a dark gray Prius; that it pulled into the MBTA parking lot after PED#1 went to get her residency manager; and lastly she observed MV#1 leave the scene of the collision. Wit#1 stated she did not get the registration for MV#1. -Ped#1 was transported to Newton-Wellesley Hospital for a minor injury to her left foot where she was struck by MV#1 while jumping out of the way. There was no cameras nor other witnesses found at this time.
