At approximately 0010HRs on Thursday, June 13, 2019, I was dispatched to 141 Dedham St. for a report of a two car crash. Upon arrival, I observed MV1 (NH Pass:3266403) to be parked on the side of the roadway in front of the aforementioned address and it appeared to have been rear ended by MV2 (MA Pass: 513WA3). Being that MV2's airbags were deployed, I requested Newton Fire to evaluate the operator of MV2, who was ultimately medically cleared and signed a patient refusal. Upon speaking to the operator of MV2, he stated that he was traveling Northbound on Dedham St. and feared that he was coming too close to the solid yellow divider lines and over steered to the right, accidently hitting MV1, which was parked. The owner of MV1 was on scene and stated that he was inside and came out to find the crash. I noted damage to the driver's side quarter panel of MV1 and damage to the front end/hood area of MV2. Todys serviced arrived and took custody of MV2. I completed/filled an NPD Towed Motor Vehicle Form. I cleared without incident.

Operator of MV1 states she arrived at The Sinclair restaurant in Cambridge with a friend at approximately 11:30pm and had 1 glass of wine. Operator of MV1 states she left at closing at approximately 2am to drive her friend home located on Mass Ave. Operator of MV1 states after dropping her friend off she traveled home to 41 Hyde St in Newton. Operator of MV1 states she took Lake Ave westbound heading towards Hyde St when she was fighting off sleep and must have passed out at the wheel and struck a city tree on the eastbound side of Lake Ave in front of 160 Lake Ave. Operator of MV1 states she contacted AAA at 3:37am, but they never showed up so she abandoned the vehicle and walked home. Operator of MV1 states she didn't sustain any injuries and refused medical attention. MV1 sustained heavy damage to the left front fender and was towed by Tody's.

On Monday, July 1st 2019 at 0740 hours, I responded to a single car Motor Vehicle Accident outside the area of 140 Vine St. The vehicle was a 2016 Black Merz Mass Reg # 5AX136 being operated by 17 year old Eleanor Grace Bomberg. The vehicle was headed north on Vine St when it veered off the side of the road and collided with a city tree outside of 140 Vine St. The vehicle flipped over and landed in the middle of the street. The vehicle suffered extensive damage to the front end and right side. The operator suffered minor injuries to her arm. Eleanor's father, Patrick Bomberg, signed the patient refusal. Todys's towed the vehicle. Pictures were taken and submitted to the IT Bureau.

On Tuesday 7/9/19 at approximately 1734 hours while assigned to marked unit n499 I was dispatched to the area of 76 Nahanton Street for a report of a 2 car MVA with no injuries. Upon arrival I spoke with the operator of MV1, BISHOP Robert, who stated that he was travelling eastbound operating a loner vehicle from Mercedes of westwood when the vehicle began "bucking" back and forth as he was slowing to a stop. He stated he didnt know why the vehicle was operating in such an erratic manner. Robert further stated that he was concerned that he would strike the vehicle in front of him so he swerved toward the middle of the road crossing the double yellow line into the path of MV2 striking the vehicle. I also spoke to the operator of MV2, LEMBERG Ekaterina, who stated that she was travelling westbound when she saw MV1 cross the double yellow line into her lane of travel. She was unable to stop and was struck by MV1.

On 7/9/2019 at approx 1406Hrs while assigned to 497 I responded to level 4A of the Newton Wellesley parking garage at 2014 Washington St for a report of a minor collision. Upon arrival I spoke with the operator of RI Reg FV705, Malorie CURLEY who stated she had parked her new work vehicle in the parking stall was exiting her vehicle to see if she was within the lines when the operator of Ma Reg 69EL83 Laurence HENDY pulled into the spot next to her, opened his door striking her truck, CURLEY stated as she was rounding the car HENDY proceeded to grab his door and slam it into her car several more times. CURLEY stated she confronted HENDY who refused to exchange information with her for the damage to her car and fled. CURLEY took pictures which she will provide in her own crash report. The front bumper of CURLEYS car had several spots where the white paint was peeled back , HENDYS vehicle had the matching white paint on the edge of his drivers side door. Although the damage was minor CURLEY requested a report for documentation as it was her work vehicle. i attempted to contact HENDY but no phone number is listed.

DUE TO NEWTONVILLE AVE BEING A NARROW RD WHEN CARS PARK ON THE EAST SIDE WHICH IS LEGAL THEY(PARKED CARS ) TAKE UP THE WHOLE EAST SIDE OF THE ROAD. BASED ON MS ROSENSWEIG STATEMENT I FEEL SHE HAD THE RIGHT OF WAY. VEHICLE #2 SHOULD HAVE YIELDED SEEING THAT VEHICLE #1 HAD NOWHERE TO GO. Mv#1 was parked unoccupied in a marked space in front of 1353 Washington St W/B. An anonymous witness reported #1 was sideswiped there by a light colored MV (#2) bearing partial NH reg 333. I later located Mv#2 a white Subaru NH reg 333 3334, parked in the West Newton Municipal lot directly behind 1353 Washington St. I also met the #2 operator who stated, while pulling out forward, #2 front passenger side bumper sideswiped #1 driver's side rear bumper. Both vehicles sustained minor damage. All parties were advised and satisfied with this investigation and documentation of this collision. No further police action at this time.

OPMV#1 Was traveling Southbound on Chestnut St when she saw an object in the middle of the road and swerved into the Northbound lane of Chestnut St. OPMV#1 Drove up over the curb and onto the sidewalk colliding with a stone wall at 542 Chestnut St and a Utility pole. On 7/15/19, I responded to the West Newton commuter lot to speak with a party who had her vehicle struck in front of 305 Centre St. The operator of MV1 (MA Reg:4YK555) stated that she was travelling southbound on Centre St when a white Mercedes SUV came into her lane and collided with her drivers side. The Mercedes appeared to acknowledge it made contact with the car, but did not pull over. The Mercedes travelled onto the Mass Pike westbound. The operator of MV1 observed a partial registration of Mass Reg "35M." MV1 stated that she did not see the remaining letters/numbers of the registration, however she believes it was an Asian female driver in her 30's. I had dispatch attempt to run the partial plate several ways with negative results. I observed minor damage to the drivers side of MV1. I advised MV1 to contact her insurance. On the above date and time Mary Lacamera (operator # 1) came to NPD Headquarters to report a traffic accident that occurred on 07/14/2019 in the parking lot of Brae Burn Country Club, 326 Fuller St. Lacamera stated that she found a note on her M/V stating that the note writer had witnessed a black Buick Terraz (Ma Reg 9LW783), back into Lacamera's car, and leave the scene. The author of the note did not leave any contact information, or name. Ma Reg 9LW783 came back as a 2006 Black Buick Terraz, registered to Mirna Espinal, of 12 Delford St. Roslindale Ma. 02131. I attempted to contact Espinal with negative results. Citation #T1442113 was mailed to Espinal for MGL 90/24 Leaving The Scene?Property Damage. Lacamera stated that there was damage to the right rear of her M/V. (See NPD Incident Report #19029166)

On 07/16/19 at approximately 09:38 I responded to a two vehicle crash on Needham St. outside of #141. The Operator of Veh #1 stated she was turning left on Needham St when the South bound traffic stopped to left her turn. As she turned, Veh#2 drove South down the multi-turn lane and they collided nearly head on. Heavy damage and air bag deployment to Veh #1. The operator was checked out by Medics and signed a patient refusal. The Operator of Veh #2 stated she was driving down the multi-turn lane South on Needham St.. She was preparing to take a left onto Jaconnet St when Veh #1 pulled out of parking lot and collided with her vehicle. I observed moderate to heavy damage to Veh #2 and noted that the vehicle didn't have its directional on. The operator of Veh #2 complained to left leg pain and was transported to Newton-Wellesley Hospital By Ambulance #1. Both vehicles were towed by Tody's Towing. At the conclusion of my investigation it appears as if both vehicles were at fault during this crash; Veh#1 for not yielding to right of way and Veh #2 for traveling too soon in a turn lane, no directional and driving too fast for the heavy traffic conditions. Both operators were given verbal warnings and there will be no citations issued at this time.

Operator of vehicle 1 states she was sitting inside her parked vehicle at a metered space, when she was struck in the left rear by vehicle 2 as it was parallel parking. The operator of vehicle 1 states she got out and talked to the operator of vehicle 2 but then got back into her vehicle to call a friend as she did not know what to do in a situation like this. While the operator of vehicle 1 was on the phone, the operator of vehicle 2 left the area. I was able to get in contact with the company vehicle 2 is registered to. I spoke to the operator of vehicle 2 who states he was in Newtonville today and did strike another vehicle while parallel parking The operator of vehicle 2 states he spoke to the other operator thought they agreed neither vehicle had damage and went their separate ways. Both operators were informed to contact their respective insurance companies. No injuries reported.

On 07/18/2019 Gladys Bisono came into NPD Headquarters to report a past-hit and run. Bisono states that between 1500hrs on 7/13/2019 and 1100hrs on 7/14/2019 her leased vehicle (Vehicle 1) was parked and unoccupied on the eastbound side of Border St near 85 Border St. During this time frame an unknown vehicle (Vehicle 2) struck vehicle 1. As a result of the impact Vehicle 1 sustained damage to its front end and front bumper. Vehicle 2 left the scene with out leaving any identifying information. No injuries were reported. No vehicles were towed. Bisono has since reported the crash to her insurance company who advised her to file a police report. At this time the identity of Vehicle 2 and its operator is unknown. Border St is a public way in the city of Newton.

On Friday, July 19, 2019, while assigned to traffic unit N525, I responded to the area of 845 Beacon Street, Newton for a report of a motor vehicle accident involving a bicyclist. It was reported that the bicyclist sustained minor injuries as a result of the accident. The weather at the time of the accident was clear and sunny. The road surface was dry. Beacon Street is a public way maintained by the City of Newton. I spoke with the operator of MV1 while Newton Fire and Medics attended to the injured bicyclist. Andrei Jordao (S09978620) stated he was parked in his 2019 Hyundai Kona (NY: JFP5331) at a metered parking spot in front of 847 Beacon Street, Newton. Jordao stated he opened his driver door to exit his vehicle when a passing bicyclist crashed into the door. Jordao stated he did not look back at traffic prior to opening the door. Jordao stated the bicyclist fell to the roadway. I observed moderate damage to the driver side door and it was unable to close as a result of the collision. I spoke with the injured bicyclist, Leonard Engel (S53573803). Engel stated he was operating his bicycle Westbound on Beacon Street in the travel lane. Engel stated a vehicle to his right opened it's driver door into the roadway and he was unable to avoid hitting it. Engel stated he fell over the top of his handlebars and onto the roadway. Engel stated he injured his right shoulder/arm/neck area and his right ankle. Engel also had a small injury to his left knee. Engel stated he does not believe he hit his head as a result of the accident. Engel was wearing a helmet, I observed some scratch marks on it. Engel signed a patient refusal with Newton Medics. I observed damage to the rear tire area of Engel's black and green Giant bicycle. There was also damage to chain/pedal area. Engel's estimated the value of the bicycle at $4,000.00 USD. Massachusetts Uniform Citation T1271041 was mailed to Jordao for Chapter 90, Section 14 (Open car door into traffic). Tody's responded the removed MV1 from the roadway. Photos were taken of MV1 and the bicycle and submitted to the IT Bureau.

On 7/20/19 at 23:24 hrs I responded to the area of 552 Commonwealth Avenue for a report of a MVA. Upon arrival it was determined MV#1 had struck two unoccupied parked vehicles in the eastbound parking shoulder. The operator of MV#1 stated he was on his phone while driving and when he picked his head up another vehicle coming westbound was in his lane so he swerved to avoid collision and struck the parked vehicles. The impact caused MV#1 to spin and came to final rest facing Westbound. All vehicles were towed from he scene by Tody's towing. The operator of MV#1 was transported to the hospital for potential injuries. The Operator of MV#1 is being issued MA Citation T1442692 for 90/24 OUI Drugs, 89/4a Marked Lane Violation and 90/13B Impeded Operation/Phone Use and MA Citation T1442693 for 90/24 Operating to Endanger.

On Tuesday, July 23rd 2019, at approximately 5:24pm, I, Officer Brooks, responded to 444 Lowell avenue for a 2 car MVA. Upon my arrival I observed MV1(NH REG 4120041) with heavy front end damage. The vehicle was located in the driveway of 444 Lowell avenue, with the front end sticking out into the roadway. I spoke with the operator of MV1 who stated he was trying to exit the driveway and turn left onto Lowell avenue southbound, when he was struck by MV2(MA REG 3AP243) which had been traveling northbound on Lowell avenue. According to the operator of MV1, MV2 had been traveling at a high rate of speed and tried to swerve around him. There was airbag deployment in MV1 however the operator signed a patient refusal with Cataldo. MV1 was towed by Tody's. I then spoke with the operator of MV2 who stated she was traveling northbound on Lowell avenue and saw MV1 exiting the driveway. She assumed he was going to stop pulling out of the driveway, and when he did not she stated she attempted to swerve around him. MV1 made contact with the passenger side rear wheel well of MV2 causing MV2 to spin out. The operator was not injured and arranged for a private tow due to the damage in the wheel well area.

At approximately 1620HRs on Thursday, July, 25, 2019, I was dispatched to 444 Washington St. in reference to a truck vs tree motor vehicle crash. Upon arrival, I noted MV1 (MA Vet: 22098) had partially left the roadway and came to rest when it struck a city tree on the sidewalk. Additionally, I noted MV1 struck MV2 (MA Pass: 5CN826), which was parked on the road in front of the tree/aforementioned address. MV1 sustained damage to its front clip and was leaking fluids. MV2 sustained damage to its right rear bumper/quarter panel area. Upon speaking with the operator of MV1, an employee of Tony's Auto Services, he stated that he was test riding the vehicle after they made repairs to its power steering system. He stated that as he traveled eastbound on Watertown St. at approx 30MPH, the vehicle's brakes failed and he guided MV1 towards the tree in an attempt to stop it without injuring anyone. Todys Services took custody of MV1. Newton Fire cleaned up the leaking fluids and the operator of MV1 signed a patient refusal with Newton Medics. Newton Dispatch notified the city to see if they wished to inspect the tree which was still freestanding and appeared to only suffer minor damage. Tony's Auto Services notified the owner of MV1 and I notified the owner of MV2 of the crash. I submitted a picture of the tree to IT to be attached to this report. I cleared without further incident.

On 07/29/19 at 11:13 hours, I responded to the Dunkin Donuts parking lot located at 138 Needham Street for a three car motor vehicle accident. Operator of MV1 stated at first she could not recall what occurred during the accident due to lightheadedness at the time. After, she then stated after pulling in to the Dunkin Donuts lot she was attempting to back in to a parking spot in the back of the lot. While backing up, she attempted to hit the brake pedal but must have pushed the gas pedal instead. The vehicle then struck parked/unoccupied MV2 and MV2 was then pushed in to parked/unoccupied MV3. The operator was checked out by Cataldo Ambulance and signed a patient refusal. MV1 sustained moderate driver's side rear bumper area damage but it was still driveable. MV2 sustained damage along both sides of the vehicle and a rear broken axle and it was towed by Tody's. MV3 sustained scrapes and dents to the entire driver's side but the vehicle was driveable.

On August 1, 2019 at approximately 17:34 hours I, Officer Guarino, responded with Newton Fire and Medics for a report of a single vehicle crash into the building of 335 Boylston St. Upon arrival, the operator and sole occupant of the vehicle was out of the vehicle and had minor injuries. MV1 struck a stop sign and was up against 335 Boylston St, which appeared to have a broken window and minor structural damage. Operator of MV1 said that he was traveling in the right lane of Boylston St (Westbound) when a white Mercedes sedan (unknown model or registration number) turned into his lane and almost struck his vehicle. Operator of MV1 swerved to the right to avoid a collision and as a result crashed into the stop sign on John St. and the side of 335 Boylston St. Witness 1, who was parked at 325 Boylston St. saw the incident and corroborated MV1's story. A second witness, who I was unable to make contact with stopped and gave MV1 his phone number of (508)215-4951 and texted MV1 that he caught up with the possible vehicle, MA Reg 9BT373, which comes back to DESOUZA PLASTERING SERVICES INC. I will attempt to make contact with an operator during their business hours. The owner of 335 Boylston St, Onir Spiegel, was on scene, was given the incident number and was advised to contact her insurance company. MV1 was towed from the scene by Tody's and the operator signed a patient refusal from the Medics. Pictures were taken at the scene of the damage and submitted to IT to be attached to this report. Sergeant McLean notified the DPW for the stop sign and glass in the roadway. Sergeant McLean also left a message for Newton Inspectional Services to come inspect damage to the building at a later time.

The operator was picked up on scene by his daughter. Traffic Officer McSweeney arrived and took pictures of the damage to the city tree.

Operator of vehicle 1 states she was traveling westbound on Washington St when the vehicle to her right began to enter her lane and then struck her passenger side pushing her onto the raised island. As a result vehicle 1 suffered a flat front driver side tire. Vehicle 2 did not stop and continued westbound on Washington St towards Wellesley. Operator of vehicle 1 was able to follow the other vehicle and got a license plate of MA reg#149DE7 and described the vehicle as a black sedan. AAA responded and fixed vehicle 1s flat tire and she was able to go on her way. I ran the license plate that was given for the other vehicle and it came back to a 2013 black Kia Rio sedan owned by a Dan Kuksin of 24 Bryon Rd apt 2 in Boston. I was unable to get a phone number for Kuksin. Dispatch requested Boston send a unit by to try and make contact and as of finishing this report I have not heard back. Traffic Bureau Update (Officer Gaudet): On Tuesday, July 6, 2019, I attempted to make contact with the registered owner of MV2, Daniel Kuksin (S61105670), with a negative result. A Hit and Run inquiry was mailed to the Kuksin. On 08/07/2019 the owner of MA reg#149DE7 came into headquarters to discuss what happened. With him was his father, Arkady Kuksin who was operating the vehicle at the time of the incident. Kuksin admitted to operating the vehicle in the area at the time of the incident but stated that someone had thrown a rock at his vehicle causing damage. He then stated that it was not thrown but kicked up from his tire. Myself and Officer Gaudet went outside with Kuskin and examined his vehicle and saw obvious damage that corresponded with the damage to vehicle 1. There were obvious signs of fresh paint transfer. I informed Kuksin that I would be updating my report and he would be receiving a summons in the mail for 90/24 leaving the scene property damage. i mailed Kuksin MA uniform citation T1444589.

On 08/06/19 at 12:34 hours, I responded to Industrial Place by The Lakeshore Learning Store at the request of Newton Police Officer Justin March who witnessed an accident in the parking lot of 300 Needham Street. Officer March was working a road construction detail at the time in the City of Newton. He states that at approximately 12:20 hours he observed MV1 exiting the parking lot of 300 Needham Street( a drive way to a building that the public has access to). There is an island in the middle that separates the entrance and exit of the lot as it goes out to Needham Street. As MV1 was exiting, he drove up on the island and pushed two large stone builders out of place. There was also damage to numerous different flowers planted in the ground. MV1 then exited the lot and went eastbound on Needham Street. Officer March then followed in his vehicle and kept beeping his horn to alert the driver to stop. The operator turned on to Industrial Place and pulled over. Ofc. March attempted to speak to the operator, Gelvin Perez-Ramirez but there was a language barrier. Next, Ofc. Calderon responded and assisted with Spanish translation. He asked the operator what occurred and he stated that he never saw the island. Due to the fact that the operator left the scene of the accident and never reported it, he was issued MA Uniform Citation # T1445351 for Ch. 90/24/C. MV1 sustained damage to the two exhaust pipes on the passenger side of the truck. There is also possible undercarriage damage. Ofc. M. Wade took photos of the scene.

On Thursday, August 8th 2019, at approximately 8:25pm, I, Officer Brooks, was parked in front of 719 Washington street facing westbound. I looked into my side view mirror to pull into traffic and observed MV1 (MA REG 118GL4) parked behind me with the driver's side door open. The operator was leaning into the car to retrieve an item, and then stood up and turned around to close the door. I observed MV2(IN REG VWI565) traveling westbound, at what I estimated to be 45 miles per hour. MV2 struck the open door of MV1, narrowly missing operator #1. MV1 sustained major damage to the driver's side door and was towed by Tody's. MV2 had heavy damage to the passenger side front end. The operator of MV2 was placed under arrest for Operating under the influence of alcohol after investigation. MV2 was towed by Tody's. There were no injuries as a result of the accident.

He was evaluated by Newton Paramedics and refused medical attention. The operator of MV#2 does not have a driver's license and only provided a Brazilian passport. He was issued in hand MA uniform citation #T1268350 for violation of c90 s10 unlicensed operation. A licensed operator (MA license #S48386590) took possession of MV#2 and transported the operator of MV#2 back to his home. Based on the statements made to me I issued in hand to the operator of MV#3 MA uniform citation #T1268351 for violation of C.O 19-71 following too closely. MV#3 was towed by AAA towing and the operator of MV#3 was transported home by his parents.

On 08/13/19 at 14:53 hours, I responded to a hit and run motor vehicle accident located at 244 Needham Street. The operator of MV1 states she was parked in a parking spot to the right of the Splash business at 244 Needham Street. The parking spot is located on Charlemont Street. After parking, she was sitting in her motor vehicle when she heard a bump to her driver's side rear end from MV2 backing up from a parking spot across from her. She says the operator of MV2 turned and looked at her and kept proceeding (NB) on Charlemont towards Needham Street. She then exited her vehicle to try and get the operator's attention to no avail. She took a photo of MV2 which was a black Toyota Highlander bearing MA reg.# R85045. She described the operator as a white female in her forties with possibly dark curly hair. I observed minor paint damage and scratches to the driver's side rear quarter panel area. The operator is unsure if that damage is from the accident. The owner of MV2, Steven Masterson has a residential address of 40 Wild Acres Rd in North Attleboro, MA and a mailing address of 34 Central Street Foxborough, MA. A telephone search revealed a phone number of (269)857-7814 for him out of Aiken, SC and the Foxborough address but the number is not in service. UPDATE: I was provided a phone number for Masterson from N. Attleboro PD. On 08/16/19 at 12:45 hours, I spoke to Sheila Masterson who stated she was driving MV2 at the time of the reported accident. She stated that she was unaware that she struck MV1 and saw the other operator looking at her but thought nothing of it. Due to the accident being very minor or possibly no damage at all, no further action will be taken. Masterson was provided with the other operator's information.

The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave looking for 950 Commonwealth Ave when a vehicle behind him honked its horn. The operator of MV#1 stated he then abruptly made a right turn into the driveway of 940 Commonwealth Ave when he struck a retaining wall (The retaining wall is between 940 Commonwealth and 950 Commonwealth Ave) causing MV#1 to go over the wall and finally resting on top of it. The retaining wall sustained heavy damages, approximately 4 feet in length. MV#1 sustained heavy front right side and undercarriage damages. There were no reported injuries to the operator of MV#1 or its passengers. Newton Paramedics evaluated all occupants of MV#1 and all occupants signed refusals of treatment. The owners of 940 Commonwealth were advised of this accident and provided with the report number. Based on the information provided to me. the operator of MV#1 was issued in hand MA uniform citation #T1268352 for violation of c89 s4A failure to stay within marked lanes. Tody's Towing took possession of MV#1.

MV1 operator, David Gorman (2017 Mazda Cx3, MA Reg:6TL761), stated that he was traveling from Commonwealth Ave turning right on Lexington St heading NORTH when he realized the vehicle behind him was following him very closely. MV1 continued to drive on Lexington St when MV2 (2015 Toyota Highlander, MA Reg: 6VS151), started to beep at him continuously and went over the double yellow line in an attempt to pass MV1. MV2 was unable to pass MV1 because of oncoming traffic so MV2 steered to get back into his lane. MV2 collided with MV1 in the area of 281 Lexington St. A witness, John Maclellan (off-duty police officer Watertown) who was in the area of Lexington St and Orris St. stated that he heard a vehicle beeping repeatedly and then heard a collision. Maclellan came outside to find MV2 over the double yellow lines facing oncoming traffic. MV2 had damage to its passenger front and MV1 had damage to its drivers side rear. Maclellan told MV2 operator, Renato Ribeirodasilva, to move his vehicle to a safe location on the right side of the road and get out of the vehicle to exchange paperwork with the other driver involved (MV1, Gorman). Maclellan phoned Newton PD to notify of the accident and as he was on the line, he witnessed both parties getting into a heated arguement on the sidewalk outside of their vehicles. MV2 operator, Renato Ribeirodasilva was taken into custody for the following charges: 90/24J - OUI Liquor, 90/24E - Neglegant Operator of a MV, and 89/4A - Marked Lanes Violation. Claudineia called a sober friend to drive her and her three children home to Waltham in Claudineia's vehicle. Claudineia was also under the influence of alcohol.

Mv#2 operator states she was travelling on Beacon St W/B and was struck on her passenger side by #1 as #1 was pulling out of the parking space. I observed the initial contact damage on #2 on its passenger side rear door running down to the front of the passenger side wheel well and passenger side rear tire (tire wall partly sliced from impact). When #2 operator was asked if she crossed over the double yellow line, #2 replied only after the impact. #2 added prior to the collision, she briefly looked at her GPS. After statements were received from both operators and at the conclusion of this investigation, a witness identified herself on scene. The witness stated she was sitting in her driver's seat in her parked Mv directly in front of #1. Witness stated she observed #1 pull out from behind her into the travel lane. Witness stated #1's front end had almost fully reached her driver's side door before #2 made impact. Based on statements and witness statements and it appears #2 operator did not see #1 enter the roadway until the last second before impact by overcorrecting and entering the opposite lane of travel.

The OP. of MV#2 states she was attempting to leave 1897 Washington St. (Woodland Golf Club) to make a left turn onto Washington St. Traffic was stopped for a nearby red light and a vehicle on Washington St. gave her room to exit. She slowly pulled out into traffic and thought MV#1 was slowing down to let her out as well, that's when there was a collision.

Vehicle 1 was travelling southbound on Centre St, behind Vehicle 2. Vehicle 2 started to stop because of traffic ahead. Vehicle 1 attempted to slow down, and hydroplaned into the left rear of vehicle 1. I observed minor damage to the left rear of vehicle 2, and moderate damage to the right front of vehicle 1. No one was injured due to the accident.
