The operator of MV#1 stated she was travelling northbound on Walnut St attempting to make a left turn onto Hull St when her front passenger mirror was struck by MV#2 who was travelling northbound on Walnut St. The operator of MV#1 stated the operator of MV#2 must have known their vehicles made contact because he stopped momentarily, looked at her and drove away at a high rate of speed. The operator of MV#2 was described as a white male in his 40's. The registration for MV#2 was 7KX977. There were no reported injuries to the operator of MV#1 or its passenger. There were no visible damages to MV#1. A query of MV#2 (MA REG 7KX977) showed that it is registered to a female party out of a Newton MA address. I was able to speak to the registered owner of MV#2 and she stated that her husband was driving her vehicle this morning and gave me his cell phone. I spoke to the husband (Operator of MV#2) and he verified that he was travelling northbound on Walnut St earlier this morning but was unaware that he had made contact with any other vehicles on the road. I examined the driver's side mirror of MV#2 and did not observe any visible damages. The operator of MV#2 was forthcoming with his statements. There were no reported injuries to the operator of MV#2. Based on the statements made to me I advised all parties involved that this accident will be documented.

On 01/26/19 at 09:58 hours, I responded to a two car motor vehicle accident at Chestnut and Elliot Streets with no reported injuries. Both streets are public ways and maintained by the City of Newton. Operator of M/V # 1 (Quinn) states that she was stopped at the intersection of the above mentioned intersection going (EB) on Elliot Street. Once she received the green light signal she proceeded straight through and was struck by M/V # 2 on her passenger side. The vehicle sustained heavy damage to the passenger side front quarter panel area and it was towed by Tody's towing. Operator of M/V # 2 (Sanaguaray) states that he was operating straight ahead (NB) on Chestnut Street. He states that he thought the signal light was yellow when he proceeded through and does not recall what happened next. The vehicle sustained heavy front end damage and it was towed by Tody's towing. There were two witnesses to the accident. The first one, Karen Cherson, stated that she was traveling behind M/V # 2 when she observed him speeding and run through the red light striking M/V # 1. She said that the light was red by at least two seconds when M/V # 2 went through it. The second witness, John Quackenbush, states he was operating behind M/V # 1 when he observed their travel lane receive the green light signal. M/V # 1 then attempted to proceed through the intersection when it was struck on the passenger side by M/V # 2. Based on the two witness statements, it appears that Sanaguaray (M/V # 2) proceeded straight through the above mentioned intersection against the red light signal and struck M/V # 1. Sanaguaray was also operating a motor vehicle without a license and was issued a MA Uniform Citation # T1441052 for Ch. 89/9 and Ch. 90/10/A. He further stated that he has resided in Massachusetts for five years and was assigned a number of #S19552221 through the MA RMV but it shows no license status.

On 01/31/19 at 08:56 hours, I responded to Dedham and Walnut Streets for a two car motor vehicle accident with no injuries. Operator of M/V 1 states he was stopped for the stop sign facing northbound on Dedham St towards Walnut St. He then proceeded straight through and observed M/V 2 coming at him so he tried to speed up but M/V 2 ended up striking him on the driver's side rear quarter panel area. He did state that M/V 2 did stop for the stop sign after him but did not understand why they were attempting to come through the intersection. The vehicle sustained minor damage to the driver's side rear quarter panel area and had a flat tire. The operator waited for Triple AAA for the flat. Operator of M/V 2 states she was stopped at the stop sign facing eastbound on Dedham St attempting to take a left turn on to Walnut St. Due to the bright sun and glare, she did not observe M/V 1 coming through the intersection and struck him. The vehicle sustained front end damage to the grille and bumper but the vehicle was still driveable. At the time, I did observe the sun to be blinding at the accident location and there was a lot of glare.

MV#1 Stated while traveling Westbound on Watertown St at the intersection of Eliot Ave his vehicle was struck on the driver side by MV#2. MV#1 stated MV#2 was attempting to cross Watertown St from Eddy St when MV#2 entered his lane of travel.MV#1 had heavy front and driver side damage following the collision. MV#2 Stated while traveling Northbound on Eddy St she attempted to cross Watertown St and on to Eliot Ave. MV#2 stated MV#1 had dim headlights and appeared to be traveling at a high rate of speed. MV#2 stated the front passenger side of her vehicle struck MV#1 on the driver side. Both MV#1 and MV#2 were towed from the scene due to heavy damage to both vehicles. MV1 was traveling SOUTH on Cherry St through the intersection of Webster St. MV2 was traveling WEST on Webster St through the same intersection. MV2 collided with MV1 inside of the Cherry St/Webster St intersection causing heavy front end damage to MV2 and damage to the rear drivers side wheel and fender. MV1 operator, Leah Wilson, stated that she had a green light and proceeded through the intersection when she was hit by MV2. MV2 operator, Caroline Steiner, and her 12yo daughter (passenger/front seat) Lorelai Steiner, stated that they were proceeding through the green light and observed MV1 going at a high rate of speed through the intersection. No injuries reported. Both vehicles towed by Tody's. MV1 operator, Leah Wilson was issued in hand MA Uniform Citation T1440472 for 89/9 - Red Light/Fail. On scene, I observed debris surrounding the four-way intersection and MV1 laid to rest past the intersection and facing the wrong direction in traffic. The damage to both vehicles was severe and MV1 had air bag deployment. The reporting party was an NPD auxiliary officer who had responded prior to my arrival because he heard the crash from the backyard of the police station which is approximately 350-400ft. Based on my training and experience, I believed Leah Wilson to be traveling through the intersection while failing to yield to the traffic control device.

MV1 was coming down the Rte 9 West off ramp to Chestnut St. Operator was traveling too fast for the snowy road conditions and failed to stop for the stop sign at the bottom of the ramp. A snow storm had started 2 hours prior to the accident and road conditions were slippery. MV1 struck MV2 broadside who was traveling north on Chestnut St with the right of way. MV1 struck MV2 hard enough that MV2 was pushed across the lane and into MV3 that was stopped at the stop sign southbound. According to the operator of MV2, MV1 was traveling "way too fast for conditions" over 30mph. Operator MV1 was cited for 89/9 failed to stop for stop sign and 90/17 speeding too fast for road conditions. Operator was also cited on Citation T1272992 for 90/23 operating after suspended license. Boylston St and Chestnut St are public ways in the city of Newton.

On 2/25/2019 at approx 1130hrs while assigned to 497 I responded to the intersection of Commonwealth Ave and Chestnut St for a two car crash. Upon arrival I met with the operator of Ma reg RDTAIL ,Steve MCCULLOUGH who stated he was traveling NB on Chestnut St through the green light when he was suddenly struck by the operator of Ma Reg 298CBV George DIARBAKERLY who was travelling EB on Commonwealth Ave. I spoke with DIABAKERLY who initially stated he thought he had the green light but perhaps it changed, than stated maybe he was too far back when he saw the green light. MCCULLOUGH gave me the names of two people who had stopped , Matt DOLITKA 617-719-7621 and Dr J LAUFFLER 617-968-6343. I was able to speak with DOLITKA who stated he was travelling NB on Chestnut ST one car back from MCCULLOUGH, they were traveling through the green light which was solid green, had not just changed, when he saw MCCULLOUGH get hit by DIARBAKERLY at a high rate of speed, DIARBAKERLY struck MCCULLOUGH than bounced off and went up onto the island. DIARBAKERLY issued Mass Uniform Citation T1441861 for violation MGL 89/9 Failure to Stop, Red Light. All parties signed patient refusals with Cataldo. both vehicles towed by Todys.

Operator of MV1 states she was driving Northbound on Centre St. She states she was in the right (straight ) lane, going the speed limit. She states MV2 attempted to turn left in front of her, and she was unable to avoid them. She stated MV2 struck her vehicle on the left front side, sending her off the road and into the traffic signal on the northeast side of the roadway. Operator of MV2 states he was driving southbound on Centre St, attempting to turn left onto Walnut St. He stated he did not see MV1 until the least second, because of the cars in the opposite turn lane. He stated he attempted to stop, but believed MV1 was speeding and he did not have time. Witness 1 was directly behind MV2. He stated it looked like MV2 was attempting to turn left in front of him. He stated MV1 did not appear to be speeding, and that MV2 turned into MV1. He stated he did not think the two cars could see each other until the last second. Witness 2 was in the opposite turn lane. She also stated it appeared MV1 was driving the proper speed, and MV2 turned into MV1. MV1 was towed by Tody's. MV2 was moved off to the side on Walnut St to wait for AAA. 2 parties from MV2 were transported to NWH with minor injuries. Operator of MV2 was given in hand MA citation# T1441667 for 89/8, Fail to yield while turning left. A traffic signal was taken down, and Dagle electric was notified to respond. All other lights were functioning properly. 4 photos were taken of the damage.

On the above date/time I responded to the area of Cherry Street and Webster Street for a three motor vehicle crash with injuries. Upon arrival along with Sgt. Dempsey, Fire/medics. I spoke with the operator of vehicle one. He stated he was stopped on Webster Street facing westbound a the red traffic signal. He further stated the traffic signal on Webster Street turned green, he accelerated forward. He stated vehicle two traveled north on Cherry Street at a high rate of speed through the red traffic signal. He stated he had no reaction time due to the speed of vehicle two and had impact. I spoke to the operator of vehicle two. He stated he was traveling north on Cherry Street going too fast, he stated he thought it was a school zone so he slowed down and he did not observe the color of the traffic signal. He stated he observed vehicle one come out of the Webster Street at the last second at impact. I spoke with the operator of vehicle three. She stated only remembered being stopped on Cherry Street facing south. She stated she did not remember what color the traffic signal was, just knew she was stopped when she felt heavy impact from vehicle two. Injuries: Oper of V1 none at this time- patient refusal Oper of V2 none at this time - patient refusal Oper of V3 Head laceration and head trauma, transported to Newton- Wellesley Hospital Tody's towing arrived on scene and towed vehicles two, three to their tow lot. All proper paper work was done for a towed vehicle(s). All parties advised of the process. After the vehicles were towed, and the scene was cleared I watched the cycling of the traffic signals in the intersection and observed they were functioning properly. It appears a this time based on statements and physical characteristics of the impact area on the roadway, that vehicle two went through the red light on Cherry Street, which caused the crash.

Mv#1 operator was travelling on the Boylston St W/B off ramp at the Chestnut St intersection. #1 front seat passenger spoke for #1 operator who spoke limited English. #1 passenger stated #1 operator came to a stop at the posted stop sign, prior to attempting to cut straight across the intersection further W/B on the ramp. After stopping, #1 proceeded straight ahead into the intersection and collided with Mv#2 which was travelling straight ahead on Chestnut St. #1 driver's side front struck #2 passenger side front. No injuries. #1 appeared to sustain moderate damage and did not require towing. #2 stated he was travelling straight ahead on Chestnut St N/B (right of way). #2 stated he was travelling at a low speed. #2 stated when he arrived at the off ramp intersection, #1 drove out straight ahead from his right and crashed into his passenger side front. No injuries. #2 appeared to sustain moderate damage, was leaking fluids and required towing. Tody's towed #2 to their storage yard without incident. Witness was walking on the Chestnut St sidewalk. Witness stated he observed #1 edging out (stop and go fashion) into the intersection before he observed #1 fully proceed forward colliding with #2.

Operator of MV1 (Ma Reg 35T230) stated she was exiting the JCC parking lot, turning right onto Nahanton Street when a black SUV (No further description was provided), came around the corner, heading Southbound on Nahanton Street when MV2 (Black SUV) struck MV1's driver side front bumper. Operator of MV1 stated MV2 was traveling at a speed greater than it should around the corner prior to striking her vehicle. Operator of MV1 stated she got out of her vehicle to check on the damage and saw MV2 stopped further down the road. When she got back into her vehicle to meet with the operator of MV2, the vehicle was no longer there. No direction of travel of provided. Operator of MV1 and her daughter were not injured. There was minor damage to the front driver side bumper of MV1. I spoke with the JCC Facility Manager, Brian Morris (617-775-1719) who stated he will check the cameras to see if the cameras recorded the accident. I provided Mr. Morris with the time of the accident and my name and advised him to contact NPD if he has any information to provide. Prior to clearing, I canvassed the area for the vehicle with the description provided, yielding negative results.

On 05/20/2018 at 17:30 hours I was notified by Captain Anastasia of a motor vehicle crash involving car, and an electric scooter. Captain Anastaisa stated that the operator of the scooter was a 12 year old male who had sustained unknown injures and had been transported to Boston Children's Hospital via ambulance. Upon my arrival at the crash scene I made the following observations, Putnam and Winthrop streets are public ways in the city of Newton that are two way single lane non divided roadways. The roadway surface was wet from rain earlier in the day. Putnam St runs north to south and Winthrop St runs east to west. There is a stop sign at the end of Winthrop St at Putnam St. I observed MA reg 54MN41 a 2005 Buick LeSabre stopped in the south bound lane of Putnam St approximately 1/2 of the way into the intersection with Winthrop St. I observed approximately three feet of skid marks from the Buick's rear tires. I also observed at the crash scene a Segway Ninebot electric scooter that was on the ground near the Buick(cid:31)s rear passenger side door with its handlebars detached. I spoke with the operator of the Buick, Robert DeSouza who stated that on 05/20/2019 at approximately 16:50 hours he was driving south bound on Putnam St at approximately 25 miles per hour. DeSouza stated that he saw a man, later identified as Jose Estabil standing on the east sidewalk of Putnam St facing Winthrop St who was yelling and waving his arms at something on Winthrop St. DeSouza stated he looked to his right towards Winthrop St and his view was obstructed by a row of hedges. DeSouza stated that as he was looking to his right he began to apply his vehicle's brakes when he saw someone coming at me ". DeSouza stated that he heard a crashing noise from the rear passenger's side of his vehicle. DeSouza stated that he stopped his vehicle after the crash and saw a boy lying on the ground at the rear passenger side of his vehicle. DeSouza stated that he was not injured in the crash. DeSouza's vehicle had minor damage to its rear passenger side door, and did not require a tow. Witness Charlotte Hart stated that at approximately 16:30 hours on 05/19/2019 she was diving south bound on Winthrop St. Hart stated that she had to stop short on Putnam St to avoid a crash " with a boy on a scooter that was racing around on Winthrop St. " Hart sated that she told the boy on the scooter that he should not be ridding the scooter in the street. Witness Jose Estabil stated that he was walking on the east side walk of Putnam St across from Winthrop St. Estabil stated that he saw a boy an electric scooter on Winthrop St approaching Putnam St. Estabil stated that he saw DeSouza's vehicle driving south bound on Putnam St approaching Winthrop St. Estabil stated that the boy on the scooter made no effort to stop, slow or avoid the crash with DeSouza's vehicle. Witness Laurel Fransworth stated that she was driving north on Putnam St approaching Winthrop St when she saw the crash occur. Farnsworth stated a boy on an electric scooter on Winthrop St crash into the rear passenger side door of DeSouza's vehicle. Fransworth stated that she believed that the row of hedges at the end of Winthrop St obstructed DeSouz's vehicle. Rowan Delaney ( 02/27/2007 ) stated that on 05/20/2019 at approximately 16:00 hours he and his fellow F.A. Day Middle School student Thiaugo Rubino ( 03/04/2007 ) were having a race around the area of Winthrop St, Putnam St and Shawn St. Delaney stated that he was on foot running on sidewalks while Rubino was on an electric Ninebot Segway scooter. Delaney stated that Rubion was not wearing a helmet or any other safety equipment. I spoke with Delaney(cid:31)s father Royston Delaney, who stated that the scooter was his and that he was unaware that Rubino was using his scooter, and that he has never seen him ridding it before. Royston Delaney stated that the scooter is battery powered and has a maximum speed of 19 MPH. Royston Delaney stated that his son told him that only Rubino was ridding the scooter. Rubino was transported via ambulance to Boston Children(cid:31)s Hospital. On 05/20/2019 Rubino underwent surgery for a brain bleed. I spoke with Rubion(cid:31)s mother Sonia Rubino who stated that as of 05/21/2019 her son was under observation and in addition to the brain surgery her son had a broken arm, broken leg and a broken back. Based upon witnessed statements and my observations of the crash area I believe that Rubion was at fault for the cause of this crash. Myself and Officer Helms took photos of the crash area and the disks were turned over to the NPD(cid:31)s IT bureau for downloading.

On 06/08/2019 at 0150 hours I received a call from Sgt Lee in regard to a head on motor vehicle crash with entrapment that had occurred on Center St at the four way intersection with Cabot St and Sergeant St. Upon my arrival at the crash scene I was advised by Sgt Lee that all occupants of the involved vehicles had been transported to hospitals for treatment. I made the following observations of the crash scene , Center St is a two lane public way in the city of Newton that runs north to south. I observed a city of Newton contracted Cataldo ambulance stopped in the south bound lane of Center St at Cabot St. The ambulance was MA AMN registration AMB 370 a 2017 Ford Econo Line that had heavy damage to the front end and driver's side fender. I observed that the front tire of the ambulance was snapped off and was pushed back towards the driver's side door. I further observed that both of the front and side air bags of the ambulance were deployed. The front of the ambulance's final point of rest was 8'8" from a fixed overhead light pole on the northeast corner of Center St at Cabot St. The ambulance's front passenger side tire was 4'3" front the curb on the Cabot St side. The ambulance's rear passenger side tire was 2'3" from the Cabot St side curb. The ambulance's rear driver side tire was 5'1" from the double yellow line on Center St. The ambulance's front driver side tire was 6'3"from the Center St double yellow line. The width of Center St from the double yellow line to the Center St curb is 15'' and 16'" to the Sergeant St side. I observed a 2011 Mazda 3 with MA PAN registration 6KJ783 facing south bound in the north bound lane. The Mazda had heavy damage to it's front end, and I observed that it's front tires had snapped off. The Mazda's roof had been removed by Newton Fire to extract the occupant. The front of the Mazda's final point of rest was 40'3" from a fixed overhead light pole on the northeast corner of Center St and Sergeant St. The Mazda's front passenger tire was 4'6" from the double yellow line on Center St. The Mazda's rear passenger side tire was 7'6" from the Center St double yellow line. The Mazda's rear passenger side tire was 1'2" from the Center St double yellow line. The Mazda's front driver's side tire was on the curb on the Sergeant St side of Center St. There were no skid marks from the Mazda which indicates that no braking occurred. At the time of the crash, the Cataldo Ambulance was being operated by Paramedic Corey Bohan and Paramedic Tom White was in the front passenger seat. Bohan stated that he was operating his vehicle southbound on Center St towards Cabot St after clearing a medical call. There were no patients in the ambulance at the time of the crash, and Bohan estimated he was travelling 25 MPH . Bohan stated within less than two seconds he observed a vehicle traveling towards him northbound on Center St in the southbound lane and estimated it's speed to be approximately 80 to 100 MPH.Bohan stated he was able to slow to a near stop and had moved his vehicle towards the right shoulder on Center St before the Mazda struck his vehicle head on. Bohan stated that after forcing his way out of the driver's seat of his vehicle, he rendered medical aid to the operator and only occupant of the Mazda, Jessica Pondish. Bohan stated that Pondish speech was slurred and he detected an odor of an alcoholic beverage emanating from her breath and person. Bohan was later transported to Newton Wellesley Hospital for evaluation. The passenger in the ambulance, Tom White stated to officers that he was working on the ambulance's laptop computer and did not see anything leading up to the impact from the crash. White stated that he was able to force himself free of the passenger side of the ambulance, and rendered medical aid to Pondish. White stated that Pondish was " in and out of it " and did not appear coherent.White stated that he asked Pondish if she had consumed any alcoholic beverage and she nodded in the affirmative. White Stated that he was unsure if Pondish understood his questions. White stated that he did not detect any odor of an alcohol beverage emanating from Pondish's breath or person. White was transported to Newton Wellesley Hospital for evaluation. Pondish was freed from her vehicle after approximately 22 minutes by Newton Fire Department units by utilizing special extraction tools. Pondish was transported by Newton Medic Unit Two to the Brigham and Women's Hospital in Boston for treatment. From 06/08/2019 to 06/10/2019 Pondish had two surgeries and was unable to speak due to her condition. On 06/10/2019 myself and Officer Gaudet were able to speak with Pondish in her hospital ICU room at the Brigham and Women's Hospital. Pondish's parents Lorena and John Pondish were present while we spoke with her. Officer Gaudet asked Pondish about the events that led up to the crash. Pondish stated that on 06/07/2019 she was at a friend's residence in Norton, MA. Pondish stated that while at her friend's residence she consumed a bottle and a half of Riesling wine. After Pondish's admission myself and Officer Gaudet stopped our conversation with her. I read Pondish her rights directly from a Miranda card and explained them in full before allowing her to continue speaking with myself and Officer Gaudet about the events of that led to the crash. Pondish stated that she understood her rights. Officer Gaudet asked Pondish if she had consumed any alcoholic beverages prior to driving home to her residence in Newton. Pondish stated that she drank a bottle and a half of Riesling wine by herself between the hours of 1700 and 0000 hours on 06/08/2019. Officer Gaudet asked Pondish how large the bottles of wine was that she drank, and she stated that they were average size. Officer Gaudet asked Pondish how she perceived her state of being was after consuming a bottle and a half of wine. Pondish stated that she felt " buzzed " . Pondish stated that she left her friend's residence in Norton at approximately 0100 hours on 06/08/2019 to return to her residence in Newton. Pondish stated that she does recall the events leading up to the crash, except that she tried to make a phone call but the person did not pick up. Pondish stated that her seat belt was on and that she was not texting while operating her vehicle. Pondish stated that her injuries consisted of a broken left elbow and internal injures to her intestines. On 06/09/2019 I spoke to Cataldo Paramedic Robert Canning and Stephen Heirholcer who were the medic unit that transported Pondish to the Brigham and Women's Hospital. Canning stated that he rendered medical aid to Pondish for approximately 20 minutes while Fire units worked to free her. Canning stated that Pondish's legs were pinned under her vehicle's dashboard. Canning stated that during the approximately 20 minutes that he was in close proximity to Pondish he could detect a strong odor of an alcoholic beverage that was emanating from her breath and person. Canning stated that once Pondish was freed he continued to treat and monitor her during the transport to the Brigham and Women's Hospital. Canning stated that when he asked Pondish if she had consumed any alcoholic beverage she stated that she had. Paramedic Heirholcer stated that he did not detect any odor of an alcoholic beverage emanating from Pondish's breath or person. Based upon the physical evidence at the crash scene, and statements made to officers by all of the above mentioned persons, Pondish is being charged with violation of MGLs 90/24j OUI liquor, 90/24e negligent operation of a motor vehicle, 89/4a failure to stay within marks lanes, and 90/17 speeding. On 06/12/2019 at the conclusion of the crash investigation I issued MA Criminal Application T 1269996 in hand to Pondish at her hospital room and explained to her what action she needed to take with court. On 06/11/2019 a letter of preservation was sent to Brigham and Women's Hospital requesting that a sample of Pondish's blood, urine and medical records be saved. On 06/12/2019 Brigham and Women's Hospital confirmed that a sample of Pondish's blood had been preserved. As of 06/12/2019 a search warrant for Pondish's blood sample was pending. Photos of the involved vehicles and of the crash scene were taken and downloaded by the NPD's IT Bureau. Both involved vehicles were towed from the crash scene and towed motor vehicle forms were filled out. An Iphone and backpack belonging to Pondish were taken from her vehicle for safekeeping and returned to her on 06/11/2019. A copy of this report was faxed to the MA RMV Driver Control Unit to request an immediate threat suspension.

On 06/30/2019 at 15:30 hours I was notified by Sgt Lee that a crash involving a motor vehicle and a bicyclist had occurred on Wiswall Rd at M Roadway. Upon my arrival at the crash scene I made the following observations, Wiswall Rd is a two lane non divided public way in the city of Newton that runs north to south from a dead end to Dedham St. The posted speed limit on Wiswall Rd is 25 MPH. M Roadway is a two lane non divided public way that runs west to east from Wiswall Rd to a dead end cul-de-sac. The weather conditions at the at of the crash were cloudy, and the roadway surface was wet from rain. I observed MA commercial registration M82794 a gray 2014 Honda Accord stopped in the north bound lane of Wiswall Rd at M Roadway. The Honda is registered to Newton Driving School. I observed two smudge marks approximately four inches in length on the center portion of the front and rear passenger side doors of the Honda. I also observed a 20 inch Kent bike that had be moved from it's final point of rest prior to my arrival. The bike had damage to it's front handle bars, front tire and it's two front forks. The bike's front tire had gray scuff marks on it. At the crash scene I spoke with the operator of the Honda, David Dombrovsky. Dombrovsky stated that he was driving north bound on Wiswall Rd near M Roadway approximately 20 to 25 MPH. Dombrovsky stated that he has had his learner's permit for six months and is a student at the Newton Driving School. Dombrovsky stated that he did not see the bicyclist but heard the impact of the bicyclist stricking the passenger side of the Honda. At the crash scene I spoke with driving school instructor, Igor Drizik. Drizik stated that the Honda was driving north on Wiswall Rd near M Roadway at approximately 20 to 25 MPH . Drizik stated that he did not see the bicyclist until approximately 1 to 2 seconds before impact on the driver's side of the Honda. Drizik stated the Honda is equipped with dual brakes and that he applied the brakes at the same time as Dombrovsky. Drizik stated that he and Dombrovsky got out of the Honda to render aid to the bicyclist. There were three other driving school students in the rear of the Honda at the time of the crash, neither of them stated that they witnessed the crash. None of the occupants of the Honda stated that they were injured and the vehicle did not require a tow. The bicyclist was identified as David Kerwin-Derrick. Kerwin-Derrick was transported via EMS to Boston Children's Hospital form treatment. At the hospital I spoke with one of Kerwin-Derrick's mother's, Patricia Kerwin-Derrick, who stated " We all know that David rode his bike into the car " and that her son had a broken left leg. Kerwin-Derrick is a student at the Memorial Spaulding School. Based upon my observations of the crash scene and statements made to me , I believe that Kerwin-Derrick's unintentionally rode his bike into the passenger side of the Honda. Photos were taken of the Honda, bike and crash scene and the disk was down loaded by the NPD's IT Bureau.

Operator of MV1 was traveling southbound on Centre Street approaching the intersection of Pleasant Street. He failed to apply the brakes in time to stop when he saw the operator of MV2 had stopped to allow another vehicle enter onto Centre Street from Pleasant Street. He estimated his speed at the time of the accident to be approximately 10 mph. Operator of MV2 was stopped on Centre Street allowing another driver pull out in front of her when she was struck from behind. Operator of MV2 complained of neck and back pain and was transported to NWH by Cataldo MED1. I observed very minimal damage to both the rear of MV2 and front of MV2. There were a few very minor scratches, but no other damage to either bumpers. The operator of MV1 and his passenger both signed patient refusals. Both vehicles were driveable and operator of MV2 was able to pull her vehicle into a parking spot on Centre Street.

Motor Vehicle 1 (MV1) was traveling northbound on Centre St. at the intersection of Walnut St. when motor vehicle 2 (MV2), which was also traveling northbound on Centre St., attempted to go straight from the left turn only lane. MV2 then sideswiped MV1, running MV1 off the road and into the curb. The operator of MV1, Drew Brookins, stated that the operator of MV2 pulled over at first, but when traffic cleared, immediately got back into the car and left at a high rate of speed heading northbound on Centre St. Mr. Brookins attempted to follow MV2, and observed MV2 take a right onto Paul Street (eastbound), but was unable to continue to follow MV2, as MV1's front drivers side wheel was blown out as a result of colliding with the curb. MV1 sustained drivers side damage, as well as a blown out right drivers side tire. MV2 likely sustained moderated drivers side damage, and possibly damage to the rear bumper. Mr. Brookins was able to get the license plate of MV2 (MA Reg. 9LE764), which comes back to a female (Desiree Allman) out of Braintree. Mr. Brookins described the operator of MV2 as a white male with a thin build, dirty blond hair and scruffy beard. Dispatch contacted Braintree PD who was able to provide a telephone number that is believed to be Ms. Allman's. I attempted to contact Ms. Allman, however, the number went directly to voicemail. I left a message asking Ms. Allman to call the NPD so that we could find out who was operating her vehicle at the time of the crash. Traffic Bureau Update (Officer Gaudet): On August 6, 2019, I attempted to make contact with the registered owner of MV2, Desiree Allman (S85529796), with a negative result. A Hit and Run inquiry was mailed to the Allman. As of August 8, 2019, I have left several voice mails for Ms. Allman, she has not returned my calls. Ms. Allman had a hearing today in Hingham District Court. I contacted them to confirm we have her current contact information.

The operator of MV#1 stated he was travelling eastbound on Commonwealth Ave making a left turn onto Morton St when he was struck by MV#2. The operator of MV#1 stated he was unsure of where MV#2 came from. There was a strong solar glare facing westbound on Commonwealth Ave during this accident. MV#1 sustained heavy driver's side front end damages. Both front airbags were deployed. The operator of MV#1 stated he was not injured. The front passenger of MV#1 was not injured and had already left the scene to attend school. The driver's side rear passenger sustained a bleeding nose from this accident. She was unsure what caused her injuries because it happened too quickly. She was evluated by Newton Paramedics and signed a patient refusal of treatment. The initial point of impact for both vehicles was in the middle intersection of Commonwealth Ave at Morton St. There were broken vehicle parts and tire skid marks starting from the point of impact and onto the berm on Commonwealth Ave on the carriage lane side. MV#2's final resting spot was directly accross from 808 Commonwealth Ave. MV#2 sustained heavy front end damages to its driver's side wheel (Broken axle) and flat front right side tire. MV#2's driver's side airbag was deployed. The operator of MV#2 was transported by Newton Paramedics to the Boston Medical Center. I spoke to the operator of MV#2 via telephone and she stated she was travelling westbound on Commonwealth Ave when she was struck by MV#1 and caused her swerve onto the carriage lane berm. The operator of MV#1 is 6.5 months into her pregnancy and sustained neck, back, bruises and left hand injuries. She was transported to Boston Medical Center and was later discharged the same day. She was provided with this accident report number and was advised to contact her insurance provider. I spoke to the witness who was at the intersection of Cedar St and Commonwealth Ave. The witness stated MV#1 was travelling eastbound on Commonwealth Ave and had stopped, attempting to make a left turn onto Morton St. The witness stated she then observed MV#2 travelling westbound on Commonwealth Ave and struck MV#1. The witness stated she believed MV#2 was travelling at a high rate of speed because it continued to travel onto the berm after striking MV#1. I took digital photographs of the damaged berm on Commonwealth Ave. Based on the statements made to me, I issued in hand to the operator of MV#1 citation #T1268357 for violation of c89 s8 failure to yield when turning left. Both vehicles were towed by "Tody's Towing".

On Saturday, September 28, 2019, I responded to the intersection of Crafts Street and Albemarle Road, Newton with Sgt. Lee for a report of a motor vehicle crash involving a motorcycle with serious injury to it's operator. The weather at the time of the crash was clear and sunny. The road surface was dry. Crafts Street and Albemarle Road are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Jussara Pontes (S92930243). Pontes stated she was operating a 2017 Toyota Sienna (MA: 4GX777) at the time of the crash. Pontes stated she was traveling North on Albemarle Road and came to a stop at the stop sign located at the Crafts Street intersection. Pontes stated she looked both ways and began to inch out into the roadway. Pontes stated she stopped the vehicle again and was assisting a child sitting in the 2nd row of the vehicle. Pontes stated she then turned her attention back to the roadway. Pontes stated she looked to her right and left again and did not see any vehicles in either direction. Pontes stated she proceeded to cross the intersection to continue North on Albemarle Road. Pontes stated the front bumper area of MV1 crashed into the left side of a passing motorcycle. Pontes stated the motorcycle and it's operator fell to the roadway and slid across the street to the side of the road. Pontes stated she pulled her vehicle over onto Albemarle Road. Pontes stated she did not see the motorcycle prior to the crash and was not distracted. The operator of the motorcycle (MV2), Irene Pistoftzian (S06099449) was transported to Brigham and Women's Hospital with a serious left leg injury prior to my arrival. I observed a red 1990 Kawasaki ZR550 (MA MCN: 1V1063) resting on it's right side on the NE sidewalk area of Albemarle Road at Crafts Street with it's front wheel/handlebar facing Crafts Street. The motorcycle had moderate damage to it's left side from the impact of MV1 crashing into it. The right side of the motorcycle had damage from sliding on the roadway and the crashing into a fence on the sidewalk on Crafts Street (W) at Albemarle Road. There was also damamge to the front headlight/right handlebar area. The motorcycle and Toyota Sienna were towed from the scene by Tody's towing at Sgt. Lee's request. Pictures were taken of the scene and submitted to the IT Bureau. I spoke with Pistoftzian at Brigham and Women's Hospital in Boston on Monday, September 30, 2019. Pistoftzian stated she was operating her motorcycle on Crafts Street (W). Pistoftzian stated as she approached the intersection of Crafts Street and Albemarle Road, she observed a mini van to her left out of her peripheral. Pistoftzian stated she remembers being hit on her left side and then laying on the roadway looking up at the sky on the sidewalk with her head facing Albemarle Field. Pistoftzian stated as a result of the crash she has a significant injury to her left leg and also has an injury to her left wrist/arm. Pistoftzian stated she was wearing a helmet at the time of the crash. I spoke with Kymberley Mitchell via telephone who stated she witnessed the crash. Mitchell stated she was traveling behind the motorcycle involved in the crash on Craft Street (W) at North Street. Mitchell stated they were stopped at a red light at this time. Mitchell stated there were no vehicles in front of the motorcycle at the light. Mitchell stated the light turned green and they continued on Crafts Street at a low rate of speed. Mitchell stated she could see ahead at the intersection of Albemarle Street and Crafts Street, traffic was stopped on Crafts Street (E) letting traffic exit Albemarle Road (N). Mitchell stated vehicles were taking left and right turns out of Albemarle Road before the motorcycle reached the intersection. Mitchell stated a mini van was crossing past the double yellow line of Crafts Street to continue straight when it crashed into the left side of the motorcycle. I spoke with Cheryl Wermer via telephone who stated she witnessed the crash. Wermer stated she was traveling on Crafts Street (E) towards Albemarle Road. Wermer stated traffic ahead of her was stopped to allow vehicles to turn off of Albemarle Road. Wermer stated she observed a motorcycle traveling towards her on Crafts Street (W). Wermer stated she then saw a vehicle travel straight from Albemarle Road across Crafts Street and crash into the motorcycle. Wermer stated the operator of the motorcycle flew off the bike. Wermer stated the car then came to a stop and the driver got out and stated "Oh my god! I didn't see you!" Wermer stated both vehicles were traveling at a low rate of speed prior to the crash. I spoke with Bill Frabizio via telephone who stated he witnessed the crash. Frabizio stated he was behind the mini van (MV1) on Albemarle Road prior to the crash. Frabizio stated the vehicle in front of him began to cross the intersection as he was monitoring the traffic on to take a right turn onto Crafts Street. Frabizio stated he heard a loud "Boom" and saw a motorcycle slide across the street with its operator. Frabizio stated the motorcycle crashed into a fence post and the operator came off the motorcycle. Frabizio stated he pulled over and assisted operator of the motorcycle. After concluding my investigation, the operator of MV1, Jussara Pontes, was mailed Massachusetts Uniform Citation T2016424 for Newton City Ordinance Chapter 19, Section 75 (Failure To Use Care).

Operator of motor vehicle 1 states he came to a stop on Grove Street headed north at the stop sign at the intersection of Woodland Rd. Vehicles on Woodland Rd. stopped to let him go. As motor vehicle 1 proceeded into the intersection a bicycle came out of no where and the operator of motor vehicle 1 struck the bicycle at a low speed with the front of motor vehicle 1. The cyclist states he was traveling east on Woodland Rd. when he proceeded into the intersection he was struck on his right side by motor vehicle 1. The crash was at a low speed and the bicycle tipped over to the left side, causing an injury to his left arm. The cyclist states it was hard to see due to the rain. When the cyclist was asked if he had stopped at the intersection he stated he was not sure if he stopped or just slowed down. When asked if he came to a complete stop and placed a foot onto the ground, the cyclist stated no. The cyclist stated normally when he stops he just balances in place and doe not take his foot off the pedals. When I asked the cyclist if he passed other vehicles on the right to get up to the intersection he stated yes, he normally does so. It should be noted there is no dedicated bike lane and both streets are narrow at this intersection. MED2 transported the cyclist to NWH for his arm injury. I took pictures of the roadway as well and the bicycle and motor vehicle 1. N497 transported the bicycle to property and evidence for safe keeping, N497 submitted memory chip with photos to IT.

Operator of vehicle 1 stated he was traveling eastbound on River St and was taking a left turn onto Cherry St and was struck head on with vehicle 2. Operator of vehicle 2 stated she was traveling westbound on River St and as she was entering the intersection of Cherry St the traffic light turned yellow and vehicle 1 which was traveling eastbound made a sudden left turn leaving her no time to stop and the vehicles collided. Vehicle 1 then sped off northbound on Cherry St and returned a minute or two later. I spoke to a witness, Jena Castro-Casbon who stated she saw vehicle 1 traveling northbound on Cherry St at a high rate of speed and drop of a child at 187 Cherry St then turn around and return to the accident scene at a high rate of speed again. 187 Cherry St is 0.4 miles from the intersection of River St. Operator of vehicle 1 stated he had two of his sons friends in the vehicle and he thought it would be best to get one of them home to 187 Cherry St and then come back to exchange information. I followed up with the child and his parents at 187 Cherry St and he was uninjured. I issued the operator of vehicle 1 MA Uniform Citation #T1445381 for 89/8 failure to yield. No one reported any injures and neither vehicle required a tow.

On 11/21/2019 at approx 0740 hrs while assigned to 497 I responded to the intersection of Washington St and Beacon St for a report of a two car crash with entrapment. Upon arrival I observed NFD and Cataldo Medics assisting the driver of Ma Reg 5RA826 which was in the left EB lane of Washington St with the driver side of the vehicle caved in , the driver of the vehicle, Dorcus BIRUNGI was crying and shaking complaining of left side pain . NFD was working on opening the driver side door which was stuck from the collision. BIRUNGI related she was travelling WB on Washington St through the intersection of Washington and Beacon St when she was struck . The crash forced her car to spin sliding over the center island coming to final rest in the EB lane of Washington St. I was approached by Newton School Crossing Guard Regina GAYLE who stated she had witnessed the crash. GAYLE stated the Operator of the black Jeep Ma Reg 8VX486 had the green light, was well into his left turn from the EB laft lane of Washington St onto Beacon St when the Suburu "came flying " through the intersection. GAYLE stated she didnt know if the Suburu driver was trying to beat the light but she was "going so fast". I spoke with the operator of Ma Reg 8VX586 , nicholas MANCINI. MANCINI stated he was taking the left onto Beacon from EB Washington St with the green light when the other operator came over the rise of WB Washington St at a extremely high rate of speed . BIRUNGI transported to NWH by Cataldo, Vehicle towed by Todys.

Based on observations and statements made, the following occurred. M/V#1 was parked facing south on Chestnut St. awaiting to pull a M/V from an unrelated accident scene, with his yellow hazard light bar activated. A Verizon truck was parked facing north in the northbound lane of Chestnut St. and through traffic was being routed up the westbound side of Amherst St. M/V#2 approached traveling south on Chestnut St., and stopped approx. 100 feet away from M/V#1. It then began to slowly drive up to M/V#1 and attempt to pass between the two parked trucks, at a slow speed, when the right fender of M/V#2 struck the towing rig of M/V#1. The right fender of M/V#2 had damage along the right front wheel well. I was unable to see the extent of the damage to the tow rig. I went to speak to the operator of the M+#2 (who was standing outside to the right of his car) and as I approached him, I observed that he was swaying while standing. I asked the operator (later identified as David Taube-Perrotta) if he was ok, and he only said yeah. I then asked him what had happened, and he just motioned to his car with his right hand. As I stood next to him, I observed his eyes to be glassy and bloodshot. I asked him for his license and registration, and he kept staring towards the car without saying anything. I had to request his license and registration three times before he produced his license from a hip pocket. I then asked again for his registration, and he took approx. five unsteady steps to his passenger side door, opened the door, and almost fell into his car as he bent from the waist. He then stood up, closed the door, and started to return to me, until I told him I needed his registration again. He returned to his door, and again, almost fell into the car as he bent over to open the glove box. As he returned to me he appeared unsteady on his feet. He handed me the registration in a plastic sleeve, and I asked him to remove it from the sleeve. He appeared to have trouble removing the registration. I asked him where he was coming from, he stated that he was coming from Cambridge. I asked where in Cambridge, and he did not reply. I asked him if he had any alcoholic beverages during the evening, he shook his head in an exaggerated manner and stated that he didn't drink tonight. As I spoke more with him, I observed that his speech was slurred. I held my pen up and asked him if he could focus on the tip of pen. Instead of answering he tried to reach out and touch it. I advised him, I just wanted to know if he could focus on it, he stated yes, then raised his finger in an attempt to touch his nose, but instead, touched between his upper lip and nose, then tried to touch the tip of the pen again. I asked him to keep his down, and without moving his head, follow the tip of the pen. As I moved the pen from left to right, he turned his head to follow the pen. I had to remind him several times, to keep his head still, and follow only with his eyes. I asked him if he would be willing to take a field sobriety test, and he declined. I had also observed a small scratch on his forehead, and requested medics to evaluate him. As I took digital pictures of the accident, Taube-Perrotta was seen in the back of a Cataldo Ambulance, while Sgt. Devine observed. When I entered the ambulance, Sgt. Devine informed me that Taube-Perrotta stated that he was at "The Lizzard Lounge" in Cambridge earlier in the evening, and admitted that he had a few drinks. In my presence Taube-Perotta stated that he had two drinks, much earlier in the evening. While in the ambulance, Taube-Perrotta had trouble answering common questions by the medics, such as what his middle name was and where he lived. The operator of M/V#2 was placed under arrest for OUI. (See NPD Incident Report # 19053361)

Operator of vehicle 1 states she was traveling westbound on Washington St when vehicle 2 turned quickly in front of her giving her no time to stop and they crashed. As a result of the crash her car rolled backwards into the opposite curb. Operator of vehicle 2 states he was traveling eastbound on Washington St and was waiting to turn left onto Concord St. The traffic light on Washington turned yellow and he went to turn when vehicle 1 came at him at a high rate of speed and hit him head on causing his vehicle to spin around and strike vehicle 3 on Concord St head on. Operator of vehicle 3 stated he was stopped on Concord St at the light to Washington St when vehicle 1 struck vehicle 2 and pushed him into his vehicle. After the accident I spoke to a witness who stated he had video of the incident. Jose Buenrrostro works at 2310 Washington St and was able to show me the footage. In the footage you can see vehicle 1 not slow down for the yellow light on Washington which turns red before she reaches the intersection. She then attempts to swerve around vehicle 2 but was unable to causing the collision. All three vehicles were towed by todys. The operator of vehicle 1 was transported to Brighams Hospital by Cataldo for injuries. Both other operators signed patient refusals with the medics.

Mv#1 on the right lane and Mv#2 on the left turning lane. He stated the parties exchanging words at the intersection and a few minutes both parties spat on each other's vehicles. When the traffic control light changed he observed Mv#2 speed off and Mv#1 suddenly changed lanes after it. They stated that they observed that when both vehicle were on Commonwealth Avenue Mv#2 cut off Mv#1, causing the vehicles collided. Mv#1 threw a cup of coffee at Mv#2. Wit#1 and Wit#2 stated they intervened and de-escalated the situation until police arrived. - None of the parties reported injuries to them at the scene not either vehicle was towed. Op MV#1 was cited for Reckless Operation Ch 90s24. (Cit#2079508) by suddenly changing lanes and cutting across the intersection to follow Mv#2. Op MV#2 (Cit#2079507) was cited because his driving caused the collision by shifting lanes with out signaling, cutting off MV#1.

Operator of MV1 was traveling Westbound on Rt. 9 at a high rate of speed and veered off the road colliding with a street sign then landing in the backyard of 4 Meredith Ave damaging a fence. It should be noted MV1 failed to stop for police previously in a motor vehicle pursuit. MV1 sustained heavy front end damage. Operator sustained minor injuries and was transported to Newton Wellesley Hospital on a section 12. The vehicle was then towed by Tody's. A motor vehicle inventory form was filled out in regards. A message was left for the city to repair the street sign. Pictures were taken and placed into property and evidence. It should be noted a fence had to be removed at 4 Meredith Ave in order to removed the vehicle. I was able to get permission by the owner of the home to remove the fence.

Narrative:
On Thursday, January 23, 2020 while assigned to Traffic unit N525, I responded to the intersection of Brookline Street and Hartman Road, Newton for a report of a motor vehicle/pedestrian crash involving a City of Newton Crossing Guard. The weather at the time of the crash was overcast. The road surface where the crash took place was dry. Brookline Street and Hartman Road are both public ways maintained by the City of Newton. On arrival, the injured pedestrian was in the process of being transported to Newton Wellesley Hospital with minor injuries. I spoke with the operator of the vehicle involved in the crash, Menachem Uminer (S43996134). Uminer stated he was operating his vehicle on Brookline Street (S) towards Hartman Road. Uminer stated he was stopped in traffic on Brookline Street prior to the crash. Uminer stated the vehicle in front of him proceeded to travel straight from it's stopped position and he then began to follow the traffic on Brookline Street. Uminer stated he did not see anyone in the vicinity of his vehicle as he began to travel straight from his stopped position. Uminer stated as he was traveling by Hartman Road, the front passenger side area of his vehicle made contact with a pedestrian on the right side of Brookline Street at Hartman Road. Uminer stated he did not see this party until after they were struck and he then proceeded to pull his vehicle over. Uminer stated he was operating his vehicle at a very low speed at the time the collision took place. After speaking with Uminer, I observed the white 2017 GMC Yukon (MA: 9NX525) involved in the crash parked on Brookline Street (S) just past Hartman Road. Uminer stated the vehicle had been moved after the collision took place. I observed no obvious damage to the body of the vehicle on the front passenger side area where the collision took place. The front passenger door mirror was folded inward towards the door. Uminer stated he believes the front passenger door mirror was the point of contact between his vehicle and the pedestrian. I spoke with a witness to the crash, Ashley Garcia (S94193596). Garcia stated she was attempting to cross Brookline Street at Spaulding Lane. Garcia stated she entered the marked crosswalk and was a few steps into Brookline Street when a black colored vehicle traveling on Brookline Street (N) slammed on its brakes near the crosswalk. Garcia stated she looked to her left at the vehicle and then heard a thud to the right from the area where the crossing guard was standing near Hartman Road. Garcia stated she looked over and saw a crossing guard on the roadway on Brookline Street (S) at Hartman Road. The injured pedestrian involved in the crash is Cassandra Alexander (S23636307). Alexander is a crossing guard for the City of Newton and is assigned to the intersection of Brookline Street and Hartman Road for the Memorial Spaulding school crossings. This crash occurred while Alexander was performing her official duties. Alexander was wearing her department issued yellow reflective jacket at the time of the crash. I spoke with Alexander at Newton Wellesley Hospital where she was being treated for her injuries. Alexander stated she was performing her duties as a crossing guard at the intersection of Brookline Street and Hartman Road. Alexander stated she had just finished crossing two pedestrians in the marked crosswalk on Hartman Road when observed a pedestrian enter the crosswalk on Brookline Street at Spaulding Lane. Alexander stated she began to enter the intersection of Brookline Street at Hartman Road with her hands up and her department issued yellow reflective jacket on in an effort to stop traffic in the area to allow the pedestrian in the marked crosswalk safely cross. Alexander stated she was struck on her left side by a passing vehicle. Alexander stated she did not see the vehicle and fell to the roadway. Alexander stated as she was falling to the roadway she extended her right arm in an effort to brace her fall. Alexander stated she had injuries to her arms and legs and was sore from the collision. I spoke with a second witness to the crash via telephone, Anand Vaidya (S43996134). Vaidya stated he was stopped in traffic behind MV1 on Brookline Street (Southbound). Vaidya stated he observed the vehicle in front of MV1 proceed straight on Brookline Street after being stopped in traffic. Vaidya stated he then observed MV1 proceed straight on Brookline Street at Hartman Road at a low rate of speed. Vaidya estimated the speed of MV1 at this time to be 5 miles per hour. Vaidya stated a crossing guard was on the right side of the roadway on Brookline Street at Hartman Road wearing a yellow and black reflective jacket and snow pants. Vaidya stated the crossing guard had her back towards him and MV1, and extended her arm out to stop traffic. Vaidya stated at this time MV1 rolled by the crossing guard at a low rate of speed and the front right fender area of the vehicle hit the left rear leg/lower back area of the crossing guard. Vaidya stated the crossing guard bounced off of the car and fell to the roadway. Vaidya stated he believes MV1 was unaware he hit the crossing guard and observed the vehicle travel another 10 feet until it came to stop on Brookline Street. Vaidya stated the crossing guard was not in a marked crosswalk at the time of the crash. After speaking with all parties involved, Uminer will be cited with Massachusetts Uniform Citation T0115905 for Chapter 90, Section 14 (Failure to slow for a pedestrian in the roadway). Photos were taken of the vehicle and scene and submitted to the IT Bureau.

Narrative:
Vehicle one, Newton police marked 503 cruiser operated by Officer Hagopian, was stopped behind a stopped unknown vehicle on Watertown Street facing West waiting on a red light. Officer Hagopian stated both vehicles were in the right travel lane. Vehicle one was the second car in line at the red light. Officer Hagopian of vehicle one heard a police broadcast of a victim in cardiac arrest and activated its police emergency lights and siren to respond to the emergency call. The unknown vehicle in front of vehicle one pulled to the right and allowed vehicle one to navigate around it. Officer Hagopian stated he had his emergency lights and siren activated and went around the stopped vehicle that had pulled to the right in front of him. Vehicle one proceeded West toward the intersection at a slow rate of speed and observed the intersection to be clear and then accelerated. Officer Hagopian stated that vehicle two came from Walnut Street, traveling North, and came across the intersection where vehicle one and two collided. Officer Hagopian estimated his cruiser to be traveling at 15-20 mph upon impact with vehicle two. The front end of vehicle one made impact with the passenger side of vehicle two. Vehicle two, BMW operated by Gail Glaser, was traveling North on Walnut Street and was approaching the intersection of Walnut Street and Watertown Street. Operator of vehicle two estimated her vehicle speed to be about 20 mph as her vehicle entered the intersection. Ms. Glaser stated she had a green light. Operator of vehicle two was in the middle travel lane that was a designated lane to go straight. Mrs. Glaser stated she did not see an emergency vehicle but could hear the siren as she approached and then entered the intersection from Walnut Street (going North). She stated she had a clear view of the intersection. Vehicle two then came in contact with vehicle one (going West on Watertown Street) in the intersection. Vehicle two then came to a final rest in the Southbound lane of Walnut Street near the traffic signal posts at the corner. Vehicle two made head on contact with vehicle three upon final rest. Vehicle three, Acura operated by Jeffrey Katz, was the first car in the right travel lane on Walnut Street facing South. Mr. Katz stated he, vehicle three, was stopped at a red light that had just turned to a green light. Mr. Katz stated he heard the siren, stay stopped, and looked towards the direction of the sound and saw a marked cruiser entering the intersection from Watertown Street (West). Mr. Katz remained stopped where he was and then saw vehicles one and two collide in the intersection. Vehicle two then veered into the front of his vehicle head on and contact was made. Mr. Katz stated when he initially saw the cruiser it was already in the intersection with emergency lights and siren activated and estimated the cruiser to be going about 20 mph upon impact. Mr. Katz also indicated that vehicle two was traveling into the intersection from Walnut Street (North) at an estimated 20 mph. Witness Peter Dill was sitting in his parked car on Watertown Street facing East. Mr. Dill was parked in front of his house at 606 Watertown Street. It is the second home in from the intersection on the Eastbound travel side of Watertown Street. Mr. Dill Stated that he was looking at his cell phone when he heard the siren. When he looked up towards the sound of the siren, he saw vehicle one traveling about 5 mph in the intersection with emergency lights and siren activated. He did not know where vehicle one had originated from and just that it was traveling West on Watertown Street. Mr. Dill also saw vehicle two approaching from Walnut Street heading North and was not speeding. Mr. Dill estimated that 3-5 seconds had gone by when he first heard the siren to when the initial impact of vehicles one and two occurred. Witness James Davis-Sanon was driving a large MWRA utility work truck. He was the first car in the right turn only lane on Walnut Street. He stated that Walnut Street had the green light and he stopped before the intersection to allow vehicle one to continue going West on Watertown Street. Mr. Davis-Sanon stated he saw/heard vehicle one activate its emergency lights and siren when vehicle one was stopped behind a car waiting on a red light on Watertown Street. After vehicle one activated the emergency lights and sirens, it went around the car stopped in front of it and proceeded into the intersection at a slow rate of speed that Mr. Davis-Sanon described as a "rolling stop" but could not remember if the cruiser stopped or nearly stopped. Mr. Davis-Sanon stated vehicle one then accelerated to about 15 mph into the intersection. Mr. Davis-Sanon also looked at his driver's door rear view mirror and saw vehicle two traveling North on Walnut Street in the designated straight only lane and then proceed into the intersection. He estimated that vehicle two was traveling at 25 mph upon impact with vehicle one. Witness Joseph Giangregorio was in the front passenger seat of the same MWRA utility work truck. Mr. Giangregorio concurred with Mr. Davis-Sonon witness accounts regarding the location of their truck and vehicle one prior to emergency lights and sirens. Mr. Giangregorio did not witness the rear view mirror account since Mr. Giangregorio did not observe vehicle two in the rear view mirror. Mr. Giangregorio added that vehicle one activated its emergency lights and siren, maneuvered around the stopped car in front of it, slowed to 1-2 mph before the intersection but could not remember if it was stopped and then accelerated to about 15 mph before impact with vehicle two. He estimated that vehicle two was traveling 25 mph upon impact with vehicle one. I canvassed the area of where this incident ocurred for cameras. The only cameras I was able to locate was at 575 Watertown Street which is the second house in from the intersection on the Westbound travel side. Both cameras at this address did not have an angle to capture the intersection. Both cameras are focused on the front door to the home and the homeowner, Jose, confirmed that the angle could not capture the intersection. Operator of vehicle two was cited for failure to yield to an emergency vehicle, MA uniform citation T2012104. Citation was written on February 3, 2020 and sent via mail.

On 02/26/2020 I responded to the area of Eastbourne Rd and Ireland Rd for a motor vehicle crash involving a pedestrian. Eastbourne RD is a public way which runs from Ward St north bound intersecting with Ireland Rd on the far north end. Ireland RD is a public way which runs west to east. Both the east end and the west end of Ireland Rd are dead ends. The crash occured on Ireland Rd's east end near the intersection of Eastbourne Rd. The crash involved Vehicle 1 described as a 2016 Acura SUV color grey bearing MA passenger registration 3RB127. Vehicle 1 had a valid inspection sticker which expires on 08/30/2020. Vehicle 1 was operated by its owner identified as Barbara Ball. Ball has an active MA Operators License (S18727836). The involved pedestrian was identified as Bruce Terrio. Terrio is an employee of Feeney Brothers Utility Services. Feeney Brothers Utility Services is a contractor of National Grid Gas Company. Upon arrival I observed the following: The crash occurred in the daylight hours around 10:30am on Ireland Rd near the intersection of Eastbourne Rd. At the time of the crash the weather conditions were clear, 48 degrees Fahrenheit and overcast skies (weather.gov). The area where the crash occurred was an active construction site for Feeney Brothers Utility Services. The area of the crash is a dead end. The area is accessed from Ward St from either Eastbourne Rd or Coolidge Rd. The entrances of both Eastbourne Rd and Coolidge Rd had construction signs indicating utility work in the area. At the time of the crash, all roadways were open to vehicle traffic. On the eastbound side of Eastbourne Rd just prior to the intersection of Ireland Rd, I observed a large air compressor which was parked in the roadway facing south bound. No traffic cones were located near the air compressor. To the right of the air compressor were thick green bushes. A wooden fence was located to the right side of the green bushes. On the east side of Ireland Rd I observed a large excavated hole on the right side of the roadway. The hole was near the intersection of Eastbourne Rd and was located approximately three feet from the curb. I observed four traffic cones, two on the front side and two on the left side of the hole. Located on the front edge of the hole was a tire print. Vehicle 1 was located on the west end of Ireland Rd. The front passenger tire of Vehicle 1 had fresh dirt located on the sidewalls and the tire tread. Vehicle 1 appeared to have no visible damage. The pedestrian (Terrio) was being treated by EMS and Fire for injuries. Terrio was wearing brown pants and a green reflective safety vest. I spoke with the operator of Vehicle 1 Barbara Ball. Ball stated that she was operating Vehicle 1 northbound on Eastbourne Rd near the intersection of Ireland Rd. Ball stated that she made a right turn onto Ireland Rd and drove on to the edge of the excavated hole. Ball stated that her vehicle then made impact with Terrio who was in the hole. Ball stated that she immediately stopped, backed out of the hole and called 911. Ball stated that prior to making a right turn onto Ireland Rd, there were no traffic cones in the roadway or next to the hole. Ball stated that as she made the right turn onto Ireland Rd she did not observe Terrio or the hole until it was too late. Ball states that she resides at 80 Eastbourne Rd which is the residence located to the immediate right of the crash location. Ball states that she had been recently parking on Ireland Rd due to ongoing construction at her residence. Ball states that she was traveling at a slow speed prior to impact. At the time of the crash there were three other Feeney Brothers employees working at the site. These workers were identified as Steven Gamble, Mike Cabrinma and Gerard McComaskey. At the time of the crash all three workers were located further up Eastbourne Rd near the intersection of Coolidge Rd and did not observe the crash. At the time of the crash, Newton Police Sergeant Glenn Chisholm was working a paid detail for Feeney Brothers. Sergeant Chisholm stated that he did not observe the crash.Terrio was transported by Cataldo EMS for minor injuries. Several photos were taken of the scene. Photos were taken of the hole with traffic cones and without traffic cones. All photo's were submitted to the IT Bureau. After clearing the scene, I responded to St Elizabeth's Hospital in Brighton, MA to interview Terrio. Terrio stated that at the time of the crash he was standing in the hole on Ireland Rd performing gas utility work. Terrio stated that the front passenger side bumper of Vehicle 1 struck him in the right shoulder. Terrio stated that the impact caused him to lose his balance and fall down in the hole. Terrio states that after falling down he was able to climb out of the hole without assistance. Terrio stated that at the time of impact he was wearing a white hard hat which he thinks struck the pavement on the edge of the hole. Terrio states that at the time of the crash he was working alone in the hole. Terrio states that at the time of the crash traffic cones were located near the hole. Terrio stated that as a result of the impact he sustained minor injuries to his right shoulder and back. I then observed Terrio's hard hat which appeared to have a fresh scrape mark on the top. The hard hat also had broken interior straps. Terrio stated that the straps broke when he was struck by Vehicle 1. Photos were taken of the hard hat and were submitted to the IT Bureau. After reviewing all facts of this incident, Vehicle 1 appeared to have been traveling at a slow speed prior to the crash. This was evident due to the lack of tire skid marks prior to the tire print on the edge of the hole. It also appears that Ball's view of the hole and Terrio may have been obstructed by the parked air compressor on Eastbourne Rd. The thick green bushes and a wooden fence to the right of the air compressor also added to the obstructed view. The hole was also located in close proximity to the air compressor which reduced Vehicle 1's reaction time. Jeff Oliver of OSHA was also notified of this incident. Operator 1 stated that he was traveling Southbound on Waverley Ave when vehicle 2 pulled out of Mandalay Rd and did not stop at the stop sign. Operator 1 could not stop in time and struck vehicle 2 on the passenger side. Operator 2 stated that she stopped at the stop sign on Mandalay Rd. Operator 2 saw vehicle 1 coming; however, she felt like she had enough time to cross Waverly Ave. Operator 2 proceeded forward and believes vehicle 1 was traveling at a high rate of speed and struck vehicle 2. Both vehicles sustained moderate damage, there were no tows or injuries.

On Thursday 4/30/2020 at aprrox 1859hrs, while assigned to N-494, I responded to the intersection Washington St and Centre St for a 2 car MVA. There I observed MV2 stopped in the far left lane and MV1 stopped in the adjacent lane. Operator of MV1 stated that he was crossing over the Washington St bridge and stayed in the second lane furthest from the left and was headed to the I-90 on ramp. He said that he saw MV2 in his side view mirror try to enter his lane and hit his U haul trailer. Operator of MV2 states that he was coming from Centre St and was attempting to head WB on Washington St. It should be noted that the two left lanes are for the I-90 on ramp only and that his merging lanes had a posted yield sign. Operator of MV2 stated that MV1 was driving at a high rate of speed and that the trailer swerved into his vehicle. Operator of MV2 was cited on scene for failing to yield. Citation # T2015008

Based on observations, and statements made, the following occurred. M/V#1 was the second car in line, in the westbound lane of Elliot St. The operator stopped behind a white M/V that had stopped for a red traffic light. When the light turned green, M/V#1 followed the white M/V into the intersection of Elliot St. and Chestnut St. when it was impacted by M/V#2 (which was traveling north on Chestnut St.). Based on witness accounts, M/V#2 was traveling west on Chestnut St. at a high rate of speed, and ran the red light. The impact forced M/V#1 into a traffic light pole on the south west corner of Chestnut and Elliot St. knocking it down. Operator #1 was treated on scene by medics, Operator #2 was transported to NWH. Citation # T2080748 was issued to Operator #2 for MGL CH 89 S 9, Red Light Violation Ditigal pictures were taken of the damage to the vehicles, and city property. The memory card was placed into the IT beurue.

Operator 1 stated he was driving northbound in the southbound lane on Waverly Ave due to construction in the northbound lane. Operator 1 stated as he approached the intersection of Franklin St he was confused on which side of the cones he should be on and crossed over the marked lanes and past the cones and subsequently drove into a 2ft X 2Ft X 8 inch deep construction hole. Operator 1 stated he sustained no injuries and was checked out by medic 3 in which he signed a patient refusal. MV1 sustained minor damage, scuffs, to the right side leather saddle bag. The northbound lane was coned off for more than a block due to a Feeney Brothers construction detail with Officer Edrehi manning the post. A backhoe, a bobcat and a generator all occupied the northbound lane that was clearly blocked off. Officer Edrehi stated that due to Feeney Brother construction detail Waverly Ave was diverted to 1 way traffic travelling northbound in the southbound lane. Officer Edrehi stated he observed Operator 1 following multiple vehicle and as he approached the intersection of Franklin St Operator 1 crossed over the marked lanes and past the cones and at a low speed fell into the construction hole. Due to the statements provided I issued Operator 1 MA Uniform Citation T1269531 for C89 S4A Marked Lanes violation (warning). Operator 1 was assisted in lifitng his motorcycle and drove off without further issues.

HE STATED THAT DUE TO THE SUDDEN TURN HE WAS UNABLE TO STOP IN TIME AND HIS FRONT BUMPER COLLIDED WITH HER PASSENGER SIDE. MV 1 SUSTAINED MINOR DAMAGE AN D NO INJURIES REPORTED. OPERATOR OF MV 1 ALSO STATED HE HAD SLOWED DOWN DUE TO A PEDESTRIAN WHO WAS CROSSING THE STREET ON ELM ST. OPERATOR OF MV 2 STATED THAT SHE WAS TRAVELING ON RIVER STREET AND TURNED LEFT TRYING TO GO SOUTH ON ELM ST AND MV 1 CAME OUT OF NOWHERE AND STRUCK HER PASSENGER SIDE. OPERATOR OF MV2 STATED SHE BELIEVED MV 1 WAS SPEEDING. MODERATE DAMAGE SUSTAINED TO THE PASSENGER SIDE, THE VEHICLE WAS TOWED BY AAA AND NO INJURIES REPORTED. I ADVISED THE OPERATOR OF MV 2 THAT SHE NEEDED TO BE SURE TRAFFIC WAS CLEAR BEFORE TURNING LEFT.

On 06/10/2020 at approximately 16:30 hours, Sgt. Lee (Unit 531) and I (Unit 523) responded to Waverley Avenue and Sargent Street for a two car motor vehicle accident that involved one vehicle rolling over and a possible head injury to that driver. The accident occurred at 16:11 hours on this date. Both streets are public ways and maintained by the City of Newton. On arrival, I observed a black Hyundai Elantra (MV1) bearing MA Reg.# 1DHN15 resting in the middle of the intersection facing eastbound on Sargent Street heading towards Kenrick Street. The vehicle had heavy front end damage. I then observed a gray Honda CRV (MV2) bearing MA Reg.# 8DZB10 rolled over resting on it's hood facing in a northeasterly direction in the southbound travel lane of Waverley Avenue just past the intersection. I observed the point of impact to be on the passenger side rear wheel area due to the damage which included a large dent above the tire well. Digital photos were taken of the scene and intersection. At this point the operator, Jose Valderrama (MA DL# S27590058), of MV2 was already transported to Beth Israel Hospital in Boston, MA for a possible head injury. The operator was also not aware of what occurred at the time by the responding officers. It was also determined by the initial responding officers (Sgt. Rooney, Ofc. Pohlman & Ofc. Anderson) that Mr. Valderrama driver's license was suspended in Massachusetts by NPD Dispatch. Next, I spoke to the operator of MV1 identified as Cora Lyden (ME DL# 8625330). Lyden states that she was traveling straight ahead (EB) on Sargent Street attempting to go straight through the intersection on to Kenrick Street. She says as she approached the intersection the signal light changed to yellow and she proceeded through when she suddenly saw MV2 coming at her. She then jammed on her brakes and then made contact with the other vehicle. She says that the other driver must have been speeding and went through the red light. Next, Sgt. Lee and I responded to Beth Israel Hospital for a follow up on Mr. Valderrama. The hospital staff stated that there were no signs of a head injury and that he was now coherent. He sustained road rash on his left arm and shoulder area due to the seatbelt and front air bag deployment. We then spoke to him and he stated that he was traveling straight ahead (SB) on Waverley Avenue at about 30 to 35 miles per hour. He says that his signal light was green when he attempted to go through the intersection and another motor vehicle made contact with him. After that, he does not recall what occurred. I asked him if he saw the other vehicle approaching and he stated that he did not see it. After at the station, I followed up with Dispatch and they stated that there was two people that called in to report the accident that they attempted to call back to see if they witnessed the event. They had since not returned their call. I then called both numbers and both parties stated that they were only notifying us and did not observe the accident occur. Due to conflicting statements by both operators and no witnesses on scene, I am unable to determine fault for the accident. Mr. Valderrama will be issued MA Uniform Citation #T2080466 for Ch. 90/S23/E Operating a Motor Vehicle after Suspension of License. Tody's towing responded and towed both vehicles. M/V inventories completed by me.

On 6/26/2020 at approx 1214hrs while assigned to 497 I responded to the area of Washington St at Quinobequin Rd for a report of a 2 car crash with air bag deployment. Upon arrival I observed Ma Reg 579AM3 a gray Toyt Rav 4 and Ma Reg 219w70 a red Toyota Corolla in the middle of the EB lanes of Washington St both with significant damage, front and side airbag deployment and fluids leaking. I ascertained the operators were not hurt and called for two tows and NFD for speedy dry. I spoke with the operator of the red Toyt Brendan OCONNOR who stated he was travelling EB on Washington St, had the green light, was proceeding through the intersection when he was struck by the operator of the Toyt Rav 4,Gabriella NUGENT. I spoke with NUGENT who stated she was attempting to take the left onto Quinobequin Rd from the WB lane of Washington St when she collided with OCONNOR. NUGENT acknowledged that although the light was green WB she did not have the left arrow to turn. Both cars towed by Todys, both Nugent and OCONNOR signed patient refusals with Cataldo ambulance.

Vehicle 1 was stopped in traffic on Commonwealth Ave at the intersection of Washington St. Both Commonwealth Ave and Washington St are public ways in the City of Newton. Vehicle 1 was located in the left turn lane of the westbound side of Commonwealth Ave. Vehicle 2 was located westbound side of Commonwealth Ave in the left turn lane. Vehicle 2 was located directly behind Vehicle 1. Vehicle 2 then moved forward colliding with the rear of Vehicle 1. Vehicle 2 then drove around Vehicle 1 and made a left hand turn on to Washington St from the center lane of Commonwealth Ave. Vehicle 2 left the area without leaving any identifying information. As a result of the impact Vehicle 1 sustained damage to the rear bumper. Since Vehicle 2 left the scene the damage to Vehicle 2 is unknown. No injuries were reported on scene, no transports were required. No tows were required. The operator of Vehicle 1 identified as Borris Partensky stated that he was stopped in traffic in the left turn only lane of Commonwealth Ave westbound awaiting the red light. Partensky stated that he was then rear ended by Vehicle 2. Partensky stated that he then got out of this vehicle and signaled to the operator of Vehicle 2 to stop. Partensky stated that Vehicle 2 did not stop, quickly drove around Vehicle 1 and made a left hand turn from the center lane of Commonwealth Ave on to Washington St. Partensky described the operator of Vehicle 2 as a white male in his early 20's with dark hair. Partensky described Vehicle 2 as a newer model Chevrolet Silverado Pickup color white with a paper temporary license plate. Partensky states that the license plate may have started with the numbers "33" and ended with "IT". The state of the license plate is unknown. Partensky stated that his vehicle is a Tesla which has the ability to take photographs. Partensky states he will download the images and send them to me. I also spoke to witness Deb Finamore. Finamore states that she is a city of Newton employee and at the time of the crash was operating her personal vehicle (Ford Escape color Black).Finamore also stated that prior to the crash between Vehicle 1 and Vehicle 2 she observed Vehicle 2 tailgating her vehicle as she was traveling westbound on Commonwealth Ave. Finamore stated that Vehicle 2 tried to pass her several times on Commonwealth Ave and eventually made a left turn off of Commonwealth Ave on to Temple St. Finamore states that she was stopped in traffic at the intersection of Commonwealth Ave and Washington St and was located directly in front of Vehicle 1. Finamore states that she then observed Vehicle 2 located behind Vehicle 1. Finamore states that she that when she got a green signal she made a left turn onto Washington St and heard the impact of the collision between Vehicle 1 and Vehicle 2. Finamore states that she then observed in her rear view mirror Vehicle 2 drive around Vehicle 1 and make a left turn southbound onto Washington St. Finamore states that she attempted to get the registration information of Vehicle 2 however Vehicle 2 continued southbound on Washington St at a high rate of speed. Finamore states that she then followed Vehicle 2 at a safe distance and observed Vehicle 2 commit a red light violation at the intersection of Washington St and the entrance of Newton Wellesley Hospital (2014 Washington St). Finamore states that Vehicle 2 went around stopped traffic at the intersection by entering the left turn only lane to pass stopped traffic to the right. Finamore states that she then observed Vehicle 2 commit another red light violation at the intersection of Beacon St and Washington St. Finamore states that Vehicle 2 entered the left turn only lane at the intersection to pass stopped traffic to the right. Finamore states that she then lost sight of Vehicle 2 as it continued southbound on Washington St. Finamore described Vehicle 2 as a newer model Chevrolet pickup truck color white. Finamore believes that Vehicle 2 had a paper temporary plate, unknown registration number or state of registration. Finamore described the operator of Vehicle 2 as a young white male in his 20's. On 08/03/2020 I was able to view photo's of Vehicle 2 taken by witness Deb Finamore. The photo's revealed Vehicle 2's rear registration plate as 333991T. An NCIC check revealed that this registration belongs to a 2019 Chevrolet Silverado color white out of Maryland. The Maryland RMV also lists the vehicle registration as stolen. No further information was provided by the Maryland RMV. In the photo's Vehicle 2 also appears to be a 2019 Chevrolet Silverado color white. At this time I am unable to identify the operator of Vehicle 2. I called and notified the owner of Vehicle 1 Borris Partensky of my findings. Partensky will be providing this information to his insurance company.

On Friday, September 18, 2020, at 88update88approximately 0657 hrs, while assigned to N495, I was dispatched to the area of 1221 Centre St for a report of a MV accident. When I arrived on scene I observed a Motor Cycle (MC) laying on its side in the middle of the street. I observed a party laying on the ground, while medical personnel were tending to him. I also observed a white Honda (MV#1) pulled off to the side of Pelham St, and the operator standing next to his vehicle. Operator of MV#1 stated that as he was attempting to make a left turn off of Centre St, onto Pelham St. Operator of MV#1 stated that while he was in the middle of his turn, he observed a MC driving at a high rate of speed coming towards him. Operator of MV#1 stated that he was not able to get out of the way of the MC before it crashed into him I was not able to speak with the operator of the MC, due to him being transported to Brigham and Womens Hospital. Ofc McSweeney stated that he will be following up with the operator of the MC and will advise me. It should be noted that the operator of the MC appeared to be wearing headphones while operating, picture was taken and submitted. An anonymous party stated to me that he observed the MV traveling at a high rate of speed, heading North on Centre St. The party stated that he observed the MC weaving in and out of traffic. Multiple pictures were taken of MV#1 and MC and were submitted to IT to be attached to this report. Operator of MC was transported to Brigham and Womens. Operator of MV#1 was cleared with a patient refusal. Both vehicles were towed by Todys. **UPDATE** I spoke with the father of Scott Mahon, Keith Mahon. Keith stated that his son is still in the hospital, and has received several surgeries. Keith stated that the injuries his son received are a Open Tibia/Fibula fracture of the right leg, and a concussion. Keith stated that Scott has no recollection of the incident, as far as to not remembering leaving the house that morning.

Narrative:
On 09/30/2020 at approximately 19:32hrs members of the Newton Police Patrol Bureau responded to a motor vehicle crash near the intersection of Washington St and Eddy St. After an initial investigation of the crash the Patrol Bureau secured the crash scene and called in the Newton Police Traffic Bureau to investigate. At approximately 19:45hrs Sgt. Devine, Ofc Wilson and I (Ofc Schlegel) from the NPD Traffic Bureau responded to the scene to investigate. Upon arrival to the scene I was met by Ofc Kane of the NPD Patrol Bureau. Ofc Kane stated that the crash was located in the driveway of 74 Eddy St and that it involved a Ford F350 pickup striking two parked cars. Ofc Kane stated that the operator identified as Kyle Butler had already been taken to Newton Wellesley Hospital by Cataldo EMS for injuries. . Upon approaching the crash scene I observed the following: The crash occurred at night time hours at approximately 19:32EST near the intersection of Washington St and Eddy St. Both Washington St and Eddy St are public ways in the City of Newton. The weather conditions at the time of the crash were a dark and clear with a temperature of approximately 60 degrees F. Washington St is a four lane undivided roadway (2 westbound travel lanes and 2 eastbound travel lanes). Washington St lighted roadway which has concrete light poles on both the westbound and eastbound sides. Washington St was under construction at the time and had a base coat of asphalt applied. At the time of the crash Washington St did not have any painted lane markings due to ongoing construction. Washington St is separated from the road sides on both the eastbound edge and the westbound edge with fixed granite curbing. The roadway also had several orange traffic barrels and traffic cones located edges of the eastbound and westbound lanes. Affixed to the light poles and the traffic barrels were temporary no parking signs for the ongoing road construction. At the time of the crash no road construction activities were taking place. No major debris or other obstructions were observed in the roadway. Washington St is a major roadway in the City of Newton and travels west to East across the entire City of Newton. This area of Washington St where the crash occurred is in between West Newton Square and Newtonville Square, this area has a posted 35 MPH speed limit. Both West Newton Square and Newtonville Square are densely populated with several businesses and residential areas. The section of Washington St where the crash occurred is also traveled frequently by pedestrian, bicycle and motor vehicle traffic. At the time the crash occurred 19:32hrs This area of Washington St is still a heavily travelled roadway. I observed Vehicle 1 described as a 2003 Ford F350 color white (MA Reg 5DT664) located in the driveway of 74 Eddy St. 74 Eddy St is a multi-family dwelling house located on the corner of Washington St and Eddy St. The driveway of 74 Eddy St is on the Washington St side. Vehicle was faced easterly across the driveway. I observed Vehicle 1to have extensive damage to the entire vehicle including but not limited to the enclosed cab area, cab roof and all windows, front end, rear end, along with both driver and passenger side. I also observed city of Newton owned concrete traffic light resting on top of the cab area of Vehicle 1. Upon further observation of Vehicle 1 it appears that both front tires and rims were flat, heavily damaged and had little to no tread left. Both front tires were worn down to the interior steel belt. I also observed that Vehicle 1 did not have a valid inspection sticker. Located to the immediate front of Vehicle 1 and resting on the front end was a white fence which had damage consistent with the impact of a motor vehicle. Immediately to the driver's side of Vehicle 1 was Vehicle 2 described as a 2007 gray Subaru Forester (MA Reg 45D610. Vehicle 2 was facing westerly across the driveway. Vehicle 2 had extensive damage to the driver's side front end, rear end, in addition to the entire driver and passenger sides. At this time Vehicle 2's drivers door was in contact with Vehicle 1's drivers door. Both drivers doors could not be opened.The damage to Vehicle 2 appeared to be consistent with impact from another motor vehicle. To immediate passenger side of the Vehicle 2 I observed Vehicle 3 described as a 2011 Toyota Rav4 color black (MA Reg 47XB99). Vehicle was facing in an easterly direction across the driveway. I observed the Vehicle 3 to have extensive damage to driver's side rear quarter panel, passenger side front quarter panel and front end. Also observed was damage to south east corner of the dwelling of 74 Eddy St. This area of the dwelling is located adjacent to the drive way. The damage appeared consistent with the impact caused by a motor vehicle. Also observed were large amounts of debris located in the area of the crash which included the sidewalk, yard and the driveway of 74 Eddy St. The debris included pieces of concrete which appeared to have been dislodged from the concrete light pole. A city of Newton no parking sign was lying in the yard of 74 Eddy St. This parking sign would have been attached to the concrete light pole due to ongoing road construction on Washington St. I also observed two tire marks on a curb stone located next the westbound lane of Washington St. These tire marks were in front of the residence of 74 Eddy St and were to the immediate left of a storm drain. The tire marks were consistent with being struck by a motor vehicle. In between the tire marks and just after the curb there was a hole in the ground with wires sticking out of it. This hole appears to be the location the concrete light pole was located prior to impact. Tire marks were located on Washington St traveling from the east bound lane easterly across the westbound lane and ending at the tire marks on the curb. The total length of the tire marks was approximately 190 feet. After conducting an on scene investigation it appears that Vehicle 1 from the start of the tire mark in eastbound travel lane of Washington St traveled 190 feet in the easterly direct across the westbound lane of Washington St and impacted the curb in front of 74 Eddy St. The tire marks had lateral striations almost the entire length indicating that the front tires were rolling and sliding with little to no braking prior to impact with the curb. After impact with the curb Vehicle 1 exited the roadway, struck a concrete light pole. Vehicle 1 then traveled over a sidewalk, across the yard of 74 Eddy St and into the driveway of 74 Eddy St. Located in the driveway of 74 Eddy St were Vehicle 2 and Vehicle 3. Vehicle 2 was parked facing toward the rear of 74 Eddy St. Vehicle 3 located to the passenger side of Vehicle 2 and was parked facing towards Washington St. When Vehicle 1 entered the driveway it struck Vehicle 2. The force of the impact of Vehicle 1 striking Vehicle 2 caused Vehicle 2 to strike Vehicle 3. The force of the impact of Vehicle 2 striking Vehicle 3 caused Vehicle 3 to strike the southeast corner of the dwelling located at 74 Elliot St. Vehicle 1 continued across the drive way and struck a white fence before coming to final rest. Vehicle traveled a approximately 55 feet from when it initially struck to the curb till final rest. No other injuries were reported on scene. All involved vehicles were towed from the scene by Tody's Towing. After the investigation the operator of Vehicle 1 Kyle Butler was issued Citations T2015882 and T2015883 for: Mgl 90/24/O Recklass Operation of a Motor Vehicle Mgl 89/4B/A Failure to Operate in the Right Lane Mgl 90/20 Failure to Submit a Motor Vehicle for Inspection Mgl 90/7 Defective Equipment (worn front tire treads) Mgl 90/13A Failure to wear a seatbelt An immediate threat form was also filed with the MA RMV. On November 1st, 2020 at approximately 14:02 hours while assigned to N491 I responded along with Newton Fire and Medic One to the intersection of Pearl St @ Jackson Rd for a MV crash with minor injuries. On my arrival I first observed in the intersection a green colored Volkswagon with Vermont tag,HPK270 with extensive damage. The other involved vehicle a white Jeep Wrangler, bearing Ma tag, 8WK846 was located on the property of the Lincoln Elliot School, 191 Pearl St, also with extensive damage. The operator of the green Volkswagon was identified as Viorel Marin. He stated he was going E/B on Pearl St and was approaching the intersection of Jackson Rd when the light had turned green. As he was entered the intersection vehicle #2 came from his right side(N/B) on Jackson Rd at, according to him, a high rate of speed causing his vehicle to collide with the Jeep. He doesn't remember much after that. I observed a small cut to his right knee and later he showed me a small abrasion to his forehead. All his air bags had deployed on contact with vehicle #2 . I located the operator of the Jeep who was sitting on the ground being assisted by an unidentified women(not a passenger). She appeared shaken but had no visible signs of any serious injury from the crash. She identified herself as Tara Feeley. According to Ms Feeley she had been travelling N/B on Jackson Rd and was attempting to continue straight across the intersection of Pearl St when she was struck by vehicle #1 in the intersection. The impact caused her vehicle to flip over unknown amount of times finally landing on the property to the Lincoln Eliot School. Based on operator #2's statements, my observations of the scene, damage to her MV, it appeared her vehicle after contact with vehicle #1, flipped over N/B across the intersection first hitting the traffic light on the northeast corner, additionally making contact with the utility pole just east of the traffic light before finally landing right side up on the property of the Lincoln Eliot School. She reported climbing out her passenger side window when her vehicle finally stopped. She said,"she was lucky she had her seat belt on". I agreed. Ms Feeley also reported that she was sure that her light was green as she entered the intersection. I attempted to locate any possible witnesses from the crowd that were gathered near the crash site. There were two that heard the crash but none that actually saw what happened. I couldn't locate any cameras near the intersection. Both vehicles were towed from the scene due to the extensive damage/both totaled. Both operators had minor injuries and both declined to be transported to the hospital. They signed patient refusals. Officer Danny Anderson later took several pictures of the crash scene and the all the property damage. Both utility companies were notified. The camera disc was placed in the technology bureau mailbox.

Operator of vehicle one ( MA reg 1HK920 2010 Toyota Tacoma ) Brian Robidoux stated that on 11/06/2020 at 14:49 hours he was driving west bound on Auburndale Ave near Lexington St. Both Auburndale Ave and Lexington St are public ways in the City of Newton. Robidoux stated that west bound traffic had a green light at the intersection of Lexington St, and that north and south bound traffic on Lexington St had a red light. Robidoux stated that as he turned his vehicle left into the north bound lane of Lexington St was hit on the front driver's side quarter panel by motorized bicycle that did not stop for the red light. Robidoux stated that he was not injured in the crash and his vehicle did not required a tow. The operator of the motorized bicycle Arif Ahmed stated that he was riding north bound on Lexington St on a Lectric XP motorized bicycle. The vehicle that Ahmed was riding on meets the definition of a moped set forth by MGL 90/1, specifically that it's top seed was not in excess of 30 MPH. According to the owners manual that I found on line the top speed for the Lectric XP is 20 MPH. Ahmed stated that he believed that north bound traffic on Lexington St had a green light but he could not be sure. Ahmed also stated that he was unfamiliar with the area and it traffic patterns. Ahmed stated that he saw Robidoux vehicle and thought it would stop. Ahmed stated that at the time of crash be estimated that he was travailing approximately 12 MPH. Ahmed was transported via EMS to Newton Wellesley Hospital form treatment of lacerations to his hands, legs and possible internal injuries. During the investigation of this crash I discovered via the MA RMV data base that Ahmed's driver's licenses was suspend on 10/05/2020 as a part of a CWOF for 90/24 OUI . Ahmed is on active probation of out Natick Distinct Court until 09/24/2021. Under Comm v. Lopez 91 Mass App. Ct. 485 ( 2017 ) the charge for operating a moped with a suspended license should be charged as 90/10 , operating with out a license as opposed to 90/23 operating with a suspended licenses. Under MGL 90/1d moped must have one number plate issued by the MA RMV , and Ahmed's moped did not. I took photos of Robidoux's vehicle and of Ahmed moped and the disk was downloaded by the NPD's IT Bureau. Based upon statements made to me as well as information from the MA RMV data base I issued Ahmed MA Criminal Application T2015903 and cited him for MGL 90/10, operating with out a license, 89/9 failure to stop for a red light and 90/1d, unregistered moped.

On 11/10/20 I was working N492 when I responded to the area of Albemarle Rd. at Crafts St. for a 2 car MVA. Upon arrival I spoke with the Operator of Veh 2 Jane Connor. Connor stated she was traveling southbound on Albemarle Rd. when she came to the stop sign at Crafts St., which she came to a complete stop at. However she believed she had enough time to get across the intersection and pulled into the roadway causing a collision with Veh 1. Connor stated it was her fault and she had misjudged the distance and speed of Veh 1. I then spoke with the operator of Veh 1 Tessa Lanfear. Lanfear stated while traveling east bound on Crafts St. Veh 2 Pulled out of Albemarle Rd. into the roadway in front of her causing a collision. Todys responded and towed Veh 2 at the owners request. Newton Medics obtained patient refusals for all parties involved. All parties were provided with a crash report number.

On 1/2/2021 at approx 0847hrs while assigned to 497 I responded to the intersection of Washington St and Quinobequin Rd for a report of a MV Crash. Upon arrival I observed Ma Reg 35GM68 with front end damage stopped in the right eastbound lane of Washington St at the intersection of Quinobequin Rd, the operator, Silvaneide FADIL was still inside the vehicle with all front airbags deployed. FADIL denied being injured and stated she was coming through the green light from Wales St when the truck in front of her suddenly stopped causing her to crash into it. I spoke with the operator of the truck, who had managed to pull off to the side of the road, Joshua VINCENT, Ma Reg 1182BY, who stated he had just started into the intersection from Wales St after getting the green light when he observed a red Ambulance with lights flashing approaching from Quinobequin Rd, he stopped to allow the ambulance to pass when he was struck from behind by FADIL. VINCENT expanded that as he had just started into the intersection, he was going at a slow speed and did not need to stop suddenly. I was approached by a witness, Donald MCCARTHY( 617-438-1207) who stated he observed the ambulance come though and felt FADIL could not have been paying attention to not see it and stop. Both FADIL and VINCENT signed patient refusals with Fallon Ambulance, FADILS vehicle towed by Todys Towing.

Operator Diego Colindres states he was stopped at the stop sign located on Central St westbound at Grove St. As he proceeded through the sign to drive onto Grove St, a female jogger ran out into the street from the sidewalk in front of 16 Grove. Colindres reports he did not see the jogger until his front bumper made contact with the jogger's leg, which in turn knocked her to the ground. The jogger then got up, said she was ok, and tried to continue on her way. Colindres asked her to stay on scene and wait for emergency personnel, to which she obliged. It should be noted that there are bushes located in front of 16 Grove St which substantially hinder the view of the sidewalk for vehicles approaching Grove from Central. Jogger Rachel Coffin confirmed that she was crossing Central St heading southbound on Grove St when she was hit by vehicle #1. Coffin was not sure if she stopped prior to crossing the street, and did not see the vehicle until contact was made. She does recall that the vehicle was traveling at a slow rate of speed when she was struck. Coffin's attire consisted of dark colored shorts, a navy t-shirt, and white sneakers. I observed a light red area on Coffin's outer left thigh consistent with the height of vehicle #1's front bumper. She was tended to by the medics and declined transport, repeatedly saying that she just wanted to get back to school (Lasell) and do her homework. Due to Coffin's uncooperativeness and multiple requests to leave the scene, pictures of her minor injury were not taken. There was no visible damage to vehicle #1. Traffic responded, however Coffin wished to leave prior to arrival.

On Friday, March 19, 2021 while assigned to Traffic unit N525, I responded to the intersection of Boylston Street (E) and Walnut Street for a report of a crash involving a City of Newton owned vehicle. Boylston Street and Walnut Street are both public ways in the City of Newton. The weather at the time of the crash was clear and sunny. The road surface was dry. I spoke with the operator of MV1, Ms. Lizet Trabelsi (S07390016). Ms. Trabelsi stated she was operating her 2013 Infinity JX35 (MA: 8SXE60) eastbound at the Boylston Street (E) on-ramp near Walnut Street. Ms. Trabelsi stated as she was merging onto Boylston Street from her stopped position, a vehicle traveling on Boylston Street (E) abruptly took a right turn in front of her and continued to travel onto Walnut Street (S). Ms. Trabelsi stated she applied her brakes to avoid a collusion. At this time, MV2's front bumper area crashed into the rear of her vehicle. Ms. Trabelsi reported no injuries. I observed minor damage to the rear driver side bumper area of MV1. I then spoke with the operator of MV2, Mr. Paul Whelan (S63212595). Mr. Whelan is an employee of the City of Newton Water Department and was operating a City of Newton owned 2016 Ford Escape (MA MVN: M2530A). Mr. Whelan stated he was stopped behind MV1 waiting to merge onto Boylston Street (E). Mr. Whelan stated as MV1 began to merge onto Boylston Street, a vehicle already traveling on Boylston Street (E) turned off onto Walnut Street in front of MV1, causing them to brake quickly. Mr. Whelan stated the front bumper area made contact with the rear bumper area of MV1. Mr. Whelan stated he was traveling at a very low speed. Mr. Whelan reported no injuries. Photos of both vehicles were taken and submitted to the IT Bureau.

On 04/29/2021 at 15:22 hours I was dispatched to 53 Brentwood Ave for a report of a motor vehicle crash involving a bicyclist. Upon my arrival, I made the following observations of the crash scene. Brentwood Ave is a two way non divided public way in the City of Newton that runs east to west from Greenlawn Ave to Walnut St. Brentwood Ave is a thickly settled residential area with a 25 MPH speed limit. The roadway conditions at the time of the crash were wet due to rain. I observed MA reg 111ZS1 a 2020 Honda Civic parked on the side of Brentwood Ave just passed the intersection of Judith Rd facing westbound. I observed that the Honda had damage to it's front driver's side bumper. When I inspected the Honda I observed that the driver's side headlight casing was cracked and a piece of it was missing. I further observed an approximately six inch in diameter spider web like crack in the lower driver's side windshield. The windshield crack was inward , indicating that an external object had caused the damage. There was another single crack onn the driver's side windshield that extended from the hood to the roof. The operator of the Honda , Lisabeth Ioannilli stated that on 04/29/2021 at 15:20 hours she was driving westbound on Brentwood Ave near the intersection of Judith Rd. Ioannilli stated that she estimated that the speed of her vehicle was approximately 25 to 30 MPH. Ioannilli stated that she did not see a bicyclist in her path of travel until the point of impact when her vehicle was aproximately 3/4 of the way passed the intersection of Judith Rd. Ioannilli stated that her vehicle's front driver's side bumper made contact with the bicyclist. Ioannilli stated that the bicyclist was tossed off of her bike and onto the hood and windshield of her vehicle before rolling onto the ground. Ioannilli stated that she pulled her vehicle to the side of the road and went to check on the bicyclist. Judith Rd is a public way in the City of Newton that runs east and then turns south from Walnut St to Brentwood Ave. There are no traffic control devices at the intersection of Judith Rd and Brentwood Ave. Ioannilli stated that she was not injured in the crash and her vehicle did not require a tow. At the crash scene, the bicyclist , Lucia Campbell, a student a FA Day Middle School ( DOB 11/08/2006 ) was being treated in an ambulance and was transported to Boston Children's Hospital for treatment. Campbell was conscious and was able to answer questions from EMS members. Campbell stated that she was wearing a helmet at the time of the crash and that she rode her bike south on Judith Rd and was turning eastbound onto Brentwood Ave when the crash occurred. I spoke with Campbell's parents Adam Campbell and Iris Delasota who stated that their daughter was under going x rays and other tests to determine if she has internal injuries. I took photo's of the bike, Honda and crash scene and the disk was downloaded by the NPD's IT Bureau. Based upon my investigation of this crash I concluded that Ioannilli was at fault for it. I issued via US mail Ioannilli MA Uniform Citation T1446633 and cited her for a violation of MGL 90/14 failure to use care while approaching a bicyclist. Campbell's bike was turned over to her parents at the crash screen.

On Monday May 10th, 2021 at approximately 0825 hours, dispatch sent units to the intersection of Eldredge Street and Vernon Street for a report of a road rage incident involving a crossing guard and a motor vehicle. When units arrived it was determined that the crossing guard who was performing her duties at the time of the incident, was struck by a motor vehicle that left the scene without stopping. I responded to the scene of the accident to conduct an investigation. At the time of the incident the weather was cloudy and the road surface was dry. Eldredge Street and Vernon Street are both public ways owned and maintained by the City of Newton. Upon arrival I spoke with the injured crossing guard inside the ambulance identified as, Margaret Monbouquette. Margaret was being treated by Fallon Medic 3 for leg pain. Margaret stated she was in the middle of the intersection holding her stop sign up as children and adults were actively crossing to get to the Underwood School. Margaret stated she had all three streets stopped when her attention was brought to the vehicle at the stop sign on Eldredge Street on the one way side. Margaret stated it kept inching forward into the intersection attempting to turn left. Margaret stated she still had people crossing at that intersection so she stepped in front of the vehicle and held up the stop sign directly in front of the vehicle and the female driver. Margaret stated she looked at the female driver and stated "No, stop" and that is when the driver started to yell back at her inside her vehicle but she could not make out what she was saying as her window was rolled up. Margaret stated the driver then tried to drive around her and struck her with the front passenger side bumper of the vehicle. She stated she was struck in her hip area which caused her to fall onto the street. Margaret stated the vehicle never stopped and sped up Vernon Street. Margaret described the driver as a slight female with dark hair driving a dark colored SUV. Margaret was able to provide a license plate of MA REG: 1XCS97. A query of the license plate through dispatch was able to provide an address of 74 Charlesbank Rd, Newton. Officers checked this address but got no response at the door and the vehicle was not located. A witness to the incident identified as Alicia Matos left her phone number and I was able to make contact with her over the phone. She stated she was in her motor vehicle stopped at the stop sign on Eldredge St, directly across from the vehicle involved. Alicia stated she was stopped because the crossing guard was in the intersection holding up her stop sign and was actively crossing children and adults. Alicia states she noticed the vehicle across from her kept creeping out into the intersection trying to take the left turn up Vernon Street. She states the crossing guard walked over to that vehicle and told her to stop and saw the female driver start yelling and pointing her hands at her inside the vehicle. Alicia states the crossing guard then backed up and was looking down at the drivers license plate when the driver drove at the crossing guard and struck her with her front passenger side and knocked the crossing guard to the ground. Alicia states the vehicle clearly made contact and did not stop and left the scene turning onto Vernon Street at a high rate of speed. Alicia states several people in the area came to the crossing guards aid and police and medics arrived shortly. Alicia described the driver of the motor vehicle to be a female with dark hair. The principal of the Underwood School, Suzie Talukdar was present after the accident and stated the secretary of the school received a phone call shortly after the incident from a grandparent of the school. The grandparent was identified as Dawn Coleman. Dawn stated her daughter was driving the vehicle involved and was having anxiety over the incident and didn't know what to do. Dawn identified the driver as Chelsey Sullivan (S83874401). I attempted to make contact with Chelsey at her residence of 74 Charlesbank Rd with negative results. The secretary was able to provide phone numbers for Dawn and Chelsey and I responded to the police station to attempt to make contact. While at the police station, I was notified that Chelsey Sullivan was downstairs in the lobby. I met with Chelsey and she stated the following: She was driving MA REG 1XCS97 a black Toyota Rav4 and was stopped at the stop sign on Eldredge Street the one way side. She noticed the crossing guard in the intersection holding up the stop sign but stated there was no one crossing at the time. Chelsey stated she started to inch forward and wanted to turn left onto Vernon Street as she was in a rush. Chelsey stated the crossing guard came right up to her vehicle and started shouting and holding up the stop sign in her hand telling her to stop. Chelsey stated she was yelling back at her stating that the cars behind her were going so she wanted to go too. Chelsey stated when the crossing guard started to walk away from her, she got very anxious and decided to take the left onto Vernon Street. She stated she saw the crossing guard stumble and fall to the ground but at no time did she think she made contact with the crossing guard. Chelsey stated she drove home and called her mother to tell her about the incident. Chelsey stated she wrote an apology letter to the crossing guard and has really bad anxiety and wasn't sure what to do. I advised Chelsey she needs to follow the crossing guards instructions when they are performing their duties no matter if she thinks the road is clear for her to enter/pass. Chelsey stated she understood and was also advised she will be getting a citation and summons in the mail. Chelsey took me to her vehicle where I took several photographs and submitted them to the IT Bureau. No damage was reported or observed. Massachusetts Uniform Citation 997066AA and criminal application was issued and mailed to Chelsey for Chapter 90 section 24 Operating to Endanger and Chapter 90 section 14 Failure to slow near pedestrian.

Operator of MV2 states that he was on Vine St and stopped at the stop sign at the Brookline St intersection. He then stopped again past the crosswalk while waiting for all traffic on Brookline St to pass. He said that MV1 came "out of nowhere" at a high rate of speed NB on Brookline St before striking his vehicle. Both vehicles were towed by Tody's towing service. Operator of MV2 was given citation T2015012 for failing to stop, yield at an intersection.

Vehicle #1 stated he was traveling southbound on hammond st. Vehicle #1 stated he was stopped at the red light and once the light turned green, he proceeded through the intersection when he witnessed Vehicle #2 speed into the intersection and he was unable to stop. Vehicle #1 struck Vehicle #2 in the rear right side. Vehicle #2 stated they were traveling westbound on commonwealth ave when they stated they had a green light and drove through the intersection. Vehicle #2 stated they were struck from the side by Vehicle #1. A witness who was in Vehicle #3 behind Vehicle #1 stated they were stopped at a red light on hammond st. and when the light turned green she witnessed Vehicle #2 blow the red light and was struck by Vehicle #1. The witness stated she and Vehicle #1 had a solid green light at the time they entered the intersection. At the time of the crash there were four parties inside Vehicle #2. After the crash Vehicle #2 left the scene after they thought Vehicle #1 had left and dropped off two passengers before returning to the scene of the accident. I spoke with the two passengers who were not present at the scene who both stated they were inside Vehicle #2 at the time of the crash and Vehicle #2 had a clear red light which they failed to stop for. One of these passengers was complaining of a head injury and was currently at a hospital being evaluated. Vehicle #1 had moderate damage to the front of the vehicle and was able to be driven away. Vehicle #2 had moderate damage to the right rear side and was able to be driven away. All drivers and passengers on scene were asked if they were injured which they stated they were not and declined any medical attention. Due to multiple witnesses stating Vehicle #2 failed to stop for the red light, a citation was issued to the driver for MGL 89/9 Fail to stop/red light on MA Uniform Citation T2080447

The Operator of MV1(MA reg 3CHE81) stated she was traveling WB on Newtonville Ave approaching the intersection of Lewis street/Lewis Terrace when her vehicle was struck on the front passenger side. Operator of MV1 stated she was approaching the intersection at a slow speed when operator of MV2 pulled onto Newtonville Ave from Lewis Terrace. Operator of MV2 (MA Con V57606) stated he was stopped at the intersection of Newtonville Ave and Lewis Terrace waiting to turn right onto Newtonville Ave. Operator of MV2 stated he attempted to pull onto Newtonville Ave when his vehicle struck MV1. The passenger side airbag of MV1 was deployed. Medics and Fire arrived on the scene to assist with injuries. The passenger in MV1 sustained minor injuries due to the airbag but signed a refusal. No one else involved claimed an injury. Both vehicles suffered significant damage and required tows. Tody towed both vehicles from the scene. The operator of MV2 was issued a citation (037739AB) and I mailed the citation.

MV 2 was headed southbound on Watertown Street and attempting to take a left turn to head eastbound on Craft street when it struck MV1. MV2 had passenger front wheel well damage. MV2 stated that it tried to brake but could not stop on time. MV 2 stated that MV 1 might have been speeding when the crash occurred. Both parties involved in the MV accident had no injuries.

On Tuesday 7/6/21 at approx 1254 hrs, while assigned to N494, I responded to the intersection of Hartman Rd and Brookline St for a 2 car MVA. Operator of MV1 states she was coming from Boston heading SB on Brookline St when MV1 abruptly pulled out in front of her from Hartman Rd. Operator of MV2 states he stopped at the STOP sign at the end of Hartman Rd (Note Brookline St has no STOP signs at this intersection). He said he saw traffic on the NB side of Brookline St stopped and then attempted to take a left. He states that MV1 was going at a high rate of speed and did not have time to avoid the accident.

On Friday, July 16, 2021, while assigned to Traffic unit N525, I responded to the intersection of Commonwealth Avenue and Temple Street, Newton for a crash involving a City if Newton Fire Department vehicle. The weather at the time of the crash was clear and sunny. The road surface was dry. Commonwealth Avenue and Temple Street are both public ways maintained by the City of Newton. I spoke with the operator of MV1, Mr. Jacob Forbes. Mr. Forbes stated he was at a stop sign on Temple Street and was attempting to cross Commonwealth Avenue (N) from Temple Street. Mr. Forbes stated he looked to his right, and then to his left and did not see any vehicle approaching. Mr. Forbes stated as the began to enter the intersection he looked to his right and saw a vehicle pulling over to the side of the road and a Newton Fire truck with it's lights on was heading towards him. Mr. Forbes stated he could not avoid the collision and the front end of MV2 crashed into his passenger side. Mr. Forbes reported no injuries and signed a patient refusal with Fallon Medics. I observed damage to the front passenger side area of MV1 and it was removed from the roadway by Tody's Towing. MV2 is a City of Newton Fire Department vehicle "C2". It is a 2019 Ford Expedition (MF455). The vehicle was responding to a Fire Alarm Activation call with emergency lights and siren activate at the time of the crash. The occupants of the vehicle were taken to Newton Wellesley Hospital for evaluation. I observed front end and driver side damage to MV2. There was air bag deployment inside the vehicle, on both the front driver and passenger side. Tody's Towing removed MV2 from the roadway. Photos were taken of the crash scene and submitted to the IT Bureau. I responded to Newton Wellesley Hospital and spoke with Newton Firefighter John Morris (Driver) and Deputy Chief Michael Mcnamara (passenger) in the emergency room. Both parties stated they were responding to a call with emergency lights and siren activated. FF Morris stated as he approached the intersection of Commonwealth Avenue (W) and Temple Street, he observed a black SUV exiting Temple Street onto Commonwealth Avenue. FF Morris stated as they were passing he believed MV1 tried to speed up to cross Commonwealth Avenue. MV2 then crashed into MV1. As a result of the crash, FF Morris suffered right hand/wrist injury. Both FF Morris and Deputy Chief Mcnamara stated they had slight respiratory issues from breathing in the powder from the air bag deployment. MV1 failed to yield to MV2's right of way as he attempted to cross Commonwealth Avenue from Temple Street. The operator of MV1, Mr. Jacob Forbes will be mailed Massachusetts Uniform Citation 071113AB for Chapter 89, Section 8 (Fail to Yield at Intersection).

On 7/27/2021 at approx 0939hrs while assigned to 497 I responded to the intersection of Fenwick Rd and Tamworth Rd for a report of a two car crash with possible injuries. Upon arrival I observed Ma Reg 1CNR59 being operated by Svertlana Savenco pinned between the southern curb of Tamworth facing Eastbound and Ma Reg 62EZ69 being operated by Renata Pawska. I spoke with Renata who stated she had been travelling NB on Fenwick Rd intending to turn right onto Tamworth when she saw Tamworth east of Fenwick was shut down due to a detour, at that time Renata turned left colliding with Svetlana. I spoke with Svetlana who stated she was travelling straight ahead when Renata turned into her. Both vehicles towed by Todys, NFD put down speedy dry for fluids, Renata signed a refusal with Fallon Ambulance, Svetlana was transported by Fallon complaining of leg and back pain.

Narrative:
On 07/28/21 at 13:39 hours, I (Traffic Unit # 523) responded to 10 Cummings Road for a past hit and run motor vehicle bicyclist crash which occurred approximately twenty minutes prior. Dispatch updated units that the accident occurred at the intersection of Watertown and Walnut Streets and that a white colored SUV left the scene after striking the bicyclist. The white SUV was last seen heading southbound on Walnut Street towards Washington Street. The bicyclist then walked back home to 10 Cummings Road and told her mother about the incident. The mother then notified NPD. Watertown Street and Walnut Street are both public ways maintained by the City of Newton. It is a four way intersection with crosswalks on every side. There are no turn on red signs on every corner of the intersection. The weather at the time was partly cloudy and sunny and the road conditions were dry. On arrival at 10 Cummings Road, I spoke to the mother identified as Vanessa Cruz and her daughter, Angelique Cruz, who was involved in the crash. Fallon Ambulance was already on scene and a patient refusal was signed as Angelique did not have any injuries. I asked Angelique what had occurred and she stated that she left FA Day Middle School at approximately 13:00 hours. It is estimated that the crash took place between 13:10 and 13:20 hours. She was then bicycling down to the intersection of Walnut & Watertown Streets. She says that she pushed the walk signal button to cross the street at the crosswalk to continue southbound on Walnut Street towards Lowell Avenue. Once the cross signal light activated, she bicycled through the crosswalk. At the tail end of the crosswalk, her rear bicycle tire on the right side was struck by a white SUV making a right turn on to Walnut Street from Watertown Street. She was then knocked to the ground where she sustained a scrape mark to her left knee on her sweat pants. The white SUV continued southbound on to Walnut Street without stopping. She feels that the driver might of misjudged taking the right turn and never realized that she struck her. Angelique says she only saw a female operator in the SUV and could not determine the race or age. She also did not observe the registration on the vehicle. She further stated that a female assisted her up from the ground. Once she said she was fine she then proceeded home. No one else phoned 911 regarding the incident. At this point, I asked to see the bicycle which was stored away. The bicycle is a Schwinn Sidewinder 21 speed color black with lime green block style writing for the brand and model on it. Both tires have white reflectors attached to them. I observed no damage to the bicycle. At the time of the crash, Angelique stated she was wearing purple sweat pants, blue sweater, and no bicycle helmet. She was also carrying a black backpack attached to her rear shoulders. I took digital photos of Angelique along with her Schwinn bicycle. After, I went over to the intersection of Watertown and Walnut Streets and took photos of the scene. A canvass was also conducted that produced negative results. I observed no surveillance cameras on the buildings or residences that are near the intersection. At 109 Walnut Street, I spoke to several tenants in the six unit apartment complex and they have no security cameras. 583 Watertown Street has no cameras along with 592 Watertown Street. I also checked near the intersection of Walnut and Washington Streets as the white SUV was last seen heading in that direction. The Trio apartment complex along with other businesses are lined against both streets and have outside security cameras. I spoke to the leasing office and they stated that the cameras do not provide any coverage of the street area.

On 08/09/2021 at 13:56 hours I responded to the intersection of California St at Adams St for a crash involving a motor vehicle and a bike. California St and Adams St are both public ways in the City of Newton. Upon my arrival I was informed the bicyclist , Joshua Cohen was transported to Newton Wellesley Hospital for treatment . I spoke with Marco Diaz who stated that he was driving his 2004 Honda Pilot ( MA reg 1YEM63 ) east bound on California St near Adams St. Diaz stated that as his vehicle passed Adams St he heard a loud bump coming from the rear passenger side of his vehicle. Diaz stated that he looked into his rearview mirror and realized that something had crashed into his vehicle. Diaz stated that he stopped his vehicle had saw a male on a bike had crash into his vehicle. Diaz stated that he was not injured in the crash and his vehicle did not require a tow. I observed small scrape marks on the rear passenger side quarter panel of Diaz's vehicle. Cohen's bike was a Metakoo 26'' electric bike, that has a top speed of 20 MPH. Cohen's bike did not appear to have any damage on it and was secured at Cohen's residence. I spoke with Cohen at the ER at Newton Wellesley Hospital were he was waiting treatment for a laceration to his forehead and a possible concussion. Cohen stated that he "was going to fast and took a too wide of a turn" from the north bound lane of Adams St onto the east bound lane of California St. Cohen stated "I hit into him " ( Diaz ) . Based upon statements made to me by Diaz and Cohen and my observations of the crash area I belive that Cohen is at fault for this crash. I took photos of Diaz's vehicle and Cohen's bike, the disk was downloaded by the NPD IT Bureau.

At approximately 1640HRs on Thursday, August 12, 2021, I came across a motor vehicle crash at The intersection of Grant ave. and Commonwealth Ave.'s Carriage Lane. MV 1 (MA Pass: 882WWK) was facing SB on Grant ave., in the intersection. I observed it to have extensive front end damage (bumper, hood, driver's side head light and a pushed in radiator core support) and it was leaking fluids. MV 2 (MA Pass:8RS517) was facing WB in the Carriage Lane and displayed damage to its front passenger's side ( hood, head light, bumper and fender). Upon speaking to the operator of MV1, they stated that they were traveling straight (SB) on Grant Ave. and had a green traffic light. As they approached the intersection, MV 2 made a left hand turn and struck MV 1. Upon speaking to the operator of MV 2, they stated they were traveling on Grant Ave. (NB) and attempted to make a left hand turn onto the Carriage Lane (WB). It was at this time they struck MV1 because they did not see them. Upon speaking to witness 1, they stated they were traveling behind MV1 and that the traffic light was indeed green. They then observed MV 2 traveling at a high rate of speed, as if they were attempting to "cut off" MV1. When MV 2 attempted to make a left hand turn, they impacted into MV1. All parties involved stated that they were not injured and did not wish to been seen by EMS. Newton Fire arrived on scene and cleaned up the fluids. MV1 was removed by Todys service and MV2 was able to drive away under its own power. I gave the operator of MV 1 a ride to NPD HQ to await their ride and then cleared.

Operator of vehicle one Sara Kolaczyk stated that while turning right from Albemarle Rd onto Crafts St her vehicle's front drive's side quarter panel struck pedestrian Mariana Pishtikova in the marked crosswalk. Kolaczyk stated that she was looking to her left at on coming traffic on Crafts St and had began to turn right when crash occurred. Kolaczyk stated that she did not see Pishtikova before the crash. Pishitkova stated that she entered the marked crosswalk at Crafts St and Albemarle Rd after she saw that Kolaczyk vehicle had stopped. Pishtikova stated that she walked around the front of Kolaczyk's vehicle and it then turned into her at a very low speed. Pishtikova stated that the front drivers side quarter panel of Kolaczyk's vehicle struck her right side. Pishtikova stated that her right ankle was sore but refused medical treatment. Pishtikova signed a patient refusal form provided to her by Newton Medic Unit Three. I took photos of Kolaczyk's vehicle and the disk was downloaded by the NPD's IT bureau. Kolaczyk's vehicle had no damage and did not require a tow. Kolaczyk stated that she was not injured in the crash. I issued Kolaczyk MA Uniform Citation T2016945 and cited her for a violation of MGL 89/11 , failure to yield to a pedestrian in a crosswalk. Crafts St and Albemarle Rd are public ways in the City of Newton.

Upon arrival, I observed vehicle 1 and vehicle 2 parked on Hammondswood Rd at the intersection of Hammond St and Hammondswood Rd. There was damage to the rear of vehicle 1 and heavy front end damage to vehicle 2. There was also a small spill of anti-freeze, that leaked from vehicle 2. Operator 1, Michael Spalla, stated he was driving with his son, Northbound on Hammond St and stated that vehicle 3 was traveling Eastbound on Hammondswood Rd. Vehicle 3 did not come to a complete stop at the stop sign at the intersection. To avoid a collision, Operator 1 stated that he stepped on his brakes and then vehicle 2 rear ended him. Operator 1 described vehicle 3 as a charcoal black small SUV, that had a Lyft decal on the window. Operator 1 was unable to provide a description of Operator 3. Operator 1 stated that vehicle 3 continued Eastbound on Hammondswood Rd and did not stop. Operator 2, Olivia Degeofroy, stated that she was traveling Northbound on Hammond St behind vehicle 1. She stated, she witnessed vehicle 3 not come to a complete stop at the intersection and when vehicle 1 stopped, she did not have enough time to stop as well. Operator 2 was unable to describe vehicle 3. Newton Fire was on scene and was able to lay down speedy dry on the roadway for the anti-freeze. Fallon Medics checked out all parties involved and they all declined medical attention. Operator 1 was able to drive his vehicle away, while Operator 2 called for a private tow company to respond to pick up her vehicle.

Upon approaching the intersection the operator of MV1 stated a vehicle entered the intersection from Hammondswood striking her rear driver's side door. After the initial impact the operator of MV1 lost control of her vehicle and it spun into oncoming traffic hitting MV3. The operator of MV2 (MA CON V29325) stated he was traveling EB on Hammondswood Rd when he approached the intersection at Hammond St. Operator of MV2 stated he stopped at the intersection and waited to proceed when it was safe to do so. While crossing the intersection he observed MV1 traveling at a high rate of speed coming NB on Hammond St. Operator of MV2 stated the operator of MV1 swerved around his vehicle striking the front bumper and then spinning out of control. The operator of MV3 (MA REG 1NTN27) stated she was traveling SB on Hammond St approaching the intersection at Hammondswood Rd. Operator of MV3 stated she observed the collision of MV1 and MV2 but she did not have enough time before MV1 struck her vehicle. Operator of MV3 stated she observe the operator of MV2 fail to come to a complete stop at the Stop Sign causing the accident. Medic 3, Medic 1, Ladder 3 and Engine 3 were on scene for the report of multiple injuries. The operator of MV1 and MV3 were transported to NWH with some minor injuries. MV1 and MV3 sustained heavy damage and were towed by Tody's. MV2 had minor damage and did not require a tow.

The operator of MV#1 stated she was travelling northbound on Centre St in the right side travel lane at the intersection of Beacon St when MV#2 merged into her lane from the left side travel lane striking her vehicle. MV#1 sustained minor damages to its driver's side front and rear doors. There were no reported injuries to the operator of MV#1 or its passenger. The operator of MV#2 stated he was travelling northbound on Centre St in the left side travel lane when the vehicle in front of him attempted to make a left turn onto Beacon St. The operator of MV#2 stated he attempted to go around the vehicle in front of him by merging into the right side travel lane when MV#1 came at a high rate of speed and made contact with his vehicle. MV#2 sustained minor damages to its entire passenger side. There were no reported injuries to the operator of MV#2. Newton Fire evaluated all parties involved and determined there were no injuries. The witness stated she was standing on Centre St at the intersection of Beacon St and observed the entire accident. The witness stated MV#1 was travelling straight on Centre St in the right side travel lane when it was struck by MV#2. The witness stated MV#2 was on Centre St in the left side travel lane directly behind a vehicle that was attempting to make a left turn. The witness stated MV#2 attempted to go around the vehicle in front it by merging into the right side travel lane striking MV#1.

On Tuesday, September 21, 2021, I received a call from Sgt. Mike Wade of the Newton Police Traffic Bureau. Sgt. Wade stated a serious MVA/Bicycle crash had just occurred at the intersection of Cherry Street and Derby Street in Newton and the bicyclist was a 10-year-old boy with a possible head injury. Sgt. Wade stated the Traffic Bureau was requested to respond by the Shift Commander and asked if I could respond to investigate the crash. On arrival, Officers reported to me that the cyclist involved, Benjamin Lavoie, was transported to Children's Hospital in Boston with a possible head injury. It was reported to them that Benjamin was traveling Southbound on Cherry Street towards Derby Street on his bicycle. Benjamin entered the intersection of Derby Street and crashed into the front passenger side area of a passing motorist that had a green light. Officers on the scene also reported that the motor vehicle and the bicycle involved had been moved from their original rest location. Cherry Street is a public roadway maintained by the City of Newton. It supports two way travel (N and S) from Washington Street (S) to Waltham Street (N). The travel lanes are divided by an unprotected double yellow line median. The posted speed limit for this street is 25 miles per hour. Derby Street is a public roadway maintained by the City of Newton. It supports two way travel (E and W) from Waltham Street (E) to Lexington Street (W). The travel lanes are divided by an unprotected double yellow line median. The posted speed limit for this street is 25 miles per hour. The weather at the time of the crash was clear and sunny. The road surface in the immediate area of the crash was dry. I spoke with the operator of MV1, Ms. Erica Pusateri (S61409866). Ms. Pusateri stated he was operating her 2021 Kia Seltos (MA: 8HC743) Westbound on Derby Street towards Cherry Street. Ms. Pusateri stated that she had a green light at the intersection and proceeded to cross Cherry Street to continue onto Derby Street. Ms. Pusateri stated halfway through the intersection she heard a noise and saw a bicyclist hit the front passenger side door area of her vehicle. The collision caused the glass window from the front driver-side door to shatter. Ms. Pusateri's vehicle controlled her vehicle to a stop and she observed a young boy lying on the roadway in the intersection behind her. Ms. Pusateri exited her vehicle, ran over to the injured bicyclist, and rendered aid to him by stabilizing his neck. Ms. Pusateri remained on scene and cooperated fully with Officers. I asked Ms. Pusateri if she was distracted at all while operating her vehicle prior to the collison. Ms. Pusateri stated she was not. I asked Ms. Pusateri if she could recall where in the intersection the bicyclist crashed into her vehicle. Ms. Pusateri stated the bicyclist crashed into her vehicle in the Northwest side of Derby Street just outside of the marked crosswalk in the intersection. Ms. Pusateri stated she knows it was in the street in the intersection because she observed a woman with a stroller on the sidewalk on the corner of Cherry Street at Derby Street on the Franklin Elementary School side and she had already travelled through the intersection passed the East side of Cherry Street. Due to the unknown injury status of the bicyclist, Ms. Pusateri's vehicle was towed to the Newton Police Station and secured in the garage area as evidence. Ms. Pusateri reported a small laceration on her right hand with no other injuries. I observed damage to the front passenger side fender/door area. There was no airbag deployment as a result of the crash. I observed glass through out the front passenger side compartment area of the vehicle from the window breaking. I contacted Benjamin' s parents, Cheryl and John, to check on the status of their son. Cheryl stated Benjamin did not suffer any life-threatening injuries and appears to have bumps, bruises, and a cut on his head. I asked if I could speak with Benjamin for his account of what happened at the crash. Benjamin stated he was riding his bicycle on Cherry Street (Southbound) on the sidewalk on the East side of Cherry Street. Benjamin stated he left the sidewalk and entered the intersection to cross Derby Street and continue on Cherry Street. Benjamin acknowledged that at this time Cherry Street had a red light and Derby Street had a green light. Benjamin stated he then crashed into a passing car in the roadway. Benjamin was wearing a helmet at the time of the crash. The helmet had significant damage to it as a result of the collision. I told Benjamin his bicycle was being held at the Newton Police Department for safekeeping and we will arrange to get it back to him. Benjamin is a student at the Franklin Elementary School. I make contact with a witness to the crash, Mr. Benjamin Crosby. Mr. Crosby stated he did not witness the actual crash, just the aftermath. I was able to reviewthe 9-1-1 call Mr. Crosby made regarding the crash in Newton Disptach. Mr. Crosby stated a boy on a bicycle was in a crash with a motor vehicle. Mr. Crosby is heard saying "he went through the light" to the Dispatcher taking the call. After completing my investigation, I have summized the following. The bicyclist (Benjamin Lavoie) traveled down Cherry Street (S) and went through a red light to cross the intersection at Derby Street (W). As a result of traveling through the red light and entering the intersection, he crashed into the front passenger side door area of a passing motorist that had a green light (Erica Pusateri). No citation will be issued as a result of this crash.

VEHICLE #1 STATES HE SWERVED TO THE RIGHT TO AVOID HITTING VEHICLE #2 STRAIGHT ON BUT WAS UNABLE TO AVOID HITTING HIM ALTOGETHER. VEHICLE #2 STATES HE WAS STOPPED AT THE STOP SIGN AT TEMPLE AND COMM. HE STATES HE HEADED ONTO COMM AVE GOING E-BOUND. AT THIS TIME VEHICLE #1 CAME OVER THE HILL AT A HIGH RATE OF SPEED AND THAT WAS WHY HE WAS HIT BY VEHICLE #1. VEHICLE #1 HAD LEFT FRONT END DAMAGE. VEHICLE #2 HAD LEFT REAR SIDE DAMAGE. BOTH PARTIES REPORTED NO INJURIES. BOTH PARTIES ADVISED TO CONTACT THEIR INSURANCE COMPANIES. CLEARED WITHOUT FURTHER INCIDENT. NEITHER VEHICLE WAS TOWED.

Operator of vehicle 1 states she was traveling northbound on Hammond St crossing Commonwealth Ave and had a green light. As she was crossing Commonwealth Ave vehicle 2 came across the front of her vehicle from her right (traveling westbound) and the vehicles collided. Operator of vehicle 2 stated she was traveling westbound on Commonwealth Ave and came to the intersection of Hammond St. Operator stated she had no idea if the light was green or red due to solar glare but continued through the intersection where she was struck on the drivers side by vehicle 1. Both operators and the two passengers of vehicle 1 signed patient refusals with Fallon medics. Vehicle 1 was able to be driven from the scene. Vehicle 2 was towed by Todys. The reporting party to the accident was an uninvolved witness. The witness told dispatch that vehicle 2 ran the red light causing the accident. I attempted to follow up with the witness by calling the number they called from (617-462-6762) with negative results. I did leave a voice mail and will attempt to follow up on my next tour of duty. ***On 10/07/2021 I was able to speak to the witness of the accident, Caroline O'Brien. O'Brien states she was on Hammond St behind vehicle 1 and states that they had the green light to proceed. She also states she saw vehicle 2 traveling westbound on Commonwealth Ave at a high rate of speed run the red light and strike vehicle 1.

On 10/06/2021 while assigned to marked cruiser N491 at approximately 15:26 hours I responded to 527 Washington Street for a report of 2 car MVA. Newton FD requested EMS for evaluation of Driver 1 because she was "feeling dizzy and shaky" after the accident. Medic 1 cleared with a patient refusal. Upon arrival on scene, I spoke to Ms. Marcia Ruth Drector (Driver 1 of MV1), who stated that she was at intersection of Washington Street and Adams St waiting for green traffic light to continue driving straight forward / eastbound on Washington Street. She said that when the traffic light turned green she started going forward, when all of a sudden a light blue sedan (MV3) cut her way driving southbound from Adams St to Lewis Ter. Ms. Drector stated that she had to slam on the brakes to avoid collision with MV3, which was traveling at high rate of speed and drove just a few inches away from the front of her car after she stopped. At the same moment, another car (MV2), which was behind her car (MV1), crashed into the rear end of her car (MV1). Ms. Drector could not recall any specific details about MV3 or its driver/occupants, other than it was a light blue sedan. I then spoke to Mr. Stephen Achille (Driver 2 of MV2), who stated that he was facing eastbound in the left lane on Washington Street. Mr. Achille said that he started changing lanes because a car (MV4) in front of him was waiting for a left turn to Adams St. He stated that when he was getting into the right lane, MV1 suddenly stopped in front of him and he crashed into it. I advised all parties that there will be an accident report on file for insurance purposes.

Operator of MV1 states he was traveling northbound on Walnut Street and entered the left turn only lane at the intersection of Walnut Street and Beacon Street. Operator of MV1 states while entering the left turn only lane, a pedestrian appeared from behind a car that was in the straight only lane and he struck her at a very low rate of speed with the front end of MV1. Operator of MV1 states the pedestrian said she was fine and kept walking across the street. Operator of MV1 states he pulled into the parking lot of the Walgreen's, got out of his vehicle and attempted to speak with the pedestrian again, which she waved him off stating she was fine and walked away from him. Operator of MV1 at this time left the scene believing the pedestrian was not injured. No damage to MV1. The pedestrian, Elizabeth Kennedy, states she was walking across Walnut Street (not in the crosswalk) when she entered the left turn only lane on Walnut Street and was struck by MV1. Ms. Kennedy states she hit the ground, but got back up and told the operator of MV1 she was fine and he left the scene. Ms. Kennedy complained of knee and elbow pain. Fallon Medics evaluated Ms. Kennedy and she signed a patient refusal, but stated she will have someone transport her to Tuft's medical center.

MV1 operator was driving the speed limit southbound on Grove St by the Recreation Road/Rt. 95 northbound on-ramp intersection. MV1 operator stated she observed MV2 stopped at the Rt.95 northbound off-ramp intersection's stop sign. MV1 operator stated as she was continuing on Grove St, she was "t-boned" on her driver side by MV2. MV1 operator said after she was struck by MV2, her MV struck the utility pole and landed on top of the southbound Grove St curb. MV1 operator reported no injuries at this time. MV1 passenger did not report any injuries but she and MV1 operator were transported to Newton-Wellesley Hospital for an evaluation by Newton Medics. MV1 sustained extensive driver side, passenger side, front end, and under carriage damage. All airbags were deployed and the front passenger door entrapped MV1 passenger. MV1 was towed by Todys. MV2 operator stated he had exited Rt.95 northbound at the Grove St exit. MV2 operator said he was attempting to drive straight on to Recreation Rd/Rt.95 northbound on-ramp. MV2 operator said he stopped at the stop sign at Grove St and looked both ways. MV2 operator stated he did observe MV1 driving southbound on Grove St at a reasonable speed. However, MV2 operator said he thought MV1 had stopped to let him go. MV2 operator said as he attempted to drive straight onto Recreation Road/Rt.95 northbound on-ramp, he struck the driver side of MV1. MV2 operator reported no injuries at this time. MV2 sustained extensive front end damage, and the driving wheel airbag was deployed. MV2 was towed by Todys.

Vehicle #2 had minor damage and was able to be driven away. Both operators were asked if they were injured to which they stated they were not. Both operators refused any medical treatment or evaluation.
